The spontaneous rupture of the extensor pollicis longus tendon does not occur very frecuently however, it does have an importance in clinical physiotherapy practice because of its late appearance. The most common cause of this injury is the traumatism of the wrist of varying degrees. The most severe being Colles fracture. An etiopathogeny study and surgical treatment of these type of injuries have been undertaken. The functional result measured by the Evaluation Scale for Geldmacher's thumb and the Specific Extensor Indicis-Extensor Pollicis Longus Evaluation Method (SEEM) of two patients who suffered the rupture of the extensor pollicis longus tendon after distal radio fractures treated with the extensor indicis tendon transposition with good results are presented.
