Problems and practicalities in student caseload holding

Nursing and Midwifery Council (NMC) standards require higher educational institutions (HEI) to introduce opportunities in which student midwives are involved in the care of a small group of women throughout their childbearing experience, enabling them to gain experience of continuity of care and carer through caseload holding ( NMC, 2009a:16 ). Bournemouth University (BU) first introduced student caseloading in 1996, and the experience has enabled us to evaluate, refine and develop this approach to learning. In this article, the problems and practicalities encountered over the years while supporting students and mentors through the everyday delivery of caseload holding will be considered. Using a case study approach, this article focuses on situational experiences in the antenatal, intrapartum and postnatal periods. The lessons learnt and resultant consequence for the development and delivery of caseload holding at BU have been important in refining our approach. It is through such reflections that we hope to assist other HEIs and midwives in practice to develop and deliver similar approaches to learning.

Roles and responsibilities of the student nurse mentor: an update

Facilitating the learning of student nurses in the workplace is an integral role of the registered nurse. This article aims to provide an overview of the role and responsibilities of the mentor in supporting pre-registration nursing students in clinical practice. The professional obligations for the mentor to meet the Nursing and Midwifery Council (NMC) standards (NMC, 2008a) will be explored, including the ongoing requirements to keep up to date in mentoring practices. Some of the challenges within the role will be identified, including the importance of recognizing and supporting the failing student. Recent changes to the preparation requirements for sign-off mentors will be discussed. The article concludes by presenting the potential benefits to both the individual and the practice placement provider.

Implementing a sign off mentor preparation workshop—A tripartite approach

The Nursing, Midwifery Council (NMC) Standards to Support Learning and Assessment in Practice (SLAiP NMC, 2008) were implemented aiming to ensure that all pre-registration nursing students are assessed by mentors in practise who are knowledgeable, up to date and who can ensure that students are safe practitioners at the time of registration. These new standards place the responsibility on the sign off mentor (SOM) for ensuring that the pre-registration students are safe in practise, and 'suitable at point of entry to the register' for their practise, (NMC, 2008). Similarly the universities are responsible for signing off the theoretical aspects of the programme and confirming the students' good health and good character (Nursing and midwifery Council, 2010, 2010). However due to the extensive time commitments of training for the sign off mentor (which involves the sign-off assessment of three final placement students in practise) this is proving difficult for Trusts to achieve (Glasper, 2010) resulting in low numbers of sign off mentors. Therefore following guidance by the Nursing and midwifery Council (2010, 2010) to allow the inclusion of scenarios and OSCEs for the first two assessments, with one final practise based assessment, it was decided to implement a development programme for our existing experienced mentors. This approach was seen to be effective in developing the knowledge and skills of our mentors to ensure they were prepared for the final practise based assessment as a sign off mentor.

NMC must sanction joint nursing and social work courses

We write with reference to your editorial in the October 2010 edition of Learning Disability Practice, on the new Nursing and Midwifery Council (NMC) Standards for Pre-Registration Nursing, in which you state, ‘...the decision to stop universities running joint nursing and social work courses will be lamented in some quarters, but the NMC wants students to spend the entire three years of study it takes to obtain a degree in nursing learning about nursing...’ We would like to share with readers some facts about the dual award programmes that are currently running in England.

Implementation and sustainability of the nursing and midwifery standards for mentoring in the UK

There is growing emphasis on developing sounder processes for ensuring that nursing and midwifery students are appropriately supported and assessed in practice settings, so that they are fit to practice at the point of registration. To support this, the Nursing and Midwifery Council (NMC) in 2006, introduced new mandatory standards for practice education (NMC, 2006). The standards outline mentor responsibility for developing and ensuring the practice competence of students and provide a more defined statement regarding accountability for the decisions that lead to entry to the professional register (NMC, 2006, p. 6). Integral to the standards is the creation of a new role final placement mentors; the 'Sign-off' mentor. The concept of the 'Sign-off' mentor is new and the arrangements for implementing a sustainable approach will be challenging for placement providers. Equally challenging for universities, is the development of a framework that can support, monitor and provide evidence, that regulatory requirements have been met. This paper outlines the complexities associated with maintaining mentor competence, discusses some of the challenges for Sign-off mentors and is intended to contribute to the general discussion about specific aspects of the Nursing and Midwifery Council (NMC) standards and their sustainability in practice.

Student midwives' experiences of hospital-based postnatal care

This mixed method study explored whether concerns raised by student midwives about the quality of their mentorship and teaching in the postnatal ward environment enabled them to meet the Nursing and Midwifery Council standards of proficiency for postnatal care, to qualify as a midwife. Some concerns were supported and it was found that effective, supportive teaching and mentorship was affected by many factors owing to the physical environment and the competing demands placed on mentors. The primary findings were that students felt less valued on the wards and better supported in the community. There was recognition that some skills could only be learnt in the hospital setting. There was no significant difference between the needs of students on long or shortened pre-registration programmes. Students will therefore continue to need teaching and mentorship in the hospital setting and trusts have an obligation to actively support this commitment.

A framework for mentor support in community-based placements

This article explores the development and provision of a framework for supporting mentors working in community settings. A working group was set up to explore the provision of mentor support in community placements in Leicester, Leicestershire and Rutland, which was restructured to meet the demands of Nursing and Midwifery Council standards and to demonstrate a commitment to supporting mentors. This was done by providing regular mentoring update sessions, a regular newsletter, visiting lecturer contact and practice-held mentor resource packs. Evaluation of the update sessions was generally positive; the main criticism was the lack of pre-course information, which has since been addressed.

The duty of nurses to meet patients' spiritual and/or religious needs

As part of giving holistic care, nurses have a duty to meet the spiritual, religious and cultural needs of patients. These aspects of care are clearly identified in the Nursing and Midwifery Council's standards for nurses. According to the latest Census, 76.8% of people claim to have a religion. It is recognized that while people may not have a religious affiliation, they may have spiritual needs. The article considers the concepts of spirituality and religions and the significance of meeting these needs from the patient's perspective. Research-based evidence demonstrates that patients' spiritual and/or religious needs are not always addressed by nurses. Barriers to giving spiritual/religious care are identified and spiritual assessment tools are considered.
