Demonstrating the Value of Interventional Radiology.

While national healthcare expenditures per capita in the United States exceed those in all other Organisation for Economic Co-operation and Development (OECD) countries, measures of health outcomes in the United States lag behind those in peer nations. This combination of high healthcare spending and relatively poor health has led to attempts to identify high- and low-value healthcare services and to develop mechanisms to reimburse health care providers based on the value of the care delivered. This article investigates the meaning of value in healthcare and identifies specific services delivered by interventional radiologists that have accrued evidence that they meet criteria for high-value services. Recognizing the shift in reimbursement to high-value care, it is imperative that interventional radiology (IR) develop the evidence needed to articulate to all relevant stakeholders how IR contributes value to the system.

Trends in National Pharmaceutical Expenditure in Korea during 2011 - 2020.

This study aimed to identify the trends in pharmaceutical expenditure (PE), share of PE in health expenditure (HE), and trends in expenditure by pharmacological groups (ATC level 1 classification) in Korea for a 10-year period (2011 - 2020) and compare the data with those of other Organisation for Economic Co-operation and Development (OECD) countries. Using the findings, we determined the current status of pharmaceutical expenditure (PE) management in Korea and derived the implications for establishing future macroscopic policies on PE. We analyzed the OECD Health Statistics and the Korean national health insurance claims database from January 2011 through December 2020. The outcome measures were HE, PE, and pharmaceutical sales data for ATC level 1 medicines from OECD Health Statistics data during 2011 - 2020. As OECD collects limited ATC level 1 data, we used the HIRA health insurance claims data for PEs of ATC level-1 classification, including D, L, P, and S. PE in Korea increased by 38.5% from 19.9 billion USD in 2011 to 27.6 billion USD in 2020, whereas the share of PE in HE decreased by 6.3%p from 26.4% in 2011 to 20.1% in 2020. In 2020, Korea ranked third in PE per capita (760.9 USD PPP) and had the highest share of PE (20.1%) among the 19 OECD countries studied. By ATC level 1 class, the highest PE was A (alimentary tract and metabolism) at 4.3 billion USD, and L (antineoplastic and immunomodulating agents) had the highest increase at 13.4%; in contrast, J (anti-infectives for systemic use) had the lowest increase in annual average PE at -0.2% in 2020 relative to 2011. Among the 17 OECD countries, Korea had the highest and the third-highest expenditures for ATC codes A and J, respectively. PE in Korea has continued to increase between 2011 and 2020, indicating the need for macroscopic management of PE. Our results on PE by ATC code may help health authorities in establishing future policies on PE.

Women Physicians in Academic Medicine of Japan.

Japan is a well-qualified country in promoting scientific advancement, but female scientists are too few in academic medicine positions. The government of Japan announced that the share of women in leadership positions accounted to at least 30% by 2020 in all fields in society. The number of female students also increased, but it was not much higher than other Organisation for Economic Co-operation and Development (OECD) countries. Women students always have higher passing frequencies in national examination for medical practitioners in Japan. The potential gap between physician gender and academic advancement is mentioned in any medical fields. Women physicians in academic medicine position are still few. For women physicians, medical and familial situations are inversely affected by the coronavirus disease 2019. We propose the recommendations to support women physicians’ right in academic medicine, accordingly to patients’ benefit. Women may be stepping up and leading efforts without titles or positions in ways that are significant and meaningful for their group or organization.

Trusting the Trust Game: An External Validity Analysis with a UK Representative Sample

Using a nationally representative sample of 1052 respondents from the United Kingdom, we systematically tested the associations between the experimental trust game and a range of popular self-reported measures for trust, such as the General Social Survey (GSS) and the Rosenberg scale for self-reported trust. We find that, in our UK representative sample, the experimental trust game significantly and positively predicts generalised self-reported trust in the GSS. This association is robust across a number of alternative empirical specifications, which account for multiple hypotheses corrections and control for other social preferences as measured by the dictator game and the public good game, as well as for a broad range of individual characteristics, such as gender, age, education, and personal income. We discuss how these results generalise to nationally representative samples from six other Organisation for Economic Co-operation and Development (OECD) countries (France, Germany, Italy, Korea, Slovenia, and the US).

Policy coherence across Agenda 2030 and the Sustainable Development Goals: Lessons from Finland

AbstractMotivationAgenda 2030 and the Sustainable Development Goals (SDGs) expand the development agenda. While all major development actors support policy coherence for development (PCD), analysis has been lacking on how this can be achieved as the development agenda expands. We discuss the relationship between SDGs and PCD through a comprehensive case study from Finland.PurposeTo examine the coherence of Finland’s foreign and development policy for achieving the SDGs, while leaving no one behind. Particular focus is given to policies related to the private sector’s role in development, and attention is also paid to ministries other than the Ministry for Foreign Affairs.Approach and methodsWe carried out interviews with policy‐makers and other stakeholders, and analysed relevant documents. We reviewed governmental high‐level policy statements, sectoral alignments, and development aid documents. We also reviewed the position papers that the Finnish Government had issued on European Union (EU) processes, particularly regarding development relating to the private sector.FindingsFinland has been uniquely positioned to advance PCD in its foreign policy, thanks to co‐ordination structures across government. Nonetheless, mainstreaming of the expanded development agenda has been largely limited to the Ministry for Foreign Affairs and its development policy department. The breadth of the SDG agenda has enabled individual targets to be cherry‐picked, with less attention paid to advancing the Agenda 2030 as a whole and to implementing its Leave No One Behind (LNOB) principle. Despite an institutional framework seemingly ideal for policy coherence, traditional sectoral divisions between development policy as a separate field and sustainability as an environmental issue, remain. Five factors in particular hindered policy coherence: (a) Finland’s position papers to the EU on taxes and migration all but ignored Agenda 2030 commitments; (b) the Finnish emphasis on the private sector in development narrowed the considerations of development to economic growth; (c) sustainability was seen as green technology, with scant regard to social sustainability; (d) private firms interpreted the SDGs to mean that environmental sustainability could address human rights‐related concerns with corporate social responsibility initiatives; and (e) cuts to staffing in the Finnish Government stymied innovative thinking and working across departmental boundaries.Policy ImplicationsFinland is generally seen as a front‐runner in mainstreaming development issues, which makes it an interesting case. We outline the key challenges that Finland has faced in tackling PCD, which should be relevant for other Organisation for Economic Co‐operation and Development (OECD) countries as well. Many challenges related to PCD are political and organizational. As such, they are highly dependent on the particular institutional settings in each country. Our methodological approach could be replicated in other similar countries.

A Review of the Admission System for Mental Disorders in South Korea.

This study presents a comprehensive overview of the characteristics of mental health problems and admission system in South Korea. We compared the mental health-related indicators data from South Korea to data from other Organization for Economic Cooperation and Development (OECD) countries. South Korea was identified as the country with the highest suicide rate, the longest length of stay in hospitals for mental disorders, and the highest number of psychiatric care beds. These results can be explained by considering the admission system for mental disorders. We reviewed the admission system and the Mental Health Promotion and Welfare Act, providing direction for improving the system.

60Years after Kefauver: Household income required to buy prescription drugs in the United States and abroad.

Assessing drug prices relative to income in the US compared to other Organization for Economic Co-Operation and Development (OECD) countries provides context for policymakers seeking to improve access and affordability. Using current drug p. rice and income data, we recreate a historical analysis presented in 1960 to the Senate Subcommittee on Antitrust and Monopoly led by Sen. Estes Kefauver. We identified frequently prescribed generic and brand name drugs for US and international comparison by drug price category (low-price generics, mid-price brands, and high-price specialty brands) as a function of income. We further extend our analysis to consider US prices relative to the current Federal Poverty Level (FPL). For the low-price drugs, all fell below 1% of all of the US income levels presented. Mid-price drugs were below 10% of income for those at the US median household income level but approached 30% of income for those at the FPL. High-price drugs varied greatly, reaching over 600% FPL for one product. Americans receive bargain prices on par with international comparators for many low-priced generics drugs. For commonly used mid-priced drugs or high-priced specialty products, whether or not drug prices are considered a bargain in the US compared to international markets may depend on individual income. External reference pricing policies may help inform the negotiation for some drug prices, but affordability may still be limited for lower wage earners.

Progress and problems of gender equality in Japanese academics and geosciences

Abstract. In its gender equality status, Japan lags behind the European Union (EU) and other Organisation for Economic Co-operation and Development (OECD) countries. Only 16.7 % of academic staff in national universities are women. Although there has been a slow improvement during the last 45 years, the percentage of female staff remains particularly low in the STEM (science, technology, engineering, and mathematics) fields. In 2018, female stuff accounted for only 12.3 % in agriculture, 8.7 % in science, and 6.2 % in engineering. The number of female graduate students and the number of female members of the Japanese geoscience societies have been steadily increasing since the 1998 campaign that increased the total number of graduate school places. However, low representation of females in academic job positions, as well as among the recipients of academic awards, suggests that a strong unconscious bias against women still exists in Japan. Continuing public attention to these issues is essential for the improvement of the situation.

Health service utilisation and unmet healthcare needs of Australian children from immigrant families: A population-based cohort study.

Compared with most other Organization for Economic Co-operation and Development (OECD) countries, information about the patterns of health service use for children from immigrant families in Australia is currently limited, and internationally, data on unmet healthcare needs are scarce. This study aims to examine the distribution of health service utilisation and unmet healthcare needs for immigrant children aged 10-11years in Australia. We drew on data from the Longitudinal Study of Australian Children Birth (B; n=5,107) and Kindergarten (K; n=4,983) cohorts. The exposure was family immigration background collected at 0-1 (B-cohort) and 4-5 (K-cohort) years. Outcomes were parent-reported child health service use and unmet healthcare needs (defined as the difference between services needed and services received) at 10-11years. Logistic regression analyses were used to examine associations between family immigration background and health service use/unmet healthcare needs, adjusting for potential confounders. Results showed that one-third of Australian children (B-cohort: 29.0%; K-cohort: 33.4%) came from immigrant families. There were similar patterns of health service use and unmet healthcare needs between children from English-speaking immigrant and Australian-born families. However, children from non-English-speaking immigrant families used fewer health services, including paediatric, dental, mental health and emergency ward services. There was a disparity between the services used when considering children's health needs, particularly for paediatric specialist services (B-cohort: OR=2.43, 95% CI 1.11-5.31; K-cohort: OR=2.72, 95% CI 1.32-5.58). Findings indicate that Australian children from non-English-speaking immigrant families experience more unmet healthcare needs and face more barriers in accessing health services. Further effort is needed to ensure that the healthcare system meets the needs of all families.

The number of international students in Turkish universities was increased in recent years. Despite the increasing number of international students in Turkey, it is still below from the other Organization for Economic Co-operation and Development (OECD) countries. To understand the reasons of this situation, it is crucial to examine the selection criteria of international students. The main objective of this study was to examine the most important factors that affect students’ choice to study abroad. The study was designed as quantitative research. A correspondence analysis was conducted to identify the information from international students who chose Turkey for higher education. As a multivariate analysis technique, correspondence analysis graphically portrays the choice criteria of international students and their country of residence in a single joint space. A survey was developed by the researchers and distributed to the international students at two public universities in Turkey and the data were collected from 281 international students. The results revealed that the two-dimensional solution was accepted with a significant chi-square value and variances accounted for 99% of the total variance explained. Specifically, the selection criteria for an institution in Turkey and Eskişehir differ with international students’ home country.

Urban Air Pollutant from Motor Vehicle Emissions in Kuala Lumpur, Malaysia

The increasing amount of motor vehicles that emit pollutants are contributing significantly to urban air pollution, be it in industrial or developing countries. This study investigates the emission of particulate matter (PM10) from exhaust and non-exhaust sources and gaseous pollutants, such as carbon monoxide (CO) and nitrogen oxide (NOx) from several different classes of motor vehicles in the tropical city of Kuala Lumpur. Air pollutants from fuel consumption were obtained from emission factors, while non-exhaust particulate matter was estimated from the United States Environmental Protection Agency (U.S. EPA) Compilation of Air Pollutant Emissions Factors (AP-42). The total PM10 emissions from all classes of motor vehicles estimated from the tail-pipe exhaust was 1,029,883 kg, while non-exhaust sources were 1,573,539 kg. Emissions of PM10 from newly registered private cars was the most dominant at 214,427 kg, followed by emissions from motorcycles at 118,582 kg in 2014. Private cars also contributed 14,605 kg of CO and 5,726 kg of NOx in 2014, compared with 9,830 kg of CO and 3,854 kg of NOx in 2010. Comparison with other Organisation for Economic Co-operation and Development (OECD) countries shows that the total emissions for PM10 and NOx were lower in Malaysia than in most countries, but the CO emissions here were higher than in Asian countries such as Japan and Korea, as well as in other European countries. Various strategies and policies should be implemented by the local authorities and government agencies to reduce emissions from the transportation sector in urban areas to improve the quality of the urban environment, human health, and the urban community.

The 2017 Tax Cuts: How Polarized Politics Produced Precarious Policy

By lowering the corporate tax rate from 35% to 21%, the 2017 tax legislation brought the U.S. statutory rate into closer alignment with the rates applicable in other Organisation for Economic Co-operation and Development (OECD) nations, thereby decreasing the incentive for businesses to locate their deductions in the United States and their income abroad. Its overhaul of the U.S. international income tax rules simultaneously reduced preexisting incentives for U.S. multinationals to reinvest their foreign earnings abroad and put a floor on the benefits of shifting profits to low-tax jurisdictions. The 2017 legislation also added an unprecedented, troublesome lower rate for the income of certain categories of businesses operated as partnerships or Subchapter S corporations.In combination, the provisions of the new law have created significant new differences in income tax based on what kind of business is being conducted, where goods and services are bought and sold, to and from whom they are bought, where and how assets are owned, the taxpayer’s size, whether individual workers are employees or independent contractors, and where people live and work. The 2017 law also portends unsustainable increases in deficits and the national debt. The new tax system produced by this legislation provides neither an effective nor stable solution to the nation’s economic and fiscal challenges.

The imperial treasury: appraisal methodology and regional economic performance in the UK

ABSTRACTThe disparity between the least and most productive regions in the UK is wide by the standards of many other Organisation for Economic Co-operation and Development (OECD) economies. An important factor is the concentration of public investment around London. The appraisal process for infrastructure investment projects follows the methodology set out in HM Treasury’s The Green Book. It is argued that this methodology has reinforced the regional imbalance; that recent changes are unlikely by themselves to redress the London bias in infrastructure; and that infrastructure investments also need to be based on a strategic view about economic development for the whole of the UK.

Shared decision making: Does a physician's decision-making style affect patient participation in treatment choices for primary immunodeficiency?

Overall health care spending in the United States is equivalent to more than 15% of GDP, yet outcomes rank below the top 25 in most quality categories when compared with other Organization for Economic Cooperation and Development (OECD) countries. The majority of spending is consumed by small patient populations with chronic diseases. Experts believe increased patient‐physician shared decision making (SDM) should result in better overall longitudinal care but understanding the physician's role in facilitating SDM is limited. Structural equation modelling was applied to results of a 2016 questionnaire‐based survey of 330 US physicians who treat approximately 55% of primary immune deficiency requiring immune globulin therapy; it tested the relationship between slow/rational vs fast/intuitive decision‐making styles and SDM as mediated by patient‐centric care and moderated by physician's trust in the patient. The results showed a statistically significant relationship between slow/rational decision making and SDM. The results also suggest differences related to age, gender, education, and race but no differences related to trust.

New Evidence on Debt as an Obstacle for US Economic Growth

The US government debt is now in uncharted waters. From the founding of the nation until 1968, government debt moved up and down without a trend, but over the past 50 years, debt relative to the size of the economy has increased continuously. The United States does not appear to have a coherent debt policy. It is not Greece, and there is no evidence of a likely default on US government bonds in the near future. However, there is evidence that Americans have already borne the costs of high debt levels, and without a reform of policy these costs will continue in the future. Using a new econometric technique for threshold autoregression and a debt measure that includes private debt as well as public debt, we estimate that in the period 1995 to 2014, US economic growth was more than 1 percentage point lower than it would have been at a debt level below the threshold. Other Organisation for Economic Co-operation and Development (OECD) countries also had lower growth rates as a result of high debt levels. Many countries have recently adopted some form of fiscal rule, including balanced budgets, intended to limit debt and raise growth rates. Fiscal rules involve a tradeoff between limiting debt and preserving flexibility to respond to economic shocks. In this paper we discuss problems related to designing optimal fiscal rules.

Overeducation, skills and social background: the influence of parental education on overeducation in Spain

ABSTRACTThis article focuses on the influence of social background on overeducation in Spain, understanding family socialisation as a source of knowledge and skills gain. The dramatic education expansion experienced in Spain in combination with a high percentage of low-skilled jobs has promoted overeducation occurrence to a larger extent than in other Organisation for Economic Cooperation and Development (OECD) countries. Using the Survey of Adult Skills (PIAAC) data results suggest that overeducation affects at least over a quarter of the working population. Younger and middle aged workers are more likely to be overeducated compared to the senior ones, while women are more prone to be overeducated than men. Workers whose mothers have higher education are less likely to be overeducated compared to those whose mothers have basic education, while fathers’ education is practically irrelevant to predict workers’ overeducation. Thus, a mother’s education is the most relevant social background indicator to predict overeducation, even when controlling for firm characteristics and skills level.

Foreign Direct Investment in Real Estate and Related Business Services:Poland vs OECD Countries​

Poland has experienced a relatively large inflow of foreign direct investment (FDI) in the last two decades. FDI has been flowing mainly into services and manufacturing similarly to the trend observed in global capital movement. Within the services, the financial intermediation played the biggest role in terms of attracting FDI, but real estate and related business activities (legal, accounting, auditing, tax, marketing, etc.) also saw a high inflow of foreign capital. The paper analyzes the evolution of inward FDI stock in Poland over the period 1998–2012 with a particular emphasis on real estate and related business activities. It also discusses how Poland differs in terms of sectoral decomposition in the services of inward FDI stock from other Organization for Economic Cooperation and Development (OECD) countries representing both Central and Eastern Europe (CEE) and advanced economies. Finally, the paper answers the question how Poland is performing relative to other OECD members in terms of FDI attractiveness.

Bathtub drowning mortality among older adults in Japan

We examined the bathtub drowning mortality among older adults in Japan. Mortality data from Japan and 30 other Organization for Economic Co-operation and Development (OECD) countries were extracted from World Health Organization Cause of Death Query Online. During 2012–2014, unintentional drowning mortality rates in Japan were 9.5, 28.2 and 39.7 per 100,000 population for adults aged 65–74, 75–84 and ≥85 years, respectively—rates highest among the 31 OECD countries. In total, 6377 older adults aged ≥65 years died from unintentional drowning in 2014, of which 4857 (76%) deaths involved bathtubs. During 1995–2014, the bathtub drowning mortality rate for adults aged ≥65 years was stable in Japan. During 2011–2014, approximately 4800 older adults died from bathtub drowning annually. Death predominantly occurred ‘while in a bathtub’, rather than ‘following a fall into a bathtub’. In 2014, 95% and 87% of bathtub drowning deaths among older women and men aged ≥65 years, respectively, occurred at home. In conclusion, bathtub drowning deaths at home is an important public health problem among older adults Japanese and efforts are needed to reduce these preventable deaths.
