Construction of a Physical and Medical Care Integrated Model for the Elderly in the Community Based on Artificial Intelligence and Machine Learning.

With the increasingly serious population aging, economic system transformation, and social transformation, the elderly have an increasingly strong demand for the elderly care service industry, but the development of China's elderly care service industry started late, and there are still many problems. For how to build a better elderly care service model, under the current situation of shortage of elderly care resources, community elderly care may become a solution. By analyzing the status quo, existing problems and development trends of home-based care services in Hangzhou's community, and learning from foreign experience, it explores the establishment of a “integrated physical, medical, and nursing care” community health management model for the elderly and provides a perfect solution for the development of Hangzhou's community elderly care services countermeasures and suggestions for mode operation. Based on the evaluation, problem analysis, and empirical research on the current situation of home care services in Hangzhou communities, this paper draws on relatively mature community care practices, proposes to build a community elderly health management model that combines medical care and care, and improves and innovates the overall design and medical care of the elderly service model measures for the health management of the elderly in the integrated community. The experimental results of this study show that, according to the demand for elderly care services, elderly people in need of care account for 86.6%, 79.5%, and 68.4% of the elderly population, especially in the areas of medical care, life care, and housekeeping services.

Creation of a Five-Level Model of Professional Development Management of a Specialist in the Format of a Decimal Model of Good Human Health Management

The purpose of the study is to create a Five-Level Model of full professional development (PD) management, in the format of a decimal model of management of good human health (GHH) formation. To achieve the determined goal, the concepts of «occupational human health» and «comprehensive professional development» determining full compliance of a person’s professional activity with achievements in modern science and practice were defined. This is the highest level of professional development to achieve goals in a certain type of professional activity as a result of fulfilling the professional potential. Thereafter, to meet the objectives of the study, the last tenth letter «T» of the word «management» was used in the Conceptual decimal model of full innovative GHH management. It represents the highest, tenth level of the professional activity system, consisting of five types: work — labour — business — game — employment. These five types of human profess ional activity are at the core of the Five-Level Model and are placed at five hierarchical levels, in accordance with their relevance. To create the Five-Level Model, single-type tables were built that summarize the characteristics of each of the five types of professional activity: work — labour — business — game — employment and characteristics of the corresponding subjects. Reasonable conclusions have been drawn for each type of professional activity according to its hierarchical level in the Five-Level Model. This has helped to create the Five-Level Model of comprehensive PD management, in a format of the Conceptual model of full innovative management of GHH formation and determine that the Five-Level Model is a full hierarchical management model consisting of five types of professional activities in precise order according to their relevance: work, labour, business, game, service, which together fully cover the entire professional activity. It is proved that unique properties of the Five-Level Model allow for its regular use for full professional development management in any environment, including Ukrainian society.

Analysis of Chitosan‐Based Nanosilver Composites for the Repair of Umbilical Cord Wounds in Newborns

The study aim reports a new umbilical cord wound repair material based on modified chitosan‐based nanosilver (CS‐HDH/Ag NPs). The repair of umbilical cord wound has always been the focus of neonatal departments. The study method chitosan, which is famous for its “biological glue,” is used as a modifier to compound silver nanoparticles with strong coordination and adsorption capacity of chitosan. It is worth noting that CS‐HDH/Ag NPs has a significant inhibitory effect on common bacterial infection and reduces the incidence of umbilical cord wound inflammation. The study problem wound bleeding and stump shedding and other factors make the periumbilical cord vulnerable to bacterial infection. For study results, the porous and lamellar structure endows the repair material with breathable properties and excellent antiadhesion, which greatly reduces the number of dressing changes and effectively protects the skin of newborns. CS‐HDH/Ag NPs provides a new model for efficient neonatal health management.

The Positive Effect of Mindfulness-Based Cancer Recovery on Cancer-Related Fatigue in a Transitional Health Management Model

Compared with general fatigue, cancer-related fatigue is characterized by rapid onset, high severity, substantial energy expenditure, long duration, unpredictability, and inability of relief. There has been no effective treatment for it so far, which greatly affects the patients' self-care ability and quality of life. It has been proved in previous studies that mindfulness-based cancer recovery (MBCR) can effectively alleviate the negative reactions of cancer patients such as fatigue, pain and fear during the treatment process and enhance the level of cancer recovery. In this article, we propose to intervene in cancer-related fatigue by resorting to mindfulness based on a "hospital-family transitional health management model". It is expected that this model will play a positive role in the adjuvant treatment of cancer-related fatigue because of its operability and low medical cost.

A Comprehensive Cognitive Health Management Model for Community-Dwelling Older Adults in Shanghai

To investigate whether a comprehensive cognitive health management model improves cognitive function among community-dwelling older adults in Shanghai, China. The comprehensive cognitive health management model included brain health screening, individualized brain health consultation and referrals, and annual follow-up monitoring. We compared 161 older adults (43 with MCI, 118 with normal aging) who received the health management model, to an equal size of control group who did not receive the model on their cognition and emotion using Montreal Cognitive Assessment Scale (MoCA) and Geriatric Depression Scale (GDS). Over one-year, participants with MCI in the intervention group showed a significant increase of MoCA score than their MCI counterparts in the control group. Participants with normal aging in the intervention group showed no differences in MoCA and GDS with those in the control group. The individualized cognitive health management model is promising to assist community-dwelling older adults with MCI to maintain brain health.

EAST Meets West Symposium: Community-Based Programs for Dementia Prevention and Support

This East Meets West Symposium presents cross-cultural evidence of community-based programs developed to address cognitive health needs of older adults or to support dementia family caregivers. It includes five studies from mainland China, Taiwan, and the U.S.A. The first study explored the adherence to computerized cognitive training to promote cognitive health among Chinese older adults using a qualitative approach. External encouragement, and self-awareness of performance improvement were found influential to older adults’ adherence. The second study spearheaded by American Association of Retired Persons (AARP) in the U.S.A shed further light on technology use in promoting brain health by comparing in-person versus online cognitive training to older Americans during the COVID-19 pandemic. The third study tested the effectiveness of a cognitive health management model for community-dwelling older adults including healthy and mild cognitively impaired (MCI) ones in Shanghai, China. Older adults with MCI who received the management service showed better cognition outcomes compared to their counterparts in the control group. The fourth study focused on a community program named “Carer Café”, developed to support dementia family caregiver in Taiwan. Participants (n=375) reported reduced stress and increased access to service referrals. The last study examined the stakeholders’ perceptions of dementia friendly initiatives (DFI) being implemented in U.S.A, mainland China, and Taiwan to support persons with dementia and family caregivers. The impacts of DFI made, challenges DFI faced, and strategies DFI used during COVID-19 pandemic were discussed. The discussant will comment on each article’s contributions and limitations in the context of literature.

Study on Health Management and Service Model of Chifeng Class 3A General Public Hospital

This study investigates the construction of a health management service mode based on large-scale comprehensive hospitals by analyzing health management service modes at home and abroad, that is, taking the Affiliated Hospital of Chifeng University as an example, establish a health examination hospital, undertake core tasks such as health service operation process, system, monitoring, and system formulation, and take the best medical resources. Attract social medical resources to collaborate and give health service mode of health status monitoring, evaluation, and intervention to health service objects, aided by advanced medical technology. To extend the traditional health management service with the hospital as the protagonist to health management with participation from family and society beyond the hospital, forming a closed-loop, rapid, and efficient health service management system. Thus, through graded quality services, to lessen the difficulties of patients seeing a doctor and to solve the problem of overcrowding and minor ailments in major hospitals.

Theoretical model for mental health management of project management practitioners in architecture, engineering and construction (AEC) project organizations

PurposeThis conceptual paper aims to develop a multi-level mental health management framework for project management practitioners (PMPs) in architecture, engineering and construction (AEC) project organizations through organizational design theories to extend current knowledge on mental health by revealing organizational, project and external environmental factors contributing to mental health management in AEC project organizations.Design/methodology/approachA systematic literature review was adopted to propose a theoretical model that integrated five organizational design theories: institutional theory, agency theory, resources-based theory (RBT), contingency theory and complexity theory.FindingsThe model reveals permanent organization, project organization and external environment factors for mental health management in AEC project organizations. It further proposed hypothetical interrelationships between elements of permanent organization, project organization, external environment and mental health management indicators to unravel the resultant effects of the interactions on mental health of PMPs.Originality/valueThis research contributes to the body of knowledge by developing a multi-level mental health management framework that identify and shows how combination permanent organization, project organization and external environment elements impact mental health of PMPs in AEC project organizations. It offers a model that offers guidance to practitioners on permanent organization and project organization management practices that can be implemented to improve mental health.

Future care for long-term cancer survivors: towards a new model

PurposeThe increase in the prevalence "long-term cancer survivor” (LCS) patients is expected to increase the cost of LCS care. The aim of this study was to obtain information that would allow to optimise the current model of health management in Spain to adapt it to one of efficient LCS patient care.MethodsThis qualitative study was carried out using Delphi methodology. An advisory committee defined the criteria for participation, select the panel of experts, prepare the questionnaire, interpret the results and draft the final report.Results232 people took part in the study (48 oncologists). Absolute consensus was reached in three of the proposed sections: oncological epidemiology, training of health professionals and ICT functions.ConclusionThe role of primary care in the clinical management of LCS patients needs to be upgraded, coordination with the oncologist and hospital care is essential. The funding model needs to be adapted to determine the funding conditions for new drugs and technologies.

Li-ion battery prognostic and health management through an indirect hybrid model

The immeasurable online battery capacity hampers the accurate State of Health (SOH) estimation and Remaining Useful Life (RUL) prediction because of complex battery ageing. To resolve the issues, an indirect hybrid model for online battery Prognostics and Health Management (PHM) is put forward. First, based on partial charging dada, two battery ageing features are extracted for the quantitative evaluation of battery capacity. Second, a Multi-Kernel Relevance Vector Machine (MKRVM) is optimized by Grey Wolf Optimizer (GWO) to determine the weights and kernel parameters of different kernel functions, then establish a mapping between battery ageing features and capacity. Third, the Complementary Ensemble Empirical Mode Decomposition (CEEMD) decomposes the measurable and historical ageing features into high- and low-frequency signals, then, the signals with different frequencies are modelled and predicted by Long Short-Term Memory Neural Network (LSTMNN) and Feedforward Neural Network (FNN), respectively. Finally, the real-time ageing features are utilized for SOH estimation and the predicted ageing features are used for RUL prediction through the GWO-based MKRVM. Furthermore, the performance of the proposed method is validated by an open-access battery ageing dataset. The results demonstrate that the indirect hybrid model has high flexibility and strong robustness on battery PHM.

Redesign of US Medical Schools: A Shift from Health Service to Population Health Management.

The integration of medical schools and clinical partners is effectively established through the formation of academic medical centers (AMCs). The tripartite mission of AMCs emphasizes the importance of providing critical clinical services, medical innovation through research, and the education of future health care leaders. Although AMCs represent only 5% of all hospitals, they contribute substantially to serving disadvantaged populations of patients, including an estimated 37% of all charity care and 26% of all Medicaid hospitalizations. Currently, most AMCs use a business model centered upon revenue generated from hospital services and/or practice plans. In the last decade, mounting financial demands have placed significant pressure on AMC finances because of the rising costs associated with complex clinical care and operating diverse graduate medical education programs. A shift toward population health-centric health care management strategies will profoundly influence the predominant forms of health care delivery in the United States in the foreseeable future. Health systems are increasingly pursuing new strategies to manage financial risk, such as forming Accountable Care Organizations and provider-sponsored plans to provide value-based care. Refocusing research and operational capacity toward population health management fosters collaboration and enables reintegration with hospital and clinical partners across care networks, and can potentially create new revenue streams for AMCs. Despite the benefits of population health integration, current literature lacks a blueprint to guide AMCs in the transformation toward sustainable population health management models. The purpose of this paper is to propose a modern conceptual framework that can be operationalized by AMCs in order to achieve a sustainable future.

The methodological approaches to classification of diseases of blood circulation system according to severity degree based on statistic data of population appealing for medical care

In The Russian Federation, increasing of life expectancy and decreasing of mortality related to diseases of circulatory system are the priorities of state policy. The purpose of study was to develop approaches to the classification of cardiovascular diseases by severity degree within the framework of development of general health management model based on health care activities at the regional level. The article describes methodology of calculating indices of cardiovascular diseases severity based on statistical data of appealability for out-patient, in-patient and emergency medical care. The set of balancing coefficients reflecting input of rate of accessing for various types of medical care, as well as aggravating input of concomitant pathology, based on expert evaluation of cardiologists involved is presented. On the basis of analysis of distribution of severity index in standard region of the Russian Federation, the system of criteria was developed to classify cardiovascular diseases (according to ICD-10 sub-classes) on four degrees of severity. The approbation of the proposed method demonstrated adequacy of the results obtained to judging of experts (cardiologists). So, in standard region of the Russian Federation, in the class of diseases of circulatory system (I00-I99), 79.6% of all cases are of first degree of severity, 8.6% of cases are of second degree of severity, 3.8% of cases are of third degree, and 8% of cases are of fourth degree. The methodology is unified and can be applied to classify entire spectrum of diseases by degree of severity. Besides, the proposed methodological approaches are suitable to be applied in population health management at the municipal, regional and national levels in the Russian Federation.

EXPERIENCE OF DEVELOPED COUNTRIES OF THE WORLD ON REGULATION OF SPORTS AND HEALTH SPHERE AND PROMOTION OF ITS DEVELOPMENT

The purpose of the article is to study the experience of developed countries in the regulation of sports and health in order to stimulate its development and adaptation to new challenges and threats. In the course of the research the methods of theoretical and comparative analysis were used to reveal the peculiarities of the American and European models of sports and health man-agement; statistical and graphical - to determine the economic role and trends in the sports and health industry in the world, the impact of the pandemic on income from sports. The article develops organizational and economic approaches to intensify the development of sports and recreation. Certain features of organizational models of management, sports legislation, financ-ing, possible tools to stimulate the development of sports and health in the developed world form a scientific basis for substantiating ways to intensify this area.

Quality Evaluation of Health Services Using the Kano Model in Two Hospitals in Peru.

Public–private partnerships (PPP) represent an alternative model of health management focused on improving the quality of health services, particularly in emerging countries. To date, a systematic method to improve the perceived quality of health services by healthcare users in Peru has not been established. The purpose of this study was to evaluate the quality of health services in two PPP hospitals in Peru using the Kano model. A prospective cross-sectional descriptive observational study was carried out through a health service satisfaction survey using the Kano model methodology, measuring six categories of attributes. A total of 250 users of the health services were surveyed in the two PPP hospitals, located in Lima and Callao, using non-probability convenience sampling. Of the 31 attributes evaluated by the patients, 27 (81%) were classified as having a one-dimensional-type attribute, 3 (10%) were reported as mandatory, and 1 (3%) was considered as inverse. These results suggest that the presence of most of the attributes evaluated was relevant to maintaining the level of user satisfaction and that the absence of these attributes generated dissatisfaction in the users. The results showed that the users’ evaluation of health services was multidimensional—namely, their evaluation was focused not only on the interaction space between the patient and medical personnel but also addressed other interaction services.

Use of Mental Health Interventions by Physiotherapists to Treat Individuals with Chronic Conditions: A Systematic Scoping Review.

Purpose: Physiotherapists work with people with chronic conditions and can act as catalysts for behavioural change. Physiotherapy has also seen a shift to a bio-psychosocial model of health management and interdisciplinary care, which is important in the context of chronic conditions. This scoping review addressed the research question "How do physiotherapists use mental health-based interventions in their treatment of individuals with chronic conditions?" Method: The Embase, MEDLINE, PsycINFO, and CINAHL databases were searched, and a variety of study designs were included. Data were categorized and descriptively analyzed. Results: Data were extracted from 103 articles. Low back pain (43; 41.7%) and non-specified pain (16; 15.5%) were the most commonly researched chronic conditions, but other chronic conditions were also represented. Outpatient facilities were the most common setting for intervention (68; 73.1%). A total of 73 (70.9%) of the articles involved cognitive-behavioural therapy, and 41 (40.0%) included graded exercise or graded activity as a mental health intervention. Conclusions: Physiotherapists can use a variety of mental health interventions in the treatment of chronic conditions. More detailed descriptions of treatment and training protocols would be helpful for incorporating these techniques into clinical practice.

Type 2 Diabetes Prediction using Gray Wolf Optimization Algorithm

Background: Increasing the number of diabetic patients and the ignorance of most of these patients aboutthe dangers arising from it is a challenge that threatens human lives.Materials and Method: In this paper, a new solution based on the Gray Wolf Optimization (GWO) algorithmfor predicting type 2 diabetes is presented. The main purpose of the proposed method is to increase theaccuracy of prediction and also to reduce the probability of getting stuck in local optimal points. In moredetail, the proposed method consists of two parts: 1- data preprocessing including data preparation andnoise cancellation and 2- data classification using gray wolf algorithm. The Pima Indians Diabetes dataset inMATLAB simulation environment was used to analyze the data and compare the research results.Results: The simulation results show that by adjusting the parameters of the gray wolf algorithm, about 6%better prediction accuracy is obtained than other researches.Conclusion: Also, for a more accurate evaluation of the proposed method, two other datasets have beenused for testing. The results of experiments show that the proposed model for health management in diabetesis effective.

Aircraft Fleet Health Monitoring with Anomaly Detection Techniques

Predictive maintenance has received considerable attention in the aviation industry where costs, system availability and reliability are major concerns. In spite of recent advances, effective health monitoring and prognostics for the scheduling of condition-based maintenance operations is still very challenging. The increasing availability of maintenance and operational data along with recent progress made in machine learning has boosted the development of data-driven prognostics and health management (PHM) models. In this paper, we describe the data workflow in place at an airline for the maintenance of an aircraft system and highlight the difficulties related to a proper labelling of the health status of such systems, resulting in a poor suitability of supervised learning techniques. We focus on investigating the feasibility and the potential of semi-supervised anomaly detection methods for the health monitoring of a real aircraft system. Proposed methods are evaluated on large volumes of real sensor data from a cooling unit system on a modern wide body aircraft from a major European airline. For the sake of confidentiality, data has been anonymized and only few technical and operational details about the system had been made available. We trained several deep neural network autoencoder architectures on nominal data and used the anomaly scores to calculate a health indicator. Results suggest that high anomaly scores are correlated with identified failures in the maintenance logs. Also, some situations see an increase in the anomaly score for several flights prior to the system’s failure, which paves a natural way for early fault identification.

Cost assessment model for sustainable health and safety management of high-rise residential buildings in Korea

ABSTRACT The accident rate in the Korean construction industry is much higher compared to the one in other industries. One of the main reasons for this problem is the health and safety management cost (HSC) estimation method which does not reflect the project features at construction sites. In Korea, health and safety management cost is estimated by applying a legally designated ratio in proportion to construction cost without reflecting construction features, such as the building’s shape, number of floors, and construction period. However, such a method does not fulfill the cost for the gradually strengthening safety management of high-rise residential buildings. Therefore, the objective of this paper is to develop a cost assessment model for sustainable health and safety management of high-rise residential buildings. For this purpose, actual data of 23 completed projects were collected and analyzed to identify problems and influence factors of HSC assessment and to establish a regression model. By applying the model established for five new sites as a case, its validation was verified. The results of this study will be utilized as reference data for the HSC estimation method and its relevant policy establishment.
