Relation of Operator Volume and Access Site to Short-Term Mortality in Radial Versus Femoral Access for Primary Percutaneous Coronary Intervention

The relation between operator volume and mortality of primary percutaneous coronary intervention (PPCI) procedures for ST-elevation myocardial infarction has not been studied comprehensively. This study included patients who underwent PPCI between 2010 and 2017 in all nonfederal hospitals approved to perform PCI in New York State. We compared risk-adjusted in-hospital/30-day mortality for radial access (RA) and femoral access (FA) and the relation between risk-adjusted mortality and procedure volume for each access site. In 44,540 patients in the study period, the use of RA rose from 8% in 2,010% to 43% in 2017 (p <0.0001). There was no significant change in PPCI risk-adjusted mortality during the period (p=0.27 for trend). RA was associated with lower mortality when imposing operator exclusion criteria used in recent trials. There was a significant operator inverse volume-mortality relation for FA procedures but not for RA procedures. FA procedures performed by lower volume FA operators (lowest quartile) were associated with higher risk-adjusted mortality compared with RA procedures (3.71% vs 3.06%, p=0.01) or compared with FA procedures performed by higher volume FA operators (3.71% vs 3.16%, p=0.01). In conclusion, in patients with ST-elevation myocardial infarction referred for primary PCI in New York State, there was a significant uptake in the use of RA along with relatively constant in-hospital/30-day mortality. There was a significant inverse operator volume-mortality relation for FA procedures accompanied by higher mortality for FA procedures performed by low volume FA operators than for all other primary PCI procedures. In conclusion, this information underscores the need for operators to remain vigilant in maintaining FA skills and monitoring FA outcomes.

P65 THE USE OF CPAP IN A HEMODYNAMICS ROOM ON A PATIENT UNDERGOING CORONARY PROCEDURE – SAFETY AND FEASIBILITY

Abstract Context of the Study It’s not uncommon that patients who suffer from Acute Coronary Syndrome (ACS) also show respiratory failure from acute pulmonary edema. The need to treat these patients preemptively with an invasive approach is made harder by the difficulty of managing them in a hemodynamics room in a supine decubitus position. Therefore, the risk is that the procedure is delayed, or that it becomes necessary to proceed with an orotracheal intubation. Not much data is available on the use of non–invasive assisted ventilation with cPAP during revascularization in a hemodynamics room. In general, there is quite some distrust over both the hemodynamic effects of positive pressure and the managerial difficulties for the nursing staff. Goal of the Study To evaluate the safety and feasibility of the use of cPAP in patients with ACS and severe acute respiratory failure, radiological framework of acute pulmonary edema and severe hypoxemia, decubits semiortopnoic cumpolsory. Materials and methods Patients who suffer from ACS and acute pulmonary edema, undergoing urgent coronary catheterization with the use of cPAP in a hemodynamics room, with a “total face” mask. Description of the management by the nursing staff. Results 27 patients were analyzed. For all patients, cPAP was positioned before the procedure and maintained at all times during the coronary interventional procedure. Never was necessary the intervention of a rianimator, nor was orotracheal intubation. Conclusions The use of cPAP, in a hemodynamics room, by the nursing staff, has been proven to be feasible and safe, and it might reduce the need for orotracheal intubation in patients suffering from ACS and acute pulmonary edema. Proposal for a multicentre study.

Influence of a long-term proctoring process ontheeffectiveness of chronic total occlusion percutaneous coronary interventions.

Despite the complexity of the chronic total occlusion (CTO) percutaneous coronary intervention (PCI) procedures and unsatisfactory results in centers with low volume experience, the practice of training and certifying operators is not a routine. The study aimed to identify factors influencing the effectiveness and complications of PCI CTOs during a proctoring program. The study group consisted of 194 consecutive patients (226 PCI CTOs) as part of the proc-toring program. The relationships between clinical and treatment parameters and the experience gained along with the duration of the proctoring program on the effectiveness and safety of the procedure were assessed. The multivariable analysis showed an independent effect of CTO morphology (odds ratio [OR], 0.38; 95% confidence interval [CI], 0.21-0.71; P <0.01) and an independent effect of increasing operator's experience (OR, 2.62; 95% CI, 1.24-5.60; P = 0.01) on the effectiveness of the procedure. The increase in the efficiency of the PCI CTO, related to the treatment experience gained during the program, was observed especially in the first 50 procedures, treatment effectiveness increased from 55% to 72% (P <0.05). The success of procedures was higher in months when ≥3 procedures were performed (75% vs. 52%; P <0.001). Periprocedural complications occurred in 11 patients (4.9%). In the multivariable analysis, no independent factors influencing the risk of complications were identified. The effectiveness of PCI CTO depended on lesion complexity and broadening oper-ator's experience. No independent factors affecting the risk of complications were identified. The number of >50 procedures under the proctor's supervision should be considered in designing teaching programs.

Advancement in Coronary Angiography or Percutaneous Coronary Intervention Using the Distal Transradial Artery Access in Acute Coronary Syndrome and Complex Coronary Artery Disease.

As the incidence of coronary heart disease increases annually, coronary angiography and percutaneous coronary intervention procedures are also increasing. The femoral artery and radial artery paths are commonly used for percutaneous coronary intervention, but their clinical application is limited to a certain extent due to many postoperative complications. The distal transradial access path is a new surgical path for coronary angiography and percutaneous coronary intervention. In this study, we reviewed the most relevant and recent articles related to distal transradial access and found that coronary angiography or interventional therapy using the distal transradial access path is safe and effective in patients with acute coronary syndrome and complex coronary artery disease. The distal transradial access path is expected to be the first choice for coronary angiography or percutaneous coronary intervention in patients with acute coronary syndrome and complex coronary artery disease.

Background The transradial access has gained great prominence in interventional coronary procedures due to lower complication rates, especially when the radial artery is punctured distally, in the anatomical snuffbox. The objective of this study was to analyze the complication and crossover rates of the access routes in invasive coronary procedures, comparing the distal radial artery to the proximal transradial route in the styloid process and the transfemoral access. Methods This was a prospective, observational, and single-center cohort study. The results of access routes were compared, using the primary outcomes of puncture site-related complications and initial arterial access crossover. Results A total of 748 patients were included; in that, 152 (20.3%) in the Distal Transradial Access Group, 388 (51.9%) in the Proximal Transradial Access Group, and 208 (27.8%) in the Transfemoral Access Group. No complications were observed in the Distal Transradial Access Group, whereas two patients (0.5%) had mild local hematomas in the Proximal Transradial Access Group, and six participants (2.9%) had complications in the Transfemoral Access Group, with mild local hematomas in four patients (1.9%), a pseudoaneurysm in one (0.5%), and an active bleeding in one (0.5%) – all with no need for surgical intervention (p=0.01). The crossover rate was 9.2% in the Distal Transradial Access Group, 5.9% in the Proximal Transradial Access Group, and 0.9% in the Transfemoral Access Group (p=0.001). Conclusion The distal radial artery access had a lower rate of vascular/hemorrhagic complications when compared to the proximal transradial access in the styloid process and the transfemoral access. However, the crossover rate was higher when the distal radial artery access was the operator’s first choice.

Occupational eye dose correlation with neck dose and patient-related quantities in interventional cardiology procedures.

Occupational eye dose monitoring during interventional radiology and interventional cardiology is important to avoid radiation-induced cataracts. The aim of this study was to assess the eye dose correlation with neck dose and patient-related quantities for interventional cardiology physicians and nurses. The originality of this study lies in obtaining correlations between the location of the dosimeter and eye dose radiation readings among different procedures and practitioners. The doses were measured for each procedure (18 procedures of coronary angiography and 16 procedures of percutaneous coronary intervention) using an active personal dosimeter. The eye dose for physicians was not correlated with the neck dose. The eye dose for nurses had a good correlation with the neck dose during both coronary angiography (R2 = 0.91) and percutaneous coronary intervention (R2 = 0.93). Kerma-area product values may be used for a rough estimation of the eye dose for physicians during routine coronary angiography procedures (R2 = 0.76). For nurses, the neck dose is a good proxy for the eye dose during coronary angiography and percutaneous coronary intervention procedures.

Impact of Different Measures of Body Size on the Radiation Dose During Coronary Angiography and Percutaneous Coronary Intervention: Results from a Large Single Center Cohort.

Efforts to reduce and optimize the radiation exposure during coronary angiography and intervention have pointed at patients' body size as a major determinant of irradiation for the patients and operators. We aimed at comparing body weight and body mass index (BMI) among consecutive patients undergoing angiographic procedures (coronary angiography and/or interventions) in a single center. Patients were divided in normal weight (NW, BMI <25Kg/m2) and overweight (OW, BMI ≥25Kg/m2). Patients' dose exposure was evaluated as dose area product (DAP), time of exposure (fluoroscopy duration), and relative DAP (DAP/minutes of exposure). We included 748 patients, 61.6% undergoing percutaneous coronary interventions and 56.8% classified as OW. OW patients were more often men (P < .001), with history of hypertension (P < .001) and diabetes (P = .001). Mean DAP and relative DAP were significantly higher among OW compared with NW patients (P < .001). DAP and relative DAP were directly related with body weight (both r = .22, P < .001); a similar linear association was also described for BMI (r = .18, P < .001 and r = .19, P < .001, respectively). At multivariate analysis, however, body weight, but not BMI, independently predicted the DAP. Therefore, body weight should be considered as the preferred indicator of body size in the setting and optimization of radiation exposure during coronary diagnostic and intervention procedures.

Acceptance and safety of femoral versus radial access for percutaneous coronary intervention (PCI): results from a large monitor-controlled German registry (QuIK)

BackgroundIn 2015 and 2018, European Society of Cardiology guidelines for percutaneous coronary intervention (PCI) favoring radial access over femoral access were published. These recommendations were based on randomized trials suggesting that patients treated radially experienced reduced bleeding complications and all-cause mortality. We aimed to assess acceptance and results of radial access in a real-world scenario by analyzing all PCI cases in the Quality Assurance in Invasive Cardiology (QuIK) registry.MethodsThe QuIK registry prospectively collects data on all diagnostic and interventional coronary procedures from 148 private practice cardiology centers in Germany. Major adverse cardiac and cerebrovascular events (MACE) were defined as myocardial infarction, stroke, or death during hospitalization.ResultsFrom 2012 to 2018, 189,917 patients underwent PCI via either access method. The rate of radial approach steadily increased from 13 to 49%. The groups did not differ significantly with respect to age or extent of coronary disease. Femoral approach was significantly more common in patients with ST elevation myocardial infarction and cardiogenic shock. Overall, there were significant differences in MACE (radial 0.12%; femoral 0.24%; p < 0.0009) and access site complications (radial 0.2%; femoral 0.8% (p < 0.0009).ConclusionOur data reveals an increase in use of radial access in recent years in Germany. The radial approach emerged as favorable regarding MACE in non-myocardial infarction patients, as well as favorable regarding access site complication regardless of indication for percutaneous intervention.

Impact of Coronary Stent Price Regulation Section on the Interventional Cardiology Landscape of India: An Assessment of Health Economics Outcomes

Introduction: The cases of Coronary Artery Diseases (CAD) are on the rise in India, this can be attributed to various factors such as ageing, lifestyle, smoking, food habits, and higher income levels. Although, a large proportion of India’s population has access to some form of health insurance, the out of pocket expenditure is still extremely high as a percentage of total health spendings. Aim: To assess the impact of coronary stent price regulation on the interventional cardiology field in India. Materials and Methods: Retrospective data for this study was collected from online search at google and various other external data sources such as websites and published reports of World Health Organisation (WHO), Niti Aayog, National Pharmaceuticals Pricing Authority (NPPA) and Economic Times (ET) Health World. Line graphs for the percentage change in the number of percutaneous coronary intervention procedures, usage of Drug Eluting Stents (DES) pricing for government and private hospitals for the period 2016-2021 were plotted using Microsoft excel. Results: A high cost of coronary stents ranging from Indian Rupee (INR) 40,000 to INR 1,25,000 was a major contributor to a very high out of pocket expenditure in the field of interventional cardiology. In order to curtail this cost, a ceiling limit of INR 29,600 was imposed with respect to the prices of coronary stents in the year 2017 by the NPPA. Ever since a ceiling limit was imposed on the prices of coronary stents in India, there has been an annual increase of 3.7%, 13.1%, 12.6%, and 12.9% in the number of coronary intervention procedures for the years 2017, 2018, 2019 and 2021, respectively. Similarly, the usage of DES has also shown an annual increase of 8.9%, 14.7%, 10.5%, and 13.3%, respectively for the same years. Conclusion: At this juncture when the burden of CAD is on a rise in India, the efforts to make coronary stents more affordable will go a long way in the efficient management of CAD in the country.

A Study of Estimated Glomerular Filtration Rate in Patients Undergoing Diagnostic or Interventional Coronary Contrast Procedures.

Introduction:Angiographic procedures are underused in patients with chronic kidney disease (CKD), who present with acute coronary syndromes, due to risk of contrast-induced acute kidney injury (CI-AKI). In this study, we assessed the change in estimated glomerular filtration rate (eGFR) over 3 months following coronary procedures in CKD patients.Methods:This observational study was done from July 2017 to January 2019 in patients undergoing elective coronary procedures with an eGFR <60 mL/min/1.73 m2. CKD-EPI equation was used to calculate eGFR pre and post coronary procedure at 24, 48, and 72 hours as well as 30, 90 days. AKI was diagnosed and patients were given prophylaxis for CI-AKI as per KDIGO recommendation (intravenous normal saline and oral N-acetyl cysteine).Results:Patients studied were 282 (225 males, 57 females) of which 68.1% were diabetics. Mean eGFR was 42.91 ± 10.51 mL/min/1.73 m2 and mean hemoglobin was 12.08 ± 1.51 gm/dL. Coronary angiogram (CAG) was done in 174; percutaneous transluminal coronary angioplasty (PTCA) was done in 108. Mean contrast volume in CAG was 55.17 ± 34.45 mL and in PTCA was 156.94±±47.99 mL. CI-AKI was seen in 66 (23.4%) patients. The incidence of CI-AKI increased with severity of underlying CKD. The variability of eGFR at 1 and 3 months after coronary procedures showed no significant change from baseline, even in the patients who developed CI-AKI.Conclusions:CI-AKI is self-limiting and has no major detrimental effects on eGFR at 1 and 3 months after contrast exposure.

Body pain - An unheeded personal health hazard in interventional cardiologists: A national online cross-sectional survey study in China

IntroductionInterventional cardiology procedures (ICPs) have become the mainstay treatments in cardiology diseases and increased rapidly. This study aims to assess the occupational health hazards (OHHs) related to the long-time wearing of lead personal protective equipment and reveal health protection needs in interventional cardiologists. Methods and resultsWe invited interventional and non-interventional cardiologists in tertiary hospitals in China to participate in an online cross-sectional survey on their health status, utilization of personal protective equipment (PPE), and personal health protection (PHP) needs. Propensity score methods were used for comparisons of OHHs between the matched interventional and non-interventional cardiologists. Totally, 642 interventional and 402 non-interventional cardiologists completed the survey. The interventional cardiologists had significantly higher incidence of body pain (56.6% vs. 24.2%, p < 0.001), bone and joint disease (21.7% vs. 8.6%, p = 0.001), cataract (3.5% vs. 0%, p = 0.039), and anxiety (8.1% vs. 2.5%, p = 0.029) than the matched non-interventional cardiologists. The risk of back pain was independently associated with female gender, performing percutaneous coronary intervention procedure or ≥ 2 types of ICP, and the personal annual volume of ICPs. Only 3.3% of interventional cardiologists were satisfied with PPE and 83.0% of them complained of physical toll caused by heavy PPE. 90.7% were willing to conduct ICP without radiation exposure. ConclusionsBody pain was the main OHH in interventional cardiologists likely due to wearing heavy lead PPE for long working hours. Besides training more interventional cardiologists, the adoption of emerging technologies without heavy lead PPE will be a promising way to reduce the OHH burden.

Return to work: does cardiac rehabilitation make a difference? Danish nationwide register-based study.

to assess whether participation in cardiac rehabilitation affects the probability of returning to work after ischaemic heart disease. the study population consisted of 24,509 patients (18-70 years of age) discharged from an inpatient admission at a Danish hospital during 2014-2018 and who were working before their admission. Only patients with a percutaneous coronary intervention or coronary artery bypass grafting surgery procedure and ICD-10 codes I20-I25 as their main diagnosis or ICD-10 codes I21, I240, I248 or I249 as secondary diagnosis during an emergency admission were included. Exposure was defined as participation in cardiac rehabilitation (N = 15,742), and binary indicator of being at work in the last week of a given month were used as primary outcomes. Coarsened exact matching (CEM) of exposed and unexposed patients was used to reduce selection bias. Logistic regression models were applied on the matched population (N = 15,762). Less deprived and less comorbid patients were more likely to receive cardiac rehabilitation. CEM succeeded in arriving at a population where this selection was reduced and in this population we found that patients who received cardiac rehabilitation had a lower probability of returning to work after 3 months (OR 0.81, 95%CI: 0.77-0.84), a higher but insignificant probability after 6 (OR 1.02, 95%CI: 0.97-1.08), and a higher probability after 9 (OR 1.08, 95%CI: 1.02-1.15) and 12 months (OR 1.20, 95%CI: 1.13-1.28). Deprived and comorbid patients have lower use of cardiac rehabilitation. In a matched population where this bias is reduced, cardiac rehabilitation will increase the probability of returning to work.

The use of digital magnification to reduce radiation dose in the cardiac catheter laboratory.

To audit whether using magnification of images by use of a large viewing screen using digital matrix magnification which enlarges the image by 33% without using the X-ray machine zoom magnification protocols on a Siemens Artis Zee X-ray machine in a cardiac catheter laboratory results in a reduction of kerma-area product (KAP) for both diagnostic and interventional procedures. This reduction was predicted in an in vitro study in our laboratory, which has previously shown a 20.4% reduction in KAP. A retrospective analysis was conducted of the radiation exposure to compare the measured KAP recorded during the period when conventional magnification with automatic brightness and dose control was used on a Siemens Artis Zee X-ray machine with a flat panel detector and when magnification settings were avoided by using a large screen to enlarge and project a non-magnified image by digital magnification. The analysis was carried out for patients having a diagnostic coronary angiogram and those having an interventional coronary procedure. For diagnostic coronary angiograms the median KAP per procedure in the period using conventional magnification was 2124.5 µGy.m2 compared to 1401 µGy.m2 when image matrix magnification was used, a 34% reduction (p < 0.0001). For interventional coronary procedures, the median KAP per procedure in the period using conventional magnification was 3791 µGy.m2 compared to 2568.5 µGy.m2 when image matrix magnification was used, a 32% reduction (p < 0.0001). Avoiding using conventional magnification in the cardiac catheter laboratory and using a large screen to magnify images was associated with a statistically significant greater than 30% reduction in KAP. This paper is the proof in clinical practice of a theoretical conclusion that radiation dose (KAP) is reduced by use of Image matrix magnification using a large viewing screen without the need to use X-ray tube magnification without significant loss of image resolution in interventional cardiology. The same approach will be useful in interventional radiology.

The Life Experience of Patients With Coronary Heart Disease Post Percutaneous Coronary Intervention (PCI) in Bulukumba Regency

Coronary Heart Disease is a disease that interferes with heart function due to narrowing that occurs due to fatty deposits or plaque in the coronary arteries that block blood flow to the heart. The existence of a non-surgical PCI (Percutaneous Coronary Intervention) procedure which dilates blood vessels with the installation of a stent, makes many patients with coronary heart disease choose to undergo this procedure. The high number of choices for PCI action and the lack of research that discusses the life experiences of patients who have undergone PCI, makes researchers interested in conducting this study. This study aims to determine the life experiences of patients with coronary heart disease who have undergone percutaneous coronary intervention in Bulukumba Regency. The data in this study were collected through in-depth interviews on selected subjects (In-depth Interview) and analyzed using Thematic Analysis. Researchers found 4 themes from the results of data analysis. The themes are: (1) Lifestyle, sub-themes: diet, smoking, physical activity and medication adherence. (2) Psychological responses, sub themes: feelings of anxiety and worry, emotions and stress. (3) Forms of Support, sub-themes: family support, spiritual support. (4) Quality of Life for Post PCI Patients, sub-themes: feeling back to normal, changes in sleep patterns and pain. There are 4 themes found in this study. It is hoped that patients with coronary heart disease who have undergone PCI procedures will continue to increase their motivation to live a better life.

Implementing Machine Learning in Interventional Cardiology: The Benefits Are Worth the Trouble.

Driven by recent innovations and technological progress, the increasing quality and amount of biomedical data coupled with the advances in computing power allowed for much progress in artificial intelligence (AI) approaches for health and biomedical research. In interventional cardiology, the hope is for AI to provide automated analysis and deeper interpretation of data from electrocardiography, computed tomography, magnetic resonance imaging, and electronic health records, among others. Furthermore, high-performance predictive models supporting decision-making hold the potential to improve safety, diagnostic and prognostic prediction in patients undergoing interventional cardiology procedures. These applications include robotic-assisted percutaneous coronary intervention procedures and automatic assessment of coronary stenosis during diagnostic coronary angiograms. Machine learning (ML) has been used in these innovations that have improved the field of interventional cardiology, and more recently, deep Learning (DL) has emerged as one of the most successful branches of ML in many applications. It remains to be seen if DL approaches will have a major impact on current and future practice. DL-based predictive systems also have several limitations, including lack of interpretability and lack of generalizability due to cohort heterogeneity and low sample sizes. There are also challenges for the clinical implementation of these systems, such as ethical limits and data privacy. This review is intended to bring the attention of health practitioners and interventional cardiologists to the broad and helpful applications of ML and DL algorithms to date in the field. Their implementation challenges in daily practice and future applications in the field of interventional cardiology are also discussed.

Contemporary Clinical and Coronary Anatomic Risk Model for 30-Day Mortality After Percutaneous Coronary Intervention

Percutaneous coronary intervention (PCI) procedures are increasing in clinical and anatomic complexity, likely increasing the calculated risk of mortality. There is need for a real-time risk prediction tool that includes clinical and coronary anatomic information that is integrated into the electronic medical record system. We assessed 70 503 PCIs performed in 73 Veterans Affairs hospitals from 2008 to 2019. We used regression and machine-learning strategies to develop a prediction model for 30-day mortality following PCI. We assessed model performance with and without inclusion of the Veterans Affairs SYNTAX score (Synergy Between Percutaneous Coronary Intervention With Taxus and Cardiac Surgery), an assessment of anatomic complexity. Finally, the discriminatory ability of the Veterans Affairs model was compared with the CathPCI mortality model. The overall 30-day morality rate was 1.7%. The final model included 14 variables. Presentation status (salvage, emergent, urgent), ST-segment-elevation myocardial infarction, cardiogenic shock, age, congestive heart failure, prior valve disease, chronic kidney disease, chronic lung disease, atrial fibrillation, elevated international normalized ratio, and the Veterans Affairs SYNTAX score were all associated with increased risk of death, while increasing body mass index, hemoglobin level, and prior coronary artery bypass graft surgery were associated with lower risk of death. C-index for the development cohort was 0.93 (95% CI, 0.92-0.94) and for the 2019 validation cohort and the site validation cohort was 0.87 (95% CI, 0.83-0.92) and 0.86 (95% CI, 0.83-0.89), respectively. The positive likelihood ratio of predicting a mortality event in the top decile was 2.87% more accurate than the CathPCI mortality model. Inclusion of anatomic information in the model resulted in significant improvement in model performance (likelihood ratio test P<0.01). This contemporary risk model accurately predicts 30-day post-PCI mortality using a combination of clinical and anatomic variables. This can be immediately implemented into clinical practice to promote personalized informed consent discussions and appropriate preparation for high-risk PCI cases.

Non-Invasive Flow Ratio (NiFR) Measurement based on Angiography Images.

Background: Fractional flow reserve (FFR) is a gold standard to assess the impact of stenosis on the blood flow. The FFR method enhances diagnostic accuracy, lessens the need for stenting, and reduces costs. However, FFR is used in less than 10% of percutaneous coronary intervention (PCI) procedures because it needs pressure wires to measure the distal and proximal pressures and adenosine to create hyperemic conditions. Pressure-wire-based FFR measurement is, therefore, expensive and invasive.Objective: This study aims to introduce a new approach on the basis of 3D coronary angiography and the Thrombolysis in Myocardial Infarction (TIMI) frame count for fast computation of FFR in patients with coronary artery disease.Material and Methods: In this simulation study, we herein introduce Non-Invasive Flow Ratio drawing upon CFD to measure FFR based on coronary angiography images with less run time. In this study, 3D geometry was created based on coronary angiography images. The mean volumetric flow rate was calculated using the TIMI frame count. FFR calculated based on CFD was compared with pressure-wire-based FFR and NiFR was calculated in 85 patients. Results: The NiFR (r = 0.738, P< 0.001) exhibited a strong correlation with pressure-wire-based FFR. The result indicated that FFR was higher than 0.8 in the arteries with non-signif­icant stenosis and lower than 0.8 in the arter­ies with significant stenosis.Conclusion: The computational simulation of FFR and hemodynamic parameters such as pressure drop is a safe, efficient, and cost-effective method to evaluate the severity of coronary stenosis.

Effects of Metoprolol on Periprocedural Myocardial Infarction After Percutaneous Coronary Intervention (Type 4a MI): An Inverse Probability of Treatment Weighting Analysis.

Background: Metoprolol is the most used cardiac selective β-blocker and has been recommended as a mainstay drug in the management of acute myocardial infarction (AMI). However, the evidence supporting this regimen in periprocedural myocardial infarction (PMI) is limited.Methods: This study identified 860 individuals who suffered PMI following percutaneous coronary intervention (PCI) procedure and median followed up for 3.2 years. Subjects were dichotomized according to whether they received chronic oral sustained-release metoprolol succinate following PMI. After inverse probability of treatment weighting (IPTW) adjustment, logistic regression analysis, Kaplan-Meier curve, and Cox regression analysis were performed to estimate the effects of metoprolol on major adverse cardiovascular events (MACEs) which composed of cardiac death, myocardial infarction (MI), stroke, and revascularization. Moreover, an exploratory analysis was performed according to hypertension, cardiac troponin I (cTnI) elevation, and cardiac function. A double robust adjustment was used for sensitivity analysis.Results: Among enrolled PMI subjects, 456 (53%) patients received metoprolol treatment and 404 (47%) patients received observation. After IPTW adjustment, receiving metoprolol was found to reduce the subsequent 3-year risk of MACEs by nearly 7.1% [15 vs. 22.1%, absolute risk difference (ARD) = 0.07, number needed to treat (NNT) = 14, relative risk (RR) = 0.682]. In IPTW-adjusted Cox regression analyses, receiving metoprolol was related to a reduced risk of MACEs (hazard ratio [HR] = 0.588, 95%CI [0.385–0.898], P = 0.014) and revascularization (HR = 0.538, 95%CI [0.326–0.89], P = 0.016). Additionally, IPTW-adjusted logistic regression analysis showed that receiving metoprolol reduced the risk of MI at the third year (odds ratio [OR] = 0.972, 95% CI [0.948–997], P = 0.029). Exploratory analysis showed that the protective effect of metoprolol was more pronounced in subgroups of hypertension and cTnI elevation ≥1,000%, and was remained in patients without cardiac dysfunction. The benefits above were consistent when double robust adjustments were performed.Conclusion: In the real-world setting, receiving metoprolol treatment following PCI-related PMI has decreased the subsequent risk of MACEs, particularly the risk of recurrent MI and revascularization.

OR8. Appropriateness of percutaneous coronary intervention procedure in Indonesia

Abstract Background and Aims There is a possibility of the high variability of the appropriateness of PCI performed in Indonesia. It is assumed that appropriate PCI tends to have clinical outcomes than other categories. This study aimed to evaluate the appropriateness of the percutaneous coronary intervention (PCI) procedure in Indonesia. Method and Results We assigned appropriateness ratings to 214 acute coronary syndromes (ACS) and 191 stable ischemic heart disease (SIHD) records that underwent PCI in four hospitals from 2017 - 2018. The included hospitals consist of one public and three private hospitals, two cardiovascular centers, and two general hospitals with the most performed PCI procedures on 2016 - 2018 and accessible to the researchers. The PCI appropriateness was adjudicated using 2016/2017 ACC/AHA guidelines of the Appropriate Use of Care (AUC) for coronary revascularization in ACS and SIHD. The results were categorized into “appropriate”, “maybe appropriate”, and “rarely appropriate”. The result from this study demonstrated that in ACS patients, 76.0% and 24.0% of PCI were appropriate” and “maybe appropriate”. While, in SHID patients, 68.7%, 28.7%, and 2.6% of PCI were “appropriate”, “maybe appropriate”, and “rarely appropriate”. In ACS patients, “appropriate” PCI is more commonly found in ST-elevation myocardial infarction (STEMI) cases (62.6%). In SIHD patients, 54.0% and 46.0% of left-main diseases patients underwent “maybe appropriate” and “rarely appropriate” PCI. Conclusion The majority of PCI performed in ACS and SIHD patients from the studied hospitals are “appropriate”.
