Intraoperative Cooperation and Clinical Nursing of Patients with Choledocholithiasis Treated with Non-Ray Endoscopic Retrograde Cholangiopancreatography

A total of 98 patients with choledocholithiasis admitted to our hospital from February 2019 to February 2020 were selected. All the patients were divided into the observation group (n=49) and the control group (n=49) according to random number method. The operative time, drainage tube retention time, first anal exhaust time and hospital stay were compared between the two groups. The pain of the two groups was compared at 1st d, 3rd d and 7th d after operation. The incidence of pancreatitis, cholangitis, infection, bleeding and hyperamylasemia and the satisfaction of patients with nursing were compared between the two groups after operation. Compared with the control group, the success rate of lithotomy in the observation group was significantly increased (p<0.05). The operation time, drainage tube retention time, first anal exhaust time and hospitalization time in observation group were significantly lower than those in control group (p<0.05). Compared with the control group, the pain degree of observation group at 1st d, 3rd d and 7th d after surgery was significantly decreased (p<0.05). In addition, the incidence of postoperative complications in observation group was significantly decreased (p<0.05) and nursing satisfaction was significantly increased (p<0.05). The operation cooperation and clinical nursing for patients with choledocholithiasis treated with non-ray endoscopic retrograde cholangiopancreatography can significantly improve the success rate of operation, reduce the injury caused by operation and improve the prognosis of patients, which has high clinical application value.

Analysis on Effects of Comprehensive Nursing Care Applied in Interventional Therapy for Patients with Liver Cirrhosis and Liver Cancer

This study aimed to investigate the effects of comprehensive nursing intervention in interventional therapy for patients with liver cirrhosis and liver cancer. Overall, 512 liver cirrhosis patients complicated with liver cancer receiving interventional therapy in the Department of Oncology of the Second Hospital of Dalian Medical University (Dalian, China) from March 2010 to March 2016 were retrospectively analyzed. Patients were divided into observation group (n=310) and control group (n=202). Comprehensive nursing intervention was applied to observation group and conventional nursing care was applied to control group. The degrees of satisfaction before and after nursing intervention, quality-of-life scores, incidences of postoperative complications and survival rates at 20 months after operation of the two groups were compared. The degree of great satisfaction in observation group was significantly higher than that in control group (P<0.001). The quality-of-life scores of the patients in observation group were obviously higher than those in control group (P<0.001). The incidence of postoperative complications in observation group was significantly lower than that in control group (P<0.001). The survival rates in observation group was significantly higher than that in control group (P=0.035). The application of comprehensive nursing intervention in the interventional therapy for liver cirrhosis and liver cancer can notably improve the life quality of the patients, lower the incidence of postoperative complications and increase the survival rate, which is worthy of clinical popularization and application.

Application of perioperative meticulous nursing in patients with video-assisted thoracoscope surgery

Objective To explore the effects of perioperative meticulous nursing in patients with video-assisted thoracoscope surgery. Methods From January to December 2017, we selected 196 patients with video-assisted thoracoscope surgery at Chengdu Fifth People's Hospital as subjects by convenience sampling. All of the patients were divided into observation group and control group with the method of random number table, 98 cases in each group. Control group received perioperative routine nursing. On the bases of routine nursing, observation group carried out perioperative meticulous nursing. We counted the related indexes of postoperative recovery and incidence of complications, evaluate the Visual Analogy Score (VAS) as well as quality of life among patients between two groups at different time points after surgery. Results The times of out-of-bed activity, extubation of catheter, indwell time of chest tube, time of anal exhaust and hospital stay of observation group were shorter than those of control group with statistical differences (t=25.492, 10.205, 7.132, 23.087, 11.063; P<0.01) . The incidence of postoperative complications of observation group was 8.16% lower than that of control group with a statistical difference (χ2=10.531, P<0.01) . The scores of VAS at 2 and 48 hours after surgery of observation group were statistically lower than those of control group (t=15.717, 21.229; P<0.01) . The scores of physical functioning, role-physical, mental health and social functioning of observation group one week after surgery were higher than those of control group (t=8.321, 8.232, 9.761, 9.507; P<0.01) . Conclusions Perioperative meticulous nursing can effectively reduce the postoperative complications of patients with video-assisted thoracoscope surgery and relieve patients' postoperative pain which makes for patients' postoperative recovery and improving quality of life. Clinical observation of laparoscopic minimally invasive for 60 cases elderly patients with urinary calculi

Objective To explore the clinical efficacy of laparoscopic minimally invasive for elderly patients with urinary calculi. Methods A total of 120 cases elderly patients with urinary calculi in our hospital were selected. Sixty cases were treated by laparoscopic minimally invasive as observation group, and 60 cases were treated with conventional open stone surgery as the control group.The patient's recovery was observed. Results The operation effect, pain medication use rate, postoperative complications in observation group were significantly lower than the control group(P<0.05). The postoperative recovery of the observation group was better, and the degree of satisfaction was higher(P<0.05). Conclusions Laparoscopic minimally invasive surgery has obvious curative effect in the treatment of elderly patients with urinary calculi. It has less damage to blood vessel and normal tissue, and the patient's recovery is good, which improves the quality of life of the patients. Impact of diet nursing intervention on patients' rehabilitation effect of gallbladder removal under laparoscope based on the surgical theoretical support of quick recovery

Objective To discuss about the impact of diet nursing intervention on patients' rehabilitation effect of gallbladder removal under laparoscope based on the surgical theoretical support of quick recovery. Methods A total of 110 patients conducted with gallbladder removal under laparoscope in this hospital from June, 2014 to December, 2015 were selected as the study objects.Random number table was used to divide into observation group and control group where there were 55 patients each.Usual care was given to control group during perioperative period and diet nursing intervention was given to observation group based on surgical theoretical support of quick recovery to compare patients' satisfaction of nutritional indicator, gastrointestinal function and complication of two groups after operations. Results The postoperative off-bed activity time, anus exhaust time, time of taking food, defecation time, time of stopping transfusion and length of stay of patients in observation group were significantly shorter than control group (t=11.481, 11.171, 19.509, 8.479, 2.966, 5.889, P<0.05).7 days after operation, the indicators of ALB, PRE, TRF and RbP of the observation group were significantly higher than that of the control group(t=7.038, 5.971, 8.540, 10.383, P<0.05). The total occurrence rate (30.91%) of postoperative complication in observation group was significantly lower than 52.73% that was in control group(χ2=5.380, P<0.05). The satisfaction that was 94.55% was significantly higher than that of the control group's 81.82%(χ2=4.274, P<0.05). Conclusion The diet nursing intervention based on the surgical theoretical support of quick recovery can help improve the postoperative malnutrition state of patients for gallbladder removal under laparoscope, promote the recovery of gastrointestinal function, reduce complications and improve the satisfaction of patients. Comparison of effects of thoracoscopy combined with laparoscopy surgery and the conventional thoracotomy in treatment of esophageal cancer

Objective To analyze effects comparison of thoracoscopy combined with laparoscopy surgery and the conventional thoracotomy in treatment of esophageal cancer.Methods 88 patients with esophageal carcinoma in the past two years in our department were randomly divided into the two groups according to the digital form,each group had 44 cases.The control group was given radical operation as the treatment for esophageal carcinoma,while the observation group used thoracoscope combined with laparoscope radical operation for treatment.The clinical efficacy of the two groups was compared.Results The operation time of the observation group was slightly higher than that of the control group (t =1.372,P ＞ 0.05),the number of lymph node dissection in the observation group was more than that of the control group (t =1.573,P ＞ 0.05).The intraoperative blood loss [(85.6 ± 30.7) mL],postoperative extubation time [(4.1 ± 1.3) d],chest drainage volume [(550.0 ± 30.5) mL] and postoperative hospital stay [(10.8 ± 2.2)d] in the observation group were significantly lower than those in the control group [(220.4 ± 58.7)mL,(7.5 ±1.8)d,(1 130.0 ±50.8)mL,(17.1 ±3.8)d,(t =18.847,6.486,40.649,8.938,all P ＜0.05)].The incidence of postoperative complications in observation group(6.82％)was significantly lower than that of control group (27.27％) (x2 =5.143,P ＜ 0.05).Conclusion Thoracoscope and laparoscope combined surgery in treatment of esophageal carcinoma can shorten time of being in hospital,decrease blood loss,reduce the incidence of complications,receive high safety and remove lymph node more thoroughly.
