Consideration of Safety Management When Using Pepper, a Humanoid Robot for Care of Older Adults

Pepper, a humanoid robot, is 1.2 m in height and is designed to move its limbs. There are risks of the older adults experiencing falling and collision accidents when they interact with Pepper. When physical interaction happens between a humanoid robot and human beings, potential harmful physical contact might occur. The aim of this report was to examine the safety management aspects when using Pepper, a humanoid robot for the care of older adults. The older adults’ reactions to Pepper’s functions cannot be predicted. Hence, it is necessary to clarify methods to guarantee its safety in advance and to increase the safety and properties of the robots. The benefits of introducing support robots such as Pepper for aging medical and nursing care settings are obvious. Therefore, engagement in robot development while considering both the risks and benefits is critical. Our academic initiatives have just begun. Through information exchange among researchers, users, engineers, and law specialists, we need to identify latent and prominent risks in situations where Pepper and the older adults interact and deepen our examination of measures against such risks.

Self-Perceived Competence of Nurses and Care Aides Providing a Palliative Approach in Home, Hospital, and Residential Care Settings: A Cross-Sectional Survey.

A palliative approach involves adapting and integrating palliative care knowledge and expertise earlier on and across sectors of care for people who have life-limiting chronic conditions. This study explored the extent to which nurses' and care aides' self-perceived palliative care competence may explain variation in the application of a palliative approach across nursing care settings that do not specialize in palliative care. A secondary objective was to psychometrically evaluate an instrument for measuring self-perceived palliative care competence.Methods and procedures: Data were collected via a cross-sectional survey (N = 1468) of registered nurses, licensed practical nurses, and care aides at 114 randomly selected hospital-based medical units, home care offices, and residential care facilities. The questionnaire included the Palliative Care Nursing Self-Competence Scale. Multilevel logistic regression and multigroup confirmatory factor analyses were conducted. In addition to self-perceived competence, factors associated with a palliative approach include identification of patients who have life-limiting conditions and who would benefit from a palliative approach, and work environment. The psychometric analyses of the Palliative Care Nursing Self-Competence Scale confirmed a 10-dimensional structure, strong internal consistency reliability, and measurement equivalence.Discussion and conclusion: This study provides information for future development and research on interventions for integrating a palliative approach.

Conducting Clinical Research in Post-acute and Long-term Nursing Home Care Settings: Regulatory Challenges

Despite multiple initiatives in post-acute and long-term nursing home care settings (NHs) to improve the quality of care while reducing health care costs, research in NHs can prove challenging. Extensive regulation for both research and NHs is designed to protect a highly vulnerable population but can be a deterrent to conducting research. This article outlines regulatory challenges faced by NHs and researchers, such as protecting resident privacy as well as health information and obtaining informed consent. The article provides lessons learned to help form mutually beneficial partnerships between researchers and NHs to conduct studies that grow and advance NH research initiatives and clinical care.

Implications for successive employment after general nursing education programmes - a multi-perspective study

The passage of the new education law will restructure the entire nursing education system in Germany, including the broad implementation of two former pedagogical pilot programmes - a generalist nursing programme and an academic nursing programme. So far, little research has been done to determine whether these new programmes, particularly the generalist nursing programme, equip students with suitable competencies. This paper seeks to determine how well alumni from the generalist nursing pilot programme in Baden-Württemberg are actually prepared for the nursing profession compared to the expectations of potential employers, especially those in the field of geriatrics. In a research project conducted on behalf of the Baden-Württemberg Foundation, one hundred nurses, who were graduates from a generalist nursing pilot programme, were asked to fill out questionnaires. Furthermore, eleven human resources experts from within the nursing industry were interviewed. The results of both parts were compared and contrasted using a triangulated convergence model. The generalist nursing programme graduates' responses showed a preference for acute care nursing activities, and the respondents generally evaluated career paths within the field of geriatrics as less desirable. This sentiment mirrored employers' apprehensions. Alumni and experts alike agreed on the greater need for continuing education after the completion of a generalist nursing programme. For the implementation of the new nursing education bill, the question has to be clarified how the preferences of graduates of generalist nursing programme can be matched with the personnel needs of various nursing care settings. The anticipated skill and grade mix will require novel staff deployment strategies as well as tailor-made continuing education concepts.

Working in small-scale, homelike dementia care: effects on staff burnout symptoms and job characteristics. A quasi-experimental, longitudinal study.

AimThis paper reports on a quasi-experimental, longitudinal study on the effects of working in a new type of dementia care facility (i.e. small-scale living facilities) on staff burnout symptoms and job characteristics (job autonomy, social support, physical demands and workload).MethodsIt is hypothesised that nursing staff working in small-scale facilities experience fewer burnout symptoms, more autonomy and social support, and fewer symptoms of physical demands and workload compared with staff in regular wards. Two types of long-term institutional nursing care settings were included: 28 houses in small-scale living facilities and 21 regular psychogeriatric wards in nursing homes. At baseline and at follow-ups after 6 and 12 months nursing staff were assessed by means of self-report questionnaires. In total, 305 nursing staff members were included in the study, 114 working in small-scale living facilities (intervention group) and 191 in regular wards (control group).ResultsNo overall effects on burnout symptoms were detected. Significantly fewer physical demands and lower workload were experienced by staff working in small-scale living facilities compared with staff in regular wards. They also experienced more job autonomy. No significant effect was found for overall social support in the total group.ConclusionsThis study suggests positive effects of the work environment on several work characteristics. Organisational climate differs in the two conditions, which might account for our results. This may influence nursing staff well-being and has important implications for nursing home managers and policy makers. Future studies should enhance our understanding of the influence of job characteristics on outcomes.

Registered nurse experiences of nursing professional identity: a qualitative systematic review protocol.

The objective of this review is to identify, appraise and synthesize the best available evidence related to registered nurses' experiences of nursing professional identity in nursing care settings.

The meaning of empowerment within Italian nursing care settings.

To examine the current state of empowerment of nurses in Italy. Empowerment is a broad concept that is also relevant to the field of nursing practice. Its comprehension and use are important because empowerment allows nurses to take control of their own practice. A clear understanding of empowerment is necessary for nurses to take advantage of this important tool. Focus groups were conducted. A literature review was performed, and two focus groups were selected between January and February 2014. Sixteen nurses participated in the research. Empowerment is seen as being strongly connected to autonomy and is not associated with power, as the word itself suggests. Italian nurses define empowerment as the creation of conditions that help to establish the possibility for people to develop and express their value and potential. Empowerment is defined as a condition in which the individual nurse takes control of his/her own practice and thereby provides awareness and confidence to a group. Italian nurses need to feel appreciated and supported by their own organisations. If warranted, empowerment would allow them to contribute to enhance care, which is the core of the nursing profession.

Abstract P359: Achieving Normal Blood Pressure with Cloud-based Monitoring and Management

Introduction: We assessed the feasibility and impact of remote monitoring and medication management on hypertension control in a nursing home. Hypothesis: Uncontrolled hypertension is a causal factor for cardiovascular disease. We hypothesized that increased sensitivity to elevated blood pressure (BP) readings via remote monitoring and targeted medication management alerts would lead to higher rates of controlled hypertension. Methods: We followed 48 long-term nursing home patients (average age=68.4) over a 10-week period, of whom 21 were identified as uncontrolled hypertension with a reading >140/90 mmHg. BP readings were taken 3 times a week as compared to a norm of 1 measurement per 3 weeks. Blood pressure data was stored in a cloud and analyzed via algorithms. The physician was given a summary of data identifying those patients who met criteria for elevated BP with averaged per week mean systolic and diastolic blood pressure. Furthermore, patients were further stratified via an algorithm where systolic BP over 140 mmHg was reported as a fraction of the week with uncontrolled hypertension. Together these 3 variables guided the physician in making a determination of a decision to escalate pharmacologic therapy. Results: After monitoring and interventions there were significant decreases in the patients’ average BP (130.6 mmHg vs. 135.6 mmHg, p=0.027). Neurological (headache, vision changes) and cardiac symptoms (chest pain) were resolved or improved in 83% of patients (5 of 6). By end of the pilot 4.2% (2 of 48) remained hypertensive as compared to the initial 43.8% (21 of 48) found to be hypertensive at start of pilot; 95.8% (46 of 48) remained controlled by end of study. Furthermore, when compared with a similar cohort of patients, hospital admissions were 80% (2 versus 10) fewer in the study population. Conclusions: In agreement with our hypothesis, more frequent BP measurements and medication adjustments led to increased rates of hypertension control. We believe that normotension is achievable using cloud-based monitoring, reporting, and treatment. In conclusion, this new method requires serious consideration versus the current standard of care and could help provide improvements in skilled nursing facilities and other care settings.

'Close to' a palliative approach: nurses' and care aides' descriptions of caring for people with advancing chronic life-limiting conditions.

The purpose of the study was to examine nurses' and nursing assistants' perspectives of a palliative approach in a variety of nursing care settings that do not specialise in palliative care. Ageing populations worldwide are drawing increasing attention to palliative care. In particular, people with advancing chronic life-limiting conditions often have unmet needs and may die in acute medical, residential care and home health settings without access to palliative care. A palliative approach offers an upstream orientation to adopt palliative care principles to meet the needs of people with life-limiting chronic conditions, adapt palliative care knowledge to other chronic illness conditions, and integrate and contextualise this knowledge within the healthcare system (Sawatzky etal. 2016). A qualitative study using the method of interpretive description carried out by a nursing research-practice collaborative, Initiative for a Palliative Approach: Evidence and Leadership (iPANEL). Twenty-five nurses and five nursing assistants from across British Columbia, Canada participated in interviews and focus groups. Thematic analysis was used to analyse the data. The overarching theme was that of participants close to a palliative approach in that they cared for people who would benefit from a palliative approach, they were committed to providing better end-of-life care, and they understood palliative approach as an extension of specialised palliative care services. Participants varied in their self-reported capacity to integrate a palliative approach, as they were influenced by role clarity, interprofessional collaboration and knowledge. Integration of a palliative approach requires a conceptual shift and can be enhanced through interpersonal relationships and communication, role clarification and education. Nurses care for people with advancing chronic life-limiting conditions in a variety of settings who would benefit from a palliative approach.

Educational Needs on Health Examination for Nurse

본 연구의 목적은 건강검진에 관한 간호사의 교육요구를 파악하기 위함이었다. 본 연구는 양적방법과 질적 방법을 혼합한 연구방법을 사용하였다. 건강검진 관련 업무를 담당하는 간호사를 대상으로 3번의 포커스그룹면담(FGI)을 진행한 후 건강검진에 관한 교육요구를 측정하기 위한 질문지를 개발하였다. 일광역시 소재 보건소(8개 기관)에 근무 중인 간호사 84명에게 설문조사하여 자료를 수집하였다. 수집된 자료는 SPSS 22.0 프로그램으로 분석하였다. 포커스 그룹 연구결과, 간호사의 건강검진에 관한 교육요구는 5개의 범주인 ‘건강검진의 이해’, ‘성인질병의 진단’, ‘진단검사와 병태생리학’, ‘검진결과와 간호중재’, ‘영상검사와 안전’으로 나타났다. 질문지를 통한 건강검진에 관한 간호사의 교육요구는 평균 3.35±.44점(1-4)으로 높은 편이었고, 하부영역별로 방사선검사영역(3.55±.66점)에서 가장 높았고 검사항목의 이해영역(3.34±.48점)순이었다. 이상의 연구결과는 간호사가 건강검진에 관한 상담을 수행하는데 도움을 줄 수 있는 교육의 기초자료로 활용될 수 있을 것이다.The purpose of this study is to evaluate the educational needs on health examination for nurse. A mixed method was applied, including three focus group interviews which were conducted in order to understand nurse s experience of health examination. The survey data were collected from 84 nurses working in six public health centers located in B City and analyzed with SPSS 22.0. Educational needs on health screening as perceived by nurse were classified into 5 categories such as understand of health screening, diagnosis of disease in adults, medical test and pathological physiology, screening result and nursing intervention, and radioactive imaging test and safety. Educational needs for nurse were scored high (3.35±.44), with the highest score on understanding of radiology followed by understanding of medical screening test. The findings of this study provide a useful basis as to which areas of education would assist nurses with counselling on health examination in nursing care settings.

Nurses’ Perceptions of Spirituality and Spiritual Care in Different Health Care Settings in the Netherlands

This paper shows similarities and differences in perceptions and competences regarding spirituality and spiritual care of nurses in different health care settings. Research on this specific topic is limited and can contribute towards a nuanced implementation of spiritual care in different nursing care settings. Four hundred forty nine nurses in different health care settings completed a questionnaire concerning spirituality and spiritual care, spiritual care competence, and personal spirituality. Respondents reported a generic (instead of more specific) view of spirituality and spiritual care, and they perceived themselves to be competent in providing spiritual care. Compared to nurses in hospital settings, nurses in mental health care and home care have a more generic view of spirituality and spiritual care and report a higher level of competence. Next to this, they perceive themselves more as spiritual persons. Future research is needed to develop further understanding in setting specific factors and their influence on nurses’ views and competence regarding spiritual care. Nursing education and management should consider an emphasis on spiritual competence development related to working settings of nurses.

Nursing: Essential to Healthcare Value

Nursing is a practice discipline and occurs as 1 nurse and 1 patient, family, or community at a time. The encounter between a nurse and patient forms a fundamental bond that defines, not only nursing as a profession, but each individual nurse as a provider of care. Nursing practice drives value, and nurses have a direct and intimate influence on the quality, safety, and costs of patient-centered care. If we define nursing value as the function of outcomes divided by costs,1 there is a need to better define the measures and analytics for patient-level costs and outcomes of nursing care. This fundamental shift to capture the patient or consumer impact of nursing care is an important expansion of how nursing value is quantified. This will require rethinking how we view nursing care delivery beyond solely measuring nursing in terms of tasks or ratios and staffing levels, to one that recognizes the individual and collective accomplishments and results provided by each nurse across the broad spectrum of care. True nursing value can only be described by measurement of the clinical and financial impact of nursing care.

Development and Application of a Health Promotion Program for Marriage Immigrant Women

Purpose: This study was to develop a health promotion program for marriage immigrant women and to evaluate its efficacy. Methods: The health promotion program was comprised of eight 100-minute weekly sessions. Each session included understanding of health responsibility, nutrition, physical activity, interpersonal relations, stress management, and self actualization. The research was conducted under the principles of nonequivalent control group pretest-post test design. The outcome variables were health promoting behavior, health status, acculturation, self efficacy, and perceived barrier. The participants were 15 immigrant women in the experimental group and 16 in the control group. Data was analyzed using x 2 test, Fisher's exact test, Mann Whitney U-test and Wilcoxon signed rank test. Results: There was a significant improvement in health promoting behaviors (U=-3.08, p=.002), left shoulder flexibility (U=-3.02, p=.003), right shoulder flexibility (U=-3.02, p=.003), low back flexibility (U=-3.37, p=.001), social health status (U=-3.38 p=.001) and subjective health status (U=-2.17 p=.030) in the experimental group compared to the control group. Conclusion: The health promotion program for marriage immigrant women was an effective intervention for improving health promoting behavior, physical health status, social health, and subjective health status. Therefore, the developed health promotion program needs to be applied to married immigrant women in other kinds nursing care settings in future research.

More Ways to Measure the Impact of Published Studies in Cancer Nursing

In cancer care and in almost all nursing specialties, we have adopted quality indicators of our direct care. The measures of these indicators are purposefully linked to credible sources that provide a benchmark against which each reporting nursing care setting and the units within each setting can be compared with each other across defined periods of time. We argue about the validity of what is chosen as an indicator, what the definition of the indicator includes, and how it is measured. Despite the unsettled aspects that are the basis of these arguments, we do have consensus about the merits of having a comparison metric that allows us to learn more about our own care. In peer-reviewed publications, the journal impact factor is used as a quality indicator. The formula that underlies this impact metric is based on a calculation involving the average number of citations for that journal during the measurement period in relationship to the total number of papers published during that very same measurement period (for journals indexed by Thomson-Reuters). Arguments about the validity of this approach most certainly exist. A current conversation in different forms of media is a continuation of arguments against the journal impact factor and advocating for more of a focus on the impact of a single author or of a single article. This conversation includes the word altmetrics,1,2 which reflects the impact of the article and not the journal3 and serves as a real-time feedback indicator that can quickly alert a potential reader to the status of a published article being a “must read” or a “no need to read.” With the volume of published studies steadily increasing, having a filter that could help to guide our decisions regarding our available time to absorb new content would have its place. Comparing our impressions of the must-reads as it applies to our individual research programs and comparing those impressions with the more traditional steps to identify relevant literature could indeed be a form of measurement. But like our current measures, this too would be informative but insufficient. Trends at the individual author level are important and most certainly are used as indicators of quality when individuals are being considered for promotions or as an investigator for a submitted grant. Trends at the individual author level are also measureable within available literature for similar purposes, but a different kind of trend might be a more informative indicator of what readers of science are seeking. With that in mind, we have begun monitoring the number of citations per published paper in Cancer Nursing. We share these with you here as a way of honoring the authors involved and also as an indicator of what reviewers and readers seem to be favoring. We have chosen the publication year of 2011 for our initial examination of impact in terms of most frequently cited studies published in Cancer Nursing. Of the 8 most frequently cited studies, 6 were related to developing and testing interventions to diminish negative experiences for individuals (adults or children) receiving cancer treatment or to increasing positive experiences of being a cancer patient. Three of the studies included a focus on fatigue and 2 of these included a focus on physical activity and fatigue. One of the studies focused on the experiences of oncology nurses. There is quite definitely a breadth of interest represented in the most frequently cited papers, and there are emerging themes such as the interest in interventions and symptoms. We are also monitoring where these published studies are being cited; most of studies were cited in cancer specialty journals, including the medical oncology journal, with the highest journal impact factor as calculated by the Thomson- Reuters formula, in interdisciplinary, nursing, and review journals. We take note that in general, the trend is of an increasing number of citations over the 3-year span (2011–2013) of our examined citations, indicating that a look at citations must be longitudinal. We shall continue to examine trends in citations to see if the outcomes might contribute some degree of added guidance to our current review processes. Our interest remains in ensuring all readers of a high-quality review process such that the highest quality studies are moved forward into publication in Cancer Nursing. We have profound interest in learning the direct benefit of our published studies in patient care, but this interest exceeds the metrics now available to us. Development and Evaluation of Learning Program for Oncology Unit-based Core Nursing Practice - Outcomes based Cancer Patients Pain Management Learning Program -

Purpose: This study was conducted to develop a nurse-learning program for the pain management of cancer patients. Methods: In this methodological study, a learning program was developed between January and June 2011, following the methodological procedure based on the ADDIE Instructional System Design. Results: The learning program consists of 4 parts: learning goal, learning outcomes, learning method, and evaluation method. Learner-centered learning goal and learning outcomes were established, and lecturebased group learning and self-directed study were combined as the learning method. For the evaluation, we developed a written test, a nursing skill checklist, and a case report evaluation tool. After a pilot test, the learning program was assessed by an expert group for its construct validity and content-related conformance. Moreover, the effectiveness of the program was validated by the results of the learning outcomes evaluation and the achievement levels of 40 trainees who participated in the program. Conclusion: On the basis of the results of this study, we suggest that this unit-based core nursing practice learning program, comprising 202 nursing units developed by a research institute nursing department since 2010, be shared by all nurses in clinical nursing care settings to improve their practical performance and ensure necessary specialization.

Patient Safety Issues in Advanced Practice Nursing Students' Care Settings

The purpose of this project was to identify and characterize patient safety issues across advanced practice nursing (APN) care settings including ambulatory care visits. A total of 162 registered nurses enrolled in an APN education program completed an online survey. Respondents reported patient safety issues related to diagnosis or management and treatment in almost half of 489 encounters. The most common issues were clinician communication problems with patients, which occurred during 42.4% of encounters. Adoption of information technology may be a pathway for improving patient safety issues in APN practice settings.

Malnutrition in a home‐living older population: prevalence, incidence and risk factors. A prospective study

To prospectively investigate and describe the prevalence and incidence of malnutrition among home-living older people, related to demographic and medical factors, self-perceived health and health-related quality of life. Another aim was to find predictors for developing risk of malnutrition. Risk factors for malnutrition have previously been identified as diseases, several medications, low functional status, symptoms of depression and inadequate nutrient intake. Most studies are cross-sectionally performed at hospitals or in nursing care settings. A prospective study with a sample of 579 home-living older people, randomly selected from a local national register. Examinations were performed at baseline and yearly follow-ups two to four times. Questionnaires validated and tested for reliability, to detect risk of malnutrition (Mini Nutritional Assessment), symptoms of depression (Geriatric Depression Scale-20), cognitive function (Mini Mental State Examination), health-related quality of life (Nottingham Health Profile), well-being (Philadelphia Geriatric Center Multilevel Assessment Instrument) self-perceived health, demographic factors, anthropometry and biochemical examinations. Predictors were searched for through multiple logistic regression analysis with the MNA as dependent factor. The prevalence of risk for malnutrition was 14.5%, according to the MNA. Two risk factors for malnutrition were lower handgrip strength and lower self-perceived health. The incidence of risk for malnutrition at follow-ups was between 7.6% and 16.2%. Predictors for developing malnutrition were higher age, lower self-perceived health and more symptoms of depression. Men with symptoms of depression had a higher risk of developing malnutrition. Lower self-perceived health had the highest power to predict risk for malnutrition, with increased number of depression symptoms and higher age as second and third predictors. A regular and combined assessment using the Mini Nutrition Assessment, Geriatric Depression Scale-20 and self-perceived health as a base for identifying people in need, is one way to prevent the development of malnutrition.

Nursing and residential care settings can acknowledge the value of contact with companion animals to older people by making simple provisions for this in their policies and procedures.

Nutritional product use and practice in nursing and residential homes.

To review the use of nutritional products in nursing and residential homes in the Northern Health and Social Services Board (NHSSB) area of Northern Ireland. Two questionnaires on nutritional product use were sent to all (a total of 131) nursing and residential homes in the NHSSB area. A total of 122 nursing and residential homes completed and returned the questionnaires, giving a response rate of 93 per cent. Of these, 81 homes were administering nutritional products to some patients and residents. There were 3,208 residents in the homes that responded. Of these, 1,800 were residing in nursing homes and 1,390 in residential homes. Of the total of 3,190 people, 218 patients and 81 residents (299 in total) were receiving nutritional products. The findings suggest that oral supplements are the most commonly used means of nutritional support. There was a wide degree of variability between homes with regard to monitoring the use of nutritional products. In 39 per cent of cases (n = 118), the nutritional products was introduced into a person's diet because of frailty. Frailty was also mentioned in a further 15 per cent of cases (n = 45) as part of the reason for giving a nutritional product. The authors' findings suggest that guidelines should be developed to promote standardisation across nursing and residential care settings, where appropriate.
