Incidence of rheumatoid arthritis from 1995 to 2001: impact of ascertainment from multiple sources

The aim of this study was to describe the mean incidence rate of rheumatoid arthritis over a 7-year period from 1995 to 2001 in a population in the southern part of Denmark, using the data from several sources. Cases fulfilling the 1987 American College of Rheumatology criteria for rheumatoid arthritis were identified at hospitals and private practising rheumatologists (referral centres), and in general practice. The observed incidence was 32/100,000 person-years (95% confidence interval 29-35). Using the ratio between the number of cases known only from general practice and the number known from general practice and referral centres, the estimated incidence was 35/100,000 person-years (95% confidence interval 32-38). We suggest that the estimated rate should be viewed as a plausible upper limit for the incidence of rheumatoid arthritis in the southern part of Denmark.

Genetics and clinical characteristics to predict rheumatoid arthritis: where are we now and what are the future prospects?

In medicine, predicting disease development involves three major factors: the variability of the host, the characteristics of the disease-causing agent and the interactions among these factors (i.e., the disease process itself). This review focuses on the prediction of rheumatoid arthritis (RA) development in patients with undifferentiated arthritis (UA). Data from inception cohorts have revealed that, in approximately a third of patients presenting to a rheumatologist with recent onset arthritis, no diagnosis can be made, resulting in so-called UA. Although RA develops in a proportion of these patients, a substantial proportion spontaneously remits. The reason(s) for RA development as opposed to remission are as yet unknown. However, current epidemiological data on RA incidence rates are similar, with host (genetic) factors being an independent predictor of RA susceptibility. Low incidence of rheumatoid arthritis in France.

The incidence of rheumatoid arthritis (RA) was estimated in the Lorraine district, eastern France, over the period 1986-1989. Cases were identified from in- and outpatients at the district Rheumatology Hospital Centre, private rheumatologists, general practitioners and by regional media announcements. Supplementary cases resident in the area were sought by extending the procedure to adjacent districts. The population count was obtained from the national Census. An overall age-adjusted incidence rate of 8.8/100,000 was found using the 1987 ACR criteria (12.7/100,000 in females, 4.7/100,000 in males; sex ratio = 2.65) and 9.5/100,000 using the 1958 ARA criteria (definite and classic RA). These rates were compared, using the same standard population, with the age-adjusted incidence rates in other incidence studies mainly conducted in Caucasian populations. The RA incidence rates appeared lower in France than in any other country in Europe, USA and Japan.

The brief scale for anxiety: a subdivision of the comprehensive psychopathological rating scale.

A rating scale suitable for recording anxious symptoms is described. It is a subdivision of the Comprehensive Psychopathological Rating Scale and comprises 10 items, all of which are rated on a 7 point scale. It is suitable for the rating of pathological anxiety alone or for anxiety occurring in the setting of other psychological or medical disorder.

ORAL CONTRACEPTIVES AND RHEUMATOID ARTHRITIS: FURTHER EVIDENCE FOR A PREVENTIVE EFFECT

To investigate a reported negative association between the use of oral contraceptives (OC) and the development of rheumatoid arthritis, a case-control study was undertaken to compare the histories of OC use between 228 women with a diagnosis of probable or definite rheumatoid arthritis and 302 women with the diagnosis of soft-tissue rheumatism and/or osteoarthritis. The use of OCs before the onset of joint complaints was acknowledged by 31·1% of the rheumatoid arthritis patients and by 55·6% of the controls. After adjustment for possible confounding variables, the rate ratio for ever use became 0·42 (95% confidence interval 0·27-0·65), while it was 0·40 (0·22-0·72) for ex-users and 0·45(0·28-0·75) for current users. These findings confirm the finding from the Royal College of General Practitioners Oral Contraceptive Study that the incidence rate of rheumatoid arthritis among OC users was halved.
