Kyrle's Disease

Histologic findings in five cases of Kyrle's disease are described. Diagnostic features are: (1) A keratotic plug filling an epithelial invagination; (2) parakeratosis in parts of the plug; (3) basophilic cellular debris (not staining with elastic tissue stains) within the plug; and (4) abnormal (parakeratotic) keratinization involving the entire thickness of the epidermis in at least one region of the plug resulting in contact between keratinized cells and dermis. Associated findings are: (1) Areas of acanthosis and thinning of epidermis surrounding the plug; (2) presence of varying numbers of neutrophils at the site of contact between keratinized cells and dermis; (3) a lymphocytic and histiocytic infiltrate about the lesion; and (4) a mild perivascular lymphocytic inflammatory infiltrate in the upper dermis.

HYPERKERATOSIS PENETRANS (KYRLE'S DISEASE).

The second case of hyperkeratosis follicularis et parafollicularis in cutem penetrans (Kyrle's disease) is reported. The nature of the keratin mass in the dermis is shown by the dihydroxyl-dinaphthyl disulfide stain for sulfhydryl groups which is positive in the case presented.

Elastosis perforans serpiginosa. Report of another case.

This is indeed a strange disease of the skin. Is it really a new condition, or is it an old disease which has only recently been recognized? All but one case is reported as occurring since 1950. If elastosis perforans is increasing in incidence, how can we explain such an unusual condition? Many questions arise as we study this disease, which clinically appears as arciform, nonpruritic papules, not interfering with hair growth, located mainly on the posterior aspect of the neck in young persons. The lesions are remarkably recalcitrant to almost every known form of destructive therapy. The disease has been misdiagnosed as lichen planus, granuloma annulare, Hailey and Hailey's disease, follicular lichen planus, porokeratosis, Lutz's disease, Lutz-Miescher disease, and Kyrle's disease. <b>Kyrle's Disease: Hyperkeratosis Follicularis et Parafollicularis in Cutem Penetrans</b>. Presented byDr. Joseph Gropen. A 23-year-old Caucasian woman states that she noticed "bumps" appear on her legs and thighs when she was 9 years of age. Since that time more lesions gradually appeared, chiefly on the extensor surface of the legs and thighs and on the back. Within the past year lesions also appeared on the arms. They are asymptomatic and show no tendency towards spontaneous regression. The past history is noncontributory. No familial history of similar condition could be elicited. The single lesion represents a firm, slightly erythematous crateriform papule containing a central plug. The papules vary in size from that of a pinhead to a pea. There was no tendency to grouping.
