Trajectory of pelvic floor muscle training adherence in Chinese women with postpartum stress urinary incontinence and its related factors: A longitudinal study

ObjectiveNumerous studies have shown that pelvic floor muscle training (PFMT) adherence in women with postpartum stress urinary incontinence is low and influenced by many factors. However, most previous studies have been cross-sectional studies on PFMT adherence. Neglecting physiological and psychological recovery after childbirth may lead to changes in the adherence to PFMT. This longitudinal study investigated the developmental trajectory of PFMT adherence at 8, 10, and 12 weeks postpartum and the associated factors in China. MethodsA total of 213 women with postnatal stress urinary incontinence who visited the obstetric pelvic floor clinic of a Grade A tertiary specialist maternity hospital in Beijing, China between January 2018 and January 2019 were selected. Compliance levels were measured using PFMT compliance questionnaires at 8, 10, and 12 weeks postpartum. Data on self-efficacy, postpartum social support, and other influencing factors were collected at 12 weeks postpartum. Differences in PFMT compliance scores were analyzed using repeated-measures analysis of variance. Multiple stepwise linear regression analyses were used for multivariate analysis. ResultsThe PFMT compliance scores at 8, 10, and 12 weeks postpartum were 13.47 ​± ​4.42, 10.76 ​± ​4.46, and 7.95 ​± ​5.54, which were at a moderate level and showed a decreasing trend over time. Multiple stepwise line regression analysis showed that the factors influencing compliance with PFMT in women with postpartum stress urinary incontinence were knowledge-attitude-practice of PFMT (t ​= ​4.251, P ​< ​0.001), postpartum social support level (t ​= ​4.304, P ​< ​0.001), and self-efficacy level of PFMT (t ​= ​8.577, P ​< ​0.001). ConclusionThe PFMT compliance of Chinese women with postpartum stress urinary incontinence was moderate, with a downward trend. More effective methods should be chosen to improve women's PFMT compliance based on their situation, such as improving patient self-efficacy, social support, and knowledge-attitude-practice.

Compliance with oral nutritional supplements and its influencing factors in postoperative patients with digestive tract tumors: a cross-sectional study

BackgroundOral nutritional supplements are one of the preferred methods of nutritional support for postoperative patients. This study aims to investigate the current status of oral nutritional supplements compliance in postoperative patients with digestive tract tumors and its influencing factors.MethodsConvenience sampling was employed to select 242 patients who underwent surgery for digestive tract tumors at a tertiary hospital in Shanghai from October 2022 to July 2023 as the study subjects. Data following a normal distribution were analyzed using independent sample t-tests, ANOVA single-factor analysis, Pearson correlation analysis, and multiple linear regression analysis to determine the factors influencing compliance with oral nutritional supplements.ResultsA total of 252 questionnaires were distributed, with 10 invalid questionnaires excluded, resulting in an effective questionnaire rate of 96.03%. The compliance score for oral nutritional supplements in postoperative patients with digestive tract tumors was (2.40 ± 1.45), General Self-efficacy Scale (GSES) score was (24.72 ± 4.86), Multidimensional Scale of Perceived Social Support Scale (MSPSS) score was (58.67 ± 11.09), and Belief about Medicines Questionnaire Scale (BMQ) score was (0.17 ± 2.78). Multiple linear regression analysis revealed that age, adverse reactions, educational level, self-efficacy, medication beliefs, and social support were factors influencing compliance with oral nutritional supplements in postoperative patients with digestive tract tumors (P < 0.05).ConclusionOur study revealed that the compliance to oral nutritional supplements among postoperative patients with digestive tract tumors was at a moderate level and was closely associated with age, educational level, adverse reactions to oral nutritional supplements, medication beliefs, social support, and self-efficacy. Nursing staff should conduct nursing assessments based on the specific circumstances of patients and their families, provide personalized health education management plans based on the patients’ educational level, enhance patients’ nutrition knowledge, improve patient self-efficacy, and enhance social support for patients, while further improving patient nutrition management.

Eliciting the views of left breast cancer patients' receiving deep inspiration breath hold radiation therapy to inform the design of multimedia education and improve patient-centred care for prospective patients.

The currently accepted best practice radiation treatment for left breast cancer patients is Deep Inspiration Breath Hold (DIBH) where patients hold a deep breath to reduce late cardiac and pulmonary effects from treatment. DIBH can be challenging and induce or exacerbate anxiety in patients due to the perceived pressure to reduce radiation treatment side effects. This study explored the experiences of patients treated with Deep Inspiration Breath Hold Radiation Therapy (DIBH-RT) to improve patient-centred care and inform the design of multimedia educational tools for future patients undergoing DIBH. This descriptive qualitative study was underpinned by a social constructivist approach to create new educational and patient care approaches based on previous patients' experiences. Semi-structured interviews were conducted with patients who had completed DIBH-RT for breast cancer. Data was analysed with reflexive thematical analysis. Twenty-two patients were interviewed with five key themes identified: (1) informational needs, (2) care needs, (3) autonomy, (4) DIBH performance influencers and (5) other centredness. Recommendations were derived from these themes to improve future treatments of DIBH patients. These recommendations revolved around improvements to education, patient-centred care and strategies to improve self-efficacy with breath holding. Patients offer a wealth of knowledge regarding their lived experiences with treatment which can enhance future patients' experiences if incorporated into their education and care. Eliciting patients' views of their DIBH-RT treatment highlighted the need to improve patient self-efficacy with DIBH through familiarity with their planned treatment from new multimedia education, and foster patient care to enhance their experience.

Perceptions of the effectiveness of non-pharmacological management of respiratory disorders among CRD patients.

Chronic respiratory diseases (CRDs) are among the leading cause of mortality worldwide. While pharmacological approaches are commonly used to manage symptoms, non-pharmacological management of CRDs is considered crucial in preventing disease progression and improving patient self-efficacy. To describe the perceived effectiveness of non-pharmacological management of CRDs among CRD patients and determine whether the CRD patients perceptions of the effectiveness of non-pharmacological management are associated with their demographic characteristics. An analytic cross-sectional study design was utilized. The non-pharmacological management practices of CRD patients and their perception of the effectiveness of these therapeutic measures were assessed using a researcher-developed questionnaire. Data collection took place in primary healthcare centers in Al Ahsa, Saudi Arabia from October 2021 to March 2022. The proportion of CRD patients who perceived that non-pharmacological interventions were effective was computed. Binary logistic regression was performed to determine the association between the demographic characteristics of the respondents and their perception of the effectiveness of non-pharmacological management. Among the 390 respondents, 42% perceived that non-pharmacological measures were effective. Half of the respondents believed that smoking cessation, influenza vaccination, improving physical activity, and nutrition support helped alleviate CRD symptoms, while less than half of the respondents considered pulmonary rehabilitation, educational programs, and oxygen therapy effective measures to control symptoms. Women were found to be 3.24 times more likely to perceive non-pharmacological interventions as effective (P < .0000) compared to men. Those with university-level education were 66.6% less likely to consider non-pharmacological interventions to be effective (P < .0000) than those who completed preuniversity-level education. Age and marital status did not significantly influence perceptions of effectiveness. Differences in the perceptions of the effectiveness of various non-pharmacological measures to alleviate CRD symptoms existed among the CRD patients of Al Ahsa. The perception of effectiveness was significantly associated with the patient's gender and educational attainment.

Patient perceptions of virtual reality for pain relief in labor: A qualitative study.

Labor represents the most common reason for hospitalization, and most patients will use some form of pain management during their labor. While some studies have suggested that virtual reality (VR) may be an effective option for managing pain, more study is necessary to understand the patient experience of VR. The aim of this study is to characterize the effect of VR on patient perceptions of coping in labor and their descriptions of the VR experience. A nested prospective, descriptive study within a randomized controlled trial of VR in laboring patients. We included nulliparous, term patients, having contractions at least every 5 min, a pain score on the Wong-Baker pain scale of 4-7, and who had been randomized to receive the 30 min virtual reality intervention in the trial. Subjects completed a childbirth self-efficacy inventory prior to the intervention. After the intervention, they completed a modified childbirth self-efficacy inventory related to VR and underwent a structured interview. Self-efficacy scores were compared using t-tests and qualitative, thematic analysis was performed using Dedoose. Twenty-one subjects received the VR intervention. Twenty subjects completed the post-intervention survey and structured interview; one declined due to discomfort. Subjects noted a significant increase in perceived degree to which VR could improve their self-efficacy in managing pain during labor. Thematic analysis revealed that subjects described the VR experience as allowing them to connect with their breathing, feeling more relaxed, and being distracted from pain. In total, 70% believed VR reduced their pain, 60% felt it reduced their anxiety, and 100% would recommend VR availability for laboring patients. VR can improve patient self-efficacy for managing pain in labor. Future studies should focus on the content of the visualizations, optimized user experience and design, and effectiveness with ongoing exposure to VR content in labor.

PTSD symptom improvement and smoking cessation among a sample of veterans.

Context: Poor health behaviors are common in persons with posttraumatic stress disorder (PTSD). PTSD symptom improvement has been followed by better health behaviors such as medication adherence and use of nutrition, weight loss, and substance abuse treatment programs. Whether PTSD improvement is associated with smoking cessation is uncertain. Objective: To determine if patients with, compared to without, clinically meaningful improvement (≥20 points vs. <20 points) in PTSD Checklist (PCL) scores are more likely to stop smoking. Study Design: Retrospective cohort using entropy balancing to control for confounding in Cox proportional hazard models overall and stratified by depression and alcohol abuse/dependence. Dataset: Veterans Health Affairs (VHA) medical record data from 2008-2015. Population studied: Patients aged 18-70 years with PTSD who had ≥ 1 visit to PTSD specialty care with a PCL score ≥50, at least one PCL score from ≥8 weeks to 12 months following first PCL≥50 ('exposure year'), and persistent smokers in the exposure year (n=449). Index date is the end of the exposure year. Intervention/Instrument: Change from first to last PCL score in exposure year classified as clinically meaningful vs. less than clinically meaningful improvement (≥20 point decrease vs. <20 point decrease). Outcome measures. Time to smoking cessation as documented in VHA administrative medical record data in the 2-years after index. Follow-up time was measured as months from index to either smoking cessation or censoring. Results: Overall, patients were 39.4 (±12.9) years old, 71.5% white, 86.6% male, 19.8% had a clinically meaningful PCL score decrease, and 32.7% quit smoking in the 2-years after index. After entropy weighting, PCL decrease ≥ 20 vs. < 20 was associated with a 57% increased likelihood of smoking cessation (HR=1.57; 95% CI=1.04-2.36). The relationship of PTSD improvement with smoking cessation was similar in patients with vs. without depression and with and without alcohol abuse/dependence. Among patients who quit smoking, about half remained non-smokers in the 12-months after initial quit date. Conclusions: A clinically meaningful reduction in PTSD symptoms was associated with smoking cessation in the 2-years after PTSD improvement. Not all patients with PTSD have access to PTSD treatment modalities that integrate smoking cessation therapy; however, PTSD treatment alone may improve patient self-efficacy and enable smoking cessation.

Patients with Chronic Kidney Disease (CKD) require renal replacement therapy, one of which is Hemodialysis (HD). HD that needs to be maintained to maintain the quality of life of CKD patients. Adequate hemodialysis can be achieved by increasing adherence to HD. The purpose of this study was to determine the relationship between self-efficacy and support for hemodialysis social compliance. The research design used cross sectional with consecutive sampling. This study involved 110 respondents. Collecting data using CKD Self Efficacy questionnaire, MOS social support survey and study documentation. Data analysis using Chi-Square test. The results showed that the proportion of patients who complied with hemodialysis was 60%, there was a significant relationship between self-efficacy (p= 0.000) and social support (p= 0.002) with HD adherence. The recommendation from this study is that nurses need to improve patient self-efficacy and involve social support to improve patient compliance with the hemodialysis program.

Effectiveness of integrated education in improving patient self-efficacy after total hip and knee replacement surgeries

This study aims to find out the effectiveness of integrated education to improve self-efficacy, reduced pain, and edema, and increased muscle strength and joint mobility after total hip and knee replacement surgeries. This quasi-experimental study provided integrated education therapy. The sample consisted of 31 patients after total hip replacement and total knee replacement surgery. Self-efficacy measurement was performed a day before and ten days after surgery. Harris Hip Score and Knee Society Score grading and measurement of edema level were performed a day and ten days of surgery. In the total hip and knee replacement patients of the intervention group, improved self-efficacy, as well as reduced pain intensity and reduced edema and improved muscle strength, and improved joint mobility, were resulted. Integrated education is more effective in improving patients’ self-efficacy, with decreases in pain intensity and edema and increases in strength mobility and joint mobility.

Association between perception of caring behaviors and self-efficacy in patients with cardiovascular disease at coronary care units: a cross-sectional study.

BackgroundAssessing and improving patient self-efficacy are among the major roles of nurses. Nurses are also responsible for providing direct patient care, and they play a major role in improving patient care quality. Therefore, it is essential to evaluate nurses’ care-giving behaviors. This study aimed to determine the association between caring behaviors and self-efficacy in patients with cardiovascular disease.MethodsIn this cross-sectional study, 400 patients with cardiovascular disease who were admitted to hospitals in Jahrom, southern Iran, were selected through a stratified sampling. The Caring Behaviors Inventory and the Strategies Used by People to Promote Health questionnaires were used to collect data. Data were analyzed using descriptive statistics, Kolmogorov-Smirnov test and Spearman correlation coefficient in SPSS ver. 22.ResultsResults showed significant relationships between perception of caring behaviors and self-efficacy (r=0.16, P=0.001) as well as subscales of respectful deference to others (r=0.12, P=0.01), assurance of human presence (r=0.12, P=0.02), and positive connectedness (r=0.18, P=0.001). Additionally, among the subscales of caring behaviors, “attentive to others’ experience,” with a mean of 5.17±1.10, was the highest priority and “positive connectedness,” with a mean of 4.81±1.31, was the lowest priority for patients. The mean self-efficacy score was 73.94±29.78, and 169 patients (43.2%) had low self-efficacy.ConclusionsGiven the positive relationship between perception of caring behaviors and self-efficacy in patients with cardiovascular disease, self-efficacy could be improved by paying more attention to patient care priorities and improving patient perception of caring behaviors.

A Social Marketing Intervention to Improve Treatment Adherence in Patients with Type 1 Diabetes.

This research explores if a social marketing intervention model based on social representations theory and the health belief model can generate changes regarding treatment adherence and improve patient self-efficacy. As a pilot, a test–retest field quasi-experiment was designed to evaluate the intervention model with type 1 diabetes (T1DM) patients of families with 8- to 17-year-old children. The intervention model was designed to clarify misconceptions, increase awareness of the benefits of following doctors’ treatments and improve patients’ self-efficacy. In-depth interviews were carried out to gain a richer understanding of the intervention’s effect. The pilot intervention generated a favourable change in shared misconceptions, individual health beliefs, glycaemic control and declared treatment adherence. This paper contributes to the social marketing literature and public health by providing early support for the theoretical assumptions regarding the role of shared misconceptions in physiological and behavioural outcomes for patients with T1DM. Contrary to previous studies, instead of only focusing on individual beliefs, this study incorporates shared beliefs between patients and caregivers, generating more comprehensive behavioural change.

Self‐care management importance in kidney illness: a comprehensive and systematic literature review

Kidney illness is a key problem for individuals’ health across the world and a significant reason for death, too. Therefore, education and management are essential for prevention and treatment. Promoting self-management through improving patient self-efficacy may have lasting advantages as the management of kidney disease will bring positive results for the patient. However, the count of patients having incorrect self‐care conducts is still growing even with constant health education. Knowledge and self‐efficacy are the main parameters affecting self‐care conduct. Nevertheless, there are very few works on the significance of self‐care managing in patients suffering from kidney illness. Therefore, in this article, we have intended to conduct a systematic study on the importance of self‐care management techniques in kidney illness until Jan 2020. In addition, the advantages and disadvantages of the revised procedures have been debated, and the key issues of self‐care management have been highlighted. The results have suggested that the management and training of individuals can help prevent kidney illness. Understanding several parameters affecting the growth of communities of self‐care managing will assist the experts in modifying the data and back the learning of self‐care managing. The outcomes can be helpful for the experts managing the self‐care with other lasting conditions.

Resource to prepare patients for deep inspiration breath hold: the RESPIRE project

Introduction: Incidental irradiation of the heart during radiotherapy can have long-term consequences(1). Deep Inspiration Breath Hold (DIBH) is used for patients receiving radiotherapy for breast cancer to reduce heart doses(2). DIBH can reduce mean heart dose by between 29-67% compared with free breathing(3); differences in dose reduction may be due to patient anatomy or variability in chest expansion achieved at an individual level(4). A survey of a breast cancer special interest group (BRIG) identified patient compliance, ability to hold their breath for the required time, and reproducibility of breath hold as major challenges to implementing DIBH. Between 12-21% of patients are reported to find the technique challenging(5, 6). While coaching has been shown to reduce maximum heart dose compared with non-coached patients(7), formal coaching is not typical in most radiotherapy centres and no standard patient resource exists to support practice prior to radiotherapy planning. Aim: To develop a series of instructional videos to enhance the number of patients that can achieve breath hold, improve patient self-efficacy and patient satisfaction with care. Method: The Medical Research Council (MRC)(8) framework for complex interventions was adopted. A co-design methodology(9) involving patient representatives and health practitioners (HCP) was used to develop a series of instructional videos and podcasts. Co-design workshops were audio recorded and transcribed verbatim. Video scripts were based upon themes identified through framework analysis. To minimise bias member checking was adopted and patient representatives participated in video scripting; to maintain the outputs' authenticity they also appeared as patients in the videos. Results: Key themes from the PPI workshop included: information needs; positioning during radiotherapy; and pre-treatment readiness. Additional themes from the HCP workshop included DIBH service challenges and teaching the DIBH technique. Five videos and two podcasts were developed. Conclusion: This presentation will highlight how participatory co-design facilitated video development to support a more effective DIBH experience. Resource examples will be presented alongside initial feedback from patient representatives and HCPs on suitability and usability. The next stage will assess the resources' effectiveness in supporting breath hold capability.

Predictors of Self-Efficacy in Administering Insulin Injection.

This study aimed to identify the predictors of self-efficacy in administering insulin injection among patients with type 2 diabetes. Using a cross-sectional survey, data were collected via purposive sampling from a metabolic ward of a medical center in Southern Taiwan. Participants were 72 patients with type 2 diabetes, who had started using Lantus, Levemir, or Novomix pen injectors. Data were collected from October 2013 to August 2014, using the Diabetes and Insulin Injection Knowledge Scale, Self-Efficacy in Administering Insulin Injection Scale, and photographs illustrating insulin injection skills. The findings indicated that "knowledge of diabetes and insulin injection," "insulin injection skills," "senior high school or above education," and "diabetes duration" were predictors of self-efficacy in administering insulin injection, which explained 41% of the total variance in self-efficacy. Health care professionals can design relevant strategies for improving patient self-efficacy in administering insulin injection, thereby increasing patients' insulin self-injection abilities.

The Use and Effects of Electronic Health Tools for Patient Self-Monitoring and Reporting of Outcomes Following Medication Use: Systematic Review.

BackgroundElectronic health (eHealth) tools are becoming increasingly popular for helping patients’ self-manage chronic conditions. Little research, however, has examined the effect of patients using eHealth tools to self-report their medication management and use. Similarly, there is little evidence showing how eHealth tools might prompt patients and health care providers to make appropriate changes to medication use.ObjectiveThe objective of this systematic review was to determine the impact of patients’ use of eHealth tools on self-reporting adverse effects and symptoms that promote changes to medication use. Related secondary outcomes were also evaluated.MethodsMEDLINE, EMBASE, and CINAHL were searched from January 1, 2000, to April 25, 2018. Reference lists of relevant systematic reviews and included articles from the literature search were also screened to identify relevant studies. Title, abstract, and full-text review as well as data extraction and risk of bias assessment were performed independently by 2 reviewers. Due to high heterogeneity, results were not meta-analyzed and instead presented as a narrative synthesis.ResultsA total of 14 studies, including 13 randomized controlled trials (RCTs) and 1 open-label intervention, were included, from which 11 unique eHealth tools were identified. In addition, 14 RCTs found statistically significant increases in positive medication changes as a result of using eHealth tools, as did the single open-label study. Moreover, 8 RCTs found improvement in patient symptoms following eHealth tool use, especially in adolescent asthma patients. Furthermore, 3 RCTs showed that eHealth tools might improve patient self-efficacy and self-management of chronic disease. Little or no evidence was found to support the effectiveness of eHealth tools at improving medication recommendations and reconciliation by clinicians, medication-use behavior, health service utilization, adverse effects, quality of life, or patient satisfaction. eHealth tools with multifaceted functionalities and those allowing direct patient-provider communication may be more effective at improving patient self-management and self-efficacy.ConclusionsEvidence suggests that the use of eHealth tools may improve patient symptoms and lead to medication changes. Patients generally found eHealth tools useful in improving communication with health care providers. Moreover, health-related outcomes among frequent eHealth tool users improved in comparison with individuals who did not use eHealth tools frequently. Implementation issues such as poor patient engagement and poor clinician workflow integration were identified. More high-quality research is needed to explore how eHealth tools can be used to effectively manage use of medications to improve medication management and patient outcomes.

Influencing factors of patients activation: a study based on empowerment theory

Objective To explore the present perceptions of patient-empowering nurse behaviours, self-efficacy and patient activation among patients with chronic diseases, and analyze the influencing mechanism among these three variables. Methods Using convenient sampling method, a total of 485 inpatients with chronic diseases were sampled from two tertiary comprehensive hospitals in Guangzhou on the day of discharge between March and October 2017. They were surveyed with the demographic questionnaire, Patient Perceptions of Patient-Empowering Nurse Behaviours Scale , Chronic Disease Self-Efficacy Scale and Patient Activation Measure. Results Empowerment could positively predict self-efficacy (β=0.402, P<0.01) and patient activation (β=0.169, P<0.01). Self-efficacy could also positively predict patient activation (β=0.887, P<0.01). And self-efficacy, of which the effect accounted for 67.87% of the total effect, played a partial intermediary role between empowerment and patient activation. Conclusions To improve patient activation, empowerment and self-efficacy should be both emphasized and applied by the nurses, so that patients can move smoothly from a care receiver before discharge to one of self-management. Improving health self-efficacy at a community health fair in Lubbock, Texas

To improve the health self-efficacy of the Lubbock, Texas community, Texas Tech University Health Sciences Center School of Medicine students organized a health fair to promote patient education and improve awareness of health issues, such as diabetes, heart health, hypertension, skin care, hypercholesterolemia, nutrition, stress management, cancer screening and recommendations, physical fitness, and mental health. An evaluation of 57 health fair participants’ self-reported self-efficacy on a scale from 0 to 10 before and after visiting the health fair revealed an average increase in self-efficacy of 0.87 (before: 7.96, after: 8.83, p&lt;0.0001, std=1.78). Although the health fair was successful in marginally improving patient self-efficacy, patient education and overall community health can continue to be improved by expanding educational opportunities for health promotion and illness prevention, furthering overall access to care, and broadening the health fair’s target population.

Effect of conversation map education based on empowerment theory in patients with chronic heart failure

Objective To explore the effect of conversation map education based on empowerment theory in patients with chronic heart failure. Methods Patients of chronic heart failure with first treatment admitted to Traditional Chinese Medicine Hospital of Jiangyin City from May 2014 to April 2016 were enrolled. Totals of 61 patients from May 2014 to April 2015 were as control group, and 63 patients from May 2015 to April 2016 were as observation group. Self efficacy, treatment compliance and quality of life of patients were compared between the two groups after education. Results The degree of changes in ten dimensions of movement scope, disease information, getting help, communication with physicians, general disease control, doing housework, social entertainments, symptom management, management of rapid breathing, depression control in the observation group were all higher than those in the control group (t=4.584, 5.675, 4.779, 4.168, 3.978, 3.768, 3.878, 3.478, 3.375, 3.964; P<0.01) . The scores of seven dimensions in medication adherence, timely reviews, reasonable rest, reasonable diet, reasonable exercise, infection prevention, abstain from tobacco and drink and the total score in the observation group were significantly higher than those in the control group (t=6.042, 6.924, 5.825, 5.034, 6.552, 5.614, 4.635, 5.824; P<0.01) . Besides, the improvement of physiological function, mental function, social function, health self-awareness ability and the total score of quality of life in the observation group were higher than those in the control group (t=5.725, 4.735, 4.742, 4.332, 3.823; P<0.01) . Conclusions Conversation map education based on empowerment theory can improve self efficacy of patients with chronic heart failure, enhance the self-management level and the ability to persisting in health behavior and improve treatment compliance of patients. Correlation analysis of self-efficacy and postoperative ambulation in patients with gastric cancer

Objective To investigate the self-efficacy and postoperative ambulation in patients with gastric cancer, and analyze the effects of self-efficacy on postoperative ambulation. Methods A total of 101 patients with gastric cancer undergoing surgery in the oncological surgery of Traditional Chinese Medicine Hospital of Kunshan Affiliated to Nanjing University of Chinese Medicine from October 2014 to October 2016 were included using convenient sampling method. Normal data, self-efficacy and postoperative ambulation of patients were surveyed. The current status of postoperative ambulation and the effects of self-efficacy on postoperative ambulation were analyzed. Results The total score of postoperative self-management efficacy in patients with gastric cancer at 120 h post operation was (72.34±7.26) with a medium level; the score of positive attitude dimension was (38.52±4.09) , the score of self-decompression dimension was (26.32±5.16) , and the score of self-decision dimension was (8.46±3.87) . At 72 h post operation, the ambulation level was low with a score of (5.32±1.48) , while for 120 h post operation, the ambulation was at a medium level with a score of (10.08±1.76) . The postoperative ambulation positively correlated to the score of self-efficacy (r= 0.85, P<0.05) . Conclusions Self-efficacy is an influence factor of postoperative ambulation in patients with gastric cancer. It is possible to investigate the methods of improving self-efficacy of patients so as to increase postoperative ambulation of patients with gastric cancer. Application of positive psychological intervention in hospice care of patients with advanced cancer

Objective To explore the effect of positive psychological intervention on life quality of patients with advanced cancer. Methods A total of 50 patients with advanced cancer were included and randomly divided into intervention group (n=50) and control group (n=50) between April 2014 and July 2016. The routine nursing and tumor nursing strategies were used in both groups, and the positive psychological intervention by clinical psychology professionals was additionally adopted in the intervention group. The quality of life between the two groups was evaluated by quality of life assessment scale (SF-36) before and after intervention. Results The average score of SF-36 in intervention group and control group was (80.8±2.3) and (74.6±3.5) , respectively. The difference between two groups was statistically significant (t=10.467, P<0.01) . Conclusions The positive psychological intervention can improve self-efficacy and self happiness of patients with advanced cancer, relieve negative emotions caused by negative physical feelings, and improve the quality of life. An Online Learning Module to Increase Self-Efficacy and Involvement in Care for Patients With Advanced Lung Cancer: Research Protocol.

BackgroundImproving patient care for individuals with lung cancer is a priority due to the increasing burden of the disease globally. One way this can be done is by improving patient self-management capabilities through increasing their self-efficacy. This can improve positive outcomes for patients with chronic conditions and increase their ability to manage the challenges of such illnesses. Unfortunately, patients with chronic conditions often struggle to travel far from home to engage with patient education events, a common means of improving self-efficacy. The development of more accessible tools for improving patient self-efficacy is required to increase quality of life for patients with chronic conditions.ObjectiveTo evaluate the feasibility of delivering symptom identification and management information to patients with advanced lung cancer using an online program.MethodsThis article describes a pre-post test study to evaluate a Qstream online learning platform to improve patient self-efficacy for managing advanced lung cancer symptoms. Undertaking this program should increase participant knowledge about the side-effects they may experience as a result of their treatment and in turn increase help-seeking behavior and self-efficacy for the participant cohort. Quantitative data collected by the Qstream platform on the completion rates of participants will be used as a tool to evaluate the intervention. Additionally, validated scales will be used to collect data on patient self-efficacy. Qualitative data will also be collected via an exit survey and thematic content analysis of semi-structured interviews.ResultsThe research is in the preliminary stages but thus far a protocol has been approved in support of the project. Additionally, advisory committee members have been identified and initial meetings have been undertaken.ConclusionsDevelopment of new approaches for increasing patient understanding of their care is important to ensure high quality care continues to be delivered in the clinical setting.
