Renin-angiotensin system and other pathways induced by the increase of intraglomerular pressure due to obstruction cause renal ischemia leading to hypoxia and oxidative stress and can result in renal fibrosis, necrosis and apoptosis if not eliminated. Various biomarkers have been investigated in obstructive pathologies to date. Thiols are organic compound formed by hydrogen, sulfur and carbon atoms. In cases of oxidative stress, transient disulphide bonds are established between thiol groups. Our aim is to show the importance of the dynamic thiol-disulphide homeostasis in predicting renal injury in ureter obstruction. METHODS: 24 Wistar Albino rats were randomized and divided into 3 groups. Groups were named as sham, partial and complete. 2 cc venous blood was taken from each rat for the measurement of creatinine and thiol disulphide values and renal scintigraphy was performed. Only an abdominal incision was done in the sham group. The left ureter of the partial group was dissected and gently buried into the psoas muscle. In the complete group, the left ureter was dissected and ligated from two separate places and cut in the middle. On the postoperative 7th day, 2 cc venous blood samples were taken for biochemical examinations, control renal scintigraphy was done, left nephrectomy was performed and the kidneys were analysed histopathologically. RESULTS: There was statistically significant difference between the groups in terms of postoperative native thiol and total thiol values (p: 0.000, p: 0.001). There was also a significant difference in preoperative and postoperative comparisons of these parameters in partial and complete obstruction groups (p: 0.012, p: 0.018). There was a significant difference in terms of postoperative disulphide/native thiol and native thiol/total thiol ratios in complete obstruction group compared to the other groups (p1: 0.011; p2: 0.040 and p1: 0.015; p2: 0.028). A statistically significant association was found between native thiol, total thiol, left kidney function loss percentage and creatinine change values and histopathological EGTI scores (r: -0,867, r: -0,815, r: 0,745, r: -0,911, p: 0.000). CONCLUSIONS: Thiol-disulphide homeostasis is a valuable hematological parameter in predicting renal injury in obstructive uropathy. Decrease in levels of native thiol and total thiol is associated with pathological changes in the kidneys. Disulphide/native thiol and native thiol/total thiol ratios may be indicators of complete obstruction.
