Evaluation of twin fetal exposure to radiofrequency field during magnetic resonance imaging.

Fetal development is essential to the human lifespan. As more and more multifetal gestations have been reported recently, clinical diagnosis using magnetic resonance imaging (MRI), which introduced radiofrequency (RF) exposure, raised public concerns. The present study developed two whole-body pregnant models of 31 and 32 gestational weeks (GWs) with twin fetuses and explored RF exposure by 1.5 and 3.0T MRI. Differences in the relative position of the fetus and changes in fetal weight can cause differences in fetal peak local specific absorption rate averaged over 10g tissue (pSAR10g). Variation of pSAR10g due to different fetal positions can be ~35%. Numerically, twin and singleton fetal pSAR10g results were not significantly different, however twin results exceeded the limit in some cases (e.g. fetuses of 31 GW at 1.5T), which indicated the necessity for further research employing anatomically correct twin-fetal models coming from various GWs and particular sequence to be applied.

What can we gain from subpopulation universal pulses? A simulation-based study.

The aim of the study was to explore a novel methodology for designing universal pulses (UPs) that balances the benefits of a calibration-free approach with subject-specific online pulse design. The proposed method involves segmenting the population into subpopulations with variability in anatomical shapes and positions reduced to 75%, 50%, and 25% of their original values while keeping the mean values unchanged. An additional 25% extreme case with a large volume of interest and shifted position was included. For each group, a 5kT-points universal inversion pulse was designed and assessed by the normalized root mean square error (NRMSE) on the target longitudinal magnetization profile. The performance was compared to the conventional one-size-fits-all approach. A total of 132 electromagnetic simulations were executed to generate representative anatomies and specific absorption rate (SAR) distributions in a three-dimensional parameter space comprised of head breadth, head length, and Y-shift. The 99.9th percentile on the peak local SAR distribution was utilized to establish an intersubject variability safety margin. UPs designed for subpopulations with decreased head shape and position variability reduced the anatomical safety margin by up to 20%. Furthermore, when a head was significantly different to the average case, the proposed approach improved the inversion homogeneity by up to 24%, compared to the conventional one-size-fits-all approach. Subpopulation UPs present an opportunity to improve the homogeneity and reduce anatomical SAR safety margins at 7T without additional acquisition time for calibration.

Double-row 16-element folded-end dipole transceiver array for 3D RF shimming of the whole human brain at 9.4 T.

Homogeneity and longitudinal coverage of transmit (Tx) human head RF coils at ultrahigh field (UHF, ≥7 T) can be improved by 3D RF shimming, which requires using multi-row Tx arrays. Examples of 3D RF shimming using double-row UHF loop transceiver (TxRx) and Tx arrays have been described previously. Dipole antennas provide unique simplicity and robustness while offering comparable Tx efficiency and signal-to-noise ratio to conventional loop designs. Single-row Tx and TxRx human head UHF dipole arrays have been previously described by multiple groups. Recently, we developed a novel type of dipole antenna, a folded-end dipole, and presented single-row eight-element array prototypes for human head imaging at 7 and 9.4 T. These studies have shown that the novel antenna design can improve the longitudinal coverage and minimize peak local specific absorption rate (SAR) as compared with common unfolded dipoles. In this work, we developed, constructed, and evaluated a 16-element double-row TxRx folded-end dipole array for human head imaging at 9.4 T. To minimize cross-talk between neighboring dipoles located in different rows, we used transformer decoupling, which decreased coupling to a level below -20 dB. The developed array design was demonstrated to be capable of 3D static RF shimming and can be potentially used for dynamic shimming using parallel transmission. For optimal phase shifts between the rows, the array provides 11% higher SAR efficiency and 18% higher homogeneity than a folded-end dipole single-row array of the same length. The design also offers a substantially simpler and more robust alternative to the common double-row loop array with about 10% higher SAR efficiency and better longitudinal coverage.

Improving Specific Absorption Rate Efficiency and Coil Robustness of Self-Decoupled Transmit/Receive Coils by Elevating Feed and Mode Conductors.

Self-decoupling technology was recently proposed for radio frequency (RF) coil array designs. Here, we propose a novel geometry to reduce the peak local specific absorption rate (SAR) and improve the robustness of the self-decoupled coil. We first demonstrate that B1 is determined by the arm conductors, while the maximum E-field and local SAR are determined by the feed conductor in a self-decoupled coil. Then, we investigate how the B1, E-field, local SAR, SAR efficiency, and coil robustness change with respect to different lift-off distances for feed and mode conductors. Next, the simulation of self-decoupled coils with optimal lift-off distances on a realistic human body is performed. Finally, self-decoupled coils with optimal lift-off distances are fabricated and tested on the workbench and MRI experiments. The peak 10 g-averaged SAR of the self-decoupled coil on the human body can be reduced by 34% by elevating the feed conductor. Less coil mismatching and less resonant frequency shift with respect to loadings were observed by elevating the mode conductor. Both the simulation and experimental results show that the coils with elevated conductors can preserve the high interelement isolation, B1+ efficiency, and SNR of the original self-decoupled coils.

Actual patient position versus safety models: Specific Absorption Rate implications of initial head position for Ultrahigh Field Magnetic Resonance Imaging.

Specific absorption rate (SAR) relates power absorption to tissue heating, and therefore is used as a safety constraint in magnetic resonance imaging (MRI). This study investigates the implications of initial head positioning on local and whole-head SAR. A virtual body model was simulated at 161 positions inside an eight-channel parallel-transmit (pTx) array. On-axis displacements and rotations of up to 20 mm/degrees and off-axis axial/coronal translations were investigated. Single-channel, radiofrequency (RF) shimming (i.e., single-spoke pTx) and multispoke pTx pulses were designed for seven axial, five coronal and five sagittal slices at each position (the slices were consistent across all positions). Whole-head and local SAR were calculated using safety models consisting of a single (centred) body position, multiple representative positions and all simulated body positions. Positional mismatches between safety models and actual positions cause SAR underestimation. For axial imaging, the actual peak local SAR was up to 4.2-fold higher for both single-channel and 5-spoke pTx, 3.5-fold higher for 3-/4-spoke pTx, and 2-fold higher for RF shimming and 2-spoke pTx, compared with that calculated using the centred body position. For sagittal and coronal imaging, the underestimation of peak local SAR was up to 5.2-fold and 3.8-fold, respectively. Using all body positions to estimate SAR prevented SAR underestimation but yielded up to 11-fold SAR overestimation for RF shimming. Local SAR of single-channel and pTx multispoke pulses showed considerable dependence on the initial patient position. RF shimming yielded much lower sensitivity to positional mismatches for axial imaging but not for sagittal and coronal imaging. This was deemed attributable to the higher degrees-of-freedom of control offered by the investigated coil array for axial imaging. Whole-head SAR is less sensitive to positional mismatches compared with local SAR. Nevertheless, whole-head SAR increased by up to 80% for sagittal imaging. Local and whole-head SAR were observed to be more sensitive to positional mismatches in the axial plane, because of larger variations in coil-tissue proximity. Using all possible body positions in the safety model may become substantially over-conservative and limit imaging performance, especially for the RF shimming mode for axial imaging.

Parallel transmission 2D RARE imaging at 7T with transmit field inhomogeneity mitigation and local SAR control

PurposeWe develop and test a parallel transmit (pTx) pulse design framework to mitigate transmit field inhomogeneity with control of local specific absorption rate (SAR) in 2D rapid acquisition with relaxation enhancement (RARE) imaging at 7T. MethodsWe design large flip angle RF pulses with explicit local SAR constraints by numerical simulation of the Bloch equations. Parallel computation and analytical expressions for the Jacobian and the Hessian matrices are employed to reduce pulse design time. The refocusing-excitation “spokes” pulse pairs are designed to satisfy the Carr-Purcell-Meiboom-Gill (CPMG) condition using a combined magnitude least squares-least squares approach. ResultsIn a simulated dataset, the proposed approach reduced peak local SAR by up to 56% for the same level of refocusing uniformity error and reduced refocusing uniformity error by up to 59% (from 32% to 7%) for the same level of peak local SAR compared to the circularly polarized birdcage mode of the pTx array. Using explicit local SAR constraints also reduced peak local SAR by up to 46% compared to an RF peak power constrained design. The excitation and refocusing uniformity error were reduced from 20%–33% to 4%–6% in single slice phantom experiments. Phantom experiments demonstrated good agreement between the simulated excitation and refocusing uniformity profiles and experimental image shading. ConclusionPTx-designed excitation and refocusing CPMG pulse pairs can mitigate transmit field inhomogeneity in the 2D RARE sequence. Moreover, local SAR can be decreased significantly using pTx, potentially leading to better slice coverage, enabling larger flip angles or faster imaging.

Bench to bore ramifications of inter-subject head differences on RF shimming and specific absorption rates at 7T

PurposeThis study shows how inter-subject variation over a dataset of 72 head models results in specific absorption rate (SAR) and B1+ field homogeneity differences using common shim scenarios. MethodsMR-CT datasets were used to segment 71 head models into 10 tissue compartments. These head models were affixed to the shoulders and neck of the virtual family Duke model and placed within an 8 channel transmit surface-loop array to simulate the electromagnetic fields of a 7T imaging experiment. Radio frequency (RF) shimming using the Gerchberg-Saxton algorithm and Circularly Polarized shim weights over the entire brain and select slices of each model was simulated. Various SAR metrics and B1+ maps were calculated to demonstrate the contribution of head variation to transmit inhomogeneity and SAR variability. ResultsWith varying head geometries the loading for each transmit loop changes as evidenced by changes in S-parameters. The varying shim conditions and head geometries are shown to affect excitation uniformity, spatial distributions of local SAR, and SAR averaging over different pulse sequences. The Gerchberg-Saxton RF shimming algorithm outperforms circularly polarized shimming for all head models. Peak local SAR within the coil most often occurs nearest the coil on the periphery of the body. Shim conditions vary the spatial distribution of SAR. ConclusionThe work gives further support to the need for fast and more subject specific SAR calculations to maintain safety. Local SAR10g is shown to vary spatially given shim conditions, subject geometry and composition, and position within the coil.

Safety Analysis of Medical Implants in the Human Head Exposed to a Wireless Power Transfer System

The interactions of medical implants in the human body with electromagnetic fields from newly introduced high-field technologies such as near-field wireless power transfer (WPT) are of great concern. In this study, the effects of implants on the specific absorption rate (SAR) in a head model were computationally evaluated in the immediate vicinity of a WPT system operating at 6.78 MHz with a transferred power of 50 W. The SAR in the head model located 30 mm above the WPT system was evaluated with a skull plate, bone plates (different shapes), miniplate, fixtures, and a deep brain stimulator. The results indicated that even small implants had notable effects on the SAR distribution and corresponding local peak and mass-averaged SAR values. Apart from the size and shape of the implant, the crucial factor influencing the SAR was the implant’s position with respect to the WPT system. Therefore, various position configurations were simulated for the WPT system to determine the worst case scenario for each specific implant. Although the local peak SAR value was increased by a factor of 600 in the worst-case scenario (for the skull plate), the SAR <inline-formula xmlns:mml="http://www.w3.org/1998/Math/MathML" xmlns:xlink="http://www.w3.org/1999/xlink"><tex-math notation="LaTeX">$_{\text{1}g}$</tex-math></inline-formula> and SAR <inline-formula xmlns:mml="http://www.w3.org/1998/Math/MathML" xmlns:xlink="http://www.w3.org/1999/xlink"><tex-math notation="LaTeX">$_{\text{10}g}$</tex-math></inline-formula> were only increased by factors of seven and four, respectively. Compliance with international safety limits was studied, followed by computing the maximum allowable transmit power (MATP). It was found that, without the implant, the MATP satisfying SAR <inline-formula xmlns:mml="http://www.w3.org/1998/Math/MathML" xmlns:xlink="http://www.w3.org/1999/xlink"><tex-math notation="LaTeX">$_{\text{1}g}$</tex-math></inline-formula> was 43 W for the designed WPT system, and was reduced to 6.9 W in the presence of the skull plate.

Personalized local SAR prediction for parallel transmit neuroimaging at 7T from a single T1-weighted dataset.

PurposeParallel RF transmission (PTx) is one of the key technologies enabling high quality imaging at ultra‐high fields (≥7T). Compliance with regulatory limits on the local specific absorption rate (SAR) typically involves over‐conservative safety margins to account for intersubject variability, which negatively affect the utilization of ultra‐high field MR. In this work, we present a method to generate a subject‐specific body model from a single T1‐weighted dataset for personalized local SAR prediction in PTx neuroimaging at 7T.MethodsMulti‐contrast data were acquired at 7T (N = 10) to establish ground truth segmentations in eight tissue types. A 2.5D convolutional neural network was trained using the T1‐weighted data as input in a leave‐one‐out cross‐validation study. The segmentation accuracy was evaluated through local SAR simulations in a quadrature birdcage as well as a PTx coil model.ResultsThe network‐generated segmentations reached Dice coefficients of 86.7% ± 6.7% (mean ± SD) and showed to successfully address the severe intensity bias and contrast variations typical to 7T. Errors in peak local SAR obtained were below 3.0% in the quadrature birdcage. Results obtained in the PTx configuration indicated that a safety margin of 6.3% ensures conservative local SAR estimates in 95% of the random RF shims, compared to an average overestimation of 34% in the generic “one‐size‐fits‐all” approach.ConclusionA subject‐specific body model can be automatically generated from a single T1‐weighted dataset by means of deep learning, providing the necessary inputs for accurate and personalized local SAR predictions in PTx neuroimaging at 7T.

An endovaginal MRI array with a forward-looking coil for advanced gynecological cancer brachytherapy procedures: Design and initial results.

To develop an endovaginal MRI array that provides signal enhancement forward into the posterior parametrium and sideways into the vaginal wall, accelerating multiple-contrast detection of residual tumors that survive external beam radiation. The array's enclosure should form an obturator for cervical cancer brachytherapy, allowing integration with MRI-guided catheter placement, CT, and interstitial radiation dose delivery. The endovaginal array consisted of forward-looking and sideways-looking components. The forward-looking element imaged the cervix and posterior endometrium, and the sideways-looking elements imaged the vaginal wall. Electromagnetic simulation was performed to optimize the geometry of a forward-looking coil placed on a conductive-metallic substrate, extending the forward penetration above the coil's tip. Thereafter, an endovaginal array with one forward-looking coil and four sideways-looking elements was constructed and tested at 1.5 Tesla in saline and gel phantoms, and three sexually mature swine. Each coil's tuning, matching, and decoupling were optimized theoretically, implemented with electronic circuits, and validated with network-analyzer measurements. The array enclosure emulates a conventional brachytherapy obturator, allowing use of the internal imaging array together with tandem coils and interstitial catheters, as well as use of the enclosure alone during CT and radiation delivery. To evaluate the receive magnetic field ( ) spatial profile, the endovaginal array's specific absorption-rate (SAR) distribution was simulated inside a gel ASTM phantom to determine extreme heating locations in advance of a heating test. Heating tests were then performed during high SAR imaging in a gel phantom at the predetermined locations, testing compliance with MRI safety standards. To assess array imaging performance, signal-to-noise-ratios (SNR) were calculated in a saline phantom and in vivo. Swine images were acquired with the endovaginal array combined with the scanner's body and spine arrays. Simulated profiles for the forward-looking lobe pattern, obtained while varying several geometric parameters, disclosed that a forward-looking coil placed on a metal-backed substrate could double the effective forward penetration from approximately 25 to ∼40mm. An endovaginal array, enclosed in an obturator enclosure was then constructed, with all coils tuned, matched, and decoupled. The ASTM gel-phantom SAR test showed that peak local SAR was 1.2 W/kg in the forward-looking coil and 0.3 W/kg in the sideways-looking elements, well within ASTM/FDA/IEC guidelines. A 15-min 4 W/kg average SAR imaging experiment resulted inless than2o C temperature increase, also within ASTM/FDA/IEC heating limits. In a saline phantom, the forward-looking coil and sideways-looking array's SNR was four to eight times, over a 20-30mm field-of-view (FOV), and five to eight times, over a 15-25mm FOV, relative to the spine array's SNR, respectively. In three sexually mature swine, the forward-looking coil provided a 5+0.2 SNR enhancement factor within the cervix and posterior endometrium, and the sideways-looking array provided a 4+0.2 SNR gain factor in the vaginal wall, relative to the Siemens spine array, demonstrating that the array could significantly reduce imaging time. Higher SNR gynecological imaging is supported by forward-looking and sideways-looking coils. A forward-looking endovaginal coil for cervix and parametrium imaging was built with optimized metal backing. Array placement within an obturator enhanced integration with the brachytherapy procedure and accelerated imaging for detecting postexternal-beam residual tumors.

Safety and imaging performance of two-channel RF shimming for fetal MRI at 3T.

This study investigates whether two-channel radiofrequency (RF) shimming can improve imaging without increasing specific absorption rate (SAR) for fetal MRI at 3T. Transmit field ( ) average and variation in the fetus was simulated in seven numerical pregnant body models. Safety was quantified by maternal and fetal peak local SAR and fetal average SAR. The shim parameter space was divided into improved (magnitude and homogeneity) and improved SAR regions, and an overlap where RF shimming improved both classes of metrics compared with birdcage mode was assessed. Additionally, the effect of fetal position, tissue detail, and dielectric properties on transmit field and SAR was studied. A region of subject-specific RF shim parameter space improving both and SAR metrics was found for five of the seven models. Optimizing only metrics improved efficiency across models by 15% on average and 28% for the best-case model. variation improved by 26% on average and 49% for the best case. However, for these shim settings, fetal SAR increased by up to 106%. The overlap region, where both and SAR metrics improve, showed an average efficiency improvement of 6% on average across models and 19% for the best-case model. variation improved by 13% on average and 40% for the best case. RFS could also decrease maternal/fetal SAR by up to 49%/58%. RF shimming can improve imaging compared with birdcage mode without increasing fetal and maternal SAR when a patient-specific SAR model is incorporated into the shimming procedure.

Open birdcage coil for head imaging at 7T.

To theoretically describe, design, and test the new geometry of the birdcage coil for 7 Tesla anatomical brain imaging, which includes a large window on top, without deliberately jeopardizing its homogeneity and efficiency. This opencage will not only improve patient comfort but also enable the volunteer to follow functional MRI stimuli. This design could also facilitate the tracking of patient compliance and enable better correction of the movement. Via the transfer matrix approach, a birdcage-like coil with a nonperiodic distribution of rungs is constructed with optimized currents in the coil rungs. Subsequently, the coil is adjusted in full-wave simulations. Then, the coil is assembled, fine-tuned, and matched on the bench. Finally, these results are confirmed experimentally on a phantom and in vivo. Indeed, the computed isolation of -14.9 dB between the feeding ports of the coil and the symmetry of the circular polarized mode pattern transmit RF magnetic field ( ) showed that the coil was properly optimized. An experimental assessment of the developed coil showed competitive transmit efficiency and coverage compared with the conventional birdcage coil of similar size. The proposed opencage coil can be designed and work without a dramatic drop of performance in terms of the field homogeneity, transmit efficiency ( / ), peak local specific absorption rate ( ) and SAR efficiency ( / ).

A self-matched leaky-wave antenna for ultrahigh-field magnetic resonance imaging with low specific absorption rate

The technology of magnetic resonance imaging is developing towards higher magnetic fields to improve resolution and contrast. However, whole-body imaging at 7 T or even higher flux densities remains challenging due to wave interference, tissue inhomogeneities, and high RF power deposition. Nowadays, proper RF excitation of a human body in prostate and cardiac MRI is only possible to achieve by using phased arrays of antennas attached to the body (so-called surface coils). Due to safety concerns, the design of such coils aims at minimization of the local specific absorption rate (SAR), keeping the highest possible RF signal in the region of interest. Most previously demonstrated approaches were based on resonant structures such as e.g. dipoles, capacitively-loaded loops, TEM-line sections. In this study, we show that there is a better compromise between the transmit signal {{bf{B}}}_{{bf{1}}}^{{boldsymbol{+}}} and the local SAR using non-resonant surface coils generating a low electric field in the proximity of their conductors. With this aim, we propose and experimentally demonstrate a leaky-wave antenna implemented as a periodically-slotted microstrip transmission line. Due to its non-resonant radiation, it induces only slightly over half the peak local SAR compared to a state-of-the-art dipole antenna but has the same transmit efficiency in prostate imaging at 7 T. Unlike other antennas for MRI, the leaky-wave antenna does not require to be tuned and matched when placed on a body, which makes it easy-to-use in prostate imaging at 7 T MRI.

Real-time assessment of potential peak local specific absorption rate value without phase monitoring: Trigonometric maximization method for worst-case local specific absorption rate determination.

PurposeMulti‐transmit MRI systems are typically equipped with dedicated hardware to sample the reflected/lost power in the transmit channels. After extensive calibration, the amplitude and phase of the signal at the feed of each array element can be accurately determined. However, determining the phase is more difficult and monitoring errors can lead to a hazardous peak local specific absorption rate (pSAR10g) underestimation. For this purpose, methods were published for online maximum potential pSAR10g estimation without relying on phase monitoring, but these methods produce considerable overestimation. We present a trigonometric maximization method to determine the actual worst‐case pSAR10g without any overestimation.Theory and MethodThe proposed method takes advantage of the sinusoidal relation between the SAR10g in each voxel and the phases of input signals, to return the maximum achievable SAR10g in a few iterations. The method is applied to determine the worst‐case pSAR10g for three multi‐transmit array configurations at 7T: (1) body array with eight fractionated dipoles; (2) head array with eight fractionated dipoles; (3) head array with eight rectangular loops. The obtained worst‐case pSAR10g values are compared with the pSAR10g values determined with a commonly used method and with a more efficient method based on reference‐phases.ResultsFor each voxel, the maximum achievable SAR10g is determined in less than 0.1 ms. Compared to the reference‐phases‐based method, the proposed method reduces the mean overestimation of the actual pSAR10g up to 52%, while never underestimating the true pSAR10g.ConclusionThe proposed method can widely improve the performance of parallel transmission MRI systems without phase monitoring.

Global and peak local specific absorption rate control on parallel transmit systems using k-means SAR compression model.

To improve the specific absorption rate (SAR) compression model capability in parallel transmission (pTx) MRI systems. A k-means clustering method is proposed to group voxels with similar SAR behaviors in the scanned object, providing a controlled upper-bounded estimation of peak local SARs. This k-means compression model and the conventional virtual observation point (VOP) model were tested in a pTx MRI framework. The pTx pulse design with different SAR controlling schemes was simulated using a numerical human head model and an eight-channel 7T coil array. Multiple criteria (including RF power, global and peak local SARs, and excitation accuracy) were compared for the performance testing. The k-means compression model generated a narrower overestimation bound, leading to a more accurate local SAR estimation. Among different pTx pulse design approaches, the k-means compression model showed the best trade-off between the SAR and excitation accuracy. The developed SAR compression model is advantageous for pTx framework given the narrower overestimation bound and control over the compression ratio. Results also illustrate that a moderate increase of maximum RF power can be useful for reducing the maximum local SAR deposition.

Bent folded-end dipole head array for ultrahigh-field MRI turns "dielectric resonance" from an enemy to a friend.

To provide transmit whole-brain coverage at 9.4 T using an array with only eight elements and improve the specific absorption rate (SAR) performance, a novel dipole array was developed, constructed, and tested. The array consists of eight optimized bent folded-end dipole antennas circumscribing a head. Due to the asymmetrical shape of the dipoles (bending and folding) and the presence of an RF shield near the folded portion, the array simultaneously excites two modes: a circular polarized mode of the array itself, and the TE mode ("dielectric resonance") of the human head. Mode mixing can be controlled by changing the length of the folded portion. Due to this mixing, the new dipole array improves longitudinal coverage as compared with unfolded dipoles. By optimizing the length of the folded portion, we can also minimize the peak local SAR (pSAR) value and decouple adjacent dipole elements. The new array improves the SEE (< >/√pSAR) value by about 50%, as compared with the unfolded bent dipole array. It also provides better whole-brain coverage compared with common single-row eight-element dipole arrays, or even to a more complex double-row 16-element surface loop array. In general, we demonstrate that rather than compensating for the constructive interference effect using additional hardware, we can use the "dielectric resonance" to improve coverage, transmit field homogeneity, and SAR efficiency. Overall, this design approach not only improves the transmit performance in terms of the coverage and SAR, but substantially simplifies the common surface loop array design, making it more robust, and therefore safer.

Individual variation in simulated fetal SAR assessed in multiple body models.

We generate 12 models from 4 pregnant individuals to evaluate individual differences in local specific absorption rate (SAR) for differing body habitus and fetal and maternal positions. Structural MR images from 4 pregnant subjects (including supine and left-lateral maternal positions) were manually segmented to create 12 body models by rotating the fetus, modifying the fat content, and altering the maternal arm position in 1 of the subjects. Electromagnetic simulations modeled at 3 Tesla determined the average and peak local SAR in the maternal trunk, fetus, fetal brain, and amniotic fluid. We observed a significant range of fetal and maternal peak local SAR across the models (maternal trunk: 19.14-44.03 watts/kg, fetus: 9.93-18.79 watts/kg, fetal brain 3.36-10.3 watts/kg). We found that maternal body habitus changes introduced a significant variation in the maternal peak local SAR but not the fetal local SAR. However, the maternal position (either rotating the mother to left-lateral position or altering the arm position) introduced changes in fetal peak local SAR (range: 11.9-17.9 watts/kg). Rotating the fetus also introduced variation in the fetal and fetal brain peak local SAR. The observed variation in SAR emphasizes the need for more anatomical models to enable better safety management of individuals during fetal MRI, including a wider range of gestational ages.

A deep learning method for image-based subject-specific local SAR assessment.

PurposeLocal specific absorption rate (SAR) cannot be measured and is usually evaluated by offline numerical simulations using generic body models that of course will differ from the patient's anatomy. An additional safety margin is needed to include this intersubject variability. In this work, we present a deep learning–based method for image‐based subject‐specific local SAR assessment. We propose to train a convolutional neural network to learn a “surrogate SAR model” to map the relation between subject‐specific B1+ maps and the corresponding local SAR.MethodOur database of 23 subject‐specific models with an 8–transmit channel body array for prostate imaging at 7 T was used to build 5750 training samples. These synthetic complex B1+ maps and local SAR distributions were used to train a conditional generative adversarial network. Extra penalization for local SAR underestimation errors was included in the loss function. In silico and in vivo validation were performed.ResultsIn silico cross‐validation shows a good qualitative and quantitative match between predicted and ground‐truth local SAR distributions. The peak local SAR estimation error distribution shows a mean overestimation error of 15% with 13% probability of underestimation. The higher accuracy of the proposed method allows the use of less conservative safety factors compared with standard procedures. In vivo validation shows that the method is applicable with realistic measurement data with impressively good qualitative and quantitative agreement to simulations.ConclusionThe proposed deep learning method allows online image‐based subject‐specific local SAR assessment. It greatly reduces the uncertainty in current state‐of‐the‐art SAR assessment methods, reducing the time in the examination protocol by almost 25%.

Intersubject specific absorption rate variability analysis through construction of 23 realistic body models for prostate imaging at 7T.

For ultrahigh field (UHF) MRI, the expected local specific absorption rate (SAR) distribution is usually calculated by numerical simulations using a limited number of generic body models and adding a safety margin to take into account intersubject variability. Assessment of this variability with a large model database would be desirable. In this study, a procedure to create such a database with accurate subject-specific models is presented. Using 23 models, intersubject variability is investigated for prostate imaging at 7T with an 8-channel fractionated dipole antenna array with 16 receive loops. From Dixon images of a volunteer acquired at 1.5T with a mockup array in place, an accurate dielectric model is built. Following this procedure, 23 subject-specific models for local SAR assessment at 7T were created enabling an extensive analysis of the intersubject B1+ and peak local SAR variability. For the investigated setup, the maximum possible peak local SAR ranges from 2.6 to 4.6 W/kg for 8 × 1 W input power. The expected peak local SAR values represent a Gaussian distribution W/kg) with realistic prostate-shimmed phase settings and a gamma distribution Γ(24,0.09) with multidimensional radiofrequency pulses. Prostate-shimmed phase settings are similar for all models. Using 1 generic phase setting, average B1+ reduction is 7%. Using only 1 model, the required safety margin for intersubject variability is 1.6 to 1.8. The presented procedure allows for the creation of a customized model database. The results provide valuable insights into B1+ and local SAR variability. Recommended power thresholds per channel are 3.1 W with phase shimming on prostate or 2.6 W for multidimensional pulses.
