Influence of deep learning image reconstruction algorithm for reducing radiation dose and image noise compared to iterative reconstruction and filtered back projection for head and chest computed tomography examinations: a systematic review.

The most recent advances in Computed Tomography (CT) image reconstruction technology are Deep learning image reconstruction (DLIR) algorithms. Due to drawbacks in Iterative reconstruction (IR) techniques such as negative image texture and nonlinear spatial resolutions, DLIRs are gradually replacing them. However, the potential use of DLIR in Head and Chest CT has to be examined further. Hence, the purpose of the study is to review the influence of DLIR on Radiation dose (RD), Image noise (IN), and outcomes of the studies compared with IR and FBP in Head and Chest CT examinations. We performed a detailed search in PubMed, Scopus, Web of Science, Cochrane Library, and Embase to find the articles reported using DLIR for Head and Chest CT examinations between 2017 to 2023. Data were retrieved from the short-listed studies using Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA) guidelines. Out of 196 articles searched, 15 articles were included. A total of 1292 sample size was included. 14 articles were rated as high and 1 article as moderate quality. All studies compared DLIR to IR techniques. 5 studies compared DLIR with IR and FBP. The review showed that DLIR improved IQ, and reduced RD and IN for CT Head and Chest examinations. DLIR algorithm have demonstrated a noted enhancement in IQ with reduced IN for CT Head and Chest examinations at lower dose compared with IR and FBP. DLIR showed potential for enhancing patient care by reducing radiation risks and increasing diagnostic accuracy.

Advancing pediatric head CT dose prediction: correlating patient size metrics with AP and LAT dimensions in Moroccan population

This study aims to establish comprehensive relationships between patient size, focusing on effective diameter (Deff) and water-equivalent diameter (Dw), in conjunction with anterior-posterior (AP) and lateral (LAT) dimensions. The primary goal is to refine precise patient dose estimations in pediatric head computed tomography (CT) examinations. Our examination involved a dataset of 105 pediatric head CT images sourced from a Moroccan pediatric university hospital. These images, selected from the central scanning range, underwent quantification for LAT, AP, Deff, and Dw, with measurements carried out using RadiAnt Dicom Viewer for lateral and anteroposterior dimensions. Our analytical approach encompassed modeling Deff and Dw as functions of LAT, AP, and combined AP + LAT dimensions. Additionally, Dw was modeled as a function of Deff and calculated Size Specific Dose Estimates (SSDE) based on Dw was compared to SSDE computed from one single dimension. The outcomes revealed strong correlations among Deff, Dw, and fundamental geometric dimensions (LAT, AP, and AP + LAT), as indicated by consistently high coefficients of determination (R² > 0.8). SSDE calculated from Dw shows impressive correlation with SSDE computed from a single dimension (R² > 0.9). This study offers insights into the relationships between patient size and critical dimensions, identifying high correlations that can represent a significant advancement in predicting doses for pediatric head CT examinations. This introduces the potential for streamlining the determination of head diameter and dose by relying on a single dimension, thereby enhancing the efficiency of dose predictions by facilitating the monitoring of radiation exposure of this population in clinical routine.

Introducing fitting models for estimating age-specific dose and effective dose in paediatric patients undergoing head, chest and abdomen-pelvis imaging protocols: a patient study.

Concerns regarding the adverse consequences of radiation have increased due to the expanded application of computed tomography (CT) in medical practice. Certain studies have indicated that the radiation dosage depends on the anatomical region, the imaging technique employed and patient-specific variables. The aim of this study is to present fitting models for the estimation of age-specific dose estimates (ASDE), in the same direction of size-specific dose estimates, and effective doses based on patient age, gender and the type of CT examination used in paediatric head, chest and abdomen-pelvis imaging. A total of 583 paediatric patients were included in the study. Radiometric data were gathered from DICOM files. The patients were categorised into five distinct groups (under 15 years of age), and the effective dose, organ dose and ASDE were computed for the CT examinations involving the head, chest and abdomen-pelvis. Finally, the best fitting models were presented for estimation of ASDE and effective doses based on patient age, gender and the type of examination. The ASDE in head, chest, and abdomen-pelvis CT examinations increases with increasing age. As age increases, the effective dose in head and abdomen-pelvis CT scans decreased. However, for chest scans, the effective dose initially showed a decreasing trend until the first year of life; after that, it increases in correlation with age. Based on the presented fitting model for the ASDE, these CT scan quantities depend on factors such as patient age and the type of CT examination. For the effective dose, the gender was also included in the fitting model. By utilising the information about the scan type, region and age, it becomes feasible to estimate the ASDE and effective dose using the models provided in this study.

Evaluation of organ and effective doses using anthropomorphic phantom: A comparison between experimental measurement and a commercial dose calculator

IntroductionThe aim of this study was to experimentally measure organ doses for computed tomography (CT) procedures using thermoluminescence dosimeters (TLDs) on a RANDO anthropomorphic phantom and verify the measured doses using CT-Expo software. MethodsThe phantom was irradiated using clinical CT scan protocols routinely used for specific procedures in the radiology department. Fifty TLD chips were used in this study. The scanning parameters (kVp, mA, s) used to scan the phantom were used as input parameters for CT-Expo dose estimations. ResultsThe TLD measured organ doses varied between 3.97 mGy for the esophagus and 56.22 mGy for the brain. High doses were recorded in the brain (37.80–56.22 mGy) and the eye lens (29.94–36.16 mGy). Comparing the organ dose measurements between TLD and CT-Expo, the maximum organ dose difference was obtained for the eye lens. A comparison between the two methods for the other organs were all less than 32 %. The effective doses from the TLD measurements for the head, chest, and abdominopelvic CT examinations were 2.78, 6.67, and 17 mSv, respectively and CT-Expo were 2.20, 10.30, and 16.70 mSv, respectively. ConclusionThe experimental and computational results are comparable, and the reliability of the TLD measurements and CT-Expo dose calculator has been proven. Implications for studyA reason for the difference in dose measurements between the two methods has been attributed to the dissimilarity in the organ position in the Rando anthropomorphic phantom and the standard mathematical phantom used by CT-Expo. The experimental and computational results have been found to be comparable.

Image Quality Improvement in Deep Learning Image Reconstruction of Head Computed Tomography Examination.

In this study, we assess image quality in computed tomography scans reconstructed via DLIR (Deep Learning Image Reconstruction) and compare it with iterative reconstruction ASIR-V (Adaptive Statistical Iterative Reconstruction) in CT (computed tomography) scans of the head. The CT scans of 109 patients were subjected to both objective and subjective evaluation of image quality. The objective evaluation was based on the SNR (signal-to-noise ratio) and CNR (contrast-to-noise ratio) of the brain's gray and white matter. The regions of interest for our study were set in the BGA (basal ganglia area) and PCF (posterior cranial fossa). Simultaneously, a subjective assessment of image quality, based on brain structure visibility, was conducted by experienced radiologists. In the assessed scans, we obtained up to a 54% increase in SNR for gray matter and a 60% increase for white matter using DLIR in comparison to ASIR-V. Moreover, we achieved a CNR increment of 58% in the BGA structures and 50% in the PCF. In the subjective assessment of the obtained images, DLIR had a mean rating score of 2.8, compared to the mean score of 2.6 for ASIR-V images. In conclusion, DLIR shows improved image quality compared to the standard iterative reconstruction of CT images of the head.

Correlation Between Optic Nerve Sheath Diameter at Initial Head CT and the Rotterdam CT Score.

Introduction Intracranial findings on imaging have long been used in assessing the severity of traumatic brain injury (TBI); the Rotterdam CT scoring (RCTS) is a more recent tool. Estimating the optic nerve sheath diameter (ONSD) at computed tomography (CT) can be another valuable predictor of the severity of the injury, especially as both ONSD and the RCTS are proven to be independent predictors of raised intracranial pressure (ICP).The study objective was to determine the correlation between ONSD at initial head CT and RCTS. Material and methods We observed 40 consecutive confirmed TBI cases at their initial head CT examinations in the emergency department for ONSD and the presence of other intracranial findings necessary to derive RCTS. The data were prospectively collected and analyzed, with statistical significance set at p ≤0.05 at 95% CI. Results The mean ONSD positively correlated with the Rotterdam CT score (r=0.368, p=0.019). A cut-off value of 6.83 mm was extrapolated from the receiver operator characteristic (ROC) curve as the mean binocular ONSD that best predicted severe RCTS (≥4) (sensitivity: 73.3%, specificity: 80%, positive predictive value: 68.7%, negative predictive value: 83.3%). The area under the curve (AUC) was 0.780 (p=0.003). Binary logistic regression analysis revealed an odd ratio (OR) of 11.000(95% CI: 2.438-49.627; p=0.002). Conclusion TBI patients with high RCTS have wide mean binocular ONSD. Those with average binocular ONSD above the cut-off value are likelier to have severe TBI. With the documented good correlation, ONSDmay become very useful in informing the clinical decision for sequential CT scans in TBI patients and, therefore, reducing the cumulative radiation burden from needless exposures. Furthermore, the non-invasive nature of its assessment will have more clinical relevance in resource-limited settings, where the skills and equipment for ICP monitoring are either not readily available or too expensive to be used routinely.

Organ doses, effective dose, and cancer risk estimation from head and neck CT scans

PurposeThe goal of this study was to calculate the organ doses, effective doses, and cancer risk from head and neck computed tomography (CT) scans. Material and methodsAll patients underwent topogram, base, and cerebrum sequences during their head and neck examinations. Two techniques were used to determine the effective dose (ED). The first was based on the scanner derived dose length product, while the second involved using the software that calculated the organ and effective doses. Organ doses were computed using the tissue weighting factors from Report 103 by the International Commission on Radiation Protection (ICRP). The cancer risk values were calculated using web-based software that is based on the BEIR VII Phase 2 report. ResultsThere were 293 patients in this study (189 males, 104 females). The overall mean effective dose in the females was higher than that in the males (2.19 vs. 2.06 mSv). Male patients had a mean cancer risk of 0.011%, whereas female patients had a mean cancer risk of 0.015%. The younger females had much higher cancer risk values. ConclusionThe findings of this study will assist physicians in justifying and optimizing dose administration of head and neck CT examinations by weighing the benefits of diagnosis on the one hand and awareness of cancer risk on the other.

Development of a Deep Learning Model for Retinal Hemorrhage Detection on Head Computed Tomography in Young Children

Abusive head trauma (AHT) in children is often missed in medical encounters, and retinal hemorrhage (RH) is considered strong evidence for AHT. Although head computed tomography (CT) is obtained routinely, all but exceptionally large RHs are undetectable on CT images in children. To examine whether deep learning-based image analysis can detect RH on pediatric head CT. This diagnostic study included 301 patients diagnosed with AHT who underwent head CT and dilated fundoscopic examinations at a quaternary care children's hospital. The study assessed a deep learning model using axial slices from 218 segmented globes with RH and 384 globes without RH between May 1, 2007, and March 31, 2021. Two additional light gradient boosting machine (GBM) models were assessed: one that used demographic characteristics and common brain findings in AHT and another that combined the deep learning model's risk prediction plus the same demographic characteristics and brain findings. Sensitivity (recall), specificity, precision, accuracy, F1 score, and area under the curve (AUC) for each model predicting the presence or absence of RH in globes were assessed. Globe regions that influenced the deep learning model predictions were visualized in saliency maps. The contributions of demographic and standard CT features were assessed by Shapley additive explanation. The final study population included 301 patients (187 [62.1%] male; median [range] age, 4.6 [0.1-35.8] months). A total of 120 patients (39.9%) had RH on fundoscopic examinations. The deep learning model performed as follows: sensitivity, 79.6%; specificity, 79.2%; positive predictive value (precision), 68.6%; negative predictive value, 87.1%; accuracy, 79.3%; F1 score, 73.7%; and AUC, 0.83 (95% CI, 0.75-0.91). The AUCs were 0.80 (95% CI, 0.69-0.91) for the general light GBM model and 0.86 (95% CI, 0.79-0.93) for the combined light GBM model. Sensitivities of all models were similar, whereas the specificities of the deep learning and combined light GBM models were higher than those of the light GBM model. The findings of this diagnostic study indicate that a deep learning-based image analysis of globes on pediatric head CTs can predict the presence of RH. After prospective external validation, a deep learning model incorporated into CT image analysis software could calibrate clinical suspicion for AHT and provide decision support for which patients urgently need fundoscopic examinations.

Usefulness of the superior orbitomeatal line without the lens included in the scan range at different tube voltage during pediatric head CT

Introduction and objectivesTo compare the real time skin dosimeter values at lens between with the lens included in the scan range (orbitomeatal base line [OML]) or without the lens included in the scan range (superior orbitomeatal line [SOML]) at different tube voltages. Materials and methodsWe used three pediatric anthropomorphic phantoms with a 64 detector-row computed tomography (CT) scanner with the OML- or SOML-protocol at different tube voltages during the head CT. A real time skin dosimeter was inserted into the phantom center of the head, and surfaces of the lens. We compared the real time skin dosimeter values at lens between the OML- and SOML-protocol. ResultsThere were no significant differences in the real time skin dosimeter values for the head in the scan area for each phantom at different tube voltages between the OML- and SOML-protocol (p > 0.05 for all phantom). Compared with the OML protocol, it is possible to reduce the real time skin dosimeter values at lens by approximately 80% by using the SOML protocol (p < 0.05) at all tube voltages. Compared with the OML protocol, it is possible to reduce the real time skin dosimeter values at mammary gland by approximately 20% by using the SOML protocol (p < 0.05) at all tube voltages. ConclusionsDuring the pediatric head CT examination, SOML protocol was possible to reduce the real time skin dosimeter values at lens by approximately 80% compared with OML protocol at all tube voltages.

Applying the AAPM 293 report to estimate the absorbed dose during head computed tomography: using head circumference for rapid dose estimation.

The American Association of Physicists in Medicine (AAPM) report 293 is more accurate than report 220 in evaluating the absorbed radiation dose during head computed tomography (CT) examination. We aimed to investigate the associations between age, head circumference (HC), the conversion factor (f293), and specific-size dose estimation (SSDE293) during these procedures. The rapid radiation dose was also estimated based on the AAPM report 293. In this retrospective, cross-sectional study, unenhanced CT images of the head were retrospectively collected from 1,222 participants from Union Hospital and Hubei Cancer Hospital between December 2018 and September 2019. Scan parameters, including age, HC, water-equivalent diameter (DW), and volumetric computed tomography dose index (CTDIvol), were generated automatically using indigenously-developed image processing software. The corresponding f293 and SSDE293 were calculated according to the AAPM report 293. The analyses were performed using linear regression. In the younger group, age and HC were significantly negatively correlated with SSDE293 (r=-0.33 and -0.44, respectively; both P values ≤0.001). No significant correlation was reported between age, HC, and SSDE293 in the older group. Moreover, age was significantly negatively associated with f293 in the younger and older groups (r=-0.80 and -0.13, respectively; both P values ≤0.001). A significantly negative association was seen between f293 and increased HC in both age groups (r=-0.92 and -0.82, respectively; both P values ≤0.001). The HC of patients was associated with head conversion. HC is a feasible indicator for rapidly estimating the radiation dose in head CT examinations based on the AAPM report 293.

Spontaneous bilateral epidural hematomas caused by chronic sinusitis: illustrative case.

Spontaneous bilateral epidural hematomas (EDHs) are rare. The aim of this study was to report a 21-year-old male with spontaneous bilateral EDHs to discuss the pathogenesis of spontaneous bilateral EDHs caused by chronic sinusitis. A 21-year-old male with no history of head trauma was admitted to the hospital for headache and unconsciousness. The patient had bilateral nasal bleeding on the day before admission and had chronic sinusitis since childhood. The head computed tomography examination after admission showed bilateral EDHs and bilateral sinusitis, the head magnetic resonance imaging showed chronic sinusitis, and the endoscopic examination during surgery further confirmed that the patient had severe sinusitis with erosion of the bilateral nasal mucosae. The patient underwent emergent surgical treatment. The cerebral vascular malformation, autoimmune diseases, low intracranial pressure, blood system diseases (such as sickle cell disease), abnormal blood coagulation, and skull or meningeal lesions were all excluded after operation. Chronic sinusitis may lead to EDHs through causing vascular degeneration, and abruption of the dura mater and skull. For young patients with spontaneous EDHs, neurosurgeons should carefully ask patients whether they have a history of chronic sinusitis to exclude the possibility of bleeding caused by chronic sinusitis.

Feasibility of size-specific organ-dose estimates based on water equivalent diameter for common head CT examinations: a Monte Carlo study

Computed tomography dose index (CTDI) is an unreliable dose estimate outside of the standard CTDI phantom diameters (16 and 32 cm). Size-specific dose estimate (SSDE) for head computed tomography (CT) examination was studied in the American Association of Physicists in Medicine Report 293 to provide SSDE coefficient factors based on water equivalent diameter as size metrics. However, it is limited to one protocol and for a fully irradiated organ. This study aimed to evaluate the dependency of normalized organ dose (ND) on water equivalent diameter as a size metric in three common protocols: routine head, paranasal sinus, and temporal bone. CTDIw measurements were performed for outlined protocols in the Siemens Emotion 16-slice-configuration scanner. Geant4 Application for Tomographic Emission Monte Carlo simulation platform, coupled with ten GSF patient models, was used to estimate organ doses. CT scanner system was modeled. Helical CT scans were simulated using constructor scan parameters and calculated scan lengths of each patient model. Organ doses provided by simulations were normalized to CTDIvol. The water equivalent diameters (D w) of patient models were obtained via relationships between D w and both effective diameter for a sample of patients’ data.NDs received by fully, partially, and non-directly irradiated organs were then reported as a function of D w. For fully irradiated organs, brain (R 2 > 0.92), eyes (R 2 > 0.88), and eye lens (R 2 > 0.89) correlate well with D w. For the rest of the results, a poor correlation was observed. For partially irradiated organs, the exception was scalp (R 2 = 0.93) in temporal bone CT. For non-directly irradiated organs, the exception was thyroid (R 2 > 0.90) and lungs (R 2 > 0.91) in routine head CT. ND correlates well in routine head CT than other protocols. For the most part, no relationship seems to exist between R 2 and scan percentage coverage. The results have revealed additional factors that may influence the ND and D w relationship, which explains the need for more studies in the future to investigate the effect of scan conditions and organ anatomy variation.

Evaluation of radiation dose reduction in head CT using the half-dose method

PurposeThe present study introduced the half-dose method (HDM), which halves the radiation dose for conventional head computed tomography (CT), for postoperative hydrocephalus and follow-up for craniosynostosis at a children’s hospital. This study aimed to evaluate the contribution of selective head CT scanning optimization towards the overall reduction of radiation exposure.Materials and methodsWe retrospectively assessed 1227 and 1352 head CT examinations acquired before and after the introduction of the HDM, respectively, in children aged 0–15 years. The radiation exposure was evaluated using the CT dose index volume (CTDI-vol), dose-length product (DLP), rate of HDM introduction, and effect of reducing in-hospital radiation dose before and after the introduction of the HDM. For an objective evaluation of the image quality, head CT scans acquired with HDM and full-dose method (FDM) were randomly selected, and the image noise standard deviation (SD) was measured for each scan. In addition, some HDM images were randomly selected and independently reviewed by two radiologists.ResultsThe HDM was introduced in 27.9% of all head CTs. The mean CTDI-vol of all head CTs was 21.5 ± 6.9 mGy after the introduction, a 14.9% reduction. The mean DLP was 418.4 ± 152.9 mGy.cm after the introduction, a 17.2% reduction. Compared to the FDM images, the noise SD of the HDM ones worsened by almost 0.9; however, none of the images were difficult or impossible to evaluate.ConclusionThe HDM yielded diagnostically acceptable images. In addition, a change in protocol for only two diseases successfully reduced the patients’ overall radiation exposure by approximately 15%. Introducing and optimizing the HDM for frequently performed target diseases will be useful in reducing the exposure dose for the hospital’s patient population.

Development of size-specific dose estimates for common computed tomography examinations: a study in Ghana

This study determined the size-specific dose estimate (SSDE) of computed tomography (CT) examinations and derived mathematical expressions for dose output estimation and optimization in a teaching hospital in Ghana. Demographic and scanner output indices, including CT dose index (CTDIvol) and dose length product for adult head, chest and abdominopelvic (ABP) CT examinations carried out at the hospital from 2018 to 2020, were retrieved from the picture archiving and communication system of the CT scanner machine. Other indices such as the antero-posterior diameter (D AP), lateral diameter (D L) and diagonal diameter (D dia) of the patients’ bodies were measured on the mid-slice axial image using a digital caliper. The effective diameter (D eff) was then calculated as the square root of the product of the D AP and D L. The SSDEs were calculated as the product of the CTDIvol and the size-specific conversion factors obtained from Report 204 of the American Association of Physicists in Medicine. Regression analyses were performed to find the relationship between SSDE and the various parameters to derive mathematical equations for the dose estimations. There were more female samples (n= 468, 56.3%) than male samples (n = 364, 43.7%) for each CT procedure. The SSDEs and size-specific diagnostic reference levels (SSDRLs) were: head (83.9 mGy; 86.9 mGy), chest (8.1 mGy; 8.7 mGy) and ABP (8.4 mGy; 9.2 mGy). The variations between CTDIvol and SSDEs for head (2.50%), chest (25.9%), and ABP (26.2%) showed an underestimation of radiation dose to patients, especially in chest and ABP examinations, if CTDIvol is used to report patient doses. The SSDEs of the chest and ABP CT examinations showed linear correlations with the CTDIvol. The estimated values could be used to optimize radiation doses in the CT facility. The SSDE and SSDRLs for head, chest and ABP CT examinations have been developed at a teaching hospital in Ghana. The SSDEs of chest and ABP examinations showed linear correlations with the CTDIvol and hence can be calculated using the mathematically derived equations in the study.

Patient dose reduction for a localizer radiograph with an additional tin filter in chest-abdomen-pelvis, spine, and head computed tomography examinations.

This study aimed to evaluate the dose reduction potential of adding a tin filter to localizer radiographs (LR) on computed tomography (CT) examinations in both phantom and clinical studies. LRs were performed using combinations of 120 kVp and 20mA (120/20), 100 kVp with a tin filter, and 50mA or 20mA (Sn100/50, Sn100/20). For the phantom experiment, entrance surface doses (ESD) of the LRs were evaluated for each protocol using an anthropomorphic phantom. This retrospective clinical study included 700 patients (300 for chest-pelvis, 200 for spine, and 200 for head CTs). The volume CT dose indices (CTDIvols) of the main CT scans were recorded and placed into one of three groups based on body mass index (BMI): underweight, normal-weight, and overweight, to evaluate the effect of LR acquisition conditions on the performance of the automatic tube current modulation technique of subsequent CT scans. The ESDs of all LRs with the Sn100/50 protocol were 0.03mGy, a decrease of more than 80% compared to those of the 120/20 protocol. Moreover, the Sn100/20 protocol reduced ESD to 0.02mGy. In chest-pelvis CT, there were no significant differences in the CTDIvol between with and without a tin filter for each BMI group. However, the lateral LRs with the tin filter on the spine CT slightly reduced the CTDIvol in normal-weight and overweight patients. Although there is room to optimize the acquisition conditions for larger patients, an additional tin filter for LR is a useful means to efficiently reduce ESDs.

Establishment of local diagnostic reference levels for common adult CT examinations: a multicenter survey in Addis Ababa

BackgroundIn medical imaging, a computed tomography (CT) scanner is a major source of ionizing radiation. All medical radiation exposures should be justified and optimized to meet the clinical diagnosis. Thus, to avoid unnecessary radiation doses for patients, diagnostic reference levels (DRLs) have been used. The DRLs are used to identify unusually high radiation doses during CT procedures, which are not appropriate for the clinical diagnosis. It has been successfully implemented in Europe, Canada, Australia, the United States, several industrialized countries, and a few underdeveloped countries. The present study aimed to establish DRLs for the head, chest, and abdominopelvic (AP) CT procedures in Addis Ababa, Ethiopia.MethodsA pilot study identified the most frequent CT examinations in the city. At the time of the pilot, eighteen CT scan facilities were identified as having functioning CT scanners. Then, on nine CT facilities (50% of functional CT scanners), a prospective analysis of volume CT dose index (CTDIvol) and dose length product (DLP) was performed. We collected data for 838 adult patients’ head, chest, and AP CT examinations. SPSS version 25 was used to compute the median values of the DLP and CTDIvol dose indicators. The rounded 75th percentile of CTDIvol and DLP median values were used to define the DRLs. The results are compared to DRL data from the local, regional, and international levels.ResultThe proposed DRLs using CTDIvol (mGy) are 53, 13, and 16 for the head, chest, and AP examinations respectively, while the DLP (mGy.cm) for the respective examinations were 1210, 635, and 822 mGy.cm.ConclusionBaseline CT DRLs figures for the most frequently performed in Addis Ababa were provided. The discrepancies in dose between CT facilities and as well as between identical scanners suggests a large potential for dose optimization of examinations. This can be actually achieved through appropriate training of CT technologists and continuous dose audits.

Assessment of Organ Dose and Effective Dose from Head Computed Tomography in Northwestern Nigeria

The amount of radiation dose received by patient undergoing Head computed tomography examination in General Amadi rimi orthopedics and specialist hospital, Katsina has been estimated. Computed tomography dose output was used to determine the effective dose and organ dose of non-contrast to the head region of 30 patients. The tube potential ranged from 127 kVp to 130 kVp, while the mAs ranged from 130 mAs to 140. The aims of this study are, first, to assess the mean effective doses received by patients undergoing Head CT examinations and compare them with other studies. Mean values of CTDIvol. was (43.1mGy), the mean DLP values was (1206 mGy.cm), the mean effective dose (HEDLP) value was (2.4 mSv). The result was compared with the European Union reference dose level and other published factor. HEDLP was slightly higher than the values reported from the literature for the united nation and European union report by 0.4 factor. It was concluded that patient organ doses could be substantially minimized, optimized and justified. The high variation in the doses in this study may be due to differences in imaging protocols such as large range of mAs and scan length and also algorithms of the scanner.

Dose optimization of adult head computed tomography examination in an academic hospital in Ghana

The study investigated radiation doses and image quality of adult head CT and optimization options using an anthropomorphic RANDO phantom on the only available AquilionONE CT scanner in Ghana. Dose length product (DLP) and volume-weighted CT dose index (CTDIvol) dose descriptors were retrospectively obtained from 402 adult head CT examinations performed (from 2020 to 2021) at a Ghanaian hospital, while the effective dose (Deff) was estimated from the product of the DLP and a convention coefficient (k = 0.0023). Using the same routine head CT protocol, the anthropomorphic RANDO phantom was scanned with the hospital's 640-slice Toshiba AquilionONE CT scanner. Subsequently, exposure factors were varied to study their effects on the dose and image quality. The adult head and phantom images were obtained in Digital Imaging and Communications in Medicine (DICOM) format, and their signal-to-noise (SNR) ratios were analyzed with ImageJ software. The facility's mean CTDIvol, DLP, and effective dose (Deff) were 86.00 ± 0.00 mGy, 1559.68 ± 197.18 mGy cm, and 3.57 ± 0.46 mSv respectively (SNR = 7.04). For a fixed tube potential, CTDIvol, DLP, and Deff of 51.30 mGy, 1013.80 mGy cm, and 2.33 mSv were respectively achieved (SNR = 5.49) after optimization. Using automatic exposure control (AEC) in the optimization process, the respective CTDIvol, DLP and Deff values were 59.50 mGy, 1176.00 mGy cm and 2.70 mSv (SNR = 5.62). We conclude that substantial Deff reductions of 40.4% and 31.0% using a fixed tube potential, and AEC while maintaining diagnostic image quality were respectively achieved. The protocols associated with these dose-reductions are, therefore, recommended as optimization measures for head CT on the AquilionONE scanner in Ghana.

EYE RADIATION DOSE SAVING IN HEAD CT EXAMINATIONS USING COPPER-BISMUTH RADIATION SHIELD.

The aim of the present study is to fabricate a new shield with an optimal combination of copper and bismuth to protect the eyes in the head computed tomography (CT) examinations without compromising image quality. Radiation shields with different compositions were constructed. Computed Tomography Dose Index phantom was used to evaluate the effectiveness of shields in dose reduction and their impact on image quality quantitatively. The shield that caused the least noise in the phantom study was selected for human study. The 10%Bi-90%Cu shield had the least effect on increasing the image noise, and also no remarkable artifact was seen in the CT image of the phantom. The patient study showed that only in 25% of the study group the artifact was observed so that it did not distort the interpretation of the image. It can be concluded that the 10%Bi-90%Cu shield is flexible and durable and would be safely used in the clinic to reduce the eye radiation dose in head CT imaging without compromising image quality.
