Asymptomatic bilateral facial swelling

A 54-year-old Black woman sought treatment at the oral medicine clinic at the School of Dental Medicine at the University of Pennsylvania for evaluation of bilateral facial swelling. The patient endorsed that the swelling had been present for the past 18 months but denied any associated symptoms, including pain, drainage, facial weakness, facial numbness, or any history of facial swelling. The patient denied constitutional symptoms, including fatigue, fever, chills, or unintentional weight loss. She denied history of trauma to the face and endorsed that the swelling had increased slowly over time. The patient denied alleviating or exacerbating factors. She reported that the amount of swelling did not change with oral nutritional intake. The patient denied a history of head and neck cancer and head and neck radiation therapy. Her medical history included hypertension, anxiety, and insomnia. Medications included metoprolol, nifedipine, and olmesartan. Social history included smoking 6 to 7 cigarettes per day for the past 15 years and consuming 1 to 2 alcoholic drinks per month without any history of alcohol abuse or recreational drug use. The review of systems was negative after detailed evaluation. ICTP as a Diagnostic Marker of Bone Resorption and Its Relation to Disease Progression and Treatment Response in Multiple Myeloma

Subject and methods: ICTP was measured in 40 newly diagnosed MM patients (including 22 patients with osteolytic lesion), and 20 ageand sexmatched controls. Patients were followed up for detection of their response to treatment. Results: The median and IQR levels of ICTP were significantly elevated in MM patients compared with controls (0.65 [0.15-0.8] ng/mL versus 0.05 [0.04-0.07] ng/mL, p<0.001), with significant higher median levels among patients with osteolytic lesion compared with myeloma patients without osteolytic lesion (0.7 [0.65 – 1.0] ng/mL versus 0.1 [0.1 -0.2] ng/mL, p<0.001). According to ROC curve analysis, ICTP at a cutoff 0.3 ng/mL could detect osteolytic lesion with high sensitivity and specificity. ICTP was positively correlated with markers of renal impairment, hypercalcemia, anemia, thrombocytopenia and poor prognostic factors; lactate dehydrogenase (LDH), C reactive protein (CRP), hypoalbuminaemia, and β2-microglobulin (β2M) (p<0.05). Using logistic regression analysis, ICTP was found to be a significant predictor of osteolysis. Moreover, initial ICTP levels were higher in progressive/relapsed myeloma patients compared with responsive/stable patients (p<0.001). Conclusion: We suggest that ICTP is a reliable marker that can be used in the diagnosis and prediction of osteolytic lesion and prediction of treatment response in MM.

Sudden and unexpected death in early life: proceedings of a symposium in honor of Dr. Henry F. Krous

Reported here are the proceedings of a symposium given in honor of Dr. Henry F. Krous upon his retirement as Clinical Professor of Pathology and Pediatrics at the University of California Schools of Medicine, and as Director of the San Diego SIDS/SUDC Research Project. Dr. Krous' distinguished 37-year-career was dedicated to research into sudden unexpected death in infancy and childhood, notably the sudden infant death syndrome (SIDS) and sudden unexplained death in childhood (SUDC). The presentations were given at the International Conference on Stillbirth, SIDS, and infant survival on October 5, 2012, in Baltimore, MD, USA. Eight colleagues of Dr. Krous whose own professional careers were touched by his efforts discussed forensic issues related to SIDS, tissue banking, animal models in SIDS, brainstem studies in SIDS, genetic studies in SIDS, establishment of a SUDC registry, neuropathologic research in SUDC, and potential shared mechanisms underlying sudden and unexpected death in early life. The wide scope of the presentations crossed the disciplines of forensic pathology, pediatric pathology, neuropathology, neuroscience, physiology, genetics, and bereavement, and attest to Dr. Krous' far-reaching influence upon SIDS and SUDC research.

She was treated for gastroesophageal reflux disease without relief. An esophagram revealed mild compression of the cervical esophagus. Chest MRI revealed a congenital double aortic arch with a larger and more cephalad right arch (Figures 1 and 2, R) than left arch (Figures 1 and 2, L). Separate subclavian and common carotid arteries arose from each arch. Within the vascular ring the trachea was normal; however, the esophagus was obstructed. The patient underwent a left posterolateral thoracotomy with division of the left aortic arch just distal to the origin of the left subclavian artery and division of the ligamentum arteriosum, thus freeing the esophagus. Images in cardiovascular medicine. Avoiding papillary muscle infarction with myocardial contrast echocardiographic guidance of nonsurgical septal reduction therapy for hypertrophic obstructive cardiomyopathy.

Intracoronary myocardial contrast echocardiography (MCE) can be used to guide the delivery of ethanol during nonsurgical septal reduction therapy for hypertrophic obstructive cardiomyopathy. The echocardiographic contrast agent is injected immediately before the injection of ethanol, down the lumen of the inflated balloon that resides in the target septal coronary artery. This is the artery that supplies the area of the septum involved with the mitral leaflet(s) in causing dynamic outflow tract obstruction. MCE provides a direct visualization of the myocardial territory that should receive ethanol and consequently undergo infarction. With this technique, a double-orifice mitral valve is artificially created by approximating the free edges of the leaflets at the site of the regurgitation with a suture. This is a technically simple, rapidly feasible operation that can be performed even in the presence of complex anatomical lesions. The further postoperative course of the patient was uneventful. The echocardiographic examination 3 months later showed only a mild residual mitral regurgitation and an image, unique to this operation, mimicking the face of Santa Claus in the parasternal short axis view (Figure 1). Left Ventricular Asynchrony Caused by an Intramuscular Lipoma

Routine chest x-ray of a 67-year-old woman revealed a mass contiguous with the cardiac silhouette. Echocardiogram located the mass in the anterior wall of the left ventricle. 50-year-old air-conditioning repairman presented to our emergency room with a 12-hour history of progressively worsening pain, decreased temperature, and bluish discoloration involving the third right finger. The patient denied a history of prior cardiovascular problems but did smoke tobacco heavily. Careful questioning revealed that he often utilized his hand as a “hammer” and had been particularly vigorous with this technique a few days prior to admission. Physical examination demonstrated ischemic changes in the tip of the middle finger of his right hand. An x-ray (Figure 1) of the right hand demonstrated an old un-united fracture of the ulnar styloid. He was treated with intravenous heparin and underwent angiography of the right upper extremity via the right femoral approach. The study demonstrated subtotal occlusion of the right ulnar artery at the wrist with poor filling of the digital arteries supplying the lateral 3 fingers (arrows, Figure 2). The results were compatible with hypothenar hammer syndrome, a term first coined by Conn et al 1 in 1970 although Guttani 2,3 (1772) and Von Rosen 2,4,5 (1934) had previously described similar cases. The syndrome is seen in the dominant hand of males who, during occupational or recreational activities, use their hand as a hammer. The mechanism of the injury relates to repetitive trauma to the superficial division of the ulnar artery in the hypothenar region of the hand. He was treated with several boluses of nitroglycerin, verapamil, and papaverine administered into the brachial artery. Repeat angiography demonstrated improved filling. His symptoms improved over the next 24 hours and did not recur. He was discharged on a regimen of subcutaneous dalteparin for 5 days and oral aspirin, clopidogrel, and nifedipine. He was also advised to quit smoking and refrain from using his hand in the manner described.
