Acid fast bacteria (AFB) are cell walls containing complex lipids so that in Ziehl-Neelsen staining they can bind dyes because they are not washed off by 3% HCl Alcohol (Alcoholic Acid). One of the host factors in pulmonary tuberculosis is smoking habits and exposure. Smoking habits will damage the defense mechanism of the lungs, which causes the lungs to be more easily infected with tuberculosis germs. The purpose of this study was to determine the results of analysis of acid-fast bacteria in the sputum of active smokers. The research method used was descriptive cross-sectional by examining acid-fast bacteria in the sputum of active smokers suspected of having pulmonary tuberculosis by microscopic examination of sputum (direct smear smear) using the Ziehl-Neelsen staining method. smear examination with Ziehl-Neelsen staining in patients with tuberculosis at Putri Hijau Hospital in 2022, totaling 20 samples, found 4 positive results and 16 samples with negative results.

Risk Factors of Pulmonary Tuberculosis in the Working Area of Perumnas Public Health Center Kendari City

Kendari City is the area with the highest number of TB cases in Southeast Sulawesi Province with a total of 488 cases in 2019. Preliminary data at the Perumnas Public Health Center showed that there were 49 TB cases in 2019. This study aims to determine the risk factors for the incidence of pulmonary TB in the Work Area. Public Health Center. This study uses a case control design. The study population was 105 patients, with a sample consisting of 44 case samples and 44 control samples, which were taken by simple random sampling. Data analysis using Chi Square test and Odds Ratio. From the results of the study, it was found that there were significant risk factors between smoking habits (OR = 5,156), contact history (OR = 8,333), occupancy density (OR = 2,544), knowledge (OR = 3,852) and ventilation (OR = 3,071) with the incidence of pulmonary tuberkulosis. The conclusion of this study is smoking habits, contact history, occupancy density, knowledge, and ventilation are risk factors for the incidence of pulmonary tuberkulosis at the Perumnas Public Health Center. Therefore, it is suggested to the health workers of the Puskesmas are expected to continue to provide health promotion and improve work programs related to pulmonary TB in order to increase knowledge and awareness of the community to prevent transmission of pulmonary TB.

Prevalence and risk factors of pulmonary tuberculosis among people living with diabetes mellitus at the National Referral Hospital, Thimphu, Bhutan

Introduction: Diabetes mellitus (DM) is increasing the risk of pulmonary tuberculosis (PTB) with concern over the convergence of these two diseases. Although screening for PTB among people with DM has been recommended by WHO, it has not been implemented in Bhutan yet. Objective: The aim of this study was to determine the prevalence and risk factors of PTB among people living with DM at the National Referral Hospital (NRH), Thimphu, Bhutan. Methods: This was a cross-sectional study conducted from 1st June – 9th October 2018. All 343 consenting DM patients were screened for PTB using the WHO recommended questionnaire and those with TB positive symptoms were tested for active PTB using sputum smear microscopy and Gene-Xpert. A descriptive statistical analysis was performed using SPSS 21. Results: The prevalence of PTB among people living with DM was 0.87% (95% CI: 0.20-2.50).The mean age was 54.95 (± 13.2 years) and all were DM type II. The median duration of diabetes was 4 years (range of 0.1-40 years), an average level of HbA1c 7.47 (± 2.6), and median duration on DM medication was 4 years (range of 0.1-40 years) with the majority (88.9%) on oral hypoglycemic drugs. Good glycemic control (HbA1c< 7) was achieved by 39.4% of individuals. PTB risk factors included PTB positive symptoms (7.0%), prior PTB history (5.5%) and recent contact with PTB patient (5.0%). Additional known PTB risk factors were smoking (7.6%), alcohol use (4.1%) and low body mass index (1.5%). Conclusions: Although the prevalence PTB was low, health education, proper case management and risk based screening for PTB among DM is recommended.

Relapse and influencing factors of pulmonary tuberculosis in Chongqing

Objective To study the epidemiological characteristics and influencing factors of relapse in new tuberculosis patients who treated successfully in Chongqing, we provide the basis for formulating the intervention measures to reduce the relapse. Methods With a retrospective cohort study, the data of new tuberculosis patients registered and their relapse condition in 2009 were derived from the Tuberculosis Information Management System (TBMIS). A proportional risk regression model (Cox regression model) was used to analyze the relapse and its influencing factors. Results A total of 23 034 new patients with successful treatment in 2009 were included in this study. During the observation period of 2009-2019, a total of 519 new tuberculosis patients relapsed after successful treatment, with a relapse rate of 2.25% (519 / 23 034). Patients with relapse at 3 years and 5 years accounted for 60.31% (313 / 519) and 77.07% (400 / 519), respectively. The univariate analysis showed that the relapse rate was higher in males, aged≥25 years, ethnic minorities, farmers, positive etiology patients, severe cases and patients with pulmonary cavity, the difference was statistically significant ( P <0.05). Multifactorial analysis showed that males, aged≥25 years, ethnic minorities, farmers, positive etiology and pulmonary cavity were the risk factors for pulmonary tuberculosis relapse ( P <0.05). Conclusion After successful treatment, the proportion of new tuberculosis patients in Chongqing is relatively high at 5 years, multiple risk factors could lead to relapse. Therefore, we should focus on the high-risk population of tuberculosis, strengthen the follow-up of patients after successful treatment, and take intervention measures as soon as possible, reduce the relapse, so as to finally reduce the incidence of tuberculosis. 摘要：目的 分析重庆市初治肺结核患者成功治疗后复发流行病学特征及复发影响因素, 为制定减少复发干预措 施提供基础依据。 方法 采用回顾性队列研究方法, 从结核病信息管理系统中导出 2009 年登记的成功治疗的初治肺 结核患者信息及其复发病案信息, 采用比例风险回归模型 (Cox 回归模型) 对复发相关因素进行分析。 结果 共导出 2009 年成功治疗的初治患者 23 034 例资料, 在 2009—2019 年观察期间共有 519 例成功治疗后的初治肺结核患者复发, 复发率为 2.25% (519/23 034) , 其中 3 年内复发患者和 5 年内复发患者分别占总复发患者的 60.31% (313/519) 和 77.07% (400/519)。单因素分析显示, 男性、≥25 岁人群、少数民族、农民、病原学阳性、重症和肺部有空洞者复发比例较高, 差 异具有统计学意义 ( P <0.05)。多因素分析结果显示, 男性、≥25 岁人群、少数民族、农民、病原学阳性、肺部有空洞是肺 结核复发的危险因素 ( P <0.05)。 结论 重庆市成功治疗后的初治肺结核患者 5 年复发比例较高, 多种危险因素可导致 复发的发生, 应重视复发高危人群, 强化对成功治疗患者的随访工作, 尽早采取干预措施, 减少复发, 最终达到降低肺 结核疫情的目的。

Prevalence and Associated Risk Factors of Pulmonary Tuberculosis in Diabetic Patients

Background: One-third of India’s population is infected with mycobacterium tuberculosis; there are 12 million active tuberculosis cases in India. One person dies of Tuberculosis every minute in India. No other disease has so much sociological, economic and health significance as Tuberculosis has. In 2005 there were an estimated 8.8 million new cases of Tuberculosis worldwide, with 1.9 million of those occurring in India. In India in 2000, there were an estimated 481,573,000 people over the age of 25 years. Among these, 4.3% i.e. around 20,707,639 had diabetes, 19 and 939,064 developed. Subjects and Methods: The study conducted over a period of 2 yrs. Adult patients with diabetes mellitus with pulmonary Tuberculosis were included, while Diabetic patients with extrapulmonary Tuberculosis were excluded from the study. Results: The maximum incidence of pulmonary Tuberculosis was noted above the age of 30yrs were 13(26.0%). The peak incidence was in the age group of 31 – 40 and 41 – 50 yrs. The mean age is 52.62. As Shown in [Figgure 1], the total numbers of males were 64% and females were 36%. The male to female ratio was 2.11:1 As Shown in [Table 3], the distribution of patient according to their occupation noted were Skilled (18%), Unskilled (38%), Business (16%), Service (2%), Housewife (22%), and Student (4%). As shown in [Table 4], the socio-economic status among 50 patients was distributed into: Upper (2%), Upper Middle (22%), Upper lower (32%), Lower (12%), Lower middle (32%), and Lower-middle (32%). Conclusion: Early diagnosis and properly monitored treatment regimen is the only time tested answer to this problem. All diabetics at the initial diagnosis and every year must have an x-ray chest done. All diabetics with abnormal weight loss, unexplained cough, or sudden increase of insulin requirement should have sputum examination and chest x-ray done.

Prognostic value of serum macrophage migration inhibitory factor levels in pulmonary tuberculosis

BackgroundMacrophage migration inhibitory factor (MIF) makes chemokine-like functions and plays critical roles in various inflammatory diseases. This study was designed to explore the significance of MIF serum levels in predicting the prognosis of pulmonary tuberculosis (PTB) following anti-TB treatment.MethodsPatients diagnosed with culture-confirmed PTB without treatment were included and the serum was collected. Levels of MIF in serum were quantified with immunoassay, and the levels of established biomarkers were also determined, including C-reactive protein (CRP) and Interleukin 6 (IL-6). The outcome was estimated with all-cause mortality, with the mortality in 12 months as the primary outcome and the mortality in 3, 6, 9 months as other outcomes. The prognostic value of MIF and other factors in PTB were tested.ResultsTwo hundred eighty-seven PTB patients were included. The median MIF levels in patients with advanced disease, disseminated and drug-resistant TB were significantly higher than that observed in mild -to- moderate disease, non-disseminated and drug-sensitive TB. MIF levels in patients with the outcome of death were higher than those survived [28.0 ng/ml (Inter-quartile range [IQR]: 24.2–33.1) vs. 22.3 ng/ml (IQR: 18.7–26.5); P < 0.001]. Multivariate model analysis was performed for comparing the highest quartiles to the lowest quartile of MIF levels. MIF levels were related to the mortality, with an elevated mortality risk of 236% [Odds ratio (OR) = 3.36; 95% Confidence interval (CI): 1.21–15.14; P = 0.012]. The model was re-analysis after combing MIF with currently established risk indicators. The obtained Area Under the Receiver Operating Characteristic Curve (±standard error) was elevated from 0.81 (±0.035) to 0.84 (±0.031), with a significant difference before and after adding the MIF (difference, 0.03[0.004]; P = 0.03).ConclusionSerum level of MIF was a better biomarker than CRP or IL-6 for predicting death in HIV-negative PTB patients, and increased MIF serum levels were related to higher mortality.

Space-time clustering and associated risk factors of pulmonary tuberculosis in southwest China

BackgroundPulmonary tuberculosis (PTB,both smear positive and smear negative) is an airborne infectious disease of major public health concern in China and other parts of the world where PTB endemicity is reported. This study aims at identifying PTB spatio-temporal clusters and associated risk factors in Zhaotong prefecture-level city, located in southwest China, where the PTB notification rate was higher than the average rate in the entire country.MethodsSpace-time scan statistics were carried out using PTB registered data in the nationwide TB online registration system from 2011 to 2015, to identify spatial clusters. PTB patients diagnosed between October 2015 and February 2016 were selected and a structured questionnaire was administered to collect a set of variables that includes socio-economic status, behavioural characteristics, local environmental and biological characteristics. Based on the discovery of detailed town-level spatio-temporal PTB clusters, we divided selected subjects into two groups including the cases that resides within and outside identified clusters. Then, logistic regression analysis was applied comparing the results of variables between the two groups.ResultsA total of 1508 subjects consented and participated in the survey. Clusters for PTB cases were identified in 38 towns distributed over south-western Zhaotong. Logistic regression analysis showed that history of chronic bronchitis (OR = 3.683, 95% CI: 2.180–6.223), living in an urban area (OR = 5.876, 95% CI: 2.381–14.502) and using coal as the main fuel (OR = 9.356, 95% CI: 5.620–15.576) were independently associated with clustering. While, not smoking (OR = 0.340, 95% CI: 0.137–0.843) is the protection factor of spatial clustering.ConclusionsWe found PTB specially clustered in south-western Zhaotong. The strong associated factors influencing the PTB spatial cluster including: the history of chronic bronchitis, living in the urban area, smoking and the use of coal as the main fuel for cooking and heating. Therefore, efforts should be made to curtail these associated factors.

Tuberculosis (TB) continues to be an important public health problem worldwide. It is a leading cause of disability and death in many parts of the world especially in the developing countries. Ethiopia has already a record of history of TB around greater than 58 years. The aim of the study was to determine the prevalence and associated factors of pulmonary tuberculosis (PTB) in Harar TB control center. A health institution based cross sectional study was conducted in Harar town, Eastern Ethiopia from March 5, 2009 to May 7, 2009. All patients who were above 5 years of age and visited Harar TB control center during the study period were enrolled consecutively. Sputum specimens were collected based on standard operating procedures and processed using hot Ziehl-Neelson staining technique. Structured questionnaires were used to collect data on socio-demographic and associated factors. Data were cleaned, coded and entered using EPIDATA version 3.1 and analyzed using SPSS version 16 software. P-values < 0.05 were taken as statistically significant. A total of 108 Pulmonary Tuberculosis (PTB) suspected patients were enrolled in the study. The overall prevalence of PTB was 16.7% where 22.8% of males and 9.8% of females were PTB smear positive. The prevalence of PTB was significantly higher among smokers (50%), alcohol drinkers (29.3%) and rural residents (29.3) (P<0.05). The prevalence of PTB in the study area was considerably high. Smoking, drinking alcohol and rural residence (areas nominated by the government as “rural” which lack important infrastructures) were significant predictors of PTB. Therefore; health education, early diagnosis and treatment are vital to combat PTB and further longitudinal studies are recommended. Keywords: Prevalence, PTB, Harar, Ethiopia

Risk factors of pulmonary tuberculosis among diabetes mellitus patients in Denpasar City

Background and Purpose: Tuberculosis (TB) is more common in developing countries, including Indonesia. Denpasar City contributes the most on the total number of TB patients in Bali Province. There is evidence on increased burden of diabetes mellitus (DM) patients with TB comorbidity. This study aims to examine risk factors of TB among DM patients.Method: A case control study was conducted in Denpasar City. A total of 135 samples, 45 cases and 90 control, were recruited to participate in the study. Cases were DM patients with TB comorbidity and were taken from patients register between 2013 to 2015. Control were DM patients without TB comorbidity and were taken from Bali Province screening operational study. Data were collected using interviews, observations, measurements and document review. Data were analysed using bivariate analysis and multivariate with logistic regression model.Results: Demographic characteristics between cases and controls were comparable in sex and family income but not comparable in age groups, education, domicile, and employment. Risk factors of TB among DM patients were house density (AOR=36.11; 95%CI: 5.0-259.9), undernutrition (AOR=24.76; 95%CI: 3.7-162.3), inadequate glycemic control (AOR=12.64; 95%CI: 2.4-66.2), and being employed (AOR=10.55; 95%CI: 1.2-92.7). TB infection among DM patients were associated with aged&lt;60 years (AOR=7.47; 95%CI: 1.2-44.3) and being male (AOR=5.42 ;95%CI: 1.2-24.0). TB Co-infection among DM patients were also associated with low education level (AOR=6.96; 95%CI: 1.0-48.6), contact with TB patients (AOR=5.84; 95%CI: 0.7-46.6) and inadequate house ventilation (AOR=2.92; 95%CI: 0.6-13.1).Conclusion: Socio-demographic characteristic, physical environments of the house and clinical conditions are risk factors of TB among DM patiens.

Clinical characteristics and prognostic factors of pulmonary tuberculosis with concurrent lung cancer

To explore the clinical characteristics and prognostic factors of pulmonary tuberculosis with concurrent lung cancer. Comprehensive analyses were conducted for 58 cases of pulmonary tuberculosis patients with lung cancer. Their clinical symptoms, signs and imaging results were analyzed between January 1998 and January 2005 at Qingdao Chest Hospital. Kaplan-Meier method was utilized to calculate their survival rates. Nine prognostic characteristics were analyzed. Single factor analysis was performed with Logrank test and multi-factor analysis with Cox regression model. The initial symptoms were cough, chest tightness, fever and hemoptysis. Chest radiology showed the coexistence of two diseases was 36 in the same lobe and 22 in different lobes. And there were pulmonary nodules (n = 24), cavities (n = 19), infiltration (n = 8) and atelectasis (n = 7). According to the pathological characteristics, there were squamous carcinoma (n = 33), adenocarcinoma (n = 17), small cell carcinoma (n = 4) and unidentified (n = 4) respectively. The TNM stages were I (n = 13), II(n = 22), III (n = 16) and IV (n = 7) respectively. The median survival period was 24 months. And the 1, 3, 5-year survival rates were 65.5%, 65.5% and 29.0% respectively. Single factor analysis showed that lung cancer TNM staging (P = 0.000) and tuberculosis activity (P = 0.024) were significantly associated with patient prognosis. And multi-factor analysis showed that lung cancer TNM staging (RR = 2.629, 95%CI: 1.759-3.928, P = 0.000) and tuberculosis activity (RR = 1.885, 95%CI: 1.023-3.471, P = 0.042) were relatively independent prognostic factors. The clinical and radiological characteristics contribute jointly to early diagnosis and therapy of tuberculosis with concurrent lung cancer. And TNM staging of lung cancer and activity of tuberculosis are major prognostic factors.

Usefulness of serum procalcitonin levels in pulmonary tuberculosis

There are very few data on serum procalcitonin (PCT) levels in pulmonary tuberculosis (PTB) patients who are negative for HIV. We assessed serum PCT in consecutive patients diagnosed with pulmonary tuberculosis or community-acquired pneumonia (CAP) on admission to discriminate between PTB and CAP, and examined the value of prognostic factors in PTB. 102 PTB patients, 62 CAP patients, and 34 healthy volunteers were enrolled. Serum PCT in PTB patients was significantly lower than in CAP patients (mean ± sd 0.21 ± 0.49 versus 4.10 ± 8.68 ng·mL⁻¹; p < 0.0001). By receiver-operating characteristic curve analysis, serum PCT was an appropriate discrimination marker for PTB and CAP (area under the curve 0.866). PTB patients with ≥ 0.5 ng·mL⁻¹ (normal cut-off) had significantly shorter survival than those with < 0.5 ng·mL⁻¹ (p < 0.0001). Serum PCT is not habitually elevated in HIV-negative PTB patients and is a useful biomarker for discriminating between PTB and CAP; however, when serum PCT is outside the normal range, it is a poor prognostic marker.

Prognostic factors in pulmonary tuberculosis requiring mechanical ventilation for acute respiratory failure

The prognosis in patients with pulmonary tuberculosis and acute respiratory failure requiring mechanical ventilation is believed to be poor. The aim of this study was to identify factors contributing to in-hospital mortality in these patients. The medical records of 32 patients with active pulmonary tuberculosis as a primary cause of acute respiratory failure requiring mechanical ventilation in the medical intensive care unit (ICU) of a tertiary referral hospital over a 10-year period were reviewed retrospectively, and predictors of mortality were assessed. The patients' median age was 69 years (range 25-88 years). The median length of intensive care unit stay was 11 days (range 2-88 days), and the median duration of mechanical ventilation was 9 days (range 2-86 days). Overall in-hospital mortality was 59% (19/32). Independent predictive factors of in-hospital mortality included tuberculous-destroyed lungs (hazard ratio 6.61, 95% CI: 1.21-36.04, P = 0.029), Acute Physiology and Chronic Health Evaluation II scores > or =20 (hazard ratio 4.90, 95% CI: 1.43-16.80, P = 0.012) and sepsis (hazard ratio 5.84, 95% CI: 1.63-20.95, P = 0.007). Acute respiratory failure caused by pulmonary tuberculosis necessitating mechanical ventilation has a high mortality rate and poor prognosis, particularly in patients with tuberculous-destroyed lungs, high Acute Physiology and Chronic Health Evaluation II scores and sepsis.

The Psychosomatic Factors in Pulmonary Tuberculosis: A Study of One Hundred Consecutive Army Cases

1. A psychosomatic study of one hundred army patients with pulmonary tuberculosis was made. 2. This study was conducted early in the course of pulmonary tuberculosis. 3. In these patients there are indications of a characteristic personality pattern. 4. The characteristic personality pattern found was used as the basis for an educational, occupational and recreational program. 5. It would appear to be important that further psychosomatic studies be made early and late in the course of pulmonary tuberculosis.
