Genetic study of Welsh gypsies.

A South Wales gypsy kindred of Romany origin had a high incidence of phenylketonuria along with other recessively inherited disorders. There was a high degree of consanguinity (F = 0-017) with an excess of non-specific mental subnormality among known consanguineous matings. Phenylketonuria and a number of other recessively inherited disorders have been recorded from other Romany gypsy populations, but it is uncertain whether this results from a generally high gene frequency for the disorders or from consanguinity and other more local factors.

Estimate of mental ill health in children of an urban community.

This study was conducted in an urban locality to determine the psychiatric morbidity in children up to 12 years of age. All 109 families having children up to 12 years of age were studied. The results revealed a period prevalence of psychiatric morbidity in these children of 356 per 1000. Further more, mental subnormality, either alone or in association with neurotic behaviour or organic brain syndrome, formed the largest diagnostic category. A large number of neurotic and allied disorders was also observed but there was complete absence of functional psychosis. Out of those who were mentally retarded a majority had only borderline retardation and a majority of healthy children possessed an I.Q. between 91–120. A comparison of socio-demographic variables of the total and the sick population revealed that children above 6 years of age were more frequently sick (p<0.01). Analysis was made of the physical and emotional stresses during pregnancy, psychiatric disorders in the children having delayed and normal milestones, and neurotic traits up to 5 years of age along with malnutrition in association with psychiatric disorders in these children.

A predischarge unit in a long-stay hospital for subnormality.

A newly built residential ward in the grounds of a long-stay mental subnormality hospital was converted into a predischarge unit to cater for those patients who could probably be discharged after some special training. So far 31 residents have been trained over the past two years; 18 (58%) have been discharged to community residences, and five were waiting for discharge. The high success rate shows the value and usefulness of a predischage unit in a long-stay hospital, and also shows that a purpose-built building is not necessary so long as the staff have the initiative and will to succeed.

The mentally backward child.

From time to time various terms have been used to denote intellectual backwardness in a child. These include amentia, mental deficiency, mental subnormality, mental retardation, idiocy, oligophrenia, feeble-mindedness and more recently mental handicap. All these terms have their limitations and negative connotations. Moreover, all these terms used to describe the arrested or retarded mental development refer to the backwardness of the child in relation to other children of his own age group and thus they are relative terms. Such backwardness is reflected in the rate of maturation of the mental faculties, learning capacities, emotional adjustment, socioenvironmental adaptation and all other factors which contribute to proper intellectual and socio-psychological functioning of the child. At the present moment the classification of diseases by the World Health Organization and classification of mental illnesses by the American Psychiatrist Association designating the degree of severity by neutral terms llke borderline, mild, moderate, severe and profound is the most acceptable one. Such classification is based on the Intelligence Quotient (I.Q.) level as assessed by various psychometric methods, though there are several limitations of the currently employed I.Q. tests. White cell function in Down's syndrome.

Neutrophil function was studied in 25 patients with Down's syndrome at a mental subnormality hospital and compared with 26 normal controls. In vitro killing of Candida albicans was significantly lower in the Down's group, but there was no difference in the percentage of cells actively involved in phagocytosis or in the phagocytic index. The spontaneous nitroblue tetrazolium reduction was increased in 10 patients, but no abnormality of peroxidase activity or leucocyte alkaline phosphatase activity was found.
