Prostatic Urethral Polyp Causing Lower Urinary Tract Obstruction: Report of a Case

Polyps of the male urethra are relatively uncommon lesions. They usually arise from the prostatic urethra mostly verumontanum or Just lateral to the verumontanum. Posterior urethral polyp often presents with hematuria, hemospermia and sometimes lower urinary tract obstruction. We recently experienced a pedunculated prostatic urethral polyp in a 63-year-old man who complained of gross total hematuria and difficult micturition for several years. On rectal examination the prostate was slightly enlarged with normal consistency. Excretory urogram showed a round filling defect in the right side of the bladder neck measuring about in size. Cystourethroscopy revealed trabeculation of the vesical wall, mild bilateral prostatic hypertrophy and a round cystic mass with a long stalk arising from the prostatic floor 1 cm. Proximal to the verumontanum which caused ball-valve obstruction in the bladder neck. This pedunculated polypoid mass was then resected at its base with resectoscope and was removed transurethrally using Lowsley's grasping forceps. The specimen was proved as fibrous polyp histologically.

Fibrous polyp of the ureter.

The most frequent ureteral tumors are transitional carcinoma. Benign neoplasms are uncommon. In this paper we present 6 cases of ureteral polyps, which represent 11.1% of all the tumors in our series of ureteral tumors. We have reviewed the possible etiopathogenesis and exposed the clinical characteristics. Secondary to the obstructive phenomenon and the radiologic signs, it is the tigroid appearance, the localization and the absence of total obstruction which allows the diagnostic suspicion. The treatment of choice is partial resection. Ureterostomy with peroperatory biopsy, in cases in which the malignancy is not yet established, is recommended in order to avoid unnecessary nephroureterectomies.

The diagnosis and management of benign fibrous ureteric polyps.

The diagnosis and management of benign fibrous ureteric polyps are discussed. A pre-operative diagnosis is suggested by a long history of loin pain or haematuria, both, in a younger patient with the radiological findings of ureteric obstruction or a radiolucent ureteric filling defect. The diagnosis is confirmed at operation by opening the ureter and exposing characteristic fronds of polyps arising from a common base. It is suggested that management should be conservative local excision and not nephroureterectomy, as previously practised.

Obstructing Polyps of the Posterior Urethra in Boys: Embryology and Management

Solitary fibrous polyps are an uncommon but important cause of obstructive uropathy, infection and hematuria in boys. The clinical presentation and management in 5 boys are discussed and the literature is reviewed. Emphasis is placed on the pitfalls in diagnosis and recent endoscopic techniques of treatment.

GIANT FIBROUS POLYP OF THE LOWER END OF THE URETER: CASE REPORT AND REVIEW OF THE LITERATURE

Primary benign mesodermal tumours of the ureter mostly occur in the proximal part of the ureter. A case of a large fibrous polyp of the lower end of the ureter is described. The clinical presentation, radiological findings and treatment are discussed.

Well-differentiated adenoma at the anorectal junction.

Twenty-six cases of patients having adenomatous polyps at the anorectal junction are presented. Most polyps in this region were well-differentiated, and atypical polyps were infrequently seen. Well-differentiated adenomatous polyps in the anorectal junction were larger in average size than ordinary colorectal polyps having the same grade of epithelial pseudostratification. The reason adenomatous polyps at the anorectal junction grow to large sizes with only a relatively rare occurrence of malignant degeneration is discussed in relation to their specific location. Some well-differentiated adenomas were considered to develop secondarily in the bases of hemorrhoids or in pre-existing fibrous polyp that were the end result of hemorrhoids.

Fibrous Polyps of the Ureter

Two cases of fibrous polyps of the ureter are reported. In 1 case there was coexistent transitional cell carcinoma. Certain changes suggesting a benign lesion are described. Fibrous polyps are best treated by local excision.

Lymphangiectatic fibrous polyp of tonsil

A case of lymphangiectatic fibrous polyp of the tonsil is reported. Relavant literature is discussed in relation to the present findings.

Multiple fibrous polyps of ureter

A case is presented of multiple fibrous polyps of the upper ureter associated with ureterovesical obstruction in an eleven-year-old girl. Local excision of the polyps was done with reimplantation of the ureter in the bladder. Follow-up for forty four months showed a good functional result with no evidence of recurrent disease.

Inflammatory fibrous polyp (pseudotumor) of ileum.

Clinically and grossly, an inflammatory fibrous polyp of the ileum may resemble an ulcerated polypoid carcinoma and may produce intussusception or partial bowel obstruction. Only eight cases of this distinctive lesion have been reported in the literature; the findings in these cases and in three new cases discussed here.

Papillary adenofibroma of the uterine cervix

Three papillary tumors of the uterine cervix considered to be true fibromas of the substantia propria are described. Their clinical and histologic features are different from those of the usual endocervical mucosal polyp. The descriptive term papillary adenofibroma is used to avoid confusion with polypoid “fibroid” and fibrous polyp. There is no indication that the neoplasm has any tendency to sarcomatous change.

A Case of Resected Mediastinal Dermoid Cyst

The case is 47 years old moderately well developed poorly, nourished male farmer. Family history and past history are not contributory. Symptomes started 8 years ago with right shoulder and upper chest pain, and followed by fever and productive cough in few month. Smelled foul smell in expectoration and increased in amount gradually. Hemoptysis started 5 years ago, and repeated it several times every year thereafter. In the past several years he often spat up whitish hair mixed with sticky expectoration. On admission, he appeared poorly nourished, pale and chronically ill-appearing. But was slightly dyspneic only on exertion without high fever. Physical examination revealed no other visible defect or deformity in any part of body, no any palpaale nodules. There was dullness on percussion on right upper chest with markedly diminished breathing sound Whowed marked clubbing fingers. B.P.90/60, Hemoglobin 6.8 gm%, sputum negative for AFB though it was once diagnosed as pulmonary tuberculosis in the past, and others insignificant. During the hospitalization, he used to raise up 300-650 ml of sputum but continued with normal fever, and hemoglobin also improved to 11.3 gm%, Eryhorocyte sedimentation rate improved form 70 mm, to 23,,./hr. Totla serum protein increased slightly with great increase of γ-globulin. ECG was normal. Bronchoscopy showed a big tumor mass in posterior wall of right main bronchus, and the biopsy specimen from that lesion was reported as fibrous polyp. Bacterial culture of sputum showed ordinary throat flora bvut no predominant one. Pneumonectomied right lung contained big fabric polyps protruding into the bronchus which iave impressing of arising from the bronchus. Itwas hard to find where did it come from even in operation or on histopathological examination but he tissue was found to contain tubular structure which is covered by pseudostratified epithelium., cartilage plate, mucosal mixed glands, and also mixed with pancreatic tissues. The polyp was also covered by the suin which is consisting of hair follicles, sebaceous gland and sweat glands.

Neurofibroma of the larynx.

IN SPEAKING of of the larynx it is necessary to remember that the majority of so-called tumors are not true neoplasms at all. They are, rather, inflammatory swellings or hyper-plastic growths which originate as physiologic responses to some form of irritation. The terms fibroma, angioma, fibroangioma, or laryngeal polyps are often applied incorrectly to such lesions. Nevertheless, true benign neoplasms of the larynx do occur, though not very frequently. They may be of epithelial or connective tissue origin. Those growths that arise from connective tissue are usually termed fibromas, though true fibromas of the larynx are very rare. Such tumors are usually benign tumors of inflammatory or traumatic origin, as, for example, organizing hematomas or fibrous polyps. Even more uncommon than the true fibroma is the neurofibroma. New and Erich 1 reported 1 neurofibroma among 722 benign tumors of the larynx, which included both neoplastic and non-neoplastic growths.

On surgical treatment of fibrous polyps of the skull base

Based on the material of 18 cases, the author believes that each case of skull base polyp should be surgically strictly individualized. Where it is possible to remove the polyp through natural routes, through the nose and mouth, the latter should be used in the first place.
