Spontaneous posterior fossa hematoma is defined as an intra-parenchymal hemorrhage that occurs suddenly following a spontaneous rupture of an artery or an arteriole. The objective of this work is to evaluate the prognosis of this pathology in our context compared to the data in the literature. Method and results: Our work is a retrospective study of 05 cases collected over a period of 4 years. The symptomatology for which the patients consulted varied: intracranial hypertension syndrome, neurological deficit. The positive diagnosis is made by cerebral CT scan and in view of the suspicion of a ruptured arteriovenous malformation, we recommended an arteriography in all patients but it was only performed in 2 patients. The surgical or abstentionist management was much discussed on a case-by-case basis. The outcome was favorable in 2 patients, 1 of whom had some neurological disorders. The study of data from the literature show that among the risk factors we find age, state of consciousness, the volume of the hematoma, ventricular flooding and whatever the therapeutic decision the extremely serious forms are always lethal.
