Technique Take plain films: front and side views of the suspected eye. injection of contrast mediums into Tenon's capsule, as described by Spackman,1has proved invaluable in the localization of intraocular foreign bodies. This is especially true of the type of injury incurred in jungle warfare, in which hand grenades, booby traps and small antipersonnel mines are used extensively. These usually cause multiple, small, penetrating wounds, containing opaque foreign bodies of varying sizes, which roentgenographically look like snow flurry collections of opaque debris. When the region of the eye is involved, it is frequently impossible to distinguish intraocular foreign bodies from those outside the eyeball by any procedure except roentgenographic study with injection of a contrast medium into Tenon's capsule (fig. 1). This method is also of considerable assistance in the localization of foreign bodies lying near the limits of the globe and in diagnosing double perforations of the eyeball. Comberg's roentgenographic method of localization of intraocular foreign bodies is extremely simple and accurate. It consists of (1) placing a contact lens containing four lead markers on the cornea and (2) then taking a lateral and a posteroanterior view with the patient in the chin-nose position. From these two views all measurements can be taken and plotted on a sectional and an anterior view of the globe. The film measurements have to be reduced by a correctional factor of 10 per cent, this small distortion being due to the distance between the object and the film. The Comberg contact lens has three inherent faults. It is occasionally pushed downward by the upper lid, so that the contact lens and the visual axis are not coaxial. Localization of intraocular foreign bodies is of prime importance not only for their successful extraction but for the preservation of the greatest amount of vision. Without knowing the exact location of a foreign body, one would be at a loss as to where to make an incision and in which direction to insert the point of the magnet or, if one is dealing with a nonmagnetic foreign body, where to search for it. In fact, a roentgenogram taken without a localizer is of little value because it cannot show even whether the foreign body is within the eyeball. In the past there has been excellent work done by localizers such as the one devised by the late Dr. Sweet. However, one of the drawbacks to their use has been their cost, which has prevented many a hospital and roentgenologist from owning one, especially considering the fact that it is not

PRINCIPLES OF ROENTGEN TECHNIC The localization of intra-ocular foreign bodies is a rather special department within the general field of roentgenology and involves many problems which are of great importance to both the ophthalmologist and the roentgenologist. The ophthalmologist is far too prone to accept without question the indicated position of a foreign body on a chart showing the localization which has been handed to him from the roentgenologic department. This chart represents as accurately as possible within the scope of the method used and the experience of the roentgenologist the position of the foreign body in relation to the ocular structures. There are, however, so many possible errors that it is to be highly recommended that the ophthalmologist thoroughly familiarize himself with the principles involved and the methods used, so that he may follow the procedure intelligently and not accept the data on the localization chart at face value.

The author is connected with a clinic that has studied particularly the effects and uses of radiant energy on the eyes, and has himself contributed to the literature on this subject. The book takes up the use of the x-rays for diagnosis and therapy in ophthalmology. In the first part the following subjects are considered : the localization of intra-ocular foreign bodies and diseases of the eye, orbit, optic nerve and lacrimal passages. Roentgen examinations play only a limited rôle in ophthalmology compared to that in other branches of medicine. Diseases of the eye are usually readily accessible to direct observation, especially if the slit-lamp is used, while the conditions in the orbit surrounded by bone are recognized only with difficulty. With a skilled radiologist, a steady patient and a foreign body large enough to cast a shadow, geometric methods of localization are satisfactory. But if the eyes move during exposure the radiogram may give a false position for the foreign body. Particles of metal fastened to the eyeball move with it, preventing a false impression. <h2>Abstract</h2> The character of the accident causing the injury throws important light on the case. The greater number of magnetic foreign bodies come from use of hammers, especially in attempting to remove hoops from barrels or boxes. A broken needle rarely remains in the eye. Glass comes from an explosion. It can be recognized by the X-ray, if of low penetrating power. Cases were cited in which the source of the foreign body could not be ascertained. In some cases of cataract an unsuspected foreign body was present.
