Surgical Repair of a Congenital Left Ventricular Aneurysm

Congenital left ventricular aneurysm is an uncommon cardiac malformation. A 9-year-old boy with complaints of dyspnea and palpitation was diagnosed with a left ventricular aneurysm originating from the left ventricle free wall. Aneurysm resection and endoventricular patch repair was performed. Postoperative follow-up was uncomplicated and follow-up echocardiographs showed normal left ventricular contractility.

Left ventricular aneurysm occurs most commonly as a consequence of myocardial Infarction. Congenital Left Ventricular aneurysms occurs rarely as diverticuli of the apex, as a part of a syndrome of congenital defects or following myocardial infarction in children with anomalous origin of coronary arteries, Post-traumatic left ventricular ancurysms are also reported in literature. Chronic infections like tuberculosis with arteritis is also reported as one of the causative factor. Here we report a case of such left ventricular pseudoaneurysms in a young patient probably ischemic in origin. The patient underwent surgery with favorable outcome.

Coexistence of a huge subaortic left ventricular aneurysm and a saccular descending aortic aneurysm

A huge subaortic left ventricular aneursym was diagnosed in an asymptomatic 22-year-old male patient. The walls of aneurysm were calcific and compressed the left atrium. Also, a saccular descending aortic aneursym and moderate mitral insufficiency associated with subaortic left ventricular aneurysm were found in the current case. Up to now, the coexistence of congenital left ventricular aneurysm and saccular aortic aneursym has not been reported in the literature. We present the first case of congenital left ventricular aneurysm combined with saccular aortic aneurysm in descending thoracic aorta in this case report.

Congenital Cardiac Left Ventricular Aneurysm With Pericardial Effusion

Congenital left ventricular aneurysm is a rare condition characterized by a protrusion or outpouching of the ventricular wall. The etiology is unknown, and there is a variable prognosis in the current literature. Approximately 20 cases detected by prenatal ultrasonography have been reported, 1 - 1 2 with 4 of these cases indicating the presence of pericardial fluid. 1 , 1 0 , 1 1 We report a case of left ventricular aneurysm with a large pericardial effusion diagnosed at 17 weeks' gestation but retrospectively recognized on imaging at 15 weeks 6 days. To our knowledge, this represents the earliest reported prenatal diagnosis to date and newly reports placement of a pericardial drain catheter in this condition.

Congenital left ventricular aneurysm: Clinical, imaging, pathologic, and surgical findings in seven new cases

Background Congenital left ventricular aneurysm is a poorly understood and potentially lethal entity. Methods and Results In a clinicopathologic study of 7 new cases, the major presenting features in 6 patients were congestive heart failure in 4, ventricular arrhythmias in a 32-week fetus, and multiple congenital anomalies in a fetus with trisomy 13. Accurate diagnosis was achieved in all 3 living patients by echocardiography, angiocardiography, and magnetic resonance imaging. The aneurysm was predominantly apical in 3 and involved most of the left ventricular free wall in 4. Of the 3 living patients, medical management alone sufficed in 2. The third, a newborn boy, underwent a new and successful aneurysm-exclusion left ventriculoplasty. The mitral valve was abnormal in all 4 autopsied cases, the papillary muscles being short, thin, or absent. The aneurysm was thinner and its area was larger than that of the nonaneurysmal left ventricle in all necropsied patients. Conclusions Congenital left ventricular aneurysm appears to be a developmental anomaly, an idiopathic dysplasia of left ventricular endocardium and myocardium. No evidence of a viral etiology was found. Some neonates can be managed medically, but others require urgent surgical intervention. A new surgical operation is presented, a functional left ventricular aneurysmectomy that minimizes intraoperative and postoperative blood loss and that preserves the coronary arteries. Prenatal Diagnosis and Management of Congenital Left Ventricular Aneurysm

A pregnant woman was referred for fetal echocardiographic examination for cardiac dys-rhythmia. During serial examinations, congen-ital left ventricular aneurysm was diagnosed in the fetus. The case is discussed, and a review of the literature on this rare entity and its rela-tionship to cardiac dysrhythmias is presented. The management dilemmas practitioners must address when facing this unusual lesion are also discussed.

Giant Congenital Left Ventricular Aneurysm in an Adult— A Case Report

Congenital cardiac aneurysms are extremely rare. The echocardiographic features of this condition have never been described. The authors report a case of giant congenital left ventricular aneurysm wherein the diagnosis was made by echocardiography and confirmed at cardiac catheterization. The aneurysm was surgically removed. The histologic examination showed that the aneurysmal wall was only 1.5 to 2 mm thick but contained all the layers of the heart.

Congenital submitral left ventricular aneurysm is a rare entity, seen predominantly in the natives of South and West Africa. We report two cases, highlighting the cross-sectional echocardiographic features in each case.

A case of left ventricular aneurysm of uncertain etiology presenting as ventricular tachycardia

A case of a 59-year-old Asian male with several episodes of syncope and palpitation is reported. Electrophysiologic study disclosed that sustained ventricular tachycardia was induced by programmed electrical stimulation at the right ventricular apex. The electrogram in the left ventricular aneurysm showed delayed activities and the fastest depolarization. Entrainment was observed by faster pacing. Left ventriculography showed a large aneurysm and normal coronary arteries. Congenital left ventricular aneurysm is the most likely clinical diagnosis given the lack of past history of chest pain, myocarditis, cardiomyopathy, etc. Congenital aneurysm which presents as ventricular tachycardia is very rare, and has never been reported in an Asian. Medical management has thus far been successful.

Congenital Aneurysm of the Left Ventricle

TRUE left ventricular aneurysm, congenital or acquired, may be defined as an abnormal bulge or outpouching of a portion of the left ventricular wall with or without paradoxical (dyskinetic) systolic expansion. The wall of the aneurysm is composed of either myocardium or fibrous tissue and may be calcified. 1-3 In contrast, a false aneurysm or pseudoaneurysm of the left ventricle (LV) represents a localized myocardial rupture, with the hemorrhage limited by adherent pericardium and fibrous tissue. 4,5 Congenital and false aneurysms of the LV must be differentiated from left ventricular aneurysms resulting from myocardial infarction because the former tend to rupture. Congenital Apical Left Ventricular Aneurysm

A case of congenital apical left ventricular aneurysm in a 35-year-old man with normal coronary arteries is reported. There was no myocardial infarction evident on electrocardiogram, no previous thoracic trauma, no sarcoidosis or pyogenic infection.
