Development of Non-Invasive Continuous Blood Pressure Measurement System Based on Multiple Feature Parameters

To address the issue of the difficulty in implementing non-invasive continuous blood pressure measurement technology in China, this study developed a high-performance synchronous electrocardiogram (ECG) and photoplethysmography (PPG) signal acquisition system. A PC-based human-computer interaction software platform was constructed, and continuous blood pressure measurement-related algorithms were integrated. Multiple feature parameters such as pulse wave transit time based on synchronous ECG and PPG signals were extracted, enabling non-invasive continuous blood pressure measurement. To verify the measurement accuracy of the system, tests and comparative verifications were carried out. The results demonstrated that the system could meet the requirements of relevant standards and have good application value.

Cuff-less wearable biosensor in continuous noninvasive human radial artery pulse waveform and blood pressure measurement using self-mixing interferometry

In this paper, we propose a compact, wearable biosensor for the noninvasive measurement of human radial artery pulse waveform curve (PWC) and blood pressure (BP). In this system, self-mixing interferometry (SMI) technology is employed to measure the weak arterial vascular deformation, enabling accurate PWC retrieval. Based on the reconstructed PWC features, BP values are precisely estimated by means of deep learning method. Here continuous wavelet transform (CWT), enabling visualization of the relationship between the SMI signal temporal frequency components and the PWC characteristics, is highlighted for PWC flipping points seeking and convolutional neural network (CNN) input parameter acquisition. For the first time, a novel deep learning network preprocessing method is proposed that allows direct feature extraction from the CWT scalogram of SMI signal without the complicated PWC reconstruction algorithm. The robustness and accuracy of our device are validated by a series of clinical measurements, mean absolute error (MAE) and standard deviation (STD) values are calculated and compared with the existing models. We approach minimal BP estimation results (MAE ± STD) of 1.41 ± 1.89 mmHg for systolic blood pressure (SBP) and 1.78 ± 2.01 mmHg for diastolic blood pressure (DBP), respectively. The luxuriant novelties and remarkable performance clearly demonstrate our wearable sensor’s great potential in BP monitoring, and other clinical applications.

Atrial fibrillation ablation performed under general anesthesia is associated with significantly low arterial blood pressure and higher incidence of hypotensive events

Abstract Introduction Catheter ablation is a pivotal treatment for atrial fibrillation (AF). When performed under general anesthesia (GA), it may be associated to hypotensive events, with unclear prevalence and prognostic impact. Continuous non-invasive blood pressure (CNBP) measurement is a validated and reliable tool for intraprocedural beat-to-beat advanced hemodynamic monitoring. Purpose To assess and compare the hemodynamic impact of GA during AF ablation using a CNBP (ClearSight) system. Methods In an observational single large volume center study we prospectively enrolled all patients undergoing AF ablation at our institution with CNBP monitoring, as per local practice. Patients were assigned to deep sedation (DS) or general anesthesia (GA) group according to the chosen sedation regimen. The following parameters were compared between the two strategies: systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), cardiac output (CO), cardiac index (CI), stroke volume (SV), stroke volume index (SVI). Hypotension events (HE) (MAP < 65 mmHg) were counted and compared between groups. Results Two hundred patients were enrolled from September 2022 to August 2023. 170 patients underwent AF ablation under DS (76,7% male, mean age 56,77±15,76), while 30 patients were treated under GA (70% male, mean age 60,72±13,79). A total of 117709 measurements were analyzed and compared, for an overall monitoring time of 39293,66 minutes. Patients treated under GA had significantly lower average SBP (104,83±21,48 mmHg vs 138,05±22,09 mmHg, p<0,001), DBP (59,59±11,89 mmHg vs 76,12±12,75 mmHg, p<0,001), MAP (74,96±15,02 mmHg vs 97,12±15,38 mmHg, p<0,001). We also observed significantly lower CO (4,40±1,47 L/min vs 5,16±1,62 L/min, p<0,001), CI (2,22±0,71 L/min/m2 vs 2,61±0,78 L/min/m2, p<0,001) SV (62,37±18,04 mL/beat vs 70,65±24,95 mL/beat, p<0,001) and SVI (31,40±8,54 mL/beat/m2 vs 35,80±12,07 mL/beat/m2, p<0,001). Patients treated under GA witnessed a higher number of per-patient HE (10,53±9,16 vs 0,61±2,07, p<0,001), and higher HE duration (4,8±6,62 min vs 2,82±3,66, p<0,001). Conclusion AF ablation performed under general anesthesia is associated with significantly lower systolic, diastolic, and mean arterial pressure, reduced cardiac output, stroke volume and stroke volume index when compared to deep sedation. Patients undergoing AF ablation under GA witness higher incidence and duration of hypotensive events. CNBP is a valid tool for beat-to-beat procedural hemodynamic assessment and can help identifying and treat hypotensive events. Further studies are needed to assess the potential prognostic impact of intraprocedural hypotension during AF ablation.

ACNN-BiLSTM: A Deep Learning Approach for Continuous Noninvasive Blood Pressure Measurement Using Multi-Wavelength PPG Fusion.

As an essential physiological indicator within the human body, noninvasive continuous blood pressure (BP) measurement is critical in the prevention and treatment of cardiovascular disease. However, traditional methods of blood pressure prediction using a single-wavelength Photoplethysmographic (PPG) have bottlenecks in further improving BP prediction accuracy, which limits their development in clinical application and dissemination. To this end, this study proposed a method to fuse a four-wavelength PPG and a BP prediction model based on the attention mechanism of a convolutional neural network and bidirectional long- and short-term memory (ACNN-BiLSTM). The effectiveness of a multi-wavelength PPG fusion method for blood pressure prediction was evaluated by processing PPG signals from 162 volunteers. The study compared the performance of the PPG signals with different individual wavelengths and using a multi-wavelength PPG fusion method in blood pressure prediction, assessed using mean absolute error (MAE), root mean squared error (RMSE) and AAMI-related criteria. The experimental results showed that the ACNN-BiLSTM model achieved a better MAE ± RMSE for a systolic BP and diastolic BP of 1.67 ± 5.28 and 1.15 ± 2.53 mmHg, respectively, when using the multi-wavelength PPG fusion method. As a result, the ACNN-BiLSTM blood pressure model based on multi-wavelength PPG fusion could be considered a promising method for noninvasive continuous BP measurement.

Can Currently Available Non-invasive Continuous Blood Pressure Monitors Replace Invasive Measurement With an Arterial Catheter?

Deviations from normal blood pressure (BP) during general anesthesia have been clearly linked to several adverse outcomes. Measuring BP accurately is therefore critically important for producing excellent outcomes in health care. Normal BP does not necessarily guarantee adequate organ perfusion however and adverse events have occurred even when BP seemed adequate. Invasive blood pressure monitoring has recently evolved beyond merely measuring BP. Arterial line-derived pulse contour analysis is used now to assess both cardiac output and stroke volume variation as indices of adequate intravascular volume. Confirmation of acceptable cardiac output with data derived from invasive intra-arterial catheters has become very important when managing high-risk patients. Newer devices that measure BP continuously and non-invasively in the digital arteries via a finger cuff have also become available. Many clinicians contemplate now if these new devices are ready to replace invasive monitoring with an arterial catheter. Unlike non-invasive devices, intra-arterial catheters allow frequent blood sampling. This makes it possible to assess vital parameters like pH, hemoglobin concentration, ionized calcium, potassium, glucose, and arterial partial pressure of oxygen and carbon dioxide frequently. Non-invasive continuous BP measurement has been found to be unreliable in critically ill patients, the elderly, and patients with calcified arteries. Pulse contour-derived estimates of cardiac output and stroke volume variation have been validated better with data derived from arterial lines than that from the newer finger cuff monitors. Significant advances have been recently made with non-invasive continuous BP monitors. Invasive monitoring with an arterial line however remains the gold standard for measuring BP and assessing pulse contour analysis-derived hemodynamic variables in critically ill patients. In the future, non-invasive continuous BP monitors will likely replace intermittent oscillometers in the operating room and the postoperative period. They will however not eliminate the need for arterial catheterization in critically ill patients.

History of blood pressure measurement in newborns and infants.

The development of methods for measuring blood pressure (BP) in newborns and small children has a rich history. Methods for BP measuring in adults had to be adapted to this age group. For measuring BP in direct invasive way, a suitable approach had to be found to access the arterial circulation through the umbilical and later radialis artery. Currently, results obtained from direct invasive BP measurement are considered the "gold standard". The development of non-invasive methods for BP measuring in newborns and children began with the use of von Basch's sphygmomanometer (1880). In 1899, Gustav Gärtner constructed the device, which was the basis for the flush method. After the discovery of the palpation and auscultation methods, these methods were also used for BP measurement in newborns and children, however, the BP values obtained in these ways were typically underestimated using excessively wide cuffs. From the auscultation method, methods utilizing ultrasound and infrasound to detect arterial wall movement and blood flow were later developed. The oscillometric method for BP measurement was introduced by E. J. Marey so early as in 1876. In 1912, P. Balard used the oscillometric technique to measure blood pressure in a large group of newborns. Through different types of oscillometers using various methods for detecting vascular oscillations (such as xylol method, impedance and volume plethysmography, etc.), the development has continued to assessment of vascular oscillations by modern sensor technology and software. For continuous non-invasive blood pressure measurement, the volume-clamp method, first described by Jan Peňáz in 1968, was developed. After modification for use in newborns, application of the cuff to the wrist instead of the finger, it is primarily used in clinical physiological studies to evaluate beat-to-beat BP and heart rate pressure variability, such as in the determination of the baroreflex sensitivity.

Deep learning-based methods demonstrate promising results in continuous non-invasive blood pressure measurement, whereas those models trained on large public datasets suffer from severe performance degradation in predicting from real-world user data collected in home settings. Transfer learning has been recently introduced to personalize the pre-trained model with unseen users' data to solve the problem. However, the existing methods based on network fine-tuning for model personalization require a large amount of labeled data, lacking practicality due to labeling using a cuff-based blood pressure monitor is extremely tedious and laborious for home users. In this paper, we propose a novel few-shot transfer learning approach named FewShotBP, which addresses the above-mentioned challenges by introducing a personalization adapter at the personalization stage (i.e., the transfer learning stage), and a multi-modal spectro-temporal neural network at the pre-train stage, to bridge the gap between data-hungry models and limited labeled data in realistic scenarios. To evaluate the approach's significance, we conducted experiments using both a publicly available dataset and a real-world user experiment. The results demonstrated that the proposed approach achieves similar accuracy of blood pressure prediction with 10× less data for personalization compared with the state-of-the-art method in the public dataset and achieves a mean absolute error of 6.68 mmHg (systolic blood pressure) and 3.91 mmHg (diastolic blood pressure) with only 10 personal data samples in the real-world user experiment.

Impaired cerebrovascular reactivity correlates with reduced retinal vessel density in patients with carotid artery stenosis: Cross-sectional, single center study.

The cerebral and retinal circulation systems are developmentally, anatomically, and physiologically interconnected. Thus, we hypothesized that hypoperfusion due to atherosclerotic stenosis of the internal carotid artery (ICA) can result in disturbances of both cerebral and retinal microcirculations. We aimed to characterize parameters indicating cerebrovascular reactivity (CVR) and retinal microvascular density in patients with ICA stenosis, and assess if there is correlation between them. In this cross-sectional study the middle cerebral artery (MCA) blood flow velocity was measured by transcranial Doppler (TCD) and, simultaneously, continuous non-invasive arterial blood pressure measurement was performed on the radial artery by applanation tonometry. CVR was assessed based on the response to the common carotid artery compression (CCC) test. The transient hyperemic response ratio (THRR) and cerebral arterial resistance transient hyperemic response ratio (CAR-THRR) were calculated. Optical coherence tomography angiography (OCTA) was used to determine vessel density (VD) on the papilla whole image for all (VDP-WIall) and for small vessels (VDP-WIsmall). The same was done in the peripapillary region: all (VDPPall), and small (VDPPsmall) vessels. The VD of superficial (VDMspf) and deep (VDMdeep) macula was also determined. Significance was accepted when p<0.05. Twenty-four ICA stenotic patients were evaluated. Both CVR and retinal VD were characterized. There was a significant, negative correlation between CAR-THRR (median = -0.40) and VDPPsmall vessels (median = 52%), as well as between VDPPall vessels (median = 58%), and similar correlation between CAR-THRR and VDP-WIsmall (median = 49.5%) and between VDP-WIall (median = 55%). The significant correlation between impaired cerebrovascular reactivity and retinal vessel density in patients with ICA stenosis suggests a common mechanism of action. We propose that the combined use of these diagnostic tools (TCD and OCTA) helps to better identify patients with increased ischemic or other cerebrovascular risks.

Continuous Noninvasive Blood Pressure Measurement With “ClearSight” Compared to Standard Intermittent Blood Pressure Measurement in Patients With Peripheral Arterial Disease. Are Potential Differences Influenced by Phenylephrine or Dobutamine?

To investigate the agreement between continuous noninvasive blood pressure measurement with the ClearSight system (cNIBP-CS) and standard intermittent noninvasive blood pressure measurement (iNIBP) in patients with peripheral arterial disease (PAD). Additionally, the influence of vasoactive medication on potential measurement differences was assessed. A secondary analysis of a randomized controlled trial. At a university hospital. Thirty-four patients with PAD undergoing percutaneous transluminal angioplasty of the lower limbs. None. Continuous noninvasive blood pressures were measured with the "ClearSight" system and compared to standard iNIBPs. Bland-Altman analysis revealed a mean bias of 13 mmHg (±15) between cNIBP-CS and iNIBP, with 95% limits of agreement (LOA) ranging from -17 to 42 mmHg. When comparing both medication groups, a similar mean bias was found for phenylephrine and dobutamine (12 mmHg [±13] and 13 mmHg [±13], respectively). In this study, in patients with PAD, cNIBP-CS showed an underestimation of blood pressure compared to iNIBP in phenylephrine- and dobutamine-treated patients. Compared to previous studies, a larger bias and wider 95% LOA were found.

PPG Signals-Based Blood-Pressure Estimation Using Grid Search in Hyperparameter Optimization of CNN-LSTM.

Researchers commonly use continuous noninvasive blood-pressure measurement (cNIBP) based on photoplethysmography (PPG) signals to monitor blood pressure conveniently. However, the performance of the system still needs to be improved. Accuracy and precision in blood-pressure measurements are critical factors in diagnosing and managing patients' health conditions. Therefore, we propose a convolutional long short-term memory neural network (CNN-LSTM) with grid search ability, which provides a robust blood-pressure estimation system by extracting meaningful information from PPG signals and reducing the complexity of hyperparameter optimization in the proposed model. The multiparameter intelligent monitoring for intensive care III (MIMIC III) dataset obtained PPG and arterial-blood-pressure (ABP) signals. We obtained 75,226 signal segments, with 60,180 signals allocated for training data, 12,030 signals allocated for the validation set, and 15,045 signals allocated for the test data. During training, we applied five-fold cross-validation with a grid-search method to select the best model and determine the optimal hyperparameter settings. The optimized configuration of the CNN-LSTM layers consisted of five convolutional layers, one long short-term memory (LSTM) layer, and two fully connected layers for blood-pressure estimation. This study successfully achieved good accuracy in assessing both systolic blood pressure (SBP) and diastolic blood pressure (DBP) by calculating the standard deviation (SD) and the mean absolute error (MAE), resulting in values of 7.89 ± 3.79 and 5.34 ± 2.89 mmHg, respectively. The optimal configuration of the CNN-LSTM provided satisfactory performance according to the standards set by the British Hypertension Society (BHS), the Association for the Advancement of Medical Instrumentation (AAMI), and the Institute of Electrical and Electronics Engineers (IEEE) for blood-pressure monitoring devices.

Incremental value of continuous non-invasive blood pressure control to assess hypotension during atrial fibrillation ablation

Introduction Atrial Fibrillation (AF) ablation is associated with continuous blood pressure (BP) variations related to anesthesiological regimen and energy delivery. Conventional BP monitoring with non-invasive periodic brachial BP controls can miss potential hypotensive events. Continuous non-invasive blood pressure (CNBP) measurement using the volume-clamp method (Clearsight) was demonstrated to be a reliable and effective tool for the assessment of BP during AF ablation. Purpose Aim of the study was to investigate the incidence of hypotensive events during AF ablation. Methods We included consecutive patients scheduled for AF ablation and monitored in parallel with a periodic (10 minutes) brachial BP cuff and in continuous with the CNBP System (Clearsight). All CNBP data were analyzed with the Acumen Analytics software for hypotension evaluation. According to the anesthesiological regimen, patients were divided in 2 Groups, the General Anesthesia (GA) and the Deep Sedation (DS) group. Area under the threshold (AUT, mmHg x min) of 65 mmHg and Time Weighted Average (TWA, mmHg, i.e., the AUT divided by the total duration of procedure in minutes) of area under threshold (MAP &amp;lt;65 mmHg) were compared between the two groups. Results Forty-eight patients were included, 10 (21%) in the General Anesthesia Group and 38 (79%) in the Sedation Group. Hypotension was detected in 18 patients, 9/10 (90%) were recorded in the GA group and 9/38 (23.7%) in the DS one. A total of 85 hypotensive events were recorded, 68 (80%) in the GA and 17 (20%) in the DS Group. In the overall population, mean duration of hypotensive events was 4.87 minutes (range 0 – 14.73 minutes), mean MAP &amp;lt; 65 mmHg was 54.25 ± 7.2 mmHg, mean AUT (MAP &amp;lt;65 mmHg) was 79.4 mmHg x min (range 0 – 1237.7) and mean TWA was 0.41 mmHg (range 0 – 6.9). Hypotensions were more often documented in GA than in DS patients (mean difference 6.35 events/procedure, C.I. 2.66 – 10.10, p&amp;lt;0.05) so as AUTs (mean difference 319.90 mmHg x min, C.I. 30.00 – 609.80, p=0.034) and TWAs (mean difference 1.64 mmHg, C.I. 0.18 – 3.09, p=0.032) which were significantly higher in the GA group. Brachial periodic BP measurement every 10 minutes missed the majority (n=52, 61.2%) of hypotensive events. Conclusions CNBP demonstrated a high incidence of hypotension during AF ablation, specially under GA, which are usually missed by the standard intermittent brachial non-invasive BP monitoring. Real time hypotension predictive CNBP indexes could be valuable in this setting.

Continuous Non-Invasive Blood Pressure Measurement Using 60 GHz-Radar-A Feasibility Study.

Blood pressure monitoring is of paramount importance in the assessment of a human's cardiovascular health. The state-of-the-art method remains the usage of an upper-arm cuff sphygmomanometer. However, this device suffers from severe limitations-it only provides a static blood pressure value pair, is incapable of capturing blood pressure variations over time, is inaccurate, and causes discomfort upon use. This work presents a radar-based approach that utilizes the movement of the skin due to artery pulsation to extract pressure waves. From those waves, a set of 21 features was collected and used-together with the calibration parameters of age, gender, height, and weight-as input for a neural network-based regression model. After collecting data from 55 subjects from radar and a blood pressure reference device, we trained 126 networks to analyze the developed approach's predictive power. As a result, a very shallow network with just two hidden layers produced a systolic error of 9.2±8.3 mmHg (mean error ± standard deviation) and a diastolic error of 7.7±5.7 mmHg. While the trained model did not reach the requirements of the AAMI and BHS blood pressure measuring standards, optimizing network performance was not the goal of the proposed work. Still, the approach has displayed great potential in capturing blood pressure variation with the proposed features. The presented approach therefore shows great potential to be incorporated into wearable devices for continuous blood pressure monitoring for home use or screening applications, after improving this approach even further.

Feasibility and Accuracy of Noninvasive Continuous Arterial Pressure Monitoring during Transcatheter Atrial Fibrillation Ablation

Transcatheter atrial fibrillation (AF) ablation is still carried out with continuous invasive radial arterial blood pressure (IBP) monitoring in many centers. Continuous noninvasive blood pressure (CNBP) measurement using the volume-clamp method is a noninvasive alternative method used in ICU. No data on CNBP reliability are available in the electrophysiology lab during AF ablation, where rhythm variations are common. The objective of the present study was to compare continuous noninvasive arterial pressure measured with the ClearSight device (Edwards Lifesciences, Irvine, CA, USA) with invasive radial artery pressure used as the reference method during AF ablation. We prospectively enrolled 55 consecutive patients (age 62 ± 11 years, 80% male) undergoing transcatheter AF ablation (62% paroxysmal, 38% persistent) at our center. Standard of care IBP monitoring via a radial cannula and a contralateral noninvasive finger volume-clamp CNBP measurement device were positioned simultaneously in all patients for the entire procedure. Bland-Altman analysis was used to analyze the agreement between the two techniques. A total of 1219 paired measurements for systolic, diastolic, and mean arterial pressure were obtained in 55 subjects, with a mean (SD) of 22 (9) measurements per patient. The mean bias (SD) was -12.97 (13.89) mmHg for systolic pressure (level of agreement -14.24-40.20; correlation coefficient 0.84), -1.85 (8.52) mmHg for diastolic pressure (level of agreement -18.54-14.84; correlation coefficient 0.77) and 2.31 (8.75) mmHg for mean pressure (level of agreement -14.84-19.46; correlation coefficient 0.85). In patients undergoing AF ablation, CNBP monitoring with the ClearSight device showed acceptable agreement with IBP monitoring. Larger studies are needed to confirm the potential clinical implications of continuous noninvasive BP monitoring during AF ablation.

Continuous Noninvasive Remote Automated Blood Pressure Monitoring With Novel Wearable Technology: A Preliminary Validation Study.

BackgroundWearable continuous monitoring biosensor technologies have the potential to transform postoperative care with early detection of impending clinical deterioration.ObjectiveOur aim was to validate the accuracy of Cloud DX Vitaliti continuous vital signs monitor (CVSM) continuous noninvasive blood pressure (cNIBP) measurements in postsurgical patients. A secondary aim was to examine user acceptance of the Vitaliti CVSM with respect to comfort, ease of application, sustainability of positioning, and aesthetics.MethodsIncluded participants were ≥18 years old and recovering from surgery in a cardiac intensive care unit (ICU). We targeted a maximum recruitment of 80 participants for verification and acceptance testing. We also oversampled to minimize the effect of unforeseen interruptions and other challenges to the study. Validation procedures were according to the International Standards Organization (ISO) 81060-2:2018 standards for wearable, cuffless blood pressure (BP) measuring devices. Baseline BP was determined from the gold-standard ICU arterial catheter. The Vitaliti CVSM was calibrated against the reference arterial catheter. In static (seated in bed) and supine positions, 3 cNIBP measurements, each 30 seconds, were taken for each patient with the Vitaliti CVSM and an invasive arterial catheter. At the conclusion of each test session, captured cNIBP measurements were extracted using MediCollector BEDSIDE data extraction software, and Vitaliti CVSM measurements were extracted to a secure laptop through a cable connection. The errors of these determinations were calculated. Participants were interviewed about device acceptability.ResultsThe validation analysis included data for 20 patients. The average times from calibration to first measurement in the static position and to first measurement in the supine position were 133.85 seconds (2 minutes 14 seconds) and 535.15 seconds (8 minutes 55 seconds), respectively. The overall mean errors of determination for the static position were –0.621 (SD 4.640) mm Hg for systolic blood pressure (SBP) and 0.457 (SD 1.675) mm Hg for diastolic blood pressure (DBP). Errors of determination were slightly higher for the supine position, at 2.722 (SD 5.207) mm Hg for SBP and 2.650 (SD 3.221) mm Hg for DBP. The majority rated the Vitaliti CVSM as comfortable. This study was limited to evaluation of the device during a very short validation period after calibration (ie, that commenced within 2 minutes after calibration and lasted for a short duration of time).ConclusionsWe found that the Cloud DX’s Vitaliti CVSM demonstrated cNIBP measurement in compliance with ISO 81060-2:2018 standards in the context of evaluation that commenced within 2 minutes of device calibration; this device was also well-received by patients in a postsurgical ICU setting. Future studies will examine the accuracy of the Vitaliti CVSM in ambulatory contexts, with attention to assessment over a longer duration and the impact of excessive patient motion on data artifacts and signal quality.Trial RegistrationClinicalTrials.gov NCT03493867; https://clinicaltrials.gov/ct2/show/NCT03493867

In vitro validation of a non-invasive continuous blood pressure measurement method for the perioperative application

Abstract Hypotensive periods during surgery lead to an increase in postoperative complications. To further avoid the adverse events associated with the invasive measurement of the blood pressure, a perioperative, continuous, non-invasive blood pressure measurement method was developed. The precision of the method was tested for different mean pressure values and different thicknesses of the tissue model using a specifically designed and validated simulator that has the characteristics of the lower arm. The mean pressure difference between the pressure determined by the proposed method and simulated blood pressure from the simulator was 3.93 mmHg (±4.75 mmHg) for the systole and 4.89 mmHg (±6.10 mmHg) for the diastole. The results showed that the method can be a promising alternative to invasive blood pressure measurement methods.

Non-invasive methods of cardiac output measurement and their importance in everyday clinical practice: the current state of knowledge

Cardiac output is an important parameter in the evaluation of the patients’ haemodynamic status which can be measured using a number of methods with varying degrees of invasiveness. These include invasive (such as pulmonary artery catheterization, transpulmonary thermodilution, lithium dilution), minimally invasive (catheter-based arterial pressure curve contour analysis, transoesophageal Doppler) and non-invasive (electrical bioimpedance, transthoracic echocardiography, measurement of exhaled CO2, continuous non-invasive blood pressure measurement) techniques. With advances in medicine, the role of non-invasive methods as an alternative to pulmonary artery catheterization continues to grow. The undoubted advantage of non-invasive methods is their safety for the patients and their wide availability. In contrast, invasive methods are characterized by greater measurement precision. It is up to the physician to choose the most appropriate haemodynamic monitoring method depending on the clinical situation. The aim of this article is to discuss non-invasive cardiac output measurement techniques and their application in everyday clinical practice.

ClearSight™ finger cuff versus invasive arterial pressure measurement in patients with body mass index above 45 kg/m2

BackgroundMeasuring blood pressure in patients with obesity is challenging. The ClearSight™ finger cuff (FC) uses the vascular unloading technique to provide continuous non-invasive blood pressure measurements. We aimed to test the agreement of the FC with invasive radial arterial monitoring (INV) in patients with obesity.MethodsParticipants had a body mass index (BMI) ≥45 kg/m2 and underwent laparoscopic bariatric surgery. FC and INV measurements were obtained simultaneously every 5 min on each patient, following induction of anesthesia. Agreement over time was assessed using modified Bland-Altman plots and error grid analysis permitted clinical interpretation of the results. Four-quadrant plots allowed assessment of concordance in blood pressure changes.ResultsThe 30 participants had a median (IQR) BMI of 50.2 kg/m2 (IQR 48.3–55.3). The observed bias (SD, 95% limits of agreement) for systolic blood pressure (SBP) was 14.3 mmHg (14.1, -13.4 – 42.0), 5.2 mmHg (10.9, -16.0 – 26.5) for mean arterial pressure (MAP) and 2.6 mmHg (10.8, -18.6 – 23.8) for diastolic blood pressure (DBP). Error grid analysis showed that the proportion of readings in risk zones A-E were 90.8, 6.5, 2.7, 0 and 0% for SBP and 91.4, 4.3, 4.3, 0 and 0% for MAP, respectively. Discordance occurred in ≤8% of pairs for consecutive change in SBP, MAP and DBP.ConclusionsThe vascular unloading technique was not adequately in agreement with radial arterial monitoring. Evaluation in a larger sample is required before recommending this technique for intraoperative monitoring of patients with BMI ≥45 kg/m2.

Cerebral perfusion pressure and autoregulation in eclampsia—a case control study

Dynamic cerebral autoregulation and cerebral perfusion pressure are altered in pregnancies complicated by preeclampsia compared with normotensive pregnancies, but the connections of dynamic cerebral autoregulation, cerebral perfusion pressure, and cerebral complications in preeclampsia remain unclear. This study aimed to assess dynamic cerebral autoregulation and cerebral perfusion pressure after delivery in women with eclampsia, in women with preeclampsia both with and without severe features, and in normotensive women. This was a prospective case control study at a large referral hospital in Cape Town, South Africa. The recruitment of participants was done at diagnosis (cases) or at admission for delivery (controls). Transcranial Doppler examinations with continuous noninvasive blood pressure measurements and end-tidal CO2 monitoring were conducted for cases and controls after delivery. Cerebral perfusion pressure and dynamic cerebral autoregulation index were calculated, and values were compared among groups. We included 16 women with eclampsia, 18 women with preeclampsia with severe features, 32 women with preeclampsia without severe features, and 21 normotensive women with uncomplicated pregnancies. Dynamic cerebral autoregulation was depressed in pregnant women with eclampsia; (autoregulation index, 3.9; interquartile range, 3.1-5.2) compared with all other groups (those with preeclampsia with severe features, autoregulation index, 5.6 [interquartile range, 4.4-6.8]; those with preeclampsia without severe features, autoregulation index, 6.8 [interquartile range, 5.1-7.4]; and normotensive controls, autoregulation index, 7.1 [interquartile range, 6.1-7.9]). Pregnant women with eclampsia had increased cerebral perfusion pressure (109.5 mm Hg; interquartile range, 91.2-130.9) compared with those with preeclampsia without severe features and those with normal blood pressure (84 mm Hg [interquartile range, 73.0-122.0] and 80.0 mm Hg [interquartile range, 67.5-92.0], respectively); furthermore, there was no difference in cerebral perfusion pressure between pregnant women with eclampsia and pregnant women with preeclampsia with severe features (109.5 mm Hg [interquartile range, 91.2-130.9] vs 96.5 mm Hg [interquartile range, 75.8-110.5]). Cerebral perfusion pressure and dynamic cerebral autoregulation are altered in eclampsia and may be important in the pathophysiological pathway and constitute a therapeutic target in the prevention of cerebral complications in preeclampsia.

A novel art of continuous noninvasive blood pressure measurement

Wearable sensors to continuously measure blood pressure and derived cardiovascular variables have the potential to revolutionize patient monitoring. Current wearable methods analyzing time components (e.g., pulse transit time) still lack clinical accuracy, whereas existing technologies for direct blood pressure measurement are too bulky. Here we present an innovative art of continuous noninvasive hemodynamic monitoring (CNAP2GO). It directly measures blood pressure by using a volume control technique and could be used for small wearable sensors integrated in a finger-ring. As a software prototype, CNAP2GO showed excellent blood pressure measurement performance in comparison with invasive reference measurements in 46 patients having surgery. The resulting pulsatile blood pressure signal carries information to derive cardiac output and other hemodynamic variables. We show that CNAP2GO can self-calibrate and be miniaturized for wearable approaches. CNAP2GO potentially constitutes the breakthrough for wearable sensors for blood pressure and flow monitoring in both ambulatory and in-hospital clinical settings.
