Efficacy of Platelet-Rich Fibrin in Preserving Alveolar Ridge Volume and Reducing Postoperative Pain in Site Preservation of Post-Extracted Sockets.

Background and Objectives: In socket preservation, the goal is to minimize bone resorption after tooth extraction to maintain the volume and contour of the alveolar ridge. The use of PRF in post extraction sites may reduce ridge resorption by encouraging the growth of new bone and acting as a scaffold for tissue. In addition, PRF may enhance healing and minimize postoperative pain. The aim of this study was to evaluate the effectiveness of platelet-rich fibrin (PRF) in maintaining the ridges' dimensions at the extraction site, in the maxilla and mandible, as well as its impact on post-extraction discomfort. Methods: The study was conducted on 60 patients presenting for extraction of posterior teeth and was randomly divided into three groups: group I PRF (n = 20), group II PRF + collagen (n = 20), and group III control (n = 20). Sockets were filled with PRF (group I) and PRF + collagen (group II). At baseline and follow-up after 3 months, CBCT was used to assess the bone dimensions. The postoperative pain evaluations were performed at 24 h, 3 days, and 7 days after the tooth extraction. The pain rate was evaluated using a numerical rating scale from the British Pain Society. Results: The study examined the effects of platelet-rich fibrin (PRF) and PRF combined with collagen on the height and width of the ridges, as well as the pain experienced by the patients following alveolar ridge preservation surgery. ANOVA and t-tests were used to evaluate and compare the ridge dimensions. Comparing the results to the control group, there were no significant differences in the height or width of the ridges. However, both the PRF and PRF + Collaplug® treatments effectively reduced the short-term postoperative pain. Conclusions: The study findings suggest that platelet-rich fibrin (PRF) and PRF combined with collagen do not exert significant effects on ridge width and height compared to the standard treatment following alveolar ridge preservation. However, it is noteworthy that both the PRF and PRF + collagen treatments demonstrated efficacy in reducing postoperative pain in the short term, offering a potential advantage over standard treatment protocols.

Clinical evaluation and assessment of the efficacy of platelet rich fibrin (PRF) on the stability of orthodontic implant

The success of orthodontic treatment depends upon the anchorage. Mini-implants are widely been accepted as anchorage units, due to its size and convenience. However, implant failure is major concern. The placement of implant leads to micro-trauma leading to inflammation, and is found to be major reason of implant failure. Research to improve the stability of mini-implants are on rise. Platelet rich fibrin (PRF) is new generation platelet concentrate, which is found to increase the healing of tissues and hence used in trauma and surgeries. This study was undertaken to find whether PRF can enhance the stability of implant. This single blind split mouth study comprised of 16 subjects above 18 years of age. Group A (consisted of 16 implants which were coated before insertion) and group B (16 implants were normally inserted). The stability of the implant was recorded using resonance frequency analysis at insertion (T0), 24 hours (T1), 2 weeks (T2), at 4 weeks (T3), at 6 weeks and 8 weeks after insertion. Statistically significant findings were found when group A was compared to group B using ANOVA test (p<0.05). The stability of implant of group A (experimental group) at each time interval was greater than group B (control group). The stability of implants was found to be increased when they were coated with PRF than the normally inserted implant.

Efficacy of platelet-rich fibrin in maxillary sinus augmentation and simultaneous implant placement in deficient maxillary ridges: A pilot comparative study

BackgroundRehabilitation with dental implants has become the most widely accepted treatment protocol for rehabilitation of lost natural teeth. Direct sinus lift is one of the most predictable procedures for augmenting the deficient posterior maxillary ridges. MethodsThe study was designed in patients with insufficient bone for implants in the maxillary posterior ridges. The test group received platelet-rich fibrin alone as filler, and the control group received a bone allograft. The residual bone height (RBH), implant mobility, and soft tissue characteristics between and within the groups were compared and analyzed. ResultsBoth the groups had considerable gain in RBH, and the intragroup analysis revealed clinical gain in bone height in both the groups, with the test group exhibiting statistically significant gain which kept on improving from 6 months with z= -3.477 and a p value of 0.001 to 9 months with z=-3.531 and a p value of 0.027. ConclusionThe results suggest that the use of platelet-rich fibrin alone improves the bone regeneration, thereby achieving predictable and stable augmentation of the sinus floor required for the success of implant therapy.

Efficacy of Platelet-Rich Fibrin as Regenerative Periodontal Tissue in Chronic Periodontitis: A Case Report

Background: One of the pertinent challenges the periodontists face today is the regeneration of periodontal tissue because of its complex structure. Periodontal tissue damage is characterized by bone and soft tissue loss. At present, one of the recommended materials for regeneration is PRF or platelet-rich fibrin. It is a regeneration material containing growth factors, prepared from the patient’s own blood, that can be used as a membrane.
 Objective: To see the efficacy of healing periodontal tissue using the addition of PRF.
 Method: A 31-year-old woman presented to the Department of Periodontology of Hasanuddin University Dental and Oral Hospital with the chief complaint of elongated teeth that often bled. Intraoral examination showed pocket depths of 6 mm and 5 mm in teeth 32 and 42, respectively. Radiographs showed radiolucent features in lower anterior teeth with horizontal bone loss. Chronic periodontitis of 32 and 42. Therapeutic intervention included initial therapy by scaling and root planning, followed by flap operation and regenerative therapy with PRF, membrane and bone graft.
 Results: One week post regenerative therapy with PRF, the clinical appearance seemed better and there was no complaint from the patient.
 Conclusion: The addition of PRF to periodontal surgery as a regenerative therapy can accelerate healing in the treatment of chronic periodontitis.

Efficacy of Nanocrystalline Calcium Sulphate Bone Graft (Nanogen®) and Platelet Rich Fibrin in the Treatment of Periodontal Intrabony Defects: A Split Mouth Randomised Clinical Study

Introduction: Periodontal disease leads to the loss of supporting structures of the tooth. Recent years have witnessed the evolution of many regenerating materials that have shown to be effective in regenerating the loss structures. Aim: To evaluate and compare clinically and radiographically the efficacy of Platelet Rich Fibrin (PRF) and Nanocrystalline Calcium Sulphate Bone Graft (NanoGen®) in the treatment of intrabony defects. Materials and Methods: A split mouth randomised clinical study was done in the Department of Periodontology D.A.P.M.R.V. Dental College Bangalore from November 2018 to May 2020. In this study, 30 surgical sites were selected from 15 chronic periodontitis patients American Academy of Periodontology (AAP, 1999) of age between 35-65 years and with interproximal probing depth ≥5 mm following phase I therapy and radiographic evidence of intrabony defects ≥3 mm deep. They were divided into two groups: Group I (n=15) received open flap debridement with Nanocrystalline Calcium Sulphate (nCS) and Group II (n=15) open flap debridement with PRF. Clinical parameters assessed were Gingival Index (GI), Plaque Index (PI), Gingival Recession (GR), Probing Pocket Depth (PPD) and Clinical Attachment Level (CAL). Intragroup was compared using Repeated Measures of ANOVA followed by Bonferroni's Post hoc Test and intergroup was compared using Independent Student Test. Results: Total 15 patients were selected in each group of which 10 were male and 5 were female patients. Mean age of the patients was 39.3 years. There was significant reduction in GI (p-value=0.04,PPD (p-value=0.04) and gain in CAL (p-value=0.04) in group I. The mean difference in CAL was also statistically significant in group II (p-value=0.01).The mean difference PI was not statistically significant between baseline and three months and baseline and six months in both groups. GR increased from baseline to three months and remained same at six months (p-value for group I and group II=0.36) in both groups. On intergroup comparison, group I (nCS) showed better improvement in clinical parameters like PPD (p-value=0.01), CAL (p-value=0.01) and BF (p-value=0.002) at all time intervals compared to group II (PRF). Conclusion: There was improvement in all clinical parameters except GR in both groups. So both can be used as regenerative materials. But based on this study nanocrystalline calcium sulphate bone graft can be preferred over PRF as a regenerative material.

Efficacy of Platelet-rich Fibrin in the Treatment and Rehabilitation of Patients with Peri-implant Mucositis

Aim: To evaluate the effectiveness of injectable platelet-rich fibrin (i-PRF) in the complex treatment and rehabilitation of patients with peri-implant mucositis. Materials and Methods: Clinical examination and treatment of 23 patients with peri-implant mucositis were carried out. Patients of the main group (n=11) were injected with liquid platelets-rich fibrin. In patients of the control group (n=12) the treatment protocol involved local antibacterial and anti-inflammatory therapy. The assessment of peri-implant tissues condition was performed on the basis of visual inspection, generally accepted instrumental and X-ray examination and was based on the data of periodontal tests and indexes. Results: In 72.7% of patients after i-PRF usage, reduction of inflammation was observed on the 3rd day of treatment; on the 7th day the clinical status corresponded to the norm. At the same time, in the control group a decrease of inflammation signs was revealed only in 33.3% of patients on the 3rd day and in 66.7% on the 7th day; complete elimination of inflammation in all patients was registered on the 14th day of observation. In 12-month follow-up examination the positive dynamic of peri-implant mucositis treatment was maintained in 81.8% of patients of the main group and in 50.0% of patients of the control group. Conclusions: The use of i-PRF in the complex treatment of peri-implant mucositis contributes to the rapid elimination of inflammation signs and bleeding of the gums surround the implants, accelerates the soft tissues regeneration, shortens treatment times and provides long lasting stabilization of the process.

Comparative Healing Efficacy of Platelet Rich Fibrin and Zinc Oxide Nanoparticles on Critical-Sized Ulnar Bone Defects in Rabbits

Objective Evaluation of the ability of autogenous Platelet Rich Fibrin (PRF) and Zinc Oxide Nanoparticles (ZnONPs) to repair critical-sized ulnar defects in rabbits based on radiographic assessment. Design Randomized controlled study. Animals Twenty-four healthy male white New Zealand rabbits with an age of 6.0 ± 0.3 months and weight of 2.5 ± 0.29 Kg were used. Procedures A 12 mm defect was created in the diaphysis of the right ulnae in allrabbits,then the rabbits were randomly allocated into three groups (8 each): Control group, (the defect left for healing without grafts), platelets rich fibrin group (PRF group, the defect filled with PRF) and combination group (PRF/ZnONPs group, the defect filled with both PRF and ZnONPs). Healing capacity between the groups was evaluated by immediate postoperative radiographic assessment and subsequently at the first and the second postoperative months. Results Statistical analysis showed significant differences in the radiographic healing score between the groups (P = 0.000) at all-time points (P = 0.000- 0.003) during the study. Rabbits in the combination group showed the highest radiographic healing scores followed by the PRF group meanwhile, the Control group showed minimal radiographic healing scores. Conclusion and clinical relevance The addition of ZnONPs to PRF can accelerate the healing of ulnar critical-size defects in rabbits.

Efficacy of platelet-rich fibrin and tacrolimus on facial nerve regeneration: an animal study

This study was performed to investigate the effect of platelet-rich fibrin membrane (PRFM), alone and with topical tacrolimus application, on regeneration of the crushed facial nerve (FN). Thirty healthy 7-week-old albino rats were used. The left FN was damaged by crushing in all rats. Three random groups of rats were formed: group 1, untreated; group 2, treated with PRFM; group 3, treated with PRFM plus topical tacrolimus. Functional recovery and histological and immunohistochemical evaluations were performed 4 and 8 weeks later. Anti-S100 was used to detect myelin sheath. At 4 weeks, blinking reflex recovery was more rapid in group 3 than in groups 2 and 1 (4.30 ± 0.48, 3.40 ± 0.52, and 2.20 ± 0.42, respectively); the difference was statistically significant (P = 0.001). Histologically, group 3 showed more apparent normal FN structures than the other groups. Immunohistochemical caspase-3 evaluation of the axon area revealed a significant difference between group 2 (PRFM alone; 8.67 ± 0.029) and group 3 (PRFM plus topical tacrolimus; 4.42 ± 0.028) (P = 0.001). Group 3 showed the greatest positive staining in the myelin sheath. Based on the results of this animal study, clinical studies should be performed to determine whether the combination of PRF and tacrolimus also improves the outcome of nerve regeneration in humans.

Evaluation of Platelet Rich Fibrin Sandwiching technique with mixture of alloplastic bone graft and Hyaluronic Acid in repair of Recurrent Oronasal Fistula

Aim: is to evaluate efficacy of platelet rich fibrin (PRF) sandwiching technique associated with hyaluronic acid (HyA) mixed with alloplastic bone graft in closure of recurrent oronasal fistula after previously alveolar cleft grafting. Subjects and Methods: Our study conducted on 24 patients with recurrent unilateral oronasal fistula complaining of oronasal leakage. They were divided into two random equal groups. Group I, alveolar cleft defect was packed with alloplastic bone graft ( osteon II collagen) between two layers of PRF gel. While, in group II, alveolar cleft defect was packed with a mixture of alloplastic bone graft (osteon II collagen) and HyA between the two layers of PRF gel. All patients were observed clinically and radiographically (CBCT) for evaluation after three and six months postoperatively. Results: Our clinical results revealed that, all patients had uneventful wound healing except 4 cases in group I and one case in group II which had wound dehiscence and graft exposure at 1-week postoperatively. The radiographic results by CBCT scans showed a better new bone formation and good osseous union obliterating the oronasal defect.in group II than group I. There was a significant difference between bone density and bone resorption values in both groups at all intervals. Conclusion : Our results demonstrated that PRF sandwiching technique combined with a mixture of alloplastic bone graft and HyA could provide a favorable bony and soft tissue barriers for closure of recurrent oronasal fistula in alveolar cleft patients.

Efficacy of platelet-rich fibrin on pain, edema, and healing in mandibular third molar surgery: A split-mouth randomized study

Context: The postoperative sequelae after third molar surgery include pain, swelling and trismus, distal bone loss along with prolonged sensitivity due to root exposure or increased probing depth. Platelet rich fibrin (PRF) appears to accelerate physiologic healing due to the development of effective neovascularization, accelerated wound closing with fast cicatricial tissue remodelling and reduced incidence of infectious events. The mechanism of action of PRF on pain and edema is not fully understood and it is possible that this effect could be a reflection of better and faster healing of the socket.Aim: The aim is to assess the influence of PRF on pain, edema, and wound healing of the socket after surgical extraction of mandibular third molars.Settings and Design: This randomized controlled study was conducted on 60 patients with bilateral impacted mandibular third molars, reporting to the Department of Oral and Maxillofacial Surgery, Army Dental Centre (Research and Referral).Subjects and Methods: Surgical removal of the impacted mandibular molars of both sides was planned with a gap of 4 weeks between the two sides and appointments were given accordingly. Study side-Surgical extraction followed by placement of PRF in socket and closure and control side-Surgical extraction followed by closure.Statistical Analysis: Preoperative and postoperative measurement of visual analog scale scores for pain, edema, interincisal opening, and gray level histogram values of orthopantomograph was subjected to statistical analysis. The normality of data was analyzed using the Kolmogorov–Smirnov test. The Mann–Whitney U and Wilcoxon Signed-Rank tests were used to check mean differences.Results: We observed a reduction in pain and swelling with improvement in interincisal and greater reduction in pocket depth distal to the second molar on the PRF side. Soft-tissue healing index scores and gray level histogram values for the study side were higher compared to the control side.Conclusions: Further clinical studies to evaluate and assess the applications of PRF in maxillofacial surgery are needed.

Efficacy of Platelet Rich Fibrin in Myringoplasty.

Objective:To determine the efficacy of usage of topical autologous platelet rich fibrin (PRF) in improving outcomes of myringoplasty regarding graft uptake and hearing improvement.Methods:This study was conducted in the ENT department of Holy Family Hospital, Rawalpindi, over a period of six months from August 2019 to January 2020. It was designed as a prospective single blinded randomized controlled trial. The study involved a total of 50 participants in whom myringoplasty was done through post auricular approach using underlay technique. In 25 patients topical drops of PRF were used. Outcomes were compared after three months with the control group (n=25), who underwent myringoplasty without PRF.Results:After three months follow-up, graft uptake was reported 78% and 52% in cases and controls, respectively (P=0.070). Mean hearing improvement was 18 dB and 6 dB in cases and controls, respectively (P=0.014). Postoperative infection occurred in 8% of the cases, and in 32% controls (P=0.037).Conclusion:Topical use of Platelet-Rich Fibrin during myringoplasty results in improved graft uptake. Hence, resulting in much improved hearing, significant reduction in infection rates and decrease in perforation sizes.

Efficacy of Platelet Rich Fibrin on Angiogenesis

In this experiment we used platelet-rich fibrin (L-PRF) with the protocol of its production, in this study we tried to investigate the angiogenetic efficacy of L-PRF on s soft tissue. Blood samples of 8 sheep were used to establish the protocol of the L-PRF formation to develop the work. The mucoperiosteal flaps were filled with autologous L-PRF in the mandible and maxillary jaws of the sheep. Samples taken from the surgical sites stained with haematoxylin and eosin for diagnosis of the sections, then immunohistological tests were confirmed on these sections. We noticed that after 14 days of membranes implantation there was no significant differences between their histological sections. While in histological section of 28 days, the sections with PRF has more angiogenic effect in soft tissue with much and more organized vessels in compare with control one.

Efficacy of Platelet Rich Fibrin versus chitosan as a hemostatic agent following dental extraction in patients on antiplatelet therapy

ObjectivesThe aim of this study is to compare the effectiveness of Platelet Rich Fibrin gel and Chitosan gel dressing in providing hemostasis in patients receiving Oral Antiplatelet Therapy and also to evaluate their surgical healing outcome following dental extractions. MethodologyA total of 60 patients under Oral Antiplatelet Therapy indicated for tooth extraction were included for treatment in the study without altering the oral antiplatelet regimens. Patients were allocated equally in two groups; Group A: where PRF gel was packed into the extraction socket, while Group B: Chitosan hydrogel was packed. Timing of hemostasis was noted for each patient of both the groups. Patients were examined for any pain/secondary bleeding/healing/soft tissue dehiscence/alveolar osteitis in the extraction site on first, third and seventh post-operative days. ResultsAll extraction sockets with Platelet-rich fibrin achieved hemostasis in 2.64 min and sockets with Chitosan hydrogel achieved hemostasis in 1.182 min (p < 0.001). Post-operative pain in Group A sites (3.2, 1.4, 0.37 on 1st, 3rd & 7th day respectively) was significantly lower than the control sites (3.4, 1.67, 0.53 on 1st, 3rd & 7th day respectively) p-value 0.001, 0.001 respectively. ConclusionIn this study, Chitosan hydrogel dressing thus proved to be a superior hemostatic agent compared to PRF gel, that significantly shortens the clotting time following dental extraction in patients under antiplatelet therapy. But, PRF gel has superior wound healing properties than Chitosan with less postoperative pain following minor oral surgical procedures under local anesthesia.
