The results of the corrective rehabilitation program on the gait of amateur athletes with long-term consequences of brain injury

Aim: To study the results of the quality of life, the state of vestibular disorders and the nature of walking of amateur athletes with the consequences of a combat craniocerebral injury after rehabilitation treatment according to a correctional program. Materials and Methods: The study was conducted on the basis of the Ukrainian Scientific Research Institute of Prosthetics in Kharkov. Under observation were 38 men aged 25-42 years with long-term consequences of a closed craniocerebral injury in the late long-term period. In all patients, complications after TBI were persistent headache, decreased muscle strength in the lower extremities, impaired coordination and balance, and walking patterns. All patients were involved in amateur sports before injury. The following research methods were used during the examination: visual analogue pain scale (VAS), Lovett manual muscle test, Bohannon test, ≪Timed Up and Go test≫. Results: All patients were randomly divided into two groups. Patients Gr.1 (n=20) were trained according to the developed program, which included training according to the PNF method, kinesiotherapy, classes on the C-mill sensory treadmill and the Hunova computer device, segmental reflex massage. Patients Gr. 2 (n=18) underwent a course of physical rehabilitation according to the generally accepted methodology of the Ministry of Health of Ukraine. After working with patients according to the developed correction and rehabilitation program, the following dynamics were observed: the quality of life on the VAS scale in Gr.1 patients had a statistically significant difference (p<0.05) compared to the primary indicator. The dynamics of the Lovett manual muscle test indicated an increase in the muscle strength of the extensor and flexor muscle groups of the lower extremities, the dynamics of the balance indicator in the standing position behind Bohann in all Gr.1 patients and acquired statistical significance (p<0,05). According to the test “Timed Up and Go” patients Gr.1 approached the standard value (p<0,05). In all patients of Gr. 2, the studied characteristics had a positive trend (p>0,05). Conclusions: Individual selection of physical exercises, development of correctional and rehabilitation programs, multidisciplinary approach has a positive impact on changes in the functional state of amateur athletes, quality of life and contributes to the return to an active social life.

Application of suspension systems with rigid elastic elements in the physical rehabilitation of patients with amputation defects of the lower extremities

BACKGROUND: Exercise on suspension systems (gravitational neuromuscular activation) is a highly effective means of physical rehabilitation for patients with various pathologies. Modernization of these products through the use of rigid elastic slings or equipping standard simulators with elastic elements expands the capabilities of this type of equipment, allows you to master new types of exercises, positively affecting the strength, tone and endurance of muscle groups.
 AIMS: to study the impact of the inclusion of suspension systems with rigid elastic elements (slings) in physical rehabilitation programs for patients with amputation stumps at the hip level.
 MATERIAL AND METHODS: Twelve patients were tested before and after repeated prosthetics in the conditions of the FB MSE Clinic, which, due to the use of suspension systems, including elastic elements, not only increased strength, endurance and coordination relationships of muscles, but also changed the structure of their walking. The active part of the course of physical rehabilitation with the inclusion of suspension systems with elastic elements was less than two weeks. Setting the patient the task of increasing the speed of movement on the prosthesis in the long term, in the near future we focused exclusively on the correctness of the step structure and subjective sensations.
 RESULTS: The technical improvement of suspension systems due to elastic elements increases the convenience of their use, the speed of mastering by patients and the degree of efficiency, determining a high percentage of achieving the planned result. Inclusion in the course of training on the simulator with the use of elastic slings allows the patient to master and confidently perform various complexly coordinated movements, including squats, to participate in new types of game activities, which contributes to an increase in his adaptation and socialization in society. Subjectively, patients note greater strength of muscle groups that have undergone additional stress on these systems, improved coordination of movements and endurance when walking on a prosthesis as a whole, as well as a sense of "correctness" of walking, which is confirmed by clinical examination methods and the results of biomechanical tests (increased pace and speed of walking with a decrease in the duration of the step cycle).
 CONCLUSIONS: Suspension systems are most useful for the prevention and elimination of atrophy of the muscles that support the movement of the pelvic girdle, in comparison with conventional dynamic training and walking on a prosthesis. Modifications of suspension systems by including elastic elements in them allow functional training not only using static, but also dynamic exercises, expanding their application possibilities in medical and complex rehabilitation, adaptive physical culture and adaptive sports.

Role of ergoreflex activity in the pathogenesis of heart failure. The effectiveness of physical rehabilitation

Aim. To assess the change in ergoreflex activity in heart failure patients with reduced left ventricular systolic function in the study of the effectiveness of physical training (PT) with original regimen, compared with PT with conventionally estimated regimen.Material and methods. Single-center open-label study of 297 patients with class III HF. The patients were divided into two groups: main group (MG) — patients, in whom the PT intensity was estimated based on lactate threshold (LT); comparison group (CG), in whom the PT intensity was estimated based on 60% VO2peak. We analyzed ergoreflex activity, the relationship of ergoreflex with exercise tolerance (ET), routine systemic inflammation markers, clinical manifestations of HF, and ET based on VO2peak.Results. An increase in gait velocity at the LT level after 3 months of PT was registered in 94% (n=210) of the MG patients. After 3 months, the PT regimen was recalculated according to 60% VO2реак; an increase in gait velocity at this level after 3 months of PT was registered in 70% of CG patients (n=35). The severity of HF decreased in both groups, but the dynamics was more pronounced in the MG, where class II was achieved by 75% of patients (in CG — 44%, p=0,003). In the MG, to a greater extent than in the CG, the ET increased. Ergoreflex activity (according to ΔVE) decreased by ≥15% in 230 (97%) MG patients and in 31 (63%) patients with CG. After the completion of the PT period, when in some patients the HF severity decreased to class II, we revealed a direct association of the ergoreflex value (according to ΔVE) with a greater severity of HF (r=-0,57, p=0,01) and the relationship between the decrease in ergoreflex activity (according to ΔVE), an increase in VO2LT (r=-0,55, p=0,001), VO2peak (r=0,49, p=0,001), a decrease in monocyte count (r=0,63, p=0,01).Conclusion. In stable patients with class III HF receiving the proper disease-modifying therapy, ergoreflex activity is increased and is associated with functional class, ET, and systemic inflammation activity. In the course of physical rehabilitation, a decrease in ergoreflex activity is accompanied by a decrease in functional class, an increase in EF, which is more pronounced with personalized aerobic exercise using LT regimen assessment.

Influence of complex physical rehabilitation on the dynamics of recovery of cognitive functions in patients who have been cerebral ischemic stroke

The article presents the results of a study of the effect of a complex physical rehabilitation on the dynamics of restoration of cognitive impairments in patients in the early recovery period of the cerebral ischemic stroke. The study included 83 patients. All patients under study were distributed to the main group and the comparison group. The average age of the patients was 66,0 ± 2,3 years. Patients of two groups received standard drug therapy aimed at secondary prophylaxis. The patients under study from the main group additionally undertook a course of physical rehabilitation treatment within 21 days. Assessment of the condition of cognitive functions was carried out with the help of psychodiagnostic scales before the onset and after completion of treatment. The study showed that the use of a complex rehabilitation treatment, which includes drug therapy within the framework of secondary prophylaxis and course of physical rehabilitation can significantly affect the improvement of the state of moderately impaired cognitive functions in patients with a hemispheric ischemic stroke.

The dynamics of recovery processes of physical activity of people who recovered from COVID-19

The article shows the results of physical therapy of those who have had coronavirus infection COVID-19 and who have completed a month's course of physical rehabilitation as a part of rehabilitation measures in the conditions of the innovative clinic "Academy of Health". The aim of the research is to study the impact of pulmonary rehabilitation programs for patients with moderate to severe COVID-19. The implementation of special breathing exercises made it possible to improve the oxygen saturation in the blood, to reduce shortness of breath and indicators of respiratory symptoms. The tolerance to exercise has increased; physical activity and the quality of life of patients with pneumonia associated with COVID-19 have improved. The best results of the EQ-5D quality of life assessment were recorded in the experimental group compared to the control group.

Theoretical Foundations of Psychological and Physical Rehabilitation of Patients with Ischemic Stroke

The objectives of our article are: to study the characteristics of disorders of psychomotor function and psycho-emotional state of patients with ischemic stroke according to the initial examination; to develop a method of physical rehabilitation of patients with ischemic stroke at the inpatient stage of rehabilitation, depending on the severity of psychomotor functions and features of the psycho-emotional state; in a psychological and pedagogical experiment to evaluate the effectiveness of the proposed method of physical rehabilitation of patients with ischemic stroke at the inpatient stage of rehabilitation. The following theoretical methods of the research were used to solve the tasks formulated in the article: a categorical method, structural and functional methods, the methods of the analysis, systematization, modeling, generalization. Also in our research we used empirical methods, such as the observation, the interview, the questionnaire, the method of psychological observation, psychological and medical experiment, testing (M. Lucher’s test is used). The results of the research. It was developed a «Functional methodology for the restoration of motor function of patients with ischemic stroke, taking into account their psycho-physiological features», based on the psychological principles of stepwise, sequencing and complexity, as well as on the maximum and adequate psychological impact of the rehabilitation therapist on the patient. Also we proposed the «Functional methodology of physical rehabilitation in the early period of ischemic stroke». The main provisions of this methodology are: 1. The stabilization of the psycho-emotional state of the patient: reducing the level of anxiety, increasing motivation to exercise in the course of physical rehabilitation. 2. The actualization of stable static and dynamic stereotypes of the patient in all initial positions – from horizontal to vertical ones, using the influence of simple, eye-motor, tonic (labyrinthine tonic reflexes, symmetrical cervical tonic reflex, cervical asymmetric tonic reflex, etc.). Restoration and maintenance of stress resistance of patients is provided by symmetrical maintenance of a projection of the general center of gravity on a basic surface in such initial positions in which normal proper afferentation of joints and muscles will be stimulated. These are the positions that the human body consistently takes in the process of verticalization: a supine position on the back; a lying position on the side (right and left); a supine position; a standing position on the knees; a standing position on the knees (with additional vertical support and without such a support); a standing position (with additional vertical support and without such a support). It was proved that the starting position became a directly activated position with a torso extension, which had the aim to support all muscle groups of the patient, provided by the stimulation of a successive chain of muscle contractions directed from the center to the periphery of the body. The symmetry of maintaining the initial position of the patient during the lesson is constantly adjusted (passively or actively) in order to stimulate proper afferentation. Conclusions. The main directions of psychotherapy and psycho-correction of patients who suffered from ischemic stroke are: the assistance in the process of understanding the patients, his / her basic needs, motives, instructions, relationships; his / her internal conflicts and mechanisms of psychological protection; features of his / her behavior and emotional response, their adequacy and realism; the correction of patients’ instructions; the assistance in formulating and securing adequate forms of patients’ behavior based on personal achievements in the cognitive, motivational, and emotional spheres; the promotion of positive motivation for recovery and increased activity in treatment at the behavioral level. During the classes according to the proposed methodology, we have the aim to achieve the stabilization of a psycho-emotional state of the patient through the correction of his / her cognitive processes that motivate the patient to actions which generally affect the effectiveness of all rehabilitation measures.

Оценка эффективности гимнастики тайцзицюань в физической реабилитации коленного сустава у юных гимнасток и учащихся хореографических училищ

Various types of physical activity, including tai chi exercises, have become part of physical rehabilitation of young athletes after knee joint injury. However, many physiological mechanisms of the restorative effect of traditional systems still need to be clarified. We recruited 38 girls aged 10–16 years (mean age 12.5 years, interquartile range [10.8; 13.2] years) involved in different types of physical activity (rhythmic gymnastics and choreography) needing to recover their knee joint function after overuse injury. The subjects were divided into two groups: the main group (physical rehabilitation using a specially developed set of tai chi movements) and the control group (standard course of physical rehabilitation taking place in a physical therapy room). After 2 weeks of rehabilitation, the participants underwent isokinetic testing of the knee joint using the Biodex System 4 Pro (USA). The test results obtained by cluster analysis were typified into three characteristic patterns (χ2 test, p = 0.004): 1) insufficiency of strength parameters (peak torque (PT), peak torque to body mass (PT/BM)) at high speed and insufficiency of functional parameters (PT angle, deceleration time) at high speed; 2) insufficiency of functional parameters (time to PT, deceleration time); 3) insufficiency of strength parameters (PT, PT/BM). It was shown that while the classical means of physical rehabilitation are good for restoring the dynamic muscle parameters (PT), tai chi exercises optimally restore the functional parameters of muscle activation (TP angle and deceleration time). Ефективність комплексної програми фізичної реабілітації хворих на бронхіальну астму в поєднанні з патологією гепатобіліарної системи

Мета. Науково обґрунтувати, розробити і визначити ефективність комплексноїпрограми фізичної реабілітації хворих на бронхіальну астму в поєднанні з патологієюгепатобіліарної системи.Методи. Розрахунок індексу Робінсона (подвійного добутку) для оцінки обмінно-енергетичних процесів в міокарді; розрахунок адаптаційного потенціалу серцево-судинної системи, вегетативного індексу Кердо, експіраторного гіпоксичного індексу, інтенсивності болю за допомогою візуально-аналогової шкали ВАШ та психоемоційного стану за тестом САН. Результати. Обстежено 20 осіб віком від 36 до 60 років (з них 10 жінок і 10 чоловіків) з діагнозом бронхіальна астма і супутніми патологіями гепатобіліарної системи. У реабілітаційному комплексі використано лікувальну гімнастику, яка відрізнялась поєднаннямспеціальних фізичних вправ для бронхіальної астми та патології гепатобіліарної системи,способи вольового керування диханням для профілактики та купірування бронхоспазму,сегментарно-рефлекторний та точковий масаж, фізіотерапію, дієтотерапію. Висновки. Під впливом проведеного курсу фізичної реабілітації у експериментальній групі достовірно покращились показники психологічного тесту САН (з 40,2 ± 1,89 до 49,5 ± 2,01 ум. од.; р < 0,01), за шкалою інтенсивності болю ВАШ (з 60,9 ± 2,12 % до 45,6 ± 2,3 %; p < 0,001), адаптаційного потенціалу серцево-судинної системи (з 3,2 ± 0,16 до 2,8 ± 0,14 балів; p < 0,05), а також індексу Робінсона (з 101,4 ± 3,4 до 93,2 ± 2,46 ум. од.; p < 0,05) та експіраторного гіпоксичного індексу (з 3,42 ± 0,17 до 2,60 ± 014 ум. од.; р < 0,01). Досліджені показники функціональних резервів організму хворих на бронхіальну астму, яка поєднується з патологією гепатобіліарної системи, у основній групі кращі, ніж у контрольній. В основній групібуло виявлено більш суттєвий реабілітаційний єфект, що доказує ефективність та перевагирозробленої реабілітаційної програми і дозволяє рекомендувати її до впровадження.

Systematization of methods of physical rehabilitation of children with inherited nervo-muscle diseases

Summary. Children with hereditary neuromuscular diseases constitute one of the main groups of patients in public and private rehabilitation institutions. Therefore, there is a need to develop a rational choice of physical rehabilitation means, their systematization depending on the level of functional capabilities, which is relevant when developing an individual rehabilitation program in the centres. Purpose. Formation of an integrated approach in the rational choice of methods of physical rehabilitation and orthopedic supply for patients with hereditary neuromuscular diseases. Tasks. Development of stages for the provision of rehabilitation services, determination of the composition of an individual rehabilitation program, development of an algorithm for choosing methods of physical rehabilitation and technical means, depending on the degree of limitation of the child's motor functions. Results. The study involved 46 children aged 2 to 15 years. Assessment of the functional status of children, in accordance with the recommendations of the International Committee for the Standards of Treatment of patients with SMA, was performed on the Hammersmith scale, the assessment of muscle strength - on the Lovett test. The rehabilitation course according to the developed algorithm consisted of 4 stages: the stage of diagnosis, the stage of drawing up an individual rehabilitation program in accordance with the patient's functional capabilities, the stage of the course of physical rehabilitation and the stage of assessing its effectiveness. The main principles of rehabilitation were: stretching of contracted tissues, positioning of segments of the limbs and spine, physical activity in a submaximal mode, verticalization with the use of aids, compensation of lost functions and orthopedic correction using orthoses of various designs. The course carried out helped to stabilize muscle strength and motor activity, increase the volume of passive movements in large joints, and fine motor dexterity. Technical means of rehabilitation made it possible to correct the deformity of the segments of the limbs and the spine, to increase the dexterity of movements when performing tasks for fine motor skills. Conclusions. Adequate rehabilitation measures aimed at maintaining the motor activity of patients, maintaining muscle tone (including the respiratory muscles), timely correction of pathological attitudes of the limbs and spine in children with progressive neuromuscular diseases, inhibit the development of deformities, prolong motor activity.

PHYSICAL PART IN MULTIDISCIPLINARY APPROACH IN REHABILITATION OF CARDIAC PATIENTS


 
 
 The study was ained to analysis of the physical component in the complex rehabilitation of cardiac patients with acute coronary syndrome. Rehabilitation of patients with acute coronary syndrome was performed according to the author's "Program of Clinical and Psychological Rehabilitation of Cardiac Patients by Optimizing the Internal Health Image". The physical component in complex multidisciplinary rehabilitation of cardiac patients with acute coronary syndrome is necessary and provides the optimum possible recovery of the body, promotes adherence to treatment, provides awareness of the role of various disease development factors. In the course of physical rehabilitation, the patient chooses new strategies for recovery and preservation of health.
 
 

Comparative efficiency of continuous intensity trainings and interval loads in patients aftering myocardial infarct with a subsequent corrective coronary intervention

Aim. The purpose of the study is to conduct a comparative analysis of the effectiveness of physical aerobic training in the mode of interval and constant loads in patients who have had a myocardial infarction with subsequent percutaneous coronary intervention. Materials and methods. 35 patients aged from 46 to 67 years old were examined in the process of passing a 3-week course of physical rehabilitation in a day hospital. Of these, 18 people (group 1) performed daily training on cardiovascular machines in the mode of interval loads, 17 people (group 2) - in the mode of moderate loads with constant intensity. Results. When conducting physical training in different training regimens, both groups of patients showed an improvement in exercise tolerance. At the same time, the interval training as compared to the constant intensity training provided a more pronounced training effect on the cardiorespiratory system due to high-intensity loading phases and at the same time a low level of hemodynamic load due to the presence of relatively low-intensity phases. Conclusion. Interval training is preferred in patients who have had a myocardial infarction followed by percutaneous coronary intervention, due to their high efficacy and safety.

Физическая реабилитация после хирургического лечения эпикондилита локтевого сустава на иммобилизационном этапе

The relevance of this topic is due to the fact that elbow joint epicondylitis is relatively common among athletes engaged in tennis, golf and species associated with throwing and throwing. When choosing the treatment method, several factors are taken into account: degree of elbow joint dysfunction; character of change of muscles and tendons in the zone of forearm and hand, as well as possibility of continuation of sports career. As a rule, in case of athletes' lateral and medial epicondylitis the choice is made in favor of surgical treatment, after which the limb is rigidly fixed for 2-3 weeks. The elbow joint is extremely sensitive even to short-term immobilization, which can lead to high mobility. Therefore, in order to avoid the development of elbow joint compression, the early beginning of the course of postoperative recovery measures carried out at the immobilization stage is necessary. We tried to develop the content of the course of physical rehabilitation of 25-30 years old athletes who underwent surgical treatment of epicondylitis for periods of absolute and relative immobilization, and experimentally justify its effectiveness. To assess the effectiveness of the developed physical rehabilitation course, we used tests to determine the functional-motor state of the forearm, operated arm; compared the obtained initial and final data.

Структура и содержание физической реабилитации детей грудного возраста при дисплазии тазобедренного сустава

Hip joint dysplasia among children - congenital hypoplasia of femoral head, or congenital increased mobility of the joint due to the weakness of the ligamentous and muscular apparatus. Such a violation of the hip joint elements development (one or both) leads to an incorrect interposition of the joint structures, whereby the femoral head is displaced relative to the articular surface, formed subluxation, pre-dislocation or joint dislocation. This is a severe and common disease of the musculoskeletal system. Since medical treatment for dysplasia among newborn is not provided, and surgery is required only in extreme cases, therefore, active methods in the fight against this pathology are: specific orthopedic devices, physiotherapy, massage and exercise therapy. Thanks to these components, treatment therapy will strengthen the muscles, accelerate recovery, it will be fast and unobtrusive for the child. In this regard, we have developed the structure and content of physical rehabilitation for infants with hip joint dysplasia. This course was designed for four weeks and was developed a set of rehabilitation measures. The content of the course of physical rehabilitation includes orthopedic correction with the help of special devices, massage, therapeutic gymnastics in combination with fitball gymnastics and therapeutic swimming, physiotherapy. To assess the effectiveness of the physical rehabilitation program developed by us, we conduct an ascertaining pedagogical experiment - testing, clearly demonstrating the functional and motor state of the hip joints of the subjects before the beginning of the forming pedagogical experiment and at its end.

Динаміка показників емпіричного дослідження та біогеометричного профілю у борців – ветеранів спорту з остеохондрозом попереково-крижового відділу хребта

Purpose : to evaluate the effectiveness of the program of physical rehabilitation on the basis of studying the dynamics of indicators of empirical research, vertebro-neurological symptoms and biogeometric profile in wrestlers – veterans of sports with osteochondrosis of the lumbosacral spine . Material & Methods: theoretical analysis and generalization of literature data; use of diagnostic scales (visual analogue pain scale (VAS), scale of five-point evaluation of vertebro-neurological symptoms), determination of mobility of the lumbosacral spine, Schober's test, Tommyer test – fingers-floor test; methods of mathematical statistics . Results: based on the results of the study, a statistically significant dynamics of the parameters of the empirical study, the parameters of the biogeometric profile in the wrestlers – the veterans of the main group sport, was found in the course of physical rehabilitation according to the developed program with the application of therapeutic gymnastics, exercises in post-isometric relaxation. Conclusion: the results of the study confirmed the effectiveness of the comprehensive physical rehabilitation program developed by us, showed that a positive effect on the index of pain, vertebro-neurological symptoms, an increase in the amplitude of the spinal motion in both the sagittal and frontal planes .

Вплив оздоровчо-нормалізуючих режимів ходьби на стан дихальної системи студентів при хронічному бронхіті

Purpose: to determine the influence of the types of walking on the state of the respiratory system of students with chronic bronchitis in the period of convalescence. Material & Methods : 73 students were studied at the age of 17–20 years, MG1 (n=25) and MG2 (n=21) were engaged in the developed program, but MG1 students additionally applied the types of walking and running at various health-normalizing regimes, CG (n=27) did not have bronchopulmonary pathology and were engaged in physical culture according to the program of the university. All students underwent spirometric studies, hypoxic tests. Results : at the end of the course of physical rehabilitation of students of MG1, spirometric indicators were statistically significantly improved and reached the level of CG indices. MG2 students observed improvement in all parameters, but they did not receive any statistical significance. Conclusion : it is proved that the inclusion in the program of physical rehabilitation during the convalescence period of varieties of walking and running on various health-normalizing regimes promotes the complete restoration of the lost functions of the respiratory system.

One of the most effective methods in the system of medical rehabilitation is the use of physical exercises to restore patients after diseases and injuries. Muscle loads are an integral part of physical therapy, various health programs, and, finally, sports. All these types of motor activity, depending on the goal pursued, are based on certain principles of choice, purpose and dosage of physical exercises, taking into account their physiological substantiation of the effect on the body under various nosological forms So, any violation in a different link of the neuroreflex pathway of the motor stimulus leads to a change in the process of realization of the body's response to physical exercises. In this connection, it is important and necessary to have a clear understanding of the mechanism of the action of muscle activity, the variability of the formation of locomotion and their effects, the classification of exercises, the approaches used in the construction and conduct of both individual procedures and the course of physical rehabilitation, a qualitative realization of the tactics of managing patients with private methods of their recovery. Besides, it is always necessary to take into account currently allocated physical exercises, so-called potentially dangerous for the body and, therefore, requiring careful correlation with the nosological form of the disease in which they are used.

Purpose: to estimate efficiency of the carried out program of physical rehabilitation of children with recurrent bronchitis in the conditions of sanatorium. Material & Methods: 109 children of the younger school age are examined. The six-minute test of walking was carried out for the research of functional and adaptation opportunities, and the degree of loading is also estimated on the Borg scale. The obtained data were processed by the adequate methods of mathematical statistics. Results: the improvement of level of the studied indicators at children of the main and control groups is noted after the termination of the course of physical rehabilitation. However, statistically the best dynamics of indicators of length of distance, index of adaptation, index of adaptation of restoration of rhythm and points on the Borg scale are revealed among children of the main groups. Conclusions: the developed and introduced program of physical rehabilitation was more effective in comparison with the standard one.

Measuring movement symmetry using tibial-mounted accelerometers for people recovering from total knee arthroplasty

BackgroundThe purpose of this investigation was to examine movement symmetry changes over the first 26weeks following unilateral total knee arthroplasty in community environments using skin-mounted tibial accelerometers. Comparisons to healthy participants of similar age were also made. MethodsPatients (N=24) with unilateral knee osteoarthritis (mean (SD), 65.2 (9.2) years) scheduled to undergo total knee arthroplasty and a control group (N=19 healthy people; mean (SD), 61.3 (9.2) years) were recruited. The total knee arthroplasty group participated in a standardized course of physical rehabilitation. Tibial acceleration data were recorded during a Stair Climb Test and 6-Minute Walk Test. Tibial acceleration data were reduced to initial peak acceleration for each step. An inter-limb absolute symmetry index of tibial initial peak acceleration values was calculated. FindingsThe total knee arthroplasty group had greater between limb asymmetry for tibial initial peak acceleration and initial peak acceleration absolute symmetry index values five weeks after total knee arthroplasty, during the Stair Climb Test and the 6-Minute Walk Test. InterpretationTibial accelerometry is a potential tool for measuring movement symmetry following unilateral total knee arthroplasty in clinical and community environments. Accelerometer-based symmetry outcomes follow patterns similar to published measures of limb loading recorded in laboratory settings.

Kinesitherapy of patients with bronchial asthma and excessive body weight at the early stage of rehabilitation treatment

Patients with bronchial asthma and excessive body weight were treated with the use graduated physical exercise at the early stage of remedial treatment. The study has demonstrated clinical efficiency of kinesitherapy with the use of ergo-training devices in the course of physical rehabilitation. A course of remedial gymnastics in accordance with a specially developed program promoted exercise tolerance of the patients and eliminated factors limiting functional capacity of the cardiorespiratory system. Simultaneously, it corrected lipid metabolism, improved general well-being and quality of life of the patients.

Cardio–Pulmonary Response Of Patients With Sickle Cell Anaemia DiseaseTo Exercise Test

The purpose of this study was to compare the response of sickle cell anaemia patients with their age-matched counterparts to exercise test. This was to see whether patients with sickle cell disease could be given exercise therapy without any risk of adverse cardio-respiratory response during the course of physical rehabilitation. A total of 70 subjects participated in the study; 30 of these had Haemoglobin type SS (HBSS-or sickling group) and were aged 18.57+3.06 years. Forty had Haemoglobin AA (HBAA or control group) and were aged 19.33 + 3.02 years. The cardio-pulmonary parameters (systolic blood pressure, diastolic blood pressure, pulse rate and respiratory rate) were measured at rest, during exercise and after exercise. The subjects went through step tests at stepping rates of 18 steps per minute and 24 steps per minute. Mean pulse rate at rest for the control group (HbAA) and HbSS groups were 77.40 + 8.90/min and 95.03 + 9.25/min respectively, while pulse rate at 24 steps/min for the control and HbSS groups were 131.83+9.36/min and 158.60+7.61/min respectively. It was concluded that the response of the sickle-cell anaemia patients to exercise features a relatively high cardiac output manifesting as a higher than expected heart rate for a sub-maximal workload. This shows that the sickle-cell anaemia patients could be easily fatigued and should not be subjected to high physical exertion as their healthy peers. Exercise therapy programme in the course of Physiotherapy for sickle cell anaemia patients must be carefully prescribed, closely monitored and cautiously progressed.
