Irrigation and debridement for periprosthetic infections of the hip and factors determining outcome.

Our purpose was to identify the success rate and factors predicting outcome of irrigation and debridement (I&D) in patients with deep periprosthetic joint infection (PJI) of the hip. We retrospectively reviewed clinical characteristics of patients with deep PJI after primary/revision total hip arthroplasty (THA) between January 2000 and May 2013 treated with I&D. Implant retention was the outcome of interest. Sixty patients (29 men and 31 women; mean age 64.9 years) were identified. Mean follow-up was 59 months (range, 12-168). The implants were retained in 42 patients (70%). Failure of I&D treatment correlated with duration of symptoms >five days (p <0.001) and obesity [body mass index (BMI) ≥ 30)] (p = 0.0289). Treatment outcome was affected by the type of pathogen (p = 0.0482), with patients with methicillin-resistant staphylococci having significantly lower odds of success. I&D can be a feasible option in THA patients presenting with acute deep PJI. Duration of symptoms >five days, isolation of methicillin-resistant staphylococci and obesity should be taken into consideration in pre-operative decision making.

Isolation and Detection of Methicillin-Resistant Staphylococci in Healthy Broilers in Nsukka Southeast, Nigeria

This study was conducted to isolate and detect methicillin-resistant staphylococci (MRS) in healthy broilers in Nsukka Southeast, Nigeria and determine the antibiogram of the isolates. Cloacal and skin swabs were collected from each of 101 randomly sampled broilers meant for slaughter. The samples were processed for isolation and identification of methicillin-resistant Staphylococcus species, following standard methods. Confirmation of methicillin-resistance by the isolates was done using penicillin binding protein 2a (PBP2a) kit. Phenotypic resistance of the isolates to antimicrobial agents was determined using disc diffusion method. Out of 202 samples processed, 200 (99.01%) yielded positive growth of staphylococci on oxacillin-supplemented oxacillin-resistance staphylococcal agar base (ORSAB). A total of 200 methicillin-resistant staphylococcal isolates were obtained. Of these, 91 (45.5%) were identified as methicillin-resistant coagulase-positive Staphylococcus (MRCoPS), while 109 (54.5%) were identified as methicillin-resistant coagulase-negative Staphylococcus species (MRCoNS). Out of the 91 MRCoPS, 53 (58.2%) were identified as methicillin-resistant Staphylococcus aureus (MRSA). Resistance of the isolates was 99.5% to erythromycin and chloramphenicol, 100% to oxacillin, 76.5% to gentamicin, 96.5% to clindamycin, 92.5% to ciprofloxacin, 99% to sulphamethoxazole/trimethoprim and tetracycline, and 98.5% to streptomycin and cefoxitin. All the isolates were multidrug resistant. This study has shown that healthy broilers reared and slaughtered in Nsukka Southeast, Nigeria harbour multidrug-resistant MRS and thus serve as their reservoirs.

Molecular characterization of staphylococcal cassette chromosome mec and virulence encoding genes in methicillin-resistant staphylococci at a medical center in Lebanon

Background: Methicillin-resistant staphylococci (MRS) are major human pathogens accounting for most hospital-acquired (HA) and community acquired (CA) infections worldwide. The recent increase in MRS in a medical center in Lebanon elicited the determination of SCCmec types, genotypes, and prevalence of Panton-Valentine leucociden (PVL) and toxic shock syndrome toxin-1 (TSST-1) among the MRS isolates. Methods: Thirty-six MRS isolates collected between October 2010 and September 2011 at a medical center, Lebanon were typed using phenotypic and genotypic methods. Antimicrobial susceptibility was determined using the disk diffusion agar method. SCCmec typing was performed by multiplex PCR and sequence analysis. The prevalence of the genes encoding PVL and TSST-1 virulence factors and their transcription levels, were determined respectively by PCR and semi-quantitative real-time PCR. The genomic relatedness of the isolates was assessed by random amplified polymorphic DNA (RAPD) analysis.Results: Antimicrobial susceptibility revealed three distinct antibiotypes. The predominant SCCmec type found among the MRS isolates was type IVa (51%). Twenty-nine percent harbored SCCmec type III and 14% harbored SCCmec type II. One isolate harbored SCCmec type IVc, and another harbored SCCmec type I. All methicillin-resistant Staphylococcus aureus (MRSA) isolates were negative for the gene encoding for PVL, and two were positive for the gene encoding for TSST-1. RAPD analysis demonstrated high genomic diversity among the MRS isolates.Conclusion: This study demonstrated the SCCmec types and the clonality of the MRS strains, allowing the differentiation between HA and CA-MRS strains. CA-MRS have increased in the hospital environment and rendered highly resistant to erythromycin and clindamycin.

Determination of Vancomycin, Teicoplanin and Linezolid resistance among Staphylococcal isolates from a tertiary care hospital

Background and Objectives: Increasing reports of Vancomycin creep have been observed in both Methicillin sensitive and Methicillin resistant staphylococcal isolates. The objective of this study was to determine the sensitivity pattern among clinical isolates of Methicillin sensitive and Methicillin resistant Staphylococcus aureus (MRSA), S. epidermidis, S. haemolyticus, to Vancomycin, Teicoplanin and Linezolid. Materials and Methods: A total of 82 S. aureus, 34 S. epidermidis and 26 S. haemolyticus isolated during Jan 2011 to Dec 2011 were included in the study. None of the S. aureus isolates were resistant to Vancomycin and Linezolid. Only one MRSA isolate was resistant to Teicoplanin. Vancomycin resistant strains among Methicillin resistant S. epidermidis was one (4.3%) and among S. haemolyticus was two (8.3%) respectively. Teicoplanin resistant strains among Methicillin resistant S. epidermidis was six (26.1%) and among S. haemolyticus was two (8.3%). Linezolid resistant strains among Methicillin resistant S. epidermidis was two (8.7%) and among S. haemolyticus was one (4.2%). Conclusion: Emergence of Vancomycin, Teicoplanin and Linezolid resistance among Methicillin resistant coagulase negative staphylococci (CoNS) is alarming limiting the therapeutic options.

Production of slime by coagulase-negative staphylococci (CNS) isolated from clinical and subclinical mastitis in cows.

The aim of the study was to evaluate the slime-producing ability of coagulase-negative staphylococci (CNS) isolated from clinical and subclinical mastitis in cows. The study was carried out on 100 isolates of CNS obtained from milk of 86 cows from farms located in the Lublin region (Poland). Slime-producing ability was observed in over half of coagulase-negative staphylococci (54.0% of isolated CNS), including 19 isolates of methicillin-resistant staphylococci (95.5% of all MRCNS). Of 22 isolates of CNS responsible for the clinical form of mastitis, 20 isolates (90.9%) produced slime: S. xylosus (7 isolates), S. haemolyticus (6 isolates), S. chromogenes (4 isolates), and S. sciuri (3 isolates), including 9 isolates of MRCNS (45.0%). The remaining 34 isolates of CNS (43.6%) with the ability to produce this exopolysaccharide were isolated from the milk of cows with subclinical form of mastitis: S. xylosus (12 isolates), S. sciuri (9 isolates), S. chromogenes (6 isolates), S. haemolyticus (3 isolates), S. warneri (3 isolates) and S. saprophyticus (1 isolate), including 10 isolates of MRCNS (12.8%).

Prevalence of Methicillin-resistant Staphylococci Isolates from Horses and Horse-related Personnel in Korea

Methicillin-resistant staphylococci (MRS) are emerging as important pathogens in humans and animals worldwide. The aim of this study was to investigate the prevalence of MRS in the racehorse population and in horserelated personnel in Korea. A total of 195 horses and 18 humans (eight veterinarians, three veterinary hospital staff, and seven horse-handlers) from racehorse farms in Korea were included in the study. The samples were collected from nasal cavities using bacterial transport medium and were cultivated on tryptic soy agar with 5% sheep blood for 3 days at 37oC to confirm the presence of Staphylococcus spp. Presumptive Staphylococcus spp. isolates were identified by 16S ribosomal RNA gene analysis. The coagulase test and oxacillin susceptibility tests were performed using the tube dilution and disk diffusion methods, respectively. The presence of the mecA gene was determined using a polymerase chain reaction assay. Of the 195 horses, 29 (15.6%) yielded 29 MRS isolates. Twelve (66.7%) of the 18 horse-related personnel yielded 12 MRS isolates. All of the MRS isolates from horses or horse-related personnel were identified as methicillin-resistant coagulase-negative staphylococci (MRCNS). The result of this study suggest that the prevalence of MRS increased with the duration of antibiotic use (p = 0.002). This study also provides evidence for the zoonotic transmission of MRCNS between horses and humans, although further investigations are needed.

Efficacy of novel antibacterial compounds targeting histidine kinase YycG protein.

Treating staphylococcal biofilm-associated infections is challenging. Based on the findings that compound 2 targeting the HK domain of Staphylococcus epidermidis YycG has bactericidal and antibiofilm activities against staphylococci, six newly synthesized derivatives were evaluated for their antibacterial activities. The six derivatives of compound 2 inhibited autophosphorylation of recombinant YycG′ and the IC50 values ranged from 24.2 to 71.2 μM. The derivatives displayed bactericidal activity against planktonic S. epidermidis or Staphylococcus aureus strains in the MIC range of 1.5–3.1 μM. All the derivatives had antibiofilm activities against the 6- and 24-h biofilms of S. epidermidis. Compared to the prototype compound 2, they had less cytotoxicity for Vero cells and less hemolytic activity for human erythrocytes. The derivatives showed antibacterial activities against clinical methicillin-resistant staphylococcal isolates. A Study on Class I Integrons and Antimicrobial Resistance among Clinical Staphylococci Isolates from a Turkish Hospital

Purpose: Methicillin-resistant staphylococci (MRS) are increasingly significant in the hospital setting due to multidrug resistance. Integrons are genetic elements that can play a role in the dissemination of antimicrobial resistance. While their role is well established in gram-negative bacteria, less is known about the presence of integrons in gram-positive bacteria. Our aim was to investigate the presence of class 1 integrons in clinical MRS isolates. Methods: One hundred clinical MRS isolates were included in the study. Identification of MRS isolates was confirmed by multiplex PCR in which staphylococcal 16S rRNA, nuc and mecA genes-specific primers were used. The presence of class 1 integron was investigated by PCR with intI 1-specific primers. Results: All methicillin-resistant Staphylococcus aureus (MRSA) isolates were positive for 16S rRNA, nuc and mecA genes. All methicillin-resistant coagulase negative staphylococci (MRCoNS) were positive for 16S rRNA and mecA, and negative for nuc gene. The existence of class 1 integron was not detected in all the isolates tested. Conclusions: Class 1 integron positive clinicial MRS strains have only been shown in a few studies from East Asia. Further studies are needed in order to determine the significance of integrons in staphylococci and their role in DNA transfer between gram-positive and gram-negative bacteria.

Short communication: Antimicrobial resistance and virulence characterization of methicillin-resistant staphylococci isolates from bovine mastitis cases in Portugal

Methicillin-resistant staphylococci (MRS) have already been reported as mastitis agents. Such bacterial species are a public health concern, and the characterization of their antimicrobial resistance and virulence profile is important to better control their dissemination. The present work evaluated the distribution of methicillin-resistance among 204 staphylococci from clinical (n=50) and subclinical (n=154) bovine mastitis. The presence ofthe mecA gene was determined by PCR. Phenotypic expression of coagulase, DNase, lipase, gelatinase, hemolytic enzymes, and biofilm production was evaluated. The presence of biofilm-related genes, icaA, icaD, and bap, was also determined. Antimicrobial resistance patterns for aminoglycosides, lincosamides, macrolides, fluoroquinolones, sulphonamides, tetracyclines, and fusidic acid were determined. Nineteen (9.3%) isolates were identified as MRS, and the presence of mecA in these isolates was confirmed by PCR. Virulence factors evaluation revealed that gelatinase was the most frequently detected (94.7%), followed by hemolysins (73.7%) and lipase (68.4%); 84.2% of the MRS isolates produced biofilm and icaA and icaD were detected in almost half of the MRS isolates (52.6%), but all were bap-negative. Resistance against other antimicrobial agents ranged from 0 (fusidic acid, ciprofloxacin, norfloxacin, enrofloxacin) to 100% (nalidixic acid). Resistance to nalidixic acid and nalidixic acid-tetracycline were the most common antimicrobial resistance profiles (31.6%). This study confirms that despite the low prevalence of MRS, isolates frequently express other virulence traits, especially biofilm, that may represent a serious challenge to clinicians.

Occurrence and characterization of methicillin-resistant staphylococci from bovine mastitis milk samples in Finland

BackgroundMethicillin-resistant staphylococci (MRS) are increasingly being isolated in bovine mastitis. The aim of our study was to evaluate the occurrence of MRS in Finnish mastitis milk samples and characterize the MRS isolates using molecular methods.ResultsMethicillin-resistant S. aureus (MRSA) was a rare finding in bovine mastitis in Finland. Only two out of 135 (1.5%) S. aureus isolates were positive for mec genes. One of these carried mecA and was of spa type t172, SCCmec type IV and ST375, and the other harboured mecC, being spa type t3256, and ST130. MRSA ST375 is common among human MRSA isolates in Finland, but this is the first report in the country of bovine mecC MRSA. In coagulase-negative staphylococci (CoNS) originating from bovine mastitis, methicillin resistance was more common. In the two CoNS collections studied, 5.2% (17/324) and 1.8% (2/110) of the isolates were mecA positive. Eighteen of these were methicillin-resistant S. epidermidis (MRSE), which were divided into 6 separate PFGE clusters. One pulsotype was detected in different parts of the country, indicating clonal spread. Most MRSE (13/18) were of SCCmec type IV, one was of type V and four were non-typeable. Comparison with a human staphylococcal database indicated that bovine MRSE strains were not closely related to human MRSE isolates.ConclusionsThe occurrence of MRS, especially MRSA, in bovine mastitis in Finland was low. Most methicillin-resistant bovine CoNS are MRSE, and we found evidence of a bovine MRSE strain that may spread clonally. This is the first report of a Finnish bovine isolate of MRSAmecC ST130. The study provides a baseline for further MRS monitoring.

Identification and characterization of methicillin-resistant Staphylococcus aureus, Staphylococcus epidermidis, Staphylococcus haemolyticus and Staphylococcus pettenkoferi from a small animal clinic

The aim of this study was to isolate and characterize methicillin-resistant staphylococci (MRS) in a small animal clinic and to investigate their distribution and possible transmission. Swabs (n=72) were taken from hospitalized pets, the environment and employees of a small animal clinic and screened for the presence of MRS. The staphylococcal species was confirmed biochemically or by 16S rDNA sequencing. Susceptibility to antimicrobial agents was tested by broth dilution. The presence of mecA and other resistance genes was confirmed by PCR. Molecular typing of the isolates followed standard procedures. In total, 34 MRS belonging to the four species Staphylococcus aureus (n=5), Staphylococcus epidermidis (n=21), Staphylococcus haemolyticus (n=6) or Staphylococcus pettenkoferi (n=2) were isolated. All isolates were multidrug-resistant with resistance to at least three classes of antimicrobial agents. Among the five methicillin-resistant S. aureus (MRSA) isolates, four belonged to the clonal complex CC398; two of them were isolated from cats, the remaining two from pet cages. Overall, the MRS isolates differed in their characteristics, except for one S. epidermidis clone (n=9) isolated from hospitalized cats without clinical staphylococcal infections, pet cages, the clinic environment as well as from a healthy employee. This MRSE clone was resistant to 10 classes of antimicrobial agents, including aminocyclitols, β-lactams, fluoroquinolones, lincosamides, macrolides, phenicols, pleuromutilins, sulfonamides, tetracyclines and trimethoprim. These findings suggest a possible transmission of specific MRS isolates between animal patients, employees and the clinic environment.

Biocide and antimicrobial susceptibility of methicillin-resistant staphylococcal isolates from horses

The aim of this study was to evaluate the biocide and antimicrobial susceptibility of methicillin-resistant staphylococcal isolates from horses. Fourteen methicillin-resistant staphylococci (MRS) were subjected to an extensive genotype characterization, including SCCmec, dru, spa, PFGE and MLST typing. Antimicrobial susceptibility testing was performed and resistance genes were detected by PCR. Minimum bactericidal concentrations (MBCs) of four biocides [chlorhexidine acetate (CHA), benzalkonium chloride (BAC), triclosan (TCL) and glutaraldehyde (GLA)] were determined following the recommendations of document NF EN 1040. The presence of qac and sh-fabI genes was investigated by PCR. Several antimicrobial resistance patterns and genes were detected. When MRS strains were exposed for a longer period of time, a lower concentration of the biocide was needed to achieve lethality. TCL had the lowest MBC values. All MBC values were lower than the recommended in-use concentrations for veterinary medicine. S. haemolyticus and S. cohnii subsp. cohnii carried plasmid-borne qacA and sh-fabI or qacB and a qacH-like genes, respectively. Biocides appear to be a reliable antiseptic option against MRS, since even in the presence of bacterial efflux mechanisms, the recommended concentration is much higher than the in vitro MBC.

Novel methicillin-resistant coagulase-negative Staphylococcus clone isolated from patients with haematological diseases at the Blood Bank Centre of Amazon, Brazil

Methicillin-resistant Staphylococcus remains a severe public health problem worldwide. This research was intended to identify the presence of methicillin-resistant coagulase-negative staphylococci clones and their staphylococcal cassette chromosome mec (SCCmec)-type isolate from patients with haematologic diseases presenting bacterial infections who were treated at the Blood Bank of the state of Amazonas in Brazil. Phenotypic and genotypic tests, such as SCCmec types and multilocus sequence typing (MLST), were developed to detect and characterise methicillin-resistant isolates. A total of 26 Gram-positive bacteria were isolated, such as: Staphylococcus epidermidis (8/27), Staphylococcus intermedius (4/27) and Staphylococcus aureus (4/27). Ten methicillin-resistant staphylococcal isolates were identified. MLST revealed three different sequence types: S. aureus ST243, S. epidermidis ST2 and a new clone of S. epidermidis, ST365. These findings reinforce the potential of dissemination presented by multi-resistant Staphylococcus and they suggest the introduction of monitoring actions to reduce the spread of pathogenic clonal lineages of S. aureus and S. epidermidis to avoid hospital infections and mortality risks.

A comparative evaluation of methicillin-resistant staphylococci isolated from harness racing-horses, breeding mares and riding-horses in Italy

ObjectiveTo investigate the prevalence of methicillin-resistant staphylococci (MRS) which is a potencial risk factor of transmission between animals and humans in different types of horses (harness racing-horses, breeding mares and riding-horses) and to compare the antimicrobial resistance of the isolates. MethodsA total of 191 healthy horses, housed at different locations of the Campania Region (Italy), were included in the study. Nasal swab samples were collected from each nostril of the horses. The mecA gene was detected by a nested PCR technique. Antibiotic susceptibility was tested for each isolate. ResultsMRS was isolated from nasal samples of 68/191 (35.6%; 95%CI: 28.9%–42.9%) healthy horses. All isolates were coagulase-negative with the exception of two coagulase-positive MRS strains, identified as Staphylococcus aureus and Staphylococcus pseudintermedius, 2/83 (2.4%; 95%CI: 0.4%–9.2%). Interestingly, both coagulase-positive MRS isolates were from harness racing-horses. These horses also presented a significantly higher positivity for MRS (53.3%; 95%CI: 40.1%–66.1%) than the breeding mares and riding-horses groups. Antibiotic susceptibility testing showed difference between isolates due to different origins except for an almost common high resistance to aminopenicillins, such as ampicillin and amoxicillin. ConclusionsIt can be concluded that harness racing-horses may act as a significant reservoir of MRS as compared to breeding mares and riding-horses.

Linezolid resistant Staphylococcus haemolyticus: First case report from India

Linezolid is being increasingly used in the treatment of infections with gram-positive organisms especially methicillin resistant Staphylococcal isolates. Though resistance to this antimicrobial is emerging but it is extremely rare. Here we document first case of linezolid resistant Staphylococcus haemolyticus (S.haemolyticus) from India. This organism was isolated from pus oozing from a postsurgical site in 61 year old male hailing from an adjoining state of Haryana.

Identification of mecA genes in environmental Staphylococcal isolates

Background: In the past Hospital isolates of Staphylococcus aureus (MRSA) were considered more drug resistant than community acquired strains. However community acquired MRSA strains have now have developed drug resistance commensurate to Hospital strains. The question arise as to what environmental factors have led to this development, is it just the widespread use of antibiotics in the non-hospital environment or are there potentially other factors. Recently, increased attention has been paid to multidrug-resistant coagulase-negative Staphylococci (MRCNS) in hospitals. These strains are recognized as opportunistic pathogens in the immuno- suppressed and hospital strains are also a potential reservoir for mecA genes. This current study was to investigate antibiotic resistance in Staphylococci isolated from a non-hospital environment in common use by humans, in this case from public washrooms. Methods: Swabs taken from public washrooms were analysed. Staphylococci were identified using the Bruker MALDI-TOF biotyper system. The minimum inhibitory concentrations (MIC) of Staphylococcal isolates against 30 antibiotics, including oxacillin, erythromycin, amoxicillin, and vancomycin were determined using the MicroScan Walkway 96 plus automated system (Simens Healthcare Diagnostics, CA, USA). For methicillin resistant Staphylococcal isolates, a rapid latex agglutination assay kit was used to determine PBP2’ according the manufacturer's instructions (Oxoid, Basingtoke, UK). Genomic DNA of the isolates was prepared and mec complex was determined by polymerase chain reaction (PCR). Results: 33% of Staphylococcal isolates (both coagulase-positive and coagulase-negative) were multidrug resistant, including non-β-lactam antibiotics such as fusidic acid, gentamycin, erythromycin, chloramphenol. MICs for oxacillin varied from 0.75 to 64 mg/l. The presence of mecA was confirmed in all MRSA and MRCNS isolates. Moreover, we determined mec complexes were widely spread between isolates and represented all classes (A, B,C,C1,D) of mec complexes. Conclusion: We have demonstrated that non-healthcare establishments such as washrooms are potential reservoir for antibiotic resistant Staphylococci. Using combination of PBP2’ agglutination test and PCR methods we have shown that all methicillin resistant environmental staphylococci isolates carried mecA. Such a wide-spread presence of drug resistance determinants in these human environments is a potential source to aid the spread, sustainability and development of drug-resistance both in hospitals and community strains.

Isolation and Molecular Characterization of Methicillin-Resistant Staphylococci from Horses, Personnel and Environmental Sites at an Equine Hospital in Turkey

The present study was carried out to assess the frequency of methicillin-resistant staphylococci (MRS) among racehorses (n=209) and veterinary personnel (n=13) as well as environmental surfaces (n=14) at an equine hospital in Adana, Turkey. In addition, species distribution, antimicrobial susceptibility, resistance genes, staphylococcal chromosomal cassette mec (SCCmec) type and clonality of these isolates were also investigated. MRS were identified by 16S rRNA sequencing, and typed by pulsed-field gel electrophoresis (PFGE). As a result, MRS was isolated in horses (48.3%), clinic staff (92.3%) and environmental samples (71.4%). Of the 123 MRS isolates, 118 isolates were identified as Staphylococcus lentus, and the remaining ones were found to be S. sciuri (n=3), S. intermedius (n=1) and S. fleuretti (n=1). All isolates were found to be susceptible against vancomycin, quinupristin-dalfopristin and rifampicin. Additionally, single or various combinations of resistance genes were detected among MRS isolates. SCCmec type II was identified in all isolates. Similar PFGE patterns were observed among MRS isolated from horses, humans, and environmental samples. Since MRS were concurrently isolated from horses and humans it is suggested that cross-transmission of MRS between horses and humans might occur. However, it cannot be ruled out that transmission is human to animal or animal to human.

Screening for methicillin-resistant staphylococci in dogs admitted to a veterinary teaching hospital

This study investigated the nasal carriage of methicillin-resistant staphylococci (MRS) in dogs (n=177) prior to medical examination or surgery in a veterinary teaching hospital. Nasal swab samples were collected after induction of anaesthesia and incubated overnight in salt enriched trypticase-soy broth. Cultures were analysed on two different agar media containing cefoxitin. Suspected MRS isolates were genotypically identified and characterised by antimicrobial susceptibility testing and staphylococcal cassette chromosome mec (SCCmec)-typing. Methicillin-resistant Staphylococcus aureus (MRSA) isolates were additionally characterised by spa-typing and multilocus sequence typing. The presence of Panton-Valentine leukocidin (pvl) genes was determined by PCR. MRS carriage was compared between animals with or without an infectious process. Two MRSA were isolated, both belonging to typical Belgian human hospital clones and lacking pvl. Additionally a methicillin-resistant Staphylococcus haemolyticus carrying a type V SCCmec was detected. No relationship was observed between MRS carriage and presence of infections.The results suggest that MRS are present in dogs originating from the community, albeit at a low prevalence. This could pose risks for cross contamination of dogs and their owners.

Class 1 integron in staphylococci

As a major concern in public health, methicillin-resistant staphylococci (MRS) still remains one of the most prevalent pathogens that cause nosocomial infections throughout the world and has been recently labeled as a "super bug" in antibiotic resistance. Thus, surveillance and investigation on antibiotic resistance mechanisms involved in clinical MRS strains may raise urgent necessity and utmost significance. As a novel antibiotic resistance mechanism, class 1 integron has been identified as a primary source of antimicrobial resistance genes in Gram-negative organisms. However, most available studies on integrons had been limited within Gram-negative microbes, little is known for clinical Gram-positive bacteria. Based on series studies of systematic integrons investigation in hundreds of staphylococci strains during 2001-2006, this review concentrated on the latest development of class 1 integron in MRS isolates, including summary of prevalence and occurrence of class 1 integron, analysis of correlation between integron and antibiotic resistance, further demonstration of the role integrons play as antibiotic determinants, as well as origin and evolution of integron-associated gene cassettes during this study period.

Prevalence and antimicrobial resistance patterns of methicillin-resistant staphylococci (MRS) isolated in a Veterinary Teaching Hospital in Brazil

We investigated the prevalence of methicillin-resistant staphylococci (MRS) in humans and dogs and evaluated the antimicrobial resistance patterns of these bacteria at a Veterinary Teaching Hospital. Specimens from 50 human subjects and 50 dogs were studied. Isolates were identified by Gram-staining, biochemical reactivity and resistance to antimicrobials. While no isolates of methicillin-resistantStaphylococcus aureus (MRSA) or methicillin-resistant Staphylococcus intermedius(MRSI) were isolated, two (4%) methicillin-resistant coagulase-negative staphylococci (MRCoNS) were isolated from dogs and 18 (36%) were isolated from humans. The percentage of MRCoNS isolates resistant to penicillin (100%), ciprofloxacin (30%), gentamicin (40%), clindamycin (25%), erythromycin (70%), trimethoprim-sulfamethoxazole (20%) or vancomycin (0%) was evaluated. The absence of MRS isolates resistant to vancomycin is of interest because this antimicrobial may be used as an important therapeutic alternative in cases of MRSA infections. Surveillance programs aimed against MRS should therefore be stimulated in veterinary health units. Key words: Dogs, MRS, nosocomial, veterinary staff.
