Ability of two reciprocating Nickel-Titanium instruments for guttapercha/ sealer removal in simulated curved root canals.

The aim of this study was to compare the capacity of two reciprocating NiTi instruments in removing gutta-percha/sealer material from simulated curved root canals (SCRC). The time required for filling material removal was also recorded. Twenty SCRCs were divided into two groups of 10 (n=10) samples each. In Group 1, the SCRC were prepared to a R25 Reciproc Blue instrument (RCPB; VDW, Munich, Germany). In Group 2 the SCRC were prepared to a Primary WaveOne Gold instrument (PWOG; Dentsply, Ballaigues, Switzerland). In both groups, the canals were filled with matched-taper single gutta-percha cones and AH Plus sealer. Filling materials were removed with R25 RCPB (Group 1) and PWOG (Group 2). The amount of remaining gutta-percha/sealer was calculated at three predetermined levels of evaluation located at 2, 6 and 10 mm from the WL and expressed in percentages. Canals re-treated with RCPB contained significantly less remaining gutta-percha/sealer compared tocanalspreparedwith PWOG (P=0.02). The RCPB instruments required significantly less time to complete the retreatment procedures (P<0.01). No unwinding or instrument separation was noted. RCPB instruments removed significantly more gutta-percha/sealer from simulated curved root canals than PWOG. However, neither of the tested instruments completely removed all filling materials.

Shaping Ability of Reciprocating and Rotary Systems After Root Canal Retreatment: a CBCT Study.

The purpose of this in vitro study was to evaluate the shaping ability of reciprocating and continuous rotary systems after root canal retreatment. After preparation and root canal filling, mesial canals of 54 mandibular molars were distributed into 3 groups (n=18), according to the filling material removal and re-instrumentation protocols: WOG group - WaveOne Gold system; PTN group - ProTaper Next system; and PTU group - ProTaper Universal system. Cone-beam computed tomographic (CBCT) images acquisition of the mesial root canals was performed at different moments: (1) before instrumentation (unprepared root canals), (2) after preparation and filling, (3) after filling material removal and (4) re-instrumentation. The apical transportation (AT), centering ability (CA) and change in root canal diameter were assessed by CBCT analysis. The remaining filling material quantification was performed by radiographic examination. The statistical analyses were performed using the 3-way ANOVA, Tukey-Kramer, Kruskal-Wallis and Dunn multiple Comparison tests (p<0.05). The tested instruments did not show full CA (=1.0). PTN group had greater AT at the 5th mm in comparison with the WOG group (p<0.05). After re-instrumentation, WOG group had greater root canal diameter change at the 1st and 5th mm than PTN and PTU groups (p<0.05). There was no significant difference among groups when comparing the amount of remaining filling material after re-instrumentation (p>0.05). The tested systems provided minimal alteration in root canal morphology at the apical portion after root canal retreatment. However, WOG promoted greater change in root canal diameter.

Effectiveness and safety of rotary and reciprocating kinematics for retreatment of curved root canals: a systematic review of in vitro studies.

ObjectivesThis systematic review (register-osf.io/wg7ba) compared the efficacy and safety of rotary and reciprocating kinematics in the removal of filling material from curved root canals.Materials and MethodsOnly in vitro studies evaluating both kinematics during retreatment were included. A systematic search (PubMed/MEDLINE, Scopus, and other databases, until January 2021), data extraction, and risk of bias analysis (Joanna Briggs Institute checklist) were performed. Efficacy in filling removal was the primary outcome.ResultsThe search resulted in 2,795 studies, of which 15 were included. Efficacy was measured in terms of the remaining filling material and the time required for this. Nine studies evaluated filling material removal, of which 7 found no significant differences between rotary and reciprocating kinematics. Regarding the time for filling removal, 5 studies showed no difference between both kinematics, 2 studies showed faster results with rotary systems, and other 2 showed the opposite. No significant differences were found in apical transportation, centering ability, instrument failure, dentin removed and extruded debris. A low risk of bias was observed.ConclusionsThis review suggests that the choice of rotary or reciprocating kinematics does not influence the efficacy of filling removal from curved root canals. Further studies are needed to compare the kinematics safety in curved root canals.

XP Endo Finisher-R and PUI as supplementary methods to remove root filling materials from curved canals.

This study assessed the ability of XP-endo Finisher R (FKG, La Chaux-de-Fonds, Switzerland) to remove filling remnants from curved mesiobuccal canals of maxillary molars, using the passive ultrasonic irrigation (PUI) technique as a comparison. Twenty-four curved main mesiobuccal canals (MB1) of maxillary molars were instrumented with Wave One (#25/07) and filled with gutta-percha points and AH Plus Sealer. Samples were then re-treated with a standardized protocol with Wave One (#35/06) as the master apical file. Micro-CT scans measured baseline volume of remaining filling material (in mm3). Samples were divided into two groups (n = 12) according to the supplementary cleaning approach: (PUI) or XP-endo Finisher R. Statistics compared baseline and final volume of filling material (within-group); and the percentage of filling material reduction (between-group). Mean baseline volumes, final volumes, and percentages of reduction (%) of filling material for XP-endo Finisher R and PUI were respectively: 0.060 mm3, 0.042 mm3, and 31.28%; and 0.064 mm3, 0.054 mm3, and 16.57%. Both tested protocols reduced the amount of filling material (p < 0.05). XP-endo Finisher R had higher percentage of reduction as compared to PUI (p < 0.05). XP-endo Finisher R and PUI used as supplementary cleaning protocols during re-treatment improved the removal of root filling material in curved canals; but XP-endo Finisher R was approximately twice more efficient. The complete filling material removal during re-treatment procedures is still a challenge. Supplementary cleaning protocols may help to remove the remaining material after the complete mechanical preparation of curved canals. XP-endo Finisher R was approximately twice more efficient than PUI.

Assessment of the Effectiveness of Supplementary Methods for Residual Filling Material Removal Using Micro-computed Tomography: A Systematic Review and Meta-analysis of In Vitro Studies.

A systematic review and meta-analysis were conducted to evaluate the effectiveness of auxiliary methods in removing residual filling material (RFM). This systematic review has been registered with the International Prospective Register of Systematic Reviews (registration number CRD42020197482). A comprehensive literature search was conducted to identify relevant articles in electronic databases (PubMed, Embase, Cochrane) from January 2005 to March 2021. In vitro studies investigating or comparing at least one type of supplementary method or technique were included. A total of 26 studies were selected from the 239 records obtained after screening the databases. Ten of the included studies were suitable for meta-analysis. Strong evidence showed that ultrasonically activated irrigation (UAI) [SMD (95% CI): -0.52 (-0.88 to -0.16, P=0.266)] and XP-Endo Finisher R (XPR) [SMD (95% CI): -0.55 (-0.89 to -0.20, P=0.136)) contributed significantly to increase the removal procedure, and XPR has no significant superiority over UAI [SMD (95% CI): 0.36 (-0.12 to 0.84, P=0.994)]. Strong evidence was found to support the increased cleaning efficiency of the supplementary use of the Gentle Wave system, laser irradiation, XP Endo Finisher, and self-adjusting file. In contrast, conflicting evidence was found to support the use of sonic to improve the removal of RFM, and limited evidence was found to support the efficiency of Tornado Brush. Under in vitro conditions, UAI and XPR increase the removal of RFM from the root canal system during retreatment procedures.

Efficacy of reciprocating instruments and final irrigant activation protocols on retreatment of mesiobuccal roots of maxillary molars: a micro-CT analysis.

ObjectivesThis study evaluated the efficacy of 3 reciprocating systems and the effects of 2 instruments for irrigant activation on filling material removal.Materials and MethodsForty mesiobuccal roots of maxillary molars were prepared up to size 25.06 and obturated. Micro-computed tomography (micro-CT) examination #1 was performed. Teeth were then divided into 4 groups (n = 10), according to the retreatment protocol: (1) manual, (2) Reciproc Blue, (3) WaveOne Gold, and (4) X1 Blue. Micro-CT examinations #2 and #3 were performed after filling removal and repreparation, respectively. Next, all teeth were divided into 2 new groups (n = 20) according to the irrigant activation protocol: XP Clean (XP Clean size 25.02) and Flatsonic (Flatsonic ultrasonic tip). Micro-CT examination #4 was performed after irrigant activation. Statistical analysis was performed with a significance level set at 5%.ResultsWaveOne Gold removed a significantly greater amount of filling material than the manual group (p < 0.05). The time to reach the WL was similar for all reciprocating systems (p > 0.05). X1 Blue was faster than the manual group (p < 0.05). Only manual group improved the filling material removal after the repreparation stage (p < 0.05). Both activation protocols significantly improved the filling material removal (p < 0.05), without differences between them (p > 0.05).ConclusionsNone of the tested instruments completely removed the filling material. X1 Blue size 25.06 reached the working length in the shortest time. XP Clean and Flatsonic improved the filling material removal.

Effect of solvent on root canal filling material removal by two different rotary file systems in C-shaped root canals.

This study compared the percentage reduction of total obturation materials, gutta-percha and sealer, using two rotary file systems with/without solvent in retreating C-shaped root canals and to evaluate the effect of the solvent on instrument separation. C-shaped root canals were scanned using micro-CT to identify the root canal morphology and randomly assigned to four groups (n = 12). Retreatment was performed using ProTaper Universal Retreatment (PTUR) or XP-endo systems with/without solvent. The number of separated instruments were recorded. The samples were scanned pre- and post-retreatment. The percentage reduction of total obturation materials, gutta-percha and sealer, were calculated. The percentage reduction of gutta-percha in the PTUR without solvent group was significantly lower than the other groups (P = 0.018). The number of separated instruments in the solvent groups were significantly lower than the no solvent groups (P < 0.05). Solvent increased gutta-percha removal, when using PTUR and decreased the occurrence of separated instruments for both systems.

Effectiveness of different rotary file systems in removing the root canal filling material: A micro-computed tomography study.

Background. This study aimed to examine the retreatment efficiency of four NiTi rotary file systems with micro-computed tomography (micro-CT). Methods. Forty premolar teeth were shaped up to F2 using the ProTaper Universal rotary file system and obturated with MTA Fillapex. The root canals were scanned with micro-CT to evaluate the volume of root canal filling before retreatment. The samples were randomly divided into four groups according to the file system used to remove root canal filling material (n=10): ProTaper Universal Retreatment, ProTaper NEXT, EdgeFile XR, and EdgeFile® X3 NiTi system. All the samples were scanned with a micro-CT device for the second time to evaluate the amount of residual filling material in the root canals. The percentages of filling material removed from root canals were calculated. Additionally, the time spent on the removal of the root canal filling material was recorded. The data were analyzed using the Shapiro-Wilk and Kruskal-Wallis tests. Results. There were no significant differences between the groups in the percentage of root canal filling material removal. However, a statistically significant difference was found between the groups in the time required to reach the apex and remove the entire filling material. The time required to remove the root canal filling material was higher in the EdgeFile® X3 group. Conclusion. NiTi files manufactured for root canal preparation can be used effectively and safely to remove root canal filling materials. EdgeFile XR produced for retreatment can be used as an alternative to ProTaper Universal Retreatment files.

Comparison of the efficacy of three different supplementary cleaning protocols in root-filled teeth with a bioceramic sealer after retreatment-a micro-computed tomographic study.

The present study evaluated the efficacy of three different supplementary cleaning protocols on the removal of bioceramic-based root canal filling materials from oval-shaped canals. A total of 36 human mandibular premolars were instrumented and obturated with gutta-percha and iRoot SP using the single-cone technique. Retreatment was performed with ProTaper Universal retreatment files. The samples were randomly divided into the following supplementary cleaning protocols: conventional syringe irrigation (CSI), passive ultrasonic irrigation (PUI), and XP-endo Finisher R (XPR). The samples underwent micro-CT scanning to quantify the amount of residual filling materials after retreatment and after performing the cleaning protocols. The volume of filling material removal was analyzed by one-way analysis of variance and post hoc Bonferroni and Games-Howell tests (P < 0.05). There were significant differences in the amount of root canal filling material removal in every comparison group (P < 0.05). The XPR removed significantly more residual filling materials (P < 0.01) followed by PUI and CSI (P < 0.05). Similar efficacy was found in the total root canal and all root thirds. The additional use of XP-endo Finisher R and passive ultrasonic irrigation effectively removed the bioceramic-obturated root filling materials from the oval-shaped canals. However, none of the techniques completely removed the filling materials. The XP-endo Finisher R and passive ultrasonic irrigation exhibited greater efficacy compared with syringe irrigation in the reduction of residual filling materials after retreatment in root-filled teeth with a bioceramic sealer.

Critical analysis of research methods and experimental models to study removal of root filling materials.

Despite enjoying high favourable outcome rates, root canal treatment is not always successful. Root canal retreatment is a widely practised option for managing a non-healing root treated tooth. A basic distinction between retreatment and initial treatment is the requirement to remove the existing root filling material occupying the root canal space. Only then, can the technical deficiencies and possible causes of failure be addressed. Hence, the successful removal of the root filling material is critical to achieving the objectives of retreatment. Many different materials and techniques have been used for root canal filling, which have been documented in a plethora of studies; these are mainly laboratory studies investigating the removal of root filling materials. To help guide future research, which may then better inform clinical practice in relation to retreatment procedures, the focus of this narrative review is on the research methods and experimental models employed to study the removal of root filling materials. The current available literature demonstrates great variation in the research methodologies used to study the removal of root filling material, including direct visualization by splitting tooth samples or clearing, three-dimensional radiographic imaging including using microcomputed tomography or cone-beam computed tomography, and a combination of these techniques. Depending on the research method employed, variation also exists in quantification of root filling material removal; these include measurements of surface area, volume or weight, and criteria-based (semi-quantitative) assessment. Experimental models, using human teeth or resin models, differ with respect to standardization techniques and replication of the clinical scenario including initial sample preparation, canal filling, allocation and technical aspects of retreatment procedures. Future models should replicate, as closely as possible, the clinical scenario. Currently, microcomputed tomography provides a highly detailed, non-invasive and non-destructive method of objectively assessing and quantifying root filling removal.

Efficacy of XP-endo Finisher-R in enhancing removal of bioceramic sealer from oval root canal: a micro CT study

Aim: To investigate the efficacy of the XP-endo Finisher-R (XP-FR) and manual H-filing in enhancing the removal of Totalfill BC sealer from oval root canals. Methodology: Forty mandibular incisors were prepared using ProTaper Next up to file X3 then filled with warm vertical condensation using Totalfill BC sealer. The volume of the root canal filling post-obturation and the remaining volume post-retreatment using Mani NRT-GPR system were assessed using Micro-CT (µ-CT). The total volume in 3 mm was calculated from the cervical line till the root apex using the CT-an software. Specimens were randomly divided into two equal groups n=20 according to the supplementary approach of gutta percha removal used; where Group I: XP-FR was used for 1 min, and Group II: size 30 H-file was used in a filing motion for 1 min. Specimens were again scanned using µ-CT and the remaining gutta percha volume was calculated. Data were statistically analyzed using Wilcoxon, Man-Whitney, Friedman tests with significance level of 5%. Results: Significant reduction of the remining filling material was recorded post-retreatment with percentage of reduction of 63.34%, 78.53%, and 66.21% at the apical, middle, and coronal thirds respectively using Mani NRT-GPR system. Supplementary removal approaches significantly improved filling material removal (P<0.05). XP-FR removed significantly more filling material than manual H-filing with percentage of filling material reduction of 52.22%, 34.92%, and 40.60% compared to 21.89%, 18.43%, 31.72% in the apical, middle, and coronal thirds (Man-Whitney test, P<0.001). Conclusions: Rotary retreatment files failed to totally remove the root canal filling material. Supplementary methods have improved root canal filling material removal; where XP-FR significantly removed more filling than manual H-filing.

Contribution of XP-Endo files to the root canal filling removal: A systematic review and meta-analysis of invitro studies.

This review was designed to analyse whether the supplementary use of XP-Endo Finisher and XP-Endo Finisher R contributes to the removal of root canal filling materials. Databases were searched up to November 2020. Studies were critically assessed according to inclusion and exclusion criteria. Risk of bias evaluation was performed. Pooled standardised mean differences (SMD) and 95% confidence intervals (95% CI) were calculated. Ten studies involving 192 teeth were eligible for systematic review and meta-analysis could be performed with eight of these studies. Strong evidence showed that XP-Endo Finisher R file contributed significantly to filling material removal from root canals (SMD (95% CI): 0.41 (0.06-0.76)). Although the evidence was conflicting, XP-Endo Finisher R file was superior than other irrigant activation techniques (SMD (95% CI): 0.47 (0.02-0.93)). The supplementary use of XP-Endo Finisher and XP-Endo Finisher R is beneficial in further removal of root canal filling materials from root canal system.

Micro-computed tomography evaluation of filling material removal by three reciprocating systems with different thermal treatments

Aim: This study compared the efficacy of three reciprocating systems with different heat treatments in removing obturator material from lower molar mesial canals. Methodology: The mesial canals of 18 lower molars were instrumented with ProTaper Next 25/0.06 and filled with gutta-percha and AHPlus. The teeth were randomly divided into three groups (n=6) according to the system used to remove the filling material: Reciproc (REC R25), Reciproc Blue (RECB R25) and Wave One Primary (WO). The desobturation was considered complete when the working length was reached, and remnants of obturator material were no longer found in the instrument and also in the canal walls, observed through the dental operating microscope. Results: The results were analyzed using Biostat 4.0 software. Shapiro-Wilk and ANOVA (Tukey) tests with a significance level of 5% were applied. The volume of obturator material decreased in the cervical and middle thirds after the use of REC, RECB and WO systems (P .05). In the apical third, there was no significant reduction in the remaining obturator material independent of the operating system (P>.05). Conclusions: REC, RECB and WO systems showed similarity in the removal of obturator material and were inefficient specifically in the apical third, showing the limitation of these systems in removing obturation material from curved canals.

Safety and Effectiveness of Additional Apical Preparation using a Rotary Heat-treated Nickel-Titanium file with Larger Diameter and Minimum Taper in Retreatment of Curved Root Canals.

Objective The aim of this study was to investigate the effect of additional apical preparation using the ProDesign Logic (PDL) 50/.01 rotary heat-treated nickel–titanium (NiTi) file with a larger diameter and minimal taper for retreatment of curved root canals. Materials and Methods Mesial curved root canals of 12 mandibular molars were prepared using PDL 25/.06 and filled using the continuous wave of condensation technique and AH Plus sealer. After retreatment using ProDesign S (PDS) 25/.08, PDL 25/.06 and PDL 35/05, a complementary procedure was performed with PDL 50/.01. Microcomputed tomography (micro-CT) scanning was performed before and after retreatment procedures. The cyclic fatigue resistance of unused PDS 25/.08, PDL 25/.06, PDL 35/.05 and PDL 50/.01 instruments ( n = 12) was evaluated in a stainless-steel device. Statistical Analysis Data on the volumes of the root canals and the remaining filling materials were submitted to the paired t-test. Cyclic fatigue resistance data was submitted to one-way ANOVA and Tukey’s tests (α = 0.05). Results Use of PDL 50/.01 decreased the remaining filling materials in the apical third ( p < 0.05). The root canal volume was similar in the cervical and middle thirds before and after preparation using PDL 50/.01 ( p > 0.05). PDL 50/.01 presented the highest cyclic fatigue resistance ( p < 0.05). Conclusions Use of the PDL 50/.01 instrument as an additional apical preparation for retreatment of curved root canals improved filling material removal in the apical third, while maintaining the dentin in the cervical and middle thirds. In addition, PDL 50/.01 presented high-flexural resistance.

Experimental research of retreatability of root canals filled with bioceramic sealer

Aim. This study aimed to determine the residual filling material and the reestablishment of working length and apical patency after retreatment of TotalFill® BC Sealer.Materials and methods. Thirty maxillary incisors were used in a single cone obturation technique with TotalFill BC Sealer (FKG, Switzerland) and randomly divided into 6 groups according to the retreatment technique. The samples were scanned by micro-CT before and after retreatment in order to evaluate the residual filling material on the root dentin walls. Data were analyzed statistically with the Kruskal-Wallis tests (p &lt; 0.05).Results. No instrumental protocol was able to remove all obturation material from the root canals. The differences in reduction in the total volume of filling material percentage were statistically significant (p &lt; 0.05) among groups with and without ultrasound or sound activation in retreatment protocol.Conclusions. The most effective root canal retreatment was determined with D-Race + XP-endo Shaper + XP-Endo Finisher R + US (3.0 W) protocol; it was found that the efficiency of filling material removal was more than 92%.

Efficacy of reciprocating and rotary retreatment nickel-titanium file systems for removing filling materials with a complementary cleaning method in oval canals.

ObjectivesThis study aimed to evaluate and compare the efficacy of the S1 reciprocating system and the D-Race retreatment rotary system for filling material removal and the apical extrusion of debris.Materials and MethodsSixty-four freshly extracted maxillary canines were shaped with size 10 and size 15 K-files, instrumented using ProTaper Gold under irrigation with 2.5% sodium hypochlorite (NaOCl), obturated according to the principle of thermo-mechanical condensation with gutta-percha and zinc oxide eugenol sealer, and allowed to set for 3 weeks at 37°C. Subsequently, the teeth were divided into a control group (n = 4), the D-Race rotary instrument group (n = 30), and the S1 reciprocating instrument group (n = 30). After classical retreatment, the canals were subjected to a complementary approach with the XP-Endo Shaper. Desocclusol was used as a solvent, and irrigation with 2.5% NaOCl was performed. Each group was divided into subgroups according to the timing of radiographic readings. The images were imported into a software program to measure the remaining filling material, the apical extrusion, and the root canal space. The data were statistically analyzed using the Z-test and JASP graphics software.ResultsNo significant differences were found between the D-Race and S1 groups for primary retreatment; however, using a complementary cleaning method increased the removal of remnant filling (p < 0.05).ConclusionsClassical removal of canal filling material may not be sufficient for root canal disinfection, although a complementary finishing approach improved the results. Nevertheless, all systems left some debris and caused apical extrusion.

Removal of filling material using rotating or reciprocating systems with or without solvent: microCT analysis.

This study evaluated by the efficacy of rotary and reciprocating systems to remove filling material by micro-CT. Sixty human canines were instrumented up to F3 by ProTaper Universal system and filled with AH Plus/ gutta-percha. Specimens were submitted to 1,200 thermal cycles, scanned by microCT SkyScan 1176 and randomly distributed in groups according to the filling material removal protocol (n = 10): ProTaper Universal, ProTaper Universal/xylol, ProTaper Next, ProTaper Next/xylol, Reciproc and Reciproc/xylol. After filling removal, specimens were analyzed by microCT to separately verify the presence of gutta-percha and sealer remnants. The data (mm3) were analyzed by two way ANOVA and Tukey´s test (5%). For the remaining gutta-percha, the use of xylol provided the highest values for Reciproc (7.60 ± 0.73), which was statistically different from the other groups (p < 0.05) that presented lower values and were statistically similar to each other (p > 0.05). For the remaining sealer, xylol provided the highest values (21.25 ± 6.94) different (p < 0.05) from the use of instruments alone (11.47 ± 9.45). ProTaper Next presented the lowest values (8.16 ± 1.37) for the remaining sealer, different (p < 0.05) from that of Reciproc (24.67 ± 6.32). The qualitative analysis revealed that all groups presented some remaining root filling material, with the highest volumes presented by Reciproc and Reciproc/xylol. In conclusion the rotary systems provided greater removal of filling material regardless of the use of xylol. The use of xylol negatively interfered with the action of the reciprocating system in the removal of the gutta-percha and sealer.

Supplementing filling material removal with XP-Endo Finisher R or R1-Clearsonic ultrasonic insert during retreatment of oval canals from contralateral teeth.

This study compared the efficacy of XP-Endo Finisher R and R1-Clearsonic insert in removing filling material remnants from oval canals. Twelve pairs of contralateral premolars were treated and subsequently retreated with Reciproc 50. A supplementary procedure with XP-Endo Finisher R or R1-Clearsonic was performed. Micro-computed tomography was used to quantify the filling material volume in the full canal and apical segment lengths. Intragroup analyses revealed significant reduction of filling material after both procedures, not only in the full canal but also in the apical segment (P<0.05). The amount of filling material removed with XP-Endo Finisher R and R1-Clearsonic was 47.9% and 52.2% in the apical segment (P>0.05), and 82.1% and 64.6% in the full canal (P<0.05). None of the instruments was effective in completely removing the filling material from the apical canal, but better results were obtained with XP-Endo Finisher R in the total canal length.

Micro–computed Tomographic Assessment of Supplementary Cleaning Techniques for Removing Bioceramic Sealer and Gutta-percha in Oval Canals

IntroductionThe aim of this study was to conduct a micro–computed tomographic assessment of the effectiveness of 3 supplementary cleaning techniques in reducing the residual volume of gutta-percha and a bioceramic sealer after performing endodontic retreatment procedures in teeth with oval canals. MethodsThirty-six mandibular premolars were instrumented with the ProTaper Next system (instruments X1–X3; Dentsply Maillefer, Ballaigues, Switzerland) and filled with gutta-percha and Bio-C Sealer (Angelus, Londrina, PR, Brazil) using the single-cone technique. The teeth were reinstrumented with the Reciproc R40 instrument (VDW, Munich, Germany) and divided into 3 groups according to the supplementary cleaning technique used (n = 12): ultrasonic-assisted irrigation (UAI), EndoActivator (Dentsply Tulsa Dental Specialties, Tulsa, OK) irrigation (EAI), or the XP-endo Finisher R system (XPR; FKG Dentaire, La Chaux-de-Fonds, Switzerland). Micro–computed tomographic imaging was used to quantify the residual volume of filling material. One-way analysis of variance complemented by the Tukey test was used to perform the statistical analysis (P < .05). ResultsSignificant reductions were obtained in the residual filling material after supplementary cleaning (P < .05). XPR (47.5%) led to significantly greater (P < .05) filling material removal than UAI (16.6%) or EAI (22.6%). The removal values of the 2 latter systems were not significantly different. ConclusionsXPR was more effective than UAI and EAI in removing filling material in mandibular premolars with oval canals. None of the tested supplementary cleaning techniques completely removed the residual filling material.

Micro-CT analysis of dentinal microcracks on root canals filled with a bioceramic sealer and retreated with reciprocating instruments

This study aimed to analyze the potential occurrence of dentinal defects after the removal of a root canal filling with two different sealers using Reciproc (RC) or Reciproc Blue (RB). The mesial roots of 60 mandibular molars with a Vertucci type IV configuration were selected. The samples were initially instrumented with Reciproc (R25) and then divided into the following four experimental groups according to the endodontic sealer and retreatment instrument (n = 15): BC Sealer/Reciproc (BCRC); BC Sealer/Reciproc Blue (BCRB); AH Plus/Reciproc (AHRC); and AH Plus/Reciproc Blue (AHRB). Then, the samples were scanned under micro-CT after obturation and removal of the filling material. Two analyses were conducted. First, an evaluation was performed on all the axial images, and another analysis evaluated each millimeter of the 10 mm from the apex. Dentinal defects were observed in all the samples. All of the identified defects in the images after filling material removal were present in the corresponding images after obturation. The use of AH Plus and EndoSequence BC Sealer, and filling material removal using RC and RB instruments did not induce dentinal defects.
