Impact of COVID-19 Pandemic on Patients With Rheumatic Diseases in Medina, Saudi Arabia: An Observational Cross-Sectional Study.

The Coronavirus disease of 2019 (COVID-19) pandemic undoubtedly ranks among the most health-impacting pandemics throughout medical history.Although the COVID-19 global public health emergency has ended, lessons need to be learned to be more ready to facesimilar pandemics in the future. Few studies in Saudi Arabia discuss the impact of the COVID-19 pandemic on autoimmune rheumatic disease (AIRD) patients. Thus, this study was conducted to elaborate on the effects of the COVID-19 pandemic on AIRDpatients and rheumatology practices in Saudi Arabia. Methods: This observational cross-sectional study was conducted among patients aged over 14with AIRD using a pre-designed validated survey questionnaire. Data were collected from AIRD patients who were following up between November 2021 to April 2022 at the Rheumatology Clinicof King Fahad General Hospital in Madinah City, Saudi Arabia. This center was chosen as being the main hospital in the city following patients of AIRD. A total of 324 patients were included in our study, with the majority (n=264, 81.5%) being females. The mean age was 44.42±14.4 years. Clinical data revealed that 115(35.5%) of our patients experienced mild COVID-19 infection, 19 (5.9%) suffered from respiratory insufficiency, and seven (2.2%) required admission to the intensive care unit (ICU). Non-compliance to medication was recorded at 25.2%. There were 115 (35.5%) patients who had an AIRD flare that was significantly higher among those who were not adherent to the medications (p<0.001). Disease flare was also significantly seen among patients who were not on prednisone or were on low doses of prednisone (p<0.001). The majority (n=33, 97.1%) of the 34 infected patients who had an AIRD flarehad their flare-up at the same time as their COVID-19 infection (p<0.001). COVID-19 vaccination rate was 87.7% (n=284).The most common reason for non-vaccination in 40 (12.3%) patients was the patients' concern about disease flare-ups by the vaccine or interference of the vaccine with their medication (n=16, 4.9%). Our study showed a 35.5% (n=115) COVID-19 infection rate. The majority of our AIRDpatients sustainedminor infections that did not require hospitalization or ICU admission. The majority of the patients who underwent a severe COVID-19 infection coursewere not on prednisolone or were on low-dose prednisone. Due to COVID-19 restrictions and drug shortages, one in four patients (25.3%) stopped taking their medications and was significantly found to have a high prevalence of underlying AIRD flare. Despite the high vaccination rate, disease flare was the biggest concern for those who were not immunized. Although the COVID-19 pandemic has ended, doctors should be aware of risk factors associated with severe AIRD outcomes that should be balanced based on the infection severity, underlying disease flares, and patient-centered education about medication adherence and vaccination.

Predictors of COVID-19 vaccine uptake: an online three-wave survey study of US adults

BackgroundTo effectively promote vaccine uptake, it is important to understand which people are most and least inclined to be vaccinated and why. In this study, we examined predictors of COVID-19 vaccine uptake and reasons for non-vaccination.MethodsWe conducted an online English-language survey study in December-2020, January-2021, and March-2021. A total of 930 US respondents completed all surveys. Multiple logistic regression models were run to test whether the early vaccine eligibility, demographic factors, and psychological factors predict getting at least one dose of a COVID-19 vaccination in January-2021 and in March-2021.ResultsThe proportion of respondents who received ≥ 1-dose of a COVID-19 vaccine increased from 18% (January) to 67% (March). Older age predicted vaccine uptake in January (OR = 2.02[95%CI = 1.14–3.78], p < .001) and March (10.92[6.76–18.05], p < .001). In January, additional predictors were higher numeracy (1.48[1.20–1.86], p < .001), COVID-19 risk perceptions (1.35[1.03–1.78], p = .029), and believing it is important adults get the COVID-19 vaccine (1.66[1.05–2.66], p = .033). In March, additional predictors of uptake were believing it is important adults get the COVID-19 vaccine (1.63[1.15–2.34], p = .006), prior COVID-19 vaccine intentions (1.37[1.10–1.72], p = .006), and belief in science (0.84[0.72–0.99], p = .041). Concerns about side effects and the development process were the most common reasons for non-vaccination. Unvaccinated respondents with no interest in getting a COVID-19 vaccine were younger (0.27[0.09–0.77], p = .016), held negative views about COVID-19 vaccines for adults (0.15[0.08–0.26], p < .001), had lower trust in healthcare (0.59[0.36–0.95], p = .032), and preferred to watch and wait in clinically ambiguous medical situations (0.66[0.48–0.89], p = .007).ConclusionsEvidence that attitudes and intentions towards COVID-19 vaccines were important predictors of uptake provides validation for studies using these measures and reinforces the need to develop strategies for addressing safety and development concerns which remain at the forefront of vaccine hesitancy.

Vaccination coverage and adverse events following a reactive vaccination campaign against hepatitis E in Bentiu displaced persons camp, South Sudan.

Hepatitis E (HEV) genotypes 1 and 2 are the common cause of jaundice and acute viral hepatitis that can cause large-scale outbreaks. HEV infection is associated with adverse fetal outcomes and case fatality risks up to 31% among pregnant women. An efficacious three-dose recombinant vaccine (Hecolin) has been licensed in China since 2011 but until 2022, had not been used for outbreak response despite a 2015 WHO recommendation. The first ever mass vaccination campaign against hepatitis E in response to an outbreak was implemented in 2022 in Bentiu internally displaced persons camp in South Sudan targeting 27,000 residents 16-40 years old, including pregnant women. We conducted a vaccination coverage survey using simple random sampling from a sampling frame of all camp shelters following the third round of vaccination. For survey participants vaccinated in the third round in October, we asked about the onset of symptoms experienced within 72 hours of vaccination. During each of the three vaccination rounds, passive surveillance of adverse events following immunisation (AEFI) was put in place at vaccination sites and health facilities in Bentiu IDP camp. We surveyed 1,599 individuals and found that self-reported coverage with one or more dose was 86% (95% CI 84-88%), 73% (95% CI 70-75%) with two or more doses and 58% (95% CI 55-61%) with three doses. Vaccination coverage did not differ significantly by sex or age group. We found no significant difference in coverage of at least one dose between pregnant and non-pregnant women, although coverage of at least two and three doses was 8 and 14 percentage points lower in pregnant women. The most common reasons for non-vaccination were temporary absence or unavailability, reported by 60% of unvaccinated people. Passive AEFI surveillance captured few mild AEFI, and through the survey we found that 91 (7.6%) of the 1,195 individuals reporting to have been vaccinated in October 2022 reported new symptoms starting within 72 hours after vaccination, most commonly fever, headache or fatigue. We found a high coverage of at least one dose of the Hecolin vaccine following three rounds of vaccination, and no severe AEFI. The vaccine was well accepted and well tolerated in the Bentiu IDP camp community and should be considered for use in future outbreak response.

Missed opportunities: Reducing zero dose children among the urban poor after COVID, Mumbai India, 2022

Reaching urban poor populations poses challenges for equitable immunization coverage. Furthermore, COVID disrupted routine immunization services. In Mumbai, India, first dose diphtheria tetanus pertussis containing vaccine (DTPCV1)coverage dropped from 88% (2019) to 76% (2021).We identified and characterized 125 zero-dose (those withoutDTPCV1)migrant children in urban Mumbai in October 2022.Almost half were born elsewhere than Mumbai; 53% resided at their present location for less than a year. More than half were 12–59 months of age, well-beyond the age for first routine childhood immunizations.Three of four zero dose children had received birth dose vaccination in the hospital; but failed to receive DTPCV1. Vaccine hesitancy, awareness gaps and operational issues were common reasons for non-vaccination. Despite frequent visits to health facilities for illness,only a third of facility staff asked or advised parents about vaccination.Missed opportunities were much more common in private than government facilities.For the vast majority (88%), residential sites were included in local routine immunization micro-plans and distances to immunization sites were short (less than 1 km for 94 % of families).However, planned session frequency was inadequate half of the time.Expanded efforts to reach migrant urban poor children are needed to ensure vaccine equity.

COVID-19 vaccination status, side effects, and perceptions among breast cancer survivors: a cross-sectional study in China.

Breast cancer is the most prevalent malignancy in patients with coronavirus disease 2019 (COVID-19). However, vaccination data of this population are limited. A cross-sectional study of COVID-19 vaccination was conducted in China. Multivariate logistic regression models were used to assess factors associated with COVID-19 vaccination status. Of 2,904 participants, 50.2% were vaccinated with acceptable side effects. Most of the participants received inactivated virus vaccines. The most common reason for vaccination was "fear of infection" (56.2%) and "workplace/government requirement" (33.1%). While the most common reason for nonvaccination was "worry that vaccines cause breast cancer progression or interfere with treatment" (72.9%) and "have concerns about side effects or safety" (39.6%). Patients who were employed (odds ratio, OR = 1.783, p = 0.015), had stage I disease at diagnosis (OR = 2.008, p = 0.019), thought vaccines could provide protection (OR = 1.774, p = 0.007), thought COVID-19 vaccines were safe, very safe, not safe, and very unsafe (OR = 2.074, p < 0.001; OR = 4.251, p < 0.001; OR = 2.075, p = 0.011; OR = 5.609, p = 0.003, respectively) were more likely to receive vaccination. Patients who were 1-3 years, 3-5 years, and more than 5 years after surgery (OR = 0.277, p < 0.001; OR = 0.277, p < 0.001, OR = 0.282, p < 0.001, respectively), had a history of food or drug allergies (OR = 0.579, p = 0.001), had recently undergone endocrine therapy (OR = 0.531, p < 0.001) were less likely to receive vaccination. COVID-19 vaccination gap exists in breast cancer survivors, which could be filled by raising awareness and increasing confidence in vaccine safety during cancer treatment, particularly for the unemployed individuals.

Reasons for incomplete STI vaccination among men who have sex with men in an English sexual health service.

In England, vaccination for human papillomavirus, hepatitis A, and hepatitis B is recommended for men who have sex with men (MSM). However, uptake is sub-optimal and some men do not complete all recommended vaccine doses. This service evaluation aimed to explore reasons for lack of uptake for each of these vaccines among MSM in one English sexual health service and to inform improvements in service delivery to increase full dose completion rates. MSM, ≥18 years, who had previously attended NHS Solent Sexual Health for at least one vaccination, and who had not completed the full dosing regimen for at least one of these vaccines, were invited to participate in an anonymous, online survey between 14/12/2020-11/04/2021. Among 246 MSM (M = 42.1 years), the most common reason for non-vaccination was that participants thought it was unneeded and had not been recommended by a doctor or healthcare provider. None reported vaccine hesitancy. Likewise, the most common reasons for vaccination were doctor/healthcare provider recommendation (51.7-65.6%) and self-protection (60.9-68.1%). The most common reason for not having completed the full course of vaccination was being unaware that the next dose was due (30.0-37.8%). Many participants who had not completed vaccination indicated that a doctor/healthcare provider recommendation would be a motivating factor and that reminder messages and being able to book subsequent appointments in advance would facilitate vaccination. Sexual health clinicians should be encouraged to discuss STI vaccination with MSM and services should explore possibilities to improve ease and access to vaccine appointments to increase uptake and completion rates.

Low Pneumococcal Vaccination among Patients with Psoriasis in Germany: Results from Vac-Pso.

While suboptimal pneumococcal vaccination rates have been reported in immunosuppressed patients with rheumatic diseases, data for patients with psoriasis (PsO) or atopic dermatitis (AD) are scarce. Pneumococcal vaccination in Germany is recommended in patients with certain comorbidities, immunosuppression, and/or aged 60 years or above. The aim of this multicenter cross-sectional study was to investigate the pneumococcal vaccination rate in patients with PsO compared to patients with AD and to evaluate patient perceptions. All patients completed a questionnaire on vaccination status and perceptions, patient and disease characteristics, as well as comorbidity. Medical records and vaccination certificates were reviewed. Over the whole cohort (n = 327 PsO (41.9% female), n = 98 AD (42.9% female)), 83.8% and 42.9% of PsO and AD patients, respectively, had an indication for pneumococcal vaccination due to immunosuppressive treatment. The pneumococcal vaccination rate was 14.4% and 10.2% in PsO and AD patients, respectively. The vaccination rate depended significantly on age, working status and presence of psoriatic arthritis. The most common reason for nonvaccination was lacking recommendation by physicians. Higher awareness, particularly for vaccination indication due to immunosuppression among dermatologists, general physicians, and patients, is warranted.

Factors affecting poor measles vaccination coverage in sub-Saharan Africa with a special focus on Nigeria: a narrative review

Measles is a highly contagious infection that can cause severe illness in children. Vaccination is the primary means of controlling the infection, with elimination a possibility. However, the measles-containing vaccine coverage in sub-Saharan Africa (SSA) is 70% while that for Nigeria is 54% according to official estimates and 64.7% from household surveys. This review aims to identify factors that contribute to the poor coverage rate and proposes appropriate recommendations to address these factors. We conducted a comprehensive search of five databases (MEDLINE, Embase, Global Health, CINAHL Plus and PubMed) using the PICOTS (population, intervention, comparison, outcome, timing, study type) framework. The search was conducted in September 2017 in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines and inclusion criteria were studies conducted in SSA, that evaluated measles vaccine coverage gaps, evaluated strategies for improving measles vaccination coverage and were published in English. The database search yielded 23 publications. Several different factors influencing measles vaccine coverage were identified and were grouped into four main areas: immunization system, information and communication, family characteristics and parental attitudes and knowledge. Fears and misconceptions were common reasons for non-vaccination. Activities to improve vaccination coverage were identified, including structural reforms such as siting health centres within or proximal to target communities, improving female literacy and conducting measles vaccination campaigns. Multiple reasons for poor measles vaccination coverage were identified. Factors influencing the immunization system and information and communication factors can be fixed at the country level, but challenges relating to family characteristics and parental attitudes and knowledge require solutions adapted to the community of concern. However, vaccination campaigns are an effective means of improving vaccination coverage and they reduce geographic and socio-economic inequities.

COVID-19 Vaccination and Intent Among Healthcare Personnel, U.S.

IntroductionHealthcare personnel are at increased risk for COVID-19 from workplace exposure. National estimates on COVID-19 vaccination coverage among healthcare personnel are limited.MethodsData from an opt-in Internet panel survey of 2,434 healthcare personnel, conducted on March 30, 2021–April 15, 2021, were analyzed to assess the receipt of ≥1 dose of a COVID-19 vaccine and vaccination intent. Multivariable logistic regression was used to assess the factors associated with COVID-19 vaccination and intent for vaccination.ResultsOverall, 68.2% of healthcare personnel reported a receipt of ≥1 dose of a COVID-19 vaccine, 9.8% would probably/definitely get vaccinated, 7.1% were unsure, and 14.9% would probably/definitely not get vaccinated. COVID-19 vaccination coverage was highest among physicians (89.0%), healthcare personnel working in hospitals (75.0%), and healthcare personnel of non-Hispanic White or other race (75.7%–77.4%). Healthcare personnel who received influenza vaccine in 2020–2021 (adjusted prevalence ratio=1.92) and those aged ≥60 years (adjusted prevalence ratio=1.37) were more likely to report a receipt of ≥1 dose of a COVID-19 vaccine. Non-Hispanic Black healthcare personnel (adjusted prevalence ratio=0.74), nurse practitioners/physician assistants (adjusted prevalence ratio=0.55), assistants/aides (adjusted prevalence ratio=0.73), and nonclinical healthcare personnel (adjusted prevalence ratio=0.79) were less likely to have received a COVID-19 vaccine. The common reasons for vaccination included protecting self (88.1%), family and friends (86.3%), and patients (69.2%) from COVID-19. The most common reason for nonvaccination was concern about side effects and safety of COVID-19 vaccine (59.7%).ConclusionsUnderstanding vaccination status and intent among healthcare personnel is important for addressing barriers to vaccination. Addressing concerns on side effects, safety, and effectiveness of COVID-19 vaccines as well as their fast development and approval may help improve vaccination coverage among healthcare personnel.

Knowledge, Attitude and Practice of Hepatitis B Vaccination among Health Care Workers at Manipal Teaching Hospital

Background Hepatitis B vaccine is the single most effective and safest strategy for the prevention of the disease among health care workers. Despite the knowledge, higher occupational risk among themselves and increasing prevalence of hepatitis B virus worldwide, there is scanty information on knowledge, attitude and practice (KAP) concerning HBV vaccination among health care workers in our country.&#x0D; Objective To understand the knowledge, attitude and practice of hepatitis B vaccination among health care workers at Manipal Teaching Hospital at Pokhara, Gandaki Province in Nepal.&#x0D; Method Four hundred and eight health care workers were enrolled for an observational, cross-sectional study at Manipal Teaching Hospital, Gandaki Province, Nepal after obtaining ethical clearance from Institutional Review Committee. Pre-tested questionnaire including knowledge, attitude and practice regarding hepatitis B vaccination were studied.&#x0D; Result All participants demonstrated good knowledge and positive attitude towards Hepatitis B infection and vaccination. However many had risky practice towards it. Only about half (51.7%) of these participants were completely vaccinated. The most common reason for non vaccination was negligence.&#x0D; Conclusion Despite good knowledge and positive attitude towards hepatitis B infection and vaccination, low rates of vaccination and risky practice was observed among HCW. Various occupational, behavioural, economical and psychological factors associated with it must be explored. Easy availability of vaccine, regular hepatitis B campaigns must be conducted and policy guidelines need to be formulated by the government to manage all aspects of knowledge, attitude and practice of HCWs regarding hepatitis B vaccination.

Underreporting of the 5-year tetanus, diphtheria, pertussis and polio booster vaccination in the Danish Vaccination Register

BackgroundIn Denmark, vaccination coverage is measured using the Danish Vaccination Register (DDV). In general, the vaccination coverage is high, but for some vaccinations, the coverage is suboptimal with geographical variation. This study aims to validate the vaccination coverage of the 5-year booster and identify overall reasons for non-vaccination in Copenhagen.MethodsWe validated the coverage of the 5-year tetanus, diphtheria, pertussis and polio booster for children born in 2010 and living in Copenhagen municipality in 2018, an area with low coverage (current estimate: 89%). We identified all children born in 2010 in the Civil Registration System and sent an electronic questionnaire to parents of children without a record of the 5-year booster in the DDV.ResultsParents of 692 children were contacted and 49% participated. Of those, 186 (55%) reported that the child was vaccinated: 61% by their general practitioner and 34% abroad. The most common reason for non-vaccination was forgetfulness (31%), 26% did not want their child vaccinated and 17% had migrated from abroad and were not aware of the vaccination schedule. Considering only children with documentation for the vaccination, the corrected vaccination coverage was 91%.ConclusionsWe conclude that the coverage of the 5-year booster in Copenhagen is currently underestimated and should be adjusted by 2%. We recommend increased awareness from general practitioners and tailored communication about the vaccination programme targeting immigrants in Denmark.

Knowledge, Attitude and Practice of Hepatitis B Vaccination among Clinical Medical Students at a Medical College in Nepal

Background: Hepatitis B vaccine is the single most effective and safest strategy for the prevention of the disease among health care workers (HCW), trainees and medical students. There is scanty information on knowledge, attitude and practice (KAP) regarding Hepatitis B vaccination among medical students who are likely to get exposed in the future as they start practicing. This study was undertaken to understand the knowledge, attitude and practice of hepatitis B vaccination among clinical medical students of Manipal College of Medical sciences at Pokhara, Nepal.Methods: Two hundred and four medical students were enrolled for an observational, cross-sectional study at Manipal College of Medical sciences at Pokhara, Nepal after obtaining ethical clearance from Institutional Review Committee. Answers to pre-tested questionnaire were collected. Knowledge, attitude and practice regarding Hepatitis B vaccination were studied. Results: All participants demonstrated good knowledge and positive attitude towards Hepatitis B infection and vaccination. However majority had poor practice towards it. Only 47.1% were completely vaccinated. The most common reason for non vaccination was that many thought they will vaccinate in internship or when they start practicing.Conclusion: Despite good knowledge and positive attitude towards Hepatitis B infection and vaccination, low rates of vaccination and poor practice was observed among HCW indicating the necessity of encouragement for vaccination and proper practices among them. All medical students should be immunized against Hepatitis B during their medical school. Regular Hepatitis B vaccination, educational and awareness programs must be conducted in coordination and with active participation of the medical students.Keywords: Attitude; Hepatitis B; knowledge; Medical students; Practice; Vaccination

P207 A quality improvement intervention to increase pneumococcal and influenza vaccination rates in immunosuppressed inflammatory arthritis patients

Abstract Background Pneumococcal and influenza vaccination rates have been suboptimal in studies of immunosuppressed patients. We aimed to assess barriers to and increase rates of 23-valent pneumococcal polysaccharide vaccine (PPSV23) and influenza vaccination in this group. Methods In 2017, Rheumatology outpatients completed an anonymous questionnaire recording vaccination knowledge, status and barriers. Simultaneously, a low-cost multifaceted quality improvement (QI) intervention was performed. All outpatients on oral steroids, immunosuppressant conventional synthetic disease modifying antirheumatic drugs (csDMARDs) or biologics (bDMARDs) were included in the study. In 2018, post-intervention, the clinic was re-assessed. Demographics, diagnosis, medications, smart phone access and willingness to use this for vaccination reminders were assessed for independent vaccination predictors using binary logistic regression analysis. Results 425 patients were included (72.6% rheumatoid arthritis, 74% women, 45.6% ≥60 years old). From 2017-2018, vaccination rates increased for PPSV23 {41.0% to 47.2% (p = 0.29)} and influenza {61.8% to 62.1% (p = 0.95)}. The most common reason for non-vaccination was lack of awareness. Following the intervention, this decreased for influenza (36.7% to 34.2%) and PPSV23 (82.1% to 76.4%). General Practitioners performed most vaccinations, only 3.6% were delivered in hospital. Significant predictors of PPSV23 vaccination were older age {≥80 years had an OR 41.66 (95% CI 3.69-469.8, P = 0.003), compared to ≤ 39 years}, bDMARD use (OR 2.80, 95% CI 1.24-6.32, P = 0.013) and adequate influenza vaccination (OR 9.01, 95% CI 4.40-18.42, P &amp;lt; 0.001). Up to date PPSV23 vaccination (OR 8.93, 95% CI 4.39-18.17, P &amp;lt; 0.001) predicted influenza vaccination. The intervention did not cause a statistically significant change in vaccination rates. Differential determinants and reasons for the non- and partial vaccination of children among Nigerian caregivers

BackgroundVaccination has saved millions of lives. However, the vaccination rate in Nigeria remains low. We differentiate reasons for incomplete vaccination by status: not- and partially- vaccinated, and examine the association between sociodemographic characteristics of caregivers and reasons for incomplete vaccination. MethodsData from the Multiple Indicator Cluster Survey (MICS) conducted in Nigeria in 2016 and 2017 was analyzed. The logistic regression was used to evaluate associations between sociodemographic characteristics of caregivers and incomplete vaccination according to different reasons. ResultsThe most common reason for non-vaccination is that caregivers have no faith in immunization (27.0%), followed by lack of awareness of the need for immunization (26.0%) and the perception that the place where immunization is given is too far/inconvenient (24.3%). The most common reason for partial vaccination is that caregivers thought the children had already been fully immunized (44.8%), followed by the inconvenient location for immunization (15.3%) and supply-side issues such as shortage of vaccine stock and absence of vaccinators (14.4%). Among caregivers whose children are never vaccinated, a lower level of education is correlated with more likelihood of having no faith in immunization and poorer households are more likely to state a lack of awareness of the need for immunization need and an inconvenient place as the reasons for non-vaccination. Among caregivers whose children are only partially vaccinated, educated and wealthier caregivers are more likely to state that they thought their children were fully vaccinated, while poorer caregivers tend to give an inconvenient immunization place as the reason for incomplete vaccination. Supply-side barriers are not systematically correlated with educational attainment or wealth level of caregivers. ConclusionBarriers to vaccination are different according to vaccination status: not- and partially- vaccinated, as is the association between sociodemographic characteristics of caregivers and reasons for incomplete vaccination. The policy to increase vaccination take-up should take these differentials into consideration.

Causes of Non-Vaccination of Influenza in Healthcare Workers at Zanjan Educational Hospitals, Iran

Background: Influenza is an acute respiratory illness that can cause hospitalization, and confers a high mortality rate, especially in high-risk groups. Influenza vaccination for hospital staff can play an effective role in controlling nosocomial flu infections. The purpose of this study was to investigate causes of non-vaccination of influenza in healthcare workers at Zanjan educational hospitals. Materials and Methods: This was a cross-sectional study conducted during three months at two Zanjan educational hospitals, Iran. Two hundred and ten workers belonging to the hospital staff were randomly selected.The reasons for not receiving the vaccine and the demographic information of the staff were recorded in a special questionnaire. The questionnaire included demographic information such as: age, gender, occupation, work place, history of Influenza vaccination last year, and reason for non-vaccination of Influenza. Results:Two hundred individuals were enrolled, which included 58% of physicians and 42% of nurses. In the past year, 51% of the subjects have received influenza vaccine. Of these, 45.7% were physicians and 58.3% were nurses. The most common reasons for non-vaccination were: the belief that healthy people do not have the chance of getting the infection (28.6%), forgetfulness (24.5%), lack of adequate information and fear of complications from the vaccine (14.3%). Conclusions: The status of influenza vaccination in the staff of these hospitals was relatively acceptable (51% of the subjects were vaccinated);however, it has not yet reached the ideal.According to the results, the most common reasons for non-vaccination were: wrong thinking about getting the infection, forgetfulness, and fear of complications.

Oral cholera vaccination in hard-to-reach communities, Lake Chilwa, Malawi.

ObjectiveTo evaluate vaccination coverage, identify reasons for non-vaccination and assess satisfaction with two innovative strategies for distributing second doses in an oral cholera vaccine campaign in 2016 in Lake Chilwa, Malawi, in response to a cholera outbreak.MethodsWe performed a two-stage cluster survey. The population interviewed was divided in three strata according to the second-dose vaccine distribution strategy: (i) a standard strategy in 1477 individuals (68 clusters of 5 households) on the lake shores; (ii) a simplified cold-chain strategy in 1153 individuals (59 clusters of 5 households) on islands in the lake; and (iii) an out-of-cold-chain strategy in 295 fishermen (46 clusters of 5 to 15 fishermen) in floating homes, called zimboweras.FindingVaccination coverage with at least one dose was 79.5% (1153/1451) on the lake shores, 99.3% (1098/1106) on the islands and 84.7% (200/236) on zimboweras. Coverage with two doses was 53.0% (769/1451), 91.1% (1010/1106) and 78.8% (186/236), in the three strata, respectively. The most common reason for non-vaccination was absence from home during the campaign. Most interviewees liked the novel distribution strategies.ConclusionVaccination coverage on the shores of Lake Chilwa was moderately high and the innovative distribution strategies tailored to people living on the lake provided adequate coverage, even among hard-to-reach communities. Community engagement and simplified delivery procedures were critical for success. Off-label, out-of-cold-chain administration of oral cholera vaccine should be considered as an effective strategy for achieving high coverage in hard-to-reach communities. Nevertheless, coverage and effectiveness must be monitored over the short and long term.

Knowledge of hepatitis B among healthy population: A community-based survey from two districts of Gujarat, India.

Background:Hepatitis B is the world's most common blood-borne viral infection, accounting for 2 billion infections, 350 million carriers, and 6 lakh deaths annually. Country like India still harbors approximately 30–60 million hepatitis B virus (HBV) carriers. A modest estimate would put the number of deaths occurring due to HBV infection per year in India to around 100,000. To prevent transmission and progression of the disease, proper community awareness including prevention is necessary. Therefore, this study aims to study the knowledge awareness among the healthy population about hepatitis B including knowledge regarding vaccine.Methodology:A community-based cross-sectional study was undertaken in two districts of Gujarat. Cluster sampling (30 clusters) was used, and pretested questionnaire was administered to 600 (with a prevalence rate of 5% in Gujarat having design effect of 2 within 95% confidence interval and 10% nonrespondent) healthy individuals, who heard about hepatitis B. Data handled and analyzed in EpiData Analysis V2.2.2.183.Results:Majority of the participants knew about symptoms whereas only 41% knew about prevention methods and few 34% knew about the mode of transmission. Although 40% sample has knowledge about the availability of vaccination, only 20% were self-vaccinated. The common reason for nonvaccination was lack of awareness.Conclusions:Only one-third of the populations in study districts are aware about hepatitis B and its vaccine. Less than one-fifth of the populations are vaccinated for hepatitis B. Important knowledge deficits about the routes of hepatitis B transmission/prevention were identified. Continued efforts should be made to develop and implement hepatitis B educational campaigns/health promotion for these communities.

Vaccine hesitancy in Austria : A cross-sectional survey.

Vaccine hesitancy is an emerging phenomenon particularly in industrialized nations. It has led to repeated epidemic outbreaks of otherwise vaccine-preventable, infectious diseases. Compared to other countries very low rates of influenza and measles vaccination rates have been reported in Austria. We performed asingle-center cross-sectional, questionnaire-based survey. A total of 350 adult patients attending our emergency room participated in this survey. We assessed knowledge and attitudes towards vaccination and the associated infectious diseases. Out of 350 participants 40 (11.4 %) declared that they deliberately refused vaccinations. Most common reasons for non-vaccination were fear of adverse effects (35.9 %), doubt of effectiveness of vaccines (35.9 %) and distrust towards the pharmaceutical industry (23.1 %). Of all 350 participants only 148 (42.3 %) thought themselves to be sufficiently informed about national vaccination recommendations as stated in the Austrian National Vaccination Program (ANVP). General practitioners (GP) were the primary source of healthcare-related information for 256 (73.1 %) participants. Furthermore, GPs as well as hospital-based physicians achieved the highest level of trust in this study population. The results of our study underline the necessity of comprehensive informational campaigns on the merits of vaccination. Alack of knowledge about the benefits of vaccination, uncertainty and unfounded fears seem to prevent the achievement of recommended vaccination rates. Family GPs enjoyed the highest levels of trust in our study population. We believe that additional information communicated by GPs could help boost the low vaccination rates. This study underlines the important role of primary care practitioners in informing patients about vaccines and healthcare topics.
