Integrative training of engineer in globalized world

In this article we consider an integrative training of the engineer in the globalized world. On the basis of the analysis of the international division of labor and interaction in the field of equipment and technologies which the modern engineer faces, we note the necessity of effective interaction of the international teams. We come to light the problem of insufficient level of training of engineers within the modern higher technical education which is differing in own specifics in the different countries, where education is focused on the regional level of training of specialists. It is noted that the international activity based on the international division of labor demands global training of engineers. The analysis of publications concerning global competence, the analysis of determination of cross-cultural competence, and also the analysis of requirements of the international accreditation agencies allows to select four main characteristics of globally competent engineer: to take into account ethnic aspects of society, the nature and safety; to understand the profession and technical aspects in a global context; to be able to work in interdisciplinary and international teams; to be able to carry on dialogue in a cross-cultural and international context. The conducted research has shown that the selected characteristics are integrative qualities of the personality and the engineer who possesses such characteristics will be able to represent the country at the international level adequately and to work at an innovative factory. As a solution of the problem of modern engineers training authors offer formation of global competence of the engineer due to integrative approach in education.

The Academy of Trauma Surgery (AUC). Service provider and management organization of the DGU

At the beginning of this century the German Trauma Society (DGU) became extensively active with an initiative on quality promotion, development of quality assurance and transparency regarding treatment of the severely injured. A white book on "Medical care of the severely injured" was published, focusing on the requirements on structural quality and especially procedural quality. The impact of the white book was immense and a trauma network with approved trauma centers, structured and graded for their individual trauma care performance, was developed. In order to monitor and document the required quality of care, a registry was needed. Furthermore, for cooperation within the trauma networks innovative methods for digital transfer of radiological images and patient documents became necessary. Finally, the auditing criteria for trauma centers had and still have to be completed with advanced medical education and training programs. In order to realize the implementation of such a broad spectrum of economically relevant and increasingly complex activities the Academy of Trauma Surgery (AUC) was established as a subsidiary of the DGU in 2004. The AUC currently has four divisions: 1) networks and health care structures, 2) registries and research management, 3) telemedicine, 4) medical education and training, all of which serve the goal of the initiative. The AUC is a full service provider and management organization in compliance with the statutes of the DGU. According to these statutes the business operations of the AUC also cover projects for numerous groups of patients, projects for the joint society the German Society for Orthopedics and Trauma (DGOU) as well as other medical institutions. This article describes the success stories of the trauma network (TraumaNetzwerk DGU®), the TraumaRegister DGU®, the telecooperation platform TKmed®, the new and fast-growing orthogeriatric center initiative (AltersTraumaZentrum DGU®) and the division of medical education and training, e.g. advanced trauma life support (ATLS®) and other training programs including the innovative interpersonal competence (IC) course.

Heartbeat classification using disease-specific feature selection

Automatic heartbeat classification is an important technique to assist doctors to identify ectopic heartbeats in long-term Holter recording. In this paper, we introduce a novel disease-specific feature selection method which consists of a one-versus-one (OvO) features ranking stage and a feature search stage wrapped in the same OvO-rule support vector machine (SVM) binary classifier. The proposed method differs from traditional approaches in that it focuses on the selection of effective feature subsets for distinguishing a class from others by making OvO comparison. The electrocardiograms (ECG) from the MIT-BIH arrhythmia database (MIT-BIH-AR) are used to evaluate the proposed feature selection method. The ECG features adopted include inter-beat and intra-beat intervals, amplitude morphology, area morphology and morphological distance. Following the recommendation of the Advancement of Medical Instrumentation (AAMI), all the heartbeat samples of MIT-BIH-AR are grouped into four classes, namely, normal or bundle branch block (N), supraventricular ectopic (S), ventricular ectopic (V) and fusion of ventricular and normal (F). The division of training and testing data complies with the inter-patient schema. Experimental results show that the average classification accuracy of the proposed feature selection method is 86.66%, outperforming those methods without feature selection. The sensitivities for the classes N, S, V and F are 88.94%, 79.06%, 85.48% and 93.81% respectively, and the corresponding positive predictive values are 98.98%, 35.98%, 92.75% and 13.74% respectively. In terms of geometric means of sensitivity and positive predictivity, the proposed method also demonstrates better performance than other state-of-the-art feature selection methods.

The Environment Within: Exploring the Role of the Gut Microbiome in Health and Disease

The human genome codes for approximately 23,000 genes,1 yet some experts have suggested that the total information coded by the human genome alone is not enough to carry out all of the body’s biological functions.2 A growing number of studies suggest that part of what determines how the human body functions may be not only our own genes, but also the genes of the trillions of microorganisms that reside on and in our bodies. The genomes of the bacteria and viruses of the human gut alone are thought to encode 3.3 million genes.3 “The genetic richness and complexity of the bugs we carry is much richer than our own,” says Jayne Danska, an immunologist at the Hospital for Sick Children Research Institute in Ontario, Canada. “They serve as a buffer and interpreter of our environment. A role for gut microbes in gastrointestinal function has been well documented since researchers first described differences in the fecal bacteria of people with inflammatory bowel disease.4 The molecular mechanisms responsible for the gut microbiome’s impact on metabolism and diseases throughout the body remain largely unknown. However, researchers are beginning to decipher how the microorganisms of the human intestinal tract influence biological functions beyond the gut and play a role in immunological, metabolic, and neurological diseases.

Mixtures research at NIEHS: An evolving program

The National Institute of Environmental Health Sciences (NIEHS) has a rich history in evaluating the toxicity of mixtures. The types of mixtures assessed by the Division of the National Toxicology Program (DNTP) and the extramural community (through the Division of Extramural Research and Training, DERT) have included a broad range of chemicals and toxicants, with each study having a unique set of questions and design considerations. Some examples of the types of mixtures studied include: groundwater contaminants, pesticides/fertilizers, dioxin-like chemicals (assessing the toxic equivalency approach), drug combinations, air pollution, metals, polycyclic aromatic hydrocarbons, technical mixtures (e.g., pentachlorophenol, flame retardants), and mixed entities (e.g., herbals, asbestos). These endeavors have provided excellent data on the toxicity of specific mixtures and have been informative to the human health risk assessment process in general (e.g., providing data on low dose exposures to environmental chemicals). However, the mixtures research effort at NIEHS, to date, has been driven by test article nominations to the DNTP or by investigator-initiated research through DERT. Recently, the NIEHS has embarked upon an effort to coordinate mixtures research across both intramural and extramural divisions in order to maximize mixtures research results. A path forward for NIEHS mixtures research will be based on feedback from a Request for Information (RFI) designed to gather up-to-date views on the knowledge gaps and roadblocks to evaluating mixtures and performing cumulative risk assessment, and a workshop organized to bring together mixtures experts from risk assessment, exposure science, biology, epidemiology, and statistics. The future of mixtures research at NIEHS will include projects from nominations to DNTP, studies by extramural investigators, and collaborations across government agencies that address high-priority questions in the field of mixtures research.

Collaborative Learning and Technology Skills Development: Evaluation of an Online Annual Grantee Meeting

Annual meetings provide the opportunity to communicate with colleagues with similar professional interests and bolster training initiatives. However, the cost of face-to-face meetings, including travel expenses and lost work time, is high. Although face-to-face meetings offer unique advantages, fiscal considerations compel consideration of alternatives. Distance Learning Grantees (Division of Research, Training and Education, MCH Bureau, Health Resources and Services Administration) and MCH project staff conducted their annual grantee meeting online in 2010. The meeting featured project updates and technical skills training over the course of a three-day meeting. Direct costs of the online meeting were low, and evaluations indicated that all participants were satisfied with the online format.

Integrating Contemporary Models of Motor Control and Health in Chronic Ankle Instability: A Review of the Literature

Numerous structural and functional alterations have been identified as contributing factors associated with chronic ankle instability (CAI). However, little evidence has connected local instability with self-reported disability or functional loss. The dynamical-systems theory of motor control offers a constraints-led approach for interpreting movement dysfunction based on organismic, task-specific, and environmental factors. These factors interact to organize sensorimotor coordination, creating functional variability to complete movement goals. The International Classification of Functioning, Disability, and Health (ICF) is a model of health, examining function at the level of the body or body parts, the whole person, and the whole person in his or her environment. Combining the fundamental components of both theories offers a model that may recognize the link between instability and function. The purpose of this literature review is to discuss the dynamic systems theory, the ICF model, and how integrating both concepts can be used to address, track, and interpret deficits associated with CAI. Ankles sprains are the most frequently reported injury among physically active individuals.1 Approximately 1 of 3 people who sustain a single ankle sprain experience reoccurring sprains and residual bouts of joint instability known as chronic ankle instability (CAI).2 In addition, ankle sprains are one of the major contributing factors in the development of posttraumatic ankle osteoarthritis.3 High reoccurrence rates and the prevalence of long-term functional loss related to ankle sprains begs the question if current rehabilitation strategies are effective.4 Examining CAI through the integration of the most contemporary theory of motor control and the most contemporary model of health may provide a comprehensive assessment of a person’s overall functional ability and elucidate the link between disability and health in this population. Recent advancement in motor control theory may help interpret how sources of functional loss and disability are manifestations of impaired sensorimotor coordination.5,6 This theory, known as dynamic systems, views movement from a context-specific perspective. The dynamic systems theory aims to explain how we use the freedom of the sensorimotor system to develop strategies to cope with changes in health, task complexity, and the environment.5 Similarly, the International Classification of Functioning, Disability, and Health (ICF) examines the effects of injury and illness on the dynamic nature of function based on quality of life, overall health status, and impairment associated with societal and environmental factors.7 The authors are from the Division of Athletic Training, University of Kentucky, College of Health Sciences, Lexington, Ky. Conceptual model of comprehensive research metrics for improved human health and environment

Performance measurement predominantly consisted of near-term outputs measured through bibliometrics, but the recent focus is on accountability for investment based on long-term outcomes. Our objective is to build a logic model and associated metrics through which to measure the contribution of environmental health research programs to improvements in human health, the environment, and the economy. We developed a logic model that defines the components and linkages between extramural environmental health research grant programs and the outputs and outcomes related to health and social welfare, environmental quality and sustainability, economics, and quality of life, focusing on the environmental health research portfolio of the National Institute of Environmental Health Sciences (NIEHS) Division of Extramural Research and Training and delineates pathways for contributions by five types of institutional partners in the research process. The model is being applied to specific NIEHS research applications and the broader research community. We briefly discuss two examples and discuss the strengths and limits of outcome- based evaluation of research programs.

Toward the Assessment of Scientific and Public Health Impacts of the National Institute of Environmental Health Sciences Extramural Asthma Research Program Using Available Data

BackgroundIn the past 15 years, asthma prevalence has increased and is disproportionately distributed among children, minorities, and low-income persons. The National Institute of Environmental Health Sciences (NIEHS) Division of Extramural Research and Training developed a framework to measure the scientific and health impacts of its extramural asthma research to improve the scientific basis for reducing the health effects of asthma.ObjectivesHere we apply the framework to characterize the NIEHS asthma portfolio’s impact in terms of publications, clinical applications of findings, community interventions, and technology developments.MethodsA logic model was tailored to inputs, outputs, and outcomes of the NIEHS asthma portfolio. Data from existing National Institutes of Health (NIH) databases are used, along with publicly available bibliometric data and structured elicitation of expert judgment.ResultsNIEHS is the third largest source of asthma-related research grant funding within the NIH between 1975 and 2005, after the National Heart, Lung, and Blood Institute and the National Institute of Allergy and Infectious Diseases. Much of NIEHS-funded asthma research focuses on basic research, but results are often published in journals focused on clinical investigation, increasing the likelihood that the work is moved into practice along the “bench to bedside” continuum. NIEHS support has led to key breakthroughs in scientific research concerning susceptibility to asthma, environmental conditions that heighten asthma symptoms, and cellular mechanisms that may be involved in treating asthma.ConclusionsIf gaps and limitations in publicly available data receive adequate attention, further linkages can be demonstrated between research activities and public health improvements. This logic model approach to research impact assessment demonstrates that it is possible to conceptualize program components, mine existing databases, and begin to show longer-term impacts of program results. The next challenges will be to modify current data structures, improve the linkages among relevant databases, incorporate as much electronically available data as possible, and determine how to improve the quality and health impact of the science that we support.

On some aspects of validation of predictive QSAR models

Quantitative structure-activity relationships (QSARs) represent predictive models derived from application of statistical tools correlating biological activity (including therapeutic and toxic) of chemicals (drugs/toxicants/environmental pollutants) with descriptors representative of molecular structure and/or property. The success of any QSAR model depends on accuracy of the input data, selection of appropriate descriptors and statistical tools, and most importantly validation of the developed model. Validation is the process by which the reliability and relevance of a procedure are established for a specific purpose. Leave one-out cross-validation generally leads to an overestimation of predictive capacity, and even with external validation, no one can be sure whether the selection of training and test sets was manipulated to maximize the predictive capacity of the model being published. In this paper, we present some representative examples of validation of QSAR models in order to explore possible importance of the method of selection of training set compounds, setting training set size and impact of variable selection for training set models for determining the quality of prediction. The major conclusions from the study are: (1) K-means cluster based division of training and prediction sets can be used as a reliable method of division of data set into training and test sets for developing predictive QSAR models; (2) the training set size should be set at an optimal level so that the model is developed with proper training (learning) process and the developed model is able to satisfactorily predict the activity values of the test set compounds; (3) choice of variables for regression based only on Q2 value may not be optimum. Furthermore, predictive R2 value may not be considered as the only criterion to indicate external predictability of a model.

The purpose of this study was to determine the types of research conducted by PT Neyda RIZKI in implementing employee training programs at every new and old employees to get to know their work areas. The research method that I use is the method of observation / interview, and literature. Based on observations and interviews at the Human Resource Development Division of PT Neyda RIZKI training starts from the planning stages to the training evaluation training. This type of training is used for new employee orientation, training methods on the job training and of the job training for new employees and old employees that provide benefits for companies and employees to achieve results or targets satisfactory work. The training provided is Leadership, Decision Making, Safety Awareness, Excellent Service, Taxation, Computer Organization Development and Filing. Until recently conducted a training program on Human Resource Development division goes well and smoothly. Training is the best way to provide the skills and mastery of work in the field of their work.
