Orthopedic treatment is extremely difficult for diseases of the oral mucosa, such as chronic recurrent aphthous stomatitis, leukoplakia, lichen planus, etc. Domestic scientists have long recognized the need for a differentiated approach to the selection of material for the manufacture of dentures, taking into account the individual characteristics of the patient’s dental and general somatic status, and microflora of the oral cavity. The choice of material is carried out taking into account not only its physicochemical characteristics, but also the biological effect on the oral mucosa. Purpose of the work: to identify new scientific data by analyzing systematic reviews that reveal the etiological aspects and pathogenesis of lichen planus of the oral mucosa. A review of domestic and foreign scientific literature over the past 10 years was conducted, which analyzed dissertations and articles published only in peer-reviewed scientific journals. Acrylic resins remain the most common structural material in partial and complete removable prosthetics. But such negative properties of acrylic orthopedic structures, such as residual monomer and microporosity, often lead to an increase in the number of allergic reactions in patients, the occurrence of microcracks in the prosthesis and other negative consequences. An alternative to removable dentures made of acrylic plastics are fairly widely used orthopedic structures made of thermoplastic materials, which in their chemical structure do not have the main negative properties inherent in acrylic plastics, and in terms of strength indicators are several times higher than acrylic ones. Depending on the specific clinical situation, the type of base material from thermoplastic masses for prosthetics for patients is selected. The chemical composition and choice of such materials is quite large. The issues of studying the mechanisms of tissue stress, the reactions of elimination of polymers during prolonged stay in the oral cavity, and the development of criteria for the biocompatibility of thermoplastic materials remain relevant today.

A comprehensive view of the etiology of ChRAS, taking into account potential risk factors

Summary. Chronic recurrent aphthous stomatitis (ChRAS) is currently one of the most common diseases affecting the oral mucosa, affecting 5 to 66 % of the population in different countries worldwide. The pathogenesis of ChRAS is unknown, and it is considered a multifactorial disease associated with local and systemic factors. Further research can be valuable for the prevention and treatment of ChRAS. The aim of the study – to analyze data from scientific literature sources regarding potential risk factors, mechanisms of pathogenesis, including the immune response of mucous membranes and mechanisms of its modification to optimize the diagnosis and treatment of ChRAS. Materials and Methods. The bibliosemantic method was applied to clarify the state of the problem, study the analysis of results of previous scientific research based on literature sources and electronic resources. Results and Discussion. Clinical characteristics of ChRAS are well-defined, but its etiology and pathogenesis remain unresolved. The comprehensive investigation of the occurrence and recurrence of the disease is crucial for understanding the fundamental pathogenetic links, which would be valuable for the prevention and treatment of ChRAS. Potential risk factors for ChRAS, such as a family history, psychological stress, hormonal changes, food allergies, nutritional deficiencies, vitamins and minerals deficiencies, gastrointestinal disorders, certain viral and bacterial infections, medication use, and mechanical trauma to the oral mucosa, are reported. Genetic predisposition and genetic risk factors altering individual susceptibility to ChRAS play a significant role. Various DNA polymorphisms, widespread throughout the human genome, especially those associated with changes in interleukin metabolism, are involved. The unbalanced composition of the oral microbiota may play a key role, although no specific causative agents have been conclusively proven. Food markers related to vitamin and micro- and macroelement deficiencies are also reported, and these are more common in patients with ChRAS. These findings preliminarily explain the main mechanisms through which the oral mucosa is a potential high-risk site for ChRAS, providing evidence for future prevention and treatment strategies. Conclusions. The results of the processed literature sources show that ChRAS is considered a multifactorial disease, but little is known about the mechanisms of pathogenesis and the possibilities of influencing its main links. Therefore, a comprehensive approach and the development and organization of complex rehabilitation plans are extremely important and decisive for assessing the condition of patients suffering from ChRAS.

THE USE OF “DIPLEN-S” ADHESIVE MEMBRANE IN THE COMPLEX THERAPY OF RECURRENT APHTHOUS STOMATITIS. (CLINICAL CASE REPORT)

Recurrent aphthous stomatitis (RAS) is a common ulcerative disease of the oral mucosa. In the etiopathogenesis of this pathology, there is no single concept that would outline the data of the trigger mechanisms that determine chronic recurrent aphthous stomatitis, interpreting the statement of the manifestation of various clinical signs in the oral cavity. The paper presents the results of treatment and dental examination of a patient using the protective adhesive film “Diplen-S” in the complex therapy of chronic recurrent stomatitis (RAS). The use of the adhesive film "Diplen-S" led to a decrease and disappearance of pain, accelerated the healing time and epithelization of aphthae with no side effects. Conclusion։ The positive clinical results obtained, wound healing in an accelerated time, are the basis for recommending the use of the adhesive film “Diplen” in the complex therapy and treatment of RAS, in the practice of stomatology.

Dynamics of free radical processes in the oral fluid of patients with chronic recurrent aphthous stomatitis at the first stage of therapy

Chronic recurrent aphthous stomatitis (CRAS) is a disease of the oral mucosa, characterized by the appearance of specific round aphthous forms with hyperemic corolla, covered with grayish-yellow plaque against the background of inflamed mucous membrane, which violates the oxidative homeostasis of oral fluid and the body as a whole.The aim of our study was to reveal the dynamics of free radical processes in oral fluid of patients with chronic recurrent aphthous stomatitis on the background of the first stage of antiseptic therapy with analgesics in order to determine the optimal treatment tactics for this condition. In order to achieve the set objective we had to solve the following tasks: to evaluate the functional activity of free radical processes in the oral liquid of patients with CPAS on the background of antiseptics (chlorhexidine, octenisept, furacilin, myramistine) and anaesthetic (Camistad gel) application for 7 days.Materials and Methods. During the study the oral fluid of 120 people diagnosed with CPPS was studied, divided into 4 groups equally: Group 1 – control, patients used the antiseptic chlorhexidine for gargling; Group 2 – patients with CPPS therapy with octenisept; Group 3 – patients with CPPS therapy with furacilin; Group 4 – patients with CPPS therapy with miramistine. For pain relief, patients in all groups used Kamistad gel. Analysis of free radical processes in oral fluid of patients was carried out before the study and on the 7th day of therapy by determining products of oxidative modification of biomolecules on the basis of quantitative evaluation of stained complex with thiobarbituric acid (TBA), maximal flash (MVHL) and area (PCL) of H2O2-induced luminol-dependent chemiluminescence using LT-01 chemiluminometer ("Horos" "Joint Venture Soviet-Swedish Company"). Concentration of diene conjugates and malonic dialdehyde was also determined to estimate enzymatic part of oxidative homeostasis.Results of the study. Results of the researches permit to conclude that at chronic recurrent aphthous stomatitis enzymatic activity of an oral liquid changes. According to the presented data the initial values of concentration of TBK-RP, maximal flash of oral chemiluminescence, area of oral chemiluminescence and concentration of MDA and DC in the control group and three experimental groups reliably differ from the data received on the 7th day of the research. Significant decrease of the studied indexes of oral liquid testifies to the efficiency of the first stage of therapy of aphthous stomatitis by antiseptic preparations and decrease of intensity of oxidative processes in oral liquid. Of all the proposed antiseptics the best efficiency is demonstrated by octenisept, reliably worse are chlorhexidine and myramistine and the least effective in the therapy of CPAS is furacilin.Conclusions: treatment of CPAS with Octenisept in combination with Camistad gel is the most effective in restoration of the disturbed oxilite.

Oral mucosa diseases in seafarers during long voyages, their treatment and prevention : prospective study

OBJECTIVE: To detect oral mucous membrane disease of the mouth, lips and tongue in seafarers on long voyage, to evaluate preventive measures and treatment effectiveness. MATERIALS AND METHODS: There was a dental examination of 169 men, aged 22-52 on long voyage, who got full treatment and preventive measures of oral rehabilitation in the pre-deployment period in a time manner. The seafarers were divided into 2 groups. The 1st group included 57 seamen, who were part of the crew. The 2nd group consisted of 112 men, who were training and had the first long voyage. The oral mucosa diagnosis was based on a clinic examination. When identifying pathology, treatment was provided and there were anti-relapse measures after its completion, which were possible given the people on board on long voyage. RESULTS: It was found that the structure of oral mucosa is quite different among people of the 1st and 2nd groups. The pathology among the crew occurred more often due to the autoimmune component of pathogenesis in 5,26% and 0,9% cases respectively, while the people in the 2nd group, being on long voyage as part of their training, experienced infectious oral mucosa pathology (12,28% and 15,32% respectively) significantly more often, and also lesions of the lips due to meteorological factors in 8,77% and 10,81% cases accordingly. DISCUSSION: The article discusses the treatment of meteorological cheilitis, the fracture of the vermilion border of the lip, Vincent disease, herpetic fever and chronic recurrent aphthous stomatitis with the use of domestic medicine for topical application. CONCLUSION: On long voyage 26,04% seafarers had cases of oral mucosa, among which most frequently diagnosed ones were infectious diseases, pathology due to meteorological factors, and also diseases with the autoimmune component of pathogenesis. In all cases domestic medicine for topical application was used to reveal pathology of oral mucosa, and further, to prevent its recurrence that allowed to achieve a good therapeutic result and to avoid recurrence of oral mucosa disease, typical for seafarers during a 9-month voyage.

Рецидивуючий афтозний стоматит: дослідження можливих етіопатогенетичних факторів

Actuality: The prevalence, high frequency and deterioration in quality of life caused by chronic recurrent aphthous stomatitis have led to a significant number of studies of the etiology and effective therapy of this disease. However, the etiology of chronic recurrent aphthous stomatitis is still unclear and the currently available therapies are insufficient. Purpose: to study the clinical and pathogenetic features of chronic recurrent aphthous stomatitis. Materials and methods: The study included 80 patients: 29 men (36.25%) and 51 women (63.75%) with CRAS who sought help at the Department of Therapeutic Dentistry of the Bogomolets National Medical University. The age of the study participants was from 19 to 55 years. Patients with CRAS were divided into two groups: the first group (n=35) without gastrointestinal diseases, the second group (n=45) with gastrointestinal pathology. The control group (n=20) consisted of patients with diseases of the oral mucosa. To study the spectrum and number of microorganisms, biological material was taken from the surface of aphthous elements and PCR testing was performed (Diagen laboratory, Kyiv). The concentration of blood haemoglobin, iron, vitamin B 12, folic acid in the blood was determined using tests performed in the Diagen laboratory, Kyiv. Statistical analysis was performed using the licensed package MedStat 12 and Microsoft Excel. Results: In patients with gastrointestinal diseases (chronic gastritis and duodenitis, chronic pancreatitis), the most common recurrence of aphthae is 2 times per year, and in patients without gastrointestinal pathology, the most common recurrence of aphthae is 1 time per year. The results of the PCR study showed that in almost all clinical groups of patients examined, in samples taken from the surface of the aphthae, microorganisms of the genus Streptococcus and Staphylococcus were detected in 91.8% of cases, and Lactobacillus spp. were isolated in 51.4% of cases. Patients with CRAS had a significantly higher incidence of haemoglobin, iron, vitamin B12 and folic acid deficiency than patients in the control group. Conclusion: When recurrent oral aphthae are detected, it is advisable to conduct an examination by a dentist and a gastroenterologist to identify the presence of gastrointestinal diseases in patients. There is a significant association of haemoglobin, iron, vitamin B12 and folic acid deficiency with CRAS. Actuality. Oral health (dental health) is an important factor that affects the quality of human life. Deterioration of oral cavity health adversely affects speech, chewing and swallowing, and social activities. According to the literature, the prevalence of diseases of the oral mucosa ranges from 10.8 to 61.6% in various populations with clinically significant morbidity and high prevalence among young people. Purpose: analysis of the impact of oral mucosal diseases on patients' quality of life using the COMDQ questionnaire. Materials and methods. 90 patients aged 18–62 years with lichen planus, chronic recurrent aphthous stomatitis, leukoplakia and other chronic diseases of the oral mucosa participated in the study. Patients' quality of life was assessed using the COMDQ questionnaire on chronic diseases of the oral mucosa.Statistical analysis was performed using the MedStat program and Microsoft Excel; using the Student's test, statistically significant differences between indicators were determined; the Shapiro- Wilk test was used to check the distribution of the variation series for normality. Spearman's coefficient was used to assess the relationship between indicators. Results. The mean age of the patients was 36.5±26. The mean scores for the COMDQ were 1.3±0.7 for «pain and functional limitations», 1.5±0.7 for «medication and treatment», 2.3±0.9 for «society and emotional state» and 2.3±0.6 for «patient support». Conclusion. The COMDQ questionnaire is a short and informative tool for determining the quality of life of patients with chronic oral mucosal disease. ХАРАКТЕРИСТИКА ИММУНОЛОГИЧЕСКИХ НАРУШЕНИЙ У БОЛЬНЫХ ХРОНИЧЕСКИМ РЕЦИДИВИРУЮЩИМ АФТОЗНЫМ СТОМАТИТОМ

This work is devoted to the study of general and local immunity in chronic recurrent aphthous stomatitis. The relevance of the study of immunological mechanisms and their role in the development of aphthous stomatitis is associated with an increase in the number of relapses, the duration of the course and the complexity of the treatment. The study found an increase in the concentration of all classes of immunoglobulins, a decrease in the concentration of gamma-interferon, an increase in the production of TNF-α in the oral fluid, an increase in antiviral protection factors (CD16, CD4, CD25), a population of CD20, B-lymphocytes in blood serum.

PROBLEMS OF PROVIDING ORTHOPEDIC DENTAL CARE TO PATIENTS WITH CHRONIC RECURRENT APHTHOSIS STOMATITIS (LITERATURE REVIEW)

Abstract. According to the World Health Organization, chronic recurrent aphthous stomatitis remains an acute unresolved medical problem of the population and affects up to 20%. Such a large volume of patients stimulates the development of new treatment methods. In domestic and foreign literature, the possibilities of orthopedic treatment for chronic recurrent aphthous stomatitis are presented only in separate works. In this case, the question is not about the rejection of prosthetics. It is necessary for chronic aphthous stomatitis to choose the appropriate design of the prosthesis, which will eliminate the possibility of injury to the oral mucosa and the occurrence of an allergic reaction. Rational prosthetics is possible and necessary in terms of preventing the occurrence of relapses of chronic stomatitis. According to many authors, when preparing the oral cavity for prosthetics with chronic recurrent aphthous stomatitis, it is important to determine the amount of possible orthopedic care. There are recommendations for the elimination of possible traumatic factors, the correct restoration of the height of the lower face when providing orthopedic dental care to patients with chronic recurrent aphthous stomatitis. To prevent exacerbations in preparation for dental procedures, researchers suggest treating the oral cavity with gels or solutions containing an anesthetic. In many literature sources, when prosthetics with removable lamellar dentures of extensive defects in the dentition or the complete absence of teeth, it is necessary to use two-layer bases made of colorless plastic with an elastic lining. Aim of the study: to analyze domestic and foreign literature on the issues of orthopedic treatment of patients suffering from chronic recurrent aphthous stomatitis. Material and methods. An analysis of the literature on this issue was carried out, where a significant place is given to the importance of oral hygiene, weakening the toxic-allergic effect of polymers, improving the technology for manufacturing removable lamellar dentures, the use of magnetic therapy and low-intensity laser radiation. Conclusions. An analysis of the literature data and our own observations confirms the need for further study of the issue of providing qualified dental orthopedic care to patients with chronic recurrent aphthous stomatitis and ways to accelerate adaptation to removable partial or full dentures in patients with this pathology.

Analysis of crystallograms of oral fluid in the dynamics of treatment of patients with pathology of the oral mucosa

One of the express diagnostic methods is a crystallographic study of the oral fluid of this category of patients. Based on the quantitative and qualitative criteria of saliva obtained by the method of wedge dehydration, it is possible to non-invasively assess the effectiveness of the therapy and timely correct the prescribed treatment.Aim. To conduct a comparative analysis of the crystallograms of the oral fluid in the dynamics of the treatment of patients with pathology of the oral mucosa.Materials and methods. 90 patients were examined with a diagnosis of K12.00 aphthous stomatitis (small), chronic recurrent aphthous stomatitis (mild form). All patients were divided into 3 groups according to the methods of therapy 1:1:1 according to the principle of randomization. To assess the state of the oral fluid of patients with aphthous stomatitis against the background of various methods of treatment, the method of wedge-shaped dehydration was used, where the transition of a drying biological fluid into a solid state leads to a certain static order that can be observed and studied. Saliva was used as the material for the study.Results. A qualitative analysis of the crystallograms revealed various types of structures with inclusions of bent prismatic crystals. Prismatic, fragmentary, curved, X-shaped, skeletal types of crystals were found throughout the entire field of view. At the same time, there was no clear structured picture as a result of a violation of the normal crystallization process.Conclusion. There is a clear need for further research into the molecular etiopathogenesis of these conditions, which should allow the identification of more specific molecular targets for the development of diagnostic tests and therapeutic intervention. Based on the data obtained, the wedge dehydration method used for crystallographic examination of the oral fluid objectively reflects the state of the oral mucosa and can be used as an additional non-invasive method for diagnosing inflammatory and destructive diseases of the oral mucosa, as well as for correcting the treatment plan and monitoring the results of therapy.

Modern Approach to Prevention of Chronic Recurrent Aphthous Stomatitis

Aim: To analyse and summarize the data of modern scientific literature for the last 10 years on modern approaches to the prevention of chronic recurrent aphthous stomatitis, etiology, pathogenesis and treatment of this disease. Materials and Methods: Analysis and generalization of data from the modern scientific literature on the etiology, pathogenesis, prevention and treatment of CRAS based on databases such as Scopus, Web of Science, MedLine, PubMed, NCBI, the study of which does not exceed 10 years, including literature reviews and results of clinical trials. Conclusions: The prevention of chronic recurrent aphthous stomatitis requires a comprehensive and individualized approach to solve this issue. Reduction of this disease to one cause becomes serious due to its polyetiology, which makes it more difficult to predict the causes of recurrence and solution of the prevention issue. Trigger mechanisms that provoke the formation of CRAS differ in various patients (stress, mechanical disorders, the nature of nutrition, lack of vitamins or trace elements, infections, etc.). However, the immunopathogenetic mechanism may be a common link for the main triggers of the aphthae recurrence emergence. Due to the imperfect understanding of etiopathogenetic factors, the prevention of chronic recurrent aphthous stomatitis is aimed at increasing the intervals between the appearance of new lesions, reducing the number of aphthae and their size, so in other words, it is secondary.

Vitamin D imbalance effects on dentoalveolar system: a review

Vitamin D is a fat-soluble secosteroid that plays an important role in the human body. There are two main native forms – vitamin D3 (cholecalciferol) and vitamin D2 (ergocalciferol). The regulation of calcium-phosphate metabolism and ensuring adequate bone remodeling are the most studied function of vitamin D. In recent years, researchers have found out the «extra-bone» effects of vitamin D and it allows us to be convinced of the great role of this compound. The participation of active forms of vitamin D in the processes of immunomodulation, anti-inflammatory, antimicrobial, antiproliferative effects and stimulation of cell differentiation are reflected in diseases of the maxillofacial region.In this article, we examined the main functions of vitamin D in the human body, the mechanisms of its action and influence on the occurrence and course of oral diseases. The discovery of the relationship between vitamin D deficiency and the processes of osseointegration, bone remodeling, the severity of chronic recurrent aphthous stomatitis, squamous cell carcinoma and periodontitis allow us to conclude that it is advisable to diagnose vitamin D deficiency in appropriate time and correction its serum level in dental patients.

Complex Application Of Magnetic Laser Therapy And Propolis Tincture For The Prevention And Treatment Of Chronic Recurrent Aphthous Stomatitis

Chronic recurrent aphthous stomatitis (stomatitis chronica aphtosa recidiva) is a chronic inflammatory disease of the oral mucosa of an infectious-allergic nature, characterized by periodic remissions and frequent exacerbations. Morphological elements on the oral mucosa are the so-called "ulcerative formations", that is, apht characterized by periodic remissions and partial exacerbations with a rash of aphtha. According to the latest publications of domestic and foreign authors, HRAS is usually considered as a polyetiological disease that plays a role in the formation and the nature of the course of the pathological process. The factor causing chronic aphthous stomatitis is staphylococcal infection, adenovirus, allergies, immune disorders, as well as diseases of organs and systems. First of all, it is a local effect aimed at reducing pain that makes it difficult to eat up to forced starvation, as well as at the speedy regeneration of the defect. Secondly, it is the regulation of the body's immunocompetent systems and a decrease in the level of bacterial allergies. In addition, the identification and treatment of background diseases with the involvement of specialists of different profiles (Lukinykh L.M., 2004; Savkina G.D., 2005; Drobotko JI.H., 2008).

ТРАДИЦИОННЫЕ МЕТОДИКИ ТЕРАПИИ ХРОНИЧЕСКОГО РЕЦИДИВИРУЮЩЕГО АФТОЗНОГО СТОМАТИТА

Subject. Nowadays chronic recurrent aphthous stomatitis (CRAS) is still one of the few unsolved problems of modern dentistry, covering about 20% of the population, according to the World Health Organization. 
 This literature review provides information regarding the etiology, pathogenesis, diagnosis and treatment methods. Modern dentists dealing with the problem of CRAS, at the diagnostic stage, reveal various somatic pathologies, indicating their initial etiological cause of the disease onset. There are a lot of such reasons, but the majority of authors suppose that it is impossible to do without a complex etiopathogenetic treatment, consultations of related specialists in order to achieve a stable remission of CRAS. Many authors agree that physiotherapy should be included in the course of treatment along with medications. Despite a wide range of treatment methods, there is no really effective one, patients still suffer from relapses. The true etiological cause has also not been identified.
 Aim — the analysis of modern ideas concerning the etiology and treatment of chronic recurrent aphthous stomatitis
 Materials and methods. The study was performed on the basis of the search and analysis of the original literature sources in the following databases: Russia State Library, eLibrary, PubMed, The Cochrane Library, Google Scholar. The main selection of materials was carried out by keywords.
 Results. The review of the literature sources describes the current understanding of the etiology and treatment of chronic recurrent aphthous stomatitis, the basic principles of physiotherapy of chronic recurrent aphthous stomatitis.
 Conclusion. Analyzing the literature data, it can be concluded that the true cause of chronic recurrent aphthous stomatitis is not known. Each case must be analyzed separately, a complete examination of the patient must be carried out, identifying concomitant somatic pathology. Among the methods of treatment, the most promising is an integrated approach - drug therapy with the addition of physiotherapy. The most modern is laser therapy.

PSYCHOSOMATIC ASPECTS F DISEASES OF THE MUCOSA OF THE ORAL CAVITY

We studied the reporting and statistical documentation of the State Medical Institution "KSP No. 10" for the last 5 years (medical records, an advisory journal, laboratory tests, methods of treatment). 276 medical records were analyzed. The age of the patients ranged from 20 to 70 years. Among the treated patients, there were 104 men and 146 women. The analysis of the treatment of patients with diseases of the oral mucosa revealed that the structure of lesions of the oral mucosa is dominated by leukoplakia, decubital ulcer, glossalgia, lichen planus (CPL), chronic recurrent aphthous stomatitis (HRAS). Many patients associate the cause of glossalgia, CPL, and exfoliative cheilitis with stress and depression. The appointment of psychotherapeutic treatment in such patients led to positive dynamics and persistent remission.

Tissue Optical Clearing in the Ultraviolet for Clinical Use in Dentistry to Optimize the Treatment of Chronic Recurrent Aphthous Stomatitis

The immersion optical clearing (OC) treatment with a highly concentrated glycerol solution has induced three new tissue windows in the UV spectral range of gingival tissues – from 200 to 250 nm, from 250 to 300 nm and from 300 to 400 nm. By combining the immersion OC technique in human tissues with UV-spectroscopy, it was possible to verify and study the major OC mechanisms – tissue dehydration and refractive index matching, and that the OC efficiency is higher in the deep-UV than in the visible-NIR range. Since all biological tissues present high scattering in the UV range, the presented technology, which basically reduces the strong light scattering in the UV range, has a broad application area in medicine. The effectiveness of the developed technology combining UV phototherapy and OC in application to treatment of aphthous recurrent stomatitis in children was demonstrated.

Clinical and Laboratory Method for Assessing the Effectiveness of Diagnosis and Treatment in Patients with Chronic Recurrent Aphthous Stomatitis Against Infectious Pathologies of the Genitourinary System

Our literature on the pathology of chronic recurrent aphthous aphthous stomatitis (CRAS) reveals that there are a number of scientific views on the etiopathogenesis of this pathology as a neurogenic, immune, infectious and allergic theory of origin. Other scientists are promoting the concept of the tendency of this pathology in favor of the endocrine theory or reactivity and resistance of the body [1, 2, 6, 7, 9, 10, 11, and 20].

Improving the methods of local treatment in case of aphthous lesions on the oral mucosa

Annotation. Contamination of the oral mucosa ulcers with opportunistic microorganisms aggravates the course of chronic recurrent aphthous stomatitis (HRAC). Topical medicines help to eliminate inflammation rapidly and induce fast epithelization of the mucous membrane defect in patients with HRAC. The aim of the work was to create curative composition recipe for the local therapy at HRAC, resulting to higher effectiveness of antimicrobial therapy and acceleration of healing. 43 patients with HRAC were examined. Samples from the lesions were inoculated blood agar and Sabouraud agar. The conventional methods were used for microorganisms’ identification. The sensitivity of isolated strains to antibiotics and antifungals was determined. Antimicrobial activity of the proposed applicative composition was determined by the well method into agar media. It was established that the surface of patients` lesions on the oral mucosa was most often colonized with associations of Streptococci and Neisseriae, Streptococci and C. albicans. For every third patient, the erosive surface was infected with gram-negative microorganisms, including non-ferments and enterobacteria. Most of the isolated bacteria were highly sensitive to antibacterial compounds. Yeast-like fungi exhibited high resistance to fluconazole, itraconazole, 50% of them were resistant to ketoconazole, and resistance to nystatin were revealed in 81% of isolated strains. The proposed application dosage form containing vinyl, decamethoxin, essential oils of carnation and sea buckthorn had a high antimicrobial activity against isolated strains. Obtained results indicate the feasibility of the applicative composition for treatment of erosive and ulcerative lesions, which are caused by microorganisms, in dental practice.
