Neuronal Modeling of Cross-Sensory Visual Evoked Magnetoencephalography Responses in the Auditory Cortex.

Previous studies have demonstrated that auditory cortex activity can be influenced by cross-sensory visual inputs. Intracortical laminar recordings in nonhuman primates have suggested a feedforward (FF) type profile for auditory evoked but feedback (FB) type for visual evoked activity in the auditory cortex. To test whether cross-sensory visual evoked activity in the auditory cortex is associated with FB inputs also in humans, we analyzed magnetoencephalography (MEG) responses from eight human subjects (six females) evoked by simple auditory or visual stimuli. In the estimated MEG source waveforms for auditory cortex regions of interest, auditory evoked response showed peaks at 37 and 90 ms and visual evoked response at 125 ms. The inputs to the auditory cortex were modeled through FF- and FB-type connections targeting different cortical layers using the Human Neocortical Neurosolver (HNN), which links cellular- and circuit-level mechanisms to MEG signals. HNN modeling suggested that the experimentally observed auditory response could be explained by an FF input followed by an FB input, whereas the cross-sensory visual response could be adequately explained by just an FB input. Thus, the combined MEG and HNN results support the hypothesis that cross-sensory visual input in the auditory cortex is of FB type. The results also illustrate how the dynamic patterns of the estimated MEG source activity can provide information about the characteristics of the input into a cortical area in terms of the hierarchical organization among areas.

Multimodal non-invasive evaluation in MRI-negative epilepsy patients.

Presurgical evaluation is still challenging for MRI-negative epilepsy patients. As non-invasive modalities are the easiest acceptable and economic methods in determining the epileptogenic zone, we analyzed the localization value of common non-invasive methods in MRI-negative epilepsy patients. In this study, we included epilepsy patients undergoing presurgical evaluation with presurgical negative MRI. MRI post-processing was performed using a Morphometric Analysis Program (MAP) on T1-weighted volumetric MRI. The relationship between MAP, magnetoencephalography (MEG), scalp electroencephalogram (EEG), and seizure outcomes was analyzed to figure out the localization value of different non-invasive methods. Eighty-six patients were included in this study. Complete resection of the MAP-positive regions or the MEG-positive regions was positively associated with seizure freedom (p = 0.028 and 0.007, respectively). When an area is co-localized by MAP and MEG, the resection of the area was significantly associated with seizure freedom (p = 0.006). However, neither the EEG lateralization nor the EEG localization showed statistical association with the surgical outcome (p = 0.683 and 0.505, respectively). In conclusion, scalp EEG had a limited role in presurgical localization and predicting seizure outcome, combining MAP and MEG results can significantly improve the localization of epileptogenic lesions and have a positive association with seizure-free outcome. PLAIN LANGUAGE SUMMARY: Due to the lack of obvious structure abnormalities on neuroimaging examinations, the identification of epilepsy lesions in MRI-negative epilepsy patients can be difficult. In this study, we intended to use non-invasive examinations to explore the potential epileptic lesions in MRI-negative epilepsy patients and to determine the results accuracy by comparing the neuroimaging results with the epilepsy surgery outcomes. A total of 86 epilepsy patients without obvious structure lesions on MRI were included, and we found that the combinations of different non-invasive examinations and neuroimaging post-processing methods are significantly associated with the seizure freedom results of epilepsy surgery.

From MEG to clinical EEG: evaluating a promising non-invasive estimator of defense-related muscle sympathetic nerve inhibition

Sudden, unexpected stimuli can induce a transient inhibition of sympathetic vasoconstriction to skeletal muscle, indicating a link to defense reactions. This phenomenon is relatively stable within, but differs between, individuals. It correlates with blood pressure reactivity which is associated with cardiovascular risk. Inhibition of muscle sympathetic nerve activity (MSNA) is currently characterized through invasive microneurography in peripheral nerves. We recently reported that brain neural oscillatory power in the beta spectrum (beta rebound) recorded with magnetoencephalography (MEG) correlated closely with stimulus-induced MSNA inhibition. Aiming for a clinically more available surrogate variable reflecting MSNA inhibition, we investigated whether a similar approach with electroencephalography (EEG) can accurately gauge stimulus-induced beta rebound. We found that beta rebound shows similar tendencies to correlate with MSNA inhibition, but these EEG data lack the robustness of previous MEG results, although a correlation in the low beta band (13–20 Hz) to MSNA inhibition was found (p = 0.021). The predictive power is summarized in a receiver-operating-characteristics curve. The optimum threshold yielded sensitivity and false-positive rate of 0.74 and 0.33 respectively. A plausible confounder is myogenic noise. A more complicated experimental and/or analysis approach is required for differentiating MSNA-inhibitors from non-inhibitors based on EEG, as compared to MEG.

Utility of magnetoencephalography combined with stereo-electroencephalography in resective epilepsy surgery: A 2-year follow-up.

Precise and accurate implantation of stereo-electroencephalography (SEEG) electrodes is critical for the localization of the seizure onset zone (SOZ), which plays a leading role in the prognosis of resective epilepsy surgery. Magnetoencephalography (MEG) is a noninvasive technique which can delineate the epilepsy focus by visualizing interictal spikes into dipole clusters. MEG may provide supporting information for guiding SEEG electrode implantation and improve the long-term outcomes of epilepsy surgery. In this study, we evaluated the accuracy of MEG in determining the SOZ. We retrospectively analyzed patients with refractory epilepsy who underwent MEG examination and SEEG implantation before resective epilepsy surgery in the Shanghai Ruijin Hospital. The SEEG plan was designed according to the dipole clusters and the resections were operated according to the SEEG recordings. We investigated the relationships of the pattern of MEG dipole clusters and SEEG sampling to the final resective surgery prognosis. We included 42 patients with a postoperative follow-up of at least 2 years (mean 34.1 months). Eighteen (42%) patients who showed concordant localization between MEG and SEEG evaluation had a higher probability of seizure-free outcome (p=0.046, χ2=4.835, odds ratio=5.00, 95% CI=1.12-22.30). Complete sampling of MEG dipole clusters by SEEG electrodes was found in 23 (54%) patients, who had higher probability of seizure-free outcome that those with incomplete sampling (p<0.001, odds ratio=16.67, 95% CI=3.11-89.28). MEG results showing a single, tight cluster or stable orientation were associated to better seizure outcomes after resective surgery. MEG dipole cluster helps SEEG implantation in localizing the SOZ for better long-term epilepsy surgery outcome. The MEG results can play a role as prognostic predictors of epilepsy surgery.

Using EEG and MEG to characterize extreme delta brush in a patient with anti-NMDA receptor encephalitis

BackgroundExtreme delta brush (EDB) is considered a potential marker for anti-N-methyl-d-aspartate receptor (anti-NMDAR) encephalitis. The brain regions involved in EDB are unclear.Case presentationA 16-year-old woman with anti-NMDAR encephalitis who was experiencing psychosis was admitted. Electroencephalography (EEG) and magnetoencephalography (MEG) were used to analyze EDB in the patient. EDB on EEG could be disturbed by opening and closing the eyes, by occipital alpha rhythms and by sleep-wake cycles. The MEG results showed beta activity originating from bilateral superior parietal lobes. However, the delta wave originated from bilateral superior temporal gyri, the right middle temporal gyrus, the right inferior frontal gyrus, and the left inferior parietal lobe.ConclusionsDelta wave and beta activity might originate from different brain regions. Beta activity might be transmitted forward to the frontotemporal lobe and superimposed with delta activity to form EDB on EEG.

Dynamic analysis on simultaneous iEEG-MEG data via hidden Markov model

BackgroundIntracranial electroencephalography (iEEG) recordings are used for clinical evaluation prior to surgical resection of the focus of epileptic seizures and also provide a window into normal brain function. A major difficulty with interpreting iEEG results at the group level is inconsistent placement of electrodes between subjects making it difficult to select contacts that correspond to the same functional areas. Recent work using time delay embedded hidden Markov model (HMM) applied to magnetoencephalography (MEG) resting data revealed a distinct set of brain states with each state engaging a specific set of cortical regions. Here we use a rare group dataset with simultaneously acquired resting iEEG and MEG to test whether there is correspondence between HMM states and iEEG power changes that would allow classifying iEEG contacts into functional clusters. MethodsSimultaneous MEG-iEEG recordings were performed at rest on 11 patients with epilepsy whose intracranial electrodes were implanted for pre-surgical evaluation. Pre-processed MEG sensor data was projected to source space. Time delay embedded HMM was then applied to MEG time series. At the same time, iEEG time series were analyzed with time-frequency decomposition to obtain spectral power changes with time. To relate MEG and iEEG results, correlations were computed between HMM probability time courses of state activation and iEEG power time course from the mid contact pair for each electrode in equally spaced frequency bins and presented as correlation spectra for the respective states and iEEG channels. Association of iEEG electrodes with HMM states based on significant correlations was compared to that based on the distance to peaks in subject-specific state topographies. ResultsFive HMM states were inferred from MEG. Two of them corresponded to the left and the right temporal activations and had a spectral signature primarily in the theta/alpha frequency band. All the electrodes had significant correlations with at least one of the states (p < 0.05 uncorrected) and for 27/50 electrodes these survived within-subject FDR correction (q < 0.05). These correlations peaked in the theta/alpha band. There was a highly significant dependence between the association of states and electrodes based on functional correlations and that based on spatial proximity (p = 5.6e−6,χ2 test for independence). Despite the potentially atypical functional anatomy and physiological abnormalities related to epilepsy, HMM model estimated from the patient group was very similar to that estimated from healthy subjects. ConclusionEpilepsy does not preclude HMM analysis of interictal data. The resulting group functional states are highly similar to those reported for healthy controls. Power changes recorded with iEEG correlate with HMM state time courses in the alpha-theta band and the presence of this correlation can be related to the spatial location of electrode contacts close to the individual peaks of the corresponding state topographies. Thus, the hypothesized relation between iEEG contacts and HMM states exists and HMM could be further explored as a method for identifying comparable iEEG channels across subjects for the purposes of group analysis.

Across-session consistency of context-driven language processing: A magnetoencephalography study.

Changes in brain organization following damage are commonly observed, but they remain poorly understood. These changes are often studied with imaging techniques that overlook the temporal granularity at which language processes occur. By contrast, electrophysiological measures provide excellent temporal resolution. To test the suitability of magnetoencephalography (MEG) to track language‐related neuroplasticity, the present study aimed at establishing the spectro‐temporo‐spatial across‐session consistency of context‐driven picture naming in healthy individuals, using MEG in two test–retest sessions. Spectro‐temporo‐spatial test–retest consistency in a healthy population is a prerequisite for studying neuronal changes in clinical populations over time. For this purpose, 15 healthy speakers were tested with MEG while performing a context‐driven picture‐naming task at two time points. Participants read a sentence missing the final word and named a picture completing the sentence. Sentences were constrained or unconstrained toward the picture, such that participants could either retrieve the picture name through sentence context (constrained sentences), or could only name it after the picture appeared (unconstrained sentences). The context effect (constrained versus unconstrained) in picture‐naming times had a strong effect size and high across‐session consistency. The context MEG results revealed alpha–beta power decreases (10–20 Hz) in the left temporal and inferior parietal lobule that were consistent across both sessions. As robust spectro‐temporo‐spatial findings in a healthy population are required for working toward longitudinal patient studies, we conclude that using context‐driven language production and MEG is a suitable way to examine language‐related neuroplasticity after brain damage.

The impact of MEG results on surgical outcomes in patients with drug-resistant epilepsy associated with focal encephalomalacia: a single-center experience.

To analyze the impact of magnetoencephalography (MEG) results on surgical outcomes in patients with drug-resistant epilepsy secondary to encephalomalacia. We retrospectively reviewed 121 patients with drug-resistant epilepsy associated with encephalomalacia who underwent MEG followed by resection surgery. Patients were subdivided into concordant MEG group and dis-concordant MEG group for analysis based on whether the MEG results were in concordance with epileptogenic zones or not. 121 patients were included in the present study. The MEG spike sources of 73 (60.33%) patients were in concordance with epileptogenic zones while the MEG spike sources of the other 48 (39.67%) were in dis-concordance with epileptogenic zones. Favorable seizure outcomes were achieved in 79.45% (58 of 73) of patients with concordant MEG results while only 62.50% (30 of 48) of patients with dis-concordant MEG results were seizure free with a follow-up of 2-10years. The differences of seizure-free rate between patients with concordant MEG results and dis-concordant MEG results were statistically significant. For patients with concordant MEG results, bilateral lesions on MRI are the only independent predictor of unfavorable seizure outcomes. For patients with discordant MEG results, duration of seizures is the only independent predictor of unfavorable seizure outcomes. Concordant MEG results are associated with favorable seizure outcomes. Bilateral lesions on MRI independently predict unfavorable seizure outcomes in patients with concordant MEG results while longer seizure durations independently predict unfavorable seizure outcomes in patients with dis-concordant MEG results.

Noninvasive Stimulation of the Ventromedial Prefrontal Cortex Indicates Valence Ambiguity in Sad Compared to Happy and Fearful Face Processing.

The ventromedial prefrontal cortex (vmPFC) is known to be specifically involved in the processing of stimuli with pleasant, rewarding meaning to the observer. By the use of non-invasive transcranial direct current stimulation (tDCS), it was previously possible to show evidence for this valence specificity and to modulate the impact of the vmPFC on emotional network processing. Prior results showed increased neural activation during pleasant relative to unpleasant stimulus processing after excitatory compared to inhibitory vmPFC-tDCS. As dysfunctional vmPFC activation patterns are associated with major depressive disorder (MDD), tDCS of this region could render an attractive application in future therapy. Here, we investigated vmPFC-tDCS effects on sad compared to happy face processing, as sad faces are often used in the study of mood disorders. After counterbalanced inhibitory or excitatory tDCS, respectively, healthy participants viewed happy and sad faces during magnetoencephalography (MEG) recording. In addition, tDCS effects on an interpretational bias of ambiguous happy-sad face morphs and an attentional bias of a dot-probe task with happy and sad faces as emotional primes were investigated. Finally, in conjoint analyses with data from a previous sibling study (happy and fearful faces) we examined whether excitatory vmPFC-tDCS would reveal a general increase in processing of pleasant stimuli independent of the type of unpleasant stimuli applied (sad vs. fearful faces). MEG and behavioral results showed that happy faces promoted a relative positivity bias after excitatory compared to inhibitory tDCS, visible in left orbitofrontal cortex and in the emotion-primed dot-probe task. A converse pattern in the MEG data during sad face processing suggests the possible involvement of an empathy network and thus significantly differed from neuronal processing of fearful face processing. Implications for the bearing of vmPFC modulation on emotional face processing and the impact of specific unpleasant face expressions are discussed.

Dynamics of brain activation during learning of syllable-symbol paired associations

Initial stages of reading acquisition require the learning of letter and speech sound combinations. While the long-term effects of audio-visual learning are rather well studied, relatively little is known about the short-term learning effects at the brain level. Here we examined the cortical dynamics of short-term learning using magnetoencephalography (MEG) and electroencephalography (EEG) in two experiments that respectively addressed active and passive learning of the association between shown symbols and heard syllables. In experiment 1, learning was based on feedback provided after each trial. The learning of the audio-visual associations was contrasted with items for which the feedback was meaningless. In experiment 2, learning was based on statistical learning through passive exposure to audio-visual stimuli that were consistently presented with each other and contrasted with audio-visual stimuli that were randomly paired with each other. After 5–10 min of training and exposure, learning-related changes emerged in neural activation around 200 and 350 ms in the two experiments. The MEG results showed activity changes at 350 ms in caudal middle frontal cortex and posterior superior temporal sulcus, and at 500 ms in temporo-occipital cortex. Changes in brain activity coincided with a decrease in reaction times and an increase in accuracy scores. Changes in EEG activity were observed starting at the auditory P2 response followed by later changes after 300 ms. The results show that the short-term learning effects emerge rapidly (manifesting in later stages of audio-visual integration processes) and that these effects are modulated by selective attention processes.

Chapter 13 - Magnetoencephalography for localizing and characterizing the epileptic focus

Magnetoencephalography (MEG) is the noninvasive measurement of the miniscule magnetic fields produced by electrical currents flowing in the brain-the same neuroelectric activity that produces the EEG. MEG is one of several diagnostic tests employed in the evaluation of patients with epilepsy, but without the need to expose the patient to any potentially harmful agents. MEG is especially important in those being considered for epilepsy surgery, in whom accurate localization of the epileptic focus is paramount. While other modalities infer brain function indirectly by measuring changes in blood flow, metabolism, oxygenation, etc., MEG, as well as EEG, measures neuronal and synaptic function directly and, like EEG, MEG enjoys submillisecond temporal resolution. The measurement of magnetic fields provides information not only about the amplitude of the current but also its orientation. MEG picks up the magnetic field from neuromagnetometers surrounding the head in a helmet-shaped array of sensors. Clinical whole-head systems currently have 200-300 magnetic sensors, thereby offering very high resolution. The magnetic signals are not distorted by anatomy, because magnetic susceptibility is the same for all tissues, including the skull. Hence, MEG allows for a more accurate measurement and localization of brain activities than does EEG. Because one of its primary strengths is the ability to precisely localize electromagnetic activity within brain areas, MEG results are always coregistered to the patient's MRI. When combined in this way with structural imaging, it has been called magnetic source imaging (MSI), but MEG is properly understood as a clinical neurophysiologic diagnostic test. Signal processing and clinical interpretation in magnetoencephalography require sophisticated noise reduction and computerized mathematical modeling. Technological advances in these areas have brought MEG to the point where it is now part of routine clinical practice. MEG has become an indispensable part of the armamentarium at epilepsy centers where MEG laboratories are located, especially when patients are MRI-negative or where results of other structural and functional tests are not entirely concordant.

Age Effects on Spatiotemporal Dynamics of Response Inhibition: An MEG Study

Inhibition, the ability to suppress irrelevant information, thoughts or movements, is crucial for humans to perform context-appropriate behaviors. It was suggested that declined cognitive performance in older adults might be attributed to inhibitory deficiencies. Although previous studies have shown an age-associated reduction in inhibitory ability, the understanding regarding its cortical spatiotemporal maps remained limited. Thus, we used a whole-head magnetoencephalography (MEG) to elucidate the age effects on response inhibition, and to explore the brain activation differences in high- and low-performing seniors. We recruited 22 younger and 22 older adults to participate in the visual Go/No-go task. Both behavioral performance and neuromagnetic responses to No-go stimuli were analyzed. The behavioral results showed that the older adults made more false alarm (FA) errors than the younger adults did. The MEG results showed that the seniors exhibited declined cortical activities in middle temporal gyrus (MTG) and delayed activation in MTG, prefrontal cortex (PFC) and pre-supplementary motor area (pre-SMA). Furthermore, among the older adults, more recruitment of the left PFC was found in the high-performers than in the lower-performers. In conclusion, age-related deficiencies in response inhibition were observed in both behavioral performance and neurophysiological measurement. Our results also suggested that frontal recruitment plays a compensatory role in successful inhibition.

T19. MULTIMODAL IMAGING IN FIRST EPISODE PSYCHOSIS: MAGNETOENCEPHALOGRAPHY, 7T FMRI STROOP, AND 7T MRS SPECTROSCOPY

BackgroundSchizophrenia (SZ) is an illness whose heterogeneity has impeded understanding the underlying pathophysiology. In order to better understand this heterogeneity, here we used magnetoencephalography (MEG), 7T Magnetic Resonance Spectroscopy (MRS), and 7T fMRI during the Stroop task, on the same set of patients with first episode psychosis (SZ).Methods22 minimally treated first episode SZ and 24 healthy controls (HC) matched for age, gender, and family socio-economic status were recruited. Neurometabolite levels were obtained from the bilateral anterior cingulate cortex using 7T proton MRS with an ultra-short echo time (5 ms) STEAM sequence, and referenced to water. MEG was performed in a 4D systems 148 channel magnetometer, and both the auditory evoked potential to 40 Hz tone clicks, and the resting state (eyes closed) were recorded. The fMRI BOLD response to the Stroop task was also recorded in a 7T scanner.ResultsThe magnitude of the audio-evoked MEG responses to 40 Hz tone clicks was not significantly different between SZ and HC. However, many SZ showed high levels of theta-band activity during the resting state. The ratio of theta to alpha band activity in the anterior MEG sensors significantly differentiated SZ from HC, P<0.05 by t-test. MRS levels of glutamate and total NAA (tNAA), also separated HC from SZ, P<0.05, t-test. An across-groups whole brain analysis of the Stroop fMRI BOLD response to incongruent trials relative to congruent trials was performed (p < .01, FDR-corrected). The strongest signal came from a region in the left parietal (MNI, -30, -58.5, 48), and between-group analysis of the BOLD signal from a 4mm sphere surrounding this location revealed that the activation was greater for SZ than HC by t-test, P<0.05. The MEG theta/alpha ratio, and the left parietal fMRI Stroop effect, were significantly correlated, r=0.45, P=0.005. However, the fMRI Stroop was uncorrelated with the MRS tNAA (r = -0.11, P=0.491) and also uncorrelated with the MRS glutamate levels (r = -0.16, P=0.334).DiscussionWe speculate that the MEG and fMRI data, and the MRS neurometabolite levels, may reflect two relatively independent underlying pathological mechanisms in SZ. Possibly the MEG and fMRI results are indicative of dysfunction in long-range cortical-cortical networks, while the MRS data is more indicative of local neurometabolic dysfunction. Further exploration of SZ using multiple imaging modalities on the same subjects may help to untangle the underlying pathophysiological basis of the heterogeneity of this disorder.

Alzheimer's disease: The state of the art in resting-state magnetoencephalography.

Alzheimer's disease (AD) is accompanied by functional brain changes that can be detected in imaging studies, including electromagnetic activity recorded with magnetoencephalography (MEG). Here, we systematically review the studies that have examined resting-state MEG changes in AD and identify areas that lack scientific or clinical progress. Three levels of MEG analysis will be covered: (i) single-channel signal analysis, (ii) pairwise analyses over time series, which includes the study of interdependencies between two time series and (iii) global network analyses. We discuss the findings in the light of other functional modalities, such as electroencephalography (EEG) and functional magnetic resonance imaging (fMRI). Overall, single-channel MEG results show consistent changes in AD that are in line with EEG studies, but the full potential of the high spatial resolution of MEG and advanced functional connectivity and network analysis has yet to be fully exploited. Adding these features to the current knowledge will potentially aid in uncovering organizational patterns of brain function in AD and thereby aid the understanding of neuronal mechanisms leading to cognitive deficits.

Spatiotemporal reorganization of the reading network in adult dyslexia

Developmental dyslexia is characterized by impairments in reading fluency and spelling that persist into adulthood. Here, we hypothesized that high-achieving adult dyslexics (i.e., university students with a history of dyslexia) manage to cope with these deficits by relying to a greater extent on morphological information than do non-impaired adult readers. We used magnetoencephalography (MEG) in a primed lexical decision task, in which we contrasted orthographic, morphological and semantic processing. Behavioral results confirmed that adult dyslexics did indeed rely to a greater extent on the semantic properties of morphemes than controls. In line with this, MEG results showed early morphological effects (100–200 msec) in a frontal network, which reflected the contribution of semantic processing. The same effects occurred much later in controls (∼400 msec). In contrast, controls showed early orthographic priming effects in posterior left inferior temporal gyrus (LITG) at around 130 msec, which were not seen in dyslexics. In the LITG, dyslexics showed only late activation of semantic and orthographic information. The present results suggest a spatiotemporal reorganization of the reading network, in which morphological information located in frontal regions is activated earlier in high-achieving adults dyslexics than controls.

Accuracy of MEG in localizing irritative zone and seizure onset zone: Quantitative comparison between MEG and intracranial EEG

BackgroundWe conducted the study to examine accuracy of the magnetoencephalography (MEG) spike source localization in presurgical evaluation of patients with medically refractory focal epilepsy. MethodsTen consecutive patients with refractory focal epilepsy who were candidates for two-stage surgery with long-term intracranial electroencephalography (ICEEG) monitoring were enrolled. Interictal MEG recordings with simultaneous scalp EEG were obtained within 7days before the ICEEG electrode implantation. The location of each MEG spike source was quantitatively compared with ICEEG spike foci (focal area of interictal spikes) and ICEEG ictal foci (earliest cortical origin of seizures). Gyral-width concordance and sublobar concordance were also determined for all MEG spike sources. Gyral-width concordance was defined by distance of 15mm or less between MEG spike sources and ICEEG spike foci or ICEEG ictal foci. ResultsVisual analyses of the MEG traces of all 10 patients revealed 292 spikes (29.2±24.0 per patient). Spike yield of the MEG was similar to the simultaneously recorded scalp EEG. MEG spike sources were closely located with ICEEG spike foci (distance: 9.3±10.8mm). Clustered MEG spike sources were even closer to ICEEG spike foci (distance: 7.3±6.4mm). MEG spike sources, even clustered ones, were less concordant with ICEEG ictal foci and had significant longer distance from ICEEG ictal foci (distance: 21.5±15.6mm for all sources, 19.7±13.7mm for clustered sources). Gyral-width concordance rate and sublobar concordance rate were also higher with ICEEG interictal spike foci than with ICEEG ictal foci. On the other hand, 53.4% of interictal spike foci from ICEEG were not detected by interictal MEG recordings. ConclusionsMEG spike sources, especially clustered ones, from interictal recording could localize the irritative zone of ICEEG with a high accuracy. However, MEG spike sources have relatively poor correlation with seizure onset zone and lower sensitivity in identifying all irritative zones of ICEEG. This limitation should be considered in the interpretation of MEG results.

The post cingulate's role in triggering voluntary swallowing

Objective To investigate the function of the post cingulate in triggering voluntary swallowing by examining the activated areas in the brain of dysphagic patients with and without delayed pharyngeal swallowing using magnetoencephalography. Methods Videofluoroscopy was used to detect the latency of pharyngeal swallowing of 6 dysphagic patients. Magnetoencephalography (MEG) was used to monitor the whole course of voluntary swallowing, and an equivalent current dipole was applied to the activated areas of the brain 2500 ms before the subconscious electromyographic signals appeared. Magnetic resonance imaging (MRI) was performed just after the magnetoencephalography. A magnetic source image (MSI) was generated by overlaying the MRI and MEG results. Results The MSIs of five of the patients were derived. Three had delayed initiation of pharyngeal swallowing (with the latency being 0.03-0.12 s) and the other two showed normal latency during pharyngeal swallowing. In the former 3 patients, the post cingulate was activated at 1426 ms, 1138 ms and 1675 ms before the burst of electromyography signals, which was later than that of the anterior cingulate and the insular cingulate. The 2 patients without delayed pharyngeal swallowing had their post cingulate activated at 1971 ms and 2483 ms before or at almost the same time as the anterior cingulate and insular. Conclusion Post cingulate activation occurs later than that of the anterior cingulate and insular in patients with delayed pharyngeal swallowing, the reverse of normal swallowing. This result indicates that the post cingulate may play an important role in triggering voluntary swallowing. Averaging auditory evoked magnetoencephalographic and electroencephalographic responses: a critical discussion.

In the analysis of data from magnetoencephalography (MEG) and electroencephalography (EEG), it is common practice to arithmetically average event-related magnetic fields (ERFs) or event-related electric potentials (ERPs) across single trials and subsequently across subjects to obtain the so-called grand mean. Comparisons of grand means, e.g. between conditions, are then often performed by subtraction. These operations, and their statistical evaluation with parametric tests such as ANOVA, tacitly rely on the assumption that the data follow the additive model, have a normal distribution, and have a homogeneous variance. This may be true for single trials, but these conditions are rarely met when ERFs/ERPs are compared between subjects, meaning that the additive model is seldom the correct model for computing grand mean waveforms. Here, we summarize some of our recent work and present new evidence, from auditory-evoked MEG and EEG results, that the non-normal distributions and the heteroscedasticity observed instead result because ERFs/ERPs follow a mixed model with additive and multiplicative components. For peak amplitudes, such as the auditory M100 and N100, the multiplicative component dominates. These findings emphasize that the common practice of simply subtracting arithmetic means of auditory-evoked ERFs or ERPs is problematic without prior adequate transformation of the data. Application of the area sinus hyperbolicus (asinh) transform to data following the mixed model transforms them into the requested additive model with its normal distribution and homogeneous variance. We therefore advise checking the data for compliance with the additive model and using the asinh transform if required.

Enregistrement magnéto-encéphalographique (MEG) de réminiscences du trauma chez des femmes souffrant de stress post-traumatique : une étude pilote

The experiment studied the effects of a short duration exposure to traumatic memories using magneto-encephalography (MEG). Nine right-handed DSM-4 PTSD patients were recruited from a unit for anxiety disorders and an organisation supporting victims of violence. In order to have a homogeneous sample, we included only women who suffered from civilian PTSD. Exclusion criteria were co-morbid major medical illness, metallic dental prostheses that would interfere in the magnetic measurement, and current drug treatment. All participants were free from neurological disease and had normal hearing. They signed a written informed consent form. An ethics committee accepted the study. A tape-recorded voice administered a script-driven imagery. The patients had to imagine, successively, a neutral image, a traumatic memory and rest, while MEG measured brain activities across delta, theta, alpha and beta bands. Each condition lasted three minutes. Heart rate (HR), anxiety and the vividness of mental images were recorded at the end of each phase. MEG power analysis was carried out with Statistical Parametric Mapping (SPM) 8. The signals were averaged for each of the three conditions of threeminutes duration. The dependent variable was a subtracted value: (trauma - rest) - (neutral - rest). The significance threshold was set at P<0.01. Anxiety and HR significantly increased during the trauma condition and returned to the neutral level during rest. The vividness of the mental imagery remained stable across the three conditions. The left-brain demonstrated a statistically significant power decrease in the secondary visual cortex (BA 18-19) in the delta band, the insula (BA13) in the beta band, the insula (BA13), premotor cortex (BA 6), Broca area (BA 44), and BA 43, in the alpha band. The symptom provocation protocol was successful in eliciting subjective anxiety and HR response in relation to traumatic memories. Our MEG results are in keeping with previous neuro-imagery studies showing decreased activities in the insula and Broca area during PTSD symptom provocation. However, we did not replicate the activation in the amygdala and the cingulate and prefrontal cortex found in some studies. Moreover, the within-group design, the small sample, and the inclusion of only female patients with milder dissociative symptoms limit our conclusions. The MEG protocol we used may also explain some partial discrepancies with previous MEG studies. However, our aim was to provoke a specific autobiographic recall of a traumatic event unfolding several sequential mental images along three minutes as in exposure therapy for PTSD. Despite its limitations, this pilot study is the first to provide MEG data during trauma recall. It suggests that recalling a specific traumatic event along three minutes results in hypo-activations of the brain regions regulating language and emotions. This paves the way to recording whole sessions of specific therapies for PTSD, with MEG using the millisecond resolution. MEG might be of interest to study the suppression of traumatic memories and their activation and habituation through prolonged graduated exposure in imagination across several sessions. MEG could also be used to study the effects of medication on PTSD symptoms. A controlled replication in a larger sample including male and female patients with various traumatic experiences is needed.

Neuromagnetic coherence of epileptic activity: An MEG study

PurposeThis study was undertaken to test the hypothesis that patients with epilepsy have abnormal imaginary coherence compared with control subjects. MethodsThirty patients with seizures underwent magnetoencephalography (MEG) recording using a whole cortex MEG system. Conventional equivalent current dipoles (ECDs) and synthetic aperture magnetometry (SAM) were used to analyze MEG data. Neural synchronization was studied using imaginary coherence to analyze resting-state MEG data. The ECDs, SAM, and MEG results were then compared with intra/extra-operative EEG. ResultsAbnormal imaginary coherence was identified in all patients (30/30, 100%). The locations of abnormal imaginary coherence were in agreement with the ECDs locations of spikes in 23 patients (23/30, 76.7%). The ECD locations in 5 patients were scattered or located bilaterally. The locations of abnormal imaginary coherence were in agreement with SAM locations in 26 patients (26/30, 86.7%). One case of imaginary coherence was located in two lobes. The ECDs fit locations were in agreement with SAM locations in 21 patients (21/30, 70.0%). The locations of abnormal imaginary coherence, ECDs, and SAM were in agreement with intra/extra-operative EEG in 23 patients (23/30, 76.7%), 17 patients (17/30, 56.7%), and 20 patients (20/30, 66.7%), respectively. The results of ECDs location, SAM location, imaginary coherence, and intracranial EEG (iEEG) were consistent in 15 patients (15/30, 50%). ConclusionsThe results show that patients with epilepsy have abnormal imaginary coherence, and suggest that the location and coherence of epileptic activity could be quantitatively identified and analyzed using neuromagnetic signals.
