There has been significant interest in utilizing this platform for partial nephrectomy. The goal of this study is to compare the perioperative outcomes of single port to multiport robotic technology. METHODS: This is a prospective cohort study including multiple institutions. Peri-operative clinical and operative data was collected from 2502 patients who underwent robotic PN for renal cell carcinoma between 2006 and 2021. Baseline preoperative outcomes were compared between multi-port and SP robotic assisted partial nephrectomy (RAPN) using Chi square test, Fisher’s exact, and Mann-Whitney U test. In the multivariable analysis, controlling for all baseline characteristics and or surgeon variability, the relationship between preoperative outcomes and surgical approach was evaluated using negative binomial regression, multiple linear regression, and logistic regression. RESULTS: Of the 2502 patient included, 4.82% (n = 107) had SP RAPN. Patients who underwent SP, were more likely to have hypertension, lower renal score and smaller tumor size. There was no significant difference in age, gender, tumor size, diabetes status, body mass index. In the bivariate analysis, compared to multi-port, SP had a lower operative time (155 minutes vs 117 minutes, p value <0.001), higher ischemia time (14.74 minutes vs. 15.08 minutes, p value = 0.069), lower EBL (100 ml vs 50 ml, p value >0.001) and lower complication rate (12.28% vs 3.74%, p value = 0.008). There was no difference in positive margin rate. In the multivariate analysis, operative time, EBL, length of stay, complication rate, remain significantly lower for the SP group while ischemia time was higher. Accounting for surgeon variability, there was no significant differences in operative time between SP and multi-port. CONCLUSIONS: The study presents the largest series of SP partial nephrectomy to date. SP partial nephrectomy presented similar operative time, lower EBL, lower complication rate and higher ischemia time when compared to multi-port. This early data is encouraging, however, the role of SP requires further study and should evaluate safety and long term data when compared to the standard multi-port technique.
