Effect on work ability after team evaluation of functioning regarding pain, self-rated disability, and work ability assessment

To evaluate the effect of a 1–2 week multiprofessional team assessment, without a real rehabilitation effort, 60 patients suffering from long-standing pain and on long-lasting time on sick leave were studied. A questionnaire concerning their daily activities, quality of life, pain intensity, sick-leave level, and their work state was filled out by all patients before starting the assessment and at a 1-year follow-up. The results from the assessment period and the multiprofessional team decision of the patient’s working ability were compared with the actual working rate after 1 year. The follow-up showed a significant reduction of sick leave and a higher level of activity (P < 0.001). One year after the initial evaluation, 40% showed a reduction in sickness benefit level and 12% resumed full-time work. However, the team evaluation of the patient’s work ability did not correlate to predict the actual outcome. The patient’s pain intensity, life satisfaction, gender, age, ethnic background, and time absent from work before the start of the evaluation showed no correlation to reduction on time on sickness benefit level. These parameters could not be used as predictors in this study.

Risk of Cerebrovascular Adverse Events in Older Adults Using Antipsychotic Agents

To compare the risk of cerebrovascular adverse events with second-generation antipsychotic users versus those taking first-generation antipsychotics in community-dwelling older adults. A population-based retrospective cohort study matched on propensity score was used to examine the risk of cerebrovascular adverse events in second-generation antipsychotic users compared to first-generation antipsychotic users. IMS LifeLink Health Plan Claims Database was used to identify older adults (> or = 50 years) taking second-generation or first-generation antipsychotic agents from July 1, 2000, to December 31, 2007. Cox proportional hazards regression model stratified on matched pairs was used to examine the risk of hospitalization or emergency visits due to cerebrovascular adverse events within 1 year of follow-up (primary outcome measure). The covariates adjusted for include duration of therapy and exposure to other medication increasing risk of cerebrovascular adverse events. A total of 11,160 older adults (5,580 second-generation and 5,580 first-generation antipsychotic users) matched on propensity score was obtained. Regression analysis revealed that no statistically significant difference exists between second-generation and first-generation antipsychotic users with respect to risk of cerebrovascular adverse events (hazard ratio [HR], 0.858; 95% CI, 0.689-1.446). However, duration of therapy between 30-90 days (HR, 1.707; 95% CI, 1.174-2.481) and more than 90 days (HR, 1.570; 95% CI, 1.132-2.176) was associated with increased risk of cerebrovascular adverse events compared to duration of therapy less than 30 days. The use of second-generation antipsychotic agents was found not to be associated with increased risk of cerebrovascular adverse events compared to first-generation agents in older adults. However, long-term use of second- and first-generation antipsychotic agents is associated with increased risk of cerebrovascular adverse events.

Relationship between natriuretic peptides and echocardiography parameters in patients with poorly regulated type 2 diabetes

This study evaluated the relationship between natriuretic peptide levels and a wide range of echocardiography parameters in a population of thirty-three patients with poorly regulated type 2 diabetes, and no known heart failure. Natriuretic peptides brain natriuretic peptide (BNP) and N-terminal prohormone BNP (NT-proBNP) were measured. Transthoracic echocardiography was performed and cardiac volumes and ejection fraction were measured. Doppler and tissue Doppler were measured and diastolic function was stratified according to recent guidelines. Very few echocardiography parameters were correlated with BNP or NT-proBNP levels. However, left atrial end-systolic volume indexed for body surface area was correlated with natural logarithm (ln) BNP and ln NT-proBNP (r=0.62 and r=0.60; P<0.05). There were significant differences in ln BNP and ln NT-proBNP levels between those with normal and those with abnormal diastolic function (1.4 vs 3.1; P<0.001 and 3.4 vs 5.8; P<0.001). This study showed that very few echocardiography parameters were correlated with BNP or NT-proBNP levels in patients with poorly regulated type 2 diabetes, which in part contradicts previous studies in other diabetic populations. The exception was left atrial end-systolic volume that showed a moderate correlation with BNP or NT-proBNP levels. There were significant differences in BNP and NT-proBNP levels between the group with normal left ventricular diastolic function and the group with abnormal diastolic function.

Off-Label Use of Second-Generation Antipsychotic Agents Among Elderly Nursing Home Residents

This study examined off-label and evidence-based use of second-generation antipsychotic agents among elderly nursing home residents and factors associated with off-label use. This study involved a retrospective, cross-sectional analysis of data from the 2004 National Nursing Home Survey (NNHS). The sample included nursing home residents 65 years and older who received second-generation antipsychotic agents. This study used an indication-based definition of off-label use established by the U.S. Food and Drug Administration (FDA). Evidence-based use included FDA-approved indications and indications for which the Agency of Healthcare Research and Quality found at least moderate strength of evidence of effectiveness. Descriptive statistics were used to examine the prevalence of off-label and evidence-based use. Multiple logistic regression was used to examine the patient and facility factors associated with off-label use of second-generation antipsychotics. According to the 2004 NNHS, 308,990 (23.5%) elderly nursing home residents received at least one second-generation antipsychotic agent. Of those using second-generation antipsychotics, 86.3% received them for off-label indications and 56.9% received them for an evidence-based use. Multivariate analysis found that age (> or =75 years), self-pay for nursing home care, diagnosis of dementia, and residing in a nonprofit nursing home were positively associated with off-label use, whereas receiving Medicaid benefits was negatively associated with such use. Although second-generation antipsychotics were frequently used for off-label indications, most of the usage was evidence based among elderly nursing home residents. However, the high level of non-evidence-based use combined with recent safety and efficacy data suggests an urgent need to address the evidence base for this vulnerable population.

Magnetoencephalographic signals from sensorimotor areas devoted to lower limb and tibialis anterior (TA) electromyographic activities were collected bilaterally and simultaneously on 7 healthy subjects during rest (Rest), left and right ankle dorsiflexion imparted through the SHADE orthosis (O-PM) or neuromuscular electrical stimulation (NMES-PM), or during active isometric contraction (IC-AM). The effects of focussing attention on ankle movements were considered. Primary sensory (FS_S1) and motor (FS_M1) area activities were discriminated by the Functional Source Separation algorithm. Power in alpha [8-12.5 Hz], beta [13-33 Hz], gamma1 [33-45 Hz] and gamma2 [60-90 Hz] bands of the sources and their coherence during each condition were calculated. Source band reactivity was evaluated as reduction of power with respect to Rest. Only contralateral FS_S1 was recruited by common peroneal nerve stimulation and only contralateral FS_M1 displayed coherence with TA muscular activity. FS_M1 showed higher power of gamma rhythms than FS_S1. Both sources reacted bilaterally during IC-AM in beta band. Only FS_S1 was also reactive during O-PM. No reactivity of either source resulted during NMES-PM. FS_S1-FS_M1 coherence was higher in gamma2 band when focussing attention only during O-PM, especially when using the right foot. Somatosensory and motor counterparts of lower limb cortical representations were discriminated in both hemispheres, and a prevalence of the left dominant hemisphere was evidenced. SHADE was effective in generating repeatable dorsiflexion and inducing primary sensory involvement similarly to voluntary movement. D Annunzio University, Department of Clinical Sciences and Bioimaging , Italy 4 Ospedale Fatebenefratelli, Isola Tiberina, CNR-ISTC, Unità MEG, Italy Knowledge about the physiological organization of the areas devoted to the hand control, in terms of local connectivity and functional relationship between sensory and motor counterparts, is a cornerstone for comprehension of reorganization phenomena during learning and in reacting to pathological conditions. Through a novel cerebral source extraction method from magnetoencephalographic signals, the Functional Source Separation [1], primary cortical codes of sensorimotor dexterity were investigated: 1) in the somatosensory counterpart, through representation assessment of two fingers with different levels of functional skill (Thumb, Little finger), obtained by separately providing a passive simple sensory stimulation. In the two hemispheres, neural oscillatory activity synchronization was analysed in the characteristic frequency bands by a measure isolating the phase locking between neural network components. 2) in the motor counterpart, through assessment of cortical features related to better and worst performances. In the primary sensory cortex, in the dominant hemisphere, the phase locking selectively in the gamma band (ICC) was higher for the thumb than for the little finger and it correlated with the contra-lateral finger dexterity, scored by the Fingertip writing test. During an isometric contraction, we found that synchronous activity of primary sensory and motor areas in the high gamma band was dependent on the performance level and with low variability in the healthy population. Our data on ICC suggest the dynamic gamma band phase locking as a code for finger dexterity in the primary sensory cortex, in addition to the magnification of somatotopic maps. Moreover, connectivity in the high gamma band between primary sensory and motor cortices quantitatively estimates the continuous functional balance between primary sensory and motor areas devoted to hand control, representing a sensorimotor feedback efficiency index.

To investigate these properties, the primary component of the sensorimotor network was analyzed in mildly disabled Relapsing Remitting MS patients without sensory symptoms at the time of the investigation. By magnetoencephalography (MEG), the recruitment pattern within the primary sensory (S1) and motor (M1) areas was estimated through the morphology of the early components of somatosensory evoked magnetic fields (SEFs), after evaluating the S1 responsiveness to sensory inputs from the contralateral arm. In each hemisphere, network recruitment properties were correlated with ispi-lateral thalamus volume, estimated by morphometric techniques upon high resolution 3D structural magnetic resonance (MR) images. S1 activation was preserved while SEF morphology was strikingly distorted in MS patients, marking a disruption of primary somatosensory network patterning. An unbalance of S1-M1 dynamic recruitment was documented, correlated with the thalamic volume reduction in the left hemisphere. These findings support the model of MS as a disconnection syndrome, with major susceptibility to damage experienced by nodes belonging to more frequently recruited and highly specialized networks. Empathy for pain studies have shown that the observation of others’ pain induces the activation of discrete brain areas within the pain matrix, suggesting that the experience of pain may be socially shared. Neuroscience studies focused on whether empathic pain resonance is linked to affective or sensory nodes of the pain matrix. However, empathy, like other complex cognitive processes, is inherently linked to the activation of functional networks rather than of separate brain areas. Here, we used magnetoencephalography (MEG) to explore the relationship between empathy and functional coupling of neuronal activity in primary somatosensory (SI) and motor (MI) cortices.MEG recording was performed while healthy participants observed movie-clips depicting the static hand of a stranger model, the same hand deeply penetrated by a needle, or gently touched by a Q-tip. Subjects were asked to rate the movie-derived sensations attributed to self or to the model. For each type of clip observation, we analyzed spectral power and coherence values in α, β, and γ frequency bands. While spectral power indexes separate neural activity in SI and MI, coherence values index functional cross-talk between these two areas. No power changes of SI or MI sources were induced by observation conditions in any of the frequency bands. Crucially, γ-band coherence values were significantly higher during needle-in-hand than touch and static hand observation and correlated with self-and otherreferred pain ratings derived from needle-in-hand movies observation. Thus, observation of others’ pain increases neuronal synchronization and cross-talk between the onlookers’ sensory and motor cortices, indicating that empathic resonance relies upon the activity of functional networks more than of single areas. Immunohistochemical Analysis of Bcl-2 Protein in Canine Mammary Tumours

Malicka* *Department of Clinical Sciences, Warsaw University of Life Sciences, Warsaw and yDepartment of Physiological Sciences, Warsaw University of Life Sciences, Warsaw, Poland Introduction: Development of canine mammary tumours involves accumulation of mutant cells caused by excessive proliferation and insufficient apoptosis. Bcl-2 is an oncoprotein and plays a role in blocking programmed cell death. Moreover, p53 gene products have been linked to apoptosis pathways. Materials and Methods: Samples of 112 canine mammary tumours were fixed in 10% neutral buffered formalin. Histopathological diagnosis was based on the WHO classification system using sections stained with haematoxylin and eosin. Expression of Bcl-2, p53 protein, Ki67 antigen, oestrogen (ERa) and progesterone (PR) receptors was evaluated by immunohistochemistry. Results: Positive expression of Bcl-2 and p53, respectively, was noted in 73% and 38% of the tumours. While p53 had nuclear localization, Bcl-2 was found in the cytoplasm of tumour cells. The Bcl-2 protein was detected in 81% of benign tumours and in 40% of grade I carcinomas. Expression of Bcl-2 was significantly correlated with ERa, but not with PR or p53. Proliferative activity showed a significant positive correlation with histological type, tumour grade and p53. Conclusions: The positive correlation between Bcl-2 and ERa suggests that oestrogen may be a regulator of Bcl-2 in canine mammary tumours. Prospective determination of Bcl-2 and p53 status may be useful in tumour grading and prognosis.

First-Time Department Chairs at U.S. Medical Schools: A 29-Year Perspective on Recruitment and Retention

Given the resources that medical schools expend to recruit and develop department chairs, the authors carried out the investigation reported here to address the scarcity of national information on the recruitment and retention of first-time chairs. The authors used data from the Association of American Medical Colleges (AAMC) Faculty Roster. The data included first-time chairs at all Liaison Committee on Medical Education accredited medical schools from July 1978 through June 2007 (a total of 125 schools). Primary outcome measures were the annual percentages of new chairs, retention rates of chairs, and retention curves of chairs. The annual frequencies of first-time chairs to recruit were remarkably similar between clinical and basic science departments, yet recruitment was more frequent for larger clinical departments. On average, 8.2% of all chair positions were filled each year by new chairs. Today's first-time chairs tend to be older, with there being a gradual emergence of more women. Retention curves revealed the median tenure to be 8.0 years for the most recent cohort of chairs (starting 1994 through 1998), which was down from 9.9 years for the earliest cohort analyzed (chairs starting 1979 through 1983). Chairs with longer tenures were more likely to be less than 50 years old, hold a professor rank, and be external candidates. Retention of academic chairs in both clinical and basic science departments is cyclic but has declined in more recent years. The methodology presented here may serve as a template for calculating recruiting and retention rates of chairs in individual disciplines and at individual medical schools, to compare rates between schools.

Multidisciplinary program for stress-related disease in primary health care

To describe a multidisciplinary program, given by an occupational therapist and a physiotherapist, for patients with stress-related disease in primary health care and to measure the effect of this program in terms of self-perceived health, degree of burnout, physical activity, symptoms, recreational activities, and psychological and physical well-being. Retrospective study. At measures after three months, the thirteen patients included in this study had improved in self-estimated health, measured with EuroQol-5D Visual Analogue Scale (p = 0.000), and in degree of burnout, measured with the Shirom-Melamed Burnout Questionnaire (p = 0.001). There was also a decrease in presence of headache, in physical activity and in satisfaction with leisure time, although not statistically significant. After six months, the improvements remained for all measures except physical activity. The patients were also satisfied with the program to a high degree, measured with Client Satisfaction Questionnaire (median 3.7). This descriptive study shows that a stress-management program, provided by a team including an occupational therapist and a physiotherapist in primary health care, is both feasible and effective in terms of self-estimated health, degree of burnout, and patient satisfaction.
