Economic-mathematical model of obtaining commodity products of the green construction department of the Krasnoyarsk city

The article deals with the possibility of obtaining marketable products from plant raw materials obtained as a result of urban landscaping and gardening in Krasnoyarsk. The aim of this scientific research is to create an effective system of recycling environmentally friendly organic waste, and thus to reduce the negative impact on the environment and ensure environmental safety. The material of the research is organic waste from pruning of trees, bushes and lawn mowing, as well as means of production for their processing. A linear programming method was used in the work. Solving the posed optimization problem according to the chosen criteria and established systems of restrictions, we will receive either the maximum income, or the minimum costs, or the maximum productivity. It is realistic to formulate the optimization problem seasonally. Economic effect is achieved by reducing the cost of landfill disposal and obtaining a profit from the sale of commercial products made from green organic waste. The project is planned to be implemented on the basis of Krasnoyarsk municipal enterprise “Green Building Department”. The prospect of this research is to reduce the amount of waste by maximising recycling and incorporating it into the economy.

Prevalence of premature birth in conditions of global warming

The dynamics and likely associative link between global warming and the prevalence of preterm births in Ukraine over the years 2009-2018 was studied. to form modern ideas about the prognosis and prevention of this pathology. Data on medical care for pregnant women, mothers and parturients and adverse effects of pregnancy on preterm birth (form 21) for the period 2009-2018 were obtained from the municipal non-profit enterprise "Ternopil Regional Center of Public Health of Ternopil Regional Council". The correlation between the number of premature births per 100 births according to the average annual air temperature according to the Global Historical Climatology Network from the US Department of Ocean and Atmospheric Research in the climatically homogeneous regions of Ukraine was estimated. Predictive analysis of time series was performed by the method of integrated auto-regression of the moving average (ARIMA). The model error was estimated by calculating the absolute percentage error of the mean (MAPE). Statistical processing of materials was performed using programs Statistica 6.0 (StatSoft, USA) and open statistical package "R". The study meets modern requirements of moral and ethical standards regarding the provisions of legislative acts of Ukraine. Analysis of these reports of women's counseling shows an increase in the average number of preterm births from 2.88 per 100 births in the total number of births in Ukraine – 491445 in 2009 to 3.33, per 100 births in the total number of 309191 in 2018, which testifies to a significant increase in premature births in our country. Since 2009, there has been an annual, varying degree of increase in average annual air temperature in Ukraine. As a result of the correlation analysis, a significant strong direct correlation was established between the average level of premature birth and the average annual air temperature in Ukraine (r=0.84, p<0.05). Regression analysis revealed a significant increase in the number of premature births (per 100 births) in 1, 2, 5, 6, 7, 8 and 10 climatically homogeneous regions and a tendency to increase in, respectively, 3, 4, 9, 11 and 12 climatically homoge­neous regions of Ukraine. Based on the analysis of data on the average annual air temperature in Ukraine for 2009-2018, the average annual air temperature in Ukraine is projected to increase by 0.3 ° С in 3 years (MAPE <10%, p<0.05). The annual number of premature births is expected to increase (cases per 100 births) in 3 years by 0.4 cases per 100 births (MAPE <10%, p <0.05). In the context of global warming, the number of negative consequences of pregnancy is increasing, namely idiopathic premature termination of pregnancy with the birth of premature infants. Strong correlations have been established between the annual number of premature births and the average annual air temperature in Ukraine. Regression models of preterm birth showed a significant increase in 1, 2, 5, 6, 7, 8, and 10 cli­matically homogeneous regions and a tendency to increase in 3, 4, 9, 11 and 12 climatically homogeneous regions. The annual number of premature births in Ukraine is projected to increase by 2023 by 20 cases per 100 births compared to 2018. The strategy for preventing premature births and related adverse effects of pregnancy should include the identification of global warming as a risk factor for increasing level of this pathology.

CLOUD TECHNOLOGIES IN BUSINESS MANAGEMENT

Abstract. The modern world is in the phase of active implementation of digitalization of most processes, including cloud technologies, which is also associated with the introduction of quarantine measures in most countries. Over the past decade, the average annual growth of the cloud services market has expanded by about 50% to more than $ 220 billion. This figure is projected to exceed $ 500 billion in the near future. Currently, a large number of cloud service companies are concentrated in the market. All companies can be classified into national, operating within one country, and international, operating in many countries simultaneously. In Ukraine, the leader is the high-tech company De Novo, which provides services in the segment of corporate clients based on VMware, EMC, Microsoft Azure solutions. Competitors are well-known foreign IT giants, namely: Oracle, IBM, Google, Microsoft, Amazon, HP and others. The Ministry of Digital Transformation of Ukraine plays an important role in this area. The latter signed an agreement with Microsoft to implement the Azure Expansion Program and accelerate Ukraine's digital transformation, including the development of Azure cloud services for the sum of more than $ 500 million. The transition from physical, outdated IT technologies to new cloud services will allow companies to significantly reduce the cost of maintaining their own staff of IT professionals and engineers, reduce dependence on IT equipment suppliers and increase productivity and efficiency of the decisions made. The management of the enterprise can choose one of the offered models from the companies-providers: IaaS, PaaS, SaaS. The choice will depend on the immediate management needs and financial capabilities of the enterprise. In Ukraine, the biggest share of users of cloud services belongs to large utility companies and state enterprises, namely: Dija, Ministry of Internal Affairs, Kyiv Municipal Enterprise "Informatics", Prozorro, Ukrposhta, E-Health (National Health Center of Ukraine), Naftogaz and several other organizations. Thus, the introduction of cloud services involves digital business transformations, namely: optimization of business processes, acceleration of innovation, cost reduction and so on. ПРІОРИТЕТИ ВІДНОВЛЕННЯ МІСТООБСЛУГОВУЮЧИХ ПІДГАЛУЗЕЙ СФЕРИ ЖИТТЄДІЯЛЬНОСТІ НА ДОНБАСІ: СОЦІО-ЕКОЛОГО-ЕКОНОМІЧНИЙ АСПЕКТ

The article analyzes the state of the main city service subsectors of the sphere of life — heat-, water supply and sewerage systems of the cities affected by the armed conflict in Donbas. Also, the most acute problems of heat-, water supply and sewerage systems of the cities in Donbas which suffer from their ecological condition, quality of public utilities and economic conditions of the relevant municipal enterprises, and therefore need to be addressed restoration are systematized and generalized. Particular attention is paid to assessing the consequences of environmental threats associated with the operation of urban service subsectors in the cities of Donbas, affected by the conflict, as well as analysis of the financial, economic and technical condition of the largest enterprises of water supply and sewerage and heat utilities in post-conflict areas, the dynamics of annual deterioration of the main relevant indicators are estimated. This allows to substantiate the priorities of restoration and ensuring the reliable functioning of urban service subsectors in Donbas, including consideration of the possibility of giving priority to financing projects related to the diversification of urban water supply sources, renewal of production infrastructure, reconstruction of heating facilities with the installation of modern energy-efficient equipment, etc., expanding sources of funding for relevant measures, simplifying the conditions for receiving state aid by urban enterprises in Donetsk and Luhansk regions affected by the armed conflict, through the introduction of state programs to support and develop this functional type of territory. The developed proposals for taking priority measures aimed at restoring and ensuring the reliable operation of heat-, water supply and sewerage systems of cities in the post-conflict areas of Donbas will allow each of the stakeholders (public authorities and local governments, municipal enterprises of relevant urban sub-sectors) social (improving public utilities for all categories of consumers in the region), environmental (ensuring environmental safety and improving the environmental condition of both Donbas cities affected by the conflict and neighbouring cities and regions) and economic effects (increasing efficiency by reducing costs and losses of energy and resources in the production and transportation of relevant services and, therefore, growth of investment attractiveness of both city-serving subsectors and the sphere of life in general).

Economic Evaluation of the Management of Municipal Firms at the Level of Rural Local Self-Governments (Case Study)

The municipality generally uses its property to perform self-governing functions, and public or business activities. In the conditions of the Slovak Republic, the municipality operates either as a legal entity in its own name or carries out business activities with the help of a contributory and budgetary organization or business firm established by the municipality. Revenues from business activities form an important part of the revenue of local self-government budgets. The aim of this paper was an economic evaluation of the management of municipal firms at the level of rural local self-governments in the conditions of the Slovak Republic on the basis of selected economic indicators. At the same time, we analyzed the relationship between selected economic indicators in relation to the size, lifespan and number of employees of the firm. The analysis was performed in the time period 2015–2019 on a sample of municipal firms at the level of rural local self-governments. For the analysis, we used selected mathematical–statistical methods (Shapiro–Wilk test, Kruskal–Wallis test, and regression analysis). The analysis showed that the differences in the profitability of municipal firms from the point of view of the region in which they operate as well as from the point of view of the number of employees is not statistically significant. Statistical significance was not demonstrated even within the volume of revenue of municipal firms from the point of view of the region in which the municipal firm operates. The volume of revenues of municipal enterprises with the population of the municipality as well as the length of time of operation on the market is growing, but these are not the only factors on which these results depend.

Economic assessment of sources of investment in the development of municipal electric transport enterprises

The organizational and economic foundations for the formation of an investment strategy for the development of urban electric transport enterprises have been developed. The economic content of the main stages of the investment process at these enterprises has been clarified. A list of basic principles for the effective formation of the investment process has been compiled. It is proved that the enterprises of urban electric transport in Ukraine are currently unprofitable and the problems of investment support for them are extremely relevant and important. The main directions of the development of investment activity of urban electric transport enterprises have been investigated and strategic, tactical and operational methods of its activation have been proposed. At the same time, there are three main aspects of enhancing investment activity: investment, innovation and technological. The main provisions of the strategy of attracting investment sources by urban electric transport enterprises have been developed, which allows ensuring the required level of investment flexibility of the enterprise and independence in obtaining investment resources from various sources. The main stages of this process have been identified and substantiated, which include: the time of action, the formation of strategic goals, the algorithm for selecting and justifying the sources of investment resources and the practical implementation of the investment strategy. The practical aspects of rating sources of investment resources have been investigated, taking into account risk factors, time of use, investment prices, the ability of an enterprise and the state to influence the formation and use of investment sources. An integral indicator has been determined for each of the possible sources of investment resources, according to which the most acceptable for urban electric transport enterprises in terms of the cost of attracting and the likelihood of risk occurrence are own funds (32 points), as well as state budget funds (22 points) and local budgets (21 points). Whether and how managers' negotiable fate belief and knowledge search affect enterprise innovation

PurposeThe purpose of this paper is to explore the mechanism and boundary condition of the effect of managers' negotiable fate belief (NFB) on enterprise incremental and radical innovation based on social cognition theory and social network theory.Design/methodology/approachThe authors used Mplus and SPSS (Statistical Product and Service Solutions) to test the proposed model with data from 278 enterprises that have established national or municipal enterprise technology centers. In this study, questionnaires were collected through commission and field investigation.FindingsThe results indicate that managers' NFB has a significant positive impact on local and cross-border knowledge search and on the incremental and radical innovation of enterprises. Knowledge search is positively correlated with enterprise innovation and can play a mediating role between the managers' NFB and enterprise innovation. Network capability can positively moderate the relationship between managers' NFB and cross-border knowledge search but has no significant impact on the relationship between managers' NFB and local knowledge search.Originality/valueAlthough there have been many studies of managers' cognition, little is known about how managers' NFB influences enterprise behavior and enterprise innovation. This study incorporates managers' NFB and knowledge search into the research model and examines the moderating effect of network capability between managers' NFB and knowledge search from an interactive perspective. By verifying the research model, this study offers original views on whether and how managers' NFB and knowledge search affect enterprise innovation.

AN INTEGRATED APPROACH TO THE TREATMENT OF PATIENTS WITH ACUTE PANCREATITIS

Abstract. The aim of the study – to improve the results of surgical treatment of patients with acute pancreatitis.
 Matherials and methods of the study. The results of the treatment of 103 patients with acute pancreatitis who were treated in the surgical department of the Municipal non-commercial enterprise of the Kharkov regional council «Regional Clinical Hospital» 2015 to 2020 were analyzed. All patients were divided into two groups: the main (56 patients) and the comparison group (47 patients). In the main group, the tactics of the “step-up approach” were applied and the principles of the ERAS concept (2018-2020) were implemented. In the comparison group (2015-2017), the ERAS principles were not implemented and the open method was mainly used as the standard surgical treatment.
 Results of the study. Patients treated using the principles of “fast-track surgery” along with the “step-up approach” tactics, less likely to have postoperative complications and reduced the length of stay in the surgical department.
 Conclusions. This example confirms the appropriateness of applying the principles of “fast-track surgery” in everyday surgical practice in the complex treatment of patients with acute pancreatitis, as the results of surgical treatment of this category of patients are improved and financial costs for treatment are reduced.

Peculiarities of cellular, humoral links of immunity and dynamics of immunoglobulins in community-acquired pneumonia in adults and their importance for diagnostics and prognosis of the disease

The aim of our work was to study the characteristics of the immune status based on the analysis of the cellular, humoral links of immunity and dynamics of immunoglobulins in adults with community-acquired pneumonia (CAP), and their importance in the pathogenesis and prognosis of the disease.
 Materials and methods. The study was carried out in the period 2017-2020 on the basis of the therapeutic department of the Municipal non-profit enterprise “City Clinical Multidisciplinary Hospital № 25” of Kharkiv City Council. The study involved 20 adult patients with CAP aged 18 to 80 years (mean age 36.5±10.3). The control group consisted of 20 apparently healthy individuals (mean age 39.5±12.5). The study of the immune status of patients was carried out by determining the phagocytic index, the content of lymphocytes, CD3+, CD4+, CD8+, CD16+, CD20+ and the level of immunoglobulins of classes A, G, M in the dynamics of the disease: on the first and tenth days after hospitalization.
 Results. On the first day of hospitalization, there was a significant imbalance in phagocytosis, T- and B-links of immunity. On the tenth day of treatment, the phagocytic index, the number of CD3+, CD4+, CD8+, CD16+ and CD20+ increased. An increase in Ig M, a decrease in Ig A and Ig G were also noted. Thus, standard therapy for patients with CAP leads to its clinical and radiological resolution, but is not accompanied by the normalization of immunity parameters.
 Conclusions. The features of immune disorders in patients with CAP were revealed: pneumonia increases the imbalance in the cellular link of immunity, the level of CD3+ and CD4+ decreases, there is no dynamics of the relative amount of CD20+. There are no significant changes in the humoral link of the immune status. For the treatment of patients with CAP, in addition to standard pharmacotherapy, it is necessary to include drugs that stimulate the immune system

State and municipal unitary enterprises as an object of public control in the Russian Federation

This article is devoted to the analysis of state and municipal unitary enterprises as an object of public control in the Russian Federation. State and municipal enterprises exercise a number of public powers delegated by the state and municipalities. Their activities affect the interests of individuals and legal entities. The authors substantiate the need to organize and implement public control measures in relation to state and municipal unitary enterprises by developing a system of proposals for improving legislation in the field of public control.

Vacuum aspiration in the complex treatment of patients with destructive forms of erysipelas

The purpose of the study. is to give a clinical evaluation of the effectiveness of vacuum therapy in combination with autodermoplasty in the complex treatment of patients with necrotic erysipelas.
 Materials and methods of research. 114 case histories of patients who were hospitalized in the center of purulentseptic surgery in the Non profit municipal enterprise «City Hospital № 3» in Zaporizhzhia for the period 2019–2020 were analyzed retrospectively and prospectively.
 Results. Patients on admission to the clinic underwent surgical treatment, which consisted in opening a purulent lesion, necrectomy. After installation of the vacuum aspirator the general condition and the general state of health of patients considerably improved, already by the 5th day cleaning of a wound and emergence of granulation had been noted. The use of a vacuum device contributed to the dosed pressing of the skin flap to the wound surface, reliable fixation of the skin flap and to some extent – the improvement of local microcirculation.
 Conclusions. The use of vacuum therapy helps to clean the purulent wound, reduces the first phase of the wound process and potentiates the filling of the wound surface with mature granulation tissue. The use of vacuum therapy provides reliable fixation of skin grafts on the wound surface, eliminates the risk of their displacement in the near future after autodermoplasty

IMPROVEMENT OF THE METHODOLOGY FOR RATIONAL PARAMETERS DETERMINATION OF THE CAPACITIVE ENERGY STORAGE FOR THE METRO TRAIN

Purpose. The purpose of the work is to determine the rational capacity and energy consumption of the onboard capacitive energy storages using a complex approach for estimating the parameters, which is based on the methods of theoretical research. Methodology. The article performs a comparative analysis of existing methodologies and approaches for determination of the onboard capacitive storages parameters for subway trains with recovery systems, formulates the advantages and disadvantages of each of them. It is determined that among the existing methodologies it is most reasonable to use a complex approach, the aim of which is to determine the rational capacity and energy consumption according to two parameters of the energy storage system – weight and payback period. A procedure for determining the rational parameters of the onboard capacitive energy storage using theoretical research methods is proposed: theoretical basis of electric traction, mathematical modeling of dynamic motion and energy processes, mathematical statistics, technical-economic and comparative analysis. Findings. According to the results of the complex theoretical researches, the economic expediency of implementation of onboard capacitive energy storages on the subway train is confirmed. The energy storage system with rational parameters of the onboard capacitive energy storage for established operating conditions in the Municipal Enterprise «Kyiv Metro» and selected experimental train of the subway is determined. The amount of saved electricity due to the implementation of the energy storage system with rational parameters is estimated. Originality. The methodology for determining the rational parameters of the onboard capacitive energy storage in terms of replacing the use of experimental research data with theoretical research, the purpose of which is to model the movement of subway trains with recovery systems using software to reduce financial costs, research time and increase their accuracy. The research on determination of quantitative indicators of recovery electricity for the analysis of energy saving reserves was further developed in the subway. Practical value. The results of theoretical research can be the basis for the creation of full-scale samples of subway trains with onboard capacitive energy storages. The researches will create recommendations for the design of innovative energy-saving subway trains with improved technical and economic characteristics that can increase the efficiency of the transportation process in the subway.

VARIETY OF DIAGNOSED MICROORGANISMS IN PATIENTS WITH ERYSIPELAS

Purpose. The purpose is to find out the frequency of surgical forms of erysipelas among the general population of the disease.
 Materials and methods. Traditionally, there is a widespread point of view about streptococcal etiology of erysipelas. However, recently, staphylococcus or microbial associations are increasingly sown from the focus of inflammation in patients.
 The relevance of studying the course of erysipelas is determined by the tendency to relapse, a significant increase in complications of the disease in recent years and the final phenomena.
 The existing clinical situation is largely explained by changes in the spectrum of pathogens of purulent surgical infection of soft tissues. Its leading pathogens are mainly staphylococci and streptococci, as well as gram - negative aerobic bacteria.
 The nature of the clinical course of the disease, including the nature of the local inflammatory focus, depends on the totality of the pathogenic properties of microbes that form the microbial landscape.
 It was found that most often the local focus was localized on the lower extremities.
 114 case histories of patients who were hospitalized in the center of purulent-septic surgery in the Non-profit municipal enterprise “City Hospital № 3” in Zaporizhzhia for the period 2019-2020 were analyzed retrospectively and prospectively. All patients were diagnosed with erysipelas on admission, according to the forms: erythematous - 24 (21.0%), bullous - 28 (24.6%), phlegmonous form - 48 (42.1%), necrotic - 14 (12.3%). %). Disease relapses were noted in 21 patients. In terms of gender: men - 47 (41.2%), women - 67 (58.8%). The patients' age was 62.1 ± 2.6 years. Automated devices "Vitek-2" or "BaCT ALERT" (France) were used to determine the sensitivity of microorganisms in a purulent focus to antibacterial drugs. Statistical analysis was performed using the “Statgraphics Plus for Windows 7.0” software package.
 Research results and their analysis. It was found that most often the local focus was localized on the lower extremities. A total of 26 types of bacteria were identified from the wound contents of patients with destructive forms of erysipelas. No bacterial growth was found in 3 (4.8%) patients, and mixed flora was isolated in 27 (45.8%) patients. Gram-positive flora accounted for 61.0%, gram-negative flora - 35.6%, anaerobes - were not found, fungi - 3.4%.
 Conclusions. The number of destructive forms of erysipelas among the general population of the disease is more than 50%. The study of purulent-necrotic foci in patients with destructive forms of erysipelas makes it possible to determine the qualitative and quantitative composition of microflora, sensitivity to antibiotics, and make adjustments to the conduction of antibiotic therapy.

Gender features of structural and functional changes of the heart and levels of copeptin and NTproBNP in patients with acute Q-myocardial infarction in the presence of pulmonary hypertension

The aim. To evaluate the gender features of structural and functional changes in the heart and levels of copeptin and NTproBNP in patients with acute myocardial infarction (AMI) with concomitant pulmonary hypertension (PH). Materials and methods. 74 patients with AMI and concomitant PH who were treated in the intensive care unit and emergency cardiology for patients with myocardial infarction of the Municipal Non-Profit Enterprise “City Hospital of Emergency and Ambulance” of Zaporizhzhia City Council were examined. The patients were divided into two groups: the first group consisted of 42 male patients (mean age 71.06 ± 2.21 years) and 32 female patients (mean age 76.41 ± 2.32 years). All the patients were examined in the first three days by two-dimensional echocardiography on a device MyLab50 (“Esaote”, Italy). The serum levels of kopeptin and NTproBNP were determined by enzyme-linked immunosorbent assay using a set of reagents Elabscience (USA). Results. There was a trend towards a predominantly anterior AMI in the group of men and lower AMI – in the group of women. Individuals with functional class III and IV heart failure predominated (76.1 %) among the men with AMI and PH. At the same time, there were significantly more persons with III and IV functional class in the group of men than in the group of women (P < 0.05). There was a significant diastolic left ventricular posterior wall thickening in women compared to men (8.4 %; P < 0.05) and a tendency to diastolic interventricular septal thickening in females. There was an upward trend in end-systolic and end-diastolic sizes in the group of men compared to women; however, the differences did not reach the required level of significance. Evaluation of serum copeptin and NTproBNP in patients with AMI and PH revealed significantly higher levels of these indicators in the group of men compared to women (P < 0.05). Conclusions. Men with AMI and PH had more severe heart failure compared to women due to the predominance of patients with functional classes III and IV. Women with AMI and PH demonstrated predominantly concentric type of remodeling in the form of significantly thickened LV posterior wall and the tendency to increase interventricular septum wall thickness, and eccentric hypertrophy in the form of LV dilatation prevailed in men. Men with AMI and PH had significantly higher serum copeptin and NTproBNP levels compared to women.

SMART CITY IN THE CONTEXT OF BLOCKCHAIN TECHNOLOGY DEVELOPMENT

The article analyzes how the use of digital technologies through working databases improves the infrastructure of the "smart city", improving the integrated information environment of the" smart city", synchronizing information between databases. The general benefits of digital transformation, the lack of trivial application protection, generally accepted approaches to data protection, and the use of blockchain to protect data in the smart city ecosystem are considered. It is shown that with certain urban initiatives and with a tight link between information systems within a "smart city", it is possible to organize services that do not require initial user participation at all. Among the requirements for successful digital transformation, there are requirements for developing a strategy, team motivation, adapting staff to new teams, and introducing new technologies specific to the urban environment. In particular, there are increased requirements for the speed of response, that is, how quickly the smart city system can bring the necessary services to the market and how quickly it allows them to comply with the legislation. Attention is drawn to the following generally accepted approaches to data protection: data protection in the application using a strong cryptographic system with symmetric keys, data protection in the application using a strong cryptographic system with asymmetric keys, with possible participation of the customer, flourishing data exchange. The use of cryptographic systems with blockchain technologies allows you to create new strategies for privacy and speed of information exchange and management. An important component of such a system is the "key", which represents the first link in the blockchain chain and can be stored outside the system and used for various checks in the activities of divisions of the apparatus of local and municipal enterprises of the city. From a scientific point of view, the use of blockchain technologies in the future leads to the need for interdisciplinary research and their application for various applications to determine the strategies and social consequences of such application.

MANAGEMENT OF PRIMARY HEALTH CARE INSTITUTIONS IN MODERN ECONOMIC CONDITIONS

The purpose of the research is to substantiate the theoretical provisions, develop conceptual foundations and applied recommendations for the formation of the management of primary health care in modern business conditions. There were used the following methods to achieve this purpose: scientific knowledge, analysis, synthesis, abstraction and generalization - to reveal the essence of the categories "management", "health care", "primary care" analysis, detailing, classification and systematization - in determining the main indicators of the "Center for Primary Health Care №1 of Kropyvnytskyi" (Municipal enterprise "CPHC №1"); statistical and comparative analysis - when assessing the dynamics of the overall performance of the Center; tabular method - to display analytical information; graphic - for a visual representation of statistical material, reinforcement of certain theoretical and practical provisions of the study; SWOT-analysis of Municipal Uncommercial Enterprise "CPHC" №1. The scientific novelty of the study is the study of the management of the primary care institution and the development of proposals for its improvement on the example of the Center for Primary Health Care №1 of Kropyvnytskyi (MUE "CPHC №1").

DYNAMIC CHANGES OF IN INCIDENCE OF IXODES TICK BITE CASES IN ADULT POPULATION OF VINNITSA REGION, UKRAINE

Not all ticks carry dangerous pathogens, but their bites can threaten humans with diseases such as Ixodes tick-borne borreliosis (Lyme disease), Miyamoto disease, tick-borne viral encephalitis and about 150 other nosological entities. The season of life activity of Ixodes ticks begins in March and usually lasts until November and has two pronounced seasonal peaks during the year: over April – May and August – September. The relevance of the investigation is determined by the lack of the information on the incidence of tick bite cases among the population in Ukraine, and there no well elaborated sets of measures in order to prevent this problem. The purpose of this study is to provide a detailed analysis of the incidence of Ixodes tick bite cases among the adult population in the Vinnytsia region and in the city of Vinnytsia, to compare the number of detected cases for the period from 2013 to 2018, and to identify the most reliable and available prevention methods. The research was conducted at the laboratory of the municipal non-profit enterprise "Vinnytsia City Hospital G1" (KNP "VMKL"); statistical data were provided by the State Regional Sanitary and Epidemiological Station. We compared the prevalence of the disease in different districts of the Vinnytsia region and its insidence for the period from 2013 to 2018. During this period, the number of detected cases of tick bites resulted in the occurrence of various dangerous diseases went up from 53 to 317 cases among the entire adult population of the Vinnytsia region. For all years except 2018, the highest incidence rate was consistently observed in the Mogilev-Podilsk district district, even exceeding the level in more densely populated and large areas. Over the period from 2013 to 2018, the number of detected cases of tick bites in the adult population that resulted in various dangerous diseases rapidly and significantly increased. This necessitates to elaborate the set of preventive measures and to educate the population of all ages. Much attention should be paid to promoting vaccination as the only reliable way and the development of sanitary and hygienic recommendations for residents of municipal and rural areas (in the spring and summer).

Analysis finformation security risks of aic organizations

The article is devoted to the assessment of information security risks of organizations for the purpose of leveling or reducing them. Approaches to the content, regulatory regulation, methods of analysis and assessment of information risks are considered. Analytical procedures for assessing and managing information security risks for a specific municipal enterprise were performed. The results obtained can be used by municipal enterprises to ensure the security of the information system and sustainable business development.

Efficiency of capital formation of utility enterprises: the case of Ukraine

The paper aims to analyze the approaches to capital formation of Ukrainian utility enterprises, identify their impact on the financial results of entities and formulate recommendations to ensure the efficiency of utility enterprises. The methodological basis of the study is comparative analysis, theoretical studies, statistical information, analytical data prepared by governmental and non-governmental bodies, the best practices. In the course of the research were collected and processed empirical data of 30 utility enterprises from 11 regions of Ukraine with different economic, demographic, climatic and administrative conditions. The conducted research confirms that the capital invested in the municipal enterprises does not increase their efficiency. Funding from a municipal budget without disclosure of the criteria for providing such resources does not enlarge the profitability of utility enterprises and often used to cover their losses, which lead to capital disruption. In order to increase the efficiency of utility enterprises and the fair distribution of municipal budget funds it was proposed to introduce the disclosure of additional information about capital formation; to strengthen the managers’ responsibility for the quality of publishing information with the introduction of special KPIs; and to conduct financial and performance audit, including in terms of capital formation.

Особливості інтенсивної терапії вагітних та породіль з тяжким перебігом коронавірусної хвороби (COVID-19)

Актуальність. COVID-19 — це найбільший виклик, з яким зіткнувся увесь сучасний світ. Медики та науковці намагаються відшукати вирішення цієї актуальної проблеми. Саме тому останнім часом створено багато гайдлайнів для ведення пацієнтів з COVID-19, які постійно оновлюються з урахуванням результатів останніх досліджень для того, аби забезпечити пацієнтів найкращою допомогою. Але помітно, що існує нестача протоколів для певних особливих груп населення, ведення яких відрізняється від лікування загальної популяції. До таких груп належать діти, вагітні та породіллі, люди похилого віку, пацієнти, які потребують хірургічного втручання. Мета. Оцінювання допомоги вагітним та породіллям в умовах комунального некомерційного підприємства «Клінічна лікарня швидкої медичної допомоги міста Львова» (КНП «КЛШМД м. Львова»). Матеріали та методи. Із вересня 2020 року на базі КНП «КЛШМД м. Львова» проводилося лікування вагітних та породіль з COVID-19, три випадки з яких подано вашій увазі. Результати. На жаль, проведене лікування не завжди веде до позитивних наслідків і може закінчитися летально, оскільки перебіг часто супроводжується бактерійними ускладненнями, картина яких особливо яскраво проявляється на фоні імуносупресії. Проте визначено, що найкращою схемою лікування залишається поєднання застосування антибіотиків, антикоагулянтів та гормонів і виключає широке застосування противірусних або протималярійних препаратів. Визначальним чинником також залишається постійний моніторинг вітальних функцій та оцінка функціонування печінки, нирок, серця з метою запобігання розвитку ускладнень чи виявлення їх на ранній стадії. Висновки. При лікуванні вагітних у відділенні анестезіології та інтенсивної терапії важливо правильно підібрати емпіричну терапію, вчасно провести розродження та здійснювати кисневу терапію, ґрунтуючись на теоретичних і практичних принципах штучної вентиляції легень.
