Frequency of abruptio placentae in preterm premature rupture of membranes.

Objective: To determine frequency of abruptio placentae in preterm premature rupture of membranes. Study Design: Analytical Cross-sectional Description. Setting: Department of Gynecology and Obstetrics, Khyber Teaching Hospital, Peshawar. Period: April 23, 2021 to April 23, 2022. Methods: We enrolled 326 patients aged 18 to 35 years, all of whom had singleton pregnancies and presented with preterm premature rupture of membranes (PROM) that occurred less than 48 hours prior to their presentation. These assessments were conducted in both the outpatient and emergency departments to confirm instances of preterm PROM, identified by the pooling or leakage of amniotic fluid in the backside of vaginal vault or from the cervix during examination. Patient follow-up extended from their admission into the Obstetric suite until delivery. Abruptio placentae was diagnosed in cases where a normally positioned placenta separated after 28 weeks of gestation. This diagnosis was supported by ultrasound findings and clinically by symptoms such as painful abdomen during physical examination, whether bleeding per vagina is present or not (between 200 to 400 ml) and the presence of retroplacental clots post-delivery. Results: In our research of 326 patients, 35% (approximately 114 patients) were experiencing their first pregnancy (primigravida), while 65% (approximately 212 patients) had been pregnant before (multigravida). The research found that 6% of the total patients (about 20 patients) experienced placental abruption. This indicates an important correlation of preterm premature rupture of membranes and placental abruption. Regarding the gestational period, 124 patients (38%) were under 34 weeks of gestation, whereas 202 patients (62%) were over 34 weeks. The age distribution was also analyzed: 147 patients (45%) were between 18 to 25 years old, and 179 patients (55%) were between 26 to 35 years old. The average age was 31 years with A measure of variability of ± 9.74. After applying the post-stratification chi-square test, we obtained a P value of ≤0.05, indicating statistical importance. Conclusion: Preterm premature rupture of membranes is associated with a 6% risk of abruptio placentae.

Advances and Challenges in Prenatal Detection and Genetic Diagnosis of Upper Limb Anomalies: Analysis of a South London and Kent Cohort.

Prenatal detection and genetic diagnosis of congenital upper limb anomalies is particularly challenging due to both anatomical and technological factors. Hereby, we present a cross-sectional description of clinical and genetic findings in a 188-patient cohort. In this retrospective study, we present 188 cases with prenatally or postnatally detected upper limb anomalies, either isolated, associated with other anomalies, or syndromic. Patients were examined in four tertiary care centers in South London and Kent from 2012 to 2023. Anomalies were prenatally detected in 158/188 patients (84%), with positional defects (37), polydactyly (34) and transverse defects (25) as the most frequent. 63/188 patients (58%) received a genetic diagnosis of aneuploidy (36), Copy Number Variant (9), or monogenic disorder (18). In 39 out of 103 prenatally tested patients (38%), this diagnosis was given prenatally, contributing to termination of the pregnancy in 23 cases. Through a cross-sectional description of 188 cases with congenital upper limb anomalies, we discuss prenatal ultrasound detection (in terms of numbers and accuracy) and genetic diagnosis.

Object: Correlation between some indices on pyramidal tract magnetic resonance in predicting motor function recovery in patients with supratentorial cerebral infarction. Method: Cross-sectional description with longitudinal follow-up includes 31 patients with supratentorial cerebral infarction within seven days and had an MRI at Bach Mai Hospital from January 2023 to December 2023. Result: The proportion of male patients with ischemic stroke predominated (61,3%). Hemiplegia was the most common symptom of ischemic stroke which damaged the path of the corticospinal tract. At three months after the stroke, no patient had severe NIHSS score ; the percentage of patients with mild and moderate NIHSS score was 61,3% and 38,7%, respectively. According to mRS, 25 patients (80,6%) recovered well after three months. The patient group, who had the axons did not go through the infarct lesion, had higher at rate of better motor function recovery after three months than the other groups who had the axons go through partly or completely inside the infarction lesion (respective rates were 64,5% and 16,1%); p<0,05. FA index of the axons in the infarct side of the poor recovery group was smaller than the good recovery group, p<0.05. Conclusion: The axonal signal, location versus the infarct lesion, and FA index of the axons in the infarct side are the factors that significantly predictive of motor recovery after three months in ischemic stroke patients.

Cross-sectional description of hospital pharmacy services in Puerto Rico in 2022 using the Practice Advancement Initiative 2030 Self-Assessment Tool.

In an effort to expedite the publication of articles, AJHP is posting manuscripts online as soon as possible after acceptance. The American Society of Health-System Pharmacists (ASHP) developed the Practice Advancement Initiative 2030 (PAI 2030) to support the continuous improvement of hospital pharmacy services in the United States. Puerto Rico (PR) hospitals' level of compliance with PAI 2030 recommendations is not currently known. The primary objective of this study was to describe the hospital pharmacy scenario in PR in the 5 areas addressed in PAI 2030 recommendations. Through a collaboration between the state affiliate, a school of pharmacy, and ASHP, completion of the PAI 2030 Self-Assessment Tool was promoted among hospital pharmacy directors between August 2022 and March 2023. A total of 18 out of 66 hospitals completed the survey. The results were compared with national data provided by ASHP from 163 US hospitals. Areas where PR hospitals rated high were in PAI 2030 domain A (Pharmacy Technician Role, Education, and Training) and domain E (Pharmacist Leadership in Medication Use and Safety). PR hospitals rate their performance lower in domain A (Patient-Centered Care) and domain B (Pharmacist Role, Education, and Training). Specific focus areas for improvement by PR hospitals include pharmacist participation in medication reconciliation, 24/7 access to advanced clinical pharmacy services, expansion of the pharmacist's scope of practice, and training through the Board of Pharmacy Specialties and residency programs. This study illustrates how the PAI 2030 Self-Assessment Tool can be used to benchmark pharmacy services at the state level. We suggest that changes are needed to close the gap between hospital pharmacies working towards optimizing the role of pharmacists in healthcare systems and those still struggling with dedicating staff to well-recognized pharmacist roles and responsibilities.

Survey the frequency of some clinical symptoms and syndromes in according to traditional medicine in patients with urticaria

Background: Urticaria is a common disease, caused by many different causes, and can occur at all ages. Traditional medicine refers to it as “Hives” or “Welts” characterized by intense itching and blurry redness in the skin upon onset. Therefore, in oder to contribute to help doctors orient their examination and treatment of this disease more effectively, we conducted study. Objectives: To survey the frequency of occurrence of some clinical symptoms and syndromes according to traditional medicine in patients with urticaria and learn about some factors related to the clinical types of the disease. Matetials and Method: Including 46 patients who were diagnosed with urticaria and examined at the Dermatology Clinic of Hue University of Medicine and Pharmacy Hospital. Research method: Cross-sectional description. Results: The average age was 32.89 ± 15.82, the age group from 20 to 29 years old accounted for the highest proportion. Most lesions had an average size of < 1.27 cm (52.2%), lasted less than 4 hours (76.1%) and 17.4% of patients has an accompanying angioedema. The wheal can appear anywhere on the body, with 41.3% white, 37.0% red and 21.7% pink. About the tongue: pink tongue substance 52.2%, white tongue moss 65.2%, thin moss 78.3% accounting for the highest proportion. 65.2% of patients had difficulty sleeping or poor sleep due to discomfort caused by urticaria. Regarding the pulse: 82.6% edematous pulses, 43.5% rapid pulses and pretty rapid pulses, 84.8% forceful pulses accounts for the majority. The clinical types according to traditional medicine are: wind-cold type (47.8%), wind-heat type (26.1%), dampness - heat in stomach and intestine type (13.0%), deficiency of qi and blood type (13.0%). Conclusions: The assessment revealed that most of lesions are mild reactions to pathogens. The symptoms show that most patients have righteous qi that has not deteriorated and diseases in the shallow. Urticaria affects sleep quality. The two most common types are wind-cold and wind-heat. There were significant relationships between clinical types and age, tongue color, tongue fur color and pulse frequency (p < 0.05). Key words: urticaria, hives, traditional medicine.

OBESITY AND IN-HOSPITAL OUTCOMES AMONG PATIENTS WITH SEVERE COVID-19 IN VIET NAM

Introduction: COVID-19 (Coronavirus disease 2019) is a global pandemic. Overweight and obesity are risk factors related to mortality as well as disease severity. Overweight and obesity are also increasing in Vietnam. Data related to the characteristics of COVID-19 in this vulnerable population was limited in Vietnam. The study was conducted (1) to describe the outcomes of overweight and obese COVID-19 patients, and (2) to investigate factors associated with the outcomes of overweight and obese COVID-19 patients. Methods: A cross-sectional description was conducted at the Hospital for Tropical Diseases from July 2021 to December 2021. Patients who were (1) ≥ 18 years old (2) diagnosed with COVID-19 with a positive RT-PCR SARS-CoV-2 result and (3) had BMI ≥ 23 kg/m2. Female pregnant women or ascites were excluded from this study. To determine factors associated with mortality and ICU admission, chi-square and Fisher's Exact were used. Variables with p < 0.05 were included in the logistic regression model to investigate independent risk factors. Cases directly admitted to the ICU were excluded when analyzing risk factors for ICU admission. Results: 173 patients were recruited (70 patients (40.46%) were overweight, 74 patients (42.77%) were obesity class I and 29 patients (16.76%) were obesity class II). The overall mortality rate was 28.9%. The ICU admission rate was 46.2% with a median ICU stay of 9 (6 – 15.25) days. The rate of respiratory support was 83.8%, of which 31.8% of patients required invasive mechanical ventilation and the median duration of respiratory support was 12 (6.5 – 16.5) days. Independently factors associated with ICU admission were diabetes (OR = 2.92 (1.27 – 6.67)) and a severe or critical condition at admission (OR = 8.82 (2.51 – 31.08). Independently factors associated with death were vaccination (OR = 0.23 (0.08 – 0.63)), a severe or critical at admission (OR = 15.74 (3.46 – 71.55)) and remdesivir usage (OR = 0.78 (0.16 – 0.87)). Recommendation: Vaccination against COVID-19 has been shown to be effective in the obese and overweight population.

Management competency among head nurses at general hospitals in Ben Tre province in 2023

Objectives: Determine the level of management competency among head nurses and related factors at Ben Tre General Hospitals in 2023. Research participants and methods: A cross-sectional description with analysis on 50 Head Nurses using the Chase Nursing Management Competency questionnaire. Results: Head nurses self-assess their management competency on a 4-point Likert scale in 5 areas: professional, technical (3.4.± 0.4), human resource management (3.1 ± 0.4), thinking (3.1 ± 0.5), leadership (3.1 ± 0.4) and financial management (2.9 ± 0.6). Related factors with statistical significance include professional qualifications, management qualifications, time in charge of management, number of beds in the department, and number of nurses in the department. Conclusion: Professional and technical competencies are self-assessed by the head nurses as having the ability to be applied with the most confidence, and financial management competencies are the competencies that can be applied and implemented with the least confidence. Factors: qualifications and time in charge of management are related factors with statistical significance. Therefore, hospitals need to prioritize training, fostering, and updating professional and management knowledge for head nurses to improve the efficiency of patient care and management.

EVALUATION OF TREATMENT RESULTS IN 10 PATIENTS WITH SOFT TISSUE DEFECTS IN THE WRIST-HAND REGION USING RADIAL ARTERY PERFORATOR FLAPS

Purpose: Evaluate the treatment results of soft-tissue defects in the wrist-hand region by radial artery perforator flaps. Subjects and methods: A prospective, non-controlled study and cross-sectional description of 10 patients with soft-tissue defects in the wrist-hand region due to burns, indicated for reconstructive treatment using radial artery perforator flaps at the Center for Plastic and Aesthetic Surgery, Le Huu Trac National Burn Hospital, from January to September, 2021. Results: Patients aged between 17 and 69, mean age of 37.3 ± 14.5 years; ratio of male patients to female patients was 4/1. Regarding the location of injuries: 9 patients had soft-tissue defects in the dorsum of the wrist-hand; 1 patient had a soft-tissue defect in the volar aspect of the wrist-hand. Regarding treatment results: 8 patients showed excellent treatment results, 2 patients had satisfactory results, and there were no cases of treatment failure.

Knowledge of stroke prevention among hypertensive patients at Gia Loc district medical center in Hai Duong province in 2023

Objectives: This study aimed to describe the knowledge of stroke prevention among hypertensive patients who were treated at Gia Loc district medical center, Hai Duong province in 2023. Subjects and research methods: A cross-sectional description was conducted on 178 hypertensive patients who were treated at Gia Loc district medical center, Hai Duong province from April to August 2023, using the convenient sampling method. Results: 71.3% of the participants had knowledge at a satisfactory level, of which 45.5% of patients answered correctly about persons at high risk of stroke. 66.3% of the patients had correct knowledge in terms of all signs of stroke. There were 45.5% of patients provided correct knowledge in terms of persons at high risk of stroke. Regarding risk factors that can cause stroke, 52.2% of patients presented correct knowledge about risk factors that may cause stroke. 77% of patients had correct knowledge about methods avoid the risk of stroke. In terms of knowledge about stroke prevention, 70.9% of hypertensive patients had knowledge at a satisfactory level, 29.1% of hypertensive patients had knowledge at an unsatisfactory level. Conclusion: Patients’ knowledge of stroke prevention is relatively high.

Research of newborn screening of Glucose-6-phosphate dehydrogenase (G6PD) deficiency at Medlatec General Hospital

Objective: Survey some factors affecting Glucose-6-phosphate dehydrogenase (G6PD) enzyme activity from newborn heel blood samples and analyse screening results of some cases of G6PD enzyme deficiency. Research method: Retrospective and prospective cross-sectional description combined with case description. Results: Research on 3223 children who performed the newborn heel blood screening test from September 2022 to September 2023 showed that the high-risk rate of G6PD enzyme deficiency in boys is significantly higher than in girls. The difference is statistically significant when comparing the proportion of neonates with high-risk screening results in the Muong ethnic group and other ethnic minorities compared to the Kinh ethnic group (p<0.01). Between different sampling age groups and different weight groups, the proportion of children with enzyme activity at the high-risk threshold for G6PD enzyme deficiency did not have a statistically significant difference (p>0.05). The mutation encountered in the study was G6PD Union (c.1360C>T) (p.Arg454Cys), the enzyme activity decreased by <10% corresponding to the level of class II enzyme deficiency, showing mild hemolysis and no severe consequences in infancy. Conclusion: Children's gender and ethnicity are related to enzyme activity at risk thresholds for G6PD enzyme deficiency. Studying the correlation between genotype and phenotype is of great significance in genetic counselling, predicting prognosis and managing G6PD enzyme deficiency.

RESULTS OF IRON SUPPLEMENTATION FOR REGULAR VOLUNTARY NON-REMUNERATED BLOOD DONORS AT NATIONAL INSTITUTE OF HEMATOLOGY AND BLOOD TRANSFUSION

Objective: To evaluate the results of iron supplementation for RRVBD at the National Institute of Hematology and Blood Transfusion (NIHBT). Subjects and methods: Including 279 RRVBD with serum ferritin below 26 ng/ml taking iron tablets containing 35 mg of elemental iron. Methods: Cross-sectional description, prospective study. Results: The average age of RRVBD taking iron tablets is 27.8 years old, the average number of blood donations is 11 times, the average time of re-testing is 169.5 days. The proportion of RRVBD taking iron tablets and testing on time is 43.4%, the proportion of RRVBD not taking iron tablets or not taking enough iron tablets is 18.3%, the proportion of RRVBD taking enough iron tablets and not coming to the test on time is 18.6%. The proportion of RRVBD dropping out of the study is 19.7%. Hematological parameters (red blood cell count, hemoglobin (Hb), mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH), mean serum iron concentration) of RRVBD taking iron tablets are within the normal range, the serum ferritin concentration of RRVBD recovered after taking iron tablets is: in the group of blood donors taking enough iron tablets and testing on time, the mean serum ferritin concentration before taking iron tablets is 15.8 ± 5.5 ng/ml and after taking iron tablets is 24.7 ± 14.9 ng/ml, the proportion of blood donors with ferritin returning to normal range after taking iron tablets is 39.7%. In group of blood donors not taking iron tablets or not taking enough iron tablets, serum ferritin before iron intake is 18.1 ± 5.3 ng/ml and after iron intake is 14.2 ± 10.9 ng/ml, the proportion of blood donors having serum ferritin returning to normal range after iron intake is 7.8%. In group of RRVBD taking enough iron tablets and not coming to the test on time, the mean serum ferritin concentration before iron intake is 16.5 ± 5.9 ng/ml and after iron intake is 28.8 ± 17.1 ng/ml, the proportion of blood donors with serum ferritin concentration returning to normal range after blood donation is 51.9%. Conclusion: The proportion of regularly repeated voluntary blood donors taking enough iron tablets and testing on time is 43.4%, the proportion of regularly repeated voluntary blood donors not taking iron tablets or not taking enough iron tablets is 18.3%, the proportion of regularly repeated voluntary blood donors taking enough iron tablets and not coming to the test on time is 18.6%, the proportion of regularly repeated voluntary blood donors not taking iron tablets and not testing is 19.7%. Hematological parameters (red blood cell count, Hb, MCV, MCH and mean serum iron concentration) of regularly repeated voluntary blood donors taking iron tablets before and after iron intake are within the normal range. The proportion of blood donors with serum ferritin concentration after iron intake returning to the normal range: blood donors taking enough iron tablets and testing on time is 39.7%, blood donors not taking iron tablets or not taking enough iron tablets is 7.8% and blood donors taking enough iron tablets and not coming to the test on time is 51.9%.

Grndad and Disease Modifying Therapy (DMT): Shifts in Dmt Are Seen at the Adolescent/Young Adult Transition in Sickle Cell Disease in a Multi-Site Prospective Registry

The ability to characterize the modern person living with SCD in the US has been limited by the lack of a well-curated longitudinal registry. The Globin Research Network for Data and Discovery ( GRNDaD) registry aims to overcome this challenge by collecting data from the now 53 IRB-approved centers across the US in collaboration with the National Alliance of Sickle Cell Centers (NASCC) and the HRSA-funded Sickle Cell Disease Treatment Demonstration Project Grant. Here, we describe the use of disease-modifying therapy in (actively consented) adults and children with Hgb SS/ SB 0 thalassemia (SCA) from 37 sites. Methods: Each site consents subjects and enters extensive baseline data including SCD complications and labs, subjects also complete patient-reported outcomes (PROs)-both at baseline and annually. Each subject is expected to have an annual follow-up where data is extracted from the Electronic Health Record (EHR). All data is stored in REDCap and, for this abstract, data were extracted into R on all active subjects (data entry in the last two years) who have complete minimum data collected. Median values were compared using Mann-Whitney U tests. Disease-modifying therapy (DMT) in this study comprises hydroxyurea (HU), chronic red blood cell (RBC) transfusions, crizanlizumab, L-glutamine, and voxelotor. Results: There are 2501 active patients in GRNDaD. Twenty-six percent of subjects are pediatric </= 18 years of age, 55.5% are female and 66.7% have HgbSS or HgbSB 0 thalassemia (SCA). Here, we report data on people with SCA only, whose minimum data were complete (n=1272). Table 1 shows DMT by age. 187 (21%) adults and 31 (8.5%) children were not on DMT. Figure 2 is a box plot of the absolute neutrophil count (ANC) comparing adults and children who are and are not taking HU. Adults on HU had significantly lower ANCs than those not on HU (median = 4.4 (IQR: 3.07, 6.3) v 6.6 (IQR:4.07, 8.14), p<0.001) (Figure). Similar findings were observed for children ( median = 3.5 (IQR:2.5, 5.7) Vs 8.1 (5.8, 12.8) , p =0.04) Both children and adults on HU had higher MCVs than those not on HU (median for peds: 91.5 (IQR: 85.1, 98.75) v 86 (IQR: 83.3, 86.1), p=0.05, median for adults: 96.5 (IQR: 89, 107) v 90 (IQR: 86.1, 95.1), p<0.001). We compared hemoglobin for those on and off HU, and on and off voxelotor, excluding patients on chronic transfusion therapy. There was a statistically significant difference in hemoglobin when comparing on or off of HU (median 8.9 g/dl (IQR 7.95, 9.9) v 8.2 g/dl (IQR: 7.2, 9.7, p =0.047)). There were no statistically significant differences in hemoglobin when comparing subjects on or off of voxelotor (median = 8.4 g/dl (IQR: 7.5, 9.6) vs 7.9 (IQR: 7.4, 9.8), p =0.57). In the adults, there were statistically significantly more males than females on HU, but there were no differences seen in the pediatric cohort. Examining DMT by age group, chronic RBC transfusion use doubled from the 11-17 age group to the 18-29-year age group (12% to 25%) and HU use decreased (83%-59%) over this same period. Limitations: This cohort may over-represent those on DMT as those enrolled in GRNDaD were more likely to have clinic visits. Discussion: GRNDaD has doubled the number of sites consenting subjects and entering data over the last year. The number is expected to increase again in the next 12 months, as an additional 15 sites are already IRB-approved with a goal to include all SCD centers in NASCC. This cross-sectional description of the current population of people living with SCD and treated in NASCC-recognized SCD centers in the US shows that the majority of those with SCA are receiving DMT. There is a noticeable decrease in HU use during the transition period with an increase in the use of chronic transfusion therapy. Newer DMT has had limited uptake in this cohort. The next steps will be to provide longitudinal data on this population and examine associations of DMT and important clinical outcomes including PROs.

Eye Lesions in Patients After One Year of Kidney Transplantation.

Determine the incidence of some eye lesions in kidney transplant patients after one year at Military Hospital 103 and comment on related factors. A cross-sectional study description of 111 kidney transplant patients (222 eyes) at Military Hospital 103. We assessed several eye lesions, including dry eyes, corneal conjunctival calcification, cataracts, and retinopathy. The rate of retinopathy was 84.7%, dry eye was 59.5%, cataract was 29.7%, and corneal conjunctival calcification was 24.8%, atrophy optic nerve was 9.9%, epiretinal membrane was 1.8%. Post-transplant influence factors associated with cataracts include the dose of prednisolone (OR= 1.6, p < 0.05) and post-transplant diabetes (OR=1.4, p < 0.05). The influence factor related to the atrophy of the optic nerve is systemic infection after transplantation (OR=2.4, p < 0.05). Retinopathy accounted for the highest rate, followed by dry eye disease; cataracts ranked third; and finally, calcified corneal conjunctiva. Factors that affect cataracts are diabetes mellitus and prednisolone dose. Factors affecting optic nerve atrophy are infections after kidney transplantation.

Microbiological and echocardiography characteristics in the prosthetic valve endocarditis patients

Objectives: The goal of the study was description the status of describe microbiological and cardiac injury characteristics through echocardiographic images in prosthetic valve endocarditis patients.&#x0D; Methods &amp; Results: Cross-sectional description of 64 prosthetic valve endocarditis patients at Hanoi Heart Hospital and Cardiology Institute - Bach Mai Hospital from June 2016 to August 2018. Aged 54.01±17.25 (18-84) years old, (60.9% male). Bacterial endocarditis was found in 50% of cases, the most common causative pathogens were Gr(+) bacteria 81.25% (26/32), Gr(-) bacteria 18.75%, multi drug resistant 22.73%. The results of antibiogram showed that Staphylococcus aureus and the group of Gr (-) bacteria had the highest antibiotic resistance. Echocardiographic characteristics of infective endocarditis mitral valve 57.8%, aortic valve 34.4%, in many valve sites very few (3.1%). Biological aortic valve (27.6%) lower than mechanical aortic valve (63.6%). The aortic valve had multiple vegetations (45.65%), size of vegetations was 11.3±4.26 (mm), periprosthetic aortic abscess and dehiscence at the lateral annulus (17.39%). Biological mitral valve (48.7%), mechanical mitral valve (51.3%). Mitral valve had multiple vegetations (60.66%), size of vegetations was 8.99±5.6 (mm), prosthetic valve with dehiscence at the lateral annulus (11.48%). Early infectious endocarditis postop was characterized by heart failure, reoperation and mortality higher than lated infective endocarditis (p&gt;0.05).&#x0D; Conclusions: The half of prosthetic valve endocarditis had bacterial evidence, common causative pathogens were Gram positives. Multi drug resistant due to staphylococcus aureus and Gram negatives. Prosthetic mitral valve endocarditis had the highest rate, followed by prosthetic aortic valve, endocarditis lesions in many valve sites were few. The Characteristics of prosthetic aortic valve werre periprosthetic aortic abscess and dehiscence at the lateral annulus, the mitral valve had multiple vegetations.

Extramedullary haematopoiesis in patients with thalassemia: a cross-sectional description of its prevalence, clinical features and survival

IntroductionDespite knowledge advances on extramedullary haematopoiesis (EMH) in thalassemic patients, the real picture remains an open issue. ObjectivesTo assess EMH prevalence in patients with thalassemia major (TM) and intermedia (TI), to describe magnetic resonance imaging (MRI) findings and to explore clinical risk factors. MethodsIn this cross-sectional study, images and clinical records of 184 consecutive patients with thalassemia who underwent T2* MRI between 2004 and 2011 were reviewed. Association of EMH with survival was investigated for patients with available follow-up charts. ResultsEMH was detected in 16/168 (9.5%) patients with TM (aged 19-49 years) and in 3/16 (18.8%) with TI (aged 36-41 years). Most (88%) had paravertebral thoracic and/or abdominal masses. Age was significantly associated with EMH risk (hazard ratio, [HR] 1.10/year; confidence interval [CI]: 1.03-1.18; p-value < 0.001), while lower pancreatic iron content by T2*MRI (HR: 0.94/ms; CI: 0.89-0.99; p-value = 0.049) was a protective factor. Estimated survival rate was superior for EMH-positive (n = 19) when compared to EMH-negative patients (n = 75) (p-value = 0.013). ConclusionsThe prevalence of EMH was 10.3% (19/184), presented mainly as tumoral masses of 3 to 10 cm. Age was a risk factor for EMH development, while lower pancreatic iron might be a protective factor in this cohort.

The Relationship Between Workload and Nurse Satisfaction During Pandemic in the Pendalungan Jember Area

Workload is an analysis process of the time used by someone in completing a job task, a disproportionate workload will have an impact on stress and can have a negative impact, namely job dissatisfaction. Objectives in this study is to see how workload related to nursing satisfaction during a pandemic in the Pendaluangan Jember area. The research design is a cross-sectional correlation description, Spearman Rho data analysis, the total population of all practicing nurses is 115 nurses with the sampling technique using the Slovin formula with a margin of error of 5% so that 112 nurses are taken as a sample (simple random sampling). The results showed that there was no relationship between workload and nurse satisfaction at the Pendalungan Jember Hospital (p=0.067). The majority's workload assessment was moderate (50%) where out of 112 nurse respondents, the majority of 84 nurses had worked &gt; 5 years (75%), while in terms of education, the majority of Vocational Nursing graduates were 82 nurses (73.2%). Hospitals can increase nurse satisfaction by providing workloads for nurses according to the clinical authority and competence of each nurse so that the nurse's workload becomes lighter and nurse satisfaction can increase.

Objectives: Describe compliance and analyze some factors affecting the adherence to safe intravenous injection procedures of nurses at Ngoc General Hospital, Kon Tum province in 2022. Research Methods: Cross-sectional description combining quantitative and qualitative methods. Results: 61.7% of injections achieved safe injection. The percentage of nurses complying with the safe intravenous procedure was 37.5%. The factors that positively affect the nurse's adherence to safe intravenous injection procedures are experience, patient safety training, and regular supervision of safe injection practices. Negative influencing factors include: Age, intravenous procedure, narrow infrastructure, lack or failure of lighting equipment. Recommendation: The hospital rearranges the number of beds in accordance with the area of each department, ensuring lighting equipment. Review, develop and standardize safe intravenous injection procedures, strengthen compliance monitoring mechanisms and train and update patient safety knowledge for nurses. Anatomical and Biomechanical Characteristics of Peroneus Longus Tendon: Applications in Knee Cruciate Ligament Reconstruction Surgery.

The peroneus longus tendon is used in many orthopedic surgeries to regenerate the external ligaments of the knee. This study aims to evaluate some anatomical, biomechanical, and load-bearing properties of the peroneus longus tendon for use in cruciate ligament reconstruction. The study design is a cross-sectional description. The study subjects were 20 samples of the peroneus longus tendon from fresh carcasses. The leg is still intact, not crushed, is well preserved, and it has never been used in research. The average length of the peroneus longus tendon was 29.25 ± 2.1 cm, and the average distance from the peroneus longus tendon to the deep peroneal nerve was 71.1 ± 8.63 mm. The peroneus longus tendon did not have an accessory ligament, the maximum tension of the peroneus longus tendon was 1170.4 ± 203 N, and the maximum length at break was 14.29 ± 3.88 mm. Removing the peroneus longus tendon will not affect the surrounding anatomical components. The maximum breaking force and the diameter of the peroneus longus tendon are similar to other graft materials, such as the hamstring tendon and patellar tendon.

Studying variable in serum apolipoprotein B levels in damaged coronary artery patients

ABSTRACT Introduction: Atherosclerosis is the most important cause of increasing coronary heart disease. When cholesterol LDL-C levels are low, an increase in Apo B is associated with 60% of the cardiovascular risk factors. This study find out the association between serum Apolipoprotein B levels with the degree of coronary artery lesions Methods: The study consists of 110 patients who were diagnosed with a meaningful damaged coronary artery through angiography at Hoan My Da Nang hospital, from March 2018 to September 2019. Research method: cross-sectional description. Results: The average value of serum Apo B in group 1 was 133.67 ± 17.56 mg/dl, and in group 2 it was 113.92 ± 16.78 mg/dl. The difference is statistically significant, p &lt; 0.01. Cardiovascular risk factors in the study did not have a meaningful effect on serum Apo B levels. Dyslipidemia does not have a strong correlation with serum Apo B levels. Thereby, it is possible to recognize serum Apo B as an independent indicator along with other cardiovascular risk factors. Serum Apo B levels increase linearly according to the number of damaged coronary artery branches. There is a proportional correlation between serum Apo B levels and the degree of coronary artery lesions (r = 0.48, p &lt; 0.01). Conclusion: Apo B is associated with the degree of coronary artery lesions.

Correlation of Computed Tomography, Pathological Findings, and Clinical Outcomes for Appendicoliths in Appendicitis.

To correlate preoperative imaging of fecaliths with what is seen in surgical specimens. Early studies considered radiological findings of appendicoliths as a contraindication for nonoperative treatment of appendicitis. There is no standard definition for what is labeled as an appendicolith radiologically and little is known about the pathological correlates of these lesions. A single center, retrospective study of a consecutive series of adult patients who underwent appendectomy for acute appendicitis from March 2021 to February 2022 was performed. The primary outcome was concordance between preoperative cross-sectional imaging description of appendicolith with postoperative gross pathology description. Images were retrospectively reviewed by an independent radiologist, and the presence and characteristics of appendicoliths and appendices were examined. Of 88 cases of appendicitis, 86 were diagnosed preoperatively by computed tomography (CT) imaging. Appendicoliths were seen either on CT or pathology in 45 (51%) patients. Of these 45 patients, a total of 38 (84%) were identified radiographically, and 28 (62%) were identified on pathology. Of the 38 appendicoliths diagnosed on preoperative imaging, only 21 (55%) were confirmed pathologically. Additionally, of the 28 appendicoliths observed on pathology, only 21 (75%) were identified preoperatively on imaging. There was no appendiceal obstruction in 10 of the 40 cases (25%) in which retrospective radiological review identified appendicoliths. Discrepancies were observed between CT and pathology findings of appendicoliths. Not all appendicoliths seem to cause appendicitis. Because the presence of appendicolith influences the treatment decisions, there is a need to standardize their radiological diagnosis and better understand their pathophysiology.
