Integrating PrEP into HIV care clinics could improve partner testing services and reinforce mutual support among couples: provider views from a PrEP implementation project in Kenya.

IntroductionPartner notification services (PNS) increase the HIV status knowledge and linkage to care and treatment. However, it is unclear if PNS can facilitate linkage of HIV‐negative partners to prevention services such as pre‐exposure prophylaxis (PrEP). Using qualitative methods, we explored provider perspective regarding the interaction of PrEP availability, PNS and antiretroviral treatment (ART) outcomes within a project integrating PrEP services into HIV care clinics in eight counties in western and central Kenya.MethodsFrom May 2017 to August 2018, data on integrated PrEP service delivery including its interaction with PNS were collected through 71 key informant in‐depth interviews with healthcare providers and 24 standardized technical assistance reports summarizing implementation at the participating clinics. Thus, the perspective was from that of providers; analyses focused on emergent themes relating PNS to PrEP and ART services.ResultsProviders found that PrEP integration provided an additional concrete prevention option for HIV‐negative partners and created a motivation to offer PNS to persons living with HIV. PrEP availability also seemed to operate as an incentive for those living with HIV to participate in PNS, which in turn enhanced identification of potential PrEP clients and created an environment for discussing HIV transmission risk. Providers commented that initiating HIV‐negative partners on PrEP enhanced mutual monitoring of health outcomes, including improved adherence to ART by partners living with HIV. Clinics noted prioritizing people living with HIV with detectable viral loads for PNS in order to identify HIV‐negative partners who would benefit most from PrEP. Providers felt motivated by the apparent synergistic interaction of PNS, PrEP and ART.ConclusionsProviders felt that the integration of PrEP into HIV care clinics stimulated the provision of PNS, and PNS was used to improve the identification of potential PrEP clients. The integrated combination of PNS, PrEP and ART is synergistic and should be promoted in HIV clinics.

A Case of TASO Tororo Surge Strategy: Using Double Layered Screening to Increase the Rate of Identification of New HIV Positive Clients in the Community

Introduction: HIV testing services is the entry point to HIV prevention, care, treatment, and support services. According to Uganda Population HIV impact assessment preliminary report released in 2018, 72.5% of people living with HIV in Uganda knew their status, which is below the UNAIDS target of 90%. We proposed a double layered screening of the population using the Ministry of Health HIV Testing Services (HTS) screening tool to identify more HIV positives and start them on treatment. The objective of this study was to assess the impact of the double layered screening approach on HIV test yield. Methods: A double layered screening approach involved using community and technical teams from TASO Tororo HIV clinic through the surge strategy. The community team (first layer) comprised of expert clients, local council 1, market and church leaders who were trained on how to screen the people in the community using the HTS screening tool. The technical team (second layer) comprised of medical personnel and counselors who subjected all people mobilized and screened by the community team to a second layered screening before offering an HIV test. We compared proportions of HIV test yields before and after the implementation of the double layered HTS strategy using proportions test and we assessed the impact of the double layered screening using a difference in difference (DID) evaluation method. Results; There was a general increase in HIV test yield from 4.75% with single screening (period: January-March 2018) to 12.25% with double screening (period: April – June 2018) (P<0.001). The increase was more in males (from 3.51% to 11.06%) than in females (from 6.36% to 13.31%) and this difference was significant (P=0.035). The increase in HIV test yield did not differ by age (P=0.060), by marital status (P=0.606) or by first time tester (P=0.167), Conclusion: The double layered screening before HIV test could be an effective strategy to maximize HIV test yield in the general population, which if scaled up can save huge resources, time and help focus on actual targets for HIV testing services, leading to early attainment of the UNAIDS 1st target of 90-90-90.

Analysis on the Use of Continuous Improvement, Technology and Flipped Classroom in the Teaching-Learning Process by means of Data Science

This mixed research uses the Juran’s methodology to achieve continuous improvement in the teaching-learning process on mathematics through technology. This study proposes the creation of a virtual environment by means of the FreeDFD simulator, flipped classroom and YouTube videos with the purpose of linking the theoretical topics in the Logical of Predicates Unit with the field of computer science. The variables related to the teaching-learning process are Efficiency (assimilation of knowledge) and Technical Quality (Bloom’s Taxonomy: Memory, Comprehension and Application). On the other hand, students’ perception is analyzed through the Quality of service (teaching-learning process, technological ability and mathematical ability) and Customer satisfaction (utility, benefits and need). By means of data mining (decision trees technique), this research identifies four predictive models on assimilation of knowledge considering Bloom’s Taxonomy. The results obtained from linear regression (machine learning with 50% training and 50% evaluation) indicate that the Juran’s methodology facilitates the construction of virtual educational spaces through the stages related to the identification of clients, establishment of customer needs, translation of needs into the language of the organization, development of the product or service, creation of a process and transfer of the process to the operation. In conclusion, universities should improve educational conditions considering aspects of the technology, flipped classroom and continuous improvement.

Utilizing predictive modeling to enhance policy and practice through improved identification of at-risk clients: Predicting permanency for foster children

Child welfare agencies are increasingly required to leverage their limited resources to meet nearly limitless demands. As a result, agencies are searching for new opportunities to efficiently improve policy and practice, and advances in data availability and technology have brought increased attention to the utility of predictive modeling. While the literature has often highlighted the considerable potential of predictive models leveraging “big data”, discussions of the methodology and the associated best practices remain critically absent. To address this gap, this paper provides an illustrative case involving the development and testing of models used to predict the probability of whether U.S. foster children would achieve legal permanency. The models were trained and tested using a national administrative dataset of 233,633 foster care children that discharged from state child welfare systems in 2013. The optimal model, a boosted tree, predicted whether children would achieve permanency with 97.66% accuracy. The paper concludes with a discussion of best practices detailing how agencies can utilize predictive modeling to enhance policy and practice.

X-linked inhibitor of apoptosis protein (XIAP) is a client of heat shock protein 70 (Hsp70) and a biomarker of its inhibition

Heat shock protein 70 (Hsp70) and Hsp90 are molecular chaperones that play essential roles in tumor growth by stabilizing pro-survival client proteins. However, although the development of Hsp90 inhibitors has benefited from the identification of clients, such as Raf-1 proto-oncogene, Ser/Thr kinase (RAF1), that are particularly dependent on this chaperone, no equivalent clients for Hsp70 have been reported. Using chemical probes and MDA-MB-231 breast cancer cells, we found here that the inhibitors of apoptosis proteins, including c-IAP1 and X-linked inhibitor of apoptosis protein (XIAP), are obligate Hsp70 clients that are rapidly (within ∼3-12 h) lost after inhibition of Hsp70 but not of Hsp90. Mutagenesis and pulldown experiments revealed multiple Hsp70-binding sites on XIAP, suggesting that it is a direct, physical Hsp70 client. Interestingly, this interaction was unusually tight (∼260 nm) for an Hsp70-client interaction and involved non-canonical regions of the chaperone. Finally, we also found that Hsp70 inhibitor treatments caused loss of c-IAP1 and XIAP in multiple cancer cell lines and in tumor xenografts, but not in healthy cells. These results are expected to significantly accelerate Hsp70 drug discovery by providing XIAP as a pharmacodynamic biomarker. More broadly, our findings further suggest that Hsp70 and Hsp90 have partially non-overlapping sets of obligate protein clients in cancer cells.

Resources for Educating, Training, and Mentoring Nurses and Unregulated Nursing Care Providers in Palliative Care: A Review and Expert Consultation.

Nurses and nursing care providers provide the most direct care to patients at end of life. Yet, evidence indicates that many feel ill-prepared for the complexity of palliative care. To review the resources required to ensure adequate education, training, and mentorship for nurses and nursing care providers who care for Canadians experiencing life-limiting illness and their families. This is a systematic search and narrative review in the Canadian context. Six previous reviews and 26 primary studies were identified. Studies focusing on regulated nurses indicated that even amid variability in content, delivery methods, and duration, palliative education improves nurses' knowledge, confidence, attitudes, and communication abilities, and decreases nurses' stress. Results from palliative education in undergraduate curriculum were less definitive. However, studies on palliative simulation in undergraduate education suggest that it improves knowledge and confidence. Studies focusing on educating nursing care providers, either alone or in collaboration with regulated nurses, indicated positive outcomes in knowledge, confidence, communication, identification of clients who are dying, abilities to interact with patients and families, and a better understanding of their own contributions to care. Curricular resources in Canada have been developed. However, there is no dedicated and funded capacity-building strategy. Resources exist to support palliative education for nurses and nursing care providers. Furthermore, the evidence suggests good outcomes from this education. However, there is no dedicated strategy for implementing those resources. Furthermore, there is little evidence of the critical role of knowledge translation in preparing nurses and nursing care providers for evidence-informed palliative practice.

Growth curves of clients’ emotional experience and their association with emotion regulation and symptoms

Objective: Emotional experience during psychotherapy is considered a core mechanism of change. Yet the sheer experience itself may not necessarily be beneficial; instead, the trajectories of emotional experience need to be explored as possible predictors of treatment outcomes. This study investigated whether clients’ pre-treatment levels of emotion regulation and symptoms predicted patterns of session-to-session change in emotional experience. We also explored which patterns better predict clients’ improvement in emotion regulation and symptoms from pre- to post treatment. Method: One-hundred and seven clients undergoing psychodynamic psychotherapy completed questionnaires on their symptoms and emotion regulation at pre- and post- treatment. They also reported their level of emotional experience at the end of each session. Results: Pre-treatment symptoms and difficulties in emotion regulation predicted greater instability in emotional experience. Higher mean levels of emotional experience during treatment were associated with an improvement in emotion regulation, and greater stability during treatment was associated with improvement in emotion regulation and symptoms. Conclusions: These findings lend weight to the idea that experiencing emotion in the therapeutic environment has significant implications for clients’ ability to manage their emotions outside the session. However, emotions experienced in an unstable manner within therapy are associated with poorer outcomes.Clinical and methodological significance of this article: Therapists can benefit from observing the patterns and not only the level of their clients’ emotional experiences. The identification of clients’ difficulties early in treatment may help therapists guide clients through the delicate process of carefully attending to their emotions.

Investigating the Problems and Needs of Infertile Patients Referring to Assisted Reproduction Centers: A Review Study

Background: The provision of optimal care is the most important goal in nursing, the fulfillment of which requires the identification of clients’ problems and needs. However, based on the review of the literature, no review study has investigated the problems and needs of the infertile patients in Iran. Aim: The purpose of the present study was to investigate the problems and needs of the infertile patients referring to the assisted reproduction centers. Method: This review study was based on the traditional review procedure developed by Cronin et al., which entails five steps including: 1) choosing the topic of the review, 2) searching the manuscripts, 3) collecting, reading, and analyzing the texts, 4) writing the review, and 5) providing references. The articles published within 2003-2017 were searched in such valid databases as Google Scholar, Pub Med, Science Direct, Ovid, and Cochran. The inclusion criteria in this study were articles in Persian and English with the keywords referring to problems and needs of clients. Out of the 350 original articles, 31 cases were finally selected for this review study. Results: In general, the infertile patients’ problems were placed under four domains of mental-psychological, social, marital, and financial factors. The needs of the infertile individuals were grouped into six domains of physical, care, informational, financial, mental-psychological, and spiritual factors. Implications for Practice: The identification of the patients’ problems and needs can lead to the conceptualization of strategic points targeted toward the delivery of effective interventions facilitating the provision of patient-centered infertility care. This can enhance the quality of life and lower the levels of stress during the course of treatment.

Identification of misdiagnosed HIV clients in an Early Access to ART for All implementation study in Swaziland.

Introduction: Rapid diagnostic testing has made HIV diagnosis and subsequent treatment more accessible. However, multiple factors, including improper implementation of testing strategies and clerical errors, have been reported to lead to HIV misdiagnosis. The World Health Organization has recommended HIV retesting prior to antiretroviral therapy (ART) initiation which has become pertinent with scaling up of Early Access to ART for All (EAAA). In this analysis, misdiagnosed clients are identified from a subgroup of clients enrolled in EAAA implementation study in Swaziland. Methods: The subgroup to assess misdiagnosis was identified from enrolled EAAA study clients, who had an undetectable viral load prior to ART initiation between September 1, 2014 and May 31, 2016. One hundred and five of 2533 (4%) clients had an undetectable viral load prior to initiation to ART (pre-ART). The HIV status of clients was confirmed using the Determine HIV 1/2 and Uni-Gold HIV 1/2 rapid tests performed serially as recommended by the national testing algorithm. The status of clients on ART was additionally confirmed by fourth-generation HIV Ag/Ab combo tests, Architect and Genscreen Ultra. Results: Fourteen of the 105 (13%) clients were false positive (HIV negative) on confirmation testing, of whom five (36%) were still in pre-ART care, while nine (64%) were in ART care. Overall, proportion of false positive was 0.6% (14/2533). The false-positive clients had a median CD4 of 791 cells/ml (interquartile range (IQR): 628, 967) compared to 549 cells/ml (IQR: 387, 791) for true positives (HIV positive) (p = 0.0081) and were nearly 20 years older (p = 0.0008). Conclusions: Overall 0.6% of all enrolled EAAA clients were misdiagnosed, and 64% of misdiagnosed clients were initiated on ART. With adoption of EAAA guidelines by national governments, ART initiation regardless of immunological criteria, strengthening of proficiency testing and adoption of retesting prior to ART initiation would allow identification of misdiagnosed clients and further reduce potential of initiating misdiagnosed clients on ART.

Informing service responses to co-occurring complex needs

PurposeThe sexual assault and alcohol and other drug (AOD) sectors have much in common in the context of clients presenting with histories of abuse and trauma. The Establishing the Connection project aimed to improve understanding of the complex intersections between sexual victimisation and AOD use, and to build the capacity of the sexual assault and AOD sectors in Victoria, Australia, to respond more effectively to the needs of affected individuals. The paper aims to discuss these issues.Design/methodology/approachEstablishing the Connection involved a detailed literature review; face-to-face semi-structured interviews and an online quantitative survey with key stakeholders from the AOD and sexual assault sectors; and a cross-sector forum to bring together the practice knowledge and needs of both sectors.FindingsMost practitioners and clinicians were aware of the interrelationship between sexual victimisation and AOD use, but many lacked access to resources, formal referral processes and pathways, or information about how the other sector worked – all tools which would assist them to respond to clients with complex co-occurring issues. Participants expressed enthusiasm for greater cross-sector communication and collaboration, but emphasised the need for sophisticated governance and resourcing responses from peak bodies and government departments.Originality/valueThe new knowledge generated by this research resulted in the development of shared practice guidelines to guide practice towards the identification, assessment, response and referral of clients with co-occurring sexual victimisation and AOD use issues. The authors hope that the momentum and goodwill built between the two sectors will be harnessed by policy to drive further collaborative partnerships into the future.

HTTPS traffic analysis and client identification using passive SSL/TLS fingerprinting

The encryption of network traffic complicates legitimate network monitoring, traffic analysis, and network forensics. In this paper, we present real-time lightweight identification of HTTPS clients based on network monitoring and SSL/TLS fingerprinting. Our experiment shows that it is possible to estimate the User-Agent of a client in HTTPS communication via the analysis of the SSL/TLS handshake. The fingerprints of SSL/TLS handshakes, including a list of supported cipher suites, differ among clients and correlate to User-Agent values from a HTTP header. We built up a dictionary of SSL/TLS cipher suite lists and HTTP User-Agents and assigned the User-Agents to the observed SSL/TLS connections to identify communicating clients. The dictionary was used to classify live HTTPS network traffic. We were able to retrieve client types from 95.4 % of HTTPS network traffic. Further, we discussed host-based and network-based methods of dictionary retrieval and estimated the quality of the data.

Eye-spy wealth: cultural capital and ‘knowing luxury’ in the identification of and engagement with the superrich

Drawing upon a wider ethnographic study of superrich lifestyle mobilities, I reflect upon my experiences of working as an assistant within a yacht brokerage during a superyacht sales show in West Palm Beach, Florida. Specifically, I examine the role of dress and appearance in guiding (and at times misguiding) my identification of potential superrich clients during the yacht show. In turn I outline the four-step process utilized by yacht brokers both to qualify the wealth status of a potential client and to successfully engage with legitimate clients. In doing so, I reflect upon some of the constraints of conducting an empirical study of the superrich, including the need to demonstrate sufficient cultural capital of luxury leisure practices and material goods.

Diving deeper: ways to improve the early identification of young clients with disordered eating in rural areas

Connections with a local mental health service for young people, headspace, led to collaboration between local health service dietitian's and headspace staff. This partnership enables early detection and treatment of young people with disordered eating in Tamworth NSW. A project identified the need to enhance staff awareness of nutrition and disordered eating, improved management of young people with disordered eating and streamlined referral processes to local dietitians. Specific education was conducted for headspace, Tamworth staff about food, mood and body image in young people. Staff then attended formal training in identifying and assessing eating disorders. Staff identified the development of a dietetic service for disordered eating, as a priority for headspace, Tamworth. Standard intake forms were adapted with more detailed questions about intake and attitudes to food and body image. Referral processes to dietitians were improved and referral numbers increased markedly. The demand for eating disorder specific dietetic services led to funding for an on-site dietitian one day per week. Dietitian led monthly clinical case review, staff inservices and young person cooking skills classes have all commenced since the service began. Improved screening and referral processes have identified the need to ‘dive deeper’ into nutrition for this target group.

CloudID: Trustworthy cloud-based and cross-enterprise biometric identification

In biometric identification systems, the biometric database is typically stored in a trusted server, which is also responsible for performing the identification process. However, a standalone server may not be able to provide enough storage and processing power for large databases. Nowadays, cloud computing and storage solutions have provided users and enterprises with various capabilities to store and process their data in third-party data centers. However, maintenance of the confidentiality and integrity of sensitive data requires trustworthy solutions for storage and processing of data with proven zero information leakage. In this paper, we present CloudID, a privacy-preserving cloud-based and cross-enterprise biometric identification solution. It links the confidential information of the users to their biometrics and stores it in an encrypted fashion. Making use of a searchable encryption technique, biometric identification is performed in encrypted domain to make sure that the cloud provider or potential attackers do not gain access to any sensitive data or even the contents of the individual queries. In order to create encrypted search queries, we propose a k-d tree structure in the core of the searchable encryption. This helps not only in handling the biometrics variations in encrypted domain, but also in improving the overall performance of the system. Our proposed approach is the first cloud-based biometric identification system with a proven zero data disclosure possibility. It allows different enterprises to perform biometric identification on a single database without revealing any sensitive information. Our experimental results show that CloudID performs the identification of clients with high accuracy and minimal overhead and proven zero data disclosure.

Management of healthcare facilities and meeting cultural needs of clients

The aim of this paper was to identify the relationship between customer requirements for fulfilling their cultural needs, and the size of healthcare facilities that provide inpatient health care in Slovakia. We also investigated the rate of identification of clients’ cultural needs in selected Slovak healthcare facilities, regarding communication, alternative nutrition and the spiritual needs of clients from different cultural backgrounds. We also examined conditions for nurse training relating to meeting clients’ needs. Data were collected by means of a questionnaire distributed to management representatives of healthcare facilities in Slovakia. The sampling was deliberate. 146 facilities providing inpatient health care according to the National Health Information Centre were addressed. Methods of descriptive and inductive statistics (Cramer's contingency coefficient) were used to evaluate the questionnaires. Survey results showed that meeting culturally determined needs had a significant relationship with the size of the healthcare facility. The size of the healthcare facility did not have a significant relationship with the level of satisfaction of the following cultural needs – communication, alternative nutrition, and spiritual needs. This might lead to further research and it might challenge education of healthcare professionals regarding the changing needs of clients of healthcare facilities in the Slovak Republic.

Usage Profiles

As part of web services life-cycle, providers frequently face decision about changes without a clear understanding of the impact on their clients. The identification of clients’ consumption patterns constitutes invaluable information to support more effective decisions. In this paper, the authors present a framework that supports the discovery of service usage profiles, to bring awareness on the distinct groups of consumers, and their usage characterization in terms of detailed service functionality. The framework encompasses a process to cluster client applications and derive usage profiles. The paper also discusses how usage profiles can help to access the real impact on clients of incompatible changes performed over service descriptions, and presents a usage-oriented compatibility assessment algorithm. Experimental results are presented for both the profile discovery process and profile-based compatibility analysis.

Development of a model for the tactical identification of prospective clients in multiuser warehouses

The average contract period in multiuser warehouses, operated by third party logistics providers (3PLs), is up to five years and is still becoming shorter. So, 3PLs need to look for new contracts regularly in an efficient way. To support this process, the objective is to develop a decision-model that assists salespersons to decide, which product would be best to integrate. By a successive check of classified criteria the model tells the salesperson for which product he should invest in opening up new clients. Like this, 3PLs can check main criteria without big effort and concentrate on the main acquisition.

Food Insecurity in Canadian Adults: Receiving Diabetes Care

The prevalence of adult-level household food insecurity was examined among clients receiving outpatient diabetes health care services. Participants were adults diagnosed with diabetes mellitus, who attended individual counselling sessions at Calgary's main clinic from January to April 2010. Clinicians were trained to administer the Household Food Security Survey Module (HFSSM), and did so with clients' assent during their scheduled sessions. The prevalence of adult-level household food insecurity among 314 respondents was 15.0% (95% confidence interval [CI], 11.2 to 19.4); 6.7% (95% CI, 4.2 to 10.0) of clinic attendees were categorized as severely food insecure. The comparable rates obtained in Alberta in 2007 using the same instrument (HFSSM) were 5.6% and 1.2%, respectively. Household food insecurity rates among individuals with diabetes in active care are higher than rates reported in Canadian population surveys. Severe food insecurity, indicating reduced food intake and disrupted eating patterns, may affect this population's ability to follow a pattern of healthy eating necessary for effective diabetes management. This study reinforces the importance of assessing clients' inability to access food because of financial constraints, and indicates that screening with a validated measure may facilitate identification of clients at risk.

Identification of CSPα Clients Reveals a Role in Dynamin 1 Regulation

Cysteine string protein α (CSPα), a presynaptic cochaperone for Hsc70, is required for synapse maintenance. Deletion of CSPα leads to neuronal dysfunction, synapse loss, and neurodegeneration. We utilized unbiased, systematic proteomics to identify putative CSPα protein clients. We found 22 such proteins whose levels are selectively decreased in CSPα knockout synapses. Of these putative CSPα protein clients, two directly bind to the CSPα chaperone complex and are bona fide clients. They are the t-SNARE SNAP-25 and the GTPase dynamin 1, which are necessary for synaptic vesicle fusion and fission, respectively. Using hippocampal cultures, we show that CSPα regulates the stability of client proteins and synaptic vesicle number. Our analysis of CSPα-dynamin 1 interactions reveals unexpectedly that CSPα regulates the polymerization of dynamin 1. CSPα, therefore, participates in synaptic vesicle endocytosis and may facilitate exo- and endocytic coupling. These findings advance the understanding of how synapses are functionally and structurally maintained.
