Characterization of brass mesh bolus for electron beam therapy

Purpose. Bolus is often required for targets close to or on skin surface, however, standard bolus on complex surfaces can result in air gaps that compromise dosimetry. Brass mesh boluses (RPD, Inc., Albertville, MN) are designed to conform to the patient's surface and reduce air gaps. While they have been well characterized for their use with photons, minimal characterization exists in literature for their use with electrons.Methods and materials.Dosimetric characteristics of brass mesh bolus was investigated for use with 6, 9 and 12 MeV electrons using a 10×10 cm2applicator on standard multi-energy LINAC. Measurements for bolus equivalence and percentage depth doses (PDDs) under brass mesh, as well as surface dose measurements were performed on solid water and a 3D printed resin breast phantom (Anycubic Photon MonoX, Shenzhen, China) using Markus®parallel-plate ionization chamber (Model 34045, PTW Freiburg, Germany), thermoluminescent detectors (TLD) and EBRT film. After obtaining surface dose measurements, these were compared to dose calculated on the Pinnacle3 treatment planning system (TPS, 16.2, Koninklijke Philips N.V.).Results. Measurements of surface dose under brass mesh showed consistently higher dose than without bolus, confirming that brass mesh can increase the PDD at surface up to ∼ 94% of dose at dmax, depending on incident electron energy. This increase is equivalent to using ∼ 7.2 mm water equivalent bolus for 6 MeV, ∼ 3.6 mm for 9 MeV and ∼ 2.2 mm bolus for 12 MeV electrons. TPS results showed close agreement within-vivomeasurements, confirming the potential for brass mesh as bolus for electron irradiation, provided blousing effect is correctly modelled.Conclusions. To increase electron surface dose, a brass mesh can be used with equivalent effect of water-density bolus varying with electron energy. Proper implementation could allow for ease of treatment, as well as increase bolus conformality in electron-only plans.

Development of novel ionization chambers for reference dosimetry in electron flash radiotherapy

AbstractBackgroundReference dosimetry in ultra‐high dose rate (UHDR) beamlines is significantly hindered by limitations in conventional ionization chamber design. In particular, conventional chambers suffer from severe charge collection efficiency (CCE) degradation in high dose per pulse (DPP) beams.PurposeThe aim of this study was to optimize the design and performance of parallel plate ion chambers for use in UHDR dosimetry applications, and evaluate their potential as reference class chambers for calibration purposes. Three chamber designs were produced to determine the influence of the ion chamber response on electrode separation, field strength, and collection volume on the ion chamber response under UHDR and ultra‐high dose per pulse (UHDPP) conditions.MethodsThree chambers were designed and produced: the A11‐VAR (0.2–1.0 mm electrode gap, 20 mm diameter collector), the A11‐TPP (0.3 mm electrode gap, 20 mm diameter collector), and the A30 (0.3 mm electrode gap, 5.4 mm diameter collector). The chambers underwent full characterization using an UHDR 9 MeV electron beam with individually varied beam parameters of pulse repetition frequency (PRF, 10–120 Hz), pulse width (PW, 0.5–4 µs), and pulse amplitude (0.01–9 Gy/pulse). The response of the ion chambers was evaluated as a function of the DPP, PRF, PW, dose rate, electric field strength, and electrode gap.ResultsThe chamber response was found to be dependent on DPP and PW, and these dependencies were mitigated with larger electric field strengths and smaller electrode spacing. At a constant electric field strength, we measured a larger CCE as a function of DPP for ion chambers with a smaller electrode gap in the A11‐VAR. For ion chambers with identical electrode gap (A11‐TPP and A30), higher electric field strengths were found to yield better CCE at higher DPP. A PW dependence was observed at low electric field strengths (500 V/mm) for DPP values ranging from 1 to 5 Gy at PWs ranging from 0.5 to 4 µs, but at electric field strengths of 1000 V/mm and higher, these effects become negligible.ConclusionThis study confirmed that the CCE of ion chambers depends strongly on the electrode spacing and the electric field strength, and also on the DPP and the PW of the UHDR beam. A significant finding of this study is that although chamber performance does depend on PW, the effect on the CCE becomes negligible with reduced electrode spacing and increased electric field. A CCE of ≥95% was achieved for DPPs of up to 5 Gy with no observable dependence on PW using the A30 chamber, while still achieving an acceptable performance in conventional dose rate beams, opening up the possibility for this type of chamber to be used as a reference class chamber for calibration purposes of electron FLASH beamlines.

Long-term stability of a PTW 34070 large-area parallel ionization chamber in clinical proton scanning beams.

In the modeling of beam data for proton therapy planning systems, absolute dose measurements are performed utilizing a Bragg peak chamber (BPC), which is a parallel-plate ionization chamber. The long-term stability of the BPC is crucial for ensuring accurate absolute dose measurement. The study aims to assess the long-term stability of the BPC in clinical proton pencil beam scanning delivery. The long-term stability evaluation focused on the BPC-Type 34070 (PTW Freiburg, Germany), utilizing clinical proton scanning beams from December 2022 to November 2023. Monthly investigations were conducted to evaluate the response and cross-calibration factor of the BPC and a reference chamber, employing the spread-out Bragg peak (SOBP) field. Additionally, assessments were made regarding the BPC's response to monoenergetic beams, along with an examination of the impact of polarity and ion recombination on the BPC. The response and cross-calibration factor of the BPC varied up to 1.9% and 1.8%, respectively, while the response of the reference chamber remained within a 0.5% range. The BPC's response to the mono-energetic beams varied up to 2.0% across all energies, demonstrating similar variation trends in both the SOBP field and mono-energetic beams. Furthermore, the variations in polarity and ion recombination effect remained stable within a 0.4% range throughout the year. Notably, the reproducibility of the BPC remained high for each measurement conducted, whether for the SOBP field or mono-energetic beams, with a maximum deviation observed at 0.1%. The response and cross-calibration factor of the BPC demonstrated significant variations, with maximum changes of 1.9% and 1.8%, respectively. However, the reproducibility of the BPC remained consistently high for each measurement. It is recommended that when conducting absolute dose measurements using a BPC, its response should be compared and corrected against the reference chamber for each measurement.

Skin dose investigations on a 0.5 T parallel rotating biplanar linac-MR using Monte Carlo simulations and measurements.

The Alberta rotating biplanar linac-MR has a 0.5 T magnetic field parallel to the beamline. When developing a new linac-MR system, interactions of charged particles with the magnetic field necessitate careful consideration of skin dose and tissue interface effects. To investigate the effect of the magnetic field on skin dose using measurements and Monte Carlo (MC) simulations. We develop an MC model of our linac-MR, which we validate by comparison with ion chamber measurements in a water tank. Additionally, MC simulation results are compared with radiochromic film surface dose measurements on solid water. Variations in surface dose as a function of field size are measured using a parallel plate ion chamber in solid water. Using an anthropomorphic computational phantom with a 2mm-thick skin layer, we investigate dose distributions resulting from three beam arrangements. Magnetic field on and off scenarios are considered for all measurements and simulations. For a 20×20cm2 field size, (the minimum dose to the hottest contiguous 0.2 cc volume) for the top 2mm of a simple water phantom is 72% when the magnetic field is on, compared to 34% with magnetic field off (values are normalized to the central axis dose maximum). Parallel plate ion chamber measurements demonstrate that the relative increase in surface dose due to the magnetic field decreases with increasing field size. For the anthropomorphic phantom, (minimum skin dose in the hottest 1×1×1 cm3 cube) shows relative increases of 20%-28% when the magnetic field is on compared to when it is off. With magnetic field off, skin is 71%, 56%, and 21% for medial-lateral tangents, anterior-posterior beams, and a five-field arrangement, respectively. For magnetic field on, the corresponding skin values are 91%, 67%, and 25%. Using a validated MC model of our linac-MR, surface doses are calculated in various scenarios. MC-calculated skin dose varies depending on field sizes, obliquity, and the number of beams. In general, the parallel linac-MR arrangement results in skin dose enhancement due to charged particles spiraling along magnetic field lines, which impedes lateral motion away from the central axis. Nonetheless, considering the results presented herein, treatment plans can be designed to minimize skin dose by, for example, avoiding oblique beams and using a larger number of fields.

Active and passive dosimetry for beta-emitting radiopharmaceutical therapy agents in a custom SPECT/CT compatible phantom

Objective. This work introduces a novel approach to performing active and passive dosimetry for beta-emitting radionuclides in solution using common dosimeters. The measurements are compared to absorbed dose to water (D w) estimates from Monte Carlo (MC) simulations. We present a method for obtaining absorbed dose to water, measured with dosimeters, from beta-emitting radiopharmaceutical agents using a custom SPECT/CT compatible phantom for validation of Monte Carlo based absorbed dose to water estimates. Approach. A cylindrical, acrylic SPECT/CT compatible phantom capable of housing an IBA EFD diode, Exradin A20-375 parallel plate ion chamber, unlaminated EBT3 film, and thin TLD100 microcubes was constructed for the purpose of measuring absorbed dose to water from solutions of common beta-emitting radiopharmaceutical therapy agents. The phantom is equipped with removable detector inserts that allow for multiple configurations and is designed to be used for validation of image-based absorbed dose estimates with detector measurements. Two experiments with 131I and one experiment with 177Lu were conducted over extended measurement intervals with starting activities of approximately 150–350 MBq. Measurement data was compared to Monte Carlo simulations using the egs_chamber user code in EGSnrc 2019. Main results. Agreement within k = 1 uncertainty between measured and MC predicted D w was observed for all dosimeters, except the A20-375 ion chamber during the second 131I experiment. Despite the agreement, the measured values were generally lower than predicted values by 5%–15%. The uncertainties at k = 1 remain large (5%–30% depending on the dosimeter) relative to other forms of radiation therapy. Significance. Despite high uncertainties, the overall agreement between measured and simulated absorbed doses is promising for the use of dosimeter-based RPT measurements in the validation of MC predicted D w.

A fast response time gas ionization chamber detector with a grid structure.

The time response characteristic of the detector is crucial in radiation imaging systems. Unfortunately, existing parallel plate ionization chamber detectors have a slow response time, which leads to blurry radiation images. To enhance imaging quality, the electrode structure of the detector must be modified to reduce the response time. This paper proposes a gas detector with a grid structure that has a fast response time. In this study, the detector electrostatic field was calculated using COMSOL, while Garfield++ was utilized to simulate the detector's output signal. To validate the accuracy of simulation results, the experimental ionization chamber was tested on the experimental platform. The results revealed that the average electric field intensity in the induced region of the grid detector was increased by at least 33%. The detector response time was reduced to 27% -38% of that of the parallel plate detector, while the sensitivity of the detector was only reduced by 10%. Therefore, incorporating a grid structure within the parallel plate detector can significantly improve the time response characteristics of the gas detector, providing an insight for future detector enhancements.

Dosimetry of ultra-high dose rate electron beams using thermoluminescence and optically stimulated luminescence detectors

Objective. The aim of this work is to investigate the dose rate dependence of thermoluminescence and optically stimulated luminescence detectors (TLDs and OSLDs) in a wide uniform ultra-high dose rate electron beam and demonstrate the potential use of TLDs and OSLDs to correct the ion recombination in air-filled ionization chambers. This study avoids previously reported complications related to the field size and homogeneity. Approach. Two types of OSLDs (BeO and Al2O3:C) and three types of TLDs (LiF:Mg,Ti, LiF:Mg,Cu,P, CaF2:Tm) were irradiated simultaneously in a uniform 16 MeV electron beam generated by a clinically decommissioned C-Arm LINAC, modified to deliver doses per pulse between 8.3 × 10−4 Gy and 1.255 Gy, corresponding to instantaneous dose rates between 2 × 102Gy s−1 and 3 × 105 Gy s−1. A prototype ultra-thin parallel plate ionization chamber was employed as reference detector. Main results. Reproducible results were achieved both at conventional (standard deviation of the data <2%) and at the highest dose per pulse (standard deviation of the data <4%). No trend in the dose rate response of the TLDs and OSLDs was observed in the investigated dose per pulse range. The Al2O3:C OSLD was found to be the most precise detector, with a standard deviation of the data <2% at all investigated dose rates and dose levels. Significance. The dose rate independence of the investigated TLDs and OSLDs make them good candidates for dosimetry at ultra-high dose rates, at least up to 3 × 105 Gy s−1. A dose rate independent method to measure the dose per pulse is proposed, which can be applied to characterize ultra-high dose rate electron beams and correct for ion recombination in ionization chambers.

Development and establishment of a parallel plate ionization chamber as a transfer standard for measurement of dose to tissue in beta radiation fields

A parallel-plate ionization chamber (PPC) with a nominal volume of 8.16 cm³ was developed based on theoretically simulated design parameters. Its purpose is to serve as a transfer standard for dosimetry in a beta radiation field. The entrance window of the PPC consists of an aluminized Mylar sheet with a thickness of 1.4 mg/cm2. The collecting and guard electrodes are created by applying a graphite coating on a Poly Methyl Methacrylate (PMMA) substrate with a thickness of 5 mm. The nominal sheet resistance of the graphite-coated PMMA substrate was measured using a four-probe technique and found to be approximately 800 Ω per square (Ω/□). Dosimetric characterization of the PPC was performed in the ISO 6980 reference beta radiation field, utilizing 90Sr-90Y and 85Kr beta radiation sources. The assessment included studies on short-term stability, linearity, current-to-voltage characteristics, stabilization time, and leakage current. The PPC was calibrated and established as a transfer standard using the ‘Extrapolation Ionization Chamber,' recognized as an absolute standard for dose to tissue in 90Sr-90Y and 85Kr beta sources within the laboratory. The calibration coefficient of the PPC indicates an energy dependence of 0.6 % for 90Sr-90Y and 85Kr beta sources.

Charge collection efficiency of commercially available parallel-plate ionisation chambers in ultra-high dose-per-pulse electron beams

Objective. This investigation aims to experimentally determine the charge collection efficiency (CCE) of six commercially available parallel-plate ionisation chamber (PPIC) models in ultra-high dose-per-pulse (UHDPP) electron beams. Approach. The CCE of 22 PPICs has been measured in UHDPP electron beams at the National Metrology Institution of Germany (PTB). The CCE was determined for a dose per pulse (DPP) range between 0.1 and 6.4 Gy (pulse duration of 2.5 μs). The results obtained with the different PPICs were compared to evaluate the reproducibility, intra- and inter-model variation, and the performance of a CCE empirical model. Main results. The intra-model variation was, on average, 4.0%, which is more than three times the total combined relative standard uncertainty and was found to be greater at higher DPP (up to 20%). The inter-model variation for the PPIC with 2 mm electrode spacing, which was found to be, on average, 10%, was also significant compared to the relative uncertainty and the intra-model variation. The observed CCE variation could not be explained only by the expected deviation of the electrode spacing from the nominal value within the manufacturing tolerance. It should also be noted that a substantial polarity effect, between 0.914(5) and 1.201(3), was observed, and significant intra- and inter-model variation was observed on this effect. Significance. For research and pre-clinical study, the commercially available PPIC with a well-known CCE (directly measured for the specific chamber) and with a small electrode spacing could be used for relative and absolute dosimetry with a lower-limit uncertainty of 1.6% (k = 1) in the best case. However, to use a PPIC as a secondary standard in UHDPP electron beams for clinical purposes would require new model development to reduce the ion recombination, the polarity effect, and the total standard uncertainty on the dose measurement.

Development and evaluation of a standard for absorbed dose to water from alpha-particle-emitting radionuclides

Targeted radionuclide therapy and brachytherapy with alpha particles has gained significant clinical relevance recently. Absorbed dose traceability to a standard is currently lacking in the dosimetry chain. The short range of alpha particles in water of <100 μm complicates the absorbed dose measurements in the form of significant attenuation and perturbation effects. The aim of this work was to develop and validate a standard for absorbed dose to water from alpha-particle-emitting radionuclides. A dosimetric formalism to realize surface absorbed dose to water per unit activity using a windowless cylindrical parallel-plate ion chamber (IC) was introduced. IC-based and Monte Carlo (MC)-based correction factors were calculated for a planar circular 210Po alpha-particle emitter. The measured absorbed dose to air was compared to the MC-calculated absorbed dose. A parallel-plate IC with a nominal 4 mm collector diameter composed of polystyrene-equivalent material was utilized as a standard. MC simulations were performed using the TOPAS MC code and finite source size, backscatter, and emission angle divergence correction factors were calculated by modeling the IC and the source. Multiple measurement trials were performed to measure ionization current at air gaps in the 0.3 mm to 0.525 mm range. The proposed dosimetric formalism was employed to calculate the surface absorbed dose to water from a point-like 210Po source. The recombination and polarity correction factors were measured to be <0.50% when a 150 V mm−1 electric field strength was applied. The MC-calculated and measured absorbed dose to air agreed within 2.05%. The finite source size and backscatter corrections were <10% with the emission angle divergence correction being in the 93%–239% range. The surface absorbed dose to water was measured to be 2.8913 × 10−6 Gy s−1 Bq−1 with a combined uncertainty of 4.23% at k = 1. This work demonstrated the ability of a windowless parallel-plate IC as a standard for absorbed dose from alpha-particle-emitting radionuclides.

An Investigation of High-Z Material for Bolus in Electron Beam Therapy

Electron beams are frequently used for superficial tumors including: primary skin cancers, surgical scars, cutaneous lymphomas and benign conditions like keloids. However, due to electron beam characteristics the surface dose is 75-95% of the prescribed dose depending on beam energy thus requiring placement of bolus to augment surface dose. Various types of boluses; wet gauge/towel, superflab, superstuff, aquaplast, 3D printed plastics, customized dot-decimal bolus, thermoshield, brass and tantalum wire mesh are commonly used in clinics with variable difficulties that are not perfect. As majority of these devices do not snug to the skin contour and create air-gap that is known to produce significant dose perturbations creating hot and cold spot. The variable dosimetry has cosmetic implications and potentially increased risk of recurrence. A new cloth-like high-Z materials; Tungsten, (Z = 74) and Bismuth, (Z = 83) impregnated with silicone gel are now commercially available which is investigated in this study for bolus purpose. Commercially available, super soft silicone-gel based submillimeter thin Tungsten and Bismuth sheets were investigated for bolus properties in electron beams in the range of 6-12 MeV using 10x10 cm2 applicator. These materials were tested on various body contour for perfect snug without any air traps. Using parallel plate ion chamber measurements were performed in a solid water phantom on a medical linear accelerator machine. Depth dose characteristics were measured to optimize the thickness for surface dose to be 100% for selected electron therapy. Surface dose for a 10x10 cm2 cone for 6, 9, 12 MeV beams were measured to be 75.1%, 80.1% and 86.2%, respectively. Silicone-gel Tungsten and Bismuth sheets produce significant electrons thus increasing surface dose. Based on measured depth dose, our data showed that Tungsten sheets of 0.14 mm, 0.18 mm and 0.2 mm and Bismuth sheets of 0.42 mm, 0.18 mm and 0.2 mm provide 100% surface dose for 6, 9 and 12 MeV beams, respectively without any changes in depth dose except increasing surface dose. The new Tungsten and Bismuth based silicone-gel clothlike sheets are extremely soft but high tensile metallic bolus materials that can fit flawlessly on any skin contour without any air trap or variable thickness or spillage of the water as in conventionally used wet gauge. Only 0.2 mm thick sheets are needed for 100% surface dose without degradation of the depth dose characteristics. These materials are reusable and ideal for bolus in electron beam treatment. This investigation opens a new frontier in designing new bolus materials optimum for patient treatment.

Dosimetric evaluation of high-Z inhomogeneity with modern algorithms: A collaborative study

PurposeTo evaluate modern dose calculation algorithms with high-Z prosthetic devices used in radiation treatment. MethodsA bilateral hip prosthetic patient was selected to see the effect of modern algorithms from the commercial system for plan comparisons. The CT data with dose constraints were sent to various institutions for dose calculations. The dosimetric parameters, D98%, D90%, D50% and D2% were compared. A water phantom with an actual prosthetic device was used to measure the dose using a parallel plate ionization chamber. ResultsDosimetric variability in PTV coverage was significant (>10%) among various treatment planning algorithms. The comparison of PTV dosimetric parameters, D98%, D90%, D50% and D2% as well as organs at risk (OAR) have large discrepancies compared to our previous publication with older algorithms (https://doi.org/10.1016/j.ejmp.2022.02.007) but provides realistic dose distribution with better homogeneity index (HI). Backscatter and forward scatter attenuation of the prosthesis was measured showing differences <15.7% at the interface among various algorithms. ConclusionsModern algorithms dose distributions have improved greatly compared to older generation algorithms. However, there is still significant differences at high-Z-tissue interfaces compared to the measurements. To ensure accuracy, it's important to take precautions avoiding placing any prosthesis in the beam direction and using type C algorithms.

Quality assurance of six cylindrical and two parallel plate chambers by radioactive check device: Influence of chamber age to its performance

PurposeTo evaluate the performances of eight ionization chambers, (two types) manufactured in a time span of 14 years, with radioactive source devices and photon beams. MethodsThe measurements were performed with six Farmer and two parallel plate ionization chambers with appropriate radioactive check devices and accompanying inserts. Some tests were performed with photon beams at linear accelerator. Physical visibility check, stability, linearity of response, directional dependence, determination of leakage current, ion recombination and polarity effect and influence of background radiation tests were conducted and analyzed. ResultsThe performance of all Farmer ionization chambers was in agreement to IEC standard as well as for parallel plate type chambers. Long-term stability of older planparallel chamber was somewhat worse than expected. ConclusionRadioactive check devices have proven to be easy to use in clinical environment. According to results of this work Farmer type ionization chambers showed good quality regardless of the time of production, which make them long-term suitable for clinical dosimetry in radiotherapy. Also, for parallel plate type of ionization chambers the results showed fine agreement with the IEC standard, except long-term stability for older chamber, which needs further investigation.

Dosimetric effects and impacts caused by a carbon fiber table and its accessories in a linear accelerator

Objective : Most contemporary treatment planning systems (TPS) exclude the couch during treatment planning. The present investigation aimed to evaluate the impact of couches and accessories on radiation therapy planning by quantifying the degree of attenuation for two-photon beam energies, 6 MV and 10 MV, at two different field sizes, 5 cm x 5 cm and 10 cm x 10 cm. &#x0D; Methods: An x-ray radiation beam at two energies (6 MV and 10 MV) generated by a linear accelerator (Elekta Synergy). The output dose was measured using a digital parallel plate ionization chamber of the IBA cc13 type (IBA Dosimetry, Germany), and the readings were recorded with a 2944 Farmer Type chamber. Different commercially available couch tops and accessories evaluated in this study.&#x0D; Results: The results show that the highest attenuation values were observed at 1300 gantry angles for both energies and field sizes. The lowest attenuation values were recorded at 1000 gantry angles with 6 MV energy for 5cm x 5cm and 10cm x 10cm field sizes. The breast board with D level demonstrated the highest attenuation of 6.27% and 5.51% for 6 MV and 10 MV energies, respectively.&#x0D; Conclusion: the findings indicate that the degree of attenuation is not uniform across all angles and may exceed tolerable limits, indicating the need for careful consideration during treatment planning to ensure optimal treatment outcomes.

Analysis of different protocols using an Exradin A12 cylindrical chamber for electron beam reference dosimetry

The Technical Report Series 398 (TRS-398), Electron Dosimetry Working Party (EWDP), and Task Group 51 (TG 51) are the most important protocols for reference dosimetry. In the case of electron beam reference dosimetry, these protocols recommend using parallel-plate ionization chambers for beams with R50 values below specific thresholds. However, recent papers suggested using cylindrical chambers for reference dosimetry of all electron beam energies. Here we compared different protocols using a cylindrical chamber with the recommendations of using a parallel-plate chamber and the TRS-398 formalism for the dosimetry of several electron beam energies. We employed electron beams with nominal energies of 4, 6, 9, 12, and 15 MeV of a Varian 2100C linear accelerator, an Exradin A12, and an Exradin P11 chamber for the analysis. The results showed differences below 3% when comparing the cylindrical chamber and alternative protocols with the parallel-plate chamber and the TRS-398 formalism for electron beams reference dosimetry. These results can bring confidence in using a cylindrical chamber for reference electron beam dosimetry, which can make the electron beam dosimetry procedure simpler and faster.

Characterization of a new radiochromic film (LD-V1) using mammographic beam qualities

PurposeRadiochromic film (RCF) is a detector that can obtain a two-dimensional dose distribution with high resolution; it is widely used in medical and industrial fields. Several types of RCFs exist based on their application. The type of RCF mainly used for mammography dose assessment has been discontinued; however, a new type of RCF (LD-V1) has been distributed as a successor. Since the medical use of LD-V1 has rarely been studied, we investigated the response characteristics of LD-V1 in mammography. MethodsMeasurements were performed using Mo/Mo and Rh/Ag on a Senographe Pristina mammography device (GE, Fairfield, CT, USA). The reference air kerma was measured using a parallel-plate ionization chamber (PPIC) (C-MA, Applied Engineering Inc, Tokyo, Japan). Pieces of LD-V1 film model were irradiated at the same position where the reference air kerma in air was measured by the PPIC. Irradiation was performed using the time scale method based on the load on the equipment. Two methods of irradiation were considered: placing the detector in air and on the phantom. The LD-V1 was scanned five times at 72 dpi in RGB (48 bit) mode using a flatbed scanner (ES-G11000, Seiko Epson Corp, Nagano, Japan) 24 h following irradiation. The response ratio of the reference air kerma and the air kerma obtained from the LD-V1 were compared and examined for each beam quality and air kerma range. Results and discussionWhen the beam quality was altered, the response ratio varied from 0.8 to 1.2 with respect to the measurement value of the PPIC; however, some outliers were observed. Response ratios were highly variable in the low-dose range; however, as the air kerma increased, the ratios approached 1. Thus, LD-V1 does not need calibration for each beam quality used in mammography. LD-V1 enables air kerma evaluation by creating air kerma response curves under certain X-ray conditions used in mammography. ConclusionWe suggest that the dose range be limited to 12 mGy or more to keep the response variation with beam qualities below ±20%. If further measurement is required for reducing the response variation, the dose range should be shifted to a higher dose range.

Implementation of novel measurement-based patient-specific QA for pencil beam scanning proton FLASH radiotherapy.

Several studies have shown pencil beam scanning (PBS) proton therapy is a feasible and safe modality to deliver conformal and ultra-high dose rate (UHDR) FLASH radiation therapy. However, it would be challenging and burdensome to conduct the quality assurance (QA) of the dose rate along with conventional patient-specific QA (psQA). To demonstrate a novel measurement-based psQA program for UHDR PBS proton transmission FLASH radiotherapy (FLASH-RT) using a high spatiotemporal resolution 2D strip ionization chamber array (SICA). The SICA is a newly designed open-air strip-segmented parallel plate ionization chamber, which is capable of measuring spot position and profile through 2mm-spacing-strip electrodes at a 20kHz sampling rate (50μs per event) and has been characterized to exhibit excellent dose and dose rate linearity under UHDR conditions. A SICA-based delivery log was collected for each irradiation containing the measured position, size, dwell time, and delivered MU for each planned spot. Such spot-level information was compared with the corresponding quantities in the treatment planning system (TPS). The dose and dose rate distributions were reconstructed on patient CT using the measured SICA log and compared to the planned values in volume histograms and 3D gamma analysis. Furthermore, the 2D dose and dose rate measurements were compared with the TPS calculations of the same depth. In addition, simulations using different machine-delivery uncertainties were performed, and QA tolerances were deduced. A transmission proton plan of 250MeV for a lung lesion was planned and measured in a dedicated ProBeam research beamline (Varian Medical System) with a nozzle beam current between 100 to 215nA. The worst gamma passing rates for dose and dose rate of the 2D SICA measurements (four fields) compared to TPS prediction (3%/3mm criterion) were 96.6% and 98.8%, respectively, whereas the SICA-log reconstructed 3D dose distribution achieved a gamma passing rate of 99.1% (2%/2mm criterion) compared to TPS. The deviations between SICA measured log, and TPS were within 0.3ms for spot dwell time with a mean difference of 0.069±0.11 s, within 0.2mm for spot position with a mean difference of -0.016±0.03mm in the x-direction, and -0.036±0.059mm in the y-direction, and within 3% for delivered spot MUs. Volume histogram metric of dose (D95) and dose rate (V40Gy/s ) showed minimal differences, within less than 1%. This work is the first to describe and validate an all-in-one measurement-based psQA framework that can fulfill the goals of validating the dose rate accuracy in addition to dosimetric accuracy for proton PBS transmission FLASH-RT. The successful implementation of this novel QA program can provide future clinical practice with more confidence in the FLASH application.

Comparison of the Characteristics of Two Types of Parallel-plate Ionization Chamber Under Small-field Electron Irradiation.

This study compared two types of parallel-plate ionization chamber to clarify the pitfalls of dosimetry in electron radiation therapy. The ion recombination correction factor and polarity effect correction factor, sensitivity, and percentage depth doses (PDDs) of PPC05 and PPC40 parallel-plate ionization chambers were compared in a small-field electron beam. The output ratios were measured for 4-20 MeV electron beams with field sizes of 10 cm × 10 cm, 6 cm × 6 cm, and 4 cm × 4 cm. Furthermore, the films were placed in water and positioned in the beam with their surface perpendicular to the beam axis, and lateral profiles were obtained for each beam energy and each field. Regarding PDDs, at depths greater than the peak dose, the percentage depth dose for PPC40 was smaller than that for PPC05 in small fields and at beam energies greater than 12 MeV, which could be attributed to the lack of lateral electron equilibrium at small depths and multiple scattering events at large depths. The output ratio of PPC40 was approximately 0.025-0.038, which was lower than that of PPC05 in a 4 cm × 4 cm field. For large fields, the lateral profiles were similar, regardless of the beam energy, however, for small fields, the flatness of the lateral profile was beam energy dependent. The PPC05 chamber, which has a smaller ionization volume, is therefore more suitable than the PPC40 chamber for small-field electron dosimetry, in particular at high beam energies.

Dosimetric evaluation of an intraoperative radiotherapy system: a measurement-based and Monte-Carlo modelling investigation

Intraoperative radiotherapy (IORT) is a specialised subset of radiotherapy, where a high radiation dose is delivered to a surgically exposed tumour bed in order to eradicate any remaining cancer cells. The aim of this study was to examine the dose characteristics of the Zeiss Intrabeam IORT device which provides near-isotropic emission of up to 50 kV X-rays. The EGSnrc Monte Carlo (MC) code system was used to simulate the device and percentage depth dose (PDD) data measured with a soft X-ray parallel-plate ionisation chamber were used for model verification. The model provided energy spectra, isodose curves and mean photon energies. In addition, EBT3 Gafchromic film was used to verify the MC model by examining PDDs and 2D dose distributions for various applicators. The differences between MC model and ionisation chamber measurements were within 3% for most points, with a maximum deviation of ~ 9%. Most of the simulated PDD points were within 5% of the film-measured data, with a maximum deviation of ~ 10%. The mean energy of the bare probe was found to be 21.19 keV. The mean photon energy from applicators ranged from 29.00 to 30.85 keV. Results of this study may be useful for future work on creating a system for treatment planning.

Commissioning and Performance Evaluation of Varian TrueBeam Linear Accelerator

Aim: The aim of this study is to present a report on the commissioning results of the Varian TrueBeam machine with available photon energies so that it can benefit medical physicists during the entire commissioning process. Subjects and Methods: All tests were performed as per the recommendation of the Atomic Energy Regulatory Board. The beam profiles and percentage depth dose of both photon and electron beams were measured with Sun nuclear Radiation Field Analyzer and 0.125 cc ionization chamber. Output accuracy and consistency of photon and electron beams were measured using 0.6 cc cylindrical ionization chamber and 0.350 cc parallel plate ionization chamber, respectively. To measure the congruency of radiation and optical field and isocenter check with respect to gantry, collimator, and couch, self-developed films were used. Results: Isocenter shift due to gantry, collimator, and couch, rotation was found to be within ± 2-mm diameter circle. Congruency of optical and radiation fields was measured for symmetric fields 5 cm × 5 cm, 10 cm × 10 cm, 20 cm × 20 cm, and 30 cm × 30 cm for all available energy and was within ± 2 mm. Dosimetric leaf gap value for the multi-leaf collimator was measured with 0.125 cc ionization chamber and values are −0.853 mm, −1.106 mm, −1.097 mm, −0.765 mm, and −1.005 mm for energies 6 MV, 10 MV, 15 MV, 6 flattening filter free (FFF), and 10 FFF, respectively. Conclusions: All electrical, mechanical, dosimetric, and safety measurement were found to be within permissible limits. Before clinical use, the beam data are crosschecked with point dosimetry, portal dosimetry, and MapCHECK two-dimensional detector array and results are within tolerance.
