This commentary provides a focused analysis of the 2024 ADA guidelines for the laboratory assessment of diabetes.

The 2024 edition closely follows the ADA/EASD Consensus Statement. To evaluate various complications, BNP, NT-proBNP, ankle-brachial index (ABI), and the FIB-4 index are measured. Prescription pattern analysis of Type 2 Diabetes Mellitus: a cross-sectional study in Isfahan, Iran

BackgroundPatients with Type 2 Diabetes Mellitus (T2DM) are at a higher risk of polypharmacy and more susceptible to irrational prescriptions; therefore, pharmacological therapy patterns are important to be monitored. The primary objective of this study was to highlight current prescription patterns in T2DM patients and compare them with existing Standards of Medical Care in Diabetes. The second objective was to analyze whether age and gender affect prescription patterns.MethodThis cross-sectional study was conducted using the Iran Health Insurance Organization (IHIO) prescription database. It was mined by an Association Rule Mining (ARM) technique, FP-Growth, in order to find co-prescribed drugs with anti-diabetic medications. The algorithm was implemented at different levels of the Anatomical Therapeutic Chemical (ATC) classification system, which assigns different codes to drugs based on their anatomy, pharmacological, therapeutic, and chemical properties to provide an in-depth analysis of co-prescription patterns.ResultsAltogether, the prescriptions of 914,652 patients were analyzed, of whom 91,505 were found to have diabetes. According to our results, prescribing Lipid Modifying Agents (C10) (56.3%), Agents Acting on The Renin-Angiotensin System (C09) (48.9%), Antithrombotic Agents (B01) (35.7%), and Beta Blocking Agents (C07) (30.1%) were meaningfully associated with the prescription of Drugs Used in Diabetes. Our study also revealed that female diabetic patients have a higher lift for taking Thyroid Preparations, and the older the patients were, the more they were prone to take neuropathy-related medications. Additionally, the results suggest that there are gender differences in the association between aspirin and diabetes drugs, with the differences becoming less pronounced in old age.ConclusionsAlmost all of the association rules found in this research were clinically meaningful, proving the potential of ARM for co-prescription pattern discovery. Moreover, implementing level-based ARM was effective in detecting difficult-to-spot rules. Additionally, the majority of drugs prescribed by physicians were consistent with the Standards of Medical Care in Diabetes.

Mustafa: None. S. Aamdani: None. A. Haider: None. S. Nadeem: None. Background: Pakistan has the highest national prevalence (1 in 4 adults) of type 2 diabetes mellitus (T2DM) in the world. Most T2DM clinical practice guidelines (CPGs) originate from high-income countries and are evidence based. Due to the lack of research infrastructure and financial resources, lower-middle income countries face challenges in the de novo creation of CPGs. Thus, we aimed to develop a comprehensive, high-quality CPG with a local context for adult outpatient T2DM management in Pakistan based on the GRADE-ADOLOPMENT process. Methods: In collaboration with the GRADE working group, an adaptation of the GRADE-ADOLOPMENT approach was followed. Adolopment is a combination of adoption, adaptation, and de novo development. The American Diabetes Association (ADA) Standards of Medical Care in Diabetes - 2021 was utilized as the source CPG by local endocrinology experts in Pakistan. Recommendations from the source CPG were either adopted as is, excluded, or adapted. Adaptation was done using the evidence-to-decision framework with a panel of experts. Results: The source CPG contained 243 recommendations, 219 of which were adopted without change. 5 recommendations were adopted with minor changes related to cost effectiveness to minimize cost-related nonadherence. 18 recommendations were excluded due to reasons including but not limited to the scope being beyond the initial aim of our guideline centers’ objective (such as recommendations related to pediatric or inpatient care), unavailability of medication, or inapplicability to the existing health-care system of Pakistan. One recommendation was adapted pertaining to the screening age for the diagnosis of T2DM. This was recommended to be lowered from the age of 45 years to 30 years due to the higher prevalence of T2DM in younger Pakistanis. This will enable the early detection and management of T2DM. Earlier screening highlights the opportunity to intervene early on and reduce complications. This, coincidentally, was also addressed by the ADA in their 2022 guideline, where the screening age was revised from 45 years to 35 years. Conclusion: Our GRADE-ADOLOPED CPG for T2DM management comprises of 225 recommendations. We recommend earlier screening for T2DM in Pakistan. We additionally suggest careful consideration of cost to promote adherence while also recommending CGM use in the management plan of people with T2DM who can afford it. Our modified methodology provides an excellent resource efficient method to create locally contextualized evidence-based CPGs. Our goal is to publish and disseminate this CPG to improve the quality of T2DM care in Pakistan. Evaluation of Financial Conflicts of Interest and Quality of Evidence Underlying the American Diabetes Association Clinical Practice Guidelines: The Standards of Medical Care in Diabetes, 2021.

Clinical practice guidelines make recommendations based on the best available evidence. Proper management and disclosure of financial conflicts of interest (FCOIs) are necessary for trustworthy clinical practice guidelines. This study evaluated the prevalence of FCOIs and quality of evidence underlying the American Diabetes Association (ADA)guidelines. Using the Open Payments Database (OPD) between 2018 and 2020, we examined the research and general payments to all authors of the Standards of Medical Care in Diabetes, 2021. The quality of evidence and tone of recommendations were assessed and the associations between the two were evaluated by logistic regression analysis. Of the 25 guideline authors, 15 (60.0%) were United States (US)-based physicians eligible for the OPD search. Eight (32.0%) and 12 (48.0%) received one or more industry payments one year and three years prior to the guideline publication, respectively. The median total payments (interquartile range) per author were $33,262 ($4,638‒$101,271) in 2020 and $18,053 ($2,529‒$220,659) in 2018-2020. One author received a research payment of over $10,000 undeclared. Of 471 recommendations, 61 (13.0%) and 97 (20.6%) were supported by low-quality evidence and expert opinions, respectively. Also, 439 (93.2%) recommendations had a positive tone. The lower quality of evidence tended to recommend positively with an odds ratio of 1.56 (95% confidence interval: 0.96-2.56, p=0.075) without reaching statistical significance. A minority of the guideline authors received industry payments from the healthcare industry, and declared FCOIs were mostly accurate. However, the ADA FCOI policy required the guideline authors to declare their FCOIs for one year before publication. A more transparent and rigorous FCOI policy is needed in the ADA guidelines.

Paradigm sift of the medical care in diabetes

Clinical trials have shown that it is difficult to prevent diabetic complications, especially cardiovascular disease (CVD), when classic anti-diabetic drugs are used to control hemoglobin A1c (HbA1c) levels. Trials have also shown that while new anti-diabetic drugs are effective in significantly lowering HbA1c levels, they vary in their ability to prevent CVD complications, with dipeptidyl peptidase-4 (DPP-4) inhibitors failing to prevent CVD and the progression of renal dysfunction, while glucagon-like peptide-1 receptor agonists and sodium glucose cotransporter 2 (SGLT2) inhibitors succeed. However, SGLT2 inhibitors can cause muscle atrophy and dehydration, especially in older patients. In selecting antidiabetics, therefore, physicians should keep such potential effects in mind.

Adolopment of adult diabetes mellitus management guidelines for a Pakistani context: Methodology and challenges.

Pakistan has the highest national prevalence of type 2 diabetes mellitus (T2DM) in the world. Most high-quality T2DM clinical practice guidelines (CPGs) used internationally originate from high-income countries in the West. Local T2DM CPGs in Pakistan are not backed by transparent methodologies. We aimed to produce comprehensive, high-quality CPGs for the management of adult DM in Pakistan. We employed the GRADE-ADOLOPMENT approach utilizing the T2DM CPG of the American Diabetes Association (ADA) Standards of Medical Care in Diabetes - 2021 as the source CPG. Recommendations from the source guideline were either adopted as is, excluded, or adapted according to our local context. The source document contained 243 recommendations, 219 of which were adopted without change, 5 with minor changes, and 18 of which were excluded in the newly created Pakistani guidelines. One recommendation was adapted: the recommended age to begin screening all individuals for T2DM/pre-diabetes was lowered from 45 to 30 years, due to the higher prevalence of T2DM in younger Pakistanis. Exclusion of recommendations were primarily due to differences in the healthcare systems of Pakistan and the US, or the unavailability of certain drugs in Pakistan. A CPG for the management of T2DM in Pakistan was created. Our newly developed guideline recommends earlier screening for T2DM in Pakistan, primarily due to the higher prevalence of T2DM amongst younger individuals in Pakistan. Moreover, the systematic methodology used is a significant improvement on pre-existing T2DM CPGs in Pakistan. Once these evidence based CGPs are officially published, their nationwide uptake should be top priority. Our findings also highlight the need for rigorous expanded research exploring the effectiveness of earlier screening for T2DM in Pakistan.

Diabetes Management in Chronic Kidney Disease: A Consensus Report by the American Diabetes Association (ADA) and Kidney Disease: Improving Global Outcomes (KDIGO).

People with diabetes and chronic kidney disease (CKD) are at high risk for kidney failure, atherosclerotic cardiovascular disease, heart failure, and premature mortality. Recent clinical trials support new approaches to treat diabetes and CKD. The 2022 American Diabetes Association (ADA) Standards of Medical Care in Diabetes and the Kidney Disease: Improving Global Outcomes (KDIGO) 2022 Clinical Practice Guideline for Diabetes Management in Chronic Kidney Disease each provide evidence-based recommendations for management. A joint group of ADA and KDIGO representatives reviewed and developed a series of consensus statements to guide clinical care from the ADA and KDIGO guidelines. The published guidelines are aligned in the areas of CKD screening and diagnosis, glycemia monitoring, lifestyle therapies, treatment goals, and pharmacologic management. Recommendations include comprehensive care in which pharmacotherapy that is proven to improve kidney and cardiovascular outcomes is layered on a foundation of healthy lifestyle. Consensus statements provide specific guidance on use of renin-angiotensin system inhibitors, metformin, sodium-glucose cotransporter 2 inhibitors, glucagon-like peptide 1 receptor agonists, and a nonsteroidal mineralocorticoid receptor antagonist. These areas of consensus provide clear direction for implementation of care to improve clinical outcomes of people with diabetes and CKD.

Impact of Overbasalization on Clinical Outcomes in Patients With Type 2 Diabetes: A Post Hoc Analysis of a Large Randomized Controlled Trial.

The American Diabetes Association's Standards of Medical Care in Diabetes emphasize the need for awareness regarding overbasalization (basal insulin doses >0.5 units/kg/day without bolus insulin) in the treatment of type 2 diabetes. However, outcomes data on the impact of overbasalization are limited. This post hoc analysis of a large randomized controlled trial suggests that an insulin therapy regimen involving overbasalization compared with a basal-bolus insulin regimen that avoids overbasalization is less effective at lowering A1C and may be associated with increased cardiovascular risk. Clinicians should consider alternative approaches to glycemic control before increasing basal insulin doses to >0.5 units/kg/day.

The prevalence of diabetes is growing in the United States at an alarming rate. Early and intensive diagnosis and management of diabetes can reduce the economic burden and improve the societal burden of long-term diabetes-related complications. Healthcare providers practicing in the primary care setting are on the front line of screening, diagnosis, and managing a large majority of persons with diabetes.Until recently, blood glucose monitoring, along with timely A1C measurements, has been the recommended means by which patients can best achieve their prescribed metabolic targets. However, supplies and testing can be costly and burdensome, affecting patient compliance and medication adherence. The recent introduction of integrated diabetes technology including continuous glucose monitoring (CGM) has had a tremendous impact on treating patients to their prescribed glycemic targets safely and efficiently while minimizing their risk of developing treatment-emergent hypoglycemia. Patients who utilize CGM are able to reduce their risk of hospitalizations, minimize work absenteeism, lower their A1C, lower their risk of hypoglycemia, as well as long-term microvascular and macrovascular complications.The American Diabetes Association updated its evidence-based Standards of Medical Care in Diabetes in 2022 around the use of CGM, as has the American Association of Clinical Endocrinology. Because these devices can have a positive effect on the management of persons with diabetes, managed care and healthcare providers should allow technological integration for theirpatients.

Health care professionals with an interest in diabetes anxiously await this publication to learn of updates and changes in the management and care for people with diabetes. This year, the updates and changes are minimal for most sections of the Standards. However, several changes are significant; specifically, the updates to the pharmacologic approaches to glycemic treatment section. No longer is metformin the sole option for first-line therapy.

Association of Impaired Fasting Glucose and Diabetes with SARS-CoV-2 Spike Antibody Titers after the BNT162b2 Vaccine among Health Care Workers in a Tertiary Hospital in Japan.

Background: Hyperglycemia can alter the activation of innate and acquired immunity, but epidemiological evidence linking hyperglycemia to post-vaccination immunogenicity is limited. Objective: To examine the association between SARS-CoV-2 spike antibody titers after the COVID-19 vaccine and impaired fasting glucose (IFG) and diabetes. Methods: Participants were 953 health care workers aged 21–75 years who were tested for SARS-CoV-2 spike IgG antibodies and underwent a health checkup two months after their second dose of the BNT162b2 vaccine. IFG was defined as a fasting plasma glucose (FPG) level of 100–125 mg/dL, and diabetes was defined as an FPG level ≥ 126 mg/dL or being under medical care for diabetes. Multivariable linear regression was used to calculate the ratio of the mean. Result: Spike IgG antibody titers were lower in the presence of hyperglycemia; the ratios of the means (95% CI) were 1.00, 0.79 (0.60–1.04), and 0.60 (0.42–0.87) for individuals with normoglycemia, IFG, and diabetes, respectively (p trend < 0.001). Restricted cubic spline regression analysis showed that IgG spike antibody titers decreased linearly with increasing concentrations of FPG. Conclusion: Diabetes and, to a lesser extent, IFG may be associated with poor humoral immune response after BNT162b2 vaccination.

Bibliometric and Visualized Analysis of 2011-2020 Publications on Physical Activity Therapy for Diabetes.

BackgroundThis study was designed to investigate the global emerging trends of physical activity therapy for diabetes based on a bibliometric analysis of the publications.MethodsPublication papers from 2011 to 2020 were retrieved from the database of “Web of Science Core Collection” with the topic search. A number of papers, citations, authors, countries, institutions, and references were extracted. CiteSpace was used to analyze co-citation on authors, collaborations between countries and institutions, and detect the emerging trends of burst keywords and references.ResultsA total of 2651 publications were recruited in this study and showed an upward trend of annual publications. Diabetes obesity & metabolism (journal), the United States (country), Harvard University (institution), and Kaku K (author) published the most papers in this research field. “Impaired glucose tolerance” (2011–2012) was the highest strength burst keyword, while “cardiovascular outcome” (2017–2020) was the most burst keyword in the last 5 years. Moreover, “Standards of medical care in diabetes – 2014” was the strongest burst reference.Conclusion“Physical activity therapy for diabetes” has been accepted remarkably over the last 10 years. The keywords of “impaired glucose tolerance,” “Cardiovascular outcome,” “improves glycemic control,” “Self-management,” and exercise type including “Aerobic exercise, muscle strength” may be the latest research frontiers.

Creating diabetes guidelines for the individual not just the illness

For more than 30 years the American Diabetes Association (ADA) has developed and published recommendations and related documents for clinical practice for diabetes care. ADA's Standards of Medical Care in Diabetes document,1 referred to as the Standards of Care, is intended to inform clinicians, researchers, policy makers, payers (ie, federal and commercial insurance, and federal and states assistance programmes, and health maintenance organisations) of the components of diabetes care, general treatment goals, and tools to evaluate the quality of care.

Adherence to the American Diabetes Association's Glycemic Goals in the Treatment of Diabetes Among Older Americans, 2001-2018.

To assess trends in HbA1c and appropriateness of diabetes medication use by patient health status. We conducted cross-sectional analysis of 2001-2018 National Health and Nutrition Examination Survey (NHANES). We included older adults age ≥65 years who had ever been told they had diabetes, had HbA1c >6.4%, or had fasting plasma glucose >125 mg/dL. Health status was categorized as good, intermediate, or poor. Being below goal was defined as taking medication despite having HbA1c ≥1% below the glycemic goals of the American Diabetes Association (ADA), which varied by patient health status and time period. Drugs associated with hypoglycemia included sulfonylureas, insulin, and meglitinides. We included 3,539 patients. Mean HbA1c increased over time and did not differ by health status. Medication use increased from 59% to 74% with metformin being the most common drug in patients with good or intermediate health and sulfonylureas and insulin most often prescribed to patients with poor health. Among patients taking medications, prevalence of patients below goal increased while prevalence of those above goal decreased from 2001 to 2018. One-half of patients with poor health and taking medications had below-goal HbA1c; two-thirds received drugs associated with hypoglycemia. Patients with poor health who were below goal had 4.9 (95% CI 2.3-10.4) times the adjusted odds of receiving drugs associated with hypoglycemia than healthy patients. In accordance with ADA's newer Standards of Medical Care in Diabetes, HbA1c goals were relaxed but did not differ by health status. Below-goal HbA1c was common among patients with poor health; many were prescribed medications associated with a higher risk of hypoglycemia.
