Non-Convulsive Status Epilepticus at Pediatric Intensive Care Unit: A Single Center Experience

Objective: Our study aims to estimate the incidence of non-convulsive status epilepticus (NCSE) in comatose children and to evaluate response to acute treatment with anticonvulsants. Material and methods: This is a prospective observational cohort study conducted in Pediatric Intensive Care Unit (PICU) of Ain-Shams University. Eighty patients presented with Glasgow coma scale (GCS) <8 and/or the presence of abnormal movements or vital sign fluctuations were enrolled in the study. All were subjected to EEG monitoring for at least 1 hour within 24 hours of presentation. Patients diagnosed as NCSE received anticonvulsant treatment and were reevaluated clinically and by EEG after 1 week. Results: Twenty four patients were diagnosed as NCSE (30%). EEG was normalized in 50% of patients after treatment. Conclusion: NCSE is possible to be under-recognized in the PICU settings because of the pleomorphic clinical features. Clinical suspension of NCSE and EEG monitoring for critically ill comatose children will improve their outcome

Nonconvulsive Status Epilepticus in Metabolic Encephalopathy in Indonesia Referral Hospital.

Nonconvulsive status epilepticus (NCSE) is often underdiagnosed in patients with metabolic encephalopathy (ME). The diagnosis of ME should be made specifically to recognize the underlying etiology. Delay in seizure identification and making a diagnosis of NCSE contributed to the poor outcome. This study aimed to find the incidence and outcome of NCSE in patients with ME. This was an observational prospective cross-sectional study in patients with ME in emergency and critical care units in Cipto Mangunkusumo General Hospital. The diagnosis of NCSE was based on EEG using Salzburg Criteria for Nonconvulsive Status Epilepticus (SCNC). The outcome was assessed within 30 days after the NCSE diagnosis has been made. A total of 50 patients with ME were involved in this study. NCSE was confirmed in 32 subjects (64%). The most common etiology of ME was sepsis (58%). The mortality rate in the NCSE and non-NCSE group was 40.6% vs 44.4%. Multiple aetiologies were risk factors to poor outcome in the NCSE group. The incidence of NCSE among patients with ME at our hospital was high. Despite the anti-epileptic treatment of the NCSE group, the underlying cause of ME is still the main factor that affected the outcome. Therefore, aggressive treatment of anti-epileptic drug (AED) should be very carefully considered knowing the possible side-effect that might worsen the outcome of patients with ME.

Nonconvulsive status epilepticus after surgery for ruptured intracranial aneurysms: Incidence, associated factors, and impact on the outcome

ObjectiveTo evaluate the incidence of nonconvulsive status epilepticus (NCSE) after surgery for ruptured intracranial aneurysms, to define factors associated with this complication, and to determine its impact on the outcome. Patients and MethodsClinical and neurophysiological data of 66 patients with aneurysmal subarachnoid hemorrhage (aSAH) who underwent continuous EEG (cEEG) monitoring after microsurgical clipping (53 cases) or endovascular coiling (13 cases) of the ruptured aneurysm were analyzed retrospectively. The diagnosis of NCSE was based on the American Clinical Neurophysiology Society and Salzburg Consensus criteria. ResultsNCSE was revealed in 10 patients (15 %), all of whom underwent craniotomy and aneurysm clipping. In comparison to the subgroup without NCSE, among those who were diagnosed with this complication there was a significantly greater proportion of men (70 % vs. 34 %; P = 0.041), cases with the Glasgow Coma Scale score at admission < 13 (90 % vs. 36 %; P = 0.004), the Hunt and Hess aSAH grades 3−5 (90 % vs. 45 %; P = 0.013), and hydrocephalus (70 % vs. 29 %; P = 0.044). In addition, they required a significantly longer hospital stay (medians, 62.5 vs. 39.5 days; P = 0.015) and showed trend for the lower rate of favorable disability outcomes (20 % vs. 54 %; P = 0.084). ConclusionsNCSE is encountered rather often after the microsurgical clipping of ruptured intracranial aneurysms, especially in severely disabled patients with high-grade aSAH and/or associated hydrocpephalus, and may significantly affect the clinical course and prolong recovery. cEEG monitoring may be helpful for timely diagnosis and treatment of this complication.

Electroencephalography in acute stroke

To determine the incidence of non-convulsive status epilepticus, epileptiform activity, rhythmic and periodic patterns in patients with acute stroke. An analysis of electroencephalography (EEG) in 86 stroke patients in the neurointensive care unit of the tertiary medical center was performed. Criteria for starting EEG recording were epileptic seizures or clinical suspicion of uncontrolled epileptic status. The ictal-interictal continuum biomarkers and the diagnostic value of EEG for prediction of survival and recovery were assessed. Pathological changes on EEG were recorded in 84% of patients. These patients showed the absence of the dominant occipital rhythm (66%) and hemispheric slowing (42%). Diffuse slowing below the theta range was observed in 41% of patients. EEG reactivity was absent in 20%. Sporadic epileptiform discharges were recorded in 36% of patients and rhythmic and periodic patterns in 26%. Reliable predictors of the unfavorable outcome were the absence of dominant occipital rhythm, lack of reactivity, and low amplitude of the background EEG. No association between the recording of epileptiform activity and the probability of death was shown. The most useful EEG biomarkers for predicting survival are amplitude, dominant frequency of background EEG activity and reactivity to external stimulus. Sporadic epileptiform discharges, rhythmic, and periodic patterns are not mandatory associated with a negative prognosis in stroke patients.

Reevaluation of the Critically Ill Patients With Nonconvulsive Status Epilepticus by Using Salzburg Consensus Criteria.

We aimed to assess the usefulness of the Salzburg Consensus Criteria (SCC) for determining the prognosis of critically ill patients with nonconvulsive status epilepticus (NCSE). We retrospectively reviewed consecutive patients with unconsciousness followed up in the intensive care unit (ICU). Three clinical neurophysiologists, one of them blinded to clinical and laboratory data, reevaluated all EEG data independently and determined NCSE according to SCC. The incidence of NCSE and ictal EEG patterns and their relationship to clinical, laboratory, neuroradiological, and prognostic findings were assessed. A total of 107 consecutive patients with mean age 68.2 ± 15.3 years (57 females) were enrolled in the study. Primary neuronal injury was detected in 59 patients (55.7%). Thirty-three patients (30.8%) were diagnosed as NCSE. While authors decided to treat 33 patients (30.8%), 32 patients (29.9%) had been treated in real-life evaluation. Clinical and EEG improvement were detected in 12 patients (11.3%) in real-life treatment group showing correlation with lack of intubation and ICU stay related to postsurgical event. Rate of mortality (45.8%) was high showing association with systemic-metabolic etiology, severity of coma and presence of "plus" modifiers in the EEG. Our findings suggest that SCC is highly compatible with clinical practice in the decision for treatment of patients with NCSE. The presence of "plus" modifiers in the EEG was found to be associated with mortality in these patients and was a significant marker for the high mortality rate.

Electroencephalography features and incidence non convulsive status epilepticus in metabolic encephalopathy at CIPTO Mangunkusumo National General Hospital

Background: Non convulsive status epilepticus (NCSE) in acute metabolic disorder is often unrecognized and the symptoms mostly are not specific. Incidence of NCSE among metabolic encephalopathy has not been reported in Indonesia. Moreover, EEG abnormality could reflect the severity of brain injury.

The Clinical Characteristics and Prognostic Analysis of Status Epilepticus in Northeast China

Objective: Status epilepticus (SE) is a neurological emergency that may cause severe neurological deficiency and even death. The aim of this study was to analyze the clinical characteristics and prognosis of SE in northeast China. Additionally, the etiology and classification are discussed. Materials and Methods: We retrospectively collected the clinical profiles of patients diagnosed with SE in the Epilepsy Center of Jilin University between January 2011 and May 2015. The causes and clinical courses were investigated. The individuals were followed up by visit or telephone, and the prognoses were analyzed. Results: A total of 223 patients with SE were enrolled, including 134 cases with non-refractory SE (NRSE) and 89 cases with refractory or super-refractory SE (RSE/SRSE). Fifteen patients died during hospitalization, yielding a mortality of 6.73%; 11 of them succumbed to multiple organ dysfunction syndrome (MODS). Statistical analysis showed significant differences in age, family history of epilepsy, and the incidence of non-convulsive SE (NCSE) between the NRSE group and RSE/SRSE group (p < 0.05). We found no significant differences in the causes of SE (central nervous system infection, autoimmune encephalitis, and epilepsy) between these 2 groups. Conclusion: In northeast China, SE is associated with relatively lower mortality, and MODS is the major cause of death. Older age and NCSE are risk factors predicting potential progression to RSE. Central nervous system infection, autoimmune encephalitis, and epilepsy were common causes of SE, but there was no significant association between any of these causes and the occurrence of RSE.

Nonconvulsive status epilepticus in cancer patients with altered mental status.

e13028 Background: Non-convulsive status epilepticus (NCSE) is a potentially treatable etiology of unexplained altered mental status (AMS) in cancer patients. NCSE may be under-recognized clinically making electroencephalogram (EEG) essential for diagnosis. The purpose of this study was to determine the incidence of NCSE among patients with cancer undergoing inpatient neurology consultation for AMS, and to characterize their clinical features. Methods: We retrospectively reviewed the clinical records of 512 patients undergoing neurology consult for AMS between 1/1/2010-12/30/2011, and identified those with a diagnosis of NCSE. Basic demographics, primary cancer, clinical presentation, clinical course, and outcomes data were collected. Results: 21 patients were diagnosed with NCSE by EEG, an incidence of 4%. Nonconvulsive Status Epilepticus

SUMMARY Status epilepticus (SE) can be divided into two subgroups, convul- sive status epilepticus (CSE) and nonconvulsive status epilepticus (NCSE). NCSE is clinically characterized by recurrence or persistence of absence or complex partial seizures which unconsciousness per- sists. Seizures are lasted more than 30 minutes, and accompanied by mental and behavioral changes. Although the main feature is the change of level of consciousness, affective, memory, cognitive, speech, motor systems, behavioral and psychiatric disorders can be also seen. Electroencephalography (EEG) is the single diagnos- tic method in this situation. NCSE constitutes approximately 25% of all SE, however it is thought that sometimes it is misdiagnosed and the incidence of NCSE may be higher. The causes are disorders of central nervous system (stroke, infection, trauma, tumor), and metabolic factors (hypoxia, renal diseases, drugs, failure to use an- tiepileptic drug). The treatment contains the standard therapy of SE. Short-acting benzodiazepines are preferred in initial treatment. For more resistant cases, loading of phenytoin is applied. If there is no response to treatment, midazolam and propofol additionally to barbiturates can be used. We aimed to present a case admitted to our ED with altered mental status and diagnosed as NCSE that is under-diagnosed.

Diagnosis of Nonconvulsive Status Epilepticus (NCSE) in Adults with Altered Mental Status: Clinico - Electroencephalographic Considerations

ABSTRACT.The high incidence of nonconvulsive status epilepticus (NCSE) and difficulties encountered in rapid diagnosis have only recently become apparent and most neurophysiology textbooks have yet to fully address this topic. There is an immediate need to provide neurologists, neurology residents, and electroneurodiagnostic technologists with information to assist them in achieving early, accurate diagnosis of NCSE in patients they encounter on a daily basis. Instead of attempting to write a comprehensive review of the topic, we examined over 90 NCSE publications since 1972 and focused our efforts on extracting and summarizing those EEG and clinical considerations we found most useful for accurate diagnosis of NCSE in an adult population.The wide overlap between the clinical symptoms of NCSE and many other disorders causing change in mental status limits the usefulness of clinical measures for diagnosis. Those clinical parameters which have some utility are outlined. EEG testing and accurate interpretation is needed to identify NCSE in most instances. EEG patterns of NCSE are detailed and examples provided in order of progressive complexity. Timely treatment reduces patient risk for morbidity attributed to NCSE. Electroneurodiagnostic technologists who learn to recognize the entire spectrum of NCSE EEG patterns are better able to alert electroencephalographers to EEGs likely needing expedited review. This may facilitate early diagnosis if electroencephalographers determine that NCSE is indeed present. Electroencephalographers correlate the EEG pattern, clinical activity, and other diagnostic information to provide the treating medical doctors with an interpretation and recommendation in a timely manner.Lastly, EEG and clinical checklists and an algorithm which uses benzodiazepine response in conjunction with continuous EEG monitoring are provided to help reduce uncertainty in interpretation of equivocal EEG patterns of NCSE.

Assessment of Hairline EEG as a Screening Tool for Nonconvulsive Status Epilepticus

Because of the high incidence of nonconvulsive status epilepticus (NCSE), the attraction of a "quick and easy" screening electroencephalogram (EEG) is obvious. Previous studies have shown utility of hairline EEG in diagnosing epilepsy. However, this technique has not been evaluated as a screening tool for NCSE. We wanted to provide proof of principle that a screening hairline EEG has sufficient sensitivity to use as a screening tool for diagnosing NCSE. A total of 120, 2- to 3-min EEG samples of normal and various abnormal digital EEG studies were reformatted in three six-channel montages (A, longitudinal bipolar; B, referential to ipsilateral ear; C, referential to contralateral ear) that mimicked a hairline recording and were interpreted by five neurophysiologists. The test data interpretation was compared with the original EEG interpretation. Performance was best with montages A and B; 71% and 70.5% of the samples were interpreted correctly by using these montages. Only 65% of the samples were correctly interpreted by using montage C. With the best montage (A), the sensitivities ranged from 91% for normal EEG to 54% for periodic lateralized epileptiform discharges (PLEDs). The sensitivity for seizures was only 72%. Seizures were frequently misinterpreted as more benign patterns such as normal and diffuse slowing. EEG data reformatted to resemble a hairline EEG had low sensitivity for detecting seizures. As a result, we do not recommend further pursuit of hairline EEG as a "quick and easy" screening tool for NCSE.

Utility of electroencephalogram in altered states of consciousness in intensive care unit patients

BACKGROUND: EEG is an investigative tool for assessing cerebral activity. Although certain EEG patterns may have a specific diagnostic or prognostic inference, they may not be precise for any sole etiology in majority of cases and may need clinical correlation. OBJECTIVE: Aim of this study was to assess the severity and prognosis of cerebral dysfunction in patients admitted to Intensive Care Unit (ICU) and to evaluate the incidence of non-convulsive status epilepticus (NCSE). DESIGN: A prospective study, wherein we analyzed EEG characteristics in a series of 70 patients. SETTING: A tertiary care hospital in Mumbai, India. PATIENTS: EEG characteristics of 70 patients admitted in ICU over a period of 9 months were comprehensively analyzed. These patients were clinically examined and a questionnaire was completed without knowledge of the EEG findings. EEGs were requested for by neurologist or intensivist and our inclusion criteria were (i) patients with altered sensorium of varying etiology, (ii) unconscious patients at risk for non-convulsive status epilepticus (those with a history of epilepsy), and (iii) unconscious patients with involuntary jerky eye movements. RESULTS: Of the various clinical presentations on ICU admission, there were 20 patients with seizures, 15 with metabolic disorders, 13 with infective causes, 9 with hypoxia, 9 with cerebro-vascular accident on presentation, 1 patient with alcohol/drug overdose, 2 with intra-cerebral space occupying lesion and 1 with ambiguous etiology on admission (there being an overlap among the presentation). Mean duration from presentation to performing EEG was 13 hours. 64 (91.42%) patients had abnormal EEGs. 32(50%) patients had EEG slowing and 4(6.25%) patient had electro cerebral inactivity. Eleven (21.87%) patients had epileptiform activity on the EEG of which seven did not have overt seizures (NCSE). Follow-up EEGs of these patients showed resolution of the epileptiform activity. CONCLUSIONS: EEG is useful in patients admitted to ICU in diagnosing NCSE and various other conditions. Emergent EEG study in obtunded patients provides valuable diagnostic and prognostic information.

Is It Status?

Nonconvulsive status epilepticus (NCSE) is difficult to diagnose in the obtunded/comatose patient. Such patients often have other serious medical conditions, and the diagnosis is frequently delayed. We review criteria for diagnosis, treatment, and prognosis of NCSE in this setting. Terms that have been used to describe SE in obtunded/comatose patients without tonic-clonic convulsions include subtle generalized SE, electrographic SE, SE in comatose patients, generalized electrographic SE, non-tonic-clonic SE, subclinical SE, and NCSE. Sometimes the same term has been used when describing different disorders, and different terms are often applied for the same entity. The incidence of NCSE in obtunded/comatose patients is uncertain. Clinically they may display subtle, intermittent focal or multifocal rhythmic movements suggestive of seizures; there may not be movements. NCSE can occur in a variety of disorders, including hypoxia, metabolic disturbances, and after convulsive seizures. A number of EEG patterns have been described in NCSE, and many of these are controversial, particularly as to whether they are ictal. These include periodic lateralized epileptiform discharges (PLEDS), bilateral independent PLEDS (BIPLEDS), periodic epileptiform discharges (PEDS), which can be either focal or generalized, and generalized triphasic waves (TWs). The diagnostic criteria for NCSE also are controversial, and there are no agreed-on criteria to diagnose NCSE in obtunded/comatose patients, nor is there consensus on how it should it be treated. Furthermore, outcome is poor, and several studies suggest that treatment may not be helpful.

Drug‐induced nonconvulsive status epilepticus with low dose of tiagabine

ABSTRACT In placebo‐controlled trials, the overall incidence of nonconvulsive status epilepticus was no higher in the tiagabine‐treated group than in the placebo‐group. Case reports of nonconvulsive status epilepticus under tiagabine suggested a specific role of dose levels, since in these patients symptoms occurred mostly at 40 mg/day or higher. We report a case of complex partial status epilepticus in a patient receiving a low dose of tiagabine and review all 11 case reports of nonconvulsive status epilepticus in patients on tiagabine, with regard to daily doses. Our analysis suggests an individual risk threshold of unknown aetiology.

Non-convulsive status epilepticus: causes, treatment, and outcome in 65 patients.

The incidence of non-convulsive status epilepticus (NCSE) in The Netherlands is not known. Files of admissions in the years 1980-7 were studied from 40 adult patients (older than 15 years) with complex partial status epilepticus (CPSE) and 25 with absence status epilepticus (ASE). The clinical presentation sometimes made distinction between CPSE and ASE possible. Focal clinical signs were more frequent in CPSE; a fluctuating level of consciousness was more often present in ASE. All patients, but one, with ASE and most patients with CPSE (28) were known to have had previous epilepsy. Outcome in ASE was good in all. Outcome in CPSE depended on the underlying cause and quality of treatment. In three patients inadequate treatment probably contributed to morbidity.
