Purpose: The purpose of the study was to compare a standard diagnostic protocol for computed tomography imaging on a positron emission tomography scanner at the Department of Nuclear Medicine, and a radiotherapy imaging protocol for pre-planning needs in radiotherapy for lung cancer treatment, to determine the differences between these two protocols and to propose possible improvements in the dose optimisation for computed tomography imaging in a radiotherapy protocol. Methods: In this retrospective study, data were collected via the SyngoVia program and statistically analysed according to the patient dose load in computed tomography imaging in standard and radiotherapy protocols. The analysis encompassed data on a total of 56 patients in the period from 1 January 2017 to 1 December 2018. We compared data on patient dose load in computed tomography imaging in a standard protocol before and after the introduction of the improved sinogram-affirmed iterative reconstruction method (SAFIRE). Results and discussion: It was established that there are statistically significant differences in dose per patient (p<) in computed tomography imaging in standard and radiotherapy protocols. Statistically significant differences were also established in computed tomography imaging in the standard protocol before and after the introduction of the improved iterative reconstruction method (p=0,001). Dose load on the lung in computed tomography imaging was 67.5% lower in the standard protocol with the iterative reconstruction in image space (IRIS) method than in the radiotherapy protocol. The introduction of the improved SAFIRE method additionally lowered the dose per patient by 34.2% compared to the IRIS method. Conclusion: In the future, the introduction of the improved iterative reconstruction method is possible for the reconstruction of tomographic images, including for radiotherapy imaging protocol that takes into account the impact of the indirect reduction in the dose on the accuracy of the identification of tumour target volumes when planning radiation treatment for the patient. Evaluation of Software-Based Metal Artifact Reduction in Intraoperative 3D Imaging of the Spine Using a Mobile Cone Beam CT.

The aim of our study was to evaluate whether software-based artifact reduction can achieve an improved image quality, using intraoperative 3D imaging in spinal surgery. A total of 49 intraoperative 3D image datasets of patients, who underwent surgery with pedicle screw placement, were retrospectively evaluated. The visibility of anatomical structures and the diameter of the pedicle screws were examined, with and without the application of the artifact reduction software. All software prototypes can improve the visibility of anatomical structures (P < 0.01), except MAR (metal artifact reduction) combined with IRIS (iterative reconstruction in image space) (P = 0.04). The algorithms MAR and MAR-2 can reduce the blooming artifacts significantly (P < 0.01), but SL (Shepp & Logan) cannot (P = 0.08-0.988). In summary, software-based artifact reduction for intraoperative 3D datasets can improve the current image quality. Additional information regarding the implant placement and the fracture reduction is therefore generated for the surgeon.

Artificial intelligence machine learning-based coronary CT fractional flow reserve (CT-FFRML): Impact of iterative and filtered back projection reconstruction techniques

BackgroundThe influence of computed tomography (CT) reconstruction algorithms on the performance of machine-learning-based CT-derived fractional flow reserve (CT-FFRML) has not been investigated. CT-FFRML values and processing time of two reconstruction algorithms were compared using an on-site workstation. MethodsCT-FFRML was computed on 40 coronary CT angiography (CCTA) datasets that were reconstructed with both iterative reconstruction in image space (IRIS) and filtered back-projection (FBP) algorithms. CT-FFRML was computed on a per-vessel and per-segment basis as well as distal to lesions with ≥50% stenosis on CCTA. Processing times were recorded. Significant flow-limiting stenosis was defined as invasive FFR and CT-FFRML values ≤ 0.80. Pearson's correlation, Wilcoxon, and McNemar statistical testing were used for data analysis. ResultsPer-vessel analysis of IRIS and FBP reconstructions demonstrated significantly different CT-FFRML values (p ≤ 0.05). Correlation of CT-FFRML values between algorithms was high for the left main (r = 0.74), left anterior descending (r = 0.76), and right coronary (r = 0.70) arteries. Proximal and middle segments showed a high correlation of CT-FFRML values (r = 0.73 and r = 0.67, p ≤ 0.001, respectively), despite having significantly different averages (p ≤ 0.05). No difference in diagnostic accuracy was observed (both 81.8%, p = 1.000). Of the 40 patients, 10 had invasive FFR results. Per-lesion correlation with invasive FFR values was moderate for IRIS (r = 0.53, p = 0.117) and FBP (r = 0.49, p = 0.142). Processing time was significantly shorter using IRIS (15.9 vs. 19.8 min, p ≤ 0.05). ConclusionCT reconstruction algorithms influence CT-FFRML analysis, potentially affecting patient management. Additionally, iterative reconstruction improves CT-FFRML post-processing speed.

High fidelity system modeling for high quality image reconstruction in clinical CT.

Today, while many researchers focus on the improvement of the regularization term in IR algorithms, they pay less concern to the improvement of the fidelity term. In this paper, we hypothesize that improving the fidelity term will further improve IR image quality in low-dose scanning, which typically causes more noise. The purpose of this paper is to systematically test and examine the role of high-fidelity system models using raw data in the performance of iterative image reconstruction approach minimizing energy functional. We first isolated the fidelity term and analyzed the importance of using focal spot area modeling, flying focal spot location modeling, and active detector area modeling as opposed to just flying focal spot motion. We then compared images using different permutations of all three factors. Next, we tested the ability of the fidelity terms to retain signals upon application of the regularization term with all three factors. We then compared the differences between images generated by the proposed method and Filtered-Back-Projection. Lastly, we compared images of low-dose in vivo data using Filtered-Back-Projection, Iterative Reconstruction in Image Space, and the proposed method using raw data. The initial comparison of difference maps of images constructed showed that the focal spot area model and the active detector area model also have significant impacts on the quality of images produced. Upon application of the regularization term, images generated using all three factors were able to substantially decrease model mismatch error, artifacts, and noise. When the images generated by the proposed method were tested, conspicuity greatly increased, noise standard deviation decreased by 90% in homogeneous regions, and resolution also greatly improved. In conclusion, the improvement of the fidelity term to model clinical scanners is essential to generating higher quality images in low-dose imaging.

Computed Tomography Coronary Stent Imaging With Iterative Reconstruction

To evaluate the improvement of iterative reconstruction in image space (IRIS) technique in computed tomographic (CT) coronary stent imaging with sharp kernel, and to make a trade-off analysis. Fifty-six patients with 105 stents were examined by 128-slice dual-source CT coronary angiography (CTCA). Images were reconstructed using standard filtered back projection (FBP) and IRIS with both medium kernel and sharp kernel applied. Image noise and the stent diameter were investigated. Image noise was measured both in background vessel and in-stent lumen as objective image evaluation. Image noise score and stent score were performed as subjective image evaluation. The CTCA images reconstructed with IRIS were associated with significant noise reduction compared to that of CTCA images reconstructed using FBP technique in both of background vessel and in-stent lumen (the background noise decreased by approximately 25.4% ± 8.2% in medium kernel (P <C; 0.0001) and 30.3% ± 3.4% in sharp kernel (P <C; 0.0001). The in-stent lumen noise decreased by approximately 14.2% ± 19.2% in medium kernel (P <C; 0.0001) and 27.0% ± 17.8% in sharp kernel (P <C; 0.0001)). Subjective image assessment showed that the noise of the images reconstructed with IRIS decreased compared to that with FBP. Moreover, the images with sharp kernel showed better visualization of the stent struts and in-stent lumen than that with medium kernel. Iterative reconstruction in image space reconstruction can effectively reduce the image noise and improve image quality. The sharp kernel images constructed with iterative reconstruction are considered the optimal images to observe coronary stents in this study.

Comparison of the Effect of Iterative Reconstruction versus Filtered Back Projection on Cardiac CT Postprocessing

To investigate the impact of iterative reconstruction in image space (IRIS) on image noise, image quality (IQ), and postprocessing at coronary computed tomography angiography (cCTA) compared to traditional filtered back-projection (FBP). The cCTA results of 50 patients (26 men; 58 ± 15 years, body mass index 31.5 ± 6.7 kg/m²) were investigated using a second-generation dual-source computed tomography system. Scan data were reconstructed with the use of IRIS and FBP algorithms. Two radiologists independently evaluated the reconstructions using automated coronary tree analysis software. Image noise was measured and IQ was rated on a 5-point Likert scale. The number of manual corrections after automated vessel segmentation, the time required to complete segmentation, and the number of missed segments were assessed in both IRIS and FBP reconstructions. Results were compared using paired t-test. IRIS significantly reduced image noise compared to FBP (23.3 ± 8.8 vs. 33.5 ± 13.5 Hounsfield units, P < .001). Subjective IQ improved with IRIS (IRIS 3.2 ± 1.0 vs. FBP 3.0 ± 1.0, P < .05). IRIS decreased the time needed for coronary segmentation from 111.9 ± 40.5 seconds to 95.2 ± 38.2 seconds with FBP (P < .01) and required fewer manual corrections (5.7 ± 3.0 vs. 6.8 ± 3.6, P < .01). The number of missed vessel segments was not significantly different (3.6 ± 1.8 vs. 3.8 ± 1.9, P > .05) between IRIS and FBP, respectively. During cCTA postprocessing, IRIS significantly decreases the time and the number of manual corrections for a complete coronary segmentation compared to FBP. This effect is likely attributable to suppression of image noise by IRIS, which improves the performance of automated vessel segmentation and positively impacts cCTA analysis.

Iterative Image Reconstruction Techniques for CT Coronary Artery Calcium Quantification: Comparison with Traditional Filtered Back Projection in Vitro and in Vivo

To investigate in vitro and in vivo the use of image-based and raw data-based iterative reconstruction algorithms for quantification of coronary artery calcium by using the Agatston score and subsequent cardiac risk stratification. In vitro data were obtained by using a moving anthropomorphic cardiac phantom containing calcium inserts of different concentrations and sizes. With institutional review board approval and HIPAA compliance, coronary calcium imaging data of 110 consecutive patients (mean age ± standard deviation, 58.2 years ± 9.8; 48 men) were reconstructed with filtered back projection (FBP), iterative reconstruction in image space (IRIS), and sinogram-affirmed iterative reconstruction (SAFIRE). Image noise was measured and the Agatston score was obtained for all reconstructions. Assignment to Agatston scores and percentile-based cardiac risk categories was compared. Statistical analysis included the Cohen κ coefficient and Friedman and Wilcoxon testing. In vitro, mean Agatston scores ± standard deviation for FBP (638.9 ± 9.6), IRIS (622.7 ± 15.2), and SAFIRE (631.4 ± 17.6) were comparable (P = .30). The smallest phantom calcifications were more frequently detected when iterative reconstruction techniques were used. The Agatston scores in the patient cohort were not significantly different among FBP, IRIS, and SAFIRE in paired comparisons (median Agatston score [25th and 75th percentiles]: 76.0 [20.6, 243.9], 76.4 [22, 249.3], and 75.7 [21.5, 49.1], respectively; P = .20 each). Comparison of categorization based on Agatston score percentiles showed excellent agreement for both IRIS and SAFIRE with FBP (κ = 0.975 [0.942-1.00] and κ = 0.963 [0.922-1.00], respectively). The mean effective dose was 1.02 mSv ± 0.51. Mean image noise was significantly (P < .001) higher with FBP than that with iterative reconstructions. In comparison with FBP, iterative reconstruction techniques do not have a profound effect on the reproducible quantification of coronary calcium according to Agatston score and subsequent cardiac risk classification, although risk reclassification may occur in a small subset of subjects.

Reduction of effective dose and organ dose to the eye lens in head MDCT using iterative image reconstruction and automatic tube current modulation

To compare the effective and eye lens radiation dose in helical MDCT brain examinations using automatic tube current modulation in conjunction with either standard filtered back projection (FBP) technique or iterative reconstruction in image space (IRIS). Of 400 adult brain MDCT examinations, 200 were performed using FBP and 200 using IRIS with the following parameters: tube voltage 120 kV, rotation period 1 second, pitch factor 0.55, automatic tube current modulation in both transverse and longitudinal planes with reference mAs 300 (FBP) and 200 (IRIS). Doses were calculated from CT dose index and dose length product values utilising ImPACT software; the organ dose to the lens was derived from the actual tube current-time product value applied to the lens. Image quality was assessed by two independent readers blinded to the type of image reconstruction technique. The average effective scan dose was 1.47±0.26 mSv (FBP) and 0.98±0.15 mSv (IRIS), respectively (33.3% decrease). The average organ dose to the eye lens decreased from 40.0±3.3 mGy (FBP) to 26.6±2.0 mGy (IRIS, 33.5% decrease). No significant change in diagnostic image quality was noted between IRIS and FBP scans (P=0.17). Iterative reconstruction of cerebral MDCT examinations enables reduction of both effective and organ eye lens dose by one third without signficant loss of image quality.

Iterative reconstruction of pulmonary MDCT angiography: Effects on image quality, effective dose and estimated organ dose to the breast

To compare the image characteristics, effective dose and estimated organ dose to the female breast in pulmonary MDCT angiography (MDCTA), reconstructed with either standard filtered back projection (FBP), or iterative reconstruction in image space (IRIS). Pulmonary MDCTA performed in 116 females (age 18 - 77 years; body mass index 15 - 48) was reconstructed with FBP (n=52) or IRIS (n=64). Scans were acquired on a 128-row MDCT system using automatic tube current modulation, 100 kV tube voltage, and a quality reference mAs value of 120 (FBP) and 80 (IRIS). Dose was calculated from CT dose index (CTDIvol) and dose length product (DLP) values utilising ImPACT software. Image noise was measured within the pulmonary artery. Qualitative visual assessment of the scans was performed (1=negligible noise, 5=noise obscuring diagnostic information). The average CTDIvol yielded 4.33 mGy for FBP and 3.54 mGy for IRIS, respectively (18.2% decrease). The average effective scan dose was 2.73±0.57 mSv (FBP) and 2.29±0.68 mSv (IRIS), respectively (16.1% decrease). The estimated average organ dose to the breast decreased from 5.1±1.1 mGy (FBP) to 4.2±1.2 mGy (IRIS, 17.6% decrease). No non-diagnostic scans (score 5) were encountered in either group. Significant improvement in image noise levels (P<0.01) and subjective image quality (P<0.02) were noted in IRIS group. Pulmonary MDCTA utilizing a 100 kV technique, automatic tube current modulation, and iterative image reconstruction offers robust results in routine conditions among an unselected female population, with breast doses being comparable to two-view digital mammography. Moreover, iterative reconstruction offers improvements in both image noise and visual perception of the scans, thus suggesting a potential for further dose reduction.

CT of Urolithiasis: Comparison of Image Quality and Diagnostic Confidence Using Filtered Back Projection and Iterative Reconstruction Techniques

To compare the image quality and diagnostic confidence of low-dose computed tomography (CT) of urololithiasis using filtered back projection (FBP) and iterative reconstruction techniques (IRT).A 4.8 × 4.3 × 5.2 mm(3) uric acid ureteral stone was placed inside an anthropomorphic Alderson phantom at the pelvic level. Fifteen scans were performed on a 64-row dual-source CT system using different tube voltages (80, 100, and 120 kV) and current-time products (8, 15, 30, 70, and 100 mAs). Image reconstruction using FBP and IRT (iterative reconstruction in image space) resulted in 30 data sets. Objective image quality was evaluated by noise measurements. Effective doses were estimated for each data set with use of an established dosimetry program. Subjective image quality and confidence level were rated by two radiologists.Noise was systematically lower for images reconstructed with IRT compared to FBP (55 ± 30 vs 65 ± 26 Hounsfield units; P = .004) for volume CT dose index values above about 0.6 mGy (or an effective dose of about 0.4 mSv for both sexes). For the 14 scans rated to have diagnostic image quality, the estimated effective doses ranged from 0.3 to 2.5 mSv for males and from 0.4 to 3.1 mSv for females. Subjective image quality and diagnostic confidence for IRT was not significantly better than those for FBP.In a phantom study for CT of urolithiasis, IRT improves objective image quality compared to FBP above a certain dose threshold. However, this does not translate into improved subjective image quality or a higher degree of confidence for the diagnosis of high-contrast urinary stones.

Low Tube Voltage Intermediate Tube Current Liver MDCT: Sinogram-Affirmed Iterative Reconstruction Algorithm for Detection of Hypervascular Hepatocellular Carcinoma

The purpose of this study was to compare image quality and lesion detectability in the evaluation of hypervascular hepatocellular carcinoma (HCC) on low-tube-voltage half-dose liver CT scans subjected to sinogram-affirmed iterative reconstruction (SAFIRE) with the quality and detectability on full-dose scans reconstructed with filtered back projection (FBP). A total of 126 patients with suspected HCC who underwent liver CT including arterial phase scanning at 80 kVp in the dual-source mode (300 mAs for each tube) were included in the study. The half-dose arterial scans were reconstructed with FBP, iterative reconstruction in image space (IRIS), and five SAFIRE strengths (S1-S5) and were compared with full-dose virtual scans (600 mA) reconstructed with FBP. We assessed image noise, contrast-to-noise ratio (CNR) of the liver and blood vessels, and lesionto-liver CNR. Two radiologists evaluated image quality and lesion detectability attained with the different imaging sets. Image noise on SAFIRE images was significantly lower than that on the other images, and the CNRs on SAFIRE images were higher than those on half-dose FBP images (p < 0.001). In addition, lesion-to-liver CNR on the half-dose S5 SAFIRE images was higher than on IRIS and full-dose FBP images (p < 0.05). Among the half-dose scans, SAFIRE images had significantly better image quality than FBP images (p < 0.05). Regarding lesion detection, half-dose SAFIRE images were better than half-dose FBP images and were comparable with full-dose FBP images (observer 1, 91.8% vs 96%; observer 2, 98% vs 98%; p > 0.05). Performing half-dose 80-kVp liver CT with SAFIRE technique may increase image quality and afford comparable lesion detectability of hypervascular HCC at a reduced radiation dose compared with full-dose CT with FBP.

Low-Dose Chest Computed Tomography With Sinogram-Affirmed Iterative Reconstruction, Iterative Reconstruction in Image Space, and Filtered Back Projection

This study aimed to determine optimal strength of sinogram-affirmed iterative reconstruction (SAFIRE) and to evaluate image quality (IQ) of low-dose chest computed tomography (LDCT) using SAFIRE compared with iterative reconstruction in image space (IRIS) and filtered back projection (FBP). Thirty patients underwent LDCT. Computed tomography (CT) was reconstructed using 5 strengths of SAFIRE (S1-S5), IRIS, and FBP. Objective noise of CT was measured. Two radiologists evaluated CT for subjective IQ, beam-hardening artifacts, and overall IQ. Measured noise was highest in FBP, followed by S1, S2, S3, IRIS, S4, and S5. S2 and S3 demonstrated significantly higher overall IQ scores than the other strengths (P < 0.05). Overall IQ and beam-hardening artifacts of S2 and S3 were significantly better than those in FBP and IRIS (P < 0.001). S2 or S3 strengths of SAFIRE can be used practically in clinical routines and may have more potential than IRIS and FBP for LDCT with improved IQ.

Comparative assessment of three image reconstruction techniques for image quality and radiation dose in patients undergoing abdominopelvic multidetector CT examinations

To compare image quality and radiation dose of abdominal CT examinations reconstructed with three image reconstruction techniques. In this Institutional Review Board-approved study, contrast-enhanced (CE) abdominopelvic CT scans from 23 patients were reconstructed using filtered back projection (FBP), adaptive statistical iterative reconstruction (ASiR) and iterative reconstruction in image space (IRIS) and were reviewed by two blinded readers. Subjective (acceptability, sharpness, noise and artefacts) and objective (noise) measures of image quality were recorded for each image data set. Radiation doses in CT dose index (CTDI) dose-length product were also calculated for each examination type and compared. Imaging parameters were compared using the Wilcoxon signed rank test and a paired t-test. All 69 CECT examinations were of diagnostic quality and similar for overall acceptability (mean grade for ASiR, 3.9±0.3; p=0.2 for Readers 1 and 2; IRIS, 3.9±0.4, p=0.2; FBP, 3.8±0.9). Objective noise was considerably lower with both iterative techniques (p<0.0001 and 0.0016 for ASiR and IRIS). Recorded mean radiation dose, i.e. CTDI(vol), was 24% and 10% less with ASiR (11.4±3.4 mGy; p<0.001) and IRIS (13.5±3.7 mGy; p=0.06), respectively, than with FBP: 15.0±3.5 mGy. At the system parameters used in this study, abdominal CT scans reconstructed with ASiR and IRIS provide diagnostic images with reduced image noise and 10-24% lower radiation dose than FBP. CT images reconstructed with FBP are frequently noisy on lowering the radiation dose. Newer iterative reconstruction techniques have different approaches to produce images with less noise; ASiR and IRIS provide diagnostic abdominal CT images with reduced image noise and radiation dose compared with FBP. This has been documented in this study.

Cardiovascular CT angiography in neonates and children: Image quality and potential for radiation dose reduction with iterative image reconstruction techniques

To evaluate image quality (IQ) of low-radiation-dose paediatric cardiovascular CT angiography (CTA), comparing iterative reconstruction in image space (IRIS) and sinogram-affirmed iterative reconstruction (SAFIRE) with filtered back-projection (FBP) and estimate the potential for further dose reductions. Forty neonates and children underwent low radiation CTA with or without ECG synchronisation. Data were reconstructed with FBP, IRIS and SAFIRE. For ECG-synchronised studies, half-dose image acquisitions were simulated. Signal noise was measured and IQ graded. Effective dose (ED) was estimated. Mean absolute and relative image noise with IRIS and full-dose SAFIRE was lower than with FBP (P < 0.001), while SNR and CNR were higher (P < 0.001). Image noise was also lower and SNR and CNR higher in half-dose SAFIRE studies compared with full-and half-dose FBP studies (P < 0.001). IQ scores were higher for IRIS, full-dose SAFIRE and half-dose SAFIRE than for full-dose FBP and higher for half-dose SAFIRE than for half-dose FBP (P < 0.05). Median weight-specific ED was 0.3 mSv without and 1.36 mSv with ECG synchronisation. The estimated ED of half-dose SAFIRE studies was 0.68 mSv. IR improves image noise, SNR, CNR and subjective IQ compared with FBP in low-radiation-dose paediatric CTA and allows further dose reductions without compromising diagnostic IQ. • Iterative reconstruction techniques significantly improve non-invasive cardiovascular CT in children. • Using half traditional radiation dose image quality is higher with iterative reconstruction. • Iterative reconstruction techniques may allow further radiation reductions in paediatric cardiovascular CT.

Improved Image Quality in Dual-Energy Abdominal CT: Comparison of Iterative Reconstruction in Image Space and Filtered Back Projection Reconstruction

The purpose of this study is to investigate whether an iterative reconstruction in image space (IRIS) algorithm improves the image quality of dual-energy CT abdominal examinations performed during the hepatic arterial phase. Seventy patients with suspected liver masses underwent contrast-enhanced multiphase abdominal examination and were enrolled in the study. A dual-energy CT protocol was performed in the hepatic arterial phase (parameters: tube A, 140 kV and 90 mA; tube B, 80 kV and 382 mA; automatic tube current modulation on; and collimation, 14 × 1.2 mm). The reconstructions were performed with filtered back projection (FBP) and IRIS algorithms at a slice thickness of 3 mm and kernels of B30 and I30. The image noise was measured on the liver, aorta, and subcutaneous fat on the FBP and IRIS fusion images (m = 0.3) at the same position. The image noise and diagnostic acceptability of all images were scored by two radiologists. The image noise using the IRIS algorithm was lower than that using the standard FBP algorithm on the liver, aorta, and subcutaneous fat, respectively. The signal-to-noise ratio and contrast-to-noise ratio of images reconstructed with the IRIS algorithm also were significantly higher than for those reconstructed with the FBP algorithm. The diagnostic acceptability score using the IRIS algorithm was higher than that using the FBP algorithm at the same dose level (1.20 ± 0.40 vs 1.37 ± 0.57; p < 0.05). Compared with standard FBP reconstruction, an IRIS algorithm enables significant reduction of image noise and improvement of image quality and has the potential to decrease radiation exposure during contrast-enhanced dual-energy CT abdominal examination.

Radiation Exposure and Image Quality of Normal Computed Tomography Brain Images Acquired With Automated and Organ-Based Tube Current Modulation Multiband Filtering and Iterative Reconstruction

We sought to determine whether radiation dose can be reduced and image quality improved in computed tomography (CT) images of the brain that were acquired with automated exposure control (AEC), organ-based tube current modulation (TCM), multiband filtration (MBF), and iterative reconstruction in image space (IRIS). An Alderson-Rando-phantom, equipped with thermoluminescent dosimeters, was used to determine the radiation exposure of organs within the head and neck by different CT brain scan modes. We measured the noise and signal-to-noise ratios and subjectively graded quality criteria in different territories of the brain in spiral CT images of 150 patients. We also derived the radiation exposure from the patient protocols. In the phantom, AEC and TCM reduced the radiation exposure of the lenses, cerebrum, cerebellum, and thyroid gland by 41.9%, 34.5%, 30.5%, and 34.9%, respectively. Brain CT scans from patients investigated with AEC, TCM, MBF, and IRIS were found to have significantly better image quality than with conventional filtered back projection. In addition, the CT dose index and dose-length product were significantly lower with AEC, TCM, MBF, and IRIS by 24.1% and 20.2%, respectively. The combination of AEC, TCM, MBF, and IRIS improves image quality while radiation exposure can be reduced, particularly in dose-sensitive organs, such as the lenses and thyroid gland.

Radiation Dose Reduction of Chest CT with Iterative Reconstruction in Image Space - Part I: Studies on Image Quality Using Dual Source CT

ObjectiveTo determine whether the image quality (IQ) is improved with iterative reconstruction in image space (IRIS), and whether IRIS can be used for radiation reduction in chest CT.Materials and MethodsStandard dose chest CT (SDCT) in 50 patients and low dose chest CT (LDCT) in another 50 patients were performed, using a dual-source CT, with 120 kVp and same reference mAs (50 mAs for SDCT and 25 mAs for LDCT) employed to both tubes by modifying a dual-energy scan mode. Full-dose data were obtained by combining the data from both tubes and half-dose data were separated from a single tube. These were reconstructed by using a filtered back projection (FBP) and IRIS: full-dose FBP (F-FBP); full-dose IRIS (F-IRIS); half-dose FBP (H-FBP) and half-dose IRIS (H-IRIS). Objective noise was measured. The subjective IQ was evaluated by radiologists for the followings: noise, contrast and sharpness of mediastinum and lung.ResultsObjective noise was significantly lower in H-IRIS than in F-FBP (p < 0.01). In both SDCT and LDCT, the IQ scores were highest in F-IRIS, followed by F-FBP, H-IRIS and H-FBP, except those for sharpness of mediastinum, which tended to be higher in FBP. When comparing CT images between the same dose and different reconstruction (F-IRIS/F-FBP and H-IRIS/H-FBP) algorithms, scores tended to be higher in IRIS than in FBP, being more distinct in half-dose images. However, despite the use of IRIS, the scores were lower in H-IRIS than in F-FBP.ConclusionIRIS generally helps improve the IQ, being more distinct at the reduced radiation. However, reduced radiation by half results in IQ decrease even when using IRIS in chest CT.

Radiation Dose Reduction of Chest CT with Iterative Reconstruction in Image Space - Part II: Assessment of Radiologists' Preferences Using Dual Source CT

ObjectiveTo evaluate the impact of radiation dose and reconstruction algorithms on radiologists' preferences, and whether an iterative reconstruction in image space (IRIS) can be used for dose reduction in chest CT.Materials and MethodsStandard dose chest CT (SDCT) in 50 patients and low dose chest CT (LDCT) in another 50 patients were performed, using a dual-source CT, with 120 kVp and same reference mAs (50 mAs for SDCT and 25 mAs for LDCT) employed to both tubes by modifying the dual-energy scan mode. Full-dose data were obtained by combining the data from both tubes and half-dose data were separated from one tube. These were reconstructed by using a filtered back projection (FBP) and IRIS: full-dose FBP (F-FBP); full-dose IRIS (F-IRIS); half-dose FBP (H-FBP) and half-dose IRIS (H-IRIS). Ten H-IRIS/F-IRIS, 10 H-FBP/H-IRIS, 40 F-FBP/F-IRIS and 40 F-FBP/H-IRIS pairs of each SDCT and LDCT were randomized. The preference for clinical usage was determined by two radiologists with a 5-point-scale system for the followings: noise, contrast, and sharpness of mediastinum and lung.ResultsRadiologists preferred IRIS over FBP images in the same radiation dose for the evaluation of the lungs in both SDCT (p = 0.035) and LDCT (p < 0.001). When comparing between H-IRIS and F-IRIS, decreased radiation resulted in decreased preference. Observers preferred H-IRIS over F-FBP for the lungs in both SDCT and LDCT, even with reduced radiation dose by half in IRIS image (p < 0.05).ConclusionRadiologists' preference may be influenced by both radiation dose and reconstruction algorithm. According to our preliminary results, dose reduction at 50% with IRIS may be feasible for lung parenchymal evaluation.

Image Quality Evaluation of New Image Reconstruction Methods Applying the Iterative Reconstruction

The purpose of this study was to evaluate the image quality of an iterative reconstruction method, the iterative reconstruction in image space (IRIS), which was implemented in a 128-slices multi-detector computed tomography system (MDCT), Siemens Somatom Definition Flash (Definition). We evaluated image noise by standard deviation (SD) as many researchers did before, and in addition, we measured modulation transfer function (MTF), noise power spectrum (NPS), and perceptual low-contrast detectability using a water phantom including a low-contrast object with a 10 Hounsfield unit (HU) contrast, to evaluate whether the noise reduction of IRIS was effective. The SD and NPS were measured from the images of a water phantom. The MTF was measured from images of a thin metal wire and a bar pattern phantom with the bar contrast of 125 HU. The NPS of IRIS was lower than that of filtered back projection (FBP) at middle and high frequency regions. The SD values were reduced by 21%. The MTF of IRIS and FBP measured by the wire phantom coincided precisely. However, for the bar pattern phantom, the MTF values of IRIS at 0.625 and 0.833 cycle/mm were lower than those of FBP. Despite the reduction of the SD and the NPS, the low-contrast detectability study indicated no significant difference between IRIS and FBP. From these results, it was demonstrated that IRIS had the noise reduction performance with exact preservation for high contrast resolution and slight degradation of middle contrast resolution, and could slightly improve the low contrast detectability but with no significance.

Iterative reconstruction of dual-source coronary CT angiography: assessment of image quality and radiation dose

To assess the image quality and radiation dose of low-dose dual-source CT (DSCT) coronary angiography reconstructed using iterative reconstruction in image space (IRIS), in comparison with routine-dose CT using filtered back projection (FBP). Eighty-one patients underwent low-dose coronary DSCT using IRIS with two protocols: (a)100 kVp and 200 mAs per rotation for body mass index (BMI) < 25 (group I), (b)100 kVp and 320 mAs for BMI ≥ 25 (II). For comparison, two sex-and BMI-matched groups using standard protocols with FBP were selected: (a)100 kVp and 320 mAs for BMI < 25 (III), (b)120 kVp and 320 mAs for BMI ≥ 25 (IV). Image noise, signal to noise ratio (SNR) and modulation transfer function (MTF) 50% were objectively calculated. Two blinded readers then subjectively graded the image quality. Radiation dose was also measured. Image noise tended to be lower in IRIS of low-dose protocols: 22.0 ± 4.5 for group I versus 24.8 ± 4.0 for III (P < 0.001); 20.9 ± 4.5 for II versus 21.6 ± 4.9 for IV (P = 0.6). SNR was better with IRIS: 25.8 ± 4.4 for I versus 22.7 ± 4.6 for III (P < 0.001); 24.6 ± 5.4 for II versus 18.7 ± 4.5 for IV (P < 0.001). No differences in MTF 50% or image quality scores were seen between each two groups (P > 0.05). Radiation reduction was 40% for I and 51% for II, compared to standard protocols. Compared with routine-dose CT using FBP, low-dose coronary angiography using IRIS provides significant radiation reduction without impairment to image quality.
