Measurable transitions during seizures in intracranial EEG: A stereoelectroencephalography and SPECT study

ObjectiveIctal Single Photon Emission Computed Tomography (SPECT) and stereo-electroencephalography (SEEG) are diagnostic techniques used for the management of patients with drug-resistant focal epilepsies. While hyperperfusion patterns in ictal SPECT studies reveal seizure onset and propagation pathways, the role of ictal hypoperfusion remains poorly understood. The goal of this study was to systematically characterize the spatio-temporal information flow dynamics between differently perfused brain regions using stereo-EEG recordings. MethodsWe identified seizure-free patients after resective epilepsy surgery who had prior ictal SPECT and SEEG investigations. We estimated directional connectivity between the epileptogenic-zone (EZ), non-resected areas of hyperperfusion, hypoperfusion, and baseline perfusion during the interictal, preictal, ictal, and postictal periods. ResultsCompared to the background, we noted significant information flow (1) during the preictal period from the EZ to the baseline and hyperperfused regions, (2) during the ictal onset from the EZ to all three regions, and (3) during the period of seizure evolution from the area of hypoperfusion to all three regions. ConclusionsHypoperfused brain regions were found to indirectly interact with the EZ during the ictal period. SignificanceOur unique study, combining intracranial electrophysiology and perfusion imaging, presents compelling evidence of dynamic changes in directional connectivity between brain regions during the transition from interictal to ictal states.

Ictal and interictal SPECT with 99m Tc-HMPAO in presurgical epilepsy. II: Methodological considerations onhyper- and hypoperfusion.

Single-photon emission computed tomography (SPECT) with the tracer 99m Tc-HMPAO is a method to visualize the cerebral hyperperfusion during an epileptic seizure and thus localize the epileptogenic zone and seizure propagation. Subtraction of interictal from Ictal SPECT Co-registered to MRI (SISCOM) visualizes areas with relative increases in cerebral blood flow. The purpose of this retrospective study is to explore the added value of visualizing areas of hypoperfusion as well as hyperperfusion, so-called reversed SISCOM. Fifty-six patients operated for epilepsy who had been investigated with SISCOM were included in the analysis. The patients were divided into two groups based on seizure duration after tracer injection, above or below 30 s. The preoperative SISCOM description was compared to the area of resection and given a concordance score. The 56 SISCOM were recalculated visualizing also areas of hypoperfusion and again compared to the site of resection using the same scale of concordance. The reversed SISCOM were categorized into three subgroups: "Altered Conclusion," "Confirmed Conclusion," and "Adds Nothing." If an area of hyperperfusion had an area of hypoperfusion in close proximity, it was re-interpreted as noise, thus possibly altering the conclusion. If the areas of hypoperfusion were in the opposite hemisphere it was interpreted as confirming factor. Further the concordance scores from conventional SISCOM and reversed SISCOM was compared to surgical outcome to explore the difference in sensitivity, positive predictive value (PPV), and odds ratio. In approximately half of the cases reversed SISCOM added additional value, meaning either altered the conclusion or confirmed the conclusion. The sensitivity, PPV, and odds ratio was also better in the subgroup of long, >30 s seizure duration after injection, and got worse in the group with short, <30 s seizure duration after injection. Adding reversed SISCOM performed better than conventional SISCOM at predicting good surgical outcome.

Ex-SPECTing Success: predictors of successful SPECT radiotracer injection during pre-surgical video-EEG admissions.

To determine predictors of successful ictal Single Photon Emission Computed Tomography (SPECT) injections during Epilepsy Monitoring Unit (EMU) admissions for patients undergoing presurgical evaluation for drug resistant focal epilepsy. In this retrospective study, consecutive EMU admissions were analysed at a single centre between 2019-2021. All seizures that occurred during the admission were reviewed. 'Injectable seizures' occurred during hours when the radiotracer was available. EMU-level data were analysed to identify factors predictive of an EMU admission with a successful SPECT injection (successful admission). Seizure-level data were analysed to identify factors predictive of an 'injectable seizure' receiving a SPECT injection during the ictal phase (successful injection). A multivariate generalised linear model was used to identify predictive variables. 125 EMU admissions involving 103 patients (median 37 years, IQR27.0-45.5) were analysed. 38.8% of seizures that were eligible for SPECT (n=134) were successfully injected; this represented 17.4% of all seizures (n=298) that occurred during admission. Unsuccessful admissions were most commonly due to a lack of seizures during EMU-SPECT (19.3%) or no 'injectable seizures' (62.3%). Successful EMU-SPECT was associated with baseline seizure frequency >1 per week (95%CI 2.1-3.0, p <0.001) and focal PET hypometabolism (95%CI 2.0-3.7, p <0.001). On multivariate analysis, the only factor associated with successful injection was patients being able to indicate they were having a seizure to staff (95%CI 1.0-4.4, p=0.038). Completing a successful ictal SPECT study remains challenging. Baseline seizure frequency of >1 per-week, a PET hypometabolic focus and a patient's ability to indicate seizure onset were identified as predictors of success. These findings may assist EMUs in optimising their SPECT protocols, patient selection, and resource allocation.

AbstractBackgroundPositron emission tomography (PET) and single-photon emission computed tomography (SPECT) are applied in epilepsy mostly during presurgical assessment. Nonlesional focal epilepsy is very challenging for presurgical evaluation in cases of refractory epilepsy.ObjectivesWe aimed to investigate the contribution of PET and SPECT to focus localization in nonlesional epilepsy.MethodsThe basic principles of SPECT and PET including different tracers were reviewed. The literature and the most recent publications are discussed in view of findings in nonlesional epilepsy.ResultsIctal SPECT shows a high sensitivity of over 80% for ictal onset zone in nonlesional epilepsy. Postprocessing with subtraction ictal SPECT co-registered with MRI (SISCOM) or statistical parametric mapping are the gold standard. Histopathological findings in nonlesional epilepsies with concordant ictal SPECT imaging show focal cortical dysplasia in up to 75% of cases. Ictal injection is unsuccessful in approximately 20% of cases and the procedure is very time consuming and labor intensive. Fluorodeoxyglucose (FDG)-PET in nonlesional epilepsy shows variable sensitivity of 46–76%. As with ictal SPECT, it benefits from postprocessing of co-registration with magnetic resonance imaging (MRI) or even statistical parametric mapping. Hybrid PET/magnetic resonance imaging (MRI) scanners provide additional benefits for identifying lesions. Other PET tracers (11C‑Flumazenil (FMZ), α-11C‑methyl-L-tryptophane (AMT)) are clinically applied in special cases only. Both procedures contribute complementary information in multimodal imaging diagnostics.ConclusionsIctal SPECT and PET are optional diagnostic tools for presurgical assessment in nonlesional epilepsies. They may reverse nonlesional to lesional epilepsy status and identify nonlesional epileptic foci with a fair chance of seizure freedom after epilepsy surgery.

Ictal Single Photon Emission Computed Tomography (SPECT) In Patients with Focal Aware Seizure without Ictal Electroencephalographic Changes: A Single Center Experience (P2-1.012)

<h3>Objective:</h3> N/A <h3>Background:</h3> There is currently limited literature on the localization value of ictal SPECT tracer injection during patient-reported typical focal aware seizures (auras) that lack electrographic changes. One study reported that ictal SPECT studies in isolated auras lack reliability, with concordant localization with the seizure onset zone obtained in 5%, and lateralization in 35% of cases. In another study, localizing information of self-injection of ictal SPECT was better in three patients and obviated the need for intracranial monitoring in one patient. We present a study in which tracer injection was administered for patient-reported typical focal aware seizures, where the subsequent analysis of ictal and interictal SPECT helped inform further management. <h3>Design/Methods:</h3> We performed a retrospective review of cases who underwent ictal SPECT between 2020 to 2022 at University of Kentucky hospital. Cases with radiotracer injection during patient-reported auras lacking electrographic changes were identified. Timing of injection from onset of reported aura, hyperperfusion changes on ictal SPECT, and the outcome of imaging on further management of refractory epilepsy were noted. <h3>Results:</h3> 20 patients underwent ictal SPECT, of which 7 met our inclusion criteria. None of the 7 cases had electrographic correlate with aura on scalp EEG. Time of tracer injection ranged from 1–17 seconds following onset of aura. The ictal SPECT changes correctly localized the seizure onset zone in five patients. This was followed by intracranial monitoring and subsequent temporal lobectomy in three patients, interventricular ablation of epileptogenic focus in one patient, and Responsive Neurostimulation implant in one patient. One patient was managed medically, and one is ongoing pre-surgical evaluation. <h3>Conclusions:</h3> Ictal SPECT with radiotracer injection during typical focal aware seizures (patient reported auras) without ictal electrographic changes is helpful in the localization of the seizure onset zone. Accuracy of predicting surgical outcomes using interictal electrical source imaging in patients with MRI-negative intractable epilepsy

ObjectiveWe investigated the accuracy of interictal electrical source imaging (II-ESI) in localizing the epileptogenic zone in MRI-negative epilepsy patients who underwent epilepsy surgery. We also aimed to compare II-ESI's utility with other presurgical investigations and its role in guiding intracranial electroencephalography (iEEG) planning. MethodsWe retrospectively reviewed the medical records of patients with operated MRI-negative intractable epilepsy at our center between 2010 and 2016. All patients underwent video electroencephalography (EEG) monitoring, high-resolution MRI, 18 fluorodeoxyglucose positron emission tomography (FDG-PET) scans, ictal single-photon emission computed tomography (SPECT) and intracranial EEG (iEEG) monitoring. We computed II-ESI following the visual identification of interictal spikes, and outcomes were determined using Engel’s classification at 6 months after surgery. ResultsAmong 21 operated MRI-negative intractable epilepsy patients, 15 had sufficient data for II-ESI analysis. Of these, nine patients (60%) showed favorable outcomes corresponding to Engle's classification I and II. The localization accuracy of II-ESI was 53%, which was not significantly different from those of FDG-PET and ictal SPECT (47% and 45%, respectively). Among the patients, iEEG did not cover the areas suggested by II-ESIs in seven cases (47%). In two of those patients (29%), the regions indicated by II-ESIs were not resected, resulting in poor surgical outcomes. ConclusionThis study demonstrates that the localization accuracy of II-ESI was comparable to ictal SPECT and the brain FDG-PET scan. II-ESI is a simple, noninvasive method for evaluating the epileptogenic zone and guiding iEEG planning in patients with MRI-negative epilepsy.

Presurgical evaluation algorithm of epilepsy and its practical significance

Objective: Evaluation of the efficacy of surgical treatment for different forms of epilepsy using individualized complex presurgical diagnostic algorithms.Materials and Methods: The study included 104 patients with localized unifocal forms of epilepsy and 48 patients with multifocal epilepsy. Among them, 68 (44.7%) were children and 84 (55.3%) were adults. Patients' age ranged from 2 to 63 years. All patients had frequent epileptic seizures and unsatisfactory results of medical treatment. Recurrent status epilepticus or serial seizures in the anamnesis were presented in 74 cases (48.7%). Medication for all patients before surgery treatment included from 1 to 8 antiepileptic drugs. Postsurgical follow-up lasted from 4 months to 16 years. Main examinations included: complete medical history, types of epileptic seizures, EEG or video-EEG, brain MRI. Additional examinations included: MRI - tractography and functional MRI, single-photon emission computed tomography (SPECT), positron emission tomography, subtraction ictal SPECT co-registered to MRI, and intraoperative corticography.Results: The use of presurgical evaluation algorithm made it possible to localize epileptogenic foci (EF) in all patients. The method of surgical intervention was chosen only of the received preoperative data. Seizures free (Engel Ia) was achieved in 67 (44.1%) patients. Single focal or nocturnal seizures (Engel Ib-c) were reported in 37 (24.3%) patients. Significant regression of the number of epileptic seizures (Engel II) was achieved in 28 (18.4%) cases, and moderate regression (Engel III) - in 12 (23.1%) patients. Absence of positive dynamics after surgery (Engel IV) occurred in 7 (4.6%) patients. Surgical complications occurred in 13 (8.6%) patients.Conclusions: Presurgical evaluation algorithm helped localize unifocal and multifocal EF, which allows choosing a surgical technique, accurately planning the stategy of intervention and carrying it out safely. In cases of surgical impact on EF, it is possible to achieve complete seizure control.

The diagnostic value of ictal SPECT-A retrospective, semiquantitative monocenter study.

Ictal single photon emission computed tomography (SPECT) can be used as an advanced diagnostic modality to detect the seizure onset zone in the presurgical evaluation of people with epilepsy. In addition to visual assessment (VSA) of ictal and interictal SPECT images, postprocessing methods such as ictal-interictal SPECT analysis using SPM (ISAS) can visualize regional ictal blood flow differences. We aimed to evaluate and differentiate the diagnostic value of VSA and ISAS in the Bonn cohort. We included 161 people with epilepsy who underwent presurgical evaluation at the University Hospital Bonn between 2008 and 2020 and received ictal and interictal SPECT and ISAS. We retrospectively assigned SPECT findings to one of five categories according to their degree of concordance with the clinical focus hypothesis. Seizure onset zones could be identified more likely on a sublobar concordance level by ISAS than by VSA (31% vs. 19% of cases; OR=1.88; 95% Cl [1.04, 3.42]; P=0.03). Both VSA and ISAS more often localized a temporal seizure onset zone than an extratemporal one. Neither VSA nor ISAS findings were predicted by the latency between seizure onset and tracer injection (P=0.75). In people who underwent successful epilepsy surgery, VSA and ISAS indicated the correct resection site in 54% of individuals, while MRI and EEG showed the correct resection localization in 96% and 33% of individuals, respectively. It was more likely to become seizure-free after epilepsy surgery if ISAS or VSA had been successful. There was no MR-negative case with successful surgery, indicating that ictal SPECT is more useful for confirmation than for localization. The results of the most extensive clinical study of ictal SPECT to date allow an assessment of the diagnostic value of this elaborate examination and emphasize the importance of postprocessing routines.

Relation of Brain Perfusion Patterns to Sudden Unexpected Death Risk Stratification: A Study in Drug Resistant Focal Epilepsy.

To explore the role of the interictal and ictal SPECT to identity functional neuroimaging biomarkers for SUDEP risk stratification in patients with drug-resistant focal epilepsy (DRFE). Twenty-nine interictal-ictal Single photon emission computed tomography (SPECT) scans were obtained from nine DRFE patients. A methodology for the relative quantification of cerebral blood flow of 74 cortical and sub-cortical structures was employed. The optimal number of clusters (K) was estimated using a modified v-fold cross-validation for the use of K means algorithm. The two regions of interest (ROIs) that represent the hypoperfused and hyperperfused areas were identified. To select the structures related to the SUDEP-7 inventory score, a data mining method that computes an automatic feature selection was used. During the interictal and ictal state, the hyperperfused ROIs in the largest part of patients were the bilateral rectus gyrus, putamen as well as globus pallidus ipsilateral to the seizure onset zone. The hypoperfused ROIs included the red nucleus, substantia nigra, medulla, and entorhinal area. The findings indicated that the nearly invariability in the perfusion pattern during the interictal to ictal transition observed in the ipsi-lateral putamen F = 12.60, p = 0.03, entorhinal area F = 25.80, p = 0.01, and temporal middle gyrus F = 12.60, p = 0.03 is a potential biomarker of SUDEP risk. The results presented in this paper allowed identifying hypo- and hyperperfused brain regions during the ictal and interictal state potentially related to SUDEP risk stratification.

Common Ictal and Interictal Perfusion Patterns: A Window into the Epileptogenic Network and SUDEP Mechanism in Drug-Resistant Focal Epilepsy.

Focal epilepsies have been described as a network disease. Noninvasive investigative techniques have been used to characterize epileptogenic networks. This study aimed to describe ictal and interictal cortical and subcortical perfusion patterns using single- photon emission computed tomography (SPECT) in patients with drug-resistant epilepsy (DRE). Thirty-five interictal-ictal SPECT scans were obtained from 15 patients with DRE. A methodology was developed to get a relative perfusion index (PI) of 74 cortical and sub-cortical brain structures. K-means algorithm, together with modified v-fold cross-validation, was used to identify the two regions of interest (ROIs) that represent hypoperfused and hyperperfused areas. In common with the individual analysis, the statistical analysis evidenced that the hyperperfusion ROIs resulting from group analysis during interictal and ictal involved mainly the cingulate gyrus, cuneus, lingual gyrus, and gyrus rectus as well as the putamen. ROIs hypoperfused included the red nucleus, the substantia nigra, and the medulla. The medians of the group analysis of the hypoperfusion and hyperperfusion ROIs were 0.601-0.565 and 1.133-1.119 for the ictal and interictal states, correspondingly. A group of mostly cortical structures involved in the hyperperfused ROIs in both interictal and ictal states showed no change or negative change in the transition from interictal to ictal state (mean change of -0.002). On the other hand, the brain stem, basal ganglia, red nucleus, and thalamus revealed a mean global change of 0.19, indicating a mild increase in the PI. However, some of these structures (red nucleus, substantia nigra, and medulla oblongata) remained hypoperfused during the interictal to ictal transition. The methodology employed made it possible to identify common cortical and subcortical perfusion patterns not directly linked to epileptogenicity, for a better epileptogenic network and sudden unexpected death (SUDEP) mechanism in DRE.

EEG Driven Autonomous Injection System For An Epileptic Neuroimaging Application.

Seizure episodes are frequently observed for adults and children suffering from medically refractory epilepsy and the events remain debilitating unless treated with a more comprehensive approach. Ictal perfusion studies with single-photon emission computed tomography (SPECT) is one of the non invasive imaging modality that has been extensively used to adequately localize the seizure focus. Current practices include the tracer injection within a short time interval at the onset of seizure to generate desirable SPECT scan quality with accurate information on foci region. However, the onset of a seizure is a highly unpredictable event and also with added subclinical events, the overall procedure makes it difficult to administer the tracer manually within the ideal time frame.Hence a complete autonomous injection of radioactive tracer element without manual intervention is expected to offer a highly accurate epileptical focus region and aids in further management of the patient. Electroencephalogram (EEG) physiological signals in the preictal phase contain sufficient indicators to predict the seizure event. The proposed injection system works on the seizure prediction model from the EEG signals to release the dosage, making the system completely autonomous in action. The accuracy of the prediction model based on the publicly available seizure embedded EEG datasets was designed to achieve 94% accuracy, and the model was deployed on an edge system. The syringe based injection system was characterized to emulate dosage release action with minimum volumetric error, and low injection time, on predicting seizure Ictal event from the EEG signal. The proposed system is a step towards developing an autonomous injection system for epileptic neuroimaging applications in hospital settings.Clinical relevance- Autonomous injection of tracer dosage for obtaining accurate and high quality Ictal SPECT scan results is preferred over the manual operation in clinical and hospital residential settings as a part of pre-surgical evaluations. The EEG signal based early prediction of seizure ensures adequate time for radioactive tracer element to reach the brain cells and eventually helps to accurately localize the onset region of seizure in the brain. The EEG driven automated injection system for the noninvasive Ictal SPECT method is clinically important as a pre-surgical evaluation in MRI negative or discordant cases for further surgical actions.

The effect of injection time on rates of epileptogenic zone localization using SISCOM and STATISCOM

BackgroundThe identification of hyperperfusion on ictal single-photon emission computed tomography (SPECT) scan is a technique for the localization of the epileptogenic zone (EZ) in patients with focal epilepsy undergoing presurgical evaluation. The accuracy of this technique has been improved by subtraction from an interictal image and coregistration with magnetic resonance imaging (MRI) (subtraction ictal SPECT coregistered to MRI (SISCOM)), and subsequently by the development of Statistical Ictal SPECT Co-registered to MRI (STATISCOM) which is reported to further improve localization accuracy by statistically accounting for random variation between images. However, the use of ictal SPECT is limited by the necessity for rapid injection of the radiotracer. The purpose of this study was to investigate the effect of tracer injection time on EZ localization rates using both STATISCOM and SISCOM. MethodsConsecutive patients with drug-resistant focal epilepsy who had an ictal SPECT scan while admitted to the video-electroencephalography (EEG) monitoring unit at the Royal Melbourne Hospital, Victoria, Australia, and a subsequent interictal scan, between 2009 and 2017 were included. The information collected included age, sex, seizure type, epilepsy diagnosis, and injection time. Statistical Ictal SPECT Co-registered to MRI and SISCOM images were generated and reviewed by two blinded reviewers. The rates of potential localization of the EZ, and the agreement with the EEG, were determined for each scan. Localization rates were compared between ictal scans with different radiotracer injection time windows (<30 s, 30–45 s, 45–60 s, 60–90 s, 90–120 s, >120 s). ResultsSeventy patients (male = 32, 16–67 years) were included in the study. Overall agreement between the primary raters was moderate for STATISCOM (k = 0.44) and SISCOM (k = 0.57). The ability of SPECT to localize the potential EZ was 69% (48/70) for STATISCOM and 59% (41/70) for SISCOM. Injection time was not associated with the rate of localizing the potential EZ for STATISCOM (p = 0.64), whereas for SISCOM there was a trend that shorter injection times were associated with better ability to localize the potential EZ (p = 0.06).Agreement between SPECT and video-EEG data was 54% (38/70) for STATISCOM and 39% (27/70) for SISCOM. Statistical Ictal SPECT Co-registered to MRI did not show any difference of agreement across injection time groups (p = 0.42) whereas SISCOM showed better agreement with video-EEG data in the earlier injection time groups (p = 0.02). No differences in agreement between SPECT and video-EEG data were seen between patients with and without MRI lesions for either STATISCOM or SISCOM. Statistical Ictal SPECT Co-registered to MRI showed significantly better agreement for temporal than extratemporal seizures, with no difference of agreement between early (<45 s) and late (>45 s) injections. ConclusionStatistical Ictal SPECT Co-registered to MRI showed overall higher agreement rates with EZ localization by video-EEG than SISCOM, which was not affected by the injection times. Statistical Ictal SPECT Co-registered to MRI may provide localizing information for “late” injections where visual reads and SISCOM are inconclusive.

Surgical Outcome in Extratemporal Epilepsies Based on Multimodal Pre-Surgical Evaluation and Sequential Intraoperative Electrocorticography

Objective: to present the postsurgical outcome of extratemporal epilepsy (ExTLE) patients submitted to preoperative multimodal evaluation and intraoperative sequential electrocorticography (ECoG). Subjects and methods: thirty-four pharmaco-resistant patients with lesional and non-lesional ExTLE underwent comprehensive pre-surgical evaluation including multimodal neuroimaging such as ictal and interictal perfusion single photon emission computed tomography (SPECT) scans, subtraction of ictal and interictal SPECT co-registered with magnetic resonance imaging (SISCOM) and electroencephalography (EEG) source imaging (ESI) of ictal epileptic activity. Surgical procedures were tailored by sequential intraoperative ECoG, and absolute spike frequency (ASF) was calculated in the pre- and post-resection ECoG. Postoperative clinical outcome assessment for each patient was carried out one year after surgery using Engel scores. Results: frontal and occipital resection were the most common surgical techniques applied. In addition, surgical resection encroaching upon eloquent cortex was accomplished in 41% of the ExTLE patients. Pre-surgical magnetic resonance imaging (MRI) did not indicate a distinct lesion in 47% of the cases. In the latter number of subjects, SISCOM and ESI of ictal epileptic activity made it possible to estimate the epileptogenic zone. After one- year follow up, 55.8% of the patients was categorized as Engel class I–II. In this study, there was no difference in the clinical outcome between lesional and non lesional ExTLE patients. About 43.7% of patients without lesion were also seizure- free, p = 0.15 (Fischer exact test). Patients with satisfactory seizure outcome showed lower absolute spike frequency in the pre-resection intraoperative ECoG than those with unsatisfactory seizure outcome, (Mann– Whitney U test, p = 0.005). Conclusions: this study has shown that multimodal pre-surgical evaluation based, particularly, on data from SISCOM and ESI alongside sequential intraoperative ECoG, allow seizure control to be achieved in patients with pharmacoresistant ExTLE epilepsy.

SPECT activation patterns in psychogenic non-epileptic seizures in temporal lobe epilepsy patients

IntroductionThe diagnosis of psychogenic non-epileptic seizures (PNES) can often be challenging. When video-electroencephalography (EEG) is not conclusive, single-photon emission computed tomography (SPECT) can be useful by quantifying changes in regional cerebral blood flow (rCBF). MethodsWe conducted a retrospective case-control study in adult patients with pharmacoresistant temporal lobe epilepsy (TLE). Those patients with an ictal SPECT obtained during an event finally diagnosed as PNES were included as cases (PNES+). The control group consisted of patients with TLE without PNES (PNES-). Clinical episodes were analysed and classified according to PNES subtypes. Subtraction ictal SPECT coregistered to MRI (SISCOM) analysis was performed for the detection of areas with significant changes in perfusion compared to individual interictal studies. Group comparisons in SPM12 included paired t-tests of ictal vs. interictal studies in each group of temporal lobe seizures and PNES events. ResultsTen patients with TLE and PNES were included. We found no patterns of regional hyperperfusion typical of TLE seizures during the PNES events. In two of these cases, an ictal SPECT during a confirmed epileptic seizure was also obtained, showing antero-mesial temporal lobe hyperperfusion. Group comparisons between ictal and interictal SPECTs showed increased rCBF in the temporal lobe with reduced perfusion in the default mode network areas and cerebellum during temporal lobe seizures in PNES- patients and decreased perfusion restricted to the posterior parietal cortex without significant rCBF increases in PNES events. ConclusionsIctal SPECT can be a helpful tool to characterize rCBF changes in PNES and for differential diagnosis with seizures in TLE patients.

MNI SISCOM: an Open-Source Tool for Computing Subtraction Ictal Single-Photon Emission CT Coregistered to MRI.

Subtraction ictal single-photon emission computed tomography (SPECT) coregistered to MRI (SISCOM) is a well-established technique for quantitative analysis of ictal vs interictal SPECT images that can contribute to the identification of the seizure onset zone in patients with drug-resistant epilepsy. However, there is presently a lack of user-friendly free and open-source software to compute SISCOM results from raw SPECT and MRI images. We aimed to develop a simple graphical desktop application for computing SISCOM. MNI SISCOM is a new free and open-source software application for computing SISCOM and producing practical MRI/SPECT/SISCOM image panels for review and reporting. The graphical interface allows any user to quickly and easily obtain SISCOM images with minimal user interaction. Additionally, MNI SISCOM provides command line and Python interfaces for users who would like to integrate these features into their own scripts and pipelines. Comparative contribution of magnetoencephalography (MEG) and single-photon emission computed tomography (SPECT) in pre-operative localization for epilepsy surgery: A prospective blinded study

PurposeThe aim of this study was to compare the diagnostic value and accuracy of ictal SPECT and inter-ictal magnetoencephalography (MEG) in localizing the site for surgery in persons with drug resistant epilepsy. MethodThis was a prospective observational study. Patients expected to undergo epilepsy surgery were enrolled consecutively and the localization results from different imaging modalities were discussed in an epilepsy surgery meet. Odds ratio of good outcome (Engel I) were calculated in patients who underwent surgery in concordance with MEG and SPECT findings. Post-surgical seizure freedom lasting at least 36 months or more was considered the gold standard for determining the diagnostic output of SPECT and MEG. ResultsMEG and SPECT were performed in 101 and 57 patients respectively. In 45 patients SPECT could not be done due to delay in injection or technical factors. The accuracy of MEG and SPECT in localizing the epileptogenic zone was found to be 74.26 % and 78.57 % respectively. The diagnostic odds ratio for Engel I surgical outcome was reported as 2.43 and 5.0 for MEG and SPECT respectively. The diagnostic odds ratio for MEG in whom SPECT was non-informative was found to be 6.57 [95 % CI 1.1, 39.24], although it was not significantly associated with good surgical outcome. MEG was useful in indicating sites for SEEG implantation. ConclusionSPECT was found to be non-informative for most patients, but reported better diagnostic output than MEG. MEG may be a useful alternative for patients in whom SPECT cannot be done or was non-localizing.

Injection of prophylactic lorazepam versus antiseizure drugs on the localization value of ictal SPECT studies and treatment-emergent adverse events: A single-center prospective study

ObjectiveWe aimed to examine the impact of resumption of home antiseizure drugs alone (ASD−) compared with adjunct administration of scheduled intravenous (IV) lorazepam 2 mg every 6 h (ASD+) following ictal single-photon emission computed tomography (SPECT) injection on the localization value of SPECT studies and treatment-emergent adverse events (TEAEs). MethodsWe conducted a prospective study at Mayo Clinic inpatient epilepsy monitoring unit (EMU) between January 2018 and May 2020 in Jacksonville, Florida. The ASD− and ASD+ groups were compared for concordance of SPECT studies with the epilepsy surgical conference (ESC) consensus or intracranial electroencephalography (icEEG) findings as reference. Treatment-emergent adverse events, obtained from surveys at 24 h and one week postictal SPECT injection, were also compared between both groups. ResultsTwenty-two consecutive patients with temporal (eight patients, 36%) and extratemporal (14 patients, 64%) epilepsy were included: 12 ASD+ and 10 ASD−. The two groups were well matched with regard to clinical and ictal SPECT injection characteristics including the occurrence of seizure between ictal and interictal SPECT injections. The localization value of SPECT studies was similar in the two groups. Patients in the ASD+ group reported higher rates of dizziness and excessive sedation at 24 h (p-value = 0.008). Fourteen patients (64%) underwent icEEG monitoring. For the entire cohort, the localization concordance of SPECT analysis by statistical parametric mapping (SPM) was superior to raw ictal SPECT (p-value = 0.003) and subtraction ictal SPECT coregistered to magnetic resonance imaging (MRI) (SISCOM; p-value = 0.021). Eventually, seven patients (31.8%) underwent resective brain surgery of whom four (57.1%) became seizure-free (median follow-up = 22 months). ConclusionsOur findings suggest that resuming home ASDs without the addition of scheduled IV lorazepam following inpatient ictal SPECT injection is equally efficacious for seizure onset zone (SOZ) localization on SPECT studies, especially SPM. This approach is also associated with fewer transient TEAEs and lower financial cost with no difference in preventing seizure between ictal and interictal SPECT injections.

Visual memory recall‐induced focal seizures: A case report

AbstractAn 82‐year‐old woman presented with a complaint of sudden difficulty in chanting the Heart Sutra because visual hallucinations of Tetris‐like color blocks disturbed the image of the Sutra in her mind. Neuropsychological examination revealed transient impairment of visual memory recall with hallucinations. Physical examination showed no abnormalities except mild hypertension. She was alert, oriented with normal motor, sensory, and cerebellar function. She was diagnosed with mid‐temporal lobe seizures based on electroencephalography and single‐photon emission computed tomography (SPECT) findings. Subtraction ictal SPECT co‐registered to MRI (SISCOM) examination was useful in detecting the seizure focus. Symptoms resolved completely after the administration of antiepileptic drugs. This case report provides new insights into the mechanism of visual memory recall.

Utility of single-photon emission computed tomography (SPECT) in presurgical evaluation of children: A single-center experience

ObjectiveSingle-photon emission computed tomography (SPECT) is an ancillary noninvasive test commonly used to identify the epileptogenic zone. However, its proper utilization may be limited depending on the resources available at each center. This study aimed to investigate the utility of SPECT in presurgical evaluation of children at our center. MethodsThis was an observational retrospective study in 150 children who were admitted to the Epilepsy Monitoring Unit for presurgical evaluation between 2012 and 2019. The utility of interictal and ictal SPECT in different clinical cohorts was analyzed. ResultsOnly interictal SPECT was performed on 87 patients (58 %), while ictal SPECT was performed on 62 (41.3 %), and interictal SPECT alone was considered of low diagnostic value. Ictal SPECT was unremarkable in 27 of 62 patients and abnormal in 35. Ictal SPECT was localized to the temporal lobe in 62.8 % and to the extratemporal lobe in 25.7 % of the patients and lateralized to one hemisphere in 11.4 % of the patients. In the abnormal SPECT group, ictal SPECT was considered unnecessary in 7/35 (20 %) patients with a single lesion and 3/35 (8.5 %) patients with a hemispheric lesion. In the remaining 25 patients, surgery was recommended more frequently than invasive EEG monitoring (IEM) for diffuse lesion cases (P = 0.03), while IEM was recommended more frequently than surgery for MRI-negative cases (P = 0.03), and in this group none of the MRI-negative patients underwent surgery. In our entire cohort, epilepsy surgery was performed on 24.4 % of the patients, 64 % with a single lesion, 7.6 % with a hemispheric lesion, 25.6 % with a diffuse lesion, and only one MRI-negative patient (2.5 %). Surgery was performed in 48.7 % of single lesion cases and 20.5 % of diffuse lesion cases with either unremarkable or no ictal SPECT. Engel class I outcome was achieved in 62 % and class II outcome in 33 % of the patients. In the single lesion etiology, 72 % (18/25) patients achieved excellent outcome and within this group, 22 % (4/18) patients had a positive ictal SPECT whereas 78 % (14/18) patients either did not get an ictal SPECT or it was unremarkable. SignificanceBased on our findings, we suggest carefully selecting patients for SPECT imaging to improve its utility and prevent overutilization and potential harm to children.

Does Tc-99m ECD ictal brain SPECT have incremental value in localization of epileptogenic zone and predicting postoperative seizure freedom in cases with discordant video electroencephalogram and MRI findings?

Localization of epileptogenic focus in drug-refractory epilepsy using Tc-99m ethylene cystine dimer (ECD) brain single photon emission computed tomography (SPECT) is less studied in patients with discordant findings on video electroencephalogram (VEEG) and MRI. The study was done to evaluate brain SPECT for epileptogenic focus localization and postoperative seizure freedom. Epilepsy patients with discordant VEEG and MRI findings underwent brain SPECT at ictal and interictal phases. Various groups unilateral/bilateral mesial temporal sclerosis (MTS), solitary and multifocal lesional, nonlesional epilepsy were studied for localization of epileptogenic focus and postoperative seizure freedom (>2 years) using Engels classification. Reasons for nonoperability was evaluated in nonoperated group. SPECT could localize epileptogenic focus in 49/67 (73.13%) and guided surgery in 19/33 (57.57%) patients in operated group. SPECT was useful in 12 (46.12%) of unilateral (2)/bilateral (10) MTS. Postoperative seizure freedom of Engels Class I and II in 22 (66.67%), III in six (18.2%) and IV in one patient based on SPECT findings (P = 0.0086). Overall sensitivity and specificity were 79.3% and 85.7%, respectively. SPECT could localize epileptogenic focus in 23/34 (67.64%) patients in nonoperated group; 10 (29.41%) patients refused for surgery and no epileptogenic focus was localized in the rest of 14 (41.2%). Ictal SPECT showed incremental value and was found necessary for epileptogenic focus localization and subsequent surgery in unilateral/bilateral MTS in this study. Seizure freedom in patients undergoing epilepsy surgery based on ictal SPECT assistance was comparable to the surgical group not requiring ictal SPECT.
