The incidence rate of tuberculosis (TB) in Korea has been consistently decreasing, but the proportion of extrapulmonary tuberculosis (EPTB) remains relatively stable at around 20%. Diagnosing EPTB remains a challenge because clinical samples obtained from relatively inaccessible sites may be paucibacillary, thus reducing the sensitivity of diagnostic tests. Delayed diagnosis and treatment some form of EPTB, such as tuberculous meningitis, can lead to fatal outcomes. Therefore, it is crucial to have a high index of suspicion and perform appropriate testing early on. Obtaining a specimen from the lesion and conducting microbiological tests (acid-fast bacilli smear, mycobacterial culture, and nucleic acid amplification test) along with histopathological examination is essential. When the specimen is limited for all testing, mycobacterial culture test should be prioritized as it has the highest diagnostic yield and allows for drug susceptibility testing. All patients with presumed EPTB should be assessed for pulmonary TB to assess infectiousness and potentially assist with diagnosis.

A physiologically-based pharmacokinetic model for tuberculosis drug disposition at extrapulmonary sites.

Tuberculosis (TB) is a leading cause of mortality attributed to an infectious agent. TB primarily targets the lungs, but in about 16% cases can affect other organs as well, giving rise to extrapulmonary TB (EPTB). However, an optimal regimen for EPTB treatment is not defined. Although the recommended treatment for most forms of EPTB is the same as pulmonary TB, the pharmacokinetics of EPTB therapy are not as well studied. To address this gap, we formulate a whole-body physiologically-based pharmacokinetic (PBPK) model for EPTB that for the first time includes the ability to simulate drug concentrations in the pleura and lymph node, the most commonly affected sites of EPTB. Using this model, we estimate the time-dependent concentrations, at potential EPTB infection sites, of the following four first-line anti-TB drugs: rifampicin, ethambutol, isoniazid, and pyrazinamide. We use reported plasma concentration kinetics data to estimate model parameters for each drug and validate our model using reported concentration data not used for model formulation or parameter estimation. Model predictions match the validation data, and reported pharmacokinetic parameters (maximum plasma concentration, time to reach maximum concentration) for the drugs. The model also predicts ethambutol, isoniazid, and pyrazinamide concentrations in the pleura that match reported experimental values from an independent study. For each drug, the predicted drug concentrations at EPTB sites are compared with their critical concentration. Simulations suggest that although rifampicin and isoniazid concentrations are greater than critical concentration values at most EPTB sites, the concentrations of ethambutol and pyrazinamide are lower than their critical concentrations at most EPTB sites.

Tuberculosis of the spine.

Pott's spine, commonly known as spinal tuberculosis (TB), is an extrapulmonary form of TB caused by Mycobacterium TB. Pott's paraplegia occurs when the spine is involved. Spinal TB is usually caused by the hematogenous spread of infection from a central focus, which can be in the lungs or another location. Spinal TB is distinguished by intervertebral disc involvement caused by the same segmental arterial supply, which can result in severe morbidity even after years of approved therapy. Neurological impairments and spine deformities are caused by progressive damage to the anterior vertebral body. The clinical, radiographic, microbiological, and histological data are used to make the diagnosis of spinal TB. In Pott's spine, combination multidrug antitubercular therapy is the basis of treatment. The recent appearance of multidrug-resistant/extremely drug-resistant TB and the growth of human immunodeficiency virus infection have presented significant challenges in the battle against TB infection. Patients who come with significant kyphosis or neurological impairments are the only ones who require surgical care. Debridement, fusion stabilization, and correction of spinal deformity are the cornerstones of surgical treatment. Clinical results for the treatment of spinal TB are generally quite good with adequate and prompt care.

Role of endoscopic ultrasound guided fine needle aspiration (EUS-FNA) and Gene Xpert

BackgroundMediastinal tubercular lymphadenitis is form of extrapulmonary tuberculosis [EPTB]. Clinical presentations are non-specific and diagnosis remains great clinical challenge. Microbiological and or histopathological evidences need to be present in order make diagnosis secure before initiation of anti-tubercular therapy (ATT). Endoscopic ultrasound guided fine needle aspiration (EUS-FNA) provides tissue samples and aids management of this difficult to diagnosed entity. Current study describe role of EUS-FNA and Gene Xpert (GXP) in mediastinal tubercular lymphadenitis. MethodsRetrospective analysis of 72 patients with mediastinal lymphadenopathy who underwent EUS-FNA were carried out. Linear echoendoscope was used for evaluation mediastinum. EUS echo features of LNs were studied. Twenty two-G needle used was for aspiration tissue sample from pathologic lymph nodes (LNs). FNA samples were analysed by cytology, Acid-Fast Bacilli (AFB) staining and GXP study. All procedures were uneventful without any complications. ResultsForty two patients were diagnosed as tuberculosis (TB) following first EUS-FNA setting. Six patients underwent repeat EUS-FNA procedure following which another 3 were diagnosed as TB while remaining 3 started on empirical ATT based on additional supportive evidences. Forty five patients showed granulomatous inflammation on cytological analysis, AFB positivity noted in 16 (33.33%) patients while GXP in 26 (57.78%) patients. Rifampicin resistance detected in 3 ((6.25%) patients. All patients were followed clinico-radiologically for response to treatment. ConclusionTuberculous lymphadenitis is the most common cause of mediastinal lymphadenopathy in TB endemic countries. EUS-FNA provides microbiological and histopathological/cytological evidences in this difficult to diagnosed EPTB and thereby avoids empirical ATT.

Thoracic spinal tuberculosis in a 2.5-year-old child treated by surgical procedure with a transthoracic approach: a case report.

A 2-year-old and 6-month-old boy was admitted with fever and unspecific generalized pain, the examination revealed mild hyperreflexia in the lower extremities, isotope scan showed increased uptake in the T8 vertebra. MRI demonstrated destruction in the T8 vertebra with kyphotic deformity and abscess anterior to T7, T8, and T9 levels with an epidural abscess at the T8 level extending to the spinal canal and compressing the spinal cord. The patient underwent a surgical procedure with a transthoracic approach, the decompression of the spinal canal was performed through T8 corpectomy, the reduction of kyphosis was performed and the internal fixation with a dynamic cylinder and lateral titanium plate was carried out. Microbiologic examination suggests Mycobacterium tuberculosis. Pott's disease (spinal TB) is extremely rare in the young children population, and surgical treatment is reported only in a few reports, and it is considered a technical challenge. There are several surgical approaches during childhood, for upper thoracic spinal TB, the posterior approach is easy, minimally invasive, safe, reliable, and effective. But it had the worst outcomes. In contrast, the anterior approach provides direct access to the lesions. More research are needed to detect the best choice in the management of thoracic spinal TB in children.

Tuberculous Peritonitis: Epidemiological, Gender- and Age-Based Characteristics in the City of Moscow

The objective: to assess the incidence of tuberculous peritonitis, to identify its gender and age characteristics, to describe typical comorbidities preceding the development of peritoneal tuberculosis.Subjects and Methods. The following documents were statistically analyzed: Forms no. 003/u Medical Record of an In-patient Patient for new tuberculous peritonitis patients for 2013–2020, who were treated at Clinic 2 of Moscow Municipal Scientific Practical Center of Tuberculosis Control, Moscow Health Department, and Rosstat Forms No. 8 on Active Tuberculosis Cases for the same period, as well as information from registers of the city tuberculosis monitoring system that has been operating in Moscow since 1996.Results. According to different definitions, the expected incidence of tuberculous peritonitis in the coming years will make from 0.03 to 0.1 per 100,000 population per year in Moscow and 0.1–0.2 per 100,000 in Russia. In Moscow, the main cohort of tuberculous peritonitis patients includes young women after pregnancy and childbirth, who came from other regions of the Russian Federation or neighboring countries. Men develop tuberculous peritonitis 1.6 times less frequently versus women. The most common factors influencing the development of peritoneal tuberculosis in men are drug-induced immunosuppression, dialysis, and diabetes mellitus. HIV infection, unlike other forms of extrapulmonary tuberculosis, is not a major risk factor faced by individuals with tuberculous peritonitis.

Extrapulmonary tuberculosis: diagnostic features

Objective. To study demographic, clinical characteristics and concomitant pathology of patients with extrapulmonary tuberculosis (EPTB) in Gomel city and Gomel region for the period 2017-2021. To evaluate the informative value of different techniques for the detection of Mycobacterium tuberculosis (MBT) in non-respiratory samples in the diagnosis of extrapulmonary tuberculosis.Materials and methods. A retrospective study of extrapulmonary tuberculosis cases registered from 2017 to 2021 in Gomel and Gomel region (308 patients) was carried out. The results of microscopic, bacteriological and molecular genetic methods of non-respiratory and respiratory samples relevant in the diagnosis of extrapulmonary forms of tuberculosis were analyzed.Results. Demographic and clinical characteristics of patients with EPTB in Gomel and Gomel region for the period 20172021 were revealed. Concomitant pathology in patients with EPTB was analyzed, the features of EPTB diagnostics were studied. It was determined that EPTB is more susceptible to males aged 31 to 50 years and to women aged over 60 years. EPTB is more frequently diagnosed in patients from urban areas and when patients come to medical institutions with nonspecific complaints, except for intrathoracic lymph node tuberculosis (ITLN).Conclusion. In the structure of clinical forms of EPTB, lesions of bones and joints (62.7%) and intrathoracic lymph nodes (19.5%) predominate. HIV-positive patients are more common in the ITLN group. MBT is most often detected in the following localizations of extrapulmonary process: intrathoracic and peripheral lymph nodes, pleural tuberculosis. The most highly informative method of examination of biological nonrespiratory material for detection of MBT in extrapulmonary localization is molecular genetic study.

MTBDRplus and MTBDRsl for simultaneous diagnosis of gastrointestinal tuberculosis and detection of first-line and second-line drug resistance.

Emergence of drug resistance, especially to second-line drugs, hampers tuberculosis elimination efforts. The present study aimed to evaluate MTBDRplus and MTBDRsl assays for detecting first-line and second-line drug resistance, respectively, in gastrointestinal tuberculosis (GITB). Thirty ileocecal biopsy specimens, processed in the Department of Microbiology between 2012 and 2022, that showed growth of Mycobacterium tuberculosis on culture were included in the study. DNA, extracted from culture, was subjected to MTBDRplus and MTBDRsl (Hain Lifescience GmbH, Nehren, Germany), following manufacturer's instructions. Their performance was compared against phenotypic drug susceptibility testing (pDST) and gene sequencing. Out of the 30 specimens, 4 (13.33%) were mono-isoniazid resistant, 4 (13.33%) were multidrug resistant (MDR), 2 (6.67%) were pre-extensively drug resistant (pre-XDR), and 2 (6.67%) were mono-fluoroquinolone resistant. The results were 100% concordant with pDST and gene sequencing. In the wake of growing drug resistance in all forms of extrapulmonary tuberculosis, including GITB, MTBDRplus and MTBDRsl are reliable tools for screening of resistance to both first-line and second-line drugs.

Utility of laboratory diagnostic tests in women suspected of genital tuberculosis attending a tertiary care teaching hospital.

Genital Tuberculosis is a form of extrapulmonary tuberculosis, which if not diagnosed early can lead to complications. The objective of this study was to determine the sensitivity and specificity of Xpert Mycobacterium tuberculosis/rifampin (MTB/RIF) assay in genital tuberculosis (TB) in comparison with culture as a gold standard. The results of the Xpert MTB/RIF assay performed from January 2020 to August 2021 were compared with the results of culture by Mycobacterium Growth Indicator Tube (MGIT) 960. Out of 75 specimens, fluorescent microscopy and liquid culture using MGIT and Xpert assay were positive in 3 (4%), 21 (28%), and 14 (18%), respectively. The sensitivity and specificity of the Xpert MTB/RIF assay were 66.67% and 100%. All smear-positive specimens were positive by culture and Xpert assay. Three specimens were positive by all the tests, i.e., microscopy, culture, and Xpert assay. Fifty-four specimens were negative by microscopy, culture, and Xpert assay. Discordance between the results of culture and Xpert assay was observed in seven specimens which were culture positive and Xpert assay negative. Three (21.42%) out of 21 culture-positive specimens showed monoresistance to rifampicin by Xpert MTB/RIF assay and culture drug susceptibility testing. Xpert MTB/RIF assay showed good sensitivity and specificity compared to liquid culture in genital TB. This test is easy to perform, provides results in 2 h, and can also detect rifampicin resistance, which is a surrogate marker for multidrug-resistant TB. Hence, the Xpert assay can be used under the National TB Elimination Program for early and rapid diagnosis of TB in endometrial specimens to prevent complications like infertility.

Tubercular lymphadenopathy for postgraduates – A minireview

Extrapulmonary tuberculosis (EPTB) remains often ignored by both medical community and authorities. This review attempts to summarize the pathogenesis, clinical presentation, investigations, diagnosis, treatment, and complication of lymph node TB (LNTB) in India. LNTB refers to Mycobacterium tuberculosis infection of the lymph nodes and is the most common form of EPTB in India. Classical symptoms include low-grade fever, weight loss, loss of appetite, night sweats, and painless enlarged lymph nodes in the neck or axilla or groin. Mediastinal lymphadenopathy presents with cough and chest discomfort and uncommonly even with shortness of breath; wherever possible, a pathological and microbiological diagnosis must be attempted by fine-needle aspiration for cytology, histopathology, acid-fast bacilli smear, and molecular tests such as cartridge-based nucleic acid amplification test and culture. For deep nodes, ultrasound/computed tomography/endobronchial ultrasound-guided aspiration can be done. LNTB is primarily requires medical management, and adjunct surgical excision is generally associated with worse outcomes. Six-month antitubercular therapy standard first-line regimen is recommended for peripheral LNTB and can be provided at the nearest local directly observed treatment short-course chemotherapy center in India.

Trends in incidence of extrapulmonary tuberculosis in children in the Czech Republic in the past 35 years.

Extrapulmonary tuberculosis (EPTB) in children globally represents a proportion of all cases of tuberculosis, reaching 20% according to published reports. Children are at a higher risk for disseminated TB and extrapulmonary forms. The most prevalent clinical presentations of EPTB in children worldwide are peripheral lymphadenitis and osteoarticular TB. Peritoneal, urogenital, or meningeal tuberculosis is less frequent, and their diagnosis is often difficult. The aim of the study was the determination of EPTB incidence in children in a low-prevalence region over 35 years. Descriptive retrospective study of the incidence of EPTB in children and adolescents (aged 0-18 years) in the Czech Republic in the years 1987-2021 as reported in the tuberculosis register. Total amount of EPTB cases was 95. The most prevalent form was extrathoracic lymph node TB. The median age was 10 years. Most patients (84%) were Czechs. None of them died. The ratio of pulmonary and extrapulmonary TB was 79.8% to 20.2%. The low incidence of EPTB in Czech children is congruent with a low incidence of TB in the Czech Republic. Our study confirms that the most frequent form of EPTB is peripheral lymph node TB. Our study did not prove a significant change in the trend of EPTB after the cessation of the mandatory Bacillus Calmette-Guerin (BCG) vaccination programme.

Tuberculosis (TB) is not only an infectious disease but one of the top 10 causes of death, spreading mainly with aerosol transmission and accompanying the history of humankind for several millennia. TB is caused in humans and animals by members of the Mycobacterium tuberculosis complex. For better understanding of the disease and the evolutional background of its causative agent, involvement of palaeopathological investigations is surpassingly important. Traditionally, palaeopathology is using a broad variety of markers, which are observable by macroscopic investigations. These markers are mainly related to different extra-pulmonary forms of TB. However, these manifestations develop only in a few cases. Moreover, many markers are not pathognomonic of TB. Therefore, to avoid underestimation of TB prevalence in paleopathological studies, the diagnosis on archaeological material requires a multidisciplinary approach. For better estimation of TB incidence in past populations, an array of specific biomarkers can be brought into play to confirm initial, macromorphology-based skeletal diagnoses, namely aDNA and lipid biomarkers. The three lipid biomarker groups, which are mainly involved in such investigations are the mycolic acids (MAs), the mycocerosic acids (MCs) and the C27 mycolipenic acid (ML). These unique lipids can be located in the lipid-rich mycobacterial cell wall. The application of lipid-based TB diagnostic approach has been proved to be robust and reliable through many examples. A variety of analytical methods have been employed for mycobacterial lipid biomarker profiling. Fluorescence HPLC is well-developed for the analysis of mycolic acids and phthiocerols. Furthermore, one isolated case of direct mass spectrometric detection of M. tuberculosis free mycolic acids has been also introduced. Our aim was to establish a lipid-biomarker-based HPLC-MS method for TB diagnosis in historical human samples, as this instrumentation is available in many laboratories, has the potential of a quick and sensitive and at the same time an affordable measurement protocol. We successfully developed and optimised a method, which is capable to separate and detect MAs and MCs. Moreover, we optimised the sample pre-treatment process. We started to build an MA and MC lipid profile library, which can serve as a comparison to diagnose TB. For the verification of new methods and the estimation of reliability of new markers in macroscopic analysis, palaeopathologists usually use well-documented collections of skeletons and mummies from the pre-antibiotic era. For this purpose, we chose to test our method on the Vác Mummy Collection. In the case of four out of six mummified individuals, MC profiles of characteristic M. tuberculosis MCs were recorded. The HPLC-ESI-MS method, developed for the detection of MCs, opens a new avenue for the detection of ancient mycobacterial disease, encompassing both tuberculosis, leprosy and joint cases.

Extrapulmonary tuberculosis among children: peculiarities of age’s groups and drug resistance of mycobacterium tuberculosis

Objective — to analyze the peculiarities of detection and course of various clinical forms of extrapulmo­nary tuberculosis (EPTB), the profile of drug resistance of the Mycobacterium tuberculosis (MBT) among children of different ages.
 Materials and methods. We retrospectively analyzed the medical data of 47 children aged 0 to 14 with local forms of EPTB, hospitalized in the pediatric department of Lviv TB hospital from 2013 to 2020. Children were divided into groups: up to 1-year-olds (19 children; group I), 2—9 year-olds (19 children; group II) and 10—14-year-olds (9 children; group III).
 Results and discussion. It was found that children under 1 year of age were more likely to live in rural areas than in urban areas (78.9 vs. 21.1 %; p < 0.01). Resistant forms were diagnosed among 49.0 % of children. Among children of the first year of age this number reached 73.7 %, which is probably more often than among children aged 2—9 (26.3 %; p < 0.01). The numbers of multidrug-resistant TB (MDR-TB) and the risk of MDR-TB among children under one year of age were 35.7 and 28.6 %, respectively. The most common clinical form of EPTB was TB of peripheral lymph node (PLN) regardless of age (40.4 %). Only TB of PLN and TB of meningeal and CNS were found among children under 1 year of age. Among children aged 2—9, TB of PLN and TB of bone and joints were probably more common than all other localizations. TB of bones and joints among children aged 10—14 was more common than TB of eyes and skin. The proportion of TB of meninges and CNS in group I was probably higher compared to groups II and III (p1 < 0.01; p2 < 0.05). At the same time, TB of bones and joints among children of groups II and III was diagnosed significantly more often than in group I (p1 < 0.01; p2 < 0.05).In 89.4 % of cases EPTB was combined with pulmonary TB (PTB). Among children under one year of age, miliary PTB was diagnosed probably more often than other clinical forms (p < 0.05). Bilateral lesions were probably more common among children under one year of age than in children aged 10—14 (77.8 vs. 28.6 %; p < 0.05). Unilateral lesions were more common among children aged 10—14 than in children up to one year of age (22.2 vs. 71.4 %; p < 0.05). Children under 1 year of age and 2—9-year-olds were significantly more likely to be diagnosed with EPTB when seeking medical care than during examination of contact persons (63.2 vs. 26.3 %; 68.4 vs. 31.6 %; all p < 0.05) and during the preventive examination (10.5 % and 0; all p < 0.01). Also, EPTB was found more often among children aged 10—14 (66.7 vs. 0; p < 0.01). It was found that only 36.8 % of children of group I, 42.1 % of children of group II and 33.3 % of children of group III were sent to a specialized hospital after visiting the clinic.Patients under 1 year of age and 2—9-year-olds compared with children aged 10—14 were significantly more likely to have a combination of clinical manifestations of intoxication and bronchopulmonary syndromes (47.4 and 31.6 % vs. 0; p1 < 0.05; p2 < 0.01). At the same time, intoxication syndrome was significantly more common among children aged 10—14 than among children under one year of age (88.9 vs. 26.3 %; p < 0.01). Lack of vaccination was significantly more common among children under one year of age than in the following age groups (94.7 vs. 57.9 and 44.4 %; both p < 0.05), and the proportion of ineffective vaccinations was significantly higher among children aged 10—14 than in children under 1 year of age (55.6 vs. 5.3 %; p < 0.05).
 Conclusions. These data indicate the need to intensify preventive TB measures among children of different ages, as well as the need for enhanced control over vaccination, regular screening, monitoring of contacts among the adult population and their controlled treatment.

Minimally Invasive Surgical Approach to Decompress a Multi-Loculated Abscess in the Thoracic Spine: A Case Report

Tuberculosis (TB) remains one of the major global causes of death; Mycobacterium Tuberculosis causes it. Approximately 10% of patients with TB may develop extrapulmonary bone tuberculosis, with the spine being the most affected, representing 1 to 2% of all TB cases. Pott disease is a form of extrapulmonary TB, and recently it has shown a significant resurgence in developed nations possibly impacted by global migration. Pott disease is often misdiagnosed or sometimes diagnosed during its late stages, which can be associated with devastating consequences to the patient. Brazil is among the top 20 countries with a high burden of tuberculosis; however, the published evidence is still limited, particularly during the last two years of the COVID-19 pandemic. Under this context, we present a clinical case of a 43 years old female patient with Pott disease with a minimally invasive surgical approach to decompress a multi-loculated abscess in the thoracic spine.

Demographic and microbiological characteristics of tuberculous lymphadenitis and other extrapulmonary tuberculosis cases

Objective: Tuberculosis (TB) is a significant public health problem that remains important worldwide. This study aimed to examine the demographic characteristics and clinical features of patients with extrapulmonary tuberculosis (EPTB) and also identify the comorbidities of EPTB. Our secondary aim was to compare tuberculous lymphadenitis (TBL) with other forms of EPTB. Patients and Methods: This single-center, retrospective, observational study was conducted on patients diagnosed with EPTB in a training hospital in Turkey between January 1, 2011 and December 31, 2020. The demographic characteristics and clinical features of the patients were examined. TBL cases were compared with other forms of EPTB cases. Results: The most common clinical form was TBL. Fever, night sweats, and accelerated erythrocyte sedimentation rate were significantly more common in the other forms of the EPTB group compared to TBL. The rate of tuberculin skin test positivity was significantly higher in patients with TBL. The complication rate was 3%, and mortality rate was 4% among all patients. Conclusions: Extrapulmonary tuberculosis should be considered in the differential diagnosis by evaluating the medical history of patients who present with organ-specific symptoms, especially in TB endemic countries. The clinic may differ according to the organ involvement. TBL may be seen with milder symptoms compared to other clinical forms.

Prospective study comparing the clinical outcomes of conservative and surgically treated spinal tuberculosis

Introduction: Spinal tuberculosis is a serious form of extra pulmonary tuberculosis which if left untreated can be fatal; neurologic dysfunctions in association with active tuberculosis of spine can be prevented by early diagnosis and prompt treatment. Prompt treatment can reverse paralysis and minimize the potential disability resulting from Pott’s paraplegia. A significant dilemma exists as to which line of management a patient needs to be subjected once diagnosed to have spinal tuberculosis. Objective: Assess the functional and neurological outcome of patients treated with different modalities of treatment and attempts to ascertain the best practise for effective management of a case of spinal tuberculosis. Materials and Methods: 22 adult patients diagnosed with spinal TB and treated with either conservative line of management or CT/USG guided pigtail catheter drainage with chemotherapy or surgery combined with chemotherapy, were considered. Patients were followed up until completion of anti-TB treatment. Clinical outcomes were assessed using Visual analogue scale, Oswestry Disability Index and modified McCormick grade. Results: Patients having milder form were treated conservatively using anti-tubercular drugs alone (n=13). Patients with well-established abscess and without instability were treated using CT/USG guided pigtail catheter drainage along with antitubercular drugs (n=6).

Peritoneal Tuberculosis in Children Revealed by Appendicular Syndrome in a Case in the Pediatric Surgery Department: Fousseyni Daou Hospital in Kayes (Mali)

Peritoneal tuberculosis is a rare form of extra pulmonary tuberculosis in children. It represents 1 to 2% of all forms of tuberculosis. It is characterized above all by the occurrence of ascites, a progressive deterioration of the general condition, a fever. Its diagnosis can sometimes be very difficult; we report a case of peritoneal tuberculosis revealed by a surgical emergency.

ACUTE IZONIAZID POISONING WITH PRESENTATION OF A CLINICAL CASE

Rimicid or isonide a group of medications which belong to anti-tuberculosis medicaments. The active substance isoniazid is used to treat pulmonary and extrapulmonary forms of tuberculosis, as well as prophylaxis in case of contact with infected individuals. Isoniazid is rapidly absorbed, reaching a maximum serum concentration in 2 hours, and penetrates all body fluids. The volume of distribution is approximately 0.6 l/kg. Isoniazid causes pyridoxine deficiency in two ways: - by hydrazones, isoniazid metabolites inhibit the pyridoxal kinase, which converts pyridoxine to the active form, pyridoxal-5-phosphate, and - pyridoxine deficiency disrupts the synthesis and metabolism of GABA, the main inhibitory mediator of the central nervous system. Isonicotinic acid poisoning metabolism is subject to individualized factors, and in some people, this metabolism can be in a significantly slower. A clinical case of poisoning with 30 tablets of Rimicid by a 16-year-old girl suicidal attempt was presented. The results of the TC analysis confirm intoxication with the drug, and the retrospective assessment of the concentration over a past period of time using the elimination half-life has revealed an extremely high dose of ingested isoniazid.

Urine-lipoarabinomannan-might fill the gap in delayed diagnosis of life-threatening tuberculosis

Mycobacterium tuberculosis bacilli is a causative organism for tuberculosis disease. The lungs are the most commonly affected organ with this infection but it can infect any vital organ of the body. The person with signs and symptoms of tuberculosis disease was identified with detection of acid-fast bacilli in the biological specimen and with rapid molecular testing. However, the limiting factor for the early tuberculosis (Tb) diagnosis is the running cost of the diagnostic test. It is the common reason preventing the advancement of diagnostic laboratories in low and middle-income countries. The person and community both suffered from this diagnostic delay. This delay causes significant morbidity and mortality in the patient. An alternate test and an alternate sample that can be easily obtained would be beneficial to prevent these diagnostic delay issues. Lipoarabinomannan (LAM) antigen detection in the urine sample is one such promising diagnostic test. Mycobacterium tuberculosis’s inner layer is made up of glycolipids. LAM is derived from phosphatidylinositol. LAM is a heat-stable amphiphilic cell wall component. It is the precursor of phosphatidyl-inositol-mannosidase and lipomannan. An extra mannose cap is a characteristic feature of a virulent strain of Mycobacterium. LAM has immunogenic and immunomodulatory properties. It is a promising diagnostic test because it is simple to do, the assay can be performed at the patient’s bedside and takes a while to perform. This assay sensitivity varies from 56% to 85% and it has greater than 88% specificity. In HIV seropositive patients, use of LAM assay can reduce 8-week mortality. LAM detection is also a very good assay for the detection of tuberculosis in renal failure and disseminated tuberculosis patients.Lipoarabinomannan detection in the urine is a possible test that can prevent delay in diagnosis. This is a promising test because it's easy to perform, the test can be done with a card test besides the patient's bed. It can be used in HIV seropositive patients and various other forms of extra pulmonary tuberculosis. It can be a very useful test for diagnosing a critically ill patient who is not able to produce a target sample.

Urinary tract tuberculosis misdiagnosed and treated as renal carcinoma in the presence of diffuse interstitial nephritis: a case report

BackgroundUrinary tract tuberculosis (UTTB) is a common form of extrapulmonary tuberculosis (TB) which can infrequently present as renal carcinoma, leading to serious errors in the diagnosis and treatment of UTTB.Case presentationA 76-year-old Syrian man presented with gross hematuria as the main symptom. A urinary endoscopic examination and pelvic multi-slice computed tomography imaging increased the suspicion of a speared renal mass in the right urinary tract. The patient was treated for renal cancer. After nephrectomy and ureterctomy, the histopathology of the resected mass confirmed the diagnosis of UTTB and interstitial nephritis.ConclusionThis case should serve to increase the attention of clinicians to perform an accurate diagnosis step by step. This is especially important if they have a patient similar to the case described here who presents with a renal mass, to avoid serious results such as the loss of an essential organ system.
