INTRACAPSULAR EXTRACTION OF SENILE CATARACT

I have performed 170,000 operations for cataract, almost all of them intracapsular extractions, and the method which I have followed throughout was devised by me shortly after I began ophthalmic surgery. The controversy as to the use of the intracapsular or the extracapsular operation is always going on in all countries ; Indian surgeons as a whole, especially the ophthalmic surgeons in the Punjab, have always favored the intracapsular method, while in some localities the extracapsular operation is the rule, although some surgeons in Europe have adopted the intracapsular operation as a routine in their clinics. The advantages of the intracapsular method may be presented as follows : (1) It does not require two operations; (2) the operation can be performed even when the cataract is not completely mature; (3) as there are no shreds of capsule or lens matter to be caught in the edges of the wound, the chances of

SECONDARY GLAUCOMA DUE TO THE LENS

Practically all the tissues of the eye are capable in some manner of causing secondary glaucoma. Increased intraocular pressure is anticipated, with rupture of the capsule and lens matter streaming into the anterior chamber. The same is true of the lens luxated into the anterior chamber with the capsule intact. This tissue in the vitreous, it is well known, is also a cause of recurring iridocyclitis and associated glaucoma. Other mechanisms by which the lens induces glaucoma are, however, clinically less common and less well known. Even without actual conclusive proof, the lens may frequently be suspected of being the indirect or direct factor producing abnormally high intraocular pressure. Clinical records have been reviewed and material of the pathologic laboratory has been studied in the hope of constructing a classification of glaucoma attributable to abnormality of the lens. Chronic Postoperative or Posttraumatic Retinitis (Retinitis Serosa)

The author reviews briefly the so-called iritis serosa, then proceeds to the main theme, describing an identical and parallel disease in the retina which is called retinitis serosa. The two diseases are independent of each other. Discussion is restricted to injured eyes in which the lens has been greatly damaged. The cause of the lesion in iris and retina is attributed to the action of irritating substances derived from free lens matter. POSTOPERATIVE NONEXPULSIVE SUBCHOROIDAL HEMORRHAGE

Bleeding under the choroid may occur in connection with injuries, operations or perforating ulcers of the cornea. This contribution is restricted to cases in which operation has been performed and in which the hemorrhage has remained confined to the perichoroidal space. In the so-called expulsive hemorrhage, the bleeding takes place with such force that the choroid is ruptured and the blood in flowing out of the opening in the globe carries with it most of the intra-ocular contents. In the form here considered, the choroid is not torn. REPORT OF CASES Case1.—A woman, aged 65, had glaucoma. The cornea was steamy. The pupil was contracted by miotics to the size of a pinpoint. The anterior chamber was very shallow. Vision was 6/36. Iridectomy was performed.
