14. Increasing HIV Testing Among Adolescents: Preliminary Outcomes of A Multi-Level Intervention To Implement Universal HIV Testing In Six Urban School-Based Health Centers

HIV testing is a critical step in facilitating access to HIV care and preventing onward transmission. While HIV incidence is increasing among adolescents, only a small proportion is tested. The CDC recommends offering HIV testing in health care settings to persons 13 years and over regardless of risk. We initiated an intervention to implement universal HIV testing to for adolescents utilizing six school-based health centers (SBHCs) in public high school campuses in the Bronx, New York. The intervention model included system-level initiatives (development of practice work flows for offering oral, rapid HIV testing with input from SBHC staff, and use of performance improvement tools led by an Implementation Coach) and staff-level initiatives (trainings, technical assistance and incentives). Three matched-pairs of SBHCs were assigned to two cohorts to receive the same intervention: Cohort 1 initiated in Fall 2016, Cohort 2 in Fall 2017. All patients seen at least once in baseline and intervention years were included for analysis. Outcomes were the offer of an annual HIV test by the clinical staff and the acceptance of testing by the adolescents. Predictors examined included the intervention year, age > 16 years, gender, and ever having been sexually active. For each outcome, we fitted a logistic mixed effects model separately for each cohort. Odds ratios for each variable were calculated, p values set at < .01. Combining both cohorts, there were 5504 unique patients in the baseline year and 5925 in the intervention year, of which the mean age was 15.9 (SD = 1.43), 58.3% were female, 49.1% Hispanic, 26.5% Black and 36.3% had ever been sexually active. The odds of an HIV test offer for Cohort 1 increased by 14% (p = 0.0043) from baseline (14.1%) to intervention (16.8%), and for Cohort 2 by 40% (p < 0.0001) from baseline (8.8%) to intervention (11.6%). Among those offered HIV testing, the odds of test acceptance increased in Cohort 1 by 81% (p < 0.0001) from baseline (34.6%) to intervention (47.8%) and in Cohort 2 by 59% (p < 0.0001) from baseline (59.0%) to intervention (67.7%). Both analyses were adjusted for age, gender, and sexual activity. Females tended to have lower odds of being offered testing within both Cohort 1 (OR = 0.75) and Cohort 2 (OR = 0.67), (p < 0.0001). Ever having been sexually active was associated with increased odds of an HIV test offer (Cohort 1 OR = 1.56, p < 0.0001; Cohort 2 OR = 1.99, p < 0.0001), as well as HIV test acceptance (Cohort 1 OR=4.45, p<0.0001; Cohort 2 OR=3.78, p<0.0001). To date, the implementation intervention increased both the offer and acceptance of HIV testing, with males more likely to be offered testing and with sexually active students more likely to be offered and to accept testing. Implementation activities will continue for one more year for each Cohort as we explore further innovations to increase universal testing and efforts to sustain these improvements.

Decreasing initiation of chemotherapy time in elective patients admitted to an inpatient hematology malignancy unit.

120 Background: At our institution, we observed that 86% (n = 25) of patients admitted for elective chemotherapy experienced a delay (greater than 6 hours) in initiating their treatment. Methods: We measured time from admission to chemotherapy administration (Defined from time of vital signs taken at admission until time of chemotherapy administration) in patients admitted for elective chemotherapy. Key process measures were identified and monitored (i.e, time from admission to laboratory exam results, time from admission to chemotherapy signed, time from chemotherapy signed to chemotherapy released by nurse from the EMR). We collected data every two weeks. After collecting data and utilizing performance improvement tools such as a pareto chart and PICK chart, we developed multiple PDSA cycles as described in Table 1. Results: At the time of interim analysis, we observed a median decrease in time to chemotherapy administration from 25 hours to 8.85 hours. Median time to lab draws decreased from 2.33 hours to -0.63 hours. There was no change in time from signature to nurse releasing the chemotherapy. We noticed more providers were signing the chemotherapy prior to patient admission and more patients were receiving pre-admission alkalinization strategies. Conclusions: By implementing new admission workflows, optimizing our use of the EMR to communicate among providers, and improving pre-admission planning we were able to reduce our time to chemotherapy for elective admissions by 64.6%. Improvement still needed to meet our goals and fully standardize the processes as part of our daily workflow.[Table: see text]

Improving the evaluation of cross efficiencies: A method based on Shannon entropy weight

Data envelopment analysis (DEA) is a non-parametric statistical method used to assess the production frontiers of decision-making units (DMUs) and evaluate their relative efficiencies. However, using traditional DEA models to evaluate efficiency has certain deficiencies. For example, some DMUs cannot be ranked fully using traditional DEA models. To solve such problems, the cross-efficiency evaluation method has been proposed to replace the self-evaluation system. Nevertheless, this method, which uses a mutual evaluation system to overcome the ranking issue, still has shortcomings such as non-unique cross efficiency weights, which may result in multiple cross efficiency values. Further, providing adequate performance improvement tools to decision makers is difficult using only the average efficiency values. To address the problems of uniqueness and aggregation, this study proposes two cross efficiency models, designated MAX and MIN models. The self-evaluated optimal weight of a certain DMU derived from these MAX and MIN models can maximize or minimize the efficiency of the DMU to form two cross efficiency matrices, which can partially solve the problem that results from multiple optimal weights. To solve the aggregation problem of cross efficiency, the study also applies Shannon entropy, which classifies all cross efficiency values into one group of acquired common objective weights to avoid subjective factors. Finally, the present study confirms an improvement when using the proposed method by examining production data on 15 thermoelectric enterprises in China.

Comprehensive Unit-based Safety Program (CUSP) to Improve Patient Experience: How a Hospital Enhanced Care Transitions and Discharge Processes

To determine whether Comprehensive Unit-based Safety Program (CUSP) teams could be used to enhance patient experience by improving care transitions and discharge processes in a 318-bed community hospital. In 2015, CUSP teams produced feasible solutions by participating in a design-thinking initiative, coupled with performance improvement tools involving data analytics and peer-learning communities. Teams completed a 90-day sprint challenge, involving weekly meetings, monthly department leader meetings, and progress trackers. The Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) survey was used, and the percent top (best) scores were reported for overall hospital ratings, discharge information, and care transitions. The percentage of patients choosing the top score increased from 61.0% preintervention to 68.0% postintervention for overall hospital rating and from 71.4% to 80.7% for recommending the hospital. The top scores increased from 76.0% preintervention to 84.5% postintervention for the discharge information domain and from 49.2% to 53.6% for the care transitions domain. CUSP teams improved patient experience. The teams could expand their scope to be the unit-level resource focused not only on safety but also on external quality measures to which patient experience is a broad category for HCAHPS scores, and potentially on value in future work.

The adoption of multiple certification standards: perceived performance implications of quality, environmental and health & safety certifications

This study assesses the combined impact of multiple certifications (i.e. ISO 9001, ISO 14001, OHSAS 18001) on perceived performance dimensions related to quality, environmental and occupational health and safety. Using survey data collected from 59 Irish manufacturing plants in 2014, we employed MANCOVA and regression analysis to test our proposed hypothesis. The results suggest that companies that are simultaneously ISO 9001, ISO 14001 and OHSAS 18001 certified are significantly better performers with regard to environmental and occupational health and safety compared to companies without multiple certifications. However, from a perceived quality performance perspective, having these multiple certifications doesn’t seem to be an effective performance improvement tool.

Trial of the Primary Care Practice Improvement Tool: building organisational performance in Australian general practice and primary health care.

To nationally trial the Primary Care Practice Improvement Tool (PC-PIT), an organisational performance improvement tool previously co-created with Australian primary care practices to increase their focus on relevant quality improvement (QI) activities. The study was conducted from March to December 2015 with volunteer general practices from a range of Australian primary care settings. We used a mixed-methods approach in two parts. Part 1 involved staff in Australian primary care practices assessing how they perceived their practice met (or did not meet) each of the 13 PC-PIT elements of high-performing practices, using a 1-5 Likert scale. In Part 2, two external raters conducted an independent practice visit to independently and objectively assess the subjective practice assessment from Part 1 against objective indicators for the 13 elements, using the same 1-5 Likert scale. Concordance between the raters was determined by comparing their ratings. In-depth interviews conducted during the independent practice visits explored practice managers' experiences and perceived support and resource needs to undertake organisational improvement in practice. Data were available for 34 general practices participating in Part 1. For Part 2, independent practice visits and the inter-rater comparison were conducted for a purposeful sample of 19 of the 34 practices. Overall concordance between the two raters for each of the assessed elements was excellent. Three practice types across a continuum of higher- to lower-scoring practices were identified, with each using the PC-PIT in a unique way. During the in-depth interviews, practice managers identified benefits of having additional QI tools that relate to the PC-PIT elements. The PC-PIT is an organisational performance tool that is acceptable, valid and relevant to our range of partners and the end users (general practices). Work is continuing with our partners and end users to embed the PC-PIT in existing organisational improvement programs.

Lean six sigma performance improvement tool for automotive suppliers

Automotive industry is one of the most active industries involved in the quality effort, low production cost, and continuous improvement activities. Today, lean six sigma (LSS) is integrated with strategic control system that has assisted organizations to achieve better organizational performance through high-level continuous improvement activity and better saving in terms of operation and quality cost. Analytic hierarchy process methodology was used in developing a tool of lean six sigma performance improvement to measure the performance of automotive companies. This tool was developed using a system based on Excel. Five case studies were prepared to validate this tool. The result of these case studies suggested that this tool was timely and suitable to determine organizational performance of automotive companies. Through these case studies, this tool was able to identify the strength and weaknesses that directed to where and how improvement should be done.

Building a Foundation of Continuous Improvement in a Rapidly Changing Environment: The Dartmouth-Hitchcock Value Institute Experience

A performance improvement competency development program, known as the Value Institute (VI), was established at Dartmouth-Hitchcock (D-H; Lebanon, New Hampshire) in 2011 to develop a performance improvement-focused workforce and systems capable of meeting the challenges of creating a sustainable health system. A tiered competency development program that provides patient safety, health care quality, and improvement science education, and an execution support infrastructure that enables access to performance improvement tools for all employees, comprise the core of the VI. At 20 months after the launch of the first VI classes, more than 10% of all employees were trained to the Yellow Belt level, and approximately 1.5% of all employees became advanced practitioners (Green Belts or Black Belts). Improvement projects have focused on both clinical and business process optimization, as well as regulatory and accreditation compliance and patient safety. Project savings during the two years of operation have exceeded the investment of resources to establish this long-term performance improvement capability by 2.5 times. The D-H VI model promotes multidisciplinary team-based learning, incremental skill development, and access to a common continuous improvement vocabulary and method for all employees-all key to building the teams and momentum needed for successful execution of improvement work and to maintain outcomes. Initial outcomes, represented by organizational spread, project execution status, participants' feedback scores, and return on investment estimates, suggest that robust team-based learning combined with coaching provides sufficient depth and breadth of learning and effective opportunities to gain practical experience in continuous improvement.

Effects of Performance Feedback Reports on Adherence to Evidence-Based Guidelines in Use of CT for Evaluation of Pulmonary Embolism in the Emergency Department: A Randomized Trial.

The purpose of this study was to assess whether implementing emergency department (ED) physician performance feedback reports improves adherence to evidence-based guidelines for use of CT for evaluation of pulmonary embolism (PE) beyond that achieved with clinical decision support (CDS) alone. This prospective randomized controlled trial was conducted from January 1, 2012, to December 31, 2013, at an urban level 1 adult trauma center ED. Attending physicians were stratified into quartiles by use of CT for evaluation of PE in 2012 and were randomized to receive quarterly feedback reporting or not, beginning January 2013. Reports consisted of individual and anonymized group data on guideline adherence (using the Wells criteria), use of CT for PE (number of CT examinations for PE per 1000 patients), and yield (percentage of CT examinations for PE with positive findings). We compared guideline adherence (primary outcome) and use and yield (secondary outcomes) of CT for PE between the control and intervention groups in 2013 and with historical imaging data from 2012. Of 109,793 ED patients during the control and intervention periods, 2167 (2.0%) underwent CT for evaluation of PE. In the control group, guideline adherence remained unchanged between 2012 (78.8% [476/604]) and 2013 (77.2% [421/545]) (p = 0.5); in the intervention group, guideline adherence increased 8.8% after feedback report implementation, from 78.3% (426/544) to 85.2% (404/474) (p < 0.05). Use and yield were unchanged in both groups. Implementation of quarterly feedback reporting resulted in a modest but significant increase in adherence to evidence-based guidelines for use of CT for evaluation of PE in ED patients, enhancing the impact of CDS alone. These results suggest potentially synergistic effects of traditional performance improvement tools with CDS to improve guideline adherence.

International benchmarking and best practice management: in search of health care and hospital excellence.

Hospitals worldwide are facing the same opportunities and threats: the demographics of an aging population; steady increases in chronic diseases and severe illnesses; and a steadily increasing demand for medical services with more intensive treatment for multi-morbid patients. Additionally, patients are becoming more demanding. They expect high quality medicine within a dignity-driven and painless healing environment. The severe financial pressures that these developments entail oblige care providers to more and more cost-containment and to apply process reengineering, as well as continuous performance improvement measures, so as to achieve future financial sustainability. At the same time, regulators are calling for improved patient outcomes. Benchmarking and best practice management are successfully proven performance improvement tools for enabling hospitals to achieve a higher level of clinical output quality, enhanced patient satisfaction, and care delivery capability, while simultaneously containing and reducing costs. This chapter aims to clarify what benchmarking is and what it is not. Furthermore, it is stated that benchmarking is a powerful managerial tool for improving decision-making processes that can contribute to the above-mentioned improvement measures in health care delivery. The benchmarking approach described in this chapter is oriented toward the philosophy of an input-output model and is explained based on practical international examples from different industries in various countries. Benchmarking is not a project with a defined start and end point, but a continuous initiative of comparing key performance indicators, process structures, and best practices from best-in-class companies inside and outside industry. Benchmarking is an ongoing process of measuring and searching for best-in-class performance: Measure yourself with yourself over time against key performance indicators. Measure yourself against others. Identify best practices. Equal or exceed this best practice in your institution. Focus on simple and effective ways to implement solutions. Comparing only figures, such as average length of stay, costs of procedures, infection rates, or out-of-stock rates, can lead easily to wrong conclusions and decision making with often-disastrous consequences. Just looking at figures and ratios is not the basis for detecting potential excellence. It is necessary to look beyond the numbers to understand how processes work and contribute to best-in-class results. Best practices from even quite different industries can enable hospitals to leapfrog results in patient orientation, clinical excellence, and cost-effectiveness. Despite common benchmarking approaches, it is pointed out that a comparison without "looking behind the figures" (what it means to be familiar with the process structure, process dynamic and drivers, process institutions/rules and process-related incentive components) will be extremely limited referring to reliability and quality of findings. In order to demonstrate transferability of benchmarking results between different industries practical examples from health care, automotive, and hotel service have been selected. Additionally, it is depicted that international comparisons between hospitals providing medical services in different health care systems do have a great potential for achieving leapfrog results in medical quality, organization of service provision, effective work structures, purchasing and logistics processes, or management, etc.

Abstract T MP101: Development and Introduction of a Performance Improvement Tool to Enhance Door-To-CT-Times for the Acute Stroke Patient

Purpose/Background: The purpose of this abstract is to demonstrate rapid change improvement in SCORE quality measures specifically, Door-to-CT-times , for patients presenting to the Emergency Department with signs and symptoms of stroke.This change was implemented subsequent to review of data which revealed less than optimal Door-to-CT-times sustained over the course of several quarters. Objectives: 1. To visually demonstrate the algorithm developed to model the care pathway of the acute stroke patient from Registration to initiation of the CT radiographic exam. 2. To exhibit an educational pathway that resulted in rapid change and improvement of Door-to-CT-times as well as an unintentional favorable trend for the CT-to-Result measure. 3. To compare Door-to-CT-times pre- and post-implementation of multidisciplinary education on the stroke pathway and goal of Door-to-CT in less than 20 minutes. Results: Prior to implementation of education utilizing the stroke pathway algorithm, results were consistently inconsistent ranging from 22% compliance to 63% compliance month over month. In January 2014 we had a 47% success rate and February 33% success rate for door to CT within 20 minutes. The educational program was implemented in February of 2014. Clearly, the visual depiction of patient flow thru the ED to CT greatly enhanced the educational process. We exceeded our goal of attaining 50% compliance and stretch goal of 70% compliance to 89% compliance in Door-to-CT times in the first month (March 2014) following implementation of the process improvement initiative. Data reveals sustained improvements through Q2 as follows: April 90%, May 68%, and June 73%. Due to the significant improvement in our Door-to-CT compliance, we also noted a comparably significant enhancement in our Door-To-Results times. Conclusions/Implications for Practice: By developing an ongoing educational process with demonstrable improvements in care for the acute stroke patient, Emergency Department staff appreciated improved outcomes for this particular patient population. This is a single institution study, results may not be applicable to other institutions.

Health resource variability in the achievement of optimal performance and clinical outcome in ischemic heart disease.

A disparity between evidence and practice in the management of ischemic heart disease is frequently observed. Guideline adherence and clinical outcomes are influenced by system, provider, and patient factors. Recently, performance improvement measures for cardiovascular disease have gained a lot of popularity worldwide. These measures may facilitate the uptake of evidence-based recommendations and improve patient outcomes. While apparently valid as quality metrics, their impacts on clinical outcomes remain limited and are areas of further research. Several methods for optimizing performance have been instituted and essentially involve three different approaches—improvement in the reporting of data on guideline adherence, providing infrastructure and tools, and providing incentives to improve guideline adherence. Public reporting of quality metrics and "pay-for-performance" are some novel performance improvement tools. The impact of these approaches on patient outcomes will be pivotal in improving cardiovascular outcomes in the future.

Lactate measurements in sepsis-induced tissue hypoperfusion: results from the Surviving Sepsis Campaign database.

The Surviving Sepsis Campaign guidelines recommend obtaining a serum lactate measurement within 6 hours of presentation for all patients with suspected severe sepsis or septic shock. A lactate greater than 4 mmol/L qualifies for administration of early quantitative resuscitation therapy. We evaluated lactate elevation (with special attention to values > 4 mmol/L) and presence or absence of hypotension as a marker of clinical outcome. The Surviving Sepsis Campaign developed a database to assess the overall effect of the sepsis bundles as a performance improvement tool for clinical practice and patient outcome. This analysis focuses on one element of the Surviving Sepsis Campaign's resuscitation bundle, measuring serum lactate in adult severe sepsis or septic shock patients and its interaction with hypotension. This analysis was conducted on data submitted from January 2005 through March 2010. Data from 28,150 subjects at 218 sites were analyzed. None. Unadjusted analysis of the 28,150 observations from the Surviving Sepsis Campaign database demonstrated a significant mortality increase with the presence of hypotension in conjunction with serum lactate elevation greater than 2 mmol/L. On multivariable analysis, only lactate values greater than 4 mmol/L, in conjunction with hypotension, significantly increased mortality when compared with the referent group of lactate values less than 2 mmol/L and not hypotensive. Mortality was 44.5% in patients with combined lactate greater than 4 mmol/L and hypotension when compared with 29% mortality in patients not meeting either criteria. Serum lactate was commonly measured within 6 hours of presentation in the management of severe sepsis or septic shock in this subset analysis of the Surviving Sepsis Campaign database in accordance with the Surviving Sepsis Campaign guidelines. Our results demonstrate that elevated lactate levels are highly associated with in-hospital mortality. However, only patients who presented with lactate values greater than 4 mmol/L, with and without hypotension, are significantly associated with in-hospital mortality and is associated with a significantly higher risk than intermediate levels (2-3 and 3-4 mmol/L). This supports the use of the cutoff of greater than 4 mmol/L as a qualifier for future clinical trials in severe sepsis or septic shock in patient populations who use quantitative resuscitation and the Surviving Sepsis Campaign bundles as standard of care.

Medication use evaluation: pharmacist rubric for performance improvement.

Despite rigorous expert review, medications often fall into routine use with unrecognized and unwanted complications. Use of some medications remains controversial because information to support efficacy is conflicting, scant, or nonexistent. Medication use evaluation (MUE) is a performance improvement tool that can be used when there is uncertainty regarding whether a medication will be beneficial. It is particularly useful when limited evidence is available on how best to choose between two or more medications. MUEs can analyze the process of medication prescribing, preparation, dispensing, administration, and monitoring. MUEs can be part of a structured or mandated multidisciplinary quality management program that focuses on evaluating medication effectiveness and improving patient safety. Successful MUE programs have a structure in place to support completion of rapid-cycle data collection, analysis, and intervention that supports practice change.

Engaging the health care team through Operations Councils: strategies to improve population health from within.

Clinical front-line staff are best positioned within the organizations to identify patient safety problems and craft solutions. However, in traditional models, safety committees are led by senior executives who are not clinically responsible for patients. This top-down approach can result in missed opportunities to address patient-centered challenges and better manage the health of the defined populations served by these organizations. To foster teamwork, enhance empowerment, and improve the patient care environment, Operations Councils led by trained front-line staff were deployed in 15 clinical areas at the Ohio State University Wexner Medical Center (OSUWMC) as a performance improvement tool. Standardized training of Council facilitators was designed and implemented to guide the performance improvement process. Balanced scorecards were developed in each Council based on the risks and concerns of that particular clinical area. After initial implementation of the Operations Councils, patient safety events declined and team engagement improved by over 34% across the medical center; the highest changes were seen in areas where Operations Councils had been deployed. Additionally, outcome metrics including area-specific and system-wide mortality and readmissions improved after implementation. We suggest that this type of approach may be an appropriate strategy to consider in other health care organizations as such institutions are challenged to better manage the health of their defined patient populations.

Development and pilot study of the Primary Care Practice Improvement Tool (PC-PIT): an innovative approach.

To assess the usability and validity of the Primary Care Practice Improvement Tool (PC-PIT), a practice performance improvement tool based on 13 key elements identified by a systematic review. It was co-created with a range of partners and designed specifically for primary health care. This pilot study examined the PC-PIT using a formative assessment framework and mixed-methods research design. Six high-functioning general practices in Queensland, Australia, between February and July 2013. A total of 28 staff participated - 10 general practitioners, six practice or community nurses, 12 administrators (four practice managers; one business manager and eight reception or general administrative staff). Readability, content validity and staff perceptions of the PC-PIT. The PC-PIT offers an appropriate and acceptable approach to internal quality improvement in general practice. Quantitative assessment scores and qualitative data from all staff identified two areas in which the PC-PIT required modification: a reduction in the indicative reading age, and simplification of governance-related terms and concepts. The PC-PIT provides an innovative approach to address the complexity of organisational improvement in general practice and primary health care. This initial validation will be used to develop a suite of supporting, high-quality and free-to-access resources to enhance the use of the PC-PIT in general practice. Based on these findings, a national trial is now underway.

Implementation of benchmarking concepts in Indian automobile industry – an empirical study

Purpose – This paper aims to present existence comprehensive analysis of state of implementation of benchmarking concepts in Indian automotive companies. Design/methodology/approach – The research is carried out through a mixed method of research approach comprising of a survey of 300 auto companies in India. Out of 300, 48 valid responses together with three additional case studies were used in the data analysis. Inclusion of case studies was aspired to get deeper insight into the issues pertaining to adoption of best practices, and subsequently the implementation of benchmarking activities. Findings – Benchmarking has been unanimously accepted as an effective performance and productivity improvement tool by Indian auto companies. However, Indian automobile manufacturers still see benchmarking as a tool to compare product attributes, quality attributes, operations, and processes. Moreover, it has been perceived as being less applicable at strategic level. Results also show that benchmarking is in its primary stage in the Indian automotive industry, and it still needs much more commitment from top management for its proliferation. Lesser significance is given to competitor benchmarking due to the fear of losing competitive advantage, and the problem of confidentiality. Reasons identified in this study for not using benchmarking include “lack of human resources” as most important, followed by “financial constraints”, and “lack of internal expertise”. Research limitations/implications – Research results should be generalized and reproduced with a larger sample size. Owing to the scarce application of benchmarking in small and medium enterprises (SMEs), separate study should be carried out to find ways to encourage benchmarking implementation in Indian auto component manufacturing SMEs. Originality/value – The paper provides insight into the extent of implementation of benchmarking concepts in Indian automobile industry.
