Is obsessive-compulsive personality disorder related to stress-related exhaustion?

ObjectiveRecovery from stress‐related diagnoses can, in some cases, be long‐lasting, and several different factors could be related to such a lengthy recovery. One plausible aspect is obsessive–compulsive personality disorder (OCPD), which has previously been seen to be related to stress‐related mental health. Thus, the aim of this study was to investigate whether recovery from exhaustion disorder (ED) is associated with OCPD.MethodsThis study includes data from 147 patients (78% women, mean age 52.4 ± 9.8 years) who have been treated for ED. Clinical assessment was performed 7–10 years after first seeking care identifying patients with residual exhaustion. Symptoms of OCPD were concomitantly measured and several aspects of work‐ and private‐related stress exposure.ResultsThe main result of this study is that patients with residual clinical ED report OCPD to a greater extent, compared with patients who no longer fulfill the clinical criteria for ED, 7–10 years after seeking care. Patients with OCPD that have not recovered report “excessive devotion to work” to a higher degree than patients with OCPD that have recovered.ConclusionThe results indicate that factors related to OCPD may be of clinical importance for the patient's recovery from ED. However, prospective studies should be conducted and studies elucidating whether symptoms of exhaustion among patients with OCPD can be affected by therapeutic interventions.

Obsessive-Compulsive Personality Symptoms Predict Poorer Response to Gamma Ventral Capsulotomy for Intractable OCD.

Gamma ventral capsulotomy (GVC) is a radiosurgical procedure which aims to create lesions in the ventral part of the anterior limb of the internal capsule (ALIC). It has been used as a treatment option for patients with intractable obsessive–compulsive disorder (OCD) who do not respond to several first-line treatments attempts. However, changes in personality disorder symptoms after GVC have not been investigated. The aims of this study are to investigate changes in personality disorder symptoms after GVC and to search for baseline personality disorder symptoms that may predict clinical response to GVC. Fourteen treatment-intractable OCD patients who underwent GVC completed the Structured Clinical Interview for DSM-IV Personality Disorders (SCID-II) at baseline and one year after the procedure. Wilcoxon signed-rank test was performed to investigate personality disorder symptom changes before and after surgery. Linear regression models were utilized to predict treatment response, using baseline personality disorder symptoms as independent variables. We did not observe any quantitative changes in personality disorder symptoms after GVC, compared with baseline. Higher severity of obsessive-compulsive personality disorder symptoms at baseline was correlated with worse treatment response after GVC for OCD (β = −0.085, t-value = −2.52, p-value = 0.027). These findings advocate for the safety of the GVC procedure in this specific population of intractable OCD patients, in terms of personality disorder symptom changes. They also highlight the importance of taking into account the severity of obsessive–compulsive personality disorder symptoms when GVC is indicated for intractable OCD patients.

Feelings of incompleteness explain symptoms of OCD and OCPD beyond harm avoidance

Traditional models of Obsessive-Compulsive Disorder (OCD) suggest that neutralizing behaviors are motivated by harm avoidance. However, some individuals with OCD report an experience of discomfort, rather than anxiety per se, as motivating their behavior. Incompleteness has been proposed as another potential motivator of compulsive behaviors. We examined the relative strength of harm avoidance and incompleteness in predicting OCD symptom dimensions in a nonclinical sample (n = 238). We also examined the relationship between incompleteness and symptoms of two conditions with phenotypic similarity to OCD—Obsessive-Compulsive Personality Disorder (OCPD) and Autism Spectrum Disorder (ASD). Each involves patterns of repetitive, rigid behavior. Hierarchical regressions returned that incompleteness significantly predicted OCD and OCPD symptoms after controlling for harm avoidance; in some cases, the magnitude of association was stronger for incompleteness than for harm avoidance. We suggest that a better understanding of incompleteness may help to advance conceptualization of OCD and other symptoms.

Obsessive-compulsive personality disorder symptoms as a risk factor for postpartum depressive symptoms

For women with obsessive-compulsive personality disorder (OCPD) trait symptoms, coping with childbearing and parenting could be associated with postpartum depressive symptoms. Therefore, the possible relationship between OCPD trait symptoms and trajectories of postpartum depressive symptoms was examined. A cohort of 1427 women was followed from late pregnancy until 12 months’ postpartum. Trajectories of postpartum depressive symptoms were determined using growth mixture modeling with five repeated assessments. Next, the relationship between OCPD trait symptoms and these trajectories was examined through multinomial regression. Three postpartum depressive symptom trajectories were identified: (1) low symptoms (92%), (2) increasing-decreasing symptoms (inverted u-shape) (5%), and (3) increasing symptoms (3%). OCPD trait symptoms were associated with a higher likelihood of the trajectories increasing-decreasing symptoms (OR 1.26; 95% CI 1.14–1.39) and increasing symptoms (OR 1.16; 95% CI 1.02–1.32), compared to reference trajectory (low symptoms), adjusted for age, educational level, unplanned pregnancy, previous depressive episode (s), and parity.

Construction and Validity and reliability of Survey of Marital Pathology Woman with Symptoms of Obsessive-Compulsive Personality

Objectives: The purpose of this study was to construction and validity and reliability of a survey about marital pathology a couple of women had symptoms of obsessive-compulsive personality. Methods: The method that was used for this survey was combination of exploratory research that was conducted in two steps. The first step was a qualitative factors discovery analysis one which included 15 semi-structured interviews with women with symptoms of obsessive-compulsive personality disorder and husbands, 14 semi-structured interviews with family experts and clinical psychologists and psychiatrists and review of scientific resources. Related articles were gathered based on qualitative content analysis. In the second phase, the validity and reliability of the data collection instrument were investigated. The statistical population consisted of all married women with obsessive-compulsive symptoms referring to counseling centers in Isfahan and their husbands. Results: The research sample consisted of 70 married women with obsessive-compulsive symptoms and their husbands (140 participants) who were selected via the purposive sampling technique. Factor analysis was conducted through the SPSS software program. The research findings indicated that the questionnaire has six factors including extreme perfectionism, extreme concern, algebraic and focused (non-participatory) control, extreme sensitivity to affairs, coldness and sexual dysfunction, inflexibility. The results of factor analysis revealed a significant percentage of variance of the study variable by factors. As a result, the Cronbach’s alpha coefficient of the total test was obtained as 0.96. Conclusions: The scale enjoyed high levels of validity and reliability which contribute to obtaining factors and has the ability of identifying marital pathology of women with symptoms of obsessive-compulsive personality in clinical and couple researches.

Impact of obsessive-compulsive personality disorder symptoms in Internet users.

Internet use is pervasive in many cultures. Little is known about the impact of obsessive-compulsive personality disorder (OCPD) symptoms on impulsive and compulsive psychopathologies in people who use the Internet. Adult Internet users (N = 1,323) completed an online questionnaire quantifying OCPD symptoms, likely occurrence of select mental disorders (obsessive-compulsive disorder, attention-deficit/hyperactivity disorder [ADHD], problematic Internet use, generalized anxiety disorder), and personality questionnaires of impulsivity and compulsivity. Predictors of the presence of OCPD symptoms (endorsement of at least 4 of 8 DSM-5 criteria) were identified using binary logistic regression. In regression (P < .001, area under the curve, 0.77), OCPD symptoms were significantly associated with (in order of decreasing effect size) lower non-planning impulsivity, higher ADHD symptoms, problematic Internet use, avoidant personality disorder, female sex, generalized anxiety disorder, and some types of compulsions (checking, dressing/washing). These data suggest that OCPD symptoms, defined in terms of at least 4 of 8 DSM criteria being met, are common in Internet users. OCPD symptoms were associated with considerably higher levels of psychopathology relating to both impulsive (ADHD) and compulsive (OCD-related and problematic Internet use) disorders. These data merit replication and extension using standard in-person clinical assessments, because the current study relied on self-report over the Internet.

Behavioral Activation for a Breast Cancer Patient with Major Depression and Coexistent Personality Disorder

Behavioral activation is an empirically validated treatment for depression, with recent applications toward other psychological and medical conditions. However, modification to the treatment protocol may be necessary when symptoms, environment, or personality necessitate. This feasibility study applied Behavioral Activation Treatment for Depression (BATD) in a case of a woman with recurrent major depression and obsessive–compulsive personality disorder (OCPD) undergoing breast cancer treatment. A quasi-experimental design was used, comparing supportive psychotherapy (while she received radiation therapy) followed by eight sessions of BATD. Treatment was designed in accordance with patient and therapist goals of increasing exercise, decreasing fatigue, and reducing interference due to perfectionistic behaviors. The patient exhibited resistance to initiation of BATD, related to symptoms of OCPD, which necessitated modification of treatment strategies and scheduling. Following BATD, the patient reported a significant reduction in depressive symptoms, physical problems, and role limitations—gains not observed following supportive therapy. In addition, she demonstrated less psychological rigidity and interference due to OCPD symptoms within the therapeutic relationship and other interpersonal relationships. Consistent with previous research, results indicate BATD may effectively treat individuals with complex clinical presentations that include personality disorder pathology. In addition, methods of modification to BATD are discussed, as well as future directions for research in this area.

The Pregnancy Obsession-Compulsion-Personality Disorder Symptom Checklist

Background: Up until now, very little research has been undertaken on the possible role of personality traits, such as perfectionism and obsessive compulsive personality disorder (OCPD), on pregnancy distress. This is possibly due to the fact that no appropriate instruments are available for use during pregnancy. The current study was undertaken to develop self-rating instruments for assessing symptoms of OCPD (including perfectionism) during gestation, and to evaluate the relationship between high scores on these scales and (recurrent) depression. Method: In a large unselected sample of 1095 pregnant women, the Clinical Perfectionism Scale was adapted and modified to fit into a 15-item perfectionism scale. At the same time, ten questions taken from the SCID OCPD structural interview were modified to fit into a separate self-rating scale. The sample was randomly split into two equal sub-samples: Group I was used for reliability and Explorative Factor Analysis (EFA), and Group II for Confirmative Factor Analysis (CFA). The Edinburgh Depression Scale (EDS), completed at 12, 22 and 32 weeks’ gestation was used to assess concurrent and discriminant validity. Results: A seven-item perfectionism (Eigenvalue: 3.6, 52% explained variance) and seven-item OCPD (Eigenvalue: 3, 40% explained variance) symptom check list retained good psychometric properties: Cronbach’s alpha of 0.85 and 0.78, respectively, and good CFA model fit: a CFI of 0.96, NFI of 0.95, TLI of 0.97, and RMSEA of 0.05, with a lower limit of 0.04; and CFI of 0.97, NFI of 0.97, TLI of 0.98, and RMSEA of 0.05 with a lower limit of 0.03, respectively. Both scales correlated significantly with EDS scores at different trimesters (r: 0.32-0.43). Significantly more often, women with high scores on these scales (defined as a score of >1 SD>mean) reported single and recurrent episodes of depression during gestation and a previous history of depression earlier in life. Conclusion: Self-rating scales that assess OCPD trait symptoms are able to detect women at risk for (recurrent) depression during pregnancy.

Prevalence of depressive symptoms and obsessive–compulsive personality traits among pharmacy students

ObjectiveTo survey current pharmacy students to determine the rates of depressive and obsessive–compulsive personality symptoms. MethodsApproximately 320 students currently enrolled at a school of pharmacy were sent an online survey. Symptoms of depression were assessed using the Patient Health Questionnaire-9 (PHQ-9). Symptoms of Obsessive–Compulsive Personality Disorder (OCPD) were evaluated using questions derived from the diagnostic criteria of OCPD as found in the Diagnostic and Statistical Manual of Mental Disorders-IV-TR (DSM-IV-TR). ResultsA total of 166 students (51.8%) completed the survey. The prevalence of any type of depressive symptom (≥5 points on the PHQ-9 Scale) was 52.4% (n = 87). The majority of students (80.1%; n = 133) declared they had never been diagnosed with a mental health condition; 13.9% (n = 23) had been diagnosed with depression. Of those sampled, 30% (n = 50) of students indicated the presence of at least four out of eight symptoms of OCPD. ConclusionThe prevalence of depressive symptoms, OCPD symptoms, and other mental health disorders in a school of pharmacy appears to be relatively high. Continued recognition and awareness of these disorders is critical to the overall health and well-being of pharmacy students.

A Five-Factor Measure of Obsessive–Compulsive Personality Traits

This study provides convergent, discriminant, and incremental validity data for the Five-Factor Obsessive–Compulsive Inventory (FFOCI), a newly developed measure of traits relevant to obsessive–compulsive personality disorder (OCPD) from the perspective of the Five-factor model (FFM). Twelve scales were constructed as maladaptive variants of specific FFM facets (e.g., Perfectionism as a maladaptive variant of FFM competence). On the basis of data from 407 undergraduates (oversampled for OCPD symptoms) these 12 scales demonstrated convergent correlations with established measures of OCPD and the FFM. Further, they obtained strong discriminant validity with respect to facets from other FFM domains. Most important, the individual scales and total score of the FFOCI obtained incremental validity beyond existing measures of the FFM and OCPD for predicting a composite measure of obsessive–compulsive symptomatology. The findings support the validity of the FFOCI as a measure of obsessive–compulsive personality traits, as well as of maladaptive variants of the FFM.

Obsessive compulsive personality disorder as a predictor of exposure and ritual prevention outcome for obsessive compulsive disorder

Despite elevated rates of obsessive compulsive personality disorder (OCPD) in patients with obsessive compulsive disorder (OCD), no study has specifically examined comorbid OCPD as a predictor of exposure and ritual prevention (EX/RP) outcome. Participants were adult outpatients (n = 49) with primary OCD and a Yale-Brown Obsessive Compulsive Scale (YBOCS) total score ≥ 16 despite a therapeutic serotonin reuptake inhibitor dose for at least 12 weeks prior to entry. Participants received 17 sessions of EX/RP over 8 weeks. OCD severity was assessed with the YBOCS pre- and post-treatment by independent evaluators. At baseline, 34.7% of the OCD sample met criteria for comorbid DSM-IV OCPD, assessed by structured interview. OCPD was tested as a predictor of outcome both as a diagnostic category and as a dimensional score (severity) based on the total number of OCPD symptoms coded as present and clinically significant at baseline. Both OCPD diagnosis and greater OCPD severity predicted worse EX/RP outcome, controlling for baseline OCD severity, Axis I and II comorbidity, prior treatment, quality of life, and gender. When the individual OCPD criteria were tested separately, only perfectionism predicted worse treatment outcome, over and above the previously mentioned covariates. These findings highlight the importance of assessing OCPD and suggest a need to directly address OCPD-related traits, especially perfectionism, in the context of EX/RP to minimize their interference in outcome.

Understanding Obsessive-Compulsive Personality Disorder in Adolescence: A Dimensional Personality Perspective

The validity of the Axis II Obsessive-Compulsive Personality Disorder (OCPD) category and its position within the Cluster C personality disorder (PDs) section of the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV, APA 2000) continues to be a source of much debate. The present study examines the associations between general and maladaptive personality traits and OCPD symptoms, prior to and after controlling for co-occurring PD variance, in a general population sample of 274 Flemish adolescents and further explores the incremental validity of two different maladaptive trait measures beyond general traits. The results demonstrate that the number of (general and maladaptive) personality-OCPD associations decreases after controlling for a general personality pathology factor, with the FFM factor Conscientiousness and its maladaptive counterpart Compulsivity as remaining correlates of OCPD. The findings further suggest to complement the general NEO-PI-R (Costa and McCrae 1992) scales with more maladaptive items to enable a more comprehensive description of personality pathology variance. Implications for understanding and assessing OCPD in the developmental context of adolescence are discussed.

Obsessive-compulsive personality disorder traits and personality dimensions in parents of children with obsessive-compulsive disorder

Purpose To compare patterns of temperament and character and the prevalence of Obsessive-Compulsive Personality Disorder (OCPD) and OCPD traits in parents of children with OCD and parents of healthy controls. Methods TCI and SCID-II were administered to 63 parents of 32 children with OCD and 63 parents of age- and sex-matched controls with no psychiatric diagnosis. Interviewers were not blind to proband status. Personality dimension scores and frequencies of OCPD criteria in both groups were compared after excluding parents with a diagnosis of OCD. Relationships between TCI dimensions and OCPD symptoms in parents and the clinical characteristics of OCD children were also studied. Results Parents of OCD children presented significantly higher scores in harm avoidance and lower scores in self-directedness, cooperativeness and reward dependence than parents of healthy children. A higher incidence of OCPD was found in parents of probands ( p < 0.02). Hoarding, perfectionism and preoccupation with details were significantly more frequent in parents of OCD children. Counting, ordering and cleaning compulsions in OCD children predicted elevated odds of perfectionism and rigidity in their parents. Conclusions The existence of the dimensional personality profile associated with OCD in parents of children with OCD and the higher number of OCPD criteria in these parents in comparison to parents of healthy children highlight the importance of the role of personality factors in familial OCD.

A prospective study of obsessive‐compulsive and borderline personality traits, race and disordered eating

Most previous investigations of the relationship between personality and disordered eating have been cross-sectional and have utilised relatively homogeneous samples. The current study sought to prospectively explore two specific putative personality risk factors for disordered eating in an ethnically diverse sample of women (n = 58). During the initial and 2-year follow-up assessments, semi-structured diagnostic interviews were administered. Analyses revealed that borderline personality disorder symptoms, as well as an interaction between obsessive-compulsive personality disorder symptoms and race, significantly predicted disordered eating over a 2-year period. Obsessive-compulsive personality pathology was most strongly predictive of disordered eating for the African American women, specifically. These data supported both borderline and obsessive-compulsive personality pathology as predictors of the development of eating disorder symptoms among women over a 2-year period in college. In addition, race may be an important consideration in understanding the role of personality in the aetiology of disordered eating.

Clarifying The Convergence Between Obsessive Compulsive Personality Disorder Criteria And Obsessive Compulsive Disorder

In this study we examined the convergence between obsessive-compulsive personality disorder (OCPD) criteria and obsessive-compulsive disorder (OCD). Baseline assessments of 629 participants of the Collaborative Longitudinal Personality Disorders Study were used to examine the associations between OCPD criteria and diagnoses of OCD. Three of the eight OCPD criteria--hoarding, perfectionism, and preoccupation with details--were significantly more frequent in subjects with OCD (n = 89) than in subjects without OCD (n = 540). Logistic regressions were used to predict the probability of each OCPD criterion as a function of Axis I diagnoses (OCD, additional anxiety disorders, and major depressive disorder). Associations between OCD and these three OCPD criteria remained significant in the logistic regressions, showing unique associations with OCD and odds ratios ranging from 2.71 to 2.99. In addition, other anxiety disorders and major depressive disorder showed few associations with specific OCPD criteria. This study suggests variability in the strength of the relationships between specific OCPD criteria and OCD. The findings also support a unique relationship between OCPD symptoms and OCD, compared to other anxiety disorders or major depression. Future efforts to explore the link between Axis I and Axis II disorders may be enriched by conducting analyses at the symptom level.
