Technical Error Factors On Panoramic Radiographic Examination At The Radiology Installation Of Unjani Dental and Mouth Hospital

Panoramic is a three-dimensional curved zone where anatomical structures are well defined on panoramic radiography. Panoramic radiography includes a detailed anatomy of the jaw complex with some superimpositions and distortions that can be exacerbated by technical errors in image acquisition as well as many extra-jaw anatomical structures that are able to give clear description to interpretate. Therefore, in obtaining a panoramic radiograph it is necessary to obtain the desired structure with high quality and minimal distortion. Errors that often occur in producing good panoramic radiographs are technical errors or when positioning the patient and during the panoramic examination. The purpose of this study was to determine the main factors causing the errors so that technician can produce good quality radiographs, take into account patient placement at the time of examination, perform film processing, and handle general film procedures. The research method used was a quantitative method with a descriptive observational approach. The number of samples was 64 radiographs, which are adult patients over 18 years old with the use of exposure factors in the range of 60kV, 10mA and 16.4s. All radiographs were taken by qualified radiographers with more than 5 years of experience. The radiographs were viewed and observed without any modification on a computed radiography monitor screen. Radiographic results will be evaluated by experienced technician who have worked more than 5 years. Evaluation is obtained by analyzing the data descriptively in the form of assessment of the percentage of rejection of panoramic radiographs to make conclusions. Out of the 2418 panoramic radiographs viewed, 2354 (97%) radiographs had no errors while 64 (3%) radiographs showed one or more positioning error. The most common positioning error observed in the radiographs was failuer to position chin tipped high (56,02%). Whereas the least common error recorded were movement and apron shadow (1.15%).

THE CORRELATION LEVEL OF KNOWLEDGE OF DENTAL NURSING STUDENTS' ANXIETY REGARDING THE HAZARDS OF RADIATION IN PANORAMIC EXAMINATION AND DENTAL RADIOGRAPHY

Knowledge is information that has been combined with understanding and the potential to act; which then sticks in someone's mind. Anxiety is an unreal fear, a feeling of being threatened in response to something that is not actually threatening, according to psychodynamic theory anxiety arises when self-will demands detachment from the ego. Panoramic radiography is a supporting tool that can be used to diagnose a case, such as the presence of a jaw fracture, symmetrical or asymmetrical evaluation of the TMJ or knowing the depth of caries.In radiological examinations, especially dental and panoramic, there are many worries or fears that arise when an examination is carried out. So it is necessary to increase knowledge in order to reduce the anxiety that arises.This type of research used is descriptive correlational research. The population and sample in this study were Diploma III students at the Department of Dental Nursing, Poltekkes Semarang who coincidentally (accidentally) met with researchers who were considered suitable with the specified sample characteristics to be sampled.The results of the Spearman rank test on the relationship between the level of knowledge and the anxiety level of the respondents obtained the Spearman rank correlation value (Rs) of -0.600, meaning that the higher the patient's level of knowledge about panoramic and dental radiographic examinations, the lower the patient's anxiety level. All respondents had no anxiety when dealing with radiological examinations, especially dental and panoramic examinations.

Multiple compound odontomas in various regions of the maxilla: a rare case report

Backgrounds: Odontomas often occur at a young age and are found incidentally in disturbed tooth eruptions, mostly intraosseous lesions. Compound odontomas have been identified as small tooth-like structures known as denticles. An accurate diagnosis of odontoma cannot be made on clinical examination. The majority of odontoma diagnoses have been made by radiographic examination. Odontoma often occurs unilaterally in one area with a single or multiple numbers, in this case, several areas have been found in the maxilla. The purpose of this case report is the radiographic evaluation of compound odontomas using panoramic radiographs and 3D Cone Beam CT.
 Case reports: A 12 years old female patient with her mother came to the orthodontist with the main complaint of crowding teeth and wanted treatment. On examination, an inflammation appeared on the palate behind the left RA lateral incisor. There was no history of trauma to the orofacial area. There was no family history of unerupted teeth or hypodontia and nothing relevant medical history. The orthodontist referred the patient for a panoramic radiograph and CBCT examination.
 Results: The results of the panoramic radiograph examination have showed radiopaque images resembling denticles in several regions, was made the case has been interesting to appoint in this case report, and this result was confirmed by the results of the CBCT examination which can show the location, number, and size of denticles in this patient.

Dentigerous cyst associated with impaction of right maxillary third molar on panoramic radiograph and CBCT: a case report

Objectives: The aim of this case report is to description of a dentigerous cyst associated with impaction of right maxillary third molar on panoramic radiographs and cone-beam computed tomography (CBCT).
 Case Report: A 61-year-old male patient came to the Radiology Installation of RSGM Saraswati Denpasar to do perform a panoramic radiograph examination of tooth 15 which had cavities, pain and swelling of the upper right cheek. The results of panoramic radiograph examination showed tooth 15 had caries reaching the pulp and there was a radiolucent lesion at the apikal tooth 15, there was a radiolucent lesion from the mesio-distal CEJ on the crown of the maxillary right third molar that had not erupted, the borders were well-defined, corticated, unilocular, involving the apical of teeth 15,16 and 17. A few days later the patient came back to do CBCT to see more clearly the boundaries, diameter of the lesion and see the relationship of the lesion with surrounding structure.
 Conclusion: Panoramic radiography and CBCT can be used as a supporting examination in making a diagnosis. The dentigerous cyst associated with impacted right maxillary third molar is clearly visible on the panoramic radiograph and CBCT so that it can assist in planning the appropriate treatment for the patient.

Radiographic appearance of ossifying fibroma in the left mandible: a case report

This article reports on ossifying fibroma (OF) which was established based on panoramic radiographic, CBCT and histopathological examination and treatment performed on a 31 years old male patient. The diagnosis is made by comparing with existing theories in the literature.
 Case Report: A 31 years old man was referred to the Oral Surgery Department of Al Ihsan Hospital. The patient complained of swelling in the lower left jaw. On palpation it feels hard and cannot be moved. Panoramic radiograph examination showed loss of teeth 34-35 and a radiolucent lesion mixed with radiopaque in the left mandible which resulted in a shift in the position of teeth 36 and 37 more superiorly. The CBCT examination performed revealed a mixed radiolucent and radiopaque lesion of teeth 33-38. Histopathological examination also showed the presence of cellular fibrous with a mineralized component. The patient has been treated in the form of excision of the lesion.
 Conclusion: CBCT can be used as a reliable supporting examination in helping to diagnose cases of benign neoplasms involving hard bone tissue such as ossifying fibroma. OF has distinctive features on radiographs, one of which is the presence of mixed radiolucent and radiopaque lesions with wispy septa which result in resorption and displacement of the teeth involved. The accuracy of the diagnosis of OF can be enforced by a combination of clinical, radiographic and HPA examinations, so that the treatment given to patients is according to the procedure.

Incidental finding of impacted supernumerary teeth in cone-beam computed tomography 3D

Objectives: To evaluate the value of cone-beam CT (CBCT) in the diagnosis and orientation of impacted supernumerary teeth in the dental arches.
 Case Report: A 50 year old man came to the Udayana University Hospital with the chief complaint of missing bilateral posterior teeth of the mandible and wanted to make denture. Before the treatment begin, a panoramic radiographic examination was performed. The panoramic image shows an impacted supernumerary teeth on the inferior from lower right premolar. Due to the inability to determine the precise position of those teeth within the mandible and the possible vital structures surrounding, a CBCT imaging was taken. Examination of the images from the CBCT shows an impacted supernumerary teeth in the area of teeth 44 and 46.
 Conclusion: The position of the supernumerary teeth is varied in the mandible, and often causes permanent dentition complications. In this case CBCT imaging two supernumerary teeth were found in the tooth area 44-45 and 45-46. apical of the two germ teeth appear to be fully formed. Supernumerary crowns between teeth 44-45 inclined buccally & between teeth 45-46 lingually. CBCT is crucial for exact localization, for treatment planning, and for the surgical approach in cases of multiple supernumerary teeth.

Bilateral bifid condyle mandibula features in panoramic radiography: a case report

Objectives: The aim of this case report is to describe radiographically the specific features of bifid mandibular condyle that frequently found incidentally on panoramic radiographs.
 Case Report: A 24-year-old man came to the Installation of Dental Radiology RSGM UNPAD brought a referral letter for a panoramic photograph. From anamnesis is known patient have complaints of pain in the jaw joints left and right at the time of open mouth. Panoramic radiographs show mesioangular dental impaction of 48, and edentulous tooth 36 and presence of bifid condyle on the right and left condyle. Bifid condyle usually becomes incidental finding on routine panoramic radiographic examination. Bifid condyle features can be shown by depression or notch on the superior surface of condyle. Bifid condyle can provide a "heart-shaped" from the frontal view. For a definite feature can be seen with other modality such as CBCT. Sometimes there is also a change in the mandibular fossa to compensate the morphological changes of the condyle.
 Conclusion: Bifid condyle mandible is a variation in the shape of the condyle whose cause is not known with certainty. The bifid condyle gives the "heart-like" appearance that is often found on accidental panoramic radiographs. Generally, cases of BMC do not require treatment unless they cause symptoms.

Development of formulae for dental age estimation using digital radiographs in the Mongolian population

This study aimed to examine the applicability of the pulp/tooth ratio (PTR) method for age estimation in Mongolian populations using panoramic radiographs and derive new regression formulae. Moreover, we aimed to assess the accuracy of these formulae in other subjects from the Mongolian population and compare them with the other formulae derived from different Asian populations.The total sample size of the study was 381. The formulae were derived from the examination of panoramic radiographs of 271 individuals aged 15–62 years. Following Cameriere’s method, PTR was calculated for the upper and lower canine teeth. Linear regression analyses were performed between the actual age and that obtained from upper-lower canine PTR and established formulae for age estimation. To verify the formulae, two types of test samples were collected: 73 panoramic radiographs and 37 periapical radiographs. The estimated age was calculated using our new formulae and three other formulae derived from Asian populations.The correlation coefficient between the actual age and that obtained by PTR was significantly negative for both canines. According to our new regression formulae, the differences between the estimated age and actual age showed a bell-shaped curve distribution in both test groups. While using the other formulae derived from the Asian population, the distribution patterns obtained were significantly different in the Mongolian population.This study was the first to examine the relationship between actual age and PTR in Mongolian population, and these results advance the field of forensic science in Mongolia.

Calcified carotid artery atheromas in individuals with cognitive dysfunction

Objective The aim of this case-control study was to investigate whether cognitively impaired individuals have a higher burden of calcified carotid artery atheroma (CCAA) than controls without cognitive impairment. Material and methods The study included 154 cases with Alzheimer’s disease (n = 52), mild cognitive impairment (n = 51), or subjective cognitive decline (n = 51) diagnosed at a university memory clinic. Seventy-six cognitively healthy controls were sampled through the Swedish population register. All participants underwent clinical oral and panoramic radiographic examinations. Two oral and maxillofacial radiologists performed blinded analyses of the panoramic radiographs for signs of CCAA, which was registered as absent or present and, if present, unilateral or bilateral. Consensus assessment was used for all statistical analyses. Results CCAA was common (40%) in this middle-aged and older Swedish population. We found no differences in the prevalence of CCAA between cases and controls (40% vs. 42%). Conclusion Cognitively impaired patients do not have a higher burden of CCAA than matched controls without cognitive impairment.

Prevalence and Quality of Endodontic Treatment in Patients with Cardiovascular Disease and Associated Risk Factors.

This study aimed to determine the prevalence and quality of endodontic treatment, by radiographically assessing the periapical periodontitis and endodontic treatment status in patients with cardiovascular disease (CVD) and cardiovascular risk (CVR) factors. Patients who visited the Out Patient Department of Institute of Dental Sciences and Department of Cardiology, Institute of Medical Sciences and SUM Hospital, Siksha ‘O’ Anusandhan University, Bhubaneswar, from August 2021 to February 2022, for a check-up or dental problem were considered as participants in this study. After obtaining informed consent, the participants were enrolled on the Oral Infections and Vascular Disease Epidemiology Study (INVEST) IDS, BHUBANESWAR. After testing negative for COVID-19, patients’ demographic details, such as age and gender were recorded, followed by a panoramic radiographic examination (OPG). A total sample of 408 patients were divided into three groups: Group 1/control (without any cardiovascular manifestation) consisting of 102 samples, group 2 of 222 CVR patients, and group 3 of 84 CVD cases. The CVR and CVD groups had a preponderance of elderly age groups between 60 to 70 years, with a significantly higher proportion of males. Co-morbidities such as diabetes mellitus, hypertension, and dyslipidemia were significantly associated with the CVR and CVD groups. From OPG interpretation, it was observed that the periapical radiolucency was greater in the CVR and CVD groups than in the control group (p = 0.009). The prevalence of endodontically treated teeth was higher in CVR and CVD than in the control group (p = 0.028). A high prevalence of dental caries, about 70%, was reported in all three groups (p = 0.356). The presence of dental restoration among all the groups was low (p = 0.079). The proportion of periodontal bone loss in the control group was significantly lower than CVR and CVD (p = 0.000). There was a strong association between periapical radiolucency, endodontically treated teeth, and periodontal bone loss in CVR and CVD patients. Notably, the associations reported herein do not reflect a cause-effect relationship; however, individuals with endodontic pathologies may accumulate additional risk factors predisposing them to hypertension or other CVDs. The results emphasize that eliminating local infections may decrease the systemic infection burden.

KERATOCYST OF THE BUCCAL MUCOSA: A CASE REPORT

Keratocyst of the buccal mucosa is rare, with approximately 20 cases reported to date. A 58-year-old male patient was referred for evaluation of an exuberant swelling of the midface of 2 years' duration. Intraoral examination revealed an asymptomatic tender swelling in the right buccal mucosa, with a smooth surface and normal coloration, measuring 5 cm. Extraosseous location was confirmed by panoramic radiographic examination. Aspiration revealed a yellowish creamy material. Incisional biopsy was performed, and multiple small soft tissue fragments were microscopically evaluated. There was a cyst lined with 6 cell layers composed of columnar palisading basal cells and a corrugated parakeratinizing surface. The capsule was composed of fibrous connective tissue with blood vessels, areas of hemorrhage, nerve bundles, and absence of pilosebaceous units. The final diagnosis was of keratocyst of the buccal mucosa. The odontogenic origin of keratocyst from buccal mucosa is still debatable.

Panoramic radiographic examination revealed a multilocular radiolucent intraosseous lesion, with well-defined limits and radiopaque structure inside. Cone beam computed tomography showed root apices resorption of teeth 45, 46, and 47; expansion of the buccal and lingual cortical bone; and close contact of the lesion with the inferior alveolar vasculature-nervous bundle. Incisional biopsy revealed a calcifying odontogenic cyst. Surgical treatment was performed through enucleation, followed by curettage of the intraosseous lesion, under general anesthesia. The 1-year postoperative follow-up showed complete bone regeneration and there were no signs of recurrence. PERIPHERAL DENTINOGENIC GHOST CELL TUMOR: CASE REPORT

Peripheral dentinogenic ghost cell tumor (DGCT) is a rare benign mixed epithelial and mesenchymal odontogenic neoplasm, with approximately 52 cases reported in the literature. A 48-year-old woman was referred for evaluation of a painless, slow-growing swelling in the mandible caused by a removable denture that evolved over 1 year. Her medical history was unremarkable. Intraoral examination showed a pinkish, firm nodule, with erythematous areas, in the left mandibular alveolar ridge. Panoramic radiographic examination revealed no signs of bone involvement. Under the diagnostic hypothesis of inflammatory fibrous hyperplasia, an incisional biopsy was performed. Histopathologic analysis disclosed sheets and islands of odontogenic epithelium, with areas closely resembling ameloblastoma. Ghost cells, dentinoid material, and inflammatory foreign body reaction with multinucleated giant cells were also observed. The final diagnosis was peripheral DGCT. Treatment consisted of surgical excision, and no signs of recurrence were detected after 2 months of clinical follow-up.

CORRELATION OF PANORAMIC RADIOGRAPHIC FINDINGS AND CLINICAL FINDINGS OF DENTAL PATIENTS AT MOI TEACHING AND REFERRAL HOSPITAL, KENYA

Purpose: The purpose of this study was to determine the correlation of panoramic radiographic findings and clinical findings among dental patients at Moi Teaching and Referral Hospital (MTRH), Kenya.
 Materials and Methods: This was a hospital based cross sectional study conducted at the Radiology and Imaging department and dental department at Moi Teaching and Referral Hospital from September, 2019 to June, 2020. A total of 93 consenting patients were enrolled using systematic random sampling. Standard chart review forms were administered, and clinical examination findings documented. Panoramic radiograph findings were then discussed with a consultant radiologists and findings recorded. Descriptive statistics were carried out. Categorical variables were summarized as frequencies and proportions, and reported in tables. Numerical variables were summarized as median and interquartile ranges. Cross tabulation was done to compare clinical examination and panoramic radiograph findings where total raw agreement was reported as proportions.
 Results: 93 patients whose ages ranged from 5-73 years with a mean of 29 years were included into the study. Radiographic features of dental caries were present in 54% of panoramic radiographs compared to clinical examination (50.5%) while 23.7% of radiographs revealed impacted teeth compared to clinical examination (19.4%). Radiographic features of periodontitis were observed in 14% compared to clinical examination (16.1%). Fractures (12.9%) were observed radiographically compared to (10.7%) clinically. Periapical lesions (8.6%) were observed radiographically compared to clinical examination (6.4%). Temporo-mandibular disorder (6.5%) were observed both on radiographs and clinical examination. Radiographic features of missing teeth (3.2%) were observed compared to clinical examination (1.1%) while only 1.1% of malpositioned teeth were observed radiographically compared to clinical examination (2.2%). Notably, mandibular lesions (3.2%) and nasal congestion (14%) were only discovered radiographically. The overall raw agreement between panoramic radiograph and clinical examination was 75.3% (70/93) with a p value of less than 0.001.
 Unique contribution to theory, practice and policy: The study recommends that all patients presenting with dental conditions should be done panoramic radiograph to improve diagnosis. Moi Teaching and Referral Hospital and the Ministry of Health to consider routine panoramic radiographs in the guidelines for the assessment of dental patients in all the hospitals offering, dental health services

An evaluation of the prevalence of elongated styloid process in Taiwanese population using digital panoramic radiographs

Background/purposeStyloid process (SP) was an anatomical structure located in front of the stylomastoid foramen and the occurrence of styloid process elongation was uncertainly reported. The purpose of this study was to survey, using digital panoramic radiography, the lengths of styloid process (SP) and the prevalence of elongated SP in the Taiwanese population. Their relationships with age, gender and laterality were also determined. Materials and methodsA total of 539 patients (including 53 that were referred from the Department of Otorhinolarygngology with clinical symptoms) were studies retrospectively. Panoramic radiographic examinations were performed on all these patients at the Outpatient clinics of the Department of Stomatology, Taichung Veterans General Hospital between January 2019 and December 2019. Patients were divided into groups according to their age and gender. The length of SP from the base of temporal bone junction to the tip of the process was measured using the Sirona software. Data were analyzed using SPSS version 22.0 based on tests of Chi-square, Mann–Whitney U and Kruskal Wallis tests. Statistical significance was set at p < 0.05. ResultsA total of 539 (240 males and 299 females) digital panoramic radiographs were measured and evaluated in this study. The mean length of SP was 30 ± 0.7 mm on the right side and 29 ± 0.7 mm on the left side. The average length of SP on both sides was 29.5 ± 0.7 mm. In those cases with symptomatic Eagle syndrome, the mean length of SP was 32 ± 0.8 mm on the right side and 33 ± 0.8 mm on the left side (average 32.5 ± 0.8 mm). These lengths were statistically different between the general population and the symptomatic patients. Furthermore, the mean length of SP was 29 ± 0.7 mm in females and 30 ± 0.7 mm in males. The length of the SP grew progressively with age, and was more clearly seen within subjects from 41 to >60 years of age (p < 0.005). Meanwhile, the prevalence of elongated SP in our study was 41.5% on the right side and 36.2% on the left side, with female predominance. ConclusionThe mean length of SP was 30 ± 0.7 mm (right side) and 29 ± 0.7 mm (left side). The prevalence of elongated SP was 41.5% on the right side, and 36.2% on the left side. Our findings were consistent with some previous studies that were differences related with age, and were more prominent in those patients with symptoms.

Gardner syndrome

A 58-year-old male patient was referred for a panoramic radiograph after having presented with a hard swelling of the right mandible. Panoramic radiographic examination (Fig. 1) demonstrated multiples dental anomalies and variable bone densities in both jaws prompting clinicians to “dig” further. Teeth 17, 12, 11, 26 and 27 were missing. Teeth 13, 23, 24, 25, 38, 34, 33 and 43 were unerupted. An unerupted supernumerary left mandibular molar appeared in the region of the mandibular coronoid process. Multiple, small, well-defined radiopacities of density comparable to odontogenic material were evident in the anterior maxilla and mandible. These opacities often demonstrated radiolucent borders consistent with the finding of multiple odontomas. Both jaws demonstrated variable bone density. Cottonwool like opacities partially blending into adjacent trabeculae were evident in the mandible and maxilla posteriorly. The contour of the inferior border of the mandible bilaterally in proximity to the angle was irregular with multiple, well-defined, smooth, lobulated homogenous radiopacities suggestive of osteomas. The radiographic features of multiple osteomas, odontomas, variable bone density, supernumerary and unerupted teeth warranted the referral of the patient for gastroenterological investigation to exclude Gardner Syndrome.

Biomarker of buccal mucosa cells damaged after exposure to panoramic radiography: a literature review

Objectives: This review aimed to understand the effect of exposure to panoramic radiographs on exfoliated buccal mucosal cells at the cellular level.&#x0D; Review: The dose of radiation exposure in dentistry, both intraoral and extraoral, has been regulated by The National Radiological Protection Board (NRPB). However, even though it is given in small doses, x-ray radiation due to intraoral and extraoral radiographs still has a radiobiological effect on the exposed tissue. The radiobiological effects of X-ray exposure can cause changes in biological molecules, either directly or indirectly, within hours or days. There are two classification of this radiobiological effect, called deterministic and stochastic effect. The deterministic effect occurs when the dose given exceeds the recommended dose by the NRPB, whereas the stochastic effect does not have any threshold that needs to be exceeded to give some adverse impact to the exposed tissue One method used as a predictor or biomarker of genetic damage due to exposure to physical or chemical mutagenic agents in humans is micronucleus (MN). The biomarker for the cell damaged is the change of nucleus shape and outline, called pycnosis, karyolysis, karyorrhexis.&#x0D; Conclusion: The exposed to x-ray from panoramic could induce cell and genetic damaged. Prescription for panoramic radiographic examination in patients should be as effectively as possible according to the principles of ALADA (as low as diagnostically acceptable) to avoid adverse effects on the exposed tissue.

Quiste dentígero en odontología. Reporte de caso

El quiste dentígero es definido como una lesión quística que involucra la corona de un diente incluido que está relacionada con la unión amelocementaria. Clínicamente se manifiesta por un aumento del volumen asintomático, siendo generalmente descubierto en un examen radiográfico de rutina o por la investigación de un diente no erupcionado. Radiográficamente presenta una lesión radiolúcida unilocular asociada a un diente incluido. Este estudio presenta un caso clínico de un paciente con quiste dentígero en la región posterior de la mandíbula, tratado quirúrgicamente por medio de enucleación y exodoncia de los dientes involucrados. Paciente de género masculino, de 9 años de edad, que compareció a la Clínica de caso Odontología de la Universidad de Ribeirão Preto (UNAERP), Brasil, con queja de abultamiento inferior izquierdo de la cara y marcada asimetría facial, la radiografía panorámica y tomografía computarizada evidenciaron la presencia de un quiste dentígero extenso. Delante de este cuadro clínico y radiográfico, el plan de tratamiento establecido fue la enucleación del quiste para permitir la erupción de los dientes sucesores permanentes, seguido por la exodoncia de los elementos dentarios 74/75. El tratamiento quirúrgico se mostró rápido y resolutivo evidenciando que la elección del tratamiento debe ser cuidadosamente evaluado para cada caso.

The prevalance and radiographic evaluation of mandibular second premolar migration: A retrospective study

Purpose:To investigate the prevalence and radiographic characteristics of migrated mandibular second premolars byusing panoramic radiography.Materials &amp; Methods:The previously taken 27758 panoramic radiographs were evaluated regarding the determining theprevalence of mandibular second premolar migration. Thirty-six radiographs were excluded from the study due to notproviding the inclusion criteria. 27722 radiographs were evaluated retrospectively by the same operator. In the diagnosisof migrated teeth, radiographic features and demographic characteristics were recorded.Results:Nine migrated second premolar teeth were detected in the evaluation of all the radiographs (%0.03). The meanage of the cases was 37 years. All the migrated teeth were impacted, unilateral and the average distance between normaland migrated location was 41 mm. In most of migration case (in 6 case) absence of first permanent molar tooth wasobserved.Conclusion:It is concluded that, if the mandibular second premolar tooth are not located at physiologic position on thedental arch, a panoramic radiographic examination should be recommended to view all the jaws and related structures.Early detection of dental migration may be beneficial to prevent the possible further complications.

BACKGROUND Most commonly occurring impacted teeth encountered in dental surgery are mandibular 3rd molars. Reasons of impactions may be due to evolution and modern dietary habits, lack of space, decreased skeletal growth, and disproportionate crown size. Impacted tooth may cause various complications, if left untreated distal caries is commonly seen in mandibular 2nd molars. The aim of the current study was to determine the prevalence of distal caries of mandibular 2nd molars due to impacted mandibular 3rd molars. Symptoms like distal caries of 2rd molars is one of the major factors affecting the oral health next to periodontal disease and periodontal abscess. METHODS The study included 103 patients, reported to the dental department of Tripura Medical College and BRAM &amp;Teaching Hospital, Agartala. The study was done from July 2019 – December 2019, age ranged from 18 to 50 years. Panoramic radiographic examinations were carried out in each patient. Angulations of impaction and incidence of caries of the adjacent mandibular 2nd molars were recorded. All data were collected and analysed using percentage analysis. RESULTS 103 patients panoramic radiographs were studied, out of 103, 63 (61 %) were male, 40 (39 %) were female. 31 patients (30.1 %) out of 103 patients were found to have distal caries of mandibular 2nd molars adjacent to mandibular 3nd molars. 85 % of caries was associated with mesioangular impaction. CONCLUSIONS Careful monitoring to detect the caries lesions in early stage is necessary, as the distal caries on the mandibular 2nd molars was found to be high. Subsequently proper measures should be taken for restoration of mandibular 2nd molars and removal of impacted mandibular 3rd molars, if necessary.
