Congenital organic hyperinsulinism associated with a variant in the <i>ABCC8</i> gene. Description of a family case

Hypoglycemia is the most common metabolic disorder occurring in early childhood, which can be the first, and sometimes the only symptom of a whole range of diseases. Etiology determines the characteristic clinical features of the course of hypoglycemic syndrome. Diagnosis is complicated by the fact that the clinical manifestations of hypoglycemia are variable and little specific. The main cause of persistent hypoglycemia in children of the first years of life is congenital hyperinsulinism.
 Congenital hyperinsulinism is a hereditary disease characterized by inadequate hypersecretion of insulin by beta cells of the pancreas. Manifesting as a rule in the neonatal period, congenital hyperinsulinism is a great threat, both in terms of survival of patients with late diagnosis, and in terms of the risks of severe neurological complications with inadequate therapy of emerging hypoglycemia.
 The article describes a family case of congenital organic hyperinsulinism associated with a rare heterozygous mutation in the ABCC8 gene, the early diagnosis of which allowed avoiding severe complications of the disease and timely prescribing adequate treatment. The clinical variability of the course of the disease in related patients was shown, which required a personalized approach to diagnosis and treatment.
 Thus, early detection of hypoglycemia, clarification of its etiology, including with the help of molecular genetic analysis, timely drug therapy and monitoring of the state of carbohydrate metabolism, are important aspects in the treatment and supervision of patients with congenital hyperinsulinism and the prevention of severe neurological complications in them.

Hypoglycemia is a condition characterized by measured blood sugar levels that are lower than normal values in type 2 diabetes mellitus (DM) patients. The impact of hypoglycemia cause a decrease in peripheral tissue perfusion resulting in decreased consciousness. Hypoglycemia detection is useful to early detect of the symptoms of hypoglycemi, hence it could be treated quickly and accurately. The purpose of this study is to determine the related factors (age, knowledge, long-term of diabetes mellitus, and the availability of blood glucose measuring devices) with the ability to detect hypoglycemia in patients with diabetes mellitus tipe 2 at a provincial hospital of Banda Aceh. This study used a descriptive correlative design. The study population was all diabetes mellitus type 2 patients who experienced hypoglycemia who visited to the Endocrine Polyclinic at a provincial hospital in Banda Aceh in 2018 totaling 1,364. The sampling used in this study is a purposive sampling technique for 99 respondents. Data collection using a questionnaire in a Gutman scale totaling 25 statements. The questionnaire reliability tested used Cronbach alpha = 0.955. Data were analyzed using the Chi-square test (α = 0.05). The results showed that no relationship between patient’s age with hypoglycemia detection ability (p=0.322), there was a relationship between knowledge with hypoglycemia detection ability (p=0.003), there was no relationship between long-term of diabetes mellitus with hipoglycemia detection ability (p=0.157), and there was a relationship between the availability of an independent glucose check (glucometer) with hypoglycemia detection ability (p=0.049). Based on the results, patient knowledge, availability of glukometer have a significantly affects on the ability to detect hypoglycemia. It is recommended to nurses in hospitals to increase knowledge of early detection of hypoglycemia in patients, education in using glucose check, and routine blood sugar checks in patients with diabetes mellitus.

Early Detection of Hypoglycemia in Type 1 Diabetes Using Heart Rate Variability Measured by a Wearable Device.

Changes in heart rate variability (HRV) occur at the initiation of hypoglycemia due to sympathetic nervous system activity. We investigated the use of HRV detection by a wearable device as an early alert for hypoglycemia. This proof-of-principle study included 23 patients with type 1 diabetes (14 women, mean age 42 ± 11 years). Patients wore a VitalConnect HealthPatch for 5 days. Hypoglycemia was defined as glucose ≤70 mg/dL (≤3.9 mmol/L) by fingerstick measurement. HRV was analyzed in standardized periods before the hypoglycemic event. Sixty-six hypoglycemic events were recorded. Hypoglycemia caused a typical HRV pattern in 36 (55%) of the hypoglycemic events. Eighteen events (27%) showed an atypical pattern. Ten events were unclassified (15%), and two did not display a change in HRV (3%). Hypoglycemia causes early changes in HRV that can be detected by a wearable device. Measuring real-time HRV seems promising for early hypoglycemia detection.

An Analysis of Factors Related to the Ability of Hypoglycemic and Hyperglycemic Early Detection in patients of Diabetes Mellitus in Bima Public Hospital in 2017

This study aims to determine the ability of early detection, the factors that are most influential and related to the ability of early detection of hypoglycemia and hyperglycemia in diabetes mellitus patients in Bima Public Hospital in 2017. The method used in this research is an analytical descriptive method and uses a cross-sectional approach. The population of this study was all DM patients in Bima Public Hospital in 2017. The sample size was determined by purposive sampling technique, namely 30 people. The study was conducted in the internal medicine room of Bima Hospital. The data were retrieved by using questionnaire of Ability to Detect Hypoglycemia and Hyperglycemia which has been tested for the validity and reliability of the instrument with Cronbach's Alpha value of 0.734. The data analysis process was carried out by using Chi-Square test and logistic regression. Results: The ability of hypoglycemia and hyperglycemia to be detected in DM patients in Bima Public Hospital in 2017 was 47% who were able and 58% who were not able with the average of 42.70, with a minimum-maximum value of 34-52. The results of the multivariate test revealed that the duration of DM disease had the most significant effect on the ability of hypoglycemia and hyperglycemia detection (p-value = 0.001, OR 39.429). Discussion: Improving the quality of nursing care through self-care education in the right nursing perspective with regard to the characteristics of the respondents, one of them is a history of long-term DM disease, can prevent acute hypoglycemia and hyperglycemia.

Study on morbidity pattern of neonates admitted to special new born care unit of M.K.C.G. medical college hospital with special reference to hypoglycemia

Background: Children under five are most fragile group of population and so subjected to high mortality and morbidity. Among them, neonatal group is highly suseptible due to poor adaptation of their premature organ system. Neonatal hypoglycemia is a common co-morbid condition contributing to mortality. The objective of the study was to assess the etiology and clinical profile of neonatal hypoglycemia among the neonates admitted to special newborn care unit and to assess the co-morbidity pattern among them.Methods: Type of study was hospital based cross-sectional study at SNCU of MKCG Medical College Berhampur, Odisha. Sample size was 204 neonates who developed hypoglycaemia (blood glucose level <40 mg/dl). Study period was 2016-2017 statistical tests used-percentage and proportion. Statistical software used- SPSS 16 version.Results: Out of total 2616 admission during study period, the incidence of hypoglycemia was 7.79%.52% were male babies and 58% were out borns. Among the maternal causes, anemia was the most common cause (37.7%). Low birth weight was highest (55.8%) among neonatal factors. Jitteriness and tremor were the major symptoms (32.5% cases). Lethargy and poor feeding were presenting symptoms (19.6% cases). On analysing the outcome, 7.8% neonate died and 78.4% recovered.Conclusions: Being a common co morbid condition and presented with other neonatal problems it is more seen among outborn males and anemic mothers. Cause may be delay in initiation of feeding and interruption of feeding on the way during referral. However proper counseling should be given to caretaker of referral case about feeding on the way. Early detection of hypoglycemia is important to reduce mortality and prevent future sequelae in sick newborns.

Investigating the Efficacy of Using Hand Tremors for Early Detection of Hypoglycemic Events: A Scoping Literature Review

Diabetes is a prevalent condition affecting millions of patients globally. Some diabetic patients suffer from a deadly condition called Hypoglycemia (sudden drop in blood glucose levels). Continuous Glucose Monitors (CGMs) have been the most pervasive tool used to track blood glucose levels but these tools are invasive and costly. While early detection of hypoglycemia has been studied, current approaches do not leverage tremors; which are a primary symptom of hypoglycemia. A scoping review was conducted to understand the relationship between tremors and hypoglycemia, and to document any efforts that utilized tremor signatures non-invasively to detect hypoglycemic events. Findings suggest that hypoglycemic tremors are a medium frequency tremor, more resistant to hypoglycemic impairment than other symptoms, and have not been fully explored yet. This paper also documents the work in progress to utilize a novel wearable device that predicts the onsets of hypoglycemia using hand tremor sensing.

Model Self Management Education (SME) Dalam Meningkatkan Kemampuan Deteksi Dini Hipoglikemia Pada Diabetesi Di RSUD Dr Moewardi Surakarta

Abstract: Self-Management Education, Hypoglycemia. Diabetes Mellitus is a metabolic disease that needs attention and care for a long time in order to prevent complications. People with DM must have good knowledge, skill, and behavior so they can manage their life by themselves. Research showed that 50–80%, people with DM have not enough knowledge to manage their disease. Through the self-management, people with diabetes can improve their competence in early detection of hypoglycemia so it would increase the quality of life and reduce the risk of developing complications. The purpose of this study was to investigate the impact of diabetes self-management education toward early detection of hypoglycemia among patients with diabetes. A quasi-experimental pre and post design with a control group was used to investigate the impact of using education on self-management. The participants were 40 that divided into intervention and control group. Purposive sampling was used to take the samples. T-Test was used to analyze the data. The competence of diabetes in early detection of hypoglycemia was an increase, from 6,1 with SD 13.7 to 71,7 with SD 19.2. Then, the model of Self Management education influenced the competence of people with diabetes in the early detection of hypoglycemia, P value 0,011 (α = 0,05). The findings demonstrate that self-management education could improve the competence of diabetes in the early detection of hypoglycemia.

Fiabilité de la mesure de la glycémie capillaire par rapport à la glycémie plasmatique chez le nouveau-né

The reliability of blood glucose monitoring in neonatology is not always confirmed. The aim of this study was to evaluate the reliability of blood glucose measurements made with three different devices in newborns. The study was prospective, conducted in a medical and neonatal intensive care department over a period of 4 months. Capillary glucose level was measured with three different glucometers and compared with venous glucose level determined using the hexokinase method. An ANOVA and Scheffe test were used for the correlation analysis. Three hundred and nine infants were included, with a mean age of 55h and a mean term of 39weeks of gestation. Mean blood glucose in the laboratory was 0.62±0.15g/L, 0.71±0.17g/L for Accu-Chek(®) Active, 0.80±0.17g/L for Accu-Chek(®) Performa, and 0.83±0.12g/L for Bionime. An ANOVA showed statistically significant differences between the measurements made by glucometers compared to the reference blood glucose levels, and the Scheffé method showed that glucometers overestimated the real plasma glucose levels. None of the devices used in this study was satisfactory. However, an estimation of blood glucose taking into consideration this numerical overestimation would allow early detection of hypoglycemia.
