The making of the psychiatrist-executive.

The authors reviewed the literature on the factors influencing the development of psychiatrist-executives. The results indicate that personality traits, clinical psychiatry training, training in administration, mentors, and administrative experience have an impact on the making of the psychiatrist-executive. On the basis of these findings, the authors make recommendations for further research.

Where Can we Teach Clinical Psychiatry?

Most medical students in the United States receive their clinical psychiatry training in community facilities with varied populations, clinical orientations and management systems. Given a substantial diversity in training sites available to the University of Nevada School of Medicine, we wondered if all of our students were learning the basic principles of psychiatry and if there were any differences based on placement. We analyzed outcomes of 139 students assigned to five different facilities. We concluded, based on a statistical analysis of a number of evaluation techniques, that our students did indeed meet the objectives of the educational experience without major differences.

In 1849 official mental hospital committee pointed out a number of factors that could improve the development of mental care. It was considered necessary to establish a central board for all mental hospitals which at that time were old and inadequately functioning. The hospitals had to be rebuilt and modernized. It was also necessary to improve the economy with funds from the government. All mental hospitals should have at least one consultant. During the first decades of the 19th century there were only two specially trained psychiatrists in Sweden. There was no clinical education on mental diseases in Sweden. In 1855 the faculty of medicine of Uppsala University proposed that education in psychiatry should be given to all medical students, and four years later, in 1859, Nils Gustav Kjellberg was appointed as the first assistant professor of psychiatry at Uppsala University. MENTAL HEALTH OF STUDENTS RECEIVING CLINICAL PSYCHIATRIC TRAINING

322 medical students in their clinical training period were instructed in psychiatry by two different methods; method A centred around clinical case material whereas method B emphasised the group discussion of such material in the light of students' subjective feelings and impressions about it. Method B led to the ventilation of many students' anxieties and worries about their possible psychiatric disorders through talking about the patients' symptoms they identified with. The number of students presenting for psychiatric difficulties from method-B group was significantly less than from method-A group. It is suggested that the group-centred technique might be more suitable than the orthodox case-material-centred method for clinical training in psychiatry. The incorporation of group-psychotherapeutic principles in the instruction setting seems to have a beneficial effect on the mental health of students diminishing their need to seek psychiatric help.

Training in community psychiatry: Its relation to clinical psychiatry

The community psychiatry training programs, in response to national need, have begun to train psychiatrists who are not given much basic clinical psychiatry training. This poses some serious problems. The community psychiatrist does not usually function in immediate relationship to the people with whom he is concerned. It is thus easier to dehumanize people and see them as a mass to be manipulated. Clinical training, with continued clinical work, not only sensitizes one to transference and countertransference problems of individuals and group psychotherapeutic work vital to self-awareness in all interpersonal situations, but it also keeps one focused on the individual human being and his needs and reactions to programs and process. It also makes it possible to utilize clinical acumen in all of one's interpersonal relations at all levels.
