Reorientation of the hospital planning in NRW-Importance for cardiovascular medicine and thoracic surgery

The year 2022 will mark the beginning of a new era for hospitals in North Rhine-Westphalia. With the reorganization of hospital planning in NRW to an allocation of treatment assignments via specialized medical service groups with personnel and infrastructural structural quality requirements specific for this purpose instead of via specialized departments and beds, a new form of hospital planning and structuring is implemented. This method is now proposed by the "government commission for a modern and needs-based hospital treatment" implemented by the Minister of Health Lauterbach as a structurization option for the whole of Germany in combination with hospital treatment levels. Therefore, it would be advisable to become acquainted with the possible effects on cardiovascular medicine in a timely manner, in order to anticipate possible alterations in the treatment assignments of one's own hospital as well as other hospitals with repercussions for cooperations with cardiac surgery.

Principles of fast-track surgery in the treatment of giant uterine fibroids: а clinical case

The article addresses the treatment of uterine fibroids, which is still relevant. According to numerous studies, about 70% of females under 50 have uterine fibroids, the most common condition among perimenopausal women. The prevalence of giant forms is not high and accounts for less than 5% of all cases, especially at early diagnosis. The surgical strategy in such cases is individual and depends on many factors: size, location of nodes, and women's reproduction plans. In these clinical cases, the management of patients with giant uterine fibroids is presented according to the multimodal principles of fast-track surgery in a modern gynecological hospital. We described the main approaches in diagnosis, preoperative preparation, surgery features, and subsequent recovery to reduce the complication rate, the duration of inpatient treatment, and postoperative rehabilitation in the treatment of giant uterine fibroids.

Food without nutritional purpose in the medical therapeutics in the modern age at Hospital de Santiago in Vitoria (Alava, Spain)

Introduction: although important, food in medieval and modern hospitals was far from the excellence and abundance suggested by some historians, probably due to an incorrect assessment of hospital documentation, considering all food expenditure to be for food when much of it was destined for the apothecary's shop. Aim/method: to identify the foodstuffs used for non-nutritional therapeutic purposes during the modern age at Hospital de Santiago in Vitoria (Alava, Spain), to describe the system of consignment, and to review the bibliography of the period in order to facilitate documentary assessment strategies for researchers. Results: between 1592 and 1813, 42 groups of foodstuffs acquired for non-nutritional therapeutic purposes were identified. The system of annotation in the expenditure books is neither systematic nor homogeneous, but highly variable and dependent on who made the entry. Twenty-seven terms were identified for the recognition that a given foodstuff was intended for the apothecary's shop and not the kitchen. Fourteen sanitary texts of the period were chosen as clarifying bibliography, finding the 17th century nursing manuals most useful for the proposed purposes. Conclusions: the variety and quantity of foodstuffs destined for the apothecary's shop shows the risk of confusion in unfamiliar researchers when analysing hospital diets from account books. A proposal of terms and strategies for discriminating the nutritional or non-nutritional use of the food acquired, together with bibliographical recommendations, is essential for an adequate assessment of historical hospital diets.

Infertility Due to Low Ovarian Reserve – Treated by Matra basti-A Case Report

ABSTRACT Reduction in quantity and quality of oocytes due to poor ovarian reserve is an important cause of infertility in many couples of reproductive age groups. Evaluating ovarian reserve and individualizing the therapeutic strategies are very important for optimizing the success rate. A 35-year-old female presented with difficulty in conception for 4 years. She was diagnosed with primary infertility due to a low anti-Mullerian hormone (AMH) level in a modern hospital. She was given assisted reproductive treatments at various hospitals and was not successful. Hence, the patient approached our outpatient department during the year 2021 for the management. A course of Matra basti (unctuous enema) was administered along with fertility awareness information without any oral medication. After the completion of treatment, the patient conceived naturally within the duration of 3 months. Low values of AMH are observed in low ovarian reserve causing infertility. Matra basti was found to be a beneficial procedure to treat infertility occurring due to low ovarian reserve in reproductive-aged women with no complications.

Emotional factors, medical interventions and mode of birth among low-risk primiparous women in Poland.

Birth is a critical event in women's lives. Since humans have evolved to give birth in the context of social support, not having it in modern settings might lead to more complications during birth. Our aim was to model how emotional factors and medical interventions related to birth outcomes in hospital settings in Poland, where c-section rates have doubled in the last decade. We analysed data from 2363 low-risk primiparous women who went into labor with the intention of giving birth vaginally. We used a model comparison approach to examine the relationship between emotional and medical variables and birth outcome (vaginal or c-section), including sociodemographic control variables in all models. A model with emotional factors better explained the data than a control model (ΔAIC = 470.8); women with continuous personal support during labor had lower odds of a c-section compared to those attended by hospital staff only (OR = 0.12, 95% CI = 0.09 - 0.16). A model that included medical interventions also better explained the data than a control model (ΔAIC = 133.6); women given epidurals, in particular, had increased odds of a c-section over those who were not (OR = 3.55, 95% CI = 2.95 - 4.27). The best model included variables for both the level of personal support and the use of epidural (ΔAIC = 598.0). Continuous personal support during childbirth may be an evolutionarily informed strategy for reducing complications, including one of the most common obstetrical complications in modern hospital settings, the c-section.

It is hard to imagine a modern hospital ward without a mobile ultrasound, bedside (point-of-care) sonography (POCUS) is a technique of the 21st century, which in emergency medicine settings (for example during primary survey) might replace the stethoscope at times. In emergency medicine - which is the hospital discipline closest to primary care - bedside ultrasound is part of the routine care by now, and its use is based on evidences of thorough research. Given that the emergency outpatient population is getting closer to primary care patients both in presentation and demography, we believe that the current POCUS evidences are probably applicable to primary care as well. Based on the clinical experience gained in emergency medicine, we assume that bedside ultrasound may also be helpful in general practice reliably diagnosing certain, potentially life-threatening pathologies, reducing the length of time until definitive treatment, increasing the success rate of certain interventions, improving patients' experience, and potentially alleviating staff burnout. The present article summarizes experiences with bedside sonography in the Anglo-Saxon world and tries to find its place in the Hungarian primary care. The widespread use of bedside ultrasound, however, in the Hungarian general practice is still lagging due to numerous barriers, the most pressing of those are training, legal responsibility and financing. Regardless, we believe, that even in this early, unregulated phase, it is worth considering to introduce bedside ultrasound into daily primary care. Competent use, however, requires continuous practice; based on the average adult patient turnover in an urban Hungarian "adult only" surgery, it might take six months to gain the necessary skills to provide high level, safe patient care. THE MANAGEMENT OF DISEASE IN A TRADITIONAL MEDICINE PERSPECTIVE: THE CASE OF NAMPULA, NORTH OF MOZAMBIQUE

Within a province (Nampula) of one of the world's poorest countries, Mozambique, the spread of traditional medicine is particularly significant. This study aims to delve into the main figures of traditional medicine as well as the way in which they manage the care pathways in Nampula City. This treatment must be framed within a very specific cultural, cosmological, and ethical context, that of the Makhuwa culture, the largest language group in Mozambique. The research was carried out through direct observation work between 2015 and 2018, with the advantage that the author is a Nampula-born researcher and a native Emakhuwa speaker. This circumstance allowed her to understand the dialogues and interviews done on site without the need for interpreters. The result was that important local cultural elements, together with the almost total absence of modern hospital facilities and conventional doctors made the city of Nampula an urban center where traditions of medical care related to traditional knowledge still prevail, and where caregivers perform social, pedagogical and medical functions. Do improved structural surroundings reduce restrictive practices in psychiatry?

Background and objectivesThere is sparse evidence that modern hospital architecture designed to prevent violence and self-harm can prevent restrictive practices (RP). We examine if the use of RPs was reduced by the structural change of relocating a 170-year-old psychiatric university hospital (UH) in Central Denmark Region (CDR) to a new modern purpose-built university hospital.MethodsThe dataset includes all admissions (N = 19.567) and RPs (N = 13.965) in the self-contained CDR one year before and after the relocation of the UH. We compare RPs at the UH a year prior to and after relocation on November 16th (November 2017, November 2019) with RPs at the other psychiatric hospitals (RH) in CDR. We applied linear regression analysis to assess the development in the monthly frequency of RPs pre- and post-relocation and examine underlying trends.ResultsAt UH, RPs performed decreased from 4073 to 2585 after relocation, whereas they remained stable (from 3676 to 3631) at RH. Mechanical restraint and involuntary acute medication were aligned at both UH and RH. Using linear regression analysis, we found an overall significant decrease in the use of all restrictive practices at UH with an inclination of -9.1 observations (95% CI − 12.0; − 6.3 p < 0.0001) per month throughout the two-year follow-up. However, the decrease did not deviate significantly from the already downward trend observed one year before relocation. Similar analyses performed for RH showed a stable use of coercion.ConclusionThe naturalistic features of the design preclude any definitive conclusion whether relocation to a new purpose-built psychiatric hospital decreased the RPs. However, we argue that improving the structural environment at the UH had a sustained effect on the already declining use of RPs, particularly mechanical restraint and involuntary acute medication.

Flexibility concept in design of advanced multi-energy carrier systems driven by biogas fuel for sustainable development

In this paper, a new and comprehensive multigeneration system is developed exploiting biogas fuel. The system is examined from the perspective of energy, exergy, exergoeconomic, exergoenvironment, and safety for supplying the energy demands of large buildings such as modern hospitals located near industrial places. Multi-objective optimization is applied along with a new concept of flexibility in the system considering these five indicators. The Brayton cycle and closed Brayton cycle for power generation, dual-loop organic Rankine cycle and ejector refrigeration for power and cooling production, multi-effect distillation unit for freshwater production, carbon dioxide absorption unit by microalgae for biodiesel production, water heaters for domestic heating production, and PEM electrolyzer for hydrogen production are integrated into the devised system. From the environmental point of view, using biogas fuel and absorption of carbon dioxide leads to the best environmental efficiency in the proposed system. Moreover, the safety facet is analyzed to lower the level of damage and risk in the system. Based on the results of the optimization, the net output power, air conditioning refrigeration, freezing, and heating are reckoned at 1864, 286.6, 75.89, and 398.9 kW, and the production of freshwater, hydrogen, biodiesel, and glycerol is predicted to be 2.111, 0.00038, 0.0088, and 0.00091 kg/s, respectively. Also, the system performance ratio, unit cost of product, exergy efficiency, environmental effect of product, and overall system risk are equal to 1.828, 40.58 $/GJ, 31.11%, 6232 mPts/GJ, and 1656 $/year.

The Role of Information in Management of Modern Hospital: Experience of Medical Reform in Shenmu, China

Objective: The objective of this review is to explore the role of information technology in the construction and management of modern hospitals, and how to use information technology to promote the development and management of hospitals.&#x0D; Methods: The relevant medical information and data at home and abroad were collected from the Internet, and the actual situation of the hospital, the diagnosis, treatment and management of outpatients were compared and analyzed to compare the role of medical information in hospital development.&#x0D; Results: After the effective implementation of medical information in the hospital, the technical level and efficiency of diagnosis, treatment and management technology in the hospital were obviously improved, so that patients could see a doctor more conveniently, and doctors and nurses could have a higher level of diagnosis and treatment.&#x0D; Conclusions: The use of medical information can significantly improve the overall management level of the hospital, make it more convenient for patients to see a doctor, and improve the construction and management of modern hospitals, which is of great significance to the improvement of the level of medical technology.

Medical waste management improvement in several private hospitals in Amanat Al-Asimah –Yemen

Handling medical waste presents several environmental challenges in most Yemeni hospitals that need immediate attention before they get out of control. This study aims to assess current practices of medical waste management in some of the major private hospitals in Amanat AlAsimah-Yemen. This study relied on several methods of data collection, visits, observation, and interviews with health cadres, cleaners, and workers in the municipality, with the use of a checklist that contains several questions and then follow-up. The results of the study, which were conducted in the four largest private hospitals in Amanat Al-Asimah, which are the Science and Technology Hospital, Azal Hospital, European Modern Hospital, Dr. Abdul Qader Al Mutawakil Hospital showed that there were no independent and specialized administrative units within the hospitals studied how to manage, follow up on the collection, transport, and disposal of medical waste and the lack of laws and regulations on medical waste management and how to deal with them. Moreover, liquid medical waste is discharged through a sewage network that is treated only from organic matter regardless of harmful chemicals that in turn affect humans, the environment, and agriculture.

Healthcare quality and safety assessment based on annual scorekeeping.

ObjectiveTo explore the practice of medical quality and safety evaluation system based on annual score under the background of establishing modern hospital management system and strengthening national public hospital performance evaluation.MethodsStatistical analysis was used to study the improvement of medical quality and safety in hospitals after the implementation of score evaluation, and the existing problems were analyzed according to the actual situation and related requirements.ResultsThe hospital's medical quality and safety evaluation system ran smoothly, the evaluation indexes could be implemented, and the evaluation results were used properly. The improvement of hospital medical quality and operation efficiency has achieved good results.ConclusionThe evaluation system of medical quality and safety for physicians and medical technicians based on annual score can achieve the whole process, all-round, personalized and information-based evaluation, and promote the high-quality development of hospitals. It is necessary to further improve the range of evaluation and carry out the evaluation of the evaluation system by relevant personnel.

Specificity of general zones in large modern European multispeciality hospitals – selected case studies

The present paper shows the results of research on general zones in large European multispeciality hospitals. A dozen or so institutions, flagship examples of large contemporary hospitals, were selected for the study. The research focused on internal zones, in particular the reception area linked to the main communication system of the hospital, which functions as a public space offering a range of services and provides space for communication, waiting and meetings. This work aims to determine design trends in the location and forms of general zones, their characteristics and proportion to other zones in hospitals, architectural features and functional-spatial solutions. The relationships between the shape of the hospital, its internal general zones, and their percentage share in the total building area were examined. Art and greenery in these zones were also investigated, along with the presence of natural lighting, the colours, and the type of finishing materials used in this type of space today. The study revealed noticeable differences between the form of general zones in large contemporary as well as 20th-century hospitals. In addition to their form, the arrangement of zones has also changed, and they often do not resemble a hospital space. Art and greenery play an increasingly important role. The general zone is a hospital’s essential communication hub, constituting the main public space where people may meet.

The Intervention of Data Mining in the Allocation Efficiency of Multiple Intelligent Devices in Intelligent Pharmacy.

With the wide application of artificial intelligence and big data technology in the medical field, the problems of high cost and low efficiency of traditional pharmacy management were becoming more and more obvious. Therefore, this paper proposed to use data mining technology to design and develop the dispensing process and equipment of intelligent pharmacy. Firstly, it summarized the existing data mining technology and association rule methods and expounded its application value in the related fields. Secondly, the data standard and integration platform of dispensing in intelligent pharmacy were established. Web service technology was used to design the interactive interface and call it to the intelligent device of pharmacy. Finally, an intelligent pharmacy management system based on association rule mining was constructed through the data mining of intelligent pharmacy equipment, in order to improve the intelligence and informatization of modern pharmacy management. For the emergency dispensing process of intelligent equipment failure, data mining was used to optimize the intelligent pharmacy equipment and dispensing process and change the pharmacy management from traditional prescription to patient drug treatment, so as to improve the dispensing efficiency of intelligent pharmacy equipment. Through the systematic test and analysis, the results showed that through the real-time risk prevention and control, the formula accuracy and operation speed of the intelligent dispensing machine were improved and the dispensing time was shortened. Through intelligent drug delivery, the unreasonable drug use of patients was reduced, the safety and effectiveness of clinical drug use were ensured, and the contradiction between doctors and patients was reduced. This study can not only optimize the medical experience of patients and provide patients with more high-quality and humanized pharmaceutical technical services but also provide some support for the intelligent management of modern hospitals.
