The clinical effects of continuous nursing intervention combined with chronic disease management center in patients with severe hypertension.

The construction and application of chronic disease management centers are increasing. However, the effect of continuing nursing combined with intervention measures provided by chronic disease management centers in patients with severe hypertension is still unclear. To analyze the application effect of continuous nursing intervention combined with chronic disease management center in patients with severe hypertension. 240 patients with severe hypertension were randomly divided into control group (N = 120) and study group (N = 120). The study group adopted the intervention measures of continuous nursing combined with chronic disease management center, while the control group adopted routine nursing. The nursing effects of the 2 groups were compared. After nursing, the short form 36-item health survey, total satisfaction rate and medication compliance rate of the study group were higher than those of the control group (P < .001). Blood pressure, self-rating anxiety scale, and self-rating depression scale scores were lower than those of the control group (P < .05). Continuing nursing combined with the intervention of chronic disease management center can effectively reduce the blood pressure of patients with severe hypertension, and the overall nursing effect is remarkable.

Analysis of the Effect of Continuous Nursing Care in Patients with COPD

Objective: to study the effect of continuous nursing intervention in patients with chronic obstructive pulmonary disease (COPD). Methods: In this paper, 56 COPD patients were selected and grouped by drawing lots, with 28 cases in each group. The study group underwent continuous nursing intervention, while the control group underwent conventional care, and the parameters of both groups of patients were compared. Results: Compared with the control group, the patients in the study group had a significantly higher forced expiratory volume in one second to forced vital capacity ratio (FEV1/FVC), a significantly lower COPD Assessment Test (CAT) score, and Hamilton Depression (HAM-D) score, and a significantly higher 6-Minute Walk Test (6WMT) score after nursing care. Besides, their self-care ability score and SaO2 were significantly higher, while their PaCO2 and coagulation indexes were significantly lower (P &lt; 0.05). Conclusion:Continuous nursing intervention is beneficial for COPD patients.

The effect evaluation of continuous nursing intervention in patients with type 2 diabetic retinopathy.

To evaluate the application effect of continuous nursing intervention in type 2 diabetic retinopathy (DR). Patients with type 2 DR were selected and divided into intervention group and control group by random. The control group received routine nursing intervention, and the intervention group received continuous nursing intervention on the basis of the control group. The clinical effects of the two groups were compared. After 1 and 2 years of intervention, the intervention group compared to the control group. The rate of visual acuity decrease was significantly lower (p < 0.05). Fasting blood glucose, 2 h postprandial blood glucose, and glycosylated hemoglobin were significantly lower (p < 0.05). The self-management ability and satisfaction were significantly higher, and the readmission rate was significantly lower (p < 0.05). The continuous nursing intervention model has a good clinical effect on the visual acuity of patients with type 2 DR.

Continuous Nursing Intervention Based on Network Interactive in Early Diabetic Kidney Disease Patients.

To analyze the effect of continuous nursing intervention based on network interactive platform on improving blood glucose level and self-efficacy in patients with early diabetic kidney disease. The control group received basic routine nursing, and the study group received continuous nursing based on the network interactive platform. The blood glucose level, renal function, inflammatory factors, self-efficacy, self-management behavior, nursing efficacy and adverse reactions were compared. There was no significant difference in baseline data between the two groups. After the application of continuous nursing based on network interactive platform, the blood glucose level and renal function of patients were significantly decreased, and those of the study group were lower than those of the control group. The inflammatory factors in the study group were significantly lower than those in the control group. In the nursing of patients with early-onset diabetic kidney disease, the application of continuous nursing based on network interactive platform can effectively reduce the level of inflammatory factors, improve the level of blood glucose and renal function, improve the self-efficacy and self-management behavior of patients, and reduce the occurrence of adverse reactions. It is worthy of promotion and application in nursing.

Nursing effect of continuous nursing intervention based on "Internet Plus" on patients with severe adrenal tumor.

Adrenal tumors are a common type of tumor whose incidence increases with age. This study aims to apply the continuous nursing mode of "Internet Plus" to patients with severe adrenal tumors, and preliminarily evaluate the nursing effect of continuous nursing intervention based on "Internet Plus" on patients with severe adrenal tumors. A single-center, retrospective, observational study was carried out on severe adrenal tumor patients. A total of 128 patients admitted to our hospital from June 2020 to August 2021 were selected and divided into 2 groups: the observation group (n = 64) received routine care and the control group (n = 64) received continuing care based on "Internet Plus." The first time to get out of bed, 72 hours postoperative sleep time, 72 hours postoperative visual analog scale score, hospital length of stay, upper limb swelling to subside time, self-rating anxiety scale, Symptom Checklist-90, quality of life scores, and self-rating depression scale of cancer patients were compared between the 2 groups. T test and χ2 test were used for statistical analysis. The first time to get out of bed (t = 10.64, 95% confidence interval [CI] = 5.32-16.53, P < .001), upper limb swelling to subside time (t = 16.50, 95% CI = 7.21-26.15, P < .001) and the length of hospital stay (t = 11.82, 95% CI = 5.61-17.95, P < .001) were significantly shorter, 72 hours postoperative sleep time (t = 9.46, 95% CI = 4.93-15.48, P < .001) was significantly longer, and the visual analog scale score of 72 hours after operation (t = 15.95, 95% CI = 7.32-24.09, P < .001) was significantly lower in the observation group than that in the control group. After receiving nursing intervention, the scores of somatization (t = 17.56, 95% CI = 9.51-27.96, P < .001), anxiety (t = 21.85, 95% CI = 12.35-33.71, P < .001), depression (t = 18.29, 95% CI = 9.63-28.22, P < .001), self-rating anxiety scale (t = 33.67, 95% CI = 19.65-46.13, P < .001), self-rating depression scale (t = 31.92, 95% CI = 20.73-45.88, P < .001), and the quality of life score (t = 21.54, 95% CI = 8.92-40.37, P < .001) were significantly lower, and the positive coping (t = 16.30, 95% CI = 5.15-18.14, P < .001) and negative coping (t = 20.54, 95% CI = 9.34-33.12, P < .001) scores were significantly higher in the observation group than those in the control group. Nursing intervention based on "Internet Plus" continuous mode can promote the recovery of physical function, reduce psychological pressure and negative emotions, and then improve the quality of life of severe adrenal tumor patients.

Effects of Continuous Nursing Intervention on Exercise Tolerance and Rehospitalization Rate in Patients with Chronic Heart Failure

To observe the effect of continuous nursing intervention on exercise tolerance and rehospitalization rate in patients with chronic heart failure. Methods 134 patients with chronic heart failure admitted to our hospital were divided into two groups, routine nursing intervention group (control group) and continuous nursing intervention group (observation group), with 67 cases in each group. The resting and peak heart rate (HR), systolic blood pressure (SBP) and diastolic blood pressure (DBP) of the two groups were recorded. The change of 6min walking distance, modified European Heart Failure Self-Care Behavior Scale (EHFSCB-9) and quality of life (SF-36) of the two groups were compared before and after intervention, and moreover, rehospitalization rate of heart failure of two groups 6 months after discharge was compared between the two groups. Results : Before intervention, there was no significant difference between the two groups (P&gt;0.05). After intervention, there was no significant difference in resting and peak HR, SBP and DBP between the two groups and those before intervention (P&gt;0.05). Further comparison between the two groups showed that there was no significant difference in resting and peak HR, SBP and DBP between the observation group and the control group (P&gt;0.05). After intervention, 6min walking distance and SF-36 scale scores (role physical, physiological function, physical pain, energy, health status, social function, mental health and emotional function) were increased in the two groups (P&lt;0.05). Further comparison between the two groups showed that 6min walking distance and SF-36 scale scores (except somatic pain score and role physical score) in the observation group were higher than those in the control group (P&lt;0.05), and the EHFSCB-9 scores in the two groups decreased gradually after intervention (P&lt;0.05). Further comparison between the two groups showed that the EHFSCB-9 scores in the observation group (except low salt diet score and taking medicine based on doctor’s advice score) were lower than those in the control group (P&lt;0.05). The rehospitalization rate of heart failure within 6 months after discharge was 11.91% in the observation group, significantly lower than 25.37% in the control group, and the difference was significant (P&lt;0.05). Conclusion : Continuous nursing intervention can strengthen the self-care ability of patients with chronic heart failure, improve exercise tolerance and quality of life, and reduce the rehospitalization rate to heart failure within 6 months.

Effect of Continuous Nursing on Nursing Effect, Quality of Life and Satisfaction of Children with Pneumonia

To explore the effect of continuous nursing intervention on children with pneumonia, survival status and nurse-patient relationship, 90 patients with pediatric pneumonia admitted to the Hebei Hengshui People's Hospital from May 2017 to June 20, 2017 were divided into the regular group and the continuous group according to the order of admission. Each group had 45 children. The routine group received general care and the continuity group received continuous care and compared with the previous group to observe the correlation between the nursing effect, clinical symptom improvement time, quality of life, satisfaction and complications of the two groups. The probability of effectiveness in the continuous group was 95.55 %, which is higher than the other group’s 75.55 %. The two groups were meaningful (X2=4.094, p 0.05). After the nursing, the above factors were increased in both groups (p<0.05), and the continuous group improved better (p<0.05). The satisfaction degree of the two groups was 41 (91.11 %), 33 (73.33 %), the difference between the 2 groups was large (p<0.05); the side effects during the 2 groups of care 8.89 %, 26.67 %; the two groups had a change (p<0.05). Continuous care helps children with pneumonia to improve disease, reduce the time of pain, reduce medical disputes, reduce heart muscle and lung failure, and is worthy of application.

The influence of continuous nursing intervention on the trot walk test and quality of life after PCI in young patients with coronary heart disease

Objective To explore the effect of continuous nursing intervention on the quality of life of young patients with coronary heart disease after PCI. Methods From January 2015 to January 2017, 64 cases of young patients with coronary heart disease who received PCI surgery in the hospital were selected. According to the different nursing schemes of the patients, the patients were divided into control group and observation group, with 32 cases in each group. Patients in the control group were given routine nursing care, while patients in the observation group were given continuous nursing intervention. Patients in both groups received 10 000 steps of quickstep exercise. The compliance, exercise endurance and quality of life of the patients in the two groups were compared. Results The compliance of patients in the observation group with 10 000 steps of brisk walking was significantly better than that in the control group, and the difference was statistically significant (P<0.05). After nursing, the VO2peak, VO2At in the observation group was significantly higher than those in the control group, and the difference were statistically significant (P<0.05). The RP, BP, MH, RE, VT and SF scores of patients in the observation group were significantly higher than those in the control group after 3 months of nursing, and the differences were statistically significant (P<0.05). Conclusions Continuous nursing intervention can effectively improve the compliance and exercise endurance of young patients with coronary heart disease after PCI, and improve the quality of life of the patients. Effect of continuous nursing intervention on psychological state and medication compliance of patients with acute myocardial infarction after PCI

Objective: To explore the effect of continuous nursing intervention on psychological status and medication compliance of patients with acute myocardial infarction after PCI operation. Methods: from February 2013 to September 2016, 102 patients with acute myocardial infarction treated by PCI were selected and divided into two groups, 51 cases in each group according to the different nursing methods. The observation group was added continuous nursing on the basis of routine nursing, while the control groupwas the usual nursing mode. The mental state of the two groups before and after treatment was evaluated, and the patients were followed up for 6 months after discharge. The compliance of the two groups at 1, 3 and June after discharge was statistically analyzed. Results: before nursing, there was no signifcant difference in the mental state evaluation between the two groups (P &gt; 0.05). After nursing, the mental status of the two groups was improved, while the psychological state of the observation group was better than that of the control group (P &lt; 0.05). After 6 months' follow-up, there was no signifcant difference in the compliance rate between the two groups at 1 months after discharge (P &gt; 0.05). In 3 and June, the compliance of the patients in the observation group was better than that in the control group (P &lt; 0.05). Conclusion: continuous nursing intervention for patients with acute myocardial infarction after PCI can effectively adjust the unhealthy psychological state of patients, improve medication compliance and promote early rehabilitation of patients.

Clinical effect of continuous nursing intervention for elderly patients with hip fracture after surgery

目的 探讨连续护理干预护理措施对老年髋部骨折术后患者的临床效果。 方法 回顾性分析97例接受手术治疗的髋部骨折患者的临床资料。按照患者是否采用连续护理干预，将患者分为对照组和观察组。对照组47例患者采用的是常规护理干预，观察组50例患者采用连续护理干预。比较两组患者术前、出院时、出院后1个月、出院后3个月、出院后6个月时Harris髋关节功能评分以及FIM功能独立性评分和两组患者护理满意度情况。 结果 两组出院时、出院后1个月、3个月、6个月Harris髋关节功能评分及FIM功能独立性评分均显著好于术前，差异均有统计学意义(均P<0.05)；观察组出院后1个月、3个月、6个月Harris髋关节功能评分及FIM功能独立性评分均显著高于对照组，差异均有统计学意义(均P<0.05)。观察组护理满意度：非常满意30例，较满意12例，基本满意6例，不满意2例，对照组分别为7例、11例、22例、7例，两组差异有统计学意义(Z＝－4.996，P<0.01)；观察组患者护理总满意度为96.00%，显著优于对照组的85.11%，差异有统计学意义(χ2＝0.669，P＝0.041)。 结论 连续护理干预护理措施能有效的改善患者的髋关节功能，促进患肢的康复，并提高患者满意度，值得临床推广。

Observation on the effect of continuous nursing intervention on improving the quality of life of elderly patients with lower extremity arterial disease

Objective To observe the effect of continuous nursing intervention on improving the quality of life of elderly patients with lower extremity arterial disease. Methods Elderly patients with lower extremity arterial disease treated in our hospital from October 2013 to October 2015 were selected as the observation object, and were divided into the intervention group and control group according to the random number table method, 50 cases in each group.The control group was given the routine nursing, while the intervention group was given the continuous nursing intervention.The differences in terms of pain, mental state and quality of life were compared between the two groups before and after the intervention. Results There was no significant difference in terms of VAS scores of the two groups before the intervention, and the VAS scores of the two groups were both lower than that before the intervention, and the VAS scores of the intervention group was decreased more significantly(P<0.05). There was no significant difference in terms of SAS and SDS scores between the two groups before the intervention.The SAS and SDS scores of the two groups were both lower than that before the intervention, and the SAS and SDS scores of the the intervention group was decreased more significantly(P<0.05). There was no significant difference in terms of quality of life of the two groups before the intervention.The scores of quality of life of the two groups were both higher than that before treatment, and the scores of quality of life of the intervention group was significantly higher(P<0.05). Conclusion The continuous nursing intervention can improve the psychological status and improve the quality of life of elderly patients with lower extremity arterial disease. Effect of continuous nursing intervention on health behaviors in patients with stroke

Objective To explore the effect of continuous nursing on the health behaviors of patients with stroke, so as to reduce the recurrence of stroke. Methods A total of 115 stroke patients were randomly divided into experimental group and control group.The patients in experimental group were given continuous nursing by telephone and home follow-up visits.All patients with stroke were investigated with exercise of Health-Promoting Lifestyle ProfileⅡ (HPLPⅡ) at 3 months after discharge. Results Health behaviors in experimental group were significantly better than those incontrol group at 3 months after discharge (P<0.05). Conclusions Continuous nursing could effectively modify the unhealthy way of life in stroke patients and strengthen their health behaviors. Key words: Continuous nursing; Stroke; Health behaviors

Effect of continuous nursing intervention on postoperative gastric cancer patients with chemotherapy adverse reaction and quality of life

Objective To investigate the effect of continuous nursing intervention on postoperative gastric cancer patients with chemotherapy adverse reaction and quality of life.Methods A total of 152 gastric cancer patients treated with postoperative chemotherapy were randomly divided into control group and intervention group.The control group was given conventional nursing, patients in the intervention group were added continuous nursing intervention.The questionnaire survey was used to assess quality of life after 1-3 treatment.The adverse reactions and quality of life were compared between the two groups.Results The incidence of adverse reaction of intervention group was lower than that of control group, the incidence of liver and kidney functions had no significant defference between the two groups.The quality of life of intervention group was better than that of control group after 1-2 courses of treatment （ P〈0.05） and 3 courses of treatment （ P〈0.01） .Conclusions The continuous nursing intervention can effectively reduce incidence of adverse reaction of chemotherapy, im-prove the quality of life of postoperative gastric cancer patients. The effect of continuous nursing intervention guided by chronotherapeutics on ambulatory blood pressure of older hypertensive patients in the community

To explore the effect of continuous nursing intervention guided by chronotherapeutics so as to provide the easy, noninvasive, effective and acceptable intervention for older hypertensive patients in the community. Many researchers studied the effect of administration at different times on blood pressure control and circadian rhythm. However, the individual administrative time was set ambiguously in previous studies. A semi-experimental study. In the study, 90 eligible patients were recruited and separated into three groups randomly, which were the control group, intervention group A (behaviour and chronotherapy intervention) and intervention group B (behaviour intervention). At 6 and 12months after the study, the intervention groups were measured 24-hour ambulatory blood pressure monitoring. There were significant differences in ambulatory blood pressure monitoring parameters of the two intervention groups at different measurement times, and there were interaction between measurement time and different groups. The number of patients with dipper increased and reverse dipper decreased in group A as the intervention applied. There were statistical differences between two groups. The number of patients with morning surge in group A decreased more, and there were statistical differences between two groups at six months after the intervention. The behaviour and chronotherapy intervention based on the patients' ambulatory blood pressure monitoring can control casual blood pressure much better and last longer, which can also improve patients' indexes of ambulatory blood pressure monitoring better than behaviour intervention only. The behaviour and chronotherapy intervention can increase patients' nocturnal blood pressure drop, increase the number of patients with dipper and decrease reverse dipper, and improve blood pressure surge in the morning. Nurses can use continuous nursing intervention guided by chronotherapeutics to help improve hypertension of older patients better in the community.

Effect of continuous nursing intervention on health related behaviors in patients undergoing coronary artery bypass graft

Objective To explore the effects of continuous nursing intervention with regular care on health related behaviors in patients undergoing coronary artery bypass graft (CABG). Methods 80 undergoing CABG patients were recruited to the control group and the study group. All the subjects received regular care.Besides this, patients in the study group received continuous nursing intervention, which include: carrying out education and handing out handbooks before discharging, implementing telephone follow-up after discharging.The health related behaviors, which included 6 parts: nutrition, exercise, stress management, medicine management, complication management and smoking, were measured in the 4th week, 8th week and 12th week after discharging from the hospital. Results Patients in the study group had better health related behaviors than those who received regular care alone, specifically including better nutrition, exercise, medicine management and complication management behaviors. However, the differences of stress management behaviors and smoking between the two groups were non-significant.
