Ring artifacts suppression for X-ray CT images by fusion of dual-domain images based on improved UNet

The ring artifact is a typical artifact in X-ray Computed Tomography (CT) images, the existence of which can reduce the image's quality, change the structure and details and affects the interpretation of image information. This paper conducts research from two aspects: tomographic images and sinograms, and designs a dual-domain artifact suppression network. By introducing the Inception module and the residual module, the network's ability to extract features is improved, and the gradient diffusion is suppressed. By customizing the loss function, the network's recognition of ring artifacts is improved and the network's convergence is accelerated. Finally, the outputs of the dual-domain are fused based on the Nonsubsampled Contourlet Transform (NSCT) fusion theory to improve the quality of the image. In addition, three experiments of CT scanning are designed based on microfocus cone-beam CT system to verify the effectiveness. The model is trained and tested using real datasets. Peak Signal-to-Noise Ratio (PSNR), Structural Similarity (SSIM), and Mean Squared Error (MSE) are employed as evaluation indicators of image quality. Finally, the proposed method is compared with typical algorithms for artifact suppression. The experimental results show that the method effectively protects the structure information in CT images while suppressing ring artifacts to the greatest extent. The PSNR, SSIM, and MSE of the processed images can respectively reach 36.167 dB, 0.9847 and 6.87 × e-4.

Image quality evaluation of a new high-performance ring-gantry cone-beam computed tomography imager

Objective. Newer cone-beam computed tomography (CBCT) imaging systems offer reconstruction algorithms including metal artifact reduction (MAR) and extended field-of-view (eFoV) techniques to improve image quality. In this study a new CBCT imager, the new Varian HyperSight CBCT, is compared to fan-beam CT and two CBCT imagers installed in a ring-gantry and C-arm linear accelerator, respectively. Approach. The image quality was assessed for HyperSight CBCT which uses new hardware, including a large-size flat panel detector, and improved image reconstruction algorithms. The decrease of metal artifacts was quantified (structural similarity index measure (SSIM) and root-mean-squared error (RMSE)) when applying MAR reconstruction and iterative reconstruction for a dental and spine region using a head-and-neck phantom. The geometry and CT number accuracy of the eFoV reconstruction was evaluated outside the standard field-of-view (sFoV) on a large 3D-printed chest phantom. Phantom size dependency of CT numbers was evaluated on three cylindrical phantoms of increasing diameter. Signal-to-noise and contrast-to-noise were quantified on an abdominal phantom. Main results. In phantoms with streak artifacts, MAR showed comparable results for HyperSight CBCT and CT, with MAR increasing the SSIM (0.97–0.99) and decreasing the RMSE (62–55 HU) compared to iterative reconstruction without MAR. In addition, HyperSight CBCT showed better geometrical accuracy in the eFoV than CT (Jaccard Conformity Index increase of 0.02–0.03). However, the CT number accuracy outside the sFoV was lower than for CT. The maximum CT number variation between different phantom sizes was lower for the HyperSight CBCT imager (∼100 HU) compared to the two other CBCT imagers (∼200 HU), but not fully comparable to CT (∼50 HU). Significance. This study demonstrated the imaging performance of the new HyperSight CBCT imager and the potential of applying this CBCT system in more advanced scenarios by comparing the quality against fan-beam CT.

AAPM Task Group Report 261: Comprehensive quality control methodology and management of dental and maxillofacial cone beam computed tomography (CBCT) systems.

Cone-beam computed tomography (CBCT) systems specifically designed and manufactured for dental, maxillofacial imaging (MFI) and otolaryngology (OLR) applications have been commercially available in the United States since 2001 and have been in widespread clinical use since. Until recently, there has been a lack of professional guidance available for medical physicists about how to assess and evaluate the performance of these systems and about the establishment and management of quality control (QC) programs. The owners and users of dental CBCT systems may have only a rudimentary understanding of this technology, including how it differs from conventional multidetector CT (MDCT) in terms of acceptable radiation safety practices. Dental CBCT systems differ from MDCT in several ways and these differences are described. This report provides guidance to medical physicists and serves as a basis for stakeholders to make informed decisions regarding how to manage and develop a QC program for dental CBCT systems. It is important that a medical physicist with experience in dental CBCT serves as a resource on this technology and the associated radiation protection best practices. The medical physicist should be involved at the pre-installation stage to ensure that a CBCT room configuration allows for a safe and efficient workflow and that structural shielding, if needed, is designed into the architectural plans. Acceptance testing of new installations should include assessment of mechanical alignment of patient positioning lasers and x-ray beam collimation and benchmarking of essential image quality performance parameters such as image uniformity, noise, contrast-to-noise ratio (CNR), spatial resolution, and artifacts. Several approaches for quantifying radiation output from these systems are described, including simply measuring the incident air-kerma (Kair) at the entrance surface of the image receptor. These measurements are to be repeated at least annually as part of routine QC by the medical physicist. QC programs for dental CBCT, at least in the United States, are often driven by state regulations, accreditation program requirements, or manufacturer recommendations.

Novel Detector Configurations in Cone-Beam CT Systems: A Simulation Study

Cone-beam computed tomography (CBCT) has emerged in recent years as an adequate alternative to mammography and tomosynthesis due to the several advantages over traditional mammography, including its ability to provide 3D images, its reduced radiation dose, and its ability to image dense breasts more effectively and conduct more effective breast compressions, etc. Furthermore, CBCT is capable of providing images with high sensitivity and specificity, allowing a more accurate evaluation, even of dense breasts, where mammography and tomosynthesis may lead to a false diagnosis. Clinical and experimental CBCT systems rely on cesium iodine (CsI:Tl) scintillators for X-ray energy conversion. This study comprises an investigation among different novel CBCT detector technologies, consisting either of scintillators (BGO, LSO:Ce, LYSO:Ce, LuAG:Ce, CaF2:Eu, LaBr3:Ce) or semiconductors (Silicon, CZT) in order to define the optimum detector design for a future experimental setup, dedicated to breast imaging. For this purpose, a micro-CBCT system was adapted, using GATE v9.2.1, consisting of the aforementioned various detection schemes. Two phantom configurations were selected: (a) an aluminum capillary positioned at the center of the field of view in order to calculate the system’s spatial resolution and (b) a breast phantom consisting of spheres of different materials, such that their characteristics are close to the breast composition. Breast phantom contrast-to-noise ratios (CNRs) were extracted from the phantom’s tomographic images. The images were reconstructed with filtered back projection (FBP) and ordered subsets expectation-maximization (OSEM) algorithms. The semiconductors acted satisfactorily in low-density matter, while LYSO:Ce, LaBr3:Ce, and LuAG:Ce presented adequate CNRs for all the different spheres’ densities. The energy converters that are presented in this study were evaluated for their performance against the standard CsI:Tl crystal.

Image quality characterization of an ultra-high-speed kilovoltage cone-beam computed tomography imaging system on an O-ring linear accelerator.

The quality of on-board imaging systems, including cone-beam computed tomography (CBCT), plays a vital role in image-guided radiation therapy (IGRT) and adaptive radiotherapy. Recently, there has been an upgrade of the CBCT systems fused in the O-ring linear accelerators called HyperSight, featuring a high imaging performance. As the characterization of a new imaging system is essential, we evaluated the image quality of the HyperSight system by comparing it with Halcyon 3.0 CBCT and providing benchmark data for routine imaging quality assurance. The HyperSight features ultra-fast scan time, a larger kilovoltage (kV) detector, a more substantial kV tube, and an advanced reconstruction algorithm. Imaging protocols in the two modes of operation, treatment mode with IGRT and the CBCT for planning(CBCTp) mode were evaluated and compared with Halcyon 3.0 CBCT. Image quality metrics, including spatial resolution, contrast resolution, uniformity, noise, computed tomography (CT) number linearity, and calibration error, were assessed using a Catphan and an electron density phantom and analyzed with TotalQA software. HyperSight demonstrated substantial improvements in contrast-to-noise ratio and noise in both IGRT and CBCTp modes compared to Halcyon 3.0 CBCT. CT number calibration error of HyperSight CBCTp mode (1.06%) closely matches that of a full CT scanner (0.72%), making it suitable for adaptive planning. In addition, the advanced hardware of HyperSight, such as ultra-fast scan time (5.9s) or 2.5 times larger heat unit capacity, enhanced the clinical efficiency in our experience. HyperSight represented a significant advancement in CBCT imaging. With its image quality, CT number accuracy, and ultra-fast scans, HyperSight has a potential to transform patient care and treatment outcomes. The enhanced scan speed and image quality of HyperSight are expected to significantly improve the quality and efficiency of treatment, particularly benefiting patients.

Effect of scatter suppression with 2D antiscatter grids in photon counting compact CBCT.

Energy sensitive and photon counting detectors can provide improved tissue visualization and material quantification capabilities in Cone Beam Computed Tomography (CBCT) systems. However, their implementation in CBCT systems is more challenging, which is in part due to high fluence of scattered X-rays in wide cone angle CBCT geometry. Specifically, high scatter contamination in lower energy spectrum challenges reconstruction of high fidelity CBCT images by using lower energy X-rays. To address this problem, we investigated a robust scatter rejection with 2D antiscatter grids in a benchtop photon counting and compact CBCT system. The benchtop system employs a 35 cm wide CdTe photon counting detector with two energy thresholds. To reject scatter, a dedicated 2D antiscatter grid (2D grid) prototype made from tungsten was developed and mounted directly on the detector. To correct residual scatter not stopped by the 2D grid, a measurement-based scatter correction method, referred to as Grid-based Scatter Sampling (GSS), was utilized. Without 2D grid, scatter to primary ratio (SPR) reached 2.3 in the 15-40 keV energy bin. SPR was factor of 3 higher in the lowest energy bin when compared to the highest energy bin (90-120 keV). With the 2D grid, SPR was reduced below 0.14, and SPR values were more homogenous across the energy spectrum. CT number nonuniformity was factor of 3 lower in both low and high energy bin CBCT reconstructions. Improvement in contrast to noise ratio and contrast was more pronounced in the low energy bin CBCT images. This work indicates that 2D grids can significantly reduce spectral contamination caused by scatter in photon counting compact CBCT, and potentially enable higher fidelity CBCT image reconstructions.

ACR benchmark testing of a novel high-speed ring-gantry linac kV-CBCT system.

A new generation cone-beam computed tomography (CBCT) system with new hardware design and advanced image reconstruction algorithms is available for radiation treatment simulation or adaptive radiotherapy (HyperSight CBCT imaging solution, Varian Medical Systems-a Siemens Healthineers company). This study assesses the CBCT image quality metrics using the criteria routinely used for diagnostic CT scanner accreditation as a first step towards the future use of HyperSight CBCT images for treatment planning and target/organ delineations. Image performance was evaluated using American College of Radiology (ACR) Program accreditation phantom tests for diagnostic computed tomography systems (CTs) and compared HyperSight images with a standard treatment planning diagnostic CT scanner (Siemens SOMATOM Edge) and with existing CBCT systems (Varian TrueBeam version 2.7 and Varian Halcyon version 2.0). Image quality performance for all Varian HyperSight CBCT vendor-provided imaging protocols were assessed using ACR head and body ring CT phantoms, then compared to existing imaging modalities. Image quality analysis metrics included contrast-to-noise (CNR), spatial resolution, Hounsfield number (HU) accuracy, image scaling, and uniformity. All image quality assessments were made following the recommendations and passing criteria provided by the ACR. The Varian HyperSight CBCT imaging system demonstrated excellent image quality, with the majority of vendor-provided imaging protocols capable of passing all ACR CT accreditation standards. Nearly all (8/11) vendor-provided protocols passed ACR criteria using the ACR head phantom, with the Abdomen Large, Pelvis Large, and H&N vendor-provided protocols produced HU uniformity values slightly exceeding passing criteria but remained within the allowable minor deviation levels (5-7 HU maximum differences). Compared to other existing CT and CBCT imaging modalities, both HyperSight Head and Pelvis imaging protocols matched the performance of the SOMATOM CT scanner, and both the HyperSight and SOMATOM CT substantially surpassed the performance of the Halcyon 2.0 and TrueBeam version 2.7 systems. Varian HyperSight CBCT imaging system could pass almost all tests for all vendor-provided protocols using ACR accreditation criteria, with image quality similar to those produced by diagnostic CT scanners and significantly better than existing linac-based CBCT imaging systems.

A simple and precise alignment calibration method for cone-beam computed tomography with the verifications

Cone-beam computed tomography (CBCT) is widely used in dental imaging, small animal imaging, radiotherapy, and non-destructive industrial inspection. The quality of CBCT images depends on the precise knowledge of the CBCT system’s alignment. We introduce a distinct procedure, ‘precision alignment loop (PAL)’, to calibrate any CBCT system with a circular trajectory. We describe the calibration procedure by using a line-beads phantom, and how PAL determines the misalignments from a CBCT system. PAL also yields the uncertainties in the simulated calibration to give an estimate of the errors in the misalignments. From the analytical simulations, PAL can precisely obtain the source-to-rotation axis distance, and the geometric center G, ‘the point in z-axis meets the detector’, where the z-axis is coincident with the line from the x-ray source that intersects the axis of the rotation orthogonally. The uncertainties of three misalignment angles of the detector are within ±0.05°, which is close to ±0.04° for the results of Yang et al (2017 Med. Phys. 44 1692), but our method is easy and simple to implement. Our distinct procedure, on the other hand, yields the calibration of a micro-CT system and an example of reconstructed images, showing our calibration method for the CBCT system to be simple, precise, and accurate.

Swin-transformer-enhanced detector z-axis virtual alignment method for cone-beam CT system

ABSTRACT X-ray cone beam computed tomography (CBCT) is a common imaging tool in medical and industrial fields due to its non-invasive and efficient approach to 3D internal imaging. However, obtaining clear cross-section image with CBCT requires precise geometry alignment. Previous alignment methods have mostly relied on specially designed phantoms and/or the iterative optimisation of reconstructed images, both of which are costly and inefficient. This paper presented a Swin-Transformer-enhanced detector z-axis virtual geometry alignment method, which considered the tilt angle (around the central column of the detector) and the horizontal deviation of the centre of projection. The method took advantage of both cross-section and projection domains to virtually restore the geometric misalignments with the help of mathematical models. The maximum prediction error of the tilt angle was 0.0696°, and the single prediction time was about 0.0395 s. To the best of our knowledge, this work was the first study to detect the multiple geometric errors from the slice using deep learning method. It provided a new geometric alignment method for the CBCT system.

Objective image quality assurance in cone-beam CT: Test methods, analysis, and workflow in longitudinal studies.

Standards for image quality evaluation in multi-detector CT (MDCT) and cone-beam CT (CBCT) are evolving to keep pace with technological advances. A clear need is emerging for methods that facilitate rigorous quality assurance (QA) with up-to-date metrology and streamlined workflow suitable to a range of MDCT and CBCT systems. To evaluate the feasibility and workflow associated with image quality (IQ) assessment in longitudinal studies for MDCT and CBCT with a single test phantom and semiautomated analysis of objective, quantitative IQ metrology. A test phantom (CorgiTM Phantom, The Phantom Lab, Greenwich, New York, USA) was used in monthly IQ testing over the course of 1 year for three MDCT scanners (one of which presented helical and volumetric scan modes) and four CBCT scanners. Semiautomated software analyzed image uniformity, linearity, contrast, noise, contrast-to-noise ratio (CNR), 3D noise-power spectrum (NPS), modulation transfer function (MTF) in axial and oblique directions, and cone-beam artifact magnitude. The workflow was evaluated using methods adapted from systems/industrial engineering, including value stream process modeling (VSPM), standard work layout (SWL), and standard work control charts (SWCT) to quantify and optimize test methodology in routine practice. The completeness and consistency of DICOM data from each system was also evaluated. Quantitative IQ metrology provided valuable insight in longitudinal quality assurance (QA), with metrics such as NPS and MTF providing insight on root cause for various forms of system failure-for example, detector calibration and geometric calibration. Monthly constancy testing showed variations in IQ test metrics owing to system performance as well as phantom setup and provided initial estimates of upper and lower control limits appropriate to QA action levels. Rigorous evaluation of QA workflow identified methods to reduce total cycle time to ∼10min for each system-viz., use of a single phantom configuration appropriate to all scanners and Head or Body scan protocols. Numerous gaps in the completeness and consistency of DICOM data were observed for CBCT systems. An IQ phantom and test methodology was found to be suitable to QA of MDCT and CBCT systems with streamlined workflow appropriate to busy clinical settings.

Super Resolution Dual-Energy Cone-Beam CT Imaging With Dual-Layer Flat-Panel Detector.

In flat-panel detector (FPD) based cone-beam computed tomography (CBCT) imaging, the native receptor array is usually binned into a smaller matrix size. By doing so, the signal readout speed could be increased by 4-9 times at the expense of a spatial resolution loss of 50%-67%. Clearly, such manipulation poses a key bottleneck in generating high spatial and high temporal resolution CBCT images at the same time. In addition, the conventional FPD is also difficult in generating dual-energy CBCT images. In this paper, we propose an innovative super resolution dual-energy CBCT imaging method, named as suRi, based on dual-layer FPD (DL-FPD) to overcome these aforementioned difficulties at once. With suRi, specifically, a 1D or 2D sub-pixel (half pixel in this study) shifted binning is applied instead of the conventionally aligned binning to double the spatial sampling rate during the dual-energy data acquisition. As a result, the suRi approach provides a new strategy to enable high spatial resolution CBCT imaging while at high readout speed. Moreover, a penalized likelihood material decomposition algorithm is developed to directly reconstruct the high resolution bases from these dual-energy CBCT projections containing sub-pixel shifts. Numerical and physical experiments are performed to validate this newly developed suRi method with phantoms and biological specimen. Results demonstrate that suRi can significantly improve the spatial resolution of the CBCT image. We believe this developed suRi method would greatly enhance the imaging performance of the DL-FPD based dual-energy CBCT systems in future.

Reduction of cone-beam CT artifacts in a robotic CBCT device using saddle trajectories with integrated infrared tracking.

Cone beam computed tomography (CBCT) is widely used in many medical fields. However, conventional CBCT circular scans suffer from cone beam (CB) artifacts that limit the quality and reliability of the reconstructed images due to incomplete data. Saddle trajectories in theory might be able to improve the CBCT image quality by providing a larger region with complete data. Therefore, we investigated the feasibility and performance of saddle trajectory CBCT scans and compared them to circular trajectoryscans. We performed circular and saddle trajectory scans using a novel robotic CBCT scanner (Mobile ImagingRing (IRm); medPhoton, Salzburg, Austria). For the saddle trajectory, the gantry executed yaw motion up to using motorized wheels driving on the floor. An infrared (IR) tracking device with reflective markers was used for online geometric calibration correction (mainly floor unevenness). All images were reconstructed using penalized least-squares minimization with the conjugate gradient algorithm from RTK with voxel size. A disk phantom and an Alderson phantom were scanned to assess the image quality. Results were correlated with the local incompleteness value represented by , which was calculated at each voxel as a function of the source trajectory and the voxel's 3D coordinates. We assessed the magnitude of CB artifacts using the full width half maximum (FWHM) of each disk profile in the axial center of the reconstructed images. Spatial resolution was also quantified by the modulation transfer function at 10% (MTF10). When using the saddle trajectory, the region without CB artifacts was increased from 43 to 190mm in the SI direction compared to the circular trajectory. This region coincided with low values for . When was larger than 0.02, we found there was a linear relationship between the FWHM and . For the saddle, IR tracking allowed the increase of MTF10 from 0.37 to 0.98 lp/mm. We achieved saddle trajectory CBCT scans with a novel CBCT system combined with IR tracking. The results show that the saddle trajectory provides a larger region with reliable reconstruction compared to the circular trajectory. The proposed method can be used to evaluate other non-circulartrajectories.

High-density objects in exomass affect the volume of high-density objects inside the field of view.

To evaluate the effect of the presence and the number of high-density objects in the exomass on the volume of a high-density object in cone-beam CT (CBCT). Cylinders of cobalt-chromium (Co-Cr), titanium (Ti), and zirconium (Zi) were inserted into a polymethylmethacrylate phantom in five different combinations of number and position: 1-no cylinder; 2-one cylinder in a posterior region; 3-one cylinder in an anterior region; 4-two cylinders in posterior regions; and 5-three cylinders in anterior and posterior regions. The phantom underwent CBCT scanning using OP300 and X800 systems, with the afore mentioned cylinders of the same composition placed in the exomass and an additional high-density cylinder placed in the centre of the field of view (FOV), corresponding to the left-anterior region. The tomographic volume of the cylinder inside the FOV was measured using semi-automatic segmentation. The volumetric alteration (VA) between the segmented and physical volumes, in percentage, was compared among the experimental groups using repeated measures ANOVA and Tukey post-hoc (α = 5%). The factors material, combination, and their interaction affected the volume or both CBCT systems. In OP300, more cylinders in the exomass reduced the VA, mainly for Co-Cr. In X800, more cylinders in the exomass tended to increase the VA inside the FOV, except for Zi. In general, the presence of high-density objects in the exomass influences the VA of the object inside the FOV, although this oscillates according to object composition, number and position in the exomass, and CBCT system.

Joint denoising and interpolating network for low-dose cone-beam CT reconstruction under hybrid dose-reduction strategy

Cone-beam computed tomography (CBCT) is generally reconstructed with hundreds of two-dimensional X-Ray projections through the FDK algorithm, and its excessive ionizing radiation of X-Ray may impair patients’ health. Two common dose-reduction strategies are to either lower the intensity of X-Ray, i.e., low-intensity CBCT, or reduce the number of projections, i.e., sparse-view CBCT. Existing efforts improve the low-dose CBCT images only under a single dose-reduction strategy. In this paper, we argue that applying the two strategies simultaneously can reduce dose in a gentle manner and avoid the extreme degradation of the projection data in a single dose-reduction strategy, especially under ultra-low-dose situations. Therefore, we develop a Joint Denoising and Interpolating Network (JDINet) in projection domain to improve the CBCT quality with the hybrid low-intensity and sparse-view projections. Specifically, JDINet mainly includes two important components, i.e., denoising module and interpolating module, to respectively suppress the noise caused by the low-intensity strategy and interpolate the missing projections caused by the sparse-view strategy. Because FDK actually utilizes the projection information after ramp-filtering, we develop a filtered structural similarity constraint to help JDINet focus on the reconstruction-required information. Afterward, we employ a Postprocessing Network (PostNet) in the reconstruction domain to refine the CBCT images that are reconstructed with denoised and interpolated projections. In general, a complete CBCT reconstruction framework is built with JDINet, FDK, and PostNet. Experiments demonstrate that our framework decreases RMSE by approximately 8 %, 15 %, and 17 %, respectively, on the 1/8, 1/16, and 1/32 dose data, compared to the latest methods. In conclusion, our learning-based framework can be deeply imbedded into the CBCT systems to promote the development of CBCT. Full 3-D modulation transfer function estimation of tomosynthesis system using modified Richardson-Lucy deconvolution.

Tomosynthesis systems are three-dimensional (3-D) medical imaging devices that operate over limited acquisition angles using low radiation dosages. To measure the spatial resolution performance of a tomosynthesis system, the modulation transfer function (MTF) is widely used as a quantitative evaluationmetric. We previously introduced a method to estimate the full 3-D MTF of a cone-beam computed tomography system using two-dimensional (2-D) Richardson-Lucy (RL) deconvolution with Tikhonov-Miller regularization. However, this method can not be applied directly to estimate the 3-D MTF of a tomosynthesis system, since the unique artifacts (i.e., shadow artifacts, spreading tails, directional blurring, and high-level noise) of the system produce several errors that lower the estimation performance. Varying positions of the negative pixels due to shadow artifacts and spreading tails cause inconsistent deconvolution performances at each of the directional projections, and the severe noise in the reconstructed images cause noise amplification during estimation. This work proposes several modifications to the previous method to resolve the inconsistent performance and noise amplification errors to increase the full 3-D MTF estimationaccuracy. Three modifications were introduced to the 2-D RL deconvolution to prevent estimation errors and improve MTF estimation performance: non-negativity relaxation function, cost function to terminate the iterative process of RL deconvolution, and regularization strength for noise control. To validate the effectiveness of the proposed modifications, we reconstructed sphere phantoms from simulation and experimental tomosynthesis studies in the iso-center and offset-center positions as well as estimated the full 3-D MTFs using the previous and proposed methods. We compared the 3-D render images, central plane images, and center profiles of the estimated 3-D MTFs and calculated the full widths at half and tenth maximum for quantitativeevaluation. The previous method cannot estimate the full 3-D MTF of a tomosynthesis system; its inaccurate negative pixel relaxation produces circular-shaped errors, and the mean squared error based simple cost function for termination causes inconsistent estimation at each directional projection to diminish the clear edges of the low-frequency drop and missing sample regions. Noise amplification from lack of noise regularization is also seen in the previous method results. Compared to the previous method, the proposed method shows superior estimation performance at reducing errors in both the simulation and experimental studies regardless of object position. The proposed method preserves the low-frequency drop, missing sample regions from the limited acquisition angles, and missing cone region from the offset-center position; the estimated MTFs also show FWHM and FWTM values close to those of the ideal MTFs than with the previousmethod. This work presents a method to estimate the full 3-D MTF of a tomosynthesis system. The proposed modifications prevent circular-shaped errors and noise amplification due to the geometry for limited acquisition angles and high noise levels. Compared to our previous method, the proposed scheme show better performance for estimating the 3-D MTF of the tomosynthesissystem.

A simulation study to evaluate the effect of 2D antiscatter grid primary transmission on flat panel detector based CBCT image quality

Abstact Purpose. Two-dimensional antiscatter grids’ (2D-ASGs) septal shadows and their impact on primary transmission play a critical role in cone-beam computed tomography (CBCT) image noise and artifact characteristics. Therefore, a numerical simulation platform was developed to evaluate the effect of 2D-ASG’s primary transmission on image quality, as a function of grid geometry and CBCT system properties. Methods. To study the effect of 2D-ASG’s septal shadows on primary transmission and CBCT image quality, two new methods were introduced; one to simulate projection signal gradients in septal shadows, and the other to simulate septal shadow variations due to gantry flex. Signal gradients in septal shadows were simulated by generating a system point spread function that was directly extracted from projection images of 2D-ASG prototypes in experiments. Variations in septal shadows due to gantry flex were simulated by generating oversampled shadow profiles extracted from experiments. Subsequently, the effect of 2D-ASG’s septal shadows on primary transmission and image quality was evaluated. Results. For an apparent septal thickness of 0.15 mm, the primary transmission of 2D-ASG varied between 72%–90% for grid pitches 1–3 mm. In low-contrast phantoms, the effect of 2D-ASG’s radiopaque footprint on information loss was subtle. At high spatial frequencies, information loss manifested itself as undersampling artifacts, however, its impact on image quality is subtle when compared to quantum noise. Effects of additive electronic noise and gantry flex induced ring artifacts on image quality varied as a function of grid pitch and septal thickness. Such artifacts were substantially less in lower resolution images. Conclusion. The proposed simulation platform allowed successful evaluation of CBCT image quality variations as a function of 2D-ASG primary transmission properties and CBCT system characteristics. This platform can be potentially used for optimizing 2D-ASG design properties based on the imaging task and properties of the CBCT system.

Proton Online Adaptation Using Novel Cone-Beam Computed Tomography System

Cone beam computed tomography (CBCT) has been used in clinic frequently to provide quick, online, and handy three-dimensional images. However, due to its significant artifacts and inaccurate Hounsfield values (HU), CBCT based online proton adaptation is infeasible. Recently, a novel in-room imaging solution was developed to allow larger images, better contrast and faster CBCT imaging acquisition. Our work demonstrated for the first time the feasibility of using this novel CBCT images for direct intensity modulated proton planning. Three patients and three CIRS phantoms were scanned using novel imaging technique (CBCTp) for this work. CT curves were acquired by scanning a CIRS electron density phantom. Stopping power ratio was computed using the stoichiometric method. Proton plans were made on treatment simulation CT and further evaluated on CBCTp to compare the differences. The table below shows three patients and three phantoms plan comparisons. Multiple sites and dose levels were studied. The planning target volume (PTV) coverage (D95%) and mean dose difference between simulation CT and CBCTp are 0.8% and 0.3%, correspondingly. Dosimetrically, phantom and patient plans are almost identical between two imaging techniques. Lung patient plan show the largest variation due to patient tumor change and the quality of breathing. The novel and high quality of CBCT is feasible for direct proton planning with accurate CT calibration. It provides a reliable alternative solution for proton online adaptive therapy.

Implementation and evaluation of the AAPM TG 111 for radiation dosimetry in a state-of-the-art C-arm cone beam CT system

The aim of this study was to assess the dosimetric characteristics of a state-of-the-art C-arm cone beam computed tomography (CBCT) system using the methodology proposed by the American Association of Physicists in Medicine (AAPM) Task Group (TG) 111. The dose measurement methodology described in AAPM TG 111 for wide cone beam acquisitions without table translation was employed to estimate equilibrium beam length () and equilibrium dose () in various interventional task-specific protocols with different tube arc projection geometries. Dose profiles were derived from point dose measurements in the centre and peripheral locations of the ICRU/AAPM and standard polymethyl methacrylate (PMMA) body phantom. Dose measurements were performed in phantom and free-in-air using a solid-state point detector. Monte Carlo (MC) based simulation dosimetry was performed to quantify the inhomogeneous dose patterns imparted in the phantoms. Estimated and on the ICRU/AAPM phantom was up to 49.4 cm and 6.17 mGy/100 mAs, respectively. Corresponding values determined on the PMMA phantom were 139 cm and 8.8 mGy/100 mAs, respectively. Free-in-air dose measurement ranged from 1.43 mGy/100 mAs to 5.93 mGy/100 mAs. Per cent difference in and between MC simulation and solid-state point detector measurement methods in the ICRU/AAPM phantom were within 16% and 18%, respectively. Manufacturers can use the presented methodology to characterize the dosimetric properties of C-arm CBCT systems. Clinical medical physicists may follow this methodology to verify corresponding data provided by the manufacturer and check for C-arm CBCT system performance dosimetric consistency.

Comparison of intraoperative CT- and cone beam CT-based spinal navigation for the treatment of atlantoaxial instability

BACKGROUND CONTEXTDue to the complexity of neurovascular structures in the atlantoaxial region, spinal navigation for posterior C1–C2 instrumentation is nowadays a helpful tool to increase accuracy of surgery and safety of patients. Many available intraoperative navigation devices have proven their reliability in this part of the spine. Two main imaging techniques are used: intraoperative CT (iCT) and cone beam computed tomography (CBCT). PURPOSEComparison of iCT- and CBCT-based technologies for navigated posterior instrumentation in C1-C2 instability. STUDY DESIGNRetrospective study. PATIENT SAMPLEA total of 81 consecutive patients from July 2014 to April 2020. OUTCOME MEASURESScrew accuracy and operating time. METHODSPatients with C1–C2 instability received posterior instrumentation using C2 pedicle screws, C1 lateral mass or pedicle screws. All screws were inserted using intraoperative imaging either using iCT or CBCT systems and spinal navigation with autoregistration technology. Following navigated screw insertion, a second intraoperative scan was performed to assess the accuracy of screw placement. Accuracy was defined as the percentage of correctly placed screws or with minor cortical breach (<2 mm) as graded by an independent observer compared to misplaced screws. RESULTSA total of 81 patients with C1–C2 instability were retrospectively analyzed. Of these, 34 patients were operated with the use of iCT and 47 with CBCT. No significant demographic difference was found between groups. In the iCT group, 97.7% of the C1–C2 screws were correctly inserted; 2.3% showed a minor cortical breach (<2 mm); no misplacement (>2 mm). In the CBCT group, 98.9% of screws were correctly inserted; no minor pedicle breach; 1.1% showed misplacement >2 mm. Accuracy of screw placement demonstrated no significant difference between groups. Both technologies allowed sufficient identification of screw misplacement intraoperatively leading to two screw revisions in the iCT and three in the CBCT group. Median time of surgery was significantly shorter using CBCT technology (166.5 minutes [iCT] vs 122 minutes [CBCT]; p<.01). CONCLUSIONSSpinal navigation using either iCT- or CBCT-based systems with autoregistration allows safe and reliable screw placement and intraoperative assessment of screw positioning. Using the herein presented procedural protocols, CBCT systems allow shorter operating time.

A method for sensitivity analysis of deviation factor for geometric correction of cone-beam CT system

To propose a sensitivity test method for geometric correction position deviation of cone-beam CT systems. We proposed the definition of center deviation and its derivation. We analyzed the influence of the variation of the three-dimensional spatial center of the steel ball point, the projection center and the size of the steel ball point on the deviation of geometric parameters and the reconstructed image results by calculating the geometric correction parameters based on the Noo analytical method using the FDK reconstruction algorithm for image reconstruction. The radius of the steel ball point was within 3 mm. The deviation of the center of the calibration parameter was within the order of magnitude and negligible. A 10% Gaussian perturbation of a single pixel in the 3D spatial coordinates of the steel ball point produced a deviation of about 3 pixel sizes, while the same Gaussian perturbation of the 2D projection coordinates of the steel ball point produced a deviation of about 2 pixel sizes. The geometric correction is more sensitive to the deviation generated by the three-dimensional spatial coordinates of the steel ball point with limited sensitivity to the deviation generated by the two-dimensional projection coordinates of the steel ball point. The deviation sensitivity of a small diameter steel ball point can be ignored.
