Assessment of hepatitis c core antigen in epithelial salivary gland neoplasms (ex-vivo study)

BackgroundSalivary gland neoplasms (SGNs) pose a challenge to both pathologists and clinicians. Despite research, the etiology of these neoplasms remains unclear. This study aimed to identify any potential association between the presence of hepatitis C virus (HCV) at the protein or gene level and epithelial salivary gland neoplasms.MethodsFormalin-fixed paraffin-embedded (FFPE) blocks of epithelial salivary gland neoplasms were retrieved from the archives of the Oral and Maxillofacial Pathology Department, Faculty of Dentistry, Cairo University within the 5-year period from 2016 to 2020. Immunohistochemistry was used to assess HCV core antigen, while reverse transcription polymerase chain reaction was employed for the evaluation of HCV RNA.ResultsA total of 44 specimens were collected, 28 of which were benign neoplasms and 16 were malignant neoplasms. There was a statistically significant difference in HCV positivity between the two groups (P-value = 0.036). Benign tumors showed a statistically significant lower percentage of positive cases than malignant tumors. The localization of staining was also evaluated, revealing various patterns of HCV core antigen expression, including diffuse cytoplasmic, patchy cytoplasmic, nuclear, and a combination of nuclear and cytoplasmic expression. There was no statistically significant difference between the expression patterns in benign and malignant tumors (P-value = 0.616). Given that Pleomorphic Adenoma and Mucoepidermoid Carcinoma were the predominant tumor types in this study, four cases were selected for RNA detection. HCV RNA was detected in all cases using RT-PCR.ConclusionsHCV core antigen is frequently detected in SGNs and is suggested to be a potential risk factor for the development of these neoplasms. Further studies are required to discover other biomarkers, their roles, and the pathways associated with HCV in SGNs.

The article provides data on epithelial malignant tumors of the parotid salivary glands in children and adolescents. We present a historical background and modern classification proposed by the World Health Organization in 2022. We present the evolution of surgical and non-surgical treatment methods used in pediatric oncology.

FAST-FNA molecular diagnostic assay facilitates rapid diagnosis and ntrk biomarker testing of salivary gland tumors.

e18089 Background: Primary epithelial salivary gland tumors (SGTs)are classified into 31 tumor types. Diagnosing SGTs through fine needle aspiration (FNA) biopsies is challenging due to the overlapping cytomorphologic features between benign and malignant tumors. In this study, we developed an innovative, multiplexed cycling technology which allows for the rapid cellular analyses of single cells obtained from FNA which can facilitate the rapid molecular analyses and diagnosis of SGTs. Methods: Antibodies against 29 cell surface markers associated with 7 SGT subtypes were validated and chemically modified via custom linker–bio-orthogonal quencher probes (FAST). Single cell homogenates and a FNA sample set representing 7 SGT subtypes were profiled by FAST cyclic staining, imaging, and automated computational analysis. A prediction model was generated utilizing a training set of 151,926 single cells obtained from primary SGTs (N = 26) and validated on a separate cohort of SGTs (N = 30). Companion biomarker testing, such as NTRK, was also assessed with the FAST technology. Results: The customized FAST-FNA SGT molecular diagnostic and biomarker panel was validated on human cell lines and specificity confirmed by both flow cytometry and clinical immunohistochemical staining on primary SGTs. The FAST molecular diagnostic assay was able to distinguish between benign and malignant SGTs with an accuracy of 0.86 for single-cell suspension samples and 0.88 for FNA samples. Profiling of multiple markers as compared to a single tumor marker increased the diagnostic accuracy of the SGTs, 0.82 as compared to 0.65-0.74, respectively. The results of the assay were highly reproducible and independent of the cell number sampled. NTRK expression in SGTs was also assessed by the FAST assay within hours, highlighting the potential therapeutic application of this technology. Conclusions: High dimensional single-cell profiling and molecular diagnostic testing on FNA samples are feasible at relatively low cost and as a rapid test. Application of the novel multiplexed single cell technology improves the diagnostic accuracy of SGT biopsy and facilitates multiplex biomarker testing from FNA samples. The customizable and modular FAST-FNA approach has relevance to multiple pathologies and organ systems where cytologic samples are often scarce and/or indeterminate resulting in improved diagnostic workflows and timely therapeutic clinical decision making.

Parotid lymph nodes in primary malignant salivary neoplasms.

To review the literature on parotid lymph nodes and their involvement in primary malignant epithelial salivary gland neoplasms (PMESGN). The parotid gland has two identifiable lymphatic systems - extraglandular and intraglandular both topographically separate but functioning as a single unit. The publications available have concentrated on intraparotid lymph nodal metastatic (IPLN) disease only. The data reported was retrospective and observational. The limitations encountered was the variability of extent of the parotid surgery, unclear evaluation of IPLN limited or inconsistent follow-up periods, small subgroups limiting the explanatory power and the multivariate analysis. One multicentric publication reported on 289 patients with parotid PMESGN treated by surgery between 1995 and 2018 at four centres: three Italian and one in the United Kingdom. IPLN+ was proven in 22.1%, preoperative imaging failed to identify occult IPLN+ in 20.1%, IPLN+ and cervical nodal metastasis (cpN+) was 47.4% and cpN+ was 100% in patients bearing greater than 4 IPLN+ nodes. A multicentric prospective study is required detailing preoperative imaging, using a standardized surgical management, pathological specimen search, histopathological grading and patient outcome analysis specifically designed to resolve the role of IPLN in patients diagnosed with PMESGN.

Epidemiology of Primary Epithelial Salivary Gland Tumors in Southern Poland-A 26-Year, Clinicopathologic, Retrospective Analysis.

(1) Background: Epidemiological studies of epithelial salivary gland neoplasms are difficult to conduct effectively due to tumor rarity, histological heterogeneity, tumor location diversity, and a lack of national registries collecting data. This study presents 26 years of epidemiological data from a single institution in southern Poland that estimates incidence rates of primary epithelial salivary gland tumors. (2) Methods: The charts of 805 patients with epithelial salivary gland tumors were retrospectively reviewed. (3) Results: Pleomorphic adenomas occurred less frequently in elderly patients; however, Warthin tumors were more common (p < 0.001). Pediatric patients mainly suffered from mucoepidermoid carcinoma. The estimated crude and European age-standardized incidence rates of all primary epithelial salivary gland tumors were 6.7 and 6.02 per 100,000 population, respectively. The incidence rates of salivary gland tumors increased in recent years; however, this is attributed to an increase in benign tumors (p < 0.001). (4) Conclusions: The incidence of primary epithelial salivary gland tumors in southern Poland has increased over the past 26 years. This increase is attributed to a rise in the number of patients with benign tumors, particularly Warthin tumors in elderly patients. Moreover, the incidence of malignant salivary gland tumors appears to be higher in pediatric patients.

Epithelial salivary gland tumors - a monocentric retrospective studyof South Saxony-Anhalt

The purpose of this research was to analyze all epithelial salivary gland tumors in this region in a comprehensive monocentric, retrospective study. In the period from 1993 to 2017, all patients with the diagnosis of epithelial salivary gland tumors either treated at the Department of Oral and Maxillofacial Plastic Surgery of the Martin Luther University, Halle-Wittenberg (MLU), University hospital and/or processed at the Institute of Pathology of the MLU, University hospital and/or registered between 2000 and 2017 by the "Statistisches Landesamt" Sachsen-Anhalt were analyzed. The following parameters were summarized and statistically analyzed in a database using SPSS 21.5: demographic data, tumor localization, entity, therapy and disease course. 382 patients with the diagnosis of epithelial salivary gland neoplasia were identified. With 71 % the most frequent tumor localization was the glandula parotis [n = 271]. 15 % of the tumors originated from minor salivary glands [n = 57]. Most tumors were benign at over 80 % [n = 307]. In Saxony-Anhalt, 5586 patients with epithelial salivary gland tumors were reported in the mentioned period. To the best of our knowledge this is the first epidemiologic analysis of frequency, valency and therapy of salivary gland tumors in Saxony-Anhalt. The results confirm the predominance of benign epithelial salivary gland tumors, most of all pleomorphic adenoma in the glandula parotis. Concerning the group of malignant epithelial salivary gland tumors adenoid cystic carcinoma located in the minor salivary glands were most common.

Incidence and profile of benign epithelial tumors of salivary glands from a single center in Northeast of Brazil.

Background Benign tumors of the salivary glands are a group of lesions with varied histopathological and clinical spectrum. The aim was to determine the incidence and clinicopathological characteristics of benign salivary gland neoplasms diagnosed between 2007 and 2016 in a single center located in northeastern Brazil. Material and Methods Records regarding sex, age, anatomical location, histopathological subtype and treatment were retrieved, and data were analyzed using the Stata/IC software (version 12.0). Results There were above 7,100 cases of neoplasms in the head and neck region, of which 403 corresponded to salivary gland neoplasms. Of these, 238 (59%) were benign, being pleomorphic adenoma (PA) the most frequent neoplasm (n=178; 74.8%), followed by Warthin's tumor (WT) (n=23; 9.7%). Overall, most cases occurred in females (n=136; 57.1%) and age ranged from 11 to 83 years. The parotid gland (n=188; 79%) was the most common anatomical site, and all patients were treated by surgical excision. Of the cases diagnosed as PA, malignant transformation to carcinoma ex-pleomorphic adenoma (CAEXPA) occurred in 7 (3.9%) cases. Conclusions The present study confirmed the clinical and demographic profile of benign salivary gland neoplasms, which contributes to the continuous knowledge of current data about these lesions. A large malignant oncocytoma of parotid gland: a case report

&lt;p class="abstract"&gt;Malignant oncocytoma is an extremely rare neoplasm of the salivary gland accounting for 0.5% of epithelial salivary gland malignancies and 0.18% of epithelial salivary gland tumors. We report a case of malignant oncocytoma of left parotid gland in a 61-year old female patient. Patient presented with a 6-year history of painless swelling in the left parotid region. Fine needle aspiration cytology of the swelling was suggestive of pleomorphic adenoma. A computed tomography scan performed did not reveal any features suggestive of malignancy. Patient underwent left superficial parotidectomy. However, histopathological examination of the specimen turned out to be malignant oncocytoma. Malignant oncocytoma of salivary glands have rare incidence and may have similar morphologic features with other neoplasms. Despite the tumor being large and malignant in this case, there was no facial palsy preoperatively. As histopathology is often diagnostic, the possibility of malignancy always must be kept in mind preoperatively even when there is no facial palsy or fine needle aspiration cytology not showing up malignancy.&lt;/p&gt;

Epithelial salivary gland tumors: Utility of radiomics analysis based on diffusion-weighted imaging for differentiation of benign from malignant tumors.

To evaluate the utility of radiomics analysis for differentiating benign and malignant epithelial salivary gland tumors on diffusion-weighted imaging (DWI). A retrospective dataset involving 218 and 51 patients with histology-confirmed benign and malignant epithelial salivary gland tumors was used in this study. A total of 396 radiomic features were extracted from the DW images. Analysis of variance (ANOVA) and least-absolute shrinkage and selection operator regression (LASSO) were used to select optimal radiomic features. The selected features were used to build three classification models namely, logistic regression method (LR), support vector machine (SVM), and K-nearest neighbor (KNN) by using a five-fold cross validation strategy on the training dataset. The diagnostic performance of each classification model was quantified by receiver operating characteristic (ROC) curve and area under the ROC curve (AUC) in the training and validation datasets. Eight most valuable features were selected by LASSO. LR and SVM models yielded optimally diagnostic performance. In the training dataset, LR and SVM yielded AUC values of 0.886 and 0.893 via five-fold cross validation, respectively, while KNN model showed relatively lower AUC (0.796). In the testing dataset, a similar result was found, where AUC values for LR, SVM, and KNN were 0.876, 0.870, and 0.791, respectively. Classification models based on optimally selected radiomics features computed from DW images present a promising predictive value in distinguishing benign and malignant epithelial salivary gland tumors and thus have potential to be used for preoperative auxiliary diagnosis.

Surgical Treatment Outcome of Salivary Gland Tumors

Background: Salivary gland neoplasms constitute a group of heterogeneous lesions with complex clinicopathologic characteristics and distinct biological behavior. Numerous studies have suggested geographical variation, therefore the aims of this study were to analyze the characteristics of salivary gland neoplasms in two Iraqi centers and to analyze the postoperative complications that are encountered after surgical treatment of these tumors. Materials and Methods: A retrospective study of the patients who were treated for major and minor epithelial salivary gland tumors was conducted. The analyzed data included; demographic information (age and gender), the site of the tumor, the clinical manifestations, the histological type of the tumor, the type of the treatment and the postoperative complications. Results: Fifty seven patients were included in this study. The parotid gland was involved in most patients (n=37, 64.9%). Thirty five patients (61.4%) had benign tumors while 22 patients (38.6%) had malignant tumors. The mean age of patients with malignant tumors was 52.05 (±17.3) while of patients diagnosed with benign tumors was 41.6 (±11.4) years, the difference was statistically significant (p= 0.008). Surgical treatment consisted of superficial parotidectomy, total parotidectomy, surgical excision of the submandibular gland, surgical excision of minor salivary glands and maxillectomy. The most common complication was weakness of one or multiple branches of the facial nerve, complications were significantly associated with patients’ age, whereas gender and type of tumor whether benign or malignant did not affect the complication rate Conclusions: benign salivary gland tumors are more common than malignant tumors with no gender predilection and that patients who are diagnosed with malignant tumors are significantly older that patients with benign tumors, parotid gland is the most commonly affected gland with pleomorphic adenoma as the most frequent diagnosis. Facial nerve dysfunction was the most common complication and the complication rate was significantly associated with the age of patients.

Analysis of the Clinical Relevance of Histological Classification of Benign Epithelial Salivary Gland Tumours.

IntroductionA vast increase in knowledge of numerous aspects of malignant salivary gland tumours has emerged during the last decade and, for several reasons, this has not been the case in benign epithelial salivary gland tumours. We have performed a literature review to investigate whether an accurate histological diagnosis of the 11 different types of benign epithelial salivary gland tumours is correlated to any differences in their clinical behaviour.MethodsA search was performed for histological classifications, recurrence rates and risks for malignant transformation, treatment modalities, and prognosis of these tumours. The search was performed primarily through PubMed, Google Scholar, and all versions of WHO classifications since 1972, as well as numerous textbooks on salivary gland tumours/head and neck/pathology/oncology. A large number of archival salivary tumours were also reviewed histologically.ResultsPleomorphic adenomas carry a considerable risk (5–15%) for malignant transformation but, albeit to a much lesser degree, so do basal cell adenomas and Warthin tumours, while the other eight types virtually never develop into malignancy. Pleomorphic adenoma has a rather high risk for recurrence while recurrence occurs only occasionally in sialadenoma papilliferum, oncocytoma, canalicular adenoma, myoepithelioma and the membranous type of basal cell adenoma. Papillomas, lymphadenoma, sebaceous adenoma, cystadenoma, basal cell adenoma (solid, trabecular and tubular subtypes) very rarely, if ever, recur.ConclusionsA correct histopathological diagnosis of these tumours is necessary due to (1) preventing confusion with malignant salivary gland tumours; (2) only one (pleomorphic adenoma) has a considerable risk for malignant transformation, but all four histological types of basal cell adenoma can occasionally develop into malignancy, as does Warthin tumour; (3) sialadenoma papilliferum, oncocytoma, canalicular adenoma, myoepithelioma and Warthin tumour only occasionally recur; while (4) intraductal and inverted papilloma, lymphadenoma, sebaceous adenoma, cystadenoma, basal cell adenoma (apart from the membranous type) virtually never recur. No biomarker was found to be relevant for predicting recurrence or potential malignant development. Clinicopathologic and molecular features of myoepithelial tumors of salivary glands

Objective: To investigate the clinicopathological, and molecular characteristics of myoepithelial tumors (MTs) of salivary glands. Methods: A total of 37 MTs cases including 13 malignant epithelial tumors (MMTs) and 24 benign epithelial tumors (BMTs) of salivary glands were identified from the archives of the Department of Pathology, General Hospital of Eastern Theater Command, dating from 2006 to 2016. Clinical features, histological patterns, immunohistochemical characteristics and status of EWSR1 gene rearrangement by fluorescence in situ hybridization (FISH) analysis were reviewed in all cases. Results: Clinically, 37 MTs cases mainly occurred in the parotid glands, when most of the patients presented with painless masses. Of the 13 MMTs cases, male to female ratio was 7∶6, and the median age was 62 years old. Of the 24 BMTs cases, male to female ratio was 5∶7, and the median age was 54 years old. Immunohistochemically, 37 MTs cases were positive for CKpan, and at least one myoepithelial marker. Twenty six of 37 MTs cases were analyzable for the EWSR1 gene break by FISH. Based on the previous evaluation criterion, the EWSR1 translocation was detected in 4 cases of 11 MMTs, and 4 cases of 15 BMTs. According to the main histological composition of tumor cells, 4 EWSR1-positive MMTs covered 2 clear-cell cases and 2 epithelioid-cell cases, when 4 EWSR1-positive BMTs covered 2 clear-cell cases, 1 plasmacytoid-cell case, and 1 spindle-cell case. Conclusions: Males and females are affected equally. MTs express immunoreactivity for CKpan, and at least one myoepithelial marker. The EWSR1 rearrangement is present in a subset of MTs, with variable morphological characteristics, and has no statistical significance on clinical behavior.

A clinicopathological study of salivary gland tumors

Background: To evaluate tumors involving major and minor salivary glands, histopathology is an essential diagnostic method and is the gold standard. This study was carried out to know the incidence of salivary gland tumors, their clinical presentation and diverse morphological patterns. Materials and Methods: A prospective cross-sectional descriptive study was carried out over a period of two years in the department of pathology, Yenepoya Medical College hospital, Mangalore.Data regarding patient demographicswas also recorded. All the epithelial salivary gland tumors were classified according toWHO (2005) histological classification. Results: In the present study, a total of 65 cases of salivary gland tumours was found and an age range of 10-79 years was observed. The most common site affected was the parotid gland 47 (72.31%). Among the 48 (73.85%) benign tumors, pleomorphic adenoma 40 (61.54 %) was most commontumor and remaining 17(26.15%) cases were malignant, of which mucoepidermoid carcinoma 8 (12.31%) was frequently observed. Conclusion: This study highlights the usefulness of histopathologicalstudy of salivary gland tumors with various morphological patterns of benign and malignant salivary gland tumors, which are of utmost value in planning the further management of the patient.

Basal cell adenoma of the buccal mucosa masquerading as irritation fibroma: A rare case report

Basal cell adenoma (BCA) is an uncommon benign epithelial neoplasm of salivary gland consisting of 1%–2% of all salivary gland epithelial tumors. Frequently, these tumors occur in major salivary glands, mostly in the parotid gland. These tumors rarely occur in minor salivary gland. We report a case of BCA in a 54-year-old male patient having a growth in the left upper gingivobuccal sulcus at maxillary tuberosity region with respect to 27 and 28, which was clinically presented as irritation fibroma and was histologically diagnosed as BCA. The dilemma in diagnosis of this rare lesion, histologic paradox, and insufficient descriptive studies has directed us to present this case to serve as a guide for clinicians for proper clinicopathological and radiological analysis of such lesions.

Primary epithelial tumours of the salivary glands are very rare in paediatric patients. The aim of this study was to evaluate the clinical course, treatment, and outcomes of these uncommon neoplasms based on the authors’ experience and the recent literature. The medical charts of 12 female patients and seven male patients with primary epithelial salivary gland tumours were reviewed. All were under 19 years of age and underwent surgical treatment between 1994 and 2016. The results of this group of paediatric patients were compared with those of 621 adult patients. The two most common tumours in the paediatric patients were pleomorphic adenoma and mucoepidermoid carcinoma (89.4%; P = 0.004). The incidence of facial nerve palsy following surgery of the parotid tumours was similar in the two groups (P = 1.000). The most common primary cancer in the paediatric group was mucoepidermoid carcinoma (77.8%), while in the adult group, adenoid cystic carcinoma was most common (P < 0.001). The paediatric group had only low-grade cancers in early stages (P < 0.001), with an overall 5-year survival rate of 100%. These results show that the incidence of malignant salivary gland tumours is higher in paediatric patients than in adult patients. This should be taken into account during diagnosis and therapy.

Spectrum of primary epithelial tumors of major salivary glands: a 5 year record based descriptive study from a tertiary care centre

Background: Salivary gland tumors constitute a highly heterogeneous histopathologic group. There are few epidemiological studies of large series of benign and malignant epithelial tumors of major salivary glands in South India.Methods: This is a retrospective study in a tertiary care centre in Kerala, South India. A total of 180 epithelial neoplasms of major salivary gland were studied over a 5 year period. In each case age, sex, anatomical location and histopathological diagnosis were recorded.Results: Patients with benign and malignant tumors presented with a mean age of 52.3 and 55.9 years respectively. The frequency of benign tumors was 80.6% (n = 145) and of malignant tumors was 19.4% (n = 35). There was a higher prevalence of tumors in males 60.6% (n = 109) than in females 39.4% (n = 71). 91.9% (n = 164) of the tumors were localized in the parotid gland, 8.3% (n = 15) in the submandibular gland and 0.6% (n = 1) in the sublingual gland. The most common benign tumors were Pleomorphic adenoma (48.9%, n = 88) and Warthin’s tumor (28.9%, n = 52). Among malignant tumors, Mucoepidermoid carcinoma was the most common (9.5%, n = 17) followed by Carcinoma ex pleomorphic adenoma (2.8%, n = 5) and adenoid cystic carcinoma (2.2%, n = 4).Conclusions: Parotid gland was the most common site of both benign and malignant tumours. Plemorphic adenoma was the most common benign tumor and mucoepidermoid carcinoma was the most common malignant tumor.

Clinicopathological study of epithelial salivary gland tumours-a retrospective study

Introducton: Tumours of the salivary glands are infrequent, yet they represent an extensive variety of both benign and malignat tumours. Objective: The aim of this study was to evaluate clinicopathological data to determine the distribution of epithelial salivary gland tumours(SGT) in patients who were diagnosed at OMFS unit , TH-Karapitiya, Sri Lanka from 2004 to 2014. Material and methods: Total of 77 SGT were diagnosed in the period of 11 years were reviewed using biopsy reports. Results: Out of 77 epithelial SGTs 35 were benign , 42 were malignat. Indicating a benign to malignat ratio of 1: 1.2. Malignat tumours were more common in minor salivary glands indicating a percentage of 59.52% of total malignat tumours.When considering minor SGT, 73.52% were malignat. Conclusion: There was a higher occurrence of malignat tumours in minor salivary glands in this study.

Salivary gland tumours in a northern Chinese population: a 50-year retrospective study of 7190 cases

The aims of this study were to investigate the epidemiological and clinical characteristics of epithelial salivary gland tumours in a northern Chinese population and to evaluate the current TNM classification system. A demographic and descriptive analysis of 7190 epithelial salivary gland tumours was performed. There were 4654 benign tumours and 2536 malignant tumours. The percentage of tumours located in the parotid, submandibular, sublingual, and minor salivary glands was 62.66%, 9.92%, 2.57%, and 24.85%, respectively; 22.26%, 35.76%, 92.97%, and 61.89% of the tumours, respectively, were malignant. Over 90% in the tongue and maxillary sinus were malignant. Warthin tumour, salivary duct carcinoma, and squamous cell carcinoma were predominant in males, while basal cell adenoma, myoepithelioma, and pleomorphic adenoma were predominant in females. Further, 2.55% of the tumours were in children and adolescents: 44.81% of the tumours were malignant, as opposed to 35.02% in adults. According to the 7th TNM classification, the percentages of T3 and stage III tumours were approximately 10%. Salivary gland tumours show distribution patterns according to histological type, location, and patient age and sex. The limitations of the current TNM classification of salivary gland carcinoma should be considered and revisions made.
